Intracarotid administration of human bone marrow mononuclear cells in rat photothrombotic ischemia

Background: Increasing evidence suggests that cell therapy improves functional recovery in experimental models of stroke and myocardial infarction. So far only small pilot trials tested the effects of cell therapy in stroke patients, whereas large clinical trials were conducted in patients with ischemic heart disease. To investigate the therapeutic benefit of cell therapy to improve the recovery after stroke, we determined the efficacy of bone marrow derived mononuclear cells, which were shown to improve the recovery in experimental and clinical acute myocardial infarction studies, in a rat stroke model. Methods: Adult male Wistar rats were randomly assigned to receive either five million human bone marrow mononuclear cells (hBMC) or placebo intraarterially 3 days after photothrombotic ischemia. For immunosuppression the animals received daily injections of cyclosporine throughout the experiment, commencing 24 hours before the cell transplantation. A battery of behavioural tests was performed before and up to 4 weeks after ischemia. Results: Body temperature and body weight revealed no difference between groups. Neurological deficits measured by the Rotarod test, the adhesive-removal test and the cylinder test were not improved by hBMC transplantation compared to placebo. Conclusions: This study demonstrates that hBMC do not improve functional recovery when transplanted intraaterially 3 days after the onset of focal cerebral ischemia. A possible reason for the failed neurological improvement after cell therapy might be the delayed treatment initiation compared to other experimental stroke studies that showed efficacy of bone marrow mononuclear cells.
Purpose of the Study: The purpose of the current study was to evaluate the role of radiofrequency (RF) and microwave (MW) ablation in the treatment of pulmonary neoplasms. Materials and Methods: From March 2004 to January 2009, 164 patients (92 males, 72 females; mean age 59.7 years, SD: 10.2) underwent computed tomography (CT)-guided percutaneous RFA of pulmonary malignancies. RFA was performed on 248 lung lesions (20 primary lesions and 228 metastatic lesions) in 248 sessions (one lesion per session). Tumors were pathologically proven and were classified as primary lung neoplasms in 20 patients (non-small cell lung cancer) and as metastatic lung neoplasms in 144 patients. RFA was performed using: a) CelonProSurge bipolar internally cooled applicator b) RITA®StarburstTMXL. From December 2007 to October 2009, 80 patients (30 males, 50 females; mean age 59.7 years, range: 48-68, SD: 6.4) underwent computed tomography (CT) guided percutaneous MW ablation of pulmonary metastases from variable histopathological primaries. MW was performed on 130 lung lesions in 130 sessions (one lesion per session) using Valleylab TM system. Results: The overall success rate of RFA was 67.7% (168/248 lesions), with overall failure rate either due to tumor residue or recurrence on follow up in 32.3% (80/248) with mean time to tumor progress was 5.6 months SD: 2.99 (Range:1-18 months). Complete successful ablation was achieved in patients treated by MWA in 73.1% (95/130 lesions), with failure rate either due to tumor residue or recurrence on follow up in 26.9% (35/130) with mean time to tumor progress 6 months SD: 2.83 (Range:1-12months). Correlation of the histopathological type of the lesion and the end result of ablation therapy revealed insignificant correlation in both RFA and MWA (p > 0.1). The preablation tumor size was one of the most significant factors that determined the end result of ablation. In RFA successful tumor ablation was significant statistically for lesions with maximal axial diameter up to 2.5 cm (110/140) in comparison to lesions of more than 2.5 cm in maximal axial diameter (58/108) (Fisher’s exact test: p < 0.0001). While in MW ablated lesions successful tumor ablation was significant statistically for lesions with maximal axial diameter up to 3 cm (90/110) in comparison to lesions of more than 3 cm in maximal axial diameter (5/20) (Fisher’s exact test: p < 0.001). The location of the lesion was another important factor that determined the end result of ablation. In both RFA and MWA successful ablation was significantly more correlated to peripheral lesions (RFA: 120/160, 80% / MWA: 80/100, 80%) than centrally located lesions (RFA: 48/88, 50%; MWA: 15/30, 50%) (Fisher’s Exact Test: p > 0.001). For successfully RFA ablated cases mean preablation tumor volumes 1.9 cc SD: 0.9 (range: 0.3 - 4.25 cc) while for failed cases the mean tumor volume was 3.7 SD: 2.4 (range: 0.8 – 6.8cc). For successfully MW ablated cases the mean preablation tumor volume: 2.4 cc SD: 2.2 (range: 0.25-8.2 cc) while for failed cases the mean tumor volume was 3.5 SD: 2.6 (range: 0.3 – 7.1 cc). In RFA the survival rates at 12, 24 and 36 months were 90%, 78% and 68% respectively while in MWA treated patients the survival rate within 12 months follow up period was 96% while at 20 month the survival rate was 77%. Complications associated with the ablation therapy were: a) procedure related mortality: 0.4% (1/248) in RFA due to massive pulmonary hemorrhage versus 0% (0/130) in MWA, b) pneumothorax: 11.3% (28/240) in RFA versus 8.5% (11/130) in MWA, c) pulmonary Hemorrhage: 17.7% (44 of 248 sessions) of which one patient had massive uncontrolled bleeding and immediate death versus 6.2% (8/130) in MWA, d) pleural effusion: 3.2 % (8 of 248 sessions) in RFA versus 3.8 % (6/130) in MWA, e) hemoptysis: 4% (10/248) in RFA versus 4.6% (6/130) in MWA ranging from mild tinged sputum to frank bleeding, f) infection: 0.4% (1/248) in RFA, versus 0% in MWA, and g) post ablation pain: 10% (25/248) in RFA versus 9.2% (12/130) in MWA. Pain was generally adequately controlled by analgesics. Conclusion: Radiofrequency and microwave ablation are effective minimally invasive tools and may be safely applied for management of lung malignancy. The success of ablation therapy is significantly correlated to the preablation tumor size, volume and tumor location.
Patienten, die sich einer Bypassoperation am offenen Herzen unterziehen erleiden häufig kognitive Beeinträchtigungen durch Mikroembolien, insbesondere ausgelöst durch Luft. Ins Operationsfeld eingeleitetes Kohlendioxid soll durch seine physikalischen Eigenschaften protektiv auf die kognitiven Funktionen wirken. In die Studie waren 69 Patienten eingeschlossen, die sich jeweils einer aortokoronaren Bypassoperation unter Einsatz der Herz-Lungen-Maschine unterzogen. Es erfolgte randomisiert die Einteilung in eine Kontrollgruppe und eine protegierte Gruppe. Am ersten präoperativen und am fünften postoperativen Tag wurde jeweils eine Testbatterie aus sechs verschiedenen Einzeltest mit den Patienten durchgeführt. Als neurokognitives Defizit wurde eine Verschlechterung der Ergebnisse um mindestens 20% im Vergleich zu den präoperativen Werten in zwei oder mehr Einzeltests definiert. Intraoperativ wurde durch einen Zweikanal-Ultraschall-Mikroblasendetektor, der in den Kreislauf der Herz-Lungen-Maschine eingeschaltet war, ein Monitoring von Größe und Anzahl der entstehenden Gasblasen durchgeführt. Die Gruppen wiesen bezüglich der klinischen Parameter keine signifikant unterschiedlichen Werte auf. Der einzig signifikante Unterschied in den gemessenen Werten der Mikrobubbles ist eine Reduktion der Gasblasen zwischen Kanal I und Kanal II in beiden Gruppen. Dies beweist das Vorhandensein und die Funktionsfähigkeit des arteriellen Filters. Jedoch lässt sich ein Trend zur Verringerung von Blasenanzahl und Blasenvolumen unter Verwendung von CO2 erkennen. Die neurokognitiven Tests konnten keine signifikanten Unterschiede zwischen den Gruppen ausmachen. Somit war kein signifikanter Vorteil für die aortokoronare Bypassoperation unter Protektion durch CO2 auszumachen. Der Neuroscore als Instrument zur Objektivierung eines neurokognitiven Defizits ist diskussionsbedürftig. Da Studien, z. B. im Bereich der minimalinvasiven Chirurgie, den Vorteil von CO2 belegen ist die Anwendung zur Protektion weiter zu empfehlen Jedoch fehlt der endgültige Beweis für den Benefit speziell bei der aortokoronaren Bypassoperation.
Markt- und wettbewerbsorientierte Reformstrategien in den Krankenhaussystemen zahlreicher Industrieländer haben Befürchtungen vor einer kommerzialisierten Krankenhausversorgung hervorgebracht. Dieser Beitrag unterbreitet einen analytischen Interpretationsrahmen zur Erklärung der internationalen Verbreitung dieser Reformstrategien und versucht die behaupteten negativen Effekte von Kommerzialisierungsprozessen auf Versorgungsqualität und Zugänglichkeit zu untersuchen. Gestützt auf einen Vergleich eines idealtypischen Kommerzialisierungsmodells mit dem institutionellen und organisatorischen Wandel im deutschen Krankenhaussystem kommt der Beitrag zu dem Schluss, dass Kommerzialisierungsprozesse in der Krankenhausversorgung bislang noch begrenzt sind. Obwohl ein markt- und wettbewerbsbasierter Umbau der Governancestrukturen zu beobachten ist und Krankenhäuser zu einer Kommerzialisierungsstrategie gedrängt werden, lässt sich aufgrund einer unzureichenden Daten- und Forschungslage bislang nicht empirisch feststellen, ob die Kommerzialisierungsprozesse zu einer Verschlechterung der Qualität und Zugänglichkeit der Krankenhausversorgung geführt haben.
Das Diskussionspapier versucht Dimensionen und Ausmaße von Ökonomisierungs- und Kommerzialisierungsprozessen in OECD-Gesundheitssystemen explorativ zu erörtern. Hierzu wird zunächst die Hypothese entwickelt, dass sich in den (meisten) OECD-Staaten eine hegemoniale gesundheitspolitische Strategie herausbildet, die als wettbewerbsbasierte Kostendämpfungspolitik bezeichnet wird. In der Folge werden die (mutmaßlichen) Auswirkungen von Ökonomisierungs- und Kommerzialisierungsprozessen diskutiert. Erstens wird beschrieben, wie die Monetarisierung der Arzt-Patienten-Beziehung zu einer Privatisierung des Gesundheitssystems führt. Zweitens wird die sich transformierende Arzt-Patienten-Beziehung als Dialektik von Demokratisierungs- und Ökonomisierungsprozessen dargestellt. Drittens beschäftigt sich der Beitrag mit Entwicklung einer neuen Gesundheitskultur, die die gesundheitliche Eigenverantwortung des Einzelnen betont, zugleich jedoch neuen Ausgrenzungs- und Stigmatisierungsprozessen den Weg zu ebnen droht. Abschließend wird ein in groben Zügen ein Forschungsprogramm umschrieben, welches Ökonomisierungs- und Kommerzialisierungsprozesse auf diesen drei Forschungsfeldern analytisch und bewertend unter die Lupe zu nehmen versucht.
Respiratory chain complexes in dynamic mitochondria display a patchy distribution in life cells

Background: Mitochondria, the main suppliers of cellular energy, are dynamic organelles that fuse and divide frequently. Constraining these processes impairs mitochondrial is closely linked to certain neurodegenerative diseases. It is proposed that functional mitochondrial dynamics allows the exchange of compounds thereby providing a rescue mechanism. Methodology/Principal Findings: The question discussed in this paper is whether fusion and fission of mitochondria in different cell lines result in re-localization of respiratory chain (RC) complexes and of the ATP synthase. This was addressed by fusing cells containing mitochondria with respiratory complexes labelled with different fluorescent proteins and resolving their time dependent re-localization in living cells. We found a complete reshuffling of RC complexes throughout the entire chondriome in single HeLa cells within 2–3 h by organelle fusion and fission. Polykaryons of fused cells completely re-mixed their RC complexes in 10–24 h in a progressive way. In contrast to the recently described homogeneous mixing of matrix-targeted proteins or outer membrane proteins, the distribution of RC complexes and ATP synthase in fused hybrid mitochondria, however, was not homogeneous but patterned. Thus, complete equilibration of respiratory chain complexes as integral inner mitochondrial membrane complexes is a slow process compared with matrix proteins probably limited by complete fusion. In co-expressing cells, complex II is more homogenously distributed than complex I and V, resp. Indeed, this result argues for higher mobility and less integration in supercomplexes. Conclusion/Significance: Our results clearly demonstrate that mitochondrial fusion and fission dynamics favours the re-mixing of all RC complexes within the chondriome. This permanent mixing avoids a static situation with a fixed composition of RC complexes per mitochondrion.
Introduction: Acute lung injury (ALI) is an inflammatory disorder of pulmonary or extrapulmonary origin. We have previously demonstrated that netrin-1 dampens murine ALI, and in an attempt to advance this finding into future clinical practice we evaluated whether netrin-1 would reduce alveolar inflammation during porcine ALI. Methods: This was a controlled in vivo experimental study in pigs. We induced ALI through lipoploysaccharide (LPS) infusion (50 micro g/kg) for 2 hours. Following this, we exposed animals to either vehicle, intravenous netrin-1 (netrin-1 i.v.) or inhaled netrin-1 (netrin-1 inh.). Serum samples and bronchoalveolar lavage (BAL) were obtained to determine levels of tumor necrosis factor-alpha (TNF-alpha), interleukin (IL)-1beta, interleukin-6 and interleukin-8 at baseline and 6 hours following treatment. Myeloperoxidase activity (MPO) and protein levels were determined in the BAL, and tissue samples were obtained for histological evaluation. Finally, animals were scanned with spiral CT. Results: Following LPS infusion, animals developed acute pulmonary injury. Serum levels of TNF-alpha and IL-6 were significantly reduced in the netrin-1 i.v. group. BAL demonstrated significantly reduced cytokine levels 6 hours post-netrin-1 treatment (TNF-alpha: vehicle 633 ± 172 pg/ml, netrin-1 i.v. 84 ± 5 pg/ml, netrin-1 inh. 168 ± 74 pg/ml; both P < 0.05). MPO activity and protein content were significantly reduced in BAL samples from netrin-1-treated animals. Histological sections confirmed reduced inflammatory changes in the netrin-1-treated animals. Computed tomography corroborated reduced pulmonary damage in both netrin-1-treated groups. Conclusions: We conclude that treatment with the endogenous anti-inflammatory protein netrin-1 reduces pulmonary inflammation during the initial stages of ALI and should be pursued as a future therapeutic option.
Background: Haemostasis in liver surgery remains a challenge despite improved resection techniques. Oozing from blood vessels too small to be ligated necessitate a treatment with haemostats in order to prevent complications attributed to bleeding. There is good evidence from randomised trials for the efficacy of fibrin sealants, on their own or in combination with a carrier material. A new haemostatic device is Sangustop(R). It is a collagen based material without any coagulation factors. Pre-clinical data for Sangustop(R) showed superior haemostatic effect. This present study aims to show that in the clinical situation Sangustop(R) is not inferior to a carrier-bound fibrin sealant (Tachosil(R)) as a haemostatic treatment in hepatic resection. Methods: This is a multi-centre, patient-blinded, intra-operatively randomised controlled trial. A total of 126 patients planned for an elective liver resection will be enrolled in eight surgical centres. The primary objective of this study is to show the non-inferiority of Sangustop(R) versus a carrier-bound fibrin sealant (Tachosil(R)) in achieving haemostasis after hepatic resection. The surgical intervention is standardised with regard to devices and techniques used for resection and primary haemostasis. Patients will be followed-up for three months for complications and adverse events. Discussion: This randomised controlled trial (ESSCALIVER) aims to compare the new collagen haemostat Sangustop(R) with a carrier-bound fibrin sealant which can be seen as a "gold standard" in hepatic and other visceral organ surgery. If non-inferiority is shown other criteria than the haemostatic efficacy (e.g. costs, adverse events rate) may be considered for the choice of the most appropriate treatment. Background: The integration of the non-cross-resistant chemotherapeutic agents capecitabine and vinorelbine into an intensified dose-dense sequential anthracycline- and taxane-containing regimen in high-risk early breast cancer (EBC) could improve efficacy, but this combination was not examined in this context so far. Methods: Patients with stage II/IIIA EBC (four or more positive lymph nodes) received post-operative intensified dose-dense sequential epirubicin (150mg/m2 every 2 weeks) and paclitaxel (225mg/m2 every 2 weeks) with filgrastim and darbepoetin alfa, followed by capecitabine alone (dose levels 1 and 3) or with vinorelbine (dose levels 2 and 4). Capecitabine was given on days 1-14 every 21 days at 1000 or 1250 mg/m2 twice daily (dose levels 1/2 and 3/4, respectively). Vinorelbine 25 mg/m2 was given on days 1 and 8 of each 21-day course (dose levels 2 and 4). Results: Fifty-one patients were treated. There was one dose-limiting toxicity (DLT) at dose level 1. At dose level 2 (capecitabine and vinorelbine), five of 10 patients experienced DLTs. Therefore evaluation of vinorelbine was abandoned and dose level 3 (capecitabine monotherapy) was expanded. Hand-foot syndrome and diarrhoea were dose limiting with capecitabine 1250 mg/m2 twice daily. At 35.2 months' median follow-up, the estimated 3-year relapse-free and overall survival rates were 82% and 91%, respectively. Administration of capecitabine monotherapy after sequential dose-dense epirubicin and paclitaxel is feasible in node-positive EBC, while the combination of capecitabine and vinorelbine as used here caused more DLTs.
