Hintergrund:
Die Erlernung der Fähigkeit zur Befundbeschreibung im Dermatologie- Blockpraktikum ist trotz aller Bemühungen noch nicht optimal. Feedback hat sich in der Lehre als essenzielle Voraussetzung für den Lernprozess gezeigt, ist aber in der täglichen Lehrpraxis zuweilen schwer umsetzbar. Fragestellung
Lässt sich im Dermatologie-Blockpraktikum das bisher fehlende Feedback bezüglich der Befundbeschreibungen durch eine digitale Lösung in Form einer Webanwendung verbessern? Führt derartiges gegenseitiges Feedback durch die Peers zu einer Verbesserung der Ergebnisse in einem Wissenstest (Multiple-Choice-Fragen) bzw. bei der Bearbeitung eines Patientenfalls? Wie beurteilen die Studierenden diesen Ansatz?
Material und Methode:
Es wurden die Anforderungen an eine Webanwendung definiert, und diese mit Hilfe einer relationalen Datenbank programmiert und bezüglich vordefinierter Gütekriterien getestet, bis das System stabil lief. Im Sommersemester 2014 wurden 12 Gruppen (n=181 Studierende) des Blockpraktikums Dermatologie damit prospektiv untersucht. Es erfolgte eine 1:1 Randomisierung in Kontroll- und Experimentalgruppe. Durch einen organisatorischen Fehler wurde eine der Gruppen, welche als Kontrollgruppe randomisiert wurde, als Experimentalgruppe behandelt und auch so ausgewertet („As treated Analyse“). Für die Studierenden der Kontrollgruppe (n=76) erfolgte das 5-tägige Dermatologie-Blockpraktikum nach Standardablauf. Generell wurden in diesem Praktikum im Rahmen der Hospitation 2 kurze Epikrisen geschrieben. Bisher hatten die Studierenden kein Feedback bezüglich dieser Epikrisen erhalten.
Die Studierenden der Experimentalgruppe (n=105) mussten diese Epikrisen zusätzlich in die Webanwendung eintragen. Nach Ablauf einer Frist von 12 Stunden wurden die Epikrisen von der Webanwendung an zwei weitere Studierende zur Korrektur verteilt. Die korrigierte Fassung wurde den Studierenden wieder zurückgeschickt.
Neben der Abschlussklausur bearbeiteten alle Studierenden am letzten Praktikumstag einen virtuell präsentierten Fall und füllten einen Evaluationsbogen aus.
Ergebnisse:
Die Webanwendung funktionierte bezüglich Programmierung, Speicherung, Algorithmen und Hardware einwandfrei. Weder vom System noch von Studierenden wurden Fehler oder Probleme gemeldet.
Von den 105 eingeschlossen Studierenden der Experimentalgruppe hatten nur 60 Studierende eine Epikrise in der Webanwendung eingetragen. Zudem hatten nur 34 Studierende eine Korrektur für eine fremde Epikrise angefertigt. Keiner der Studierenden hatte wie vorgesehen zwei Korrekturen angefertigt. Die sekundären Studienziele (Ergebnisse der Abschlussklausur und des Abschlussfalls) setzten gemäß des Studienansatzes zwei Korrekturen einer fremden Epikrise voraus. Somit war leider keine aussagekräftige Interpretation dieser Daten möglich. Die Auswertung der vorliegenden Daten ergab geringe Unterschiede mit besserer Punktzahl der Studierenden der Experimentalgruppe.
Die Studierenden hatten keine Probleme mit der Webanwendung und gaben an, das Konzept verstanden zu haben. Es wurde aber durch Studierende der Experimentalgruppe beklagt, dass im Vergleich zum Standardkurs zusätzliche Aktivitäten gefordert wurden.
Schlussfolgerung:
Die entwickelte Webanwendung für das gegenseitige Feedback lief stabil und funktionierte gut. Ohne Kontrolle und Überprüfung wurde sie jedoch von den Studierenden nicht so genutzt wie gewünscht. Der eigentliche Nutzen muss daher in einer künftigen Untersuchung, welche diese Probleme berücksichtigt, festgestellt werden.
Interdisziplinäre Ansätze erhalten in der modernen Medizin immer mehr Bedeutung. Besonders in der Forschung stellen sich viele Themen als zu komplex dar, um nur von Spezialisten erfasst und bearbeitet werden zu können. Dabei werden oft
Verknüpfungen gefunden, die in den evidenzbasierten Kontext eingeordnet, bewertet und in den Praxisalltag implementiert werden müssen. Hierbei sind gegenseitige Einflüsse vom muskuloskelettalen und craniomandibulären System schon lange bekannt aber noch nicht hinreichend systematisch populationsbezogen untersucht.
Ein großer Anteil aktuell verfügbarer Daten über Oberkörperstatik und Okklusion sowie deren Zusammenhänge beruht auf klinischen Erhebungen, die in Zusammenhang von
Diagnostik oder Therapie von Erkrankungen durchgeführt werden.
Normwerte der Oberkörperstatik von gesunden Frauen oder auch Verbindungen zur Okklusion liegen nur für Frauen im Alter von 21-30 Jahren vor, aus diesem Grund war das Ziel dieser Studie diese Interdependenz näher zu betrachten.
Hierzu wurden 101 subjektiv gesunde freiwillige Frauen im Alter von 51-60 (55,16±2,89SD) Jahren untersucht, da zu dieser Altersgruppe keine aussagekräftige Studienlage vorliegt und diese Personengruppe, obwohl sie keinen wachstumsbedingten
Veränderungen unterliegt, weitreichende körperliche Veränderungen im Rahmen der Menopause durchläuft. Es wurden allgemeinanamnestische Daten abgefragt und Wirbelsäulenparameter mittels eines Rückenscanners (backmapper miniRot Kombi, ABW GmbH, Frickenhausen, Deutschland) erhoben, dabei wurden auch Rückenparameter während einer temporären Okklusionssperre mittels Watterollen aufgezeichnet. Zur Durchführung einer Modellanalyse nach Schopf wurden Gipsmodelle der Kiefer angefertigt und vermessen. Mithilfe des Zebris WinJawAnalyzers (Isny, Deutschland) wurde eine axiographische Analyse der Grenzbewegungen der Kiefer
durchgeführt.
Es konnten Normwerte der Oberkörperstatik für die untersuchte Probandinnengruppe erstellt werden. Diese zeigen eine ausbalancierte und nur schwach ausgeprägt asymmetrische Körperhaltung der untersuchten Frauen. Im Vergleich mit Personen anderer Altersgruppen und Geschlechter ergaben sich Unterschiede, die in einen Kontext altersbedingter oder hormoneller Konstitutionsänderungen gesetzt werden konnten.
Eine Untersuchung kurzfristiger, symmetrischer Okklusionsänderung mithilfe von Watterollen im Prämolarenbereich ergab keine Änderung von Parametern der
Wirbelsäule, des Schulter- oder Beckengürtels.
Im Anamnesebogen gesammelte Angaben zu kieferorthopädischer Behandlung, Häufigkeit sportlicher Betätigung, Vorhandensein migräneinduzierter oder anderweitiger Kopfschmerzen und Kiefergelenksgeräuschen ergaben ebenfalls keine signifikanten
Zusammenhänge zur Oberkörperstatik.
Assoziationen bestehen hingegen zwischen modellanalytischen sowie axiographischen Parametern und der Oberkörperstatik. Im Folgenden werden klinisch relevante Wechselbeziehungen aufgeführt:
Bei Betrachtung des Platzangebotes der Stützzonen im Oberkiefer sind die Dornfortsätze der Wirbelkörper bei symmetrischem Platzangebot nach rechts gedreht, wenn jedoch die linke Stützzone ein größeres Platzangebot aufweist als die rechts, so sind die Dornfortsätze eher nach links rotiert.
Im Bereich der Okklusion des linken ersten Molaren wird der Schulterblattabstand und die maximale Rotation der Wirbelkörperdornfortsätze nach rechts tendenziell stärker
wenn der Molar weiter distal okkludiert.
Bei Untersuchung der Protrusion steht bei Hypomobilität des Unterkiefers die linke Schulter cranialer als die rechte, bei Hypermobilität kehrt sich dies jedoch um und die rechte Schulter befindet sich tendenziell in einer höheren Position.
Diese Assoziationen und weitere in dieser Arbeit vorgestellten subklinische Verknüpfungen liefern vielfältige Anhaltspunkte für globale Zusammenhänge zwischen dem craniomandibulären und muskuloskelettalen System. Auf- und Absteigende
Funktionsketten, myofasciale Verbindungen und neuromuskuläre Mechanismen können diesen Ergebnissen zugrunde liegen. Darüber hinaus stellen die hier vorgestellten Messergebnisse Resultate von Momentaufnahmen dar. Deshalb werden zur exakteren Eruierung dieser Ergebnisse zusätzliche Daten zu Verhaltensweisen und Merkmalskombinationen benötigt.
Mithilfe dieser Informationen können dann auch interdisziplinäre Verfahrensweisen der Medizin unterstützt und klinische Therapieansätze verbessert werden. Zum Beispiel
könnten habituell bedingte Fehlbelastungen im Rahmen von craniomandibulären oder spinalen Krankheitsbildern besser verstanden werden und Therapiekonzepte mithilfe von
Orthopäden, Zahnärzten, Physiotherapeuten und Neurologen ausgearbeitet werden.
Simple Summary: Infections are an important cause of morbidity and mortality in childhood cancer treatment. The aim of our retrospective study was to assess the infectious burden in pediatric sarcoma patients during neoadjuvant chemotherapy administered according to the EWING 2008, CWS SoTiSaR and EURAMOS clinical trial or registry. Our analyses indicate a substantial infectious morbidity in this group of patients, with 58.8% experiencing at least one episode of febrile neutropenia (FN) and 20.6% at least one microbiologically documented infection (MDI). We also identified parameters that impact on the occurrence of FN and MDIs, including treatment protocol, patient age, and mucositis. These findings may contribute to a better risk stratification for prevention and management of FN and infections as well as for maintaining quality of life, cost control, and optimum outcomes of anticancer treatment.
Abstract: The purpose of this retrospective, single-center cohort study was to assess the infectious burden in pediatric sarcoma patients during neoadjuvant chemotherapy. The review included all patients with a new diagnosis of Ewing sarcoma, osteosarcoma or soft tissue sarcoma between September 2009 and December 2018 who were enrolled in the EWING 2008, CWS SoTiSaR and EURAMOS clinical trial or registry. Primary endpoints were the occurrence of febrile neutropenia (FN) and microbiologically documented infection (MDI). Parameters with a potential impact on FN and MDI were also analyzed. A total of 170 sarcoma patients (median age: 13 years, range: 0–21; 96 m/74 f) received 948 chemotherapy courses (median: 6; range: 2–8). Of these patients, 58.8% had ≥1 FN episode and 20.6% ≥ 1 MDI. FN occurred in 272/948 courses (28.7%) with fever of unknown origin (FUO) in 231 courses and 45 MDI and 19 clinically documented infections (CDI) occurring in a total of 57 courses. Patients enrolled in EWING 2008 had significantly more FN (p < 0.001), infections (p = 0.02) and MDI (p = 0.035). No infection-related deaths were observed. Younger age, tumor type and localization, and higher median and maximum mucositis grades were significantly associated with higher numbers of FN (p < 0.001), and younger age (p = 0.024) and higher median mucositis grade (p = 0.017) with MDI. The study shows substantial infectious morbidity in sarcoma patients during neoadjuvant chemotherapy treatment and opportunities to improve prevention and management.
Simple Summary: The study compares the effects on complete remission rate (CR) of a single dose of durvalumab/tremelimumab immediately after a single-cycle platinum and docetaxel as part of induction therapy for a controlled trial in head and neck cancer with chemotherapy alone from a historical collective. The CR rate was 60.3% after induction chemoimmunotherapy (ICIT; induction chemotherapy plus double immune checkpoint blockade) compared with 40.3% after induction chemotherapy (IC) alone. Patients with HPV-positive oropharyngeal cancer may benefit the most from additive double checkpoint inhibition, which is presumably due to the higher amount of infiltrating immune cells. Patients older than 60 years without HPV-positive oropharyngeal cancer are unlikely to benefit.
Abstract: To determine whether a single dose of double immune checkpoint blockade (induction chemoimmunotherapy (ICIT)) adds benefit to induction single-cycle platinum doublet (induction chemotherapy (IC)) in locally advanced head and neck squamous cell carcinoma (HNSCC), patients treated with cisplatin 30 mg/m2 d1-3 and docetaxel 75 mg/m2 d1 combined with durvalumab 1500 mg fixed dose d5 and tremelimumab 75 mg fixed dose d5 (ICIT) within the CheckRad-CD8 trial were compared with a retrospective cohort receiving the same chemotherapy (IC) without immunotherapy. The endpoint of this analysis was the complete response rate (CR). A total of 53 patients were treated with ICIT and 104 patients with IC only. CR rates were 60.3% for ICIT and 40.3% for IC (p = 0.018). In the total population (n = 157), the most important predictor to achieve a CR was treatment type (OR: 2.21 for ICIT vs. IC; p = 0.038, multivariate analysis). The most diverse effects in CR rates between ICIT and IC were observed in younger (age ≤ 60) patients with HPV-positive OPSCCs (82% vs. 33%, p = 0.176), while there was no difference in older patients without HPV-positive OPSCCs (53% vs. 48%). The analysis provides initial evidence that ICIT could result in higher CR rates than IC. Young patients with HPV-positive OPSCCs may have the greatest benefit from additional immune checkpoint inhibitors.
Bartonellae are facultative intracellular alpha-proteobacteria often transmitted by arthropods. Ixodes ricinus is the most important vector for arthropod-borne pathogens in Europe. However, its vector competence for Bartonella spp. is still unclear. This study aimed to experimentally compare its vector competence for three Bartonella species: B. henselae, B. grahamii, and B. schoenbuchensis. A total of 1333 ticks (1021 nymphs and 312 adults) were separated into four groups, one for each pathogen and a negative control group. Ticks were fed artificially with bovine blood spiked with the respective Bartonella species. DNA was extracted from selected ticks to verify Bartonella-infection by PCR. DNA of Bartonella spp. was detected in 34% of nymphs and females after feeding. The best engorgement results were obtained by ticks fed with B. henselae-spiked blood (65.3%) and B. schoenbuchensis (61.6%). Significantly more nymphs fed on infected blood (37.3%) molted into adults compared to the control group (11.4%). Bartonella DNA was found in 22% of eggs laid by previously infected females and in 8.6% of adults molted from infected nymphs. The transovarial and transstadial transmission of bartonellae suggest that I. ricinus could be a potential vector for three bacteria.
Despite increased public health awareness, atherosclerosis remains a leading cause of mortality worldwide. Significant variations in response to statin treatment have been noted among different populations suggesting that the efficacy of statins may be altered by both genetic and environmental factors. The existing literature suggests that certain long noncoding RNAs (lncRNAs) might be up- or downregulated among patients with atherosclerosis. LncRNA may act on multiple levels (cholesterol homeostasis, vascular inflammation, and plaque destabilization) and exert atheroprotective or atherogenic effects. To date, only a few studies have investigated the interplay between statins and lncRNAs known to be implicated in atherosclerosis. The current review characterizes the role of lncRNAs in atherosclerosis and summarizes the available evidence related to the effect of statins in regulating lncRNAs.
Objectives: A conometric concept was recently introduced in which conical implant abutments hold the matching crown copings by friction alone, eliminating the need for cement or screws. The aim of this in vitro study was to assess the presence of microgap formation and bacterial leakage at the Acuris conometric restorative interface of three different implant abutment systems. Material and methods: A total of 75 Acuris samples of three implant-abutment systems (Ankylos, Astra Tech EV, Xive) were subjected to microbiological (n = 60) and scanning electron microscopic (SEM) investigation (n = 15). Bacterial migration into and out of the conical coupling system were analyzed in an anaerobic workstation for 48, 96, 144, and 192 h. Bacterial DNA quantification using qrt-PCR was performed at each time point. The precision of the conometric coupling and internal fit of cemented CAD/CAM crowns on corresponding Acuris TiN copings were determined by means of SEM. Results: qrt-PCR results failed to demonstrate microbial leakage from or into the Acuris system. SEM analysis revealed minute punctate microgaps at the apical aspect of the conometric junction (2.04 to 2.64 µm), while mean cement gaps of 12 to 145 µm were observed at the crown-coping interface. Conclusions: The prosthetic morse taper connection of all systems examined does not allow bacterial passage. Marginal integrity and internal luting gap between the ceramic crown and the coping remained within the clinically acceptable limits. Clinical relevance: Conometrically seated single crowns provide sufficient sealing efficiency, relocating potential misfits from the crown-abutment interface to the crown-coping interface.
Hintergrund: Gut durchgeführte Wiederbelebungsmaßnahmen können bei einem Herz-Kreislauf-Stillstand das Outcome verbessern. Um praktische Fähigkeiten zu erlernen, greifen Medizinstudierende oft auf Lehrvideos zurück. Studien zeigen jedoch häufig eine unzureichende Qualität der im Internet zur Verfügung gestellten Videos zu Reanimationsmaßnahmen. Eine Bewertung anhand einer validierten, auf den aktuellen „guidelines“ basierten Checkliste fehlt bisher. Ziel der Arbeit: Entwicklung und Validierung einer Checkliste zur Bewertung von Lehrvideos zur Reanimation. Material und Methoden: In einem Expertenworkshop erfolgte basierend auf den aktuellen „guidelines“ die Formulierung der Checklistenitems. Die Checkliste wurde in einem vierstufigen Reviewprozess von Notärzten getestet. Die Bewertungen wurden analysiert und die Items angepasst und spezifiziert. Nach dem Reviewprozess wurde die Checkliste an 74 Videos zur Reanimation angewendet. Ergebnisse: Die Checkliste umfasst 25 Items in vier Kategorien (initiale Maßnahmen, Thoraxkompression, AED-Nutzung, Atmung), die auf einer 3 stufigen Likert-Skala bewertet werden. 16 NotärztInnen nahmen an der Studie teil. Sie bewerteten jeweils durchschnittlich 9,3 ± 5,7 Videos. Die Reviewer stimmten in 65,1 ± 12,6 % der Fälle überein. Die höchsten Übereinstimmungen wurden im Unterthema AED erzielt, das Item „Beim Schock Patienten nicht berühren“ wies die höchste Übereinstimmung auf. Die Items der Kategorie Thoraxkompression wurden am häufigsten unterschiedlich bewertet. Diskussion: Es konnte erstmalig für den deutschsprachigen Raum eine Checkliste zur Bewertung von Lehrvideos zur Reanimation erstellt und validiert werden.
Background: The aim of this pilot study was to analyze the work of neurologists regarding static posture (> 4 s) and to identify awkward postures. Methods: A total of 9 neurologists (assistant physicians; 3 male, 6 female) participated in this study. Kinematic data were collected using the computer-assisted acquisition and long-term analysis of musculoskeletal loads (CUELA; IFA, Sankt Augustin, Germany) system. Daily work (“office work,” “measures on patients,” and “other activities”) was analyzed with a computer-based task analysis. Results: During ”measures on patients,” more than 80% of the total percentage of non-neutral posture was assumed with a flexed position of the head and entire back, both during “blood collection” (4.7% of the time) and while “placing intravenous catheters” (8.3% of the time). In contrast, long static postures (> 30 s) in the head and neck area, including the thoracic spine, were adopted during “office work.” Despite the increased total percentage of non-neutral attitudes during measures on patients, the time share of 3.4% of the total working time is so small that the risk for developing musculoskeletal disorders (MSD) is negligible. In contrast, office work, which comprises 50.8% of the total working time and longer static postures, has a potential risk for the development of MSD. Conclusion: The present study is the first kinematic pilot analysis in the field of in-patient neurological assistants. Non-neutral as well as static postures in everyday work could be identified. Potential MSD can be reduced by optimizing the working height and by taking regular breaks to loosen the musculoskeletal system.
Mitochondrial dysfunction may activate innate immunity, e.g. upon abnormal handling of mitochondrial DNA in TFAM mutants or in altered mitophagy. Recent reports showed that also deletion of mitochondrial matrix peptidase ClpP in mice triggers transcriptional upregulation of inflammatory factors. Here, we studied ClpP-null mouse brain at two ages and mouse embryonal fibroblasts, to identify which signaling pathways are responsible, employing mass spectrometry, subcellular fractionation, immunoblots, and reverse transcriptase polymerase chain reaction. Several mitochondrial unfolded protein response factors showed accumulation and altered migration in blue-native gels, prominently the co-chaperone DNAJA3. Its mitochondrial dysregulation increased also its extra-mitochondrial abundance in the nucleus, a relevant observation given that DNAJA3 modulates innate immunity. Similar observations were made for STAT1, a putative DNAJA3 interactor. Elevated expression was observed not only for the transcription factors Stat1/2, but also for two interferon-stimulated genes (Ifi44, Gbp3). Inflammatory responses were strongest for the RLR pattern recognition receptors (Ddx58, Ifih1, Oasl2, Trim25) and several cytosolic nucleic acid sensors (Ifit1, Ifit3, Oas1b, Ifi204, Mnda). The consistent dysregulation of these factors from an early age might influence also human Perrault syndrome, where ClpP loss-of-function leads to early infertility and deafness, with subsequent widespread neurodegeneration.
In Deutschland erkranken pro Jahr bis zu 12.000 Menschen neu an Leukämie. Leukämie ist eine schwere onkologische Erkrankung, bei der reifes Knochenmarkgewebe in Folge von Mutationen unreifer und defekter Vorläuferzellen (leukämischen Blasten) verdrängt wird. Dies führt zu einer zunehmend eingeschränkten Blutbildung. Akute Leukämieformen können unbehandelt innerhalb von wenigen Wochen zum Tode führen und erfordern deshalb eine umgehende Diagnostik sowie einen raschen Therapiebeginn. Heilungschancen bestehen dann, wenn durch die Transplantation von gesunden hämatopoetischen Stammzellen (HSZT) das erkrankte Knochenmark ausreichend ersetzt wird. Leider sind Abstoßungsreaktionen des Spendermaterials (engl.: Graft-versus-Host-Disease, GvHD) keine Seltenheit.
Natürliche Killerzellen (NK-Zellen) stellen die kleinste Lymphozytenpopulation im menschlichen Blut dar und werden dem angeborenen Immunsystem zugerechnet. Sie wurden erstmals 1975 durch die Forscher Kiessling, Klein et al. entdeckt.17 Aufgrund ihrer Fähigkeit bestimmte Tumorzellen in vitro zu töten, wächst das Interesse an der Erforschung ihrer aktivierenden und inhibierenden Oberflächenrezeptoren. Die Killer-Zell-Immunoglobulin-ähnlichen Rezeptoren (KIRs) bilden dabei eine besonders diverse NKZell-Rezeptorfamilie. Lokalisiert auf Chromosom 19 liegen bis zu 17 hochpolymorphe KIRGene. Die genetische Ausstattung und Oberflächenexpression variiert von Individuum zu Individuum und bildet die Voraussetzung für die vorhandene Diversität KIR-exprimierender NK-Zellen. NK-Zellen besitzen die Fähigkeit, Gewebezellen in „körpereigen“ oder „fremd“ zu kategorisieren. Inhibitorische Killer-Immunoglobulin-ähnliche Rezeptoren (iKIR) nutzen dazu HLA-Klasse-I-Proteine (MHC-I) auf der Oberfläche gesunder Zellen. Diese schützen sie vor einem zytotoxischen NK-Zell-Angriff. NK-Zellen durchlaufen im Vorfeld einen komplexen Ausbildungssprozess48, an dessen Ende lizensierte Effektorzellen stehen. Diese können mittels gezielter Zytolyse krebstransformierte, zellulär-gestresste, sowie viralinfizierte Zellen im intakten Organismus erkennen und abtöten.
Die Spenderauswahl ist ein wichtiger Faktor für den Erfolg einer Stammzelltransplantation. Infundierte Spender NK-Zellen schützen das Transplantat, indem sie als wirksame Effektorzellen verbleibende Leukämiezellen aktiv eliminieren. Diese wünschenswerte Nebenwirkung wird als Graft-versus-Leukämie (GvL)-Effekt bezeichnet. Ruggeri et al. konnte zeigen, dass insbesondere Transplantationsstrategien, die auf KIR-Ligand-Fehlpaarungen (engl.: KIR-HLA-mismatch) basieren, zu weniger Rückfällen, weniger GvHD und einem besseren Gesamtüberleben bei Patienten mit akuter myeloischer Leukämie (AML) nach HSZT führt. Der KIR-HLA-mismatch wird mittlerweile aufgrund ausreichender Datenlage bei der Auswahl passender NK-Zell-Spender berücksichtigt und die Untersuchung auf die An- bzw. Abwesenheit bestimmter KIR-Gene (Haplotypisierung)
mittlerweile neben der HLA-Typisierung standardisiert in vielen Instituten durchgeführt. Daneben finden sich immer mehr Hinweise dafür, dass bereits einzelne allelische Polymorphismen innerhalb der KIR-Gene einzelner Spender großen Einfluss auf die Funktionalität ihrer NK-Zellen nehmen. Die allelische Subtypisierung von KIRs stellt aufgrund stetig steigender Zahlen neu entdeckter Allele eine Herausforderung dar. Im Januar 2019 sind für KIR2DL1 bereits 66 Allele beschrieben und für KIR3DL1 sogar 150 Allele in der Immuno Polymorphism Database (IPD) hinterlegt.
Die vorliegende Arbeit präsentiert ein praktikables Subtypisierungsverfahren, um allelische Unterschiede innerhalb der Genloci der NK-Zell-Rezeptoren KIR2DL1 und KIR3DL1 zu untersuchen. Für die Experimente wurden NK-Zellen von 20 gesunden Spendern funktionell untersucht und KIR-genetisch analysiert. Ziel war es innerhalb dieser Individuen besonders potente NK-Zellspender zu identifizieren und diese anhand bestimmter Polymorphismen und/ oder der Expression von KIR-Rezeptoren zu charakterisieren. Bei der Subtypisierung der KIR2DL1-Gene konnten 12 verschiedene, bereits bekannte KIR2DL1-Allele bestimmt werden. Die häufigsten Allele waren dabei 2DL1*001, *00201, *00302, *00401 und *00403. 5 der 20 Spender konnten der funktionell hochpotenten R245–Allelgruppe (AS Arginin an Pos. 245) zugeordnet werden. Spender 13 zeigte bei negativer KIR2DL1-SSP eine vermeintlich neue Nullallelvariante, Spender 20 eine neue heterozygote Variante, resultierend in der Kombination eines Arginin mit Alanin (R/A). Bei der allelischer Subtypisierung von KIR3DL1 wurden 25 verschiedene, bereits bekannte KIR3DL1-Allele bei den 20 Spendern bestimmt. Spender 2 zeigt zahlreiche, vorwiegend homozygote Abweichungen von der Referenzfrequenz, insbesondere im Exon 5, und wurde als neue Allelvariante gewertet. Die häufigsten KIR3DL1-Allele waren 3DL1*00101, *002 und *087. Mittels durchflusszytometrischer Messung konnte gezeigt werden, dass das bekannte Nullallel 3DL1*0040101 bei Spender 14 zu keiner Oberflächenexpression des Rezeptors führt215, während Spendern 11 und 16 als Träger des 3DL1*00402 Allels eine Oberflächenexpression von rund 10% präsentierten. Um die Spender NK-Zellen der gebildeten Gruppen funktionell zu testen, wurden die NK-Zellen experimentell mit vier unterschiedlichen transgenen L721.221-Zelllinien stimuliert. Die funktionelle Potenz der
gespendeten NK-Zellen wurde mittels eines CD107-Degranulationsassays gemessen. Nach aktuellem Stand sind nur für 53 der 150 KIR3DL1-Allele die allelischen Expressionsmuster untersucht worden. Dies bedeutet im Umkehrschluss, dass für rund 65% der bekannten KIR3DL1-Allele Daten zur Funktionalität fehlen, und damit der größte Anteil der ermittelten Allele von 6 Spendern der unknown-Expression (KIR3DL1u/u) Gruppe zugeordnet wurde. Die Spender 4 und 19 der high-Expressiongruppe (KIR3DL1h/h), sowie Spender 5 und 10 der KIR3DL1u/u-Gruppe mit den Allelen 3DL1*053, *087, *109 und Spender 2 als Träger zweier neuer KIR3DL1-Allele, zeigten in toto die besten funktionellen Ergebnisse in den Experimenten gegen die verwendete B-lymphoblastoide L721.221-Zellinien. Die Ergebnisse der vorliegenden Arbeit zeigen, dass bei der Spenderauswahl für NK-Zellbasierten Immuntherapie neben der Genotypisierung die allelische KIR-Subtypisierung als wertvolles Werkzeug entschiedener berücksichtigt werden sollte. Dafür ist es jedoch notwendig weiter an KIR-Subtypisierung und -Gruppierung Strategien zu arbeiten, um Natürlichen Killerzellen Wege in die klinische Standardpraxis zu bahnen.
Adolescence has been linked to an enhanced tolerance of uncertainty and risky behavior and is possibly connected to an increased response toward rewards. However, previous research has produced inconsistent findings. To investigate whether these findings are due to different reward probabilities used in the experimental design, we extended a monetary incentive delay (MID) task by including three different reward probabilities. Using functional magnetic resonance imaging, 25 healthy adolescents and 22 adults were studied during anticipation of rewards in the VS. Differently colored cue stimuli indicated either a monetary or verbal trial and symbolized different reward probabilities, to which the participants were blinded. Results demonstrated faster reaction times for lower reward probabilities (33%) in both age groups. Adolescents were slower through all conditions and had less activation on a neural level. Imaging results showed a three-way interaction between age group x condition x reward probability with differences in percent signal change between adolescents and adults for the high reward probabilities (66%, 88%) while adolescents demonstrated differences for the lowest (33%). Therefore, previous inconsistent findings could be due to different reward probabilities, which makes examining these crucial for a better understanding of adolescent and adult behavior.
Promyelocytic leukemia nuclear bodies (PML NBs) are multi-protein assemblies representing distinct sub-nuclear structures. As phase-separated molecular condensates, PML NBs exhibit liquid droplet-like consistency. A key organizer of the assembly and dynamics of PML NBs is the ubiquitin-like SUMO modification system. SUMO is covalently attached to PML and other core components of PML NBs thereby exhibiting a glue-like function by providing multivalent interactions with proteins containing SUMO interacting motifs (SIMs). PML NBs serve as the catalytic center for nuclear SUMOylation and SUMO-SIM interactions are essential for protein assembly within these structures. Importantly, however, formation of SUMO chains on PML and other PML NB-associated proteins triggers ubiquitylation and proteasomal degradation which coincide with disruption of these nuclear condensates. To date, a plethora of nuclear activities such as transcriptional and post-transcriptional regulation of gene expression, apoptosis, senescence, cell cycle control, DNA damage response, and DNA replication have been associated with PML NBs. Not surprisingly, therefore, SUMO-dependent PML NB integrity has been implicated in regulating many physiological processes including tumor suppression, metabolism, drug-resistance, development, cellular stemness, and anti-pathogen immune response. The interplay between PML NBs and viral infection is multifaceted. As a part of the cellular antiviral defense strategy, PML NB components are crucial restriction factors for many viruses and a mutual positive correlation has been found to exist between PML NBs and the interferon response. Viruses, in turn, have developed counterstrategies for disarming PML NB associated immune defense measures. On the other end of the spectrum, certain viruses are known to usurp specific PML NB components for successful replication and disruption of these sub-nuclear foci has recently been linked to the stimulation rather than curtailment of antiviral gene repertoire. Importantly, the ability of invading virions to manipulate the host SUMO modification machinery is essential for this interplay between PML NB integrity and viruses. Moreover, compelling evidence is emerging in favor of bacterial pathogens to negotiate with the SUMO system thereby modulating PML NB-directed intrinsic and innate immunity. In the current context, we will present an updated account of the dynamic intricacies between cellular PML NBs as the nuclear SUMO modification hotspots and immune regulatory mechanisms in response to viral and bacterial pathogens.
Macrophages not only represent an integral part of innate immunity but also critically contribute to tissue and organ homeostasis. Moreover, disease progression is accompanied by macrophage accumulation in many cancer types and is often associated with poor prognosis and therapy resistance. Given their critical role in modulating tumor immunity in primary and metastatic brain cancers, macrophages are emerging as promising therapeutic targets. Different types of macrophages infiltrate brain cancers, including (i) CNS resident macrophages that comprise microglia (TAM-MG) as well as border-associated macrophages and (ii) monocyte-derived macrophages (TAM-MDM) that are recruited from the periphery. Controversy remained about their disease-associated functions since classical approaches did not reliably distinguish between macrophage subpopulations. Recent conceptual and technological advances, such as large-scale omic approaches, provided new insight into molecular profiles of TAMs based on their cellular origin. In this review, we summarize insight from recent studies highlighting similarities and differences of TAM-MG and TAM-MDM at the molecular level. We will focus on data obtained from RNA sequencing and mass cytometry approaches. Together, this knowledge significantly contributes to our understanding of transcriptional and translational programs that define disease-associated TAM functions. Cross-species meta-analyses will further help to evaluate the translational significance of preclinical findings as part of the effort to identify candidates for macrophage-targeted therapy against brain metastasis.
Background: Autobiographical memory (AM) changes are the hallmark of Alzheimer's disease (AD) and mild cognitive impairment (MCI). In recent neuroimaging studies, AM changes have been associated with numerous cerebral sites, such as the frontal cortices, the mesial temporal lobe, or the posterior cingulum. Regional glucose uptake in these sites was investigated for underlying subdimensions using factor analysis. Subsequently, the factors were examined with respect to AM performance in a subgroup of patients.
Methods: Data from 109 memory clinic referrals, who presented with MCI (n = 60), mild AD (n = 49), or were cognitively intact, were analyzed. The glucose metabolic rates determined by positron emission tomography (PET) with 18F-fluorodeoxyglucose (FDG) in 34 cerebral sites important for AM were investigated for underlying subdimensions by calculating factor analysis with varimax rotation. Subsequently, the respective factor scores were correlated with the episodic and semantic AM performance of 22 patients, which was measured with a semi-structured interview assessing episodic memories (characterized by event-related emotional, sensory, contextual, and spatial–temporal details) and personal semantic knowledge from three periods of life (primary school, early adulthood, and recent years).
Results: Factor analysis identified seven factors explaining 69% of the variance. While patients with MCI and AD showed lower values than controls on the factors frontal cortex, mesial temporal substructures, and occipital cortex, patients with MCI presented with increased values on the factors posterior cingulum and left temporo-prefrontal areas. The factors anterior cingulum and right temporal cortex showed only minor, non-significant group differences. Solely, the factor mesial temporal substructures was significantly correlated with both episodic memories (r = 0.424, p < 0.05) and personal semantic knowledge (r = 0.547, p < 0.01) in patients with MCI/AD.
Conclusions: The factor structure identified corresponds by large to the morphological and functional interrelations of the respective sites. While reduced glucose uptake on the factors frontal cortex, mesial temporal substructures, and occipital cortex in the patient group may correspond to neurodegenerative changes, increased values on the factors posterior cingulum and left temporo-prefrontal areas in MCI may result from compensatory efforts. Interestingly, changes of the mesial temporal substructures were correlated with both semantic and episodic AM. Our findings suggest that AM deficits do not only reflect neurodegenerative changes but also refer to compensatory mechanisms as they involve both quantitative losses of specific memories and qualitative changes with a semantization of memories.
Die Pathophysiologie der Bandscheibendegeneration (intervertebral disc degeneration, IVDD) und ihre molekularen Mechanismen sind noch in weiten Teilen unverstanden. Ihre Ursachen und Risikofaktoren sind vielfältig und schließen unter anderem Alter, Geschlecht, Umwelteinflüsse oder mechanische Belastungen mit ein.
Für das der Bandscheibe eng verwandte Knorpelgewebe wurde in aktuellen Studien der Einfluss des Sympathikus bzw. dessen Neurotransmitters Noradrenalin (NE) via adrenerger Rezeptoren (AR) auf die Zellproliferation, die Expression von Molekülen der extrazellulären Matrix und somit auch auf die Degeneration beschrieben. In Bandscheiben wurde bereits das Vorhandensein von sympathischen Nervenendigungen nachgewiesen, allerdings wurde die Expression der Adrenozeptoren hier noch nie untersucht. Das Ziel der vorliegenden Arbeit war also die Analyse der ARs im Gewebe der Bandscheibe und die Evaluation der Korrelation mit der Bandscheibendegeneration.
Das für die Analyse benötigte Gewebe stammt von Patienten, bei welchen eine Wirbelkörperverblockung (Spondylodese) durchgeführt wurde. Im Rahmen dieser Spondylodese wird das Bandscheibengewebe des betroffenen Segmentes entfernt. Der Degenerationsgrad der anonymisierten Proben wurde prä- und intraoperativ bestimmt und im entnommenen Gewebe sowie in isolierten Zellen die Expression aller bekannten ARs mittels reverse transcription polymerase chain reaction (RT-PCR) untersucht. Zum Nachweis der ARs auf Proteinebene wurden einzelne humane Proben auch immunhistochemisch analysiert. Des Weiteren wurde anhand von Wildtyp- und sogenannten SM/J-Mäusen, die eine spontane IVDD entwickeln, die Proteinexpression der ARs und der extrazellulären Matrix (ECM) von gesunden und geschädigten Bandscheiben an histologischen Schnitten verglichen. Schließlich wurde an isolierten und kultivierten humanen Zellen ein Stimulationsversuch mit Noradrenalin durchgeführt, um zu prüfen, ob es nach Aktivierung der ARs zu einer intrazellulären Signalweiterleitung kommt.
In Nativgewebe der humanen Bandscheibe konnte die messenger Ribonukleinsäure (mRNA) von α1a-, α1b-, α2a-, α2b-, α2c-, β1- und β2-ARs nachgewiesen werden. Nach siebentägiger Zellkultur im Monolayer präsentierte sich ein nur dezent abweichendes Genexpressionsmuster. Auf Proteinebene war das Signal des β2-AR nur im Bereich des Annulus fibrosus (AF) detektierbar jedoch nicht im Nucleus pulposus (NP). Selbiges war auch in murinen Schnitten festzustellen, wobei sich bei Wildtype (WT)-Mäusen hauptsächlich im inneren AF β2-positive Zellen fanden, während sich das Signal bei der SM/J-Maus weiter in Richtung des äußeren AF und des NP ausdehnte. α2a-AR und α2c-AR waren hingegen auf Proteinebene nicht nachweisbar. Bei der immunhistochemischen Untersuchung relevanter ECM-Moleküle zeigte sich für Kollagen II, Kollagen XII, cartilage oligomeric matrix protein (COMP) und Decorin (DCN) eine Verteilung, die mit der des β2-AR-Signals korreliert. Der Stimulationsversuch in humaner Zellkultur ergab eine Aktivierung der für die ARs relevanten Proteinkinase A (PKA)- und extracellular signal–regulated kinases (ERK1/2) -Signalwege.
In der vorliegenden Arbeit konnte zum ersten Mal die Existenz und Funktionalität von Adrenozeptoren im Bandscheibengewebe nachgewiesen werden. Unterschiede in der Expression der ARs, kombiniert mit Veränderungen der ECM-Zusammensetzung könnten ein Hinweis auf den Einfluss des Sympathikus bei IVDD sein. Die aktuelle demographische Entwicklung und die sich hieraus ergebende gesundheitsökonomische Belastung machen die Ergründung molekularer Mechanismen der IVDD und die daraus resultierende Entwicklung innovativer Behandlungsmethoden zu Kardinalfragen moderner orthopädischer Grundlagenforschung.
The aim of this retrospective study was to assess the outcome of patients with metastasized castration-resistant early-onset prostate cancer refractory to chemotherapy receiving radioligand therapy with 177Lutetium-PSMA-617 (LuPSMA-RLT). Twenty-five patients of ≤55 years of age at prostate cancer diagnosis, treated with a median of four (IQR 2–6) cycles (mean of 7.7 ± 1.4 GBq per cycle) every 6–8 weeks, were analyzed. Survival outcome was calculated based on the Kaplan–Meier method. The median progression-free survival (PFS) was 3.8 months (95% CI 2.3–5.3), and overall survival (OS) was 8.5 months (95% CI 6.2–10.8). An initial PSA reduction (≥ 50%) was observed in 9/25 (36%) of patients without being significantly associated with OS (p = 0.601). PSA response (PSA decline ≥50% at 12 weeks) was observed in 12/25 (48%) of patients and significantly associated with longer OS (16.0 months, 95% CI 7.4–24.6 vs. 4.0 months, 95% CI 1.1–6.9, p = 0.002). Imaging-based response using 68Ga-PSMA-11-PET/CT after two to three cycles was seen in 11/25 (44%). Additionally, responders had a significantly longer median PFS (8.7 months, 95% CI 1.3–16.1 vs. 1.9 months, 95% CI 1.7–2.2, p < 0.001) and OS (16.0 months, 95% CI 7.6–24.4 vs. 4.0 months, 95% CI 0.9–7.1; p = 0.002). Intra- or post-therapeutic toxicity was graded according to the CTCAE v5.0 criteria. Newly developing grade ≥ 3 anemia, leukopenia, and thrombocytopenia occurred in three (12%), one (4%), and three (12%) patients, respectively. One patient showed renal toxicity (grade ≥ 3) during follow-up. Pain palliation (>2 level VAS decline) was achieved in 9/14 (64%) and performance status improvement (ECOG level decline ≥ 1) in 8/17 (47%) of patients. Compared to previous reports, radioligand therapy with 177Lu-PSMA-617 in metastasized castration-resistant early-onset prostate cancer patients refractory to chemotherapy yields similar response rates with a comparable safety profile, but is associated with shorter survival.
Lifestyle interventions, including meal replacement, are effective in the prevention and treatment of type-2-diabetes and obesity. Since insulin is the key weight regulator, we hypothesised that the addition of meal replacement to a lifestyle intervention reduces insulin levels more effectively than lifestyle intervention alone. In the international multicentre randomised controlled ACOORH (Almased Concept against Overweight and Obesity and Related Health Risk) trial, overweight or obese persons who meet the criteria for metabolic syndrome (n = 463) were randomised into two groups. Both groups received nutritional advice focusing on carbohydrate restriction and the use of telemonitoring devices. The intervention group substituted all three main meals per day in week 1, two meals per day in weeks 2–4, and one meal per day in weeks 5–26 with a protein-rich, low-glycaemic meal replacement. Data were collected at baseline and after 1, 3, 6 and 12 months. All datasets providing insulin data (n = 446) were included in this predefined subanalysis. Significantly higher reductions in insulin (−3.3 ± 8.7 µU/mL vs. −1.6 ± 9.8 µU/mL), weight (−6.1 ± 5.2 kg vs. −3.2 ± 4.6 kg), and inflammation markers were observed in the intervention group. Insulin reduction correlated with weight reduction and the highest amount of weight loss (−7.6 ± 4.9 kg) was observed in those participants with an insulin decrease > 2 µU/mL. These results underline the potential for meal replacement-based lifestyle interventions in diabetes prevention, and measurement of insulin levels may serve as an indicator for adherence to carbohydrate restriction.
Simple Summary: Acute myeloid leukemia (AML) is a genetically heterogeneous disease. Clinical phenotypes of frequent mutations and their impact on patient outcome are well established. However, the role of rare mutations often remains elusive. We retrospectively analyzed 1529 newly diagnosed and intensively treated AML patients for mutations of BCOR and BCORL1. We report a distinct co-mutational pattern that suggests a role in disease progression rather than initiation, especially affecting mechanisms of DNA-methylation. Further, we found loss-of-function mutations of BCOR to be independent markers of poor outcomes in multivariable analysis. Therefore, loss-of-function mutations of BCOR need to be considered for AML management, as they may influence risk stratification and subsequent treatment allocation.
Abstract: Acute myeloid leukemia (AML) is characterized by recurrent genetic events. The BCL6 corepressor (BCOR) and its homolog, the BCL6 corepressor-like 1 (BCORL1), have been reported to be rare but recurrent mutations in AML. Previously, smaller studies have reported conflicting results regarding impacts on outcomes. Here, we retrospectively analyzed a large cohort of 1529 patients with newly diagnosed and intensively treated AML. BCOR and BCORL1 mutations were found in 71 (4.6%) and 53 patients (3.5%), respectively. Frequently co-mutated genes were DNTM3A, TET2 and RUNX1. Mutated BCORL1 and loss-of-function mutations of BCOR were significantly more common in the ELN2017 intermediate-risk group. Patients harboring loss-of-function mutations of BCOR had a significantly reduced median event-free survival (HR = 1.464 (95%-Confidence Interval (CI): 1.005–2.134), p = 0.047), relapse-free survival (HR = 1.904 (95%-CI: 1.163–3.117), p = 0.01), and trend for reduced overall survival (HR = 1.495 (95%-CI: 0.990–2.258), p = 0.056) in multivariable analysis. Our study establishes a novel role for loss-of-function mutations of BCOR regarding risk stratification in AML, which may influence treatment allocation.
Diabetes mellitus is the fifth most common cause of death worldwide. Due to its chronic nature, diabetes is a debilitating disease for the patient and a relevant cost for the national health system. Type 2 diabetes mellitus is the most common form of diabetes mellitus (90% of cases) and is characteristically multifactorial, with both genetic and environmental causes. Diabetes patients display a significant increase in the risk of developing cardiovascular disease compared to the rest of the population. This is associated with increased blood clotting, which results in circulatory complications and vascular damage. Platelets are circulating cells within the vascular system that contribute to hemostasis. Their increased tendency to activate and form thrombi has been observed in diabetes mellitus patients (i.e., platelet hyperactivity). The oxidative damage of platelets and the function of pro-oxidant enzymes such as the NADPH oxidases appear central to diabetes-dependent platelet hyperactivity. In addition to platelet hyperactivity, endothelial cell damage and alterations of the coagulation response also participate in the vascular damage associated with diabetes. Here, we present an updated interpretation of the molecular mechanisms underlying vascular damage in diabetes, including current therapeutic options for its control.
Quantitative MRI allows to probe tissue properties by measuring relaxation times and may thus detect subtle changes in tissue composition. In this work we analyzed different relaxation times (T1, T2, T2* and T2′) and histological features in 321 samples that were acquired from 25 patients with newly diagnosed IDH wild-type glioma. Quantitative relaxation times before intravenous application of gadolinium-based contrast agent (GBCA), T1 relaxation time after GBCA as well as the relative difference between T1 relaxation times pre-to-post GBCA (T1rel) were compared with histopathologic features such as the presence of tumor cells, cell and vessel density, endogenous markers for hypoxia and cell proliferation. Image-guided stereotactic biopsy allowed for the attribution of each tissue specimen to its corresponding position in the respective relaxation time map. Compared to normal tissue, T1 and T2 relaxation times and T1rel were prolonged in samples containing tumor cells. The presence of vascular proliferates was associated with higher T1rel values. Immunopositivity for lactate dehydrogenase A (LDHA) involved slightly longer T1 relaxation times. However, low T2′ values, suggesting high amounts of deoxyhemoglobin, were found in samples with elevated vessel densities, but not in samples with increased immunopositivity for LDHA. Taken together, some of our observations were consistent with previous findings but the correlation of quantitative MRI and histologic parameters did not confirm all our pathophysiology-based assumptions.
Olfactory self-assessments have been analyzed with often negative but also positive conclusions about their usefulness as a surrogate for sensory olfactory testing. Patients with nasal polyposis have been highlighted as a well-predisposed group for reliable self-assessment. In a prospective cohort of n = 156 nasal polyposis patients, olfactory threshold, odor discrimination, and odor identification were tested using the “Sniffin’ Sticks” test battery, along with self-assessments of olfactory acuity on a numerical rating scale with seven named items or on a 10-point scale with only the extremes named. Apparent highly significant correlations in the complete cohort proved to reflect the group differences in olfactory diagnoses of anosmia (n = 65), hyposmia (n = 74), and normosmia (n = 17), more than the true correlations of self-ratings with olfactory test results, which were mostly very weak. The olfactory self-ratings correlated with a quality of life score, however, only weakly. By contrast, olfactory self-ratings proved as informative in assigning the categorical olfactory diagnosis. Using an olfactory diagnostic instrument, which consists of a mapping rule of two numerical rating scales of one’s olfactory function to the olfactory functional diagnosis based on the “Sniffin’ Sticks” clinical test battery, the diagnoses of anosmia, hyposmia, or normosmia could be derived from the self-ratings at a satisfactorily balanced accuracy of about 80%. It remains to be seen whether this approach of translating self-assessments into olfactory diagnoses of anosmia, hyposmia, and normosmia can be generalized to other clinical cohorts in which olfaction plays a role.
Objective: Current literature debates the role of newly developed three-dimensional (3D) Exoscopes in the daily routine of neurosurgical practice. So far, only a small number of cadaver lab studies or case reports have examined the novel Aesculap Aeos Three-Dimensional Robotic Digital Microscope. This study aims to evaluate the grade of satisfaction and intraoperative handling of this novel system in neurosurgery. Methods: Nineteen neurosurgical procedures (12 cranial, 6 spinal and 1 peripheral nerve) performed over 9 weeks using the Aeos were analyzed. Ten neurosurgeons of varying levels of training were included after undergoing device instruction and training. Following every surgery, a questionnaire consisting of 43 items concerning intraoperative handling was completed. The questionnaires were analyzed using descriptive statistics. Results: No intraoperative complications occurred. Surgical satisfaction was ranked high (78.95%). In total, 84.21% evaluated surgical ergonomics as satisfactory, while 78.95% of the surgeons would like to use this system frequently. Image quality, independent working zoom function and depth of field were perceived as suboptimal by several neurosurgeons. Conclusion: The use of Aeos is feasible and safe in microsurgical procedures, and surgical satisfaction was ranked high among most neurosurgeons in our study. The system might offer advanced ergonomic conditions in comparison to conventional ocular-based microscopes.
Advanced stage metastatic prostate cancer with extensive bone marrow involvement is associated with a high risk of therapy-induced myelotoxicity and unfavorable outcomes. The role of salvage radioligand therapy (RLT) with 177Lu-PSMA-617 in this subset of patients remains to be further elucidated. Forty-five patients with progressive metastatic castration-resistant prostate cancer (mCRPC) and diffuse bone marrow involvement were treated with repeated cycles of RLT after having exhausted standard treatment options. A mean treatment activity of 7.4 ± 1.4 GBq 177Lu-PSMA-617 was administered in a median of four treatment cycles (IQR 2-6) and the mean cumulative activity was 32.6 ± 20.1 GBq. After two RLT cycles, ≥50% PSA decline was observed in 25/45 (56%) patients and imaging-based partial remission (PR) was observed in 18/45 (40%) patients. Median imaging-based progression-free survival (PFS) was 6.4 mo (95% CI, 3.0–9.8) and the median overall survival (OS) was 10.2 months (95% CI, 7.2–12.8). The biochemical response translated into a significantly prolonged PFS (12.9 vs. 2.8 mo, p < 0.001) and OS (13.5 vs. 6.7 mo, p < 0.001). Patients with PR on interim imaging after two cycles had a longer median OS compared to patients with stable or progressive disease (15.5 vs. 7.1 mo, p < 0.001). Previous taxane-based chemotherapy (HR 3.21, 95%CI 1.18–8.70, p = 0.02) and baseline LDH levels (HR 1.001, 95%CI 1.000–1.001, p = 0.04) were inversely associated with OS on a Cox-regression analysis. Grade ≥ 3 hematological decline was observed after 22/201 (11%) cycles with anemia, leukopenia and thrombocytopenia in 15/45 (33%), 6/45 (13%) and 8/45 (18%) patients, respectively. Cumulative treatment activity and absorbed whole-body dose were not correlated with new onset grade ≥ 3 hematotoxicity (p = 0.91, p = 0.69). No event of grade ≥ 3 chronic kidney disease was observed during RLT or the follow-up. Last line RLT with 177Lu-PSMA-617 in mCRPC patients with diffuse bone marrow involvement may thus contribute to prolonged disease control at an acceptable safety profile.
Aim of the study: This RCT assesses patients’ 18-month clinical outcomes after the regenerative therapy of periimplantitis lesions using either an electrolytic method (EC) to remove biofilms or a combination of powder spray and an electrolytic method (PEC). Materials and Methods: Twenty-four patients (24 implants) suffering from periimplantitis were randomly treated by EC or PEC followed by augmentation and submerged healing. Probing pocket depth (PPD), Bleeding on Probing (BoP), suppuration, and standardized radiographs were assessed before surgery (T0), 6 months after augmentation (T1), and 6 (T2) and 12 (T3) months after the replacement of the restoration. Results: The mean PPD changed from 5.8 ± 1.6 mm (T0) to 3.1 ± 1.4 mm (T3). While BoP and suppuration at T0 were 100%, BoP decreased at T2 to 36.8% and at T3 to 35.3%. Suppuration was found to be at a level of 10.6% at T2 and 11.8% at T3. The radiologic bone level measured from the implant shoulder to the first visible bone to the implant contact was 4.9 ± 1.9 mm at mesial sites and 4.4 ± 2.2 mm at distal sites at T0 and 1.7 ± 1.7 mm and 1.5 ± 17 mm at T3. Conclusions: Significant radiographic bone fill and the improvement of clinical parameters were demonstrated 18 months after therapy.
Throughout life, macrophages are located in every tissue of the body, where their main roles are to phagocytose cellular debris and recycle aging red blood cells. In the tissue niche, they promote homeostasis through trophic, regulatory, and repair functions by responding to internal and external stimuli. This in turn polarizes macrophages into a broad spectrum of functional activation states, also reflected in their iron-regulated gene profile. The fast adaptation to the environment in which they are located helps to maintain tissue homeostasis under physiological conditions.
Osteoarthritis: novel molecular mechanisms increase our understanding of the disease pathology

Although osteoarthritis (OA) is the most common musculoskeletal condition that causes significant health and social problems worldwide, its exact etiology is still unclear. With an aging and increasingly obese population, OA is becoming even more prevalent than in previous decades. Up to 35% of the world’s population over 60 years of age suffers from symptomatic (painful, disabling) OA. The disease poses a tremendous economic burden on the health-care system and society for diagnosis, treatment, sick leave, rehabilitation, and early retirement. Most patients also experience sleep disturbances, reduced capability for exercising, lifting, and walking and are less capable of working, and maintaining an independent lifestyle. For patients, the major problem is disability, resulting from joint tissue destruction and pain. So far, there is no therapy available that effectively arrests structural deterioration of cartilage and bone or is able to successfully reverse any of the existing structural defects. Here, we elucidate novel concepts and hypotheses regarding disease progression and pathology, which are relevant for understanding underlying the molecular mechanisms as a prerequisite for future therapeutic approaches. Emphasis is placed on topographical modeling of the disease, the role of proteases and cytokines in OA, and the impact of the peripheral nervous system and its neuropeptides.
Background and Purpose: Sexual dysfunction (SD) is a frequent side effect associated with radical prostatectomy (RP) for prostate cancer (PCa). Some studies have showed the benefit associated with preoperative sexual rehabilitation (prehabilitation) and Enhanced Recovery After Surgery (ERAS) for RP, but no clear clinical recommendations are available yet. Our aim was to conduct a systematic review on sexual prehabilitation prior to RP for patients with a localized PCa and analyze the impact on postoperative sexual health compared with the standard post-operative care.
Methods: We performed a systematic review of the literature following the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) recommendations.
Results: Four randomized control trials and one retrospective comparative study were included in the analyses. Three of the five studies showed an improved EF recovery post-RP in the prehabilitation group compared to the standard of care represented by: higher International Index of Erectile Function 5 score (IIEF5) or IIEF score (p < 0.0001) and a higher percentage of patients reporting return of EF based on the Sexual Encounter Profile (SEP) (56 vs. 24%, p = 0.007). Self-confidence, therapeutic alliance, and adherence to treatment were stronger for patients with preoperative consultations (p < 0.05) and EF recovery was better in cases of a higher number of follow-up visits (OR 4–5 visits vs. 1:12.19, p = 0.002).
Discussion: Despite heterogenous methods and high risks of bias in this systematic review, starting sexual rehabilitation prior to surgery seems to ensure better EF recovery. This prehabilitation should include information of both the patient and his or her partner, with a closer follow up and the use of a multimodal treatment approach that still remains to be defined and validated (oral medication, vacuum devices, pelvic floor muscle training, etc.).
Aims: The study aimed to evaluate the prevalence of mental distress in patients with newly diagnosed bladder cancer, the cancer-information search behavior, and the influence of information seeking on distress. Methods: One hundred and one bladder cancer patients answered 2 established questionnaires (“Hospital Anxiety and Depression Scale” [HADS] and the “Fragebogen zur Belastung von Krebskranken” [FBK-R23]) for evaluation of mental distress and a self-developed questionnaire with questions concerning information seeking and socioeconomic facts. Results: Regarding risk group stratification, 57.4% were classified as high-risk and 42.6% as low-risk tumor-bearing patients. Analysis of mental distress showed that 23.2% had a score above the HADS-A cutoff, 25.3% above the HADS-D cutoff, and 21.4% showed a pathologic FBK-R23 score. Overall, 75% felt well informed about their illness. Risk group stratification did not correlate with HADS-A, HADS-D, or FBK-R23 score. Furthermore, active search for information or the use of the Internet did not correlate with the HADS-A, HADS-D, or FBK-R23 score. However, the quality of the urologist’s information and the feeling of being informed correlated with the grade of mental distress. Conclusion: Besides the treatment of bladder cancer, informing the patient about the disease in a psychologically wholesome manner and working together with psycho-oncologically trained psychologists are essential tasks for the treating urologist.
Introduction: Penile carcinomas are rare tumors throughout Europe. Therefore, little attention is drawn to this disease. That makes it important to study tumor-associated key metrics and relate these to known data on penile neoplasias. Materials and methods: A cohort of 60 well-defined penile invasive carcinomas with known human papillomavirus (HPV) infection status was investigated. Data on tumor type, grading and staging were recorded. Additionally, data on the peri- and intratumoral immune cell infiltrate in a semiquanititave manner applying an HE stain were assessed. Results: Our study showed a significant correlation of immune cell infiltrate and pT stage with overall survival. Therefore, in a subset of tumors, PD-L1 staining was applied. For tumor proportion score (TPS), 26 of 30 samples (87%) were scored >0%. For the immune cell score (IC), 28 of 30 samples (93%) were defined as >0% and for CPS, 29 of 30 samples (97%) scored >0. PD-L1 expression was not associated with overall survival. Conclusion: PD-L1 is expressed in penile carcinomas, providing a rationale for targeted therapy with checkpoint inhibitors. We were able to show that immune reaction appears to be prognostically relevant. These data enhance the need for further studies on the immune cell infiltrate in penile neoplasias and show that PD-L1 expression is existent in our cohort, which may be a potential target for checkpoint inhibitor therapy.
Einfluss eines Erste-Hilfe-Kurses für Medizinstudierende auf notfallmedizinische Basiskompetenzen

Medizinstudierende sind bereits in der Vorklinik häufig mit Situationen konfrontiert, in denen medizinische Notfälle passieren, die unter anderem wegen der Lage des Universitätscampus und den damit verbundenen Kontakt zu Patientinnen und Patienten oder in den Pflegepraktika. Tritt in diesen Situationen ein medizinischer Notfall ein, müssen sie Erste Hilfe leisten können. Das Ziel der vorliegenden Arbeit ist daher die Erfassung notfallmedizinischer Basiskompetenzen von Medizinstudierenden der Vorklinik und die Frage, ob diese ausreichend sind, um den Anforderungen ihres Umfeldes gerecht zu werden. Des Weiteren wurde ein Erste-Hilfe-Kurs speziell für Vorklinikstudierende basierend auf dem ABCDE-Schema konzipiert, implementiert und evaluiert, ob die erfassten Kompetenzdefizite behoben werden konnten.
In einem Pre-Post-Design wurde die theoretische Kompetenz anhand eines 10 Items Multiple-Choice-Tests und die praktischen Kompetenzen anhand von 3 OSCE-/OSPE-Stationen (je 5 min) erfasst. Sowohl der MC-Test als auch die OSCE-/OSPE-Stationen wurden in Probeläufen getestet und überarbeitet. Soziodemographische Daten wurden mittels Fragebogen erhoben. Der entwickelte Kurs umfasst 9 UE (davon 6 UE praktische Übungen) in denen die zu erlernenden Erste-Hilfe-Maßnahmen strukturiert und in logischer Reihenfolge anhand des etablierten ABCDE-Schemas vermittelt werden. Jeweils 8 Teilnehmende werden von zwei geschulten Peer-Dozierenden unterrichtet. Im Anschluss an das Training erfolgte eine erneute Erhebung der theoretischen und praktischen Kompetenzen. Die Eingabe der Rohdaten erfolgte in Microsoft Excel (Microsoft Inc., Redmond, WA, USA). Die weitere Auswertung erfolgte mittels SPSS (SPSS Inc., Chicago, IL, USA).
160 Studierende aus den ersten beiden Studienjahren konnten in die Studie eingeschlossen werden. Im Basis-Assessment demonstrierten nur 25% der Studierenden zwei Minuten lang eine suffiziente Kompression mit richtiger Frequenz und Drucktiefe, nur 29% überstreckten am Ende der Seitenlage den Kopf des Patienten und öffneten den Mund. Im Anschluss an das Training erzielten die Studierenden an allen OSCE/OSPE-Stationen signifikant bessere Ergebnisse: Bei der stabilen Seitenlage steigerten sie sich durchschnittlich um 5,3 Punkte (max. 14), bei der Reanimation um 6,8 Punkte (max. 18). Während sich vor dem Kurs 33% der Studierenden in der Lage fühlten, Wiederbelebungsmaßnahmen durchzuführen, waren dies nach dem Kurs 98%. Die Kursevaluation war durchweg positiv.
Die Erfassung notfallmedizinischer Basiskompetenzen von Medizinstudierenden der Vorklinik zeigte, dass diese nicht ausreichend geschult sind, um Notfallsituationen adäquat zu bewältigen. Vor diesem Hintergrund muss eine dem Anforderungsprofil entsprechende Ausbildung in Erster Hilfe bereits in der Vorklinik an der Universität gewährleistet werden. Die vorliegenden Ergebnisse zeigen, dass mithilfe eines speziell auf Medizinstudierende ausgerichteten Erste-Hilfe-Kurs eine deutliche Steigerung notfallmedizinischer Basiskompetenzen erfolgen kann.
Trehalose, a sugar from fungi, mimics starvation due to a block of glucose transport and induces Transcription Factor EB- mediated autophagy, likely supported by the upregulation of progranulin. The pro-autophagy effects help to remove pathological proteins and thereby prevent neurodegenerative diseases such as Alzheimer’s disease. Enhancing autophagy also contributes to the resolution of neuropathic pain in mice. Therefore, we here assessed the effects of continuous trehalose administration via drinking water using the mouse Spared Nerve Injury model of neuropathic pain. Trehalose had no effect on drinking, feeding, voluntary wheel running, motor coordination, locomotion, and open field, elevated plus maze, and Barnes Maze behavior, showing that it was well tolerated. However, trehalose reduced nerve injury-evoked nociceptive mechanical and thermal hypersensitivity as compared to vehicle. Trehalose had no effect on calcium currents in primary somatosensory neurons, pointing to central mechanisms of the antinociceptive effects. In IntelliCages, trehalose-treated mice showed reduced activity, in particular, a low frequency of nosepokes, which was associated with a reduced proportion of correct trials and flat learning curves in place preference learning tasks. Mice failed to switch corner preferences and stuck to spontaneously preferred corners. The behavior in IntelliCages is suggestive of sedative effects as a 'side effect' of a continuous protracted trehalose treatment, leading to impairment of learning flexibility. Hence, trehalose diet supplements might reduce chronic pain but likely at the expense of alertness.
Synovial adipose stem cells (sASC) can be differentiated into catecholamine-expressing sympathetic neuron-like cells to treat experimental arthritis. However, the pro-inflammatory tumor necrosis factor (TNF) is known to be toxic to catecholaminergic cells (see Parkinson disease), and this may prevent anti-inflammatory effects in inflamed tissue. We hypothesized that TNF exhibits inhibitory effects on human differentiated sympathetic tyrosine hydroxylase-positive (TH+) neuron-like cells. For the first time, iTH+ neuron-like sympathetic cells were generated from sACSs of rheumatoid arthritis (RA) and osteoarthritis (OA) synovial tissue. Compared to untreated controls in both OA and RA, TNF-treated iTH+ cells demonstrated a weaker staining of catecholaminergic markers in cell cultures of RA/OA patients, and the amount of produced noradrenaline was markedly lower. These effects were reversed by etanercept. Exposure of iTH+ cells to synovial fluid of RA patients showed similar inhibitory effects. In mixed synovial cells, significant effects of TNF on catecholamine release were observed only in OA. This study shows that TNF inhibits iTH+ synovial cells leading to the decrease of secreted noradrenaline. This might be a reason why discovered newly appearing TH+ cells in the synovium are not able to develop their possible full anti-inflammatory role in arthritis.
Einleitung: OTSC Proctology ist ein Verfahren in der Analfistelchirurgie dessen Erfolgsaussichten auch 9 Jahre nach der ersten klinischen Anwendung nicht abschließend beurteilt werden können. Die wenigen bisher publizierten Studien zeigen sehr divergente Ergebnisse mit Heilungsraten von 10 bis 90%.
Material und Methoden: Wir führten eine retrospektive Auswertung der Behandlungsergebnisse aller konsekutiven Patienten, die in dem Zeitraum vom 01.03.2014 bis 31.03.2017 in der koloproktologischen Abteilung der DKD Helios Klinik Wiesbaden mittels OTSC-Verfahren wegen Analfisteln operiert wurden, durch. Erfasst wurden Alter, Geschlecht, OP- und Aufenthaltsdauer, Operateur, Fistelart und –lokalisation, Vorhandensein von Stoma und CED, plastischer Fistelverschluss (PFV) in der Anamnese, Clipverbleib nach der OP, Dauer des Follow-ups, Komplikationen sowie postoperative Schmerzsituation. Die Datenauswertung erfolgte mittels deskriptiver Statistik bei Subgruppenanalyse unter Verwendung der Statistiksoftware SPSS 20.
Ergebnisse: Es wurden insgesamt 68 Fälle eingeschlossen, davon 37% weiblich und 63% männlich. Das durchschnittliche Alter betrug 52 Jahre (25 – 81). 19 (28%) Patienten litten an CED, 11 (16%) Patienten hatten ein Stoma. 34 (50%) der Patienten hatten plastischen Fistelverschluss in der Anamnese. Die Verteilung nach Fisteltyp war wie folgt: 58 (85%) transsphinktär, 4 (6%) suprasphinktär, 3 (4%) intersphinktär, 1 (1,5%) rektovaginal, 1 (1,5%) rektourethral und 1 (1,5%) Pouchfistel. Die häufigsten Fistelokalisationen waren bei 6 h (N = 26, 38%), 12 h (N = 14, 21%), 7 h (N = 7, 10%) und 3h (N = 5, 7%) SSL. Die durchschnittlichen OP-Dauer und stationärer Aufenthalt betrugen 25 min (6 – 90) und 7 Tage (1 – 14 Tage) entsprechend. Die durchschnittliche Dauer des Follow-ups betrug 29 Monate (10 – 36).
Die Fistelheilung im Gesamtkollektiv lag bei 48,5%, 1 (1,5%) Patient war lost-tofollow-up. In der weiblichen (N = 25) und männlichen (N = 43) Kohorte fand die Heilung in 40% und 53% der Fälle entsprechend statt. Die Heilungsraten bei intersphinktären (N = 3), transsphinktären (N = 58) und suprasphinktären (N = 4) Fisteln lagen bei 100%, 46,5% und 50% entsprechend, eine rektovaginale und eine rektourethrale Fistel sind nicht geheilt. Eine Pouchfistel ist geheilt.
Die Heilungsraten bei 6h und 12h SSL lagen bei 58% und 14% entsprechend mit deutlichem Vorteil bei posteriorer Fistellage. Dieser Vorteil blieb nach der Aufteilung der Fisteln in anteriore (N = 30) und posteriore (N = 38), mit
Heilungsraten von 33% und 60,5% entsprechend, bestehen. In den Subgruppen ohne CED (N = 49) und mit CED (N = 19) lagen die Heilungsraten bei 53% und 35% entsprechend. In den Subgruppen ohne PFV (N = 34) und mit PFV (N = 34) lagen die Heilungsraten bei 59% und 38% entsprechend. In der Subgruppe ohne Stoma (N = 57) wurde eine Heilung in 47% der Fälle, in der Subgruppe mit Stoma (N = 11) in 55% beobachtet. In der Subgruppe mit den kryploglandulären Fisteln (N = 47) war die Heilung in 55,3% zu sehen und in der Subgruppe mit
kryptoglandulären Fisteln ohne PFV (N = 27) bei 63%.
In 48 (70,6%) Fällen wurde der Clip aktiv entfernt, in 11 (16,2%) Fällen kam es zum Spontanverlust und in 8 (11,8%) Fällen blieb der Clip in situ. Die durchschnittliche Zeit bis zur Klammerentfernung betrug 4 Monate. Die Heilungsraten bei Clipentfernung, Clipverbleib und Clipspontanabgang lagen bei 42%, 100% und 45% entsprechend.
Der maximale Schmerz nach NRS 0 – 2 bei 61% der Patienten, NRS 3 – 4 bei 28% und NRS 5 – 7 nur bei 11%. Bei 50% der Fälle war kein Opiat erforderlich und bei 39% der Fälle erfolgte die Opiateinnahme nicht länger als 2 Tage.
Die Komplikationen waren sehr selten: eine Nachblutung mit Clipdislokation (1,5%), ein Analabszess (1,5%), 2 Fälle (3%) der neuaufgetretenen Stuhlschmieren und 1 (1,5%) Wundheilungsstörung intraanal, die spontan abheilte. In 4 (6%) Fällen kam es zur Klammerdislokation vom inneren
Fistelostium mit konsekutiver Fistelpersistenz.
Fazit: OTSC ist ein komplikationsarmes und schmerzarmes Verfahren mit kurzer OP-Zeit und könnte einen festen Platz in der Analfistelchirurgie einnehmen. Die bestmöglichen Ergebnisse lassen sich bei dorsal gelegenen Analfisteln in
nichtvoroperierten Patienten ohne CED erzielen.
In humans, alterations of circadian rhythms and autophagy are linked to metabolic, cardiovascular and neurological dysfunction. Autophagy constitutes a specific form of cell recycling in many eukaryotic cells. Aging is the principal risk factor for the development of neurodegenerative diseases. Thus, we assume that both the circadian clock and autophagy are indispensable to counteract aging. We have previously shown that the hippocampus of Per1−/−-mice exhibits a reduced autophagy and higher neuronal susceptibility to ischemic insults compared to wild type (WT). Therefore, we chose to study the link between aging and loss of clock gene Per1−/−-mice. Young and aged C3H- and Per1−/−-mice were used as models to analyze the hippocampal distribution of Aβ42, lipofuscin, presenilin, microglia, synaptophysin and doublecortin. We detected several changes in the hippocampus of aged Per1−/−-mice compared to their wild type littermates. Our results show significant alterations of microglia morphology, an increase in Aβ42 deposition, overexpression of presenilin, decrease in synaptophysin levels and massive accumulation of lipofuscin in the hippocampus of 24-month-old Per1−/−-mice, without alteration of adult neurogenesis. We suggest that the marked lipofuscin accumulation, Aβ42 deposition, and overexpression of presenilin-2 observed in our experiments may be some of the consequences of the slowed autophagy in the hippocampus of aged Per1−/−-mice. This may lead during aging to excessive accumulation of misfolded proteins which may, consequently, result in higher neuronal vulnerability.
The group of proton-sensing G-protein coupled receptors (GPCRs) consists of the four receptors GPR4, TDAG8 (GPR65), OGR1 (GPR68), and G2A (GPR132). These receptors are cellular sensors of acidification, a property that has been attributed to the presence of crucial histidine residues. However, the pH detection varies considerably among the group of proton-sensing GPCRs and ranges from pH of 5.5 to 7.8. While the proton-sensing GPCRs were initially considered to detect acidic cellular environments in the context of inflammation, recent observations have expanded our knowledge about their physiological and pathophysiological functions and many additional individual and unique features have been discovered that suggest a more differentiated role of these receptors in health and disease. It is known that all four receptors contribute to different aspects of tumor biology, cardiovascular physiology, and asthma. However, apart from their overlapping functions, they seem to have individual properties, and recent publications identify potential roles of individual GPCRs in mechanosensation, intestinal inflammation, oncoimmunological interactions, hematopoiesis, as well as inflammatory and neuropathic pain. Here, we put together the knowledge about the biological functions and structural features of the four proton-sensing GPCRs and discuss the biological role of each of the four receptors individually. We explore all currently known pharmacological modulators of the four receptors and highlight potential use. Finally, we point out knowledge gaps in the biological and pharmacological context of proton-sensing GPCRs that should be addressed by future studies.
Exogenous adenosine and its metabolite inosine exert anti-inflammatory effects in synoviocytes of osteoarthritis (OA) and rheumatoid arthritis (RA) patients. We analyzed whether these cells are able to synthesize adenosine/inosine and which adenosine receptors (ARs) contribute to anti-inflammatory effects. The functionality of synthesizing enzymes and ARs was tested using agonists/antagonists. Both OA and RA cells expressed CD39 (converts ATP to AMP), CD73 (converts AMP to adenosine), ADA (converts adenosine to inosine), ENT1/2 (adenosine transporters), all AR subtypes (A1, A2A, A2B and A3) and synthesized predominantly adenosine. The CD73 inhibitor AMPCP significantly increased IL-6 and decreased IL-10 in both cell types, while TNF only increased in RA cells. The ADA inhibitor DAA significantly reduced IL-6 and induced IL-10 in both OA and RA cells. The A2AAR agonist CGS 21680 significantly inhibited IL-6 and induced TNF and IL-10 only in RA, while the A2BAR agonist BAY 60-6583 had the same effect in both OA and RA. Taken together, OA and RA synoviocytes express the complete enzymatic machinery to synthesize adenosine/inosine; however, mainly adenosine is responsible for the anti- (IL-6 and IL-10) or pro-inflammatory (TNF) effects mediated by A2A- and A2BAR. Stimulating CD39/CD73 with simultaneous ADA blockage in addition to TNF inhibition might represent a promising therapeutic strategy.
The purpose of the study was to investigate the bacterial viability of the initial biofilm on the surface of experimental modified dental resin composites. Twenty-five healthy individuals with good oral hygiene were included in this study. In a split-mouth design, they received acrylic splints with five experimental composite resin specimens. Four of them were modified with either a novel polymeric hollow-bead delivery system or methacrylated polymerizable Irgasan (Antibacterial B), while one specimen served as an unmodified control (ST). A delivery system based on Poly-Pore® was loaded with one of the active agents: Tego® Protect 5000 (Antiadhesive A), Dimethicone (Antiadhesive B), or Irgasan (Antibacterial A). All study subjects refrained from toothbrushing during the study period. Specimens were detached from the splints after 8 h and given a live/dead staining before fluorescence microscopy. A Friedman test and a post hoc Nemenyi test were applied with a significance level at p < 0.05. In summary, all materials but Antibacterial B showed a significant antibacterial effect compared to ST. The results suggested the role of the materials’ chemistry in the dominance of cell adhesion. In conclusion, dental resin composites with Poly-Pore-loaded active agents showed antibacterial effectiveness in situ.
Für Patienten mit lebensbedrohlichen Herzkrankheiten ist die Extracorporeal-Life-Support-Behandlung (ECLS) eine sinnvolle Therapiemöglichkeit. Sie bietet für Patienten im kardiogenen Schock ein Zeitfenster, um eine myokardiale Erholung zu erreichen. Hierbei kann in Abhängigkeit vom Krankheitsbild die zusätzliche Anwendung von IABP die Heilungschancen begünstigen. In der durchgeführten retrospektiven Studie wurden 118 Patienten betrachtet, die in der Klinik für Thorax-, Herz- und thorakale Gefäßchirurgie der Universität Frankfurt am Main im Zeitraum von Dezember 2001 bis Ende 2013 eine ECLS-Therapie erhalten haben. Bei 59 Patienten wurde die ECLS-Unterstützung in Kombination mit IABP durchgeführt. Die beiden Patientenkollektive - mit und
ohne IABP- sind hinsichtlich ihrer Risikofaktoren vergleichbar. Ausgehend von der Zielsetzung dieser Arbeit wurde analysiert, ob der gleichzeitige Einsatz der IABP bei ECLS – Therapie von Vor- oder sogar von Nachteil ist. Hierfür wurden für die beiden Therapiegruppen Überlebenszeitanalysen nach Kaplan-Meier durchgeführt. Der statistische Vergleich der Überlebensraten und des Weaningerfolgs erfolgte mit Hilfe des Log-Rank-Tests.
Die Auswertung der erhobenen Daten hat ergeben, dass kein signifikanter Unterschied bei der 30-Tages-Überlebensrate und dem Weaningerfolg für die beiden Patientenkollektive mit und ohne zusätzliche IABP-Anwendung vorhanden ist. Risikofaktoren wie ein hohes Lebensalter oder eine bereits vor Einlieferung stattgefundene Intubation verringern außerdem die Überlebenschancen nach ECLS - Therapie. Ein fortgeschrittenes NYHA-Stadium konnte nicht als negativer prädiktiver Faktor identifiziert werden.
In der Literatur kommt man bezüglich der Mortalität unter alleiniger ECLS-Therapie oder dem zusätzlichen Einsatz einer IABP zu unterschiedlichen Ergebnissen. Diese besagen teilweise, dass ECLS und IABP einander ergänzende Methoden sind, die sich durchaus synergistisch auf den Behandlungserfolg auswirken können und dass die Mortalität beim zusätzlichen Einsatz einer IABP signifikant niedriger ist. Die Durchführung weiterer prospektiver Studien mit vergleichbaren Patientenkollektiven zur Untersuchung des Outcomes bei den unterschiedlichen Behandlungsmethoden ist jedoch erforderlich, um ein aussagekräftiges Fazit ziehen zu können.
Cancer is the major cause of death besides cardiovascular disease. Leukaemia represents the most prevalent malignancy in children with a frequency of 30 % and is one of the ten leading types of cancer in adults. Philadelphia Chromosome-positive B-ALL (Ph+ B-ALL) is driven by the cytogenetic aberration of the reciprocal chromosomal translocation t(9;22)(q34;q11) leading to the formation of the Philadelphia chromosome with a BCR-ABL1 fusion gene. This fusion gene encodes a BCR-ABL1 oncoprotein which is characterized by a constitutively enhanced tyrosine kinase activity promoting amplified proliferation, differentiation arrest and resistance to cell death. Ph+ B-ALL is considered the most aggressive ALL subtype with a long-term survival rate in the range of only 30 % despite intensive standard of care including chemotherapy in combination with a tyrosine kinase inhibitor (TKI) followed by allogeneic stem cell transplantation after remission for clinically fit patients.
The efficacy of chemotherapy has long been mainly attributed to tumour cell toxicity while immune modulating effects have been overlooked, especially in light of known immunosuppressive properties. Accumulative evidence, however, emphasizes the ability of chemotherapeutic agents, including TKIs, to normalise or re-educate a dysfunctional tumour microenvironment (TME) resulting in enhanced anti-tumour immunity. One of the underlying mechanisms of immune modulation is the induction of immunogenic cell death (ICD). ICD is an anti-tumour agent-induced cell death modality determined by the capacity to convert cancer cells into anti-cancer vaccines. The induction of ICD relies on the release of damage-associated molecular patterns (DAMPs) from dying tumour cells succumbing to ICD. Translocation of CALR to the cell surface, extracellular secretion of ATP and release of HMGB1 from the nucleus are key hallmarks of ICD that mediate anti-tumour immunity upon binding to antigen presenting cells resulting in a tumour antigen-specific immune response. Besides these molecular determinants, ICD is functionally defined by the inhibition of tumour growth in a vaccination assay in which mice are injected with tumour cells exposed to the potential ICD inducer in-vitro and then re-challenged with live tumour cells of the same cancer type. Both molecular and functional criteria determine the gold standard approach to assess ICD. By increasing the immunogenicity of cancer cells, ICD contributes to the restoration of immunosurveillance as an essential feature of tumour rejection, which is clinically reflected by improved therapeutic efficacy and disease outcome in patients. Therefore, identifying novel ICD inducers is an objective of interest in the context of cancer therapy.
In respect of these considerations, the aim addressed in the present work is the examination of the second-generation TKI Nilotinib for the ability to induce ICD. The thesis is set in the context of the group's research on the role of Gas6/TAM signalling within the TME regarding the pathogenesis of acute leukaemia. In in-vivo experiments of our research group it has been consistently observed that the use of Nilotinib enhances the anti-leukaemic immunity mediated by a deletion of Gas6. Against the background of increasing importance of chemotherapeutic agents as potent modulators of a dysregulated TME, it was hypothesized that Nilotinib may synergize with a Gas6-deficient environment by inducing ICD in Ph+ B-ALL cells.
In growth inhibition and Annexin V/Propidium iodide cell death assays Nilotinib was shown to induce cell death in concentration-dependent manner that occurs bimodally in terms of cell death modality ranging between apoptosis and necrosis. By ICD marker analysis, comprising flow-cytometric detection of CALR exposure, chemoluminescence-based ATP measurement and immunoblotting for HMGB1, it was found that Nilotinib-induced cell death is not accompanied by CALR exposure and ATP secretion, but is associated with the release of HMGB1. In macrophages co-culture experiments with Nilotinib-treated leukaemic cells, no relevant shift in terms of macrophages activation and polarisation was observed in either a juxtacrine or paracrine setup. In consistency with the results obtained in the in-vitro experiments, Nilotinib was not potent to elicit a protective immune response in mice within a vaccination assay.
Conclusively, Nilotinib was identified to not qualify as bona fide ICD inducer. The role of Nilotinib-induced cell death and HMGB1 release are proposed as objective for further investigation concerning the synergistic interplay between Nilotinib and a Gas6-deficient environment. Efforts addressing exploration and optimisation of the immunological potential of chemotherapeutic agents are a promising approach aimed at providing cancer patients with the best possible treatment in future.
Purpose This study aims to define the extent of additional surgical procedures after abdominal wall closure (AWC) in patients with gastroschisis (GS) and omphalocele (OC) with special focus on gastrointestinal related operations. Methods A retrospective chart review was performed including all operations in GS and OC patients in the first year after AWC (2010–2019). The risk for surgery was calculated using the one-year cumulative incidence (CI). Results 33 GS patients (18 simple GS, 15 complex) and 24 OC patients (12 without (= OCL), 12 OC patients with liver pro-trusion (= OCL +)) were eligible for analysis. 43 secondary operations (23 in GS, 20 in OC patients) occurred after a median time of 84 days (16–824) in GS and 114.5 days (12–4368) in OC. Patients with complex versus simple GS had a significantly higher risk of undergoing a secondary operation (one-year CI 64.3% vs. 24.4%; p = 0.05). 86.5% of surgical procedures in complex GS and 36.3% in OCL + were related to gastrointestinal complications. Complex GS had a significantly higher risk for GI-related surgery than simple GS. Bowel obstruction was a risk factor for surgery in complex GS (one-year CI 35.7%). Conclusion Complex GS and OCL + patients had the highest risk of undergoing secondary operations, especially those with gastrointestinal complications
Im Rahmen der Serie „Biomarker“, die im Zentralblatt für Arbeitsmedizin und Ergonomie publiziert wird, ist das Angiotensin-Converting Enzyme (ACE) zugehörig als häufiger Marker in der Diagnostik von pulmonalen und extrapulmonalen Erkrankungen. Die Bestimmung von ACE stellt einen wesentlichen Bestandteil der Diagnostik von pulmonalen und extrapulmonalen Erkrankungen dar. Der Einfluss von Tabakkonsum, Medikamenten, Zink-Chelatoren auf die ACE-Konzentration wird eruiert. ACE erwies sich als Marker mit einer hohen Sensitivität und Spezifität bei Lungenerkrankungen.
Hintergrund und Ziel der Studie: Steigende Erwartungen der Kataraktpatienten führen zu stetigen Weiterentwicklungen in der Linsenchirurgie. Der Wunsch nach einem perfekten Sehvermögen und nach Brillenfreiheit ist ein häufiges Anliegen insbesondere jüngerer Patienten. Die Einführung von Multifokallinsen hat die Therapie der Katarakt revolutioniert. Mit ihnen kann der Patient sowohl in der Nähe und Ferne als auch im Intermediärbereich scharf sehen. Für die präoperative Berechnung der Linsenstärke sind verschiedene Formeln verfügbar. Bis jetzt wurde keine Studie publiziert, die die Präzision dieser Formeln für die Kalkulation der Stärke von Tri- oder Quadrifokallinsen untersucht hat. Das Ziel dieser Studie war die Evaluation neun moderner Formeln für die Berechnung der Linsenstärke der quadrifokalen AcrySof IQ PanOptix TFNT00.
Methoden:
Die vorliegende Studie ist eine retrospektive konsekutive Fallserie, die an der Augenklinik der Johann Wolfgang Goethe-Universität Frankfurt am Main durchgeführt wurde. Es wurden alle Patienten eingeschlossen, die sich einer Femtosekundenlaser-assistierten Operation mit Implantation der quadrifokalen Intraokularlinse unterzogen. Mit dem IOLMaster 500 wurden die präoperativen Biometrie- und Keratometriemessungen durchgeführt. Das Scheimpflug-System Pentacam wurde zur Bestimmung der zentralen Hornhautdicke genutzt. Drei Monate nach der Operation fand eine subjektive Refraktionsbestimmung statt.
Die folgenden Formeln wurden untersucht: Holladay 1, Sanders-Retzlaff-Kraff/ theoretical (SRK/T), Hoffer Q, T2, Holladay 2, Haigis, Barrett Universal II, Olsen und Hill-radiale Basisfunktion (RBF). Aus den präoperativ gemessenen Parametern und der implantierten Linsenstärke wurde für jedes Auge die postoperative Refraktion berechnet. Der Vorhersagefehler (PE) ist definiert als die Differenz zwischen dem tatsächlich erreichten postoperativen sphärischen Äquivalent und dem vorhergesagten postoperativen sphärischen Äquivalent. Zur Reduktion systematischer Fehler und somit des mittleren PE wurden die Linsenkonstanten optimiert. Primäre Endpunkte waren die Unterschiede der mittleren absoluten Vorhersagefehler (MAE) zwischen den Formeln. Die Standardabweichung sowie mediane und maximale absolute PE wurden ebenso untersucht wie die Prozent-zahlen der Augen, deren PE innerhalb von ± 0,25 Dioptrien (dpt), ± 0,5 dpt, ± 1,0 dpt und ± 2,0 dpt lagen.
Ergebnisse: 75 Augen von 38 Patienten wurden in die vorliegende Studie eingeschlossen. Es gab signifikante Unterschiede zwischen den Formeln bezüglich des MAE (p Wert < 0,001). Die Rangfolge der Formeln entsprechend ihrer MAE lautet wie folgt: Barrett Universal II (0,294 dpt), Hill-RBF (0,332 dpt), Olsen (0,339 dpt), T2 (0,351 dpt), Holladay 1 (0,381 dpt), Haigis (0,382 dpt), SRK/T (0,393 dpt), Holladay 2 (0,399 dpt) und Hoffer Q (0,410 dpt). Den niedrigsten maximalen absoluten PE erreichte die Barrett Universal II. Bei der Verwendung der Haigis, Barrett Universal II, Olsen und Hill-RBF hatten 80% der Augen einen PE innerhalb von ± 0,5 dpt. In der Subgruppe der kurzen Augen wurde der niedrigste PE mit der Hill-RBF erreicht, in der Subgruppe der langen Augen mit der Barrett Universal II und mit der Olsen. Die Unterschiede zwischen den Formeln in den Subgruppenanalysen waren jedoch nicht signifikant.
Schlussfolgerung:Die exaktesten Vorhersagen der postoperativen Refraktion konnten mit der Barrett Universal II erzielt werden. Diese Formel sollte daher künftig für die Berechnung der Linsenstärke der quadrifokalen PanOptix genutzt werden. Die T2, Olsen und Hill-RBF führten ebenfalls zu niedrigen PE. Weitere Studien sollten durchgeführt werden, um insbesondere die neuen Formeln im Zusammenhang mit verschiedenen Multifokallinsen zu untersuchen. Um die Vorhersagegenauigkeit der Formeln in Augen mit extremen Achsenlängen zu bewerten, sind Studien mit größeren Fallzahlen nötig.
Purpose: Bilateral vocal cord dysfunction (bVCD) is a rare but feared complication of thyroid surgery. This long term retrospective study determined the effect of intraoperative neuromonitoring (IONM) of the recurrent laryngeal nerve (RLN) during thyroid surgeries with regard to the rate of bVCD and evaluated the frequency as well as the outcome of staged operations. Methods: Retrospective analysis of prospectively documented data (2000–2019) of a tertiary referral centers’ database. IONM started in 2000 and, since 2010, discontinuation of surgery was encouraged in planned bilateral surgeries to prevent bVCD, if non-transient loss of signal (ntLOS) occurred on the first side. Datasets of the most recent 40-month-period were assessed in detail to determine the clinical outcome of unilateral ntLOS in planned bilateral thyroid procedures. Results: Of 22,573 patients, 65 had bVCD (0.288%). The rate of bVCD decreased from 0.44 prior to 2010 to 0.09% after 2010 (p < 0.001, Chi2). Case reviews of the most recent 40 months period identified ntLOS in 113/3115 patients (3.6%, 2.2% NAR), of which 40 ntLOS were recorded during a planned bilateral procedure (n = 952, 2.1% NAR). Of 21 ntLOS occurring on the first side of the bilateral procedure, 15 procedures were stopped, subtotal contralateral resections were performed, and thyroidectomy was continued in 3 patients respectively, with the use of continuous vagal IONM. Eighteen cases of VCD were documented postop, and all but one patient had a full recovery. Seven patients had staged resections after 1 to 18 months (median 4) after the first procedure. Conclusion: IONM facilitates reduced postoperative bVCD rates. IONM is, therefore, recommendable in planned bilateral procedures. The rate of non-complete bilateral surgery after intraoperative non-transient LOS was 2%.
