Oligonucleotides suppress PKB/Akt and act as superinductors of apoptosis in human keratinocytes

DNA oligonucleotides (ODN) applied to an organism are known to modulate the innate and adaptive immune system. Previous studies showed that a CpG-containing ODN (CpG-1-PTO) and interestingly, also a non-CpG-containing ODN (nCpG- 5-PTO) suppress inflammatory markers in skin. In the present study it was investigated whether these molecules also influence cell apoptosis. Here we show that CpG-1-PTO, nCpG-5-PTO, and also natural DNA suppress the phosphorylation of PKB/Akt in a cell-type-specific manner. Interestingly, only epithelial cells of the skin (normal human keratinocytes, HaCaT and A-431) show a suppression of PKB/Akt. This suppressive effect depends from ODN lengths, sequence and backbone. Moreover, it was found that TGFa-induced levels of PKB/Akt and EGFR were suppressed by the ODN tested. We hypothesize that this suppression might facilitate programmed cell death. By testing this hypothesis we found an increase of apoptosis markers (caspase 3/7, 8, 9, cytosolic cytochrome c, histone associated DNA fragments, apoptotic bodies) when cells were treated with ODN in combination with low doses of staurosporin, a wellknown pro-apoptotic stimulus. In summary the present data demonstrate DNA as a modulator of apoptosis which specifically targets skin epithelial cells.
Purpose: The PELICAN trial evaluates for the first time efficacy and safety of pegylated liposomal doxorubicin (PLD) versus capecitabine as first-line treatment of metastatic breast cancer (MBC).
Methods: This randomized, phase III, open-label, multicenter trial enrolled first-line MBC patients who were ineligible for endocrine or trastuzumab therapy. Cumulative adjuvant anthracyclines of 360 mg/m2 doxorubicin or equivalent were allowed. Left ventricular ejection fraction of >50 % was required. Patients received PLD 50 mg/m2 every 28 days or capecitabine 1250 mg/m2 twice daily for 14 days every 21 days. The primary endpoint was time-to-disease progression (TTP).
Results: 210 patients were randomized (n = 105, PLD and n = 105, capecitabine). Adjuvant anthracyclines were given to 37 % (PLD) and 36 % (capecitabine) of patients. No significant difference was observed in TTP [HR = 1.21 (95 % confidence interval, 0.838–1.750)]. Median TTP was 6.0 months for both PLD and capecitabine. Comparing patients with or without prior anthracyclines, no significant difference in TTP was observed in the PLD arm (log-rank P = 0.64). For PLD versus capecitabine, respectively, overall survival (median, 23.3 months vs. 26.8 months) and time-to-treatment failure (median, 4.6 months vs. 3.7 months) were not statistically significantly different. Compared to PLD, patients on capecitabine experienced more serious adverse events (P = 0.015) and more cardiac events among patients who had prior anthracycline exposure (18 vs. 8 %; P = 0.31).
Conclusion: Both PLD and capecitabine are effective first-line agents for MBC.
Mechanical stress is known to modulate fundamental events such as cell life and death. Mechanical stretch in particular has been identified as a positive regulator of proliferation in skin keratinocytes and other cell systems. In the present study it was investigated whether antiapoptotic signaling is also stimulated by mechanical stretch. It was demonstrated that mechanical stretch rapidly induced the phosphorylation of the proto-oncogene protein kinase B (PKB)/Akt at both phosphorylation sites (serine 473/threonine 308) in different epithelial cells (HaCaT, A-431, and human embryonic kidney-293). Blocking of phosphoinositide 3-OH kinase by selective inhibitors (LY-294002 and wortmannin) abrogated the stretch-induced PKB/Akt phosphorylation. Furthermore mechanical stretch stimulated phosphorylation of epidermal growth factor receptor (EGFR) and the formation of EGFR membrane clusters. Functional blocking of EGFR phosphorylation by either selective inhibitors (AG1478 and PD168393) or dominant-negative expression suppressed stretch-induced PKB/Akt phosphorylation. Finally, the angiotensin II type 1 receptor (AT1-R) was shown to induce positive transactivation of EGFR in response to cell stretch. These findings define a novel signaling pathway of mechanical stretch, namely the activation of PKB/Akt by transactivation of EGFR via angiotensin II type 1 receptor. Evidence is provided that stretch-induced activation of PKB/Akt protects cells against induced apoptosis.
In dieser Studie wurden drei Patientenkollektive der pädiatrischen Gerinnungsambulanz der Uniklinik Frankfurt auf das Vorhandensein von Antiphospholipid-Antikörpern (APA) untersucht: 1.) Kinder mit verlängerter PTT (n=121) 2.) Kinder mit Z.n. cerebralen Infarkten (n=20) 3.) Kinder mit Z.n. venösen Thrombosen (n=29) Diese drei Kollektive wurden mit drei dem jeweiligen Patientenkollektiv alters- und geschlechtsentsprechenden Kontrollkollektiven verglichen: 1.) Kontrollgruppe A: das dem Kollektiv mit Kindern mit verlängerter PTT entsprechende Kontrollkollektiv (n=25) 2.) Kontrollgruppe B: das dem Kollektiv mit Kindern mit cerebralen Infarkten entsprechende Kontrollkollektiv (n=20) 3.) Kontrollgruppe C: das dem Kollektiv mit Kindern mit Thrombosen entsprechende Kontrollkollektiv (n=29) Der statistische Vergleich zwischen den Patientenkollektiven und dem jeweils entsprechenden Kontrollkollektiv erfolgte mit dem Fisher´s exact Test oder dem Wilcoxon-Mann-Whitney-UTest. Es wurden folgende Parameter bestimmt, um die Patienten und die Kontrollen als „Antiphospholipid-Antikörper-positiv“ bzw. „Antiphospholipid-Antikörper-negativ“ zu klassifizieren: Die Anticardiolipine (ACL) IgG und IgM, die Anti-ß2-Glykoproteine I (Anti-ß2GP I) IgG, IgM und IgA, Lupus Screen, Lupus Ratio und die PTT. Bei einzelnen Patienten konnten als weitere Bestätigungstests ebenfalls KCT und ICA herangezogen werden. Als „APA-positiv“ wurden diejenigen Kinder eingestuft, die erhöhte mittels ELISA bestimmte ACL oder Anti-ß2GP I hatten und/oder die positive, über die PTT als Screeningtest und den Lupus Screen und die Lupus Ratio als Bestätigungstests ermittelte Lupusantikoagulantien (LA) hatten. Als Grenzwerte wurden für die ACL und die Anti-ß2GP I die 95. Percentilen der drei Kontrollkollektive berechnet, wobei diese Werte, mit Ausnahme der Anti-ß2GP I, deutlich unter den Grenzwerten des Testherstellers lagen. Für die PTT, den Lupus Screen, die Lupus Ratio, die KCT und den ICA wurden als Grenzwerte die Testherstellerangaben übernommen. APA wurden bei 53/121 Kindern mit verlängerter PTT, bei 13/20 Kindern mit cerebralen Infarkten und bei 16/29 Kindern mit Thrombosen diagnostiziert und traten in allen drei Patientenkollektiven signifikant häufiger (p<0,05) als in dem jeweils entsprechenden Kontrollkollektiv auf. Es wurde eine Odds Ratio von 7,43 errechnet, bei positiven APA einen cerebralen Infarkt zu erleiden und eine Odds Ratio von 7,38, bei positiven APA eine venöse Thrombose zu bekommen. Die Titer der ACL IgG lagen bei den Kindern mit verlängerter PTT und bei den Kindern mit Thrombosen signifikant höher als in der Kontrollgruppe A bzw. C (p=0,002; p<0,001). Erhöht waren die ACL IgG signifikant häufiger bei den Kindern mit cerebralen Infarkten und bei den Kindern mit Thrombosen als in der Kontrollgruppe B bzw. C (p=0,044; p=0,002). Die Titer der ACL IgM lagen in allen drei Patientenkollektiven signifikant höher als in den drei Kontrollgruppen (p<0,05), und bei den Kindern mit cerebralen Infarkten waren die ACL IgM signifikant häufiger erhöht als in der Kontrollgruppe B (p=0,008). Die Titerhöhe der Anti-ß2GP I IgG, IgM und IgA war in keinem der Patientenkollektive signifikant höher als in den Kontrollgruppen. Auch waren Anti-ß2GP I IgG, IgM und IgA in keinem der Patientenkollektive signifikant häufiger erhöht als in der entsprechenden Kontrollgruppe. Die Werte des Lupus Screens lagen in keinem der Patientenkollektive signifikant über denen des entsprechenden Kontrollkollektivs, jedoch wurde bei den Kontrollen im Gegensatz zu den Patienten kein einziger positiver Lupus Screen durch eine erhöhte Lupus Ratio bestätigt, sodass, auch unter Berücksichtigung der PTT, der KCT und des ICA, bei den Kindern mit verlängerter PTT signifikant häufiger LA nachgewiesen wurden als in der Kontrollgruppe A (p=0,003). Bei den Kindern mit Thrombosen waren LA häufiger positiv als in dem Kontrollkollektiv C, aber ohne Signifikanz (p=0,068). APA scheinen eine wichtige Rolle in der Thrombophilie-Diagnostik zu spielen, wobei wir einen statistischen Zusammenhang zwischen ACL und Thrombosen sowie zwischen ACL und cerebralen Infarkten fanden. Auch können vermutlich APA in vielen Fällen für eine verlängerte PTT bei ansonsten klinisch unauffälligen Kindern verantwortlich gemacht werden. Ob es von Bedeutung ist, bei einem Thrombophilie-Screening bzw. zur Abklärung einer TTVerlängerung die Anti-ß2GP I zu bestimmen, wird durch unsere Studie in Frage gestellt.
