Background: The opioid system is involved in the control of pain, reward, addictive behaviors and vegetative effects. Opioids exert their pharmacological actions through the agonistic binding at opioid receptors and variation in the coding genes has been found to modulate opioid receptor expression or signaling. However, a limited selection of functional opioid receptor variants is perceived as insufficient in providing a genetic diagnosis of clinical phenotypes and therefore, unrestricted access to opioid receptor genetics is required.
Methods: Next-generation sequencing (NGS) workflow was based on a custom AmpliSeq™ panel and designed for sequencing of human genes related to the opioid receptor group (OPRM1, OPRD1, OPRK1, SIGMA1, OPRL1) on an Ion PGM™ Sequencer. A cohort of 79 previously studied chronic pain patients was screened to evaluate and validate the detection of exomic sequences of the coding genes with 25 base pair exon padding. In-silico analysis was performed using SNP and Variation Suite® software.
Results: The amplicons covered approximately 90% of the target sequence. A median of 2.54 × 106 reads per run was obtained generating a total of 35,447 nucleotide reads from each DNA sample. This identified approximately 100 chromosome loci where nucleotides deviated from the reference sequence GRCh37 hg19, including functional variants such as the OPRM1 rs1799971 SNP (118 A > G) as the most scientifically regarded variant or rs563649 SNP coding for μ-opioid receptor splice variants. Correspondence between NGS and Sanger derived nucleotide sequences was 100%.
Conclusion: Results suggested that the NGS approach based on AmpliSeq™ libraries and Ion PGM sequencing is a highly efficient mutation detection method. It is suitable for large-scale sequencing of opioid receptor genes. The method includes the variants studied so far for functional associations and adds a large amount of genetic information as a basis for complete analysis of human opioid receptor genetics and its functional consequences.
Previous study showed that kaffir lime leaf contains alkaloid, flavonoid, terpenoid, tannin and saponin. The objective of this study was to examine the cytotoxic effect of kaffir lime leaf extract on cervical cancer and neuroblastoma cell lines. The method used for this research to determine cell viability was an 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay. Results showed that an ethyl acetate extract had an IC50 for HeLa cells, UKF-NB3, IMR-5 and SK-N-AS parental cells of 40.7 μg · mL–1, 28.4 μg · mL–1, 14.1 μg · mL–1, and 25.2 μg · mL–1 respectively. Furthermore, the IC50 of chloroform extracts for HeLa cells, UKF-NB3, IMR-5 and SK-N-AS parental were 17.6 μg · mL–1, 18.9 μg · mL–1, 6.4 μg · mL–1, and 9.4 μg · mL–1 respectively. These data showed that kaffir lime extract reduces the viability of cervical and neuroblastoma cell lines and may have potential as anti-cancer compounds.
Background: Persistent pain in breast cancer survivors is common. Psychological and sleep-related factors modulate perception, interpretation and coping with pain and may contribute to the clinical phenotype. The present analysis pursued the hypothesis that breast cancer survivors form subgroups, based on psychological and sleep-related parameters that are relevant to the impact of pain on the patients’ life.
Methods: We analysed 337 women treated for breast cancer, in whom psychological and sleep-related parameters as well as parameters related to pain intensity and interference had been acquired. Data were analysed by using supervised and unsupervised machine-learning techniques (i) to detect patient subgroups based on the pattern of psychological or sleep-related parameters, (ii) to interpret the detected cluster structure and (iii) to relate this data structure to pain interference and impact on life.
Results: Artificial intelligence-based detection of data structure, implemented as self-organizing neuronal maps, identified two different clusters of patients. A smaller cluster (11.5% of the patients) had comparatively lower resilience, more depressive symptoms and lower extraversion than the other patients. In these patients, life-satisfaction, mood, and life in general were comparatively more impeded by persistent pain.
Conclusions: The results support the initial hypothesis that psychological and sleep-related parameter patterns are meaningful for subgrouping patients with respect to how persistent pain after breast cancer treatments interferes with their life. This indicates that management of pain should address more complex features than just pain intensity. Artificial intelligence is a useful tool in the identification of subgroups of patients based on psychological factors.
Prolonged treatment of leukemic cells with chemotherapeutic agents frequently results in development of drug resistance. Moreover, selection of drug-resistant cell populations may be associated with changes in malignant properties such as proliferation rate, invasiveness, and immunogenicity. In the present study, the sensitivity of cytarabine (1-β-d-arabinofuranosylcytosine, araC)-resistant and parental human leukemic cell lines (T-lymphoid H9 and acute T-lymphoblastic leukemia Molt-4) to natural killer (NK) cell-mediated killing was investigated. The results obtained demonstrate that araC-resistant H9 and Molt-4 (H9rARAC100 and Molt-4rARAC100) cell lines are more sensitive to NK cell-mediated lysis than their respective parental cell lines. This increased sensitivity was associated with a higher surface expression of ligands for the NK cell-activating receptor NKG2D, notably UL16 binding protein-2 (ULBP-2) and ULBP-3 in H9rARAC100 and Molt-4rARAC100 cell lines. Blocking ULBP-2 and ULBP-3 or NKG2D with monoclonal antibody completely abrogated NK cell lysis. Constitutive phosphorylated extracellular signal-regulated kinase (ERK) but not pAKT was higher in araC-resistant cells than in parental cell lines. Inhibition of ERK using ERK inhibitor PD98059 decreased both ULBP-2/ULBP-3 expression and NK cell cytotoxicity. Furthermore, overexpression of constitutively active ERK in H9 parental cells resulted in increased ULBP-2/ULBP-3 expression and enhanced NK cell lysis. These results demonstrate that increased sensitivity of araC-resistant leukemic cells to NK cell lysis is caused by higher NKG2D ligand expression, resulting from more active ERK signaling pathway.
Although human cytomegalovirus (HCMV) is generally not regarded to be an oncogenic virus, HCMV infection has been implicated in malignant diseases from different cancer entities. On the basis of our experimental findings, we developed the concept of “oncomodulation” to better explain the role of HCMV in cancer. Oncomodulation means that HCMV infects tumor cells and increases their malignancy. By this concept, HCMV was proposed to be a therapeutic target in a fraction of cancer patients. However, the clinical relevance of HCMV-induced oncomodulation remains to be clarified. One central question that has to be definitively answered is if HCMV establishes persistent virus replication in tumor cells or not. In our eyes, recent clinical findings from different groups in glioblastoma patients and especially the detection of a correlation between the numbers of HCMV-infected glioblastoma cells and tumor stage (malignancy) strongly increase the evidence that HCMV may exert oncomodulatory effects. Here, we summarize the currently available knowledge about the molecular mechanisms that may contribute to oncomodulation by HCMV as well as the clinical findings that suggest that a fraction of tumors from different entities is indeed infected with HCMV.

Encouraging HCWs to get vaccinated can play a vital role in stopping the transmission of pertussis, thereby reducing institutional outbreaks.
In Germany, HCWs come up with all sorts of reasons for not getting pertussis vaccination. This study was meant to provide information in order to better understand the backgrounds of these attitudes.
A survey was conducted at the children's university hospital in Frankfurt, using an anonymous questionnaire. Survey results were used to design an intervention to increase the immunization rate of staff. Disappointingly, our efforts to increase the acceptance of the immunization program by providing information in advance were not yet satisfying.

With respect to nosocomial influenza infections, the welfare of patients is best served by high rates of staff immunity against influenza. However, data from the Centers of Disease Control (CDC) in the USA and the Robert Koch-Institute (RKI) in Germany indicate that most of health care workers (HCWs) choose not to be vaccinated. Under voluntary influenza immunization standards, institutional influenza outbreaks occur every flu season. The question about the legality of implementation mandatory flu vaccination for HCWs is an ongoing debate, which covers several different positions.
To characterize the attitudes of German HCWs toward mandatory influenza immunization, an anonymous questionnaire was offered to HCWs of the University Hospital in Frankfurt/Main / Germany. Our study showed that almost 70% of the respondents would accept mandatory influenza vaccination.
In our opinion an annual influenza vaccination should be required for HCWs who care for immunocompromised patients and residents in long-term care if there will be a failure of voluntary vaccination programs. An informed declination should be obtained from employees who decline vaccination and these HCWs ought to work in uncritical areas of patient care.
Background and purpose: Transient splenial oedema, also known as reversible splenial lesion syndrome (RESLES), is a rare magnetic resonance imaging (MRI) finding that presents as a round or ovoid focal oedema in the posterior corpus callosum, and is associated with a wide range of clinical conditions. The aetiology of RESLES is not fully clear. We aimed to investigate conflicting pathophysiological hypotheses by measuring local glucose metabolism in patients with RESLES.
Methods: We retrospectively analysed patients with RESLES after reductions in antiseizure medications during in-hospital video electroencephalography monitoring. We measured local glucose uptake using positron emission tomography/computed tomography and compared matched cohorts of patients with and without MRI evidence of RESLES using nonparametric tests.
Results: Local glucose metabolism in the splenium of seven patients with RESLES was not significantly different from the glucose metabolism of the seven patients in the matched cohort. This was true using both regular and normalized standardized glucose uptake value calculation methods (p = 0.902 and p = 0.535, respectively).
Conclusion: We found no evidence of local glucose hypometabolism in RESLES, which supports previous pathophysiological considerations that suggest that RESLES is an intercellular, intramyelinic oedema rather than a typical intracellular cytotoxic oedema, which is not reversible.
Auf Grund einer hohen Inzidenz und Mortalität, welche in den nächsten Jahren voraussichtlich eine deutliche Zunahme erfahren wird, stellt die Behandlung eines HCC an alle beteiligten Fächer der Medizin, sowie an den Patienten und die Patientin, eine enorme Herausforderung dar. In der klinischen Routine hat sich die TACE, nicht nur bei Patienten im intermediären Stadium der Erkrankung, etabliert, sodass im Laufe der Erkrankung nahezu jeder zweite Patient mindestens eine TACE-Behandlung bekommt.
Der mit Radiomics betitelte, im medizinischen Bereich relativ junge, Forschungszweig beschäftigt sich mit der Idee, dass in den Schnittbildern eine für das menschliche Auge nicht sichtbare Ebene von Informationen vorliegt, welche mit den richtigen Mitteln extrahiert, relevante Daten und Informationen zur Genetik, Phänotypie und Pathophysiologie des Tumors liefern kann.
Hier greift der Ansatz dieser Arbeit an. In dieser Arbeit wird die Hypothese postuliert, dass durch die Auswertung und Integration von Lipiodolablagerungen in der Zielläsion nach der ersten durchgeführten TACE eine zuverlässigere Prognose zum Therapieansprechen und Gesamtüberleben mit Hilfe von Radiomics möglich ist, als dies klinische Scores alleine erlauben.
Dazu wurde in dieser Arbeit ein Patientenstamm von 61 Patienten untersucht. Alle Patienten litten an einem histologisch gesicherten HCC. Bei allen Patienten wurden innerhalb eines Zeitintervalls von 6 Monaten drei TACE durchgeführt mit einer nachfolgenden Verlaufskontrolle mittels kontrastmittelgestützter MRT oder CT.
In einem dezidierten, mehrstufigen Verfahren wurden aus der nativen 24 Stunden postinterventionellen CT-Kontrolle die Lipiodol anreichernden HCC-Herde segmentiert. Aus diesem segmentierten 3-D Bilddatensatz wurde eine Vielzahl von bildgebenden Biomarkern, Features, extrahiert. Die Features wurden im weiteren Prozess selektiert, redundante und nicht reproduzierbare Features wurden für das weitere Vorgehen verworfen.
Aus den vorliegenden Daten der Patienten wurden Informationen selektiert, mit welchen insgesamt 5 klinische Scores berechnet wurden, diese Scores wurden im weiteren Verfahren ebenfalls als Features angesehen.
Mehrere Machine Learning-Algorithmen wurden mit der Zielvariable: Größenregredienz des Tumors nach TACE als Folge eines annehmbaren Therapieansprechens, angelernt.
Das beste Ergebnis lieferte ein ML-Algorithmus mit einem Random Forrest Klassifikator auf der Grundlage des kombinierten, aus Radiomics-Features und klinischem Score-Features bestehendem Featuresets.
Um die initial aufgestellte Hypothese zu überprüfen wurde die Zielvariable von Größenregredienz der TL auf OS verändert. Die Performance des ML-Algorithmus in Bezug auf die neu definierte Zielvariable OS wurde hierbei mit dem C-index bewertet. Im Test-Set liegt ein C-Index von 0,67 vor. Das kombinierte Modell aus klinischem Score und Radiomics zeigt hierbei eine Überlegenheit gegenüber dem klinischen Score allein (C-Index 0,58) und dem Radiomics score (C-Index 0,60). Dies bestätigt die aufgestellte Hypothese. Das kombinierte Modell hat die Fähigkeit, anhand der Lipiodolanreicherung in der 24 Stunden postinterventionell durchgeführten CT, zur Prädiktion eines Gesamtüberlebens von HCC-Patienten nach einer TACE.
Die Patienten mit der kürzesten und längsten Überlebenszeit innerhalb der Studienpopulation dienten als Grundlage für eine Kaplan-Meier-Schätzung und Berechnung eines Risiko-Scores (siehe Abbildung 37). Dabei zeigt sich eine signifikante Differenz zwischen den Risiko-Scores. Eine Kurve dieser Art könnte zukünftig theoretisch als Schätzung zur Überprüfung der Indikation einer TACE- Wiederholung für einzelne Patienten dienen. Für eine entsprechende Generalisierbarkeit sind weiterführende Studien zur Validierung nötig. Unsere Studie liefert hier erste vielversprechende Hinweise, wobei unsere Limitationen nicht zu vernachlässigen sind, wie im Detail diskutiert.
Zusammenfassend zeigt unsere Arbeit, dass ein von uns definierter kombinierter Score, bestehend aus bildgebenden Biomarkern (Radiomics) und einem klinischen Score (m- HAP-II-Score), eine Prognose zum Gesamtüberleben nach der ersten TACE- Behandlung liefern kann. Mit Hilfe dieses kombinierten Scores war es in unserer Studienkohorte möglich abzuschätzen, ob ein Patient von weiteren TACE-Prozeduren profitieren würde. Der Behandlungsalgorithmus könnte auf dieser Basis individuell angepasst werden.
Der kombinierte Score hätte somit nicht nur das Potenzial Nebenwirkungen zu verhindern und Kosten im System einzusparen, sondern ebenfalls den Patienten potentiell individuell effektiveren Therapiealternativen zuzuführen.
Several lines of evidence suggest the ligand-sensing transcription factor Nurr1 as a promising target to treat neurodegenerative diseases. Nurr1 modulators to validate and exploit this therapeutic potential are rare, however. To identify novel Nurr1 agonist chemotypes, we have employed the Nurr1 activator amodiaquine as template for microscale analogue library synthesis. The first set of analogues was based on the 7-chloroquiolin-4-amine core fragment of amodiaquine and revealed superior N-substituents compared to diethylaminomethylphenol contained in the template. A second library of analogues was subsequently prepared to replace the chloroquinolineamine scaffold. The two sets of analogues enabled a full scaffold hop from amodiaquine to a novel Nurr1 agonist sharing no structural features with the lead but comprising superior potency on Nurr1. Additionally, pharmacophore modeling based on the entire set of active and inactive analogues suggested key features for Nurr1 agonists.
Here, we present a peptide-based linear mixed models tool—PBLMM, a standalone desktop application for differential expression analysis of proteomics data. We also provide a Python package that allows streamlined data analysis workflows implementing the PBLMM algorithm. PBLMM is easy to use without scripting experience and calculates differential expression by peptide-based linear mixed regression models. We show that peptide-based models outperform classical methods of statistical inference of differentially expressed proteins. In addition, PBLMM exhibits superior statistical power in situations of low effect size and/or low sample size. Taken together our tool provides an easy-to-use, high-statistical-power method to infer differentially expressed proteins from proteomics data.
Rhinoliths are mineralised foreign bodies in the nasal cavity that are a chance finding at anterior rhinoscopy. Undiscovered, they grow appreciably in size and can cause a foul-smelling nasal discharge and breathing problems. Giant nasal stones are now a very rare occurrence, since improved diagnostic techniques, such as endoscopic/microscopic rhinoscopy, now make it possible to identify foreign bodies at an early stage of development. We report the case of a 37-year-old patient who, at the age of 5-6 years, introduced a foreign body, probably a stone, into his right nasal cavity. On presentation, he complained of difficulty in breathing through the right nostril that had persisted for the last 10 years. For the past four years a strong fetid smell from the nose had been apparent to those in his vicinity. Under general anaesthesia, the stone was removed in toto from the right nasal cavity. The possible genesis of the rhinolith is discussed, our case compared with those described in the literature, and possible differential diagnoses are considered.
Die kongenitale Zytomegalievirus Infektion (cCMV-Infektion) ist die häufigste kongenitale Infektionskrankheit weltweit und ist der häufigste Grund für angeborene nicht-genetische Hörstörungen und eine häufige Ursache neurologische Entwicklungsstörungen. Die Inzidenz der cCMV-Infektion liegt in Deutschland zwischen 0,2 % – 0,5 %. Bei retroviral-exponierten Neugeborenen wird die Inzidenz mit 2,7 % – 11,4 % angegeben. Mit der erhöhten Inzidenz der cCMV-Infektion bei retroviral-exponierten Neugeborenen ergibt sich für diese Kinder ebenfalls ein erhöhtes Risiko für Langzeitfolgen. Die genaue Inzidenz der cCMV-Infektion variiert je nach untersuchter Population. Für Deutschland existiert eine retrospektive Studie, welche eine Inzidenz von 2,7 % für cCMV-Infektionen bei retroviral-exponierten Neugeborenen ermittelte. In der vorliegenden Studie wurde diese Inzidenz in einem prospektiven multizentrischem Studiendesign in Deutschland ermittelt.
Zur Ermittlung der Inzidenz der cCMV-Infektion bei retroviral-exponierten Neugeborenen und Beurteilung der Umsetzbarkeit eines cCMV-Neugeborenen-Screenings wurde ein selektives cCMV-Neugeborenen-Screening für retroviral-exponierte Neugeborene mittels PCR-Untersuchung auf CMV aus einem Mundschleimhautabstrich innerhalb der ersten 21 Lebenstage an drei Studienstandorten innerhalb Deutschlands, Mannheim, München und Frankfurt am Main, durchgeführt. Bei positivem Ergebnis der PCR auf CMV-DNA erfolgte eine Bestätigungsdiagnostik mittels erweiterter Urin- und Blutuntersuchung auf CMV. Zur Diagnostik von cCMV-assoziierten Symptomen erfolgte eine Sonographie des Abdomens und des Schädels sowie eine ausführliche körperliche Untersuchung, eine augenärztliche Evaluation und erweiterte Testungen der Gehörfunktion. Nachuntersuchungen und Therapien wurden den betroffenen Familien außerhalb der Studie angeboten.
122 / 184 (66,3 %) HIV-exponierte Neugeborene von 111 Müttern wurden im Studienzeitraum zwischen dem 24.11.2017 und dem 31.03.2021 eingeschlossen. Eine cCMV-Infektion wurde bei einem Neugeborenen nachgewiesen, sodass die Inzidenz der cCMV-Infektion bei retroviral-exponierten Neugeborenen in dieser Studie 0,8 % beträgt. Eine HIV-Mutter-Kind-Transmission wurde nicht detektiert. Die Seroprävalenz für CMV bei den HIV-positiven Frauen lag in diesem Kollektiv bei 96,1 %.
Das Neugeborene mit nachgewiesener cCMV-Infektion zeigte eine zerebrale Beteiligung mit ependymalen Zysten und einer thalamostriatalen Vaskulopathie und erhielt außerhalb der Studie eine zeitgerechte antivirale Therapie mit Beginn in der Neonatalper-ode. Im Verlauf zeigten sich trotz der antiviralen Therapie Entwicklungsstörungen mit autistischen Verhaltensweisen. Die cCMV-Infektion wäre ohne ein routinemäßiges Screening mit großer Wahrscheinlichkeit nicht nachgewiesen worden.
Die frühzeitige Untersuchung der Probanden auf eine cCMV-Infektion hat sich in dieser Studie als vorteilhaft gezeigt, da bei Nachweis einer cCMV-Infektion zeitnah weiterführende Diagnostik und Therapien angeboten werden konnten. Auch die relativ große Anzahl an rekrutierten retroviral-exponierten Neugeborenen im prospektiven Studiendesign in Zusammenarbeit mit mehreren Studienzentren in Deutschland spricht für die Validität dieser Studie. Als Limitation ist zu nennen, dass ein statistisch signifikantes Ergebnis nicht erzielt werden konnte. Aufgrund der Corona-Pandemie kam es organisationbedingt zu einer relativ hohen Anzahl an nicht eingeschlossenen Patienten. Auch die geplante Rekrutierung einer Vergleichsgruppe in Südafrika konnte aufgrund der Pandemie nicht umgesetzt werden. Falsch negative Befunde wurden im Sinne der Familie nicht mittels Goldstandardmethode überprüft, sodass eine Unterschätzung der Rate an cCMV-Infektionen möglich ist.
Insgesamt konnte diese Studie neben der Ermittlung der cCMV-Inzidenz bei retroviral-exponierten Neugeborenen in Deutschland von 0,8 % aufgezeigt werden, dass selbst symptomatische cCMV-Infektionen ohne ein systematisches cCMV-Neugeborenen-Screening nicht sicher nachgewiesen werden konnte. Zudem konnte gezeigt werden, dass ein systematisches cCMV-Neugeborenen-Screening mittels Mundschleimhautabstrich in Deutschland praktikabel ist und bei den Sorgeberechtigten Akzeptanz findet. Den erhobenen Daten zur Folge könnte ein Screening aller Neugeborener oder zumindest ein risikoadaptiertes Screening auf das Vorliegen einer cCMV-Infektion dazu beitragen, dass mehr Kinder mit asymptomatischer oder unentdeckter symptomatischer cCMV-Infektion diagnostiziert werden und so eine entsprechende Behandlung ermöglicht sowie ggf. Langzeitfolgen möglichst verringert werden.
Weitere Studien zum Effekt der verfügbaren antiviralen Therapie bei cCMV-Infektionen und regelmäßiger Kontrolluntersuchungen nach stattgehabter cCMV-Infektion sind zu empfehlen, um die Auswirkungen dieser Maßnahmen auf den Krankheitsverlauf zu evaluieren.

The current SARS-CoV-2 pandemic has once again highlighted the central importance of acute respiratory infections to human health. The “network of excellence on Community Acquired Pneumonia” (CAPNETZ) hosts the most comprehensive CAP database worldwide including more than 12,000 patients. CAPNETZ connects physicians, microbiologists, virologists, epidemiologists, and computer scientists throughout Europe. Our aim was to summarize the current situation in CAP research and identify the most pressing unmet needs in CAP research.
Methods: To identify areas of future CAP research, CAPNETZ followed a multiple-step procedure. First, research members of CAPNETZ were individually asked to identify unmet needs. Second, the top 100 experts in the field of CAP research were asked for their insights about the unmet needs in CAP (Delphi approach). Third, internal and external experts discussed unmet needs in CAP at a scientific retreat.
Results: Eleven topics for future CAP research were identified: detection of causative pathogens, next generation sequencing for antimicrobial treatment guidance, imaging diagnostics, biomarkers, risk stratification, antiviral and antibiotic treatment, adjunctive therapy, vaccines and prevention, systemic and local immune response, comorbidities, and long-term cardio-vascular complications.
Conclusion: Pneumonia is a complex disease where the interplay between pathogens, immune system and comorbidities not only impose an immediate risk of mortality but also affect the patients’ risk of developing comorbidities as well as mortality for up to a decade after pneumonia has resolved. Our review of unmet needs in CAP research has shown that there are still major shortcomings in our knowledge of CAP.
Recently, a 15-valent (PCV15) and a 20-valent pneumococcal conjugate vaccine (PCV20) have been licensed by the US Food and Drug Administration and are under evaluation by the European Medicines Agency. PCV15 contains all serotypes of the 13-valent conjugate vaccine (PCV13) plus serotype 22F and 33F and PCV20 includes PCV13 serotypes plus serotypes 8, 10A, 11A, 12F, 15B, 22F, 33F. We investigated pneumococcal serotype distribution, secular trends and proportion of pneumonia caused by serotypes included in PCV13, PCV15, PCV20, and the 23-valent pneumococcal polysaccharide vaccine (PPV23) among adult patients with all-cause community-acquired pneumonia (CAP) between 2013 and 2019. We applied logistic mixed regression modelling to assess annual trends. Urine samples from adult patients with CAP treated in the community or hospital in Germany and included in the CAPNETZ study, a prospective multi-centre cohort study, were analysed by two serotype-specific multiplex urinary antigen detection assays (UAD1/UAD2) at Pfizer’s Vaccines Research and Development Laboratory. UAD1 detects serotypes in PCV13, UAD2 detects additional serotypes in PCV20 plus serotypes 2, 9N, 17F and 20. Out of 1,831 patients screened, urine samples with a valid UAD test result were available for 1,343 patients (73.3%). Among those patients, 829 patients (61.7%) were male, 792 patients (59.0%) were aged ≥60 years, 1038 patients (77.3%) had at least one comorbidity and 1,204 patients (89.7%) were treated in the hospital. The overall proportion of vaccine-type pneumonia among all-cause CAP for PCV13, PCV15, PCV20 and PPV23 was 7.7% (n=103), 9.1% (n=122), 12.3% (n=165) and 13.3% (n=178). Over the entire observation period, we did not observe evidence for significant annual trends in pneumococcal vaccine serotype coverage against pneumonia in adults (PCV13: OR 0.94, 95% CI 0.83-1.05; PCV15: OR 0.93, 95% CI 0.84-1.03; PCV20: OR 0.95, 95% CI 0.86-1.04; PPV23: OR 0.99, 95% CI 0.90-1.08). In conclusion, our data show that i) the infant vaccination program of PCV13, which started in Germany 2010 did not result in a relevant and sustained decrease of PCV13 serotypes in pneumonia in adults and ii) that the gap in the coverage between PCV20 and PPV23 was small and did not increase over the entire observation time.
