From 1945 to the early 1960s, the US government undertook numerous atomic and hydrogen bomb tests. These full-scale explosions were recorded on film from various angles, and at different speeds. Indeed, it soon became required to obtain images of the very first milli-seconds of the expanding phase of the atomic fireball. Ultrahigh-speed cameras able to produce such images were specifically developed for that purpose. This article explores the different “media-temporalities” that intersect in those images. I focus on the “micro-processes happening on a technical level that are very fast,” and more specifically the ones that go into the “Rapatronic camera” designed by Harold Edgerton (head of the US national defense contractor company EG&G) to record the atomic fireball early formation. The scientific slow-motion films and high-speed photographic images operate at the junction of the micro-scale temporality of the atomic explosions’ early phases, and the macro-scale temporality of the political and ecological implications of these explosions. I argue that these films are the objects and inscriptions of micro-temporalities, macro-history and geological times.
Importance Coronavirus disease 2019 (COVID-19) continues to cause considerable morbidity and mortality worldwide. Case reports of hospitalized patients suggest that COVID-19 prominently affects the cardiovascular system, but the overall impact remains unknown.
Objective To evaluate the presence of myocardial injury in unselected patients recently recovered from COVID-19 illness.
Design, Setting, and Participants In this prospective observational cohort study, 100 patients recently recovered from COVID-19 illness were identified from the University Hospital Frankfurt COVID-19 Registry between April and June 2020.
Exposure Recent recovery from severe acute respiratory syndrome coronavirus 2 infection, as determined by reverse transcription–polymerase chain reaction on swab test of the upper respiratory tract.
Main Outcomes and Measures Demographic characteristics, cardiac blood markers, and cardiovascular magnetic resonance (CMR) imaging were obtained. Comparisons were made with age-matched and sex-matched control groups of healthy volunteers (n = 50) and risk factor–matched patients (n = 57).
Results Of the 100 included patients, 53 (53%) were male, and the mean (SD) age was 49 (14) years. The median (IQR) time interval between COVID-19 diagnosis and CMR was 71 (64-92) days. Of the 100 patients recently recovered from COVID-19, 67 (67%) recovered at home, while 33 (33%) required hospitalization. At the time of CMR, high-sensitivity troponin T (hsTnT) was detectable (greater than 3 pg/mL) in 71 patients recently recovered from COVID-19 (71%) and significantly elevated (greater than 13.9 pg/mL) in 5 patients (5%). Compared with healthy controls and risk factor–matched controls, patients recently recovered from COVID-19 had lower left ventricular ejection fraction, higher left ventricle volumes, and raised native T1 and T2. A total of 78 patients recently recovered from COVID-19 (78%) had abnormal CMR findings, including raised myocardial native T1 (n = 73), raised myocardial native T2 (n = 60), myocardial late gadolinium enhancement (n = 32), or pericardial enhancement (n = 22). There was a small but significant difference between patients who recovered at home vs in the hospital for native T1 mapping (median [IQR], 1119 [1092-1150] ms vs 1141 [1121-1175] ms; P = .008) and hsTnT (4.2 [3.0-5.9] pg/dL vs 6.3 [3.4-7.9] pg/dL; P = .002) but not for native T2 mapping. None of these measures were correlated with time from COVID-19 diagnosis (native T1: r = 0.07; P = .47; native T2: r = 0.14; P = .15; hsTnT: r = −0.07; P = .50). High-sensitivity troponin T was significantly correlated with native T1 mapping (r = 0.33; P < .001) and native T2 mapping (r = 0.18; P = .01). Endomyocardial biopsy in patients with severe findings revealed active lymphocytic inflammation. Native T1 and T2 were the measures with the best discriminatory ability to detect COVID-19–related myocardial pathology.
Conclusions and Relevance In this study of a cohort of German patients recently recovered from COVID-19 infection, CMR revealed cardiac involvement in 78 patients (78%) and ongoing myocardial inflammation in 60 patients (60%), independent of preexisting conditions, severity and overall course of the acute illness, and time from the original diagnosis. These findings indicate the need for ongoing investigation of the long-term cardiovascular consequences of COVID-19.
Short linear motifs (SLiMs) located in disordered regions of multidomain proteins are important for the organization of protein–protein interaction networks. By dynamic association with their binding partners, SLiMs enable assembly of multiprotein complexes, pivotal for the regulation of various aspects of cell biology in higher organisms. Despite their importance, there is a paucity of molecular tools to study SLiMs of endogenous proteins in live cells. LC3 interacting regions (LIRs), being quintessential for orchestrating diverse stages of autophagy, are a prominent example of SLiMs and mediate binding to the ubiquitin-like LC3/GABARAP family of proteins. The role of LIRs ranges from the posttranslational processing of their binding partners at early stages of autophagy to the binding of selective autophagy receptors (SARs) to the autophagosome. In order to generate tools to study LIRs in cells, we engineered high affinity binders of LIR motifs of three archetypical SARs: OPTN, p62, and NDP52. In an array of in vitro and cellular assays, the engineered binders were shown to have greatly improved affinity and specificity when compared with the endogenous LC3/GABARAP family of proteins, thus providing a unique possibility for modulating LIR interactions in living systems. We exploited these novel tools to study the impact of LIR inhibition on the fitness and the responsiveness to cytarabine treatment of THP-1 cells – a model for studying acute myeloid leukemia (AML). Our results demonstrate that inhibition of LIR of a single autophagy receptor is insufficient to sensitize the cells to cytarabine, while simultaneous inhibition of three LIR motifs in three distinct SARs reduces the IC50 of the chemotherapeutic.
Background: Meta-analysis of observational studies concluded that soft drinks may increase the risk of depression, while high consumption of coffee and tea may reduce the risk. Objectives were to explore the associations between the consumption of soft drinks, coffee or tea and: (1) a history of major depressive disorder (MDD) and (2) the severity of depressive symptoms clusters (mood, cognitive and somatic/vegetative symptoms). Methods: Cross-sectional and longitudinal analysis based on baseline and 12-month-follow-up data collected from four countries participating in the European MooDFOOD prevention trial. In total, 941 overweight adults with subsyndromal depressive symptoms aged 18 to 75 years were analyzed. History of MDD, depressive symptoms and beverages intake were assessed. Results: Sugar-sweetened soft drinks were positively related to MDD history rates whereas soft drinks with non-nutritive sweeteners were inversely related for the high vs. low categories of intake. Longitudinal analysis showed no significant associations between beverages and mood, cognitive and somatic/vegetative clusters. Conclusion: Our findings point toward a relationship between soft drinks and past MDD diagnoses depending on how they are sweetened while we found no association with coffee and tea. No significant effects were found between any studied beverages and the depressive symptoms clusters in a sample of overweight adults.
We explore the tilted-pulse-front excitation technique to control the superradiant emission of terahertz (THz) pulses from large-area photonconductive semiconductor switches. Two cases are studied. First, a photoconductive antenna emitting into free space, where the propagation direction of the optically generated THz beam is controlled by the choice of the tilt angle of the pump pulse front. Second, a THz waveguide structure with an integrated photoconductive window for the generation of THz radiation, where the injection of the THz radiation into a waveguide mode is optimized by the pulse front tilt. By providing long interaction lengths, such a waveguide-based optical-pump/THz-probe set-up may provide a new platform for the study of diverse short-lived optically induced excitations.
Background and Aims: Monocyte chemotactic protein-1 (MCP-1) is a potent chemoattractant for monocytes. It is involved in pathogenesis of several inflammatory diseases. Hepatic MCP-1 is a readout of macrophage activation. While inflammation is a major driver of liver disease progression, the origin and role of circulating MCP-1 as a biomarker remains unclear.
Methods: Hepatic CC-chemokine ligand 2 (CCL2) expression and F4/80 staining for Kupffer cells were measured and correlated in a mouse model of chronic liver disease (inhalative CCl4 for 7 weeks). Next, hepatic RNA levels of CCL2 were measured in explanted livers of 39 patients after transplantation and correlated with severity of disease. Changes in MCP-1 were further evaluated in a rat model of experimental cirrhosis and acute-on-chronic liver failure (ACLF). Finally, we analyzed portal and hepatic vein levels of MCP-1 in patients receiving transjugular intrahepatic portosystemic shunt insertion for complications of portal hypertension.
Results: In this mouse model of fibrotic hepatitis, hepatic expression of CCL2 (P = 0.009) and the amount of F4/80 positive cells in the liver (P < 0.001) significantly increased after induction of hepatitis by CCl4 compared to control animals. Moreover, strong correlation of hepatic CCL2 expression and F4/80 positive cells were seen (P = 0.023). Furthermore, in human liver explants, hepatic transcription levels of CCL2 correlated with the MELD score of the patients, and thus disease severity (P = 0.007). The experimental model of ACLF in rats revealed significantly higher levels of MCP-1 plasma (P = 0.028) and correlation of hepatic CCL2 expression (R = 0.69, P = 0.003). Particularly, plasma MCP-1 levels did not correlate with peripheral blood monocyte CCL2 expression. Finally, higher levels of MCP-1 were observed in the hepatic compared to the portal vein (P = 0.01) in patients receiving TIPS. Similarly, a positive correlation of MCP-1 with Child-Pugh score was observed (P = 0.018). Further, in the presence of ACLF, portal and hepatic vein levels of MCP-1 were significantly higher compared to patients without ACLF (both P = 0.039).
Conclusion: Circulating levels of MCP-1 mainly derive from the injured liver and are associated with severity of liver disease. Therefore, liver macrophages contribute significantly to disease progression. Circulating MCP-1 may reflect the extent of hepatic macrophage activation.
Alcoholism is one of the leading and increasingly prevalent reasons of liver associated morbidity and mortality worldwide. Alcoholic hepatitis (AH) constitutes a severe disease with currently no satisfying treatment options. Lipoxin A4 (LXA4), a 15-lipoxygenase (ALOX15)-dependent lipid mediator involved in resolution of inflammation, showed promising pre-clinical results in the therapy of several inflammatory diseases. Since inflammation is a main driver of disease progression in alcoholic hepatitis, we investigated the impact of endogenous ALOX15-dependent lipid mediators and exogenously applied LXA4 on AH development. A mouse model for alcoholic steatohepatitis (NIAAA model) was tested in Alox12/15+/+ and Alox12/15−/− mice, with or without supplementation of LXA4. Absence of Alox12/15 aggravated parameters of liver disease, increased hepatic immune cell infiltration in AH, and elevated systemic neutrophils as a marker for systemic inflammation. Interestingly, i.p. injections of LXA4 significantly lowered transaminase levels only in Alox12/15−/− mice and reduced hepatic immune cell infiltration as well as systemic inflammatory cytokine expression in both genotypes, even though steatosis progressed. Thus, while LXA4 injection attenuated selected parameters of disease progression in Alox12/15−/− mice, its beneficial impact on immunity was also apparent in Alox12/15+/+ mice. In conclusion, pro-resolving lipid mediators may be beneficial to reduce inflammation in alcoholic hepatitis.
Bei der Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS) handelt es sich um eine hochprävalente Erkrankung, die bisher insbesondere im Erwachsenenalter nur unzureichend berücksichtigt wurde. Neben den Kernsymptomen bestehend aus Einschränkungen in der Aufmerksamkeit sowie einem erhöhten Maß an Hyperaktivität und Impulsivität gilt es, Komorbiditäten und Beeinträchtigungen zu berücksichtigen, die mit dieser Erkrankung einhergehen. In früheren Studien konnte bereits nachgewiesen werden, dass sowohl Kinder als auch Erwachsene mit einer ADHS vermehrte Unfälle und infolgedessen eine erhöhte Rate an stationären Behandlungen aufweisen. Zusätzlich besteht bei ADHS-Betroffenen ein höheres Risiko, frühzeitig zu versterben, wobei Unfälle als unnatürliche Todesursache den häufigsten Grund darstellen. Bisher existieren auf internationaler Ebene einige Studien, die sich mit den Zusammenhängen von adulter ADHS und Unfallraten beschäftigten. Eine differenzierte Betrachtung, die eine deutsche Population einschließt und den Einfluss des Geschlechts auf das Unfallgeschehen bei ADHS-Betroffenen untersucht, wurde bisher nicht realisiert. Aus diesen Gründen führten wir eine Querschnittsstudie auf unfallchirurgischen Stationen an zwei Kliniken in Frankfurt am Main durch. Im Rahmen der Studie sollte die Prävalenz von adulter ADHS bei stationär betreuten Unfallopfern ermittelt und mögliche unfallcharakteristische Unterschiede zwischen den Geschlechtern der ADHS-Positivkohorte erfasst werden.
Mithilfe der etablierten Adult ADHD Self-Report Scale (ASRS v1.1) der WHO wurden alle Unfallopfer, die den Einschlusskriterien entsprachen, auf adulte ADHS gescreent. Bei der Auswertung des ASRS verwendeten wir zwei unterschiedliche Methoden, die bereits in früheren Studien zur Anwendung kamen. Durch einen eigens erstellten Unfallfragebogen konnten Unfallcharakteristika sowie psychische Komorbiditäten der ADHSPositivkohorte erfasst werden. Zusätzlich erhoben wir zum Vergleich die Unfallcharakteristika bei einer nicht von ADHS betroffenen Kontrollgruppe. Bei einem Teil der positiv gescreenten Patienten wurde eine ausführliche Testung in der ADHS Spezialambulanz des Universitätsklinikums Frankfurt am Main vorgenommen. Mittels diagnostischer Interviews und weiteren Selbstauskunftsfragebögen (DIVA, HASE) konnte eine definitive ADHS-Diagnose gestellt und das positive Screeningergebnis verifiziert werden.
Wir stellten fest, dass die Prävalenz adulter ADHS unter Unfallopfern mit 6,89 % bzw. 8,68 % in Abhängigkeit der ASRS Auswertungskriterien im Vergleich zu der Prävalenz der adulten ADHS in der Allgemeinbevölkerung erhöht war. Des Weiteren gab es mehr Männer mit einem positiven ADHS-Screeningergebnis und einem Unfallgeschehen. Es konnten keine signifikanten geschlechtsspezifischen Unterschiede in den Unfallcharakteristika und Unfallorten in der ADHS Positivkohorte festgestellt werden. Auch in dem Vorhandensein psychischer Komorbiditäten unterschieden sich die beiden Geschlechtergruppen der ADHS-Positivkohorte nicht. Unabhängig der Geschlechterzugehörigkeit wurden die meisten Unfälle der ADHS-Positivkohorte im Straßenverkehr verzeichnet und als selbstverschuldet eingestuft. Es konnten signifikant mehr Unfälle außerhalb des Straßenverkehrs in der nicht von ADHS betroffenen Kontrollgruppe im Vergleich zu der ADHS Positivkohorte nachgewiesen werden. Insgesamt war nur bei 14 % aller positiv gescreenten Patienten eine ADHS-Erkrankung bereits bekannt und diagnostiziert worden.
Auf Grundlage der Ergebnisse dieser Studie lässt sich schlussfolgern, dass Menschen mit einer ADHS-Symptomatik vermehrt auf unfallchirurgischen Stationen anzutreffen sind und im Einklang mit früheren Forschungsergebnissen ein erhöhtes Unfall- und Verletzungsrisiko aufweisen. Darüber hinaus folgern wir aus unseren Ergebnissen, dass sich Männer und Frauen mit einer ADHS-Erkrankung hinsichtlich Symptomatik, Komorbidität und spezifischer Unfallcharakteristika ähnlicher sein könnten, als dass sie sich in diesen Bereichen unterscheiden. Im Hinblick auf die geringen vorbestehenden ADHS-Diagnosen in der Positivkohorte erscheint eine Etablierung eines Screeningsystems bei Patienten sinnvoll, die im Rahmen von Unfallgeschehen gehäuft stationär betreut werden müssen. Da ADHS-Betroffene ein erhöhtes Risiko für multiple Unfälle zu haben scheinen, könnte unter diesen Voraussetzungen eine adulte ADHS-Symptomatik früher erkannt und negative Folgen wie eine erhöhte Unfallrate reduziert werden. Methodologische Limitierungen bestanden in unserer Erhebung durch eine geringe Anzahl an ausführlichen Testungen zur Verifizierung der ADHS-Diagnose sowie dem Informationsgewinn auf der Basis einer Selbstauskunft. Um Ergebnisse aussagekräftiger zu gestalten und auch kleinere Unfälle zu erfassen, erscheint eine Ausweitung der Testung in Ambulanzen sinnvoll.
Purpose: To develop and validate a CT-based radiomics signature for the prognosis of loco-regional tumour control (LRC) in patients with locally advanced head and neck squamous cell carcinoma (HNSCC) treated by primary radiochemotherapy (RCTx) based on retrospective data from 6 partner sites of the German Cancer Consortium - Radiation Oncology Group (DKTK-ROG).
Material and methods: Pre-treatment CT images of 318 patients with locally advanced HNSCC were collected. Four-hundred forty-six features were extracted from each primary tumour volume and then filtered through stability analysis and clustering. First, a baseline signature was developed from demographic and tumour-associated clinical parameters. This signature was then supplemented by CT imaging features. A final signature was derived using repeated 3-fold cross-validation on the discovery cohort. Performance in external validation was assessed by the concordance index (C-Index). Furthermore, calibration and patient stratification in groups with low and high risk for loco-regional recurrence were analysed.
Results: For the clinical baseline signature, only the primary tumour volume was selected. The final signature combined the tumour volume with two independent radiomics features. It achieved moderately good discriminatory performance (C-Index [95% confidence interval]: 0.66 [0.55–0.75]) on the validation cohort along with significant patient stratification (p = 0.005) and good calibration.
Conclusion: We identified and validated a clinical-radiomics signature for LRC of locally advanced HNSCC using a multi-centric retrospective dataset. Prospective validation will be performed on the primary cohort of the HNprädBio trial of the DKTK-ROG once follow-up is completed.
Understanding effects of emotional valence and stress on children’s memory is important for educational and legal contexts. This study disentangled the effects of emotional content of to-be-remembered information (i.e., items differing in emotional valence and arousal), stress exposure, and associated cortisol secretion on children’s memory. We also examined whether girls’ memory is more affected by stress induction. A total of 143 6- and 7-year-old children were randomly allocated to the Trier Social Stress Test for Children (n = 103) or a control condition (n = 40). At 25 min after stressor onset, children incidentally encoded 75 objects varying in emotional valence (crossed with arousal) together with neutral scene backgrounds. We found that response bias corrected memory was worse for low-arousing negative items than for neutral and positive items, with the latter two categories not being different from each other. Whereas boys’ memory was largely unaffected by stress, girls in the stress condition showed worse memory for negative items, especially the low-arousing ones, than girls in the control condition. Girls, compared with boys, reported higher subjective stress increases following stress exposure and had higher cortisol stress responses. Whereas a higher cortisol stress response was associated with better emotional memory in girls in the stress condition, boys’ memory was not associated with their cortisol secretion. Taken together, our study suggests that 6- and 7-year-old children, more so girls, show memory suppression for negative information. Girls’ memory for negative information, compared with that of boys, is also more strongly modulated by stress experience and the associated cortisol response.

Over the past two decades, our understanding of Parkinson's disease (PD) has been gleaned from the discoveries made in familial and/or sporadic forms of PD in the Caucasian population. The transferability and the clinical utility of genetic discoveries to other ethnically diverse populations are unknown. The Indian population has been under-represented in PD research. The Genetic Architecture of PD in India (GAP-India) project aims to develop one of the largest clinical/genomic bio-bank for PD in India. Specifically, GAP-India project aims to: (1) develop a pan-Indian deeply phenotyped clinical repository of Indian PD patients; (2) perform whole-genome sequencing in 500 PD samples to catalog Indian genetic variability and to develop an Indian PD map for the scientific community; (3) perform a genome-wide association study to identify novel loci for PD and (4) develop a user-friendly web-portal to disseminate results for the scientific community. Our “hub-spoke” model follows an integrative approach to develop a pan-Indian outreach to develop a comprehensive cohort for PD research in India. The alignment of standard operating procedures for recruiting patients and collecting biospecimens with international standards ensures harmonization of data/bio-specimen collection at the beginning and also ensures stringent quality control parameters for sample processing. Data sharing and protection policies follow the guidelines established by local and national authorities.We are currently in the recruitment phase targeting recruitment of 10,200 PD patients and 10,200 healthy volunteers by the end of 2020. GAP-India project after its completion will fill a critical gap that exists in PD research and will contribute a comprehensive genetic catalog of the Indian PD population to identify novel targets for PD.
Purpose of Review: To provide an overview of current surgical peri-implantitis treatment options.
Recent Findings: Surgical procedures for peri-implantitis treatment include two main approaches: non-augmentative and augmentative therapy. Open flap debridement (OFD) and resective treatment are non-augmentative techniques that are indicated in the presence of horizontal bone loss in aesthetically nondemanding areas. Implantoplasty performed adjunctively at supracrestally and buccally exposed rough implant surfaces has been shown to efficiently attenuate soft tissue inflammation compared to control sites. However, this was followed by more pronounced soft tissue recession. Adjunctive augmentative measures are recommended at peri-implantitis sites exhibiting intrabony defects with a minimum depth of 3 mm and in the presence of keratinized mucosa. In more advanced cases with combined defect configurations, a combination of augmentative therapy and implantoplasty at exposed rough implant surfaces beyond the bony envelope is feasible.
Summary: For the time being, no particular surgical protocol or material can be considered as superior in terms of long-term peri-implant tissue stability.
Onkologische Erkrankungen im Kindesalter und jungen Erwachsenenalter haben nicht selten eine gute Prognose. Entsprechend wird für Betroffene früher oder später die Frage relevant, inwieweit nach einer onkologischen Behandlung die Fertilität beeinträchtigt ist. Nicht nur der Zeitraum der Fertilität, sondern auch die Wahrscheinlichkeit eines vorzeitigen Ovarialversagens mit allen Risiken eines längerfristigen Östrogenmangels ist für die Lebensplanung der Frauen wichtig. Mittlerweile können vor Behandlung fertilitätserhaltende Maßnahmen angeboten werden. Sie bieten manchmal die einzige Chance, auf ovarielle Reserven nach Behandlung zurückgreifen zu können, sind aber nicht immer nötig und von späterem Nutzen. Das Anti-Müller-Hormon (AMH) hat sich als validester Marker für die Beurteilung der ovariellen Reserve herausgestellt. Mithilfe dessen sind Prognosen über die Ovarreserve vor und nach der onkologischen Therapie möglich. Dies erleichtert die Entscheidung für die Indikation für fertilitätserhaltende Maßnahmen und kann wegweisend in der Lebensplanung der Frauen und Familien sein.
We introduce a novel approach based on elas- tic and inelastic scattering rates to extract the hyper-surface of the chemical freeze-out from a hadronic transport model in the energy range from Elab = 1.23 AGeV to √sNN = 62.4 GeV. For this study, the Ultra-relativistic Quantum Molecular Dynamics (UrQMD) model combined with a coarse-graining method is employed. The chemical freeze- out distribution is reconstructed from the pions through sev- eral decay and re-formation chains involving resonances and taking into account inelastic, pseudo-elastic and string excita- tion reactions. The extracted average temperature and baryon chemical potential are then compared to statistical model analysis. Finally we investigate various freeze-out criteria suggested in the literature. We confirm within this micro- scopic dynamical simulation, that the chemical freeze-out at all energies coincides with ⟨E⟩/⟨N⟩ ≈ 1 GeV, while other criteria, like s/T 3 = 7 and nB +nB ̄ ≈ 0.12 fm−3 are limited to higher collision energies.
Bange zeigt mit seiner Studie zum römischen Kreditgeldwesen auf, dass bereits lange vor dem 13. Jahrhundert – so die ‚klassische‘ Einschätzung für das Aufkommen eines modernen Bankwesens – ein Kreditgeldsystem verwendet wurde, und unterstützt eine modernistische Deutung der römischen Wirtschaftsgeschichte. In der Einleitung (S. 14) wendet er sich gegen die negativen Ansichten von Moses I. Finley und steht dabei in mancher Weise in einer Tradition mit anderen geldwirtschaftlichen Forschungsbeiträgen, so etwa mit Sitta von Redens wichtiger Monographie "Money in Classical Antiquity", wobei die schematische bipolare Wahrnehmung primitivistischer oder modernistischer Ansätze an sich in weiten Teilen der aktuellen Wirtschaftsgeschichte als überholt anzusehen ist. Ferner beklagt Bange, dass gerade die numismatische Forschung eine lediglich verengte, letztlich in Übereinstimmung mit Finley vereinfachte "Geld gleich Münze"-Perspektive eingenommen hätte (S. 15). Groundwater is the largest source of accessible freshwater with its dynamics having significantly changed due to human withdrawals, and being projected to continue to as a result of climate change. The pumping of groundwater has led to lowered water tables, decreased base flow, and depletion.
Global hydrological models (GHMs) are used to simulate the global freshwater cycle, assessing impacts of changes in climate and human freshwater use. Currently, groundwater is commonly represented by a bucket-like linear storage component in these models. Bucket models, however, cannot provide information on the location of the groundwater table. Due to this limitation, they can only simulate groundwater discharge to surface water bodies but not recharge from surface water to groundwater and calculate no lateral and vertical groundwater flow whatsoever among grid cells. For instance this may lead to an underestimation of groundwater resources in semiarid areas, where groundwater is often replenished by surface water. In order to overcome these limitations it is necessary to replace the linear groundwater model in GHMs with a hydraulic head gradient-based groundwater flow model
This thesis presents the newly developed global groundwater model G3M and its coupling to the GHM WaterGAP spanning over 70,000 lines of newly developed code. Development and validation of the modeling software are discussed along with numerical challenges. Based on the newly developed software, a global natural equilibrium groundwater model is presented showing better agreements with observations than previous models. Groundwater discharge to rivers is found to be the most dominant flow component globally, compared to flows to other surface water bodies and lateral flows. Furthermore, first global maps of the distribution of gaining and losing surface water bodies are displayed.
For the purpose of determining the uncertainty in model outcomes a sensitivity study is conducted with an innovative approach through applying a global sensitivity analysis for a computationally complex model. First global maps of spatially distributed parameter sensitivities are presented. The results at hand indicate that globally simulated hydraulic heads are equally sensitive to hydraulic conductivity, groundwater recharge and surface water body elevation, even though parameter sensitivities do vary regionally.
A high resolution model of New Zealand is developed to further understand the involved uncertainties connected to the spatial resolution of the global model. This thesis finds that a new understanding is necessary how these models can be evaluated and that a simple increase in spatial resolution is not improving the model performance when compared to observations.
Alongside the assessment of the natural equilibrium, the concept of a fully coupled transient model as integrated storage component replacing the former model in the hydrological model WaterGAP is discussed. First results reveal that the model shows reasonable response to seasonal variability although it contains persistent head trends leading to global overestimates of water table depth due to an incomplete coupling. Nonetheless, WaterGAP-G3M is already able to show plausible long term storage trends for areas that are known to be affected by groundwater depletion. In comparison with two established regional models in the Central Valley the coupled model shows a highly promising simulation of storage declines.
Global‐scale gradient‐based groundwater models are a new endeavor for hydrologists who wish to improve global hydrological models (GHMs). In particular, the integration of such groundwater models into GHMs improves the simulation of water flows between surface water and groundwater and of capillary rise and thus evapotranspiration. Currently, these models are not able to simulate water table depth adequately over the entire globe. Unsatisfactory model performance compared to well observations suggests that a higher spatial resolution is required to better represent the high spatial variability of land surface and groundwater elevations. In this study, we use New Zealand as a testbed and analyze the impacts of spatial resolution on the results of global groundwater models. Steady‐state hydraulic heads simulated by two versions of the global groundwater model G3M, at spatial resolutions of 5 arc‐minutes (9 km) and 30 arc‐seconds (900 m), are compared with observations from the Canterbury region. The output of three other groundwater models with different spatial resolutions is analyzed as well. Considering the spatial distribution of residuals, general patterns of unsatisfactory model performance remain at the higher resolutions, suggesting that an increase in model resolution alone does not fix problems such as the systematic overestimation of hydraulic head. We conclude that (1) a new understanding of how low‐resolution global groundwater models can be evaluated is required, and (2) merely increasing the spatial resolution of global‐scale groundwater models will not improve the simulation of the global freshwater system.
The float serve is an effective weapon to impede the attack of the opposing team. Because of its great importance in indoor and beach volleyball, we measured and quantified the float effect. We recorded 24 float serves of 12 top athletes in beach volleyball and indoor volleyball, respectively, and analyzed them using video analysis. We determined the 3D trajectories of the ball flight and developed two measures to describe the size of the float effect, the mean residuals and the anticipation error. Both were derived from regression models. These measures suggest that the float effect is greater in the vertical plane than in the horizontal plane, both for indoor and beach volleyball. Analyses of ball release velocities suggest that a certain ball release velocity is a necessary, but not sufficient, condition for ball floating. A validation of the float measurements with subjective expert ratings showed a correlation with the horizontal deviations. This study provides a new approach to analyze floating in on-court volleyball serves and broadens the knowledge for float effects in sports.
Background: Essential Tremor (ET) is a progressive neurological disorder characterized by postural and kinetic tremor most commonly affecting the hands and arms. Medically intractable ET can be treated by deep brain stimulation (DBS) of the ventral intermediate nucleus of thalamus (VIM). We investigated whether the location of the effective contact (most tremor suppression with at least side effects) in VIM-DBS for ET changes over time, indicating a distinct mechanism of loss of efficacy that goes beyond progression of tremor severity, or a mere reduction of DBS efficacy.
Methods: We performed programming sessions in 10 patients who underwent bilateral vim-DBS surgery between 2009 and 2017 at our department. In addition to the intraoperative (T1) and first clinical programming session (T2) a third programming session (T3) was performed to assess the effect- and side effect threshold (minimum voltage at which a tremor suppression or side effects occurred). Additionally, we compared the choice of the effective contact between T1 and T2 which might be affected by a surgical induced “brain shift.”
Discussion: Over a time span of about 4 years VIM-DBS in ET showed continuous efficacy in tremor suppression during stim-ON compared to stim-OFF. Compared to immediate postoperative programming sessions in ET-patients with DBS, long-term evaluationshowednorelevantchangeinthechoiceofcontactwithrespecttosideeffects andefficacy.InthemajorityofthecasestheactivecontactatT2didnotcorrespondtothe most effective intraoperative stimulation site T1, which might be explained by a brain-shift due to cerebral spinal fluid loss after neurosurgical procedure.
Different experimental multiple trauma models induce comparable inflammation and organ injury

Multiple injuries appear to be a decisive factor for experimental polytrauma. Therefore, our aim was to compare the inflammatory response and organ damage of five different monotrauma with three multiple trauma models. For this, mice were randomly assigned to 10 groups: Healthy control (Ctrl), Sham, hemorrhagic shock (HS), thoracic trauma (TxT), osteotomy with external fixation (Fx), bilateral soft tissue trauma (bsTT) or laparotomy (Lap); polytrauma I (PT I, TxT + HS + Fx), PT II (TxT + HS + Fx + Lap) and one multi-trauma group (MT, TxT + HS + bsTT + Lap). The inflammatory response and organ damage were quantified at 6 h by analyses of IL-6, IL-1β, IL-10, CXCL1, SAA1, HMGB1 and organ injury. Systemic IL-6 increased in all mono and multiple trauma groups, while CXCL1 increased only in HS, PT I, PT II and MT vs. control. Local inflammatory response was most prominent in HS, PT I, PT II and MT in the liver. Infiltration of inflammatory cells into lung and liver was significant in all multiple trauma groups vs. controls. Hepatic and pulmonary injury was prominent in HS, PT I, PT II and MT groups. These experimental multiple trauma models closely mimic the early post-traumatic inflammatory response in human. Though, the choice of read-out parameters is very important for therapeutic immune modulatory approaches.
At the beginning of the 1980s, an increased frequency of immune deficiency was discovered in a population of homosexual men, which is nowadays known as the Acquired Immune Deficiency Syndrome (AIDS). A few years later, the retro virus Human Immunodeficiency Virus 1(HIV-1) has been discovered as the cause of AIDS. Since the beginning of the pandemic, more that 74 million people have become infected and more than 32 million people died. In 2018, it was estimated that 38 million people where living with HIV-1 of which 24.5 million had access to Highly Active Antiretroviral Therapy (HAART), which blocks viral replication and prevents the progression towards AIDS. In the most cases an HIV-1 infection leads to the patient’s death within a few years Without HAART.
Taken together, this thesis shows that hematopoietic stem and progenitor cells harbor the prerequisites and characteristics to form an HIV-1 reservoir in vivo. The subsets of HSCs, MPPs and CD34+CD38+ progenitors harbor CD4 & CXCR4 double-positive cells as well as a lower amount of CD4 & CCR5 doublepositive cells. In addition, the susceptibility to X4-tropic HIV-1 is shown in vitro. Susceptibility to R5-tropic HIV-1 is only seen to a very low amount for CD34+CD38+ progenitors. The results also show that transduced HSPCs are capable to pass on integrated viral genomes via proliferation and differentiation during in vitro colony formation. More over the experiments provide evidence that this can take place for long time span as the outcome of the replating assays shows. Ex vivo analysis of HSPCs isolated from PLHIV also suggests that these cells are susceptible to HIV-1. Proviral DNA detection using a nested PCR showed infection of Lin- cells of a single donor with an R5-tropic subtype B HIV-1 clone. However, the assay could not detect infection of CD34+ cells. The
received results of this thesis are in agreement with previously published results. Albeit the obvious susceptibility to HIV-1 and existing reports of viral survival within HSPCs for several years, the low frequency of detected in vivo infected HSPCs could be related to the cytopathic effects of HIV-1 during replication resulting in cell death of potentially infected CD34+ cells. Other reasons could be associated with assay sensitivity or the small number of available patient samples. This makes hematopoietic stem and progenitor cells a target, which can be infected by HIV-1. The role and the clinical relevance of hematopoietic stem and progenitor cells in contribution to the latent viral HIV-1 reservoir within an HIV-1 infected patient needs to be further analyzed.
Brain aging is one of the major risk factors for the development of several neurodegenerative diseases. Therefore, mitochondrial dysfunction plays an important role in processes of both, brain aging and neurodegeneration. Aged mice including NMRI mice are established model organisms to study physiological and molecular mechanisms of brain aging. However, longitudinal data evaluated in one cohort are rare but are important to understand the aging process of the brain throughout life, especially since pathological changes early in life might pave the way to neurodegeneration in advanced age. To assess the longitudinal course of brain aging, we used a cohort of female NMRI mice and measured brain mitochondrial function, cognitive performance, and molecular markers every 6 months until mice reached the age of 24 months. Furthermore, we measured citrate synthase activity and respiration of isolated brain mitochondria. Mice at the age of three months served as young controls. At six months of age, mitochondria-related genes (complex IV, creb-1, β-AMPK, and Tfam) were significantly elevated. Brain ATP levels were significantly reduced at an age of 18 months while mitochondria respiration was already reduced in middle-aged mice which is in accordance with the monitored impairments in cognitive tests. mRNA expression of genes involved in mitochondrial biogenesis (cAMP response element-binding protein 1 (creb-1), peroxisome proliferator-activated receptor gamma coactivator 1-alpha (PGC1-α), nuclear respiratory factor-1 (Nrf-1), mitochondrial transcription factor A (Tfam), growth-associated protein 43 (GAP43), and synaptophysin 1 (SYP1)) and the antioxidative defense system (catalase (Cat) and superoxide dismutase 2 (SOD2)) was measured and showed significantly decreased expression patterns in the brain starting at an age of 18 months. BDNF expression reached, a maximum after 6 months. On the basis of longitudinal data, our results demonstrate a close connection between the age-related decline of cognitive performance, energy metabolism, and mitochondrial biogenesis during the physiological brain aging process.
Derzeit breiten sich gebietsfremde Stechmücken (Diptera: Culicidae) aufgrund von Globalisierung und Klimawandel auf der ganzen Welt aus und bilden neue, stabile Populationen. Wegen ihrer hämatophagen Ernährungsweise sind sie Überträger von Pathogenen, die teilweise schwere bis tödliche Krankheiten beim Menschen, seinen Haustieren oder auch Wildtieren auslösen können. Mit den Stechmücken treten daher auch Infektionskrankheiten vermehrt in Gebieten auf, in denen sie vorher nicht vorkamen oder als bereits ausgerottet galten. Da die meisten im Menschen wirksamen Pathogene nicht durch Impfungen kontrolliert werden können, bleibt als eine der wenigen Möglichkeit der Krankheitsprävention die Dezimierung der Stechmückenpopulation. Daher sind Stechmücken momentan im Fokus von biologischer und epidemiologischer Forschung. Diese hat zum Ziel epidemische Krankheitsausbrüche vektorübertragener Krankheiten in der menschlichen Population zu verhindern. Eine Verringerung der lokalen Stechmückenpopulation bis hin zum Aussterben kann durch die Verwendung von Insektiziden, die Vernichtung von Bruthabitaten oder anderen Kontrollmaßnahmen erreicht werden. Jedoch sind diese Maßnahmen unterschiedlich effektiv, haben zum Teil unerwünsch-te ökologische und gesundheitsschädigende Folgen und sind unterschiedlich aufwendig und kostenintensiv in der Anwendung. Für die Entwicklung eines integrierten, effektiven, zielgerichteten und kostengünstigen Vektormanagements fehlen bislang jedoch die populationsbiologischen Grundlagen.
Ziel dieser Arbeit ist daher die Schaffung der Datengrundlage eines Integrierten Stechmückenmanagements für die Asiatische Buschmücke (Aedes japonicus japonicus THEOBALD 1901), die am weitesten verbreitete exotische Stechmücke in Deutschland. Schwerpunkte dafür wurden auf das zeitliche und räumliche Vorkommen, die Temperaturabhängigkeit des Lebenszyklus, sowie die Wirksamkeit von Kontrollmethoden gelegt.
Die Kenntnis der räumlichen Verbreitung und saisonalen Häufigkeit der Stechmücken ist notwendig, um befallene Standorte und Zeitpunkte des größten Populationszuwachses definieren zu können. Die Verbreitung und die Häufigkeit der endothermen Stechmücken sind stark von der Umgebungstemperatur abhängig, die beispielsweise deren Entwicklungsdauer und Sterblichkeit beeinflusst. Dabei entwickeln sich die verschiedenen Stadien (Ei, Larven, Puppe, Imago), die eine Stechmücke während ihres Lebens durchläuft, in Abhängigkeit von der Umgebungstemperatur unterschiedlich und haben jeweils andere Temperaturpräferenzen. Lebenszyklustabellen geben die Entwicklungsdauer und Mortalität pro Stadium in Abhängigkeit von der Temperatur an. Mit ihrer Hilfe können somit die räumlichen und zeitlichen Vorkommen und Häufigkeiten einer Stechmückenart berechnet werden. Dies ist insbesondere für Stechmücken in Gebieten mit jahreszeitlichen Temperaturveränderungen wichtig. Um Daten für eine solche Lebenszyklustabelle aufnehmen zu können, ist es notwendig Laborexperimente bei festgelegten Temperaturen durchzuführen. Die Voraussetzung dafür ist, dass die Stechmückenart im Labor optimale Bedingungen erhält, um ihren Lebenszyklus abschließen zu können. In dieser Arbeit wurde daher ein Laborprotokoll entwickelt, mithilfe dessen der Lebenszyklus der Asiatischen Buschmücke im Labor untersucht werden kann. Dazu wurden systematisch die Fütterung, die innerartliche Konkurrenz und das Wasservolumen des Brutge-fäßes für die aquatischen Stadien erprobt. Auf Basis dieses Protokolls wurden anschließend die Temperatureinflüsse auf die Entwicklung aller Stadien aufgenommen. Diese Daten dienten der Parametrisierung eines populationsdynamischen Modells. Dieses wurde verwendet, um Standorte mehrjähriger Populationen zu definieren, saisonale Häufigkeiten für Deutschland zu berechnen, durch Temperaturveränderungen hervorgerufene zukünftige Verbreitungsgebiete vorherzusagen, sowie Effekte von Kontrollmaßnahmen auf die Häufigkeit der Asiatischen Buschmücke zu modellieren.
Um eine dauerhafte Kontrolle der Stechmückenvektoren zu gewährleisten, ist weiterhin die permanente Neuentwicklung von wirksamen Kontrollmethoden notwendig. Dazu gehört die präventive Vermeidung von Bruthabitaten der aquatischen Stadien von Stechmücken. Die exotischen Stechmücken, die in Deutschland etabliert sind, gehören mehrheitlich der Gattung Aedes an und sind sogenannte Gefäßbrüter. Ihre bevorzugten Bruthabitate sind kleine Was-seransammlungen wie sie in Baumhöhlen, Gesteinsauswaschungen, Gießkannen, Regentonnen und Blumenuntersetzern vorkommen. In dieser Arbeit wurde untersucht, welche Farben und Volumina von Plastikbechern die Asiatische Buschmücke zur Eiablage bevorzugt, um präferierte Bruthabitate gezielt zu identifizieren und verringern zu können. Auch die Bereitstellung von Insektiziden wird durch in Stechmücken auftretende Insektizidresistenzen erschwert. Insektizide sollen dabei umweltfreundlich, spezifisch für den Zielorganismus und nicht gesundheitsschädlich für den Menschen sein. Weiterhin sind eine gute Anwendbarkeit, geringe Kosten und eine hohe Effizienz wünschenswert. Eine Quelle für potentielle Insektizide sind pflanzliche Stoffe, zum Beispiel ätherische Öle. Diese sind leicht erhältlich, natürlichen Ursprungs und wirksame Vergrämungsmittel gegen stechbereite Stechmückenweibchen. In dieser Arbeit wurde nach einer Literaturrecherche Nelkenöl ausgewählt und als Insektizid gegen Larven der Asiatischen Buschmücke getestet. Dafür wurden die akute toxische Wirkung von Nelkenöl bei drei Temperaturen untersucht und zusätzlich die Wirkung von Nelkenöl auf die Eiablage im Freiland. Nelkenöl zeigte dabei sowohl eine larvizide als auch eine eiablagehemmende Wirkung. Weiterhin wurde Kupfer in Form von kupferhaltigen Euromünzen als Larvizid untersucht. Kupfer ist ein wirksamer Stoff gegen die aquatischen Stadien von Stechmücken. Allerdings wurde der Stoff noch nicht in Form der einfach zu handhabenden, leicht erhältlichen Kupfermünzen getestet. Dazu wurden Vorexperimente durchgeführt, um herauszufinden, wieviel Kupferionen sich aus den Münzen lösen lassen. Anschließend wurde der akut toxische Effekt auf Larven der Asiatischen Buschmücke untersucht.
Ein Integriertes Stechmückenmanagement hat zum Ziel, die lokale Stechmückenpopulation zu kontrollieren, um so Stichen und daraus resultierender Krankheitsübertragung vorzubeugen. Dies erfolgt über die Aufklärung von Betroffenen, der Überwachung der Stechmückenpopulation, dem Testen auf Pathogenbefall und der direkten Kontrolle von Stechmücken. Diese Arbeit leistet einen Beitrag zu den Kenntnissen über die Laborhaltung einer exotischen Stechmückenart, zur Identifizierung von Bruthabitaten, zur zeitlichen und räumlichen Festlegung von Kontrollmaßnahmen und zur Anwendung von Larviziden und eines Vergrämungsmittels. Mit dieser Arbeit wurde die Grundlage eines faktenbasierten Integrativen Stechmückenmanagements für die Asiatische Buschmücke entwickelt, das eventuell auch auf weitere Aedes-Arten übertragbar ist, und als Handlungsempfehlung für politische Entscheidungstragende dienen kann.
Is free speech in danger on university campus? Some preliminary evidence from a most likely case

Although universities play a key role in questions of free speech and political viewpoint diversity, they are often associated with the opposite of a free exchange of ideas: a proliferation of restrictive campus speech codes, violent protests against controversial speakers and even the firing of inconvenient professors. For some observers these trends on university campuses are a clear indicator of the dire future for freedom of speech. Others view these incidents as scandalized singular events and regard campus intolerance as a mere myth. We take an empirical look at some of the claims in the debate and present original survey evidence from a most likely case: the leftist social science studentship at Goethe University Frankfurt. Our results show that taking offense is a common experience and that a sizable number of students are in favor of restricting speech on campus. We also find evidence for conformity pressures on campus and that both the desire to restrict speech and the reluctance to speak openly differ significantly across political ideology. Left-leaning students are less likely to tolerate controversial viewpoints and right-leaning students are more likely to self-censor on politically sensitive issues such as gender, immigration, or sexual and ethnic minorities. Although preliminary, these findings may have implications for the social sciences and academia more broadly.
In dieser Studie haben wir die Modulation von Arachidonsäure (AA)-Stoffwechselwegen während einer Wurminfektionen mit dem Fadenwurm Heligmosomoides polygyrus bakeri (Hpb) als angeborene regulatorische Strategie zur Modulation der Typ-2-Entzündung untersucht. Wir zeigten, dass Hpb in frühen Stadien der Infektion (Tag 7) die Produktion von regulatorischen Prostaglandinen (PGE2 und 6-keto PGF1-α, ein Abbauprodukt von PGI2) und COX-Metaboliten (12-HHT und TXB2) fördert, jedoch die Sekretion von entzündungsfördernden Mediatoren PGD2 und LTs (LTB4, cysLTs) unterdrückt. Die Hpb-gesteuerte Regulierung des AA-Stoffwechsels könnte eine Strategie zur Immunsuppression/ Immunevasion dieses Parasiten darstellen, die darauf abzielt, die vom Wirt ausgelösten Immunantworten des Typs-2 zu unterbinden und sowohl die Infiltration und Rekrutierung von Granulozyten als auch die Schleimproduktion zu begrenzen und auf diese Weise das Abtöten bzw. Ausscheiden der Larven zu verhindern.
Als Schwerpunkt der Arbeit, konnten wir ebenso zeigen, dass ein Larvenextrakt aus Heligmosomoides polygyrus bakeri (HpbE) den AA Stoffwechsel in myeloiden Zellen wie Makrophagen und Granulozyten moduliert, indem die Synthese von 5-LOX in Richtung COX-Metaboliten verschoben wird. Die Behandlung von murinen und humanen Makrophagen mit HpbE induzierte die Synthese von regulatorischen Prostaglandinen (PGE2) und Prostaglandinen, die an der Wundheilung und Blutgerinnung beteiligt sind (12-HHT, TXB2), wohingegen die Produktion von entzündungsfördernden Lipidmediatoren (LTs, PGD2) unterdrückt wurde. Weiter induzierte HpbE in humanen und murinen Makrophagen die Synthese der Typ-2 hemmenden Mediatoren IL-10 und IL-1β und modulierte die Produktion von Zytokinen, die an der Regulierung von M2-Polarisierung und der Typ-2-Entzündung (IL-12, IL-28, IL-27 und TNF-α) in humanen Makrophagen beteiligt sind. Ähnlich zu der HpbE-vermittelten Eicosanoid-Umprogrammierung in Makrophagen, veränderte HpbE den AA-Stoffwechsel humaner Granulozyten und zeigt eine Verschiebung von LOX- in Richtung COX-Metabolismus. Außerdem kann HpbE direkt auf humane Granulozyten wirken und die Chemotaxis von Granulozyten effizienter hemmen als zur Asthmabehandlung verwendete Standardarzneimittel, indem es die Expression von LT synthetisierenden Enzymen (LTA4H und LTC4S) verringert und die Expression von chemotaktischen Rezeptoren (CCR3 und CRTH2) herunterreguliert.
Darüber hinaus, konnten wir die Mechanismen identifizieren, die der HpbE-gesteuerten Eicosanoid-Umprogrammierung in Makrophagen zugrunde liegen. Hpb Produkte induzierten die Aktivierung von p38 MAPK, welche COX und die Transkriptionsfaktoren HIF-1α und NFκβ aktiviert und die Produktion von Prostaglandinen (PGE2 and TXB2) sowie der Typ-2 unterdrückenden Zytokine IL-10 and IL-1β fördert. Der der Induktion des COX-Signalwegs zugrunde liegende Upstream-Mechanismus umfasste mehrere PPRs (TLR2, Dectin-1/2). Diese Rezeptoren waren allerdings nicht an der HpbE-gesteuerten Induktion von IL-10 beteiligt. Die Mechanismen der Modulation des 5-LOX-Signalweges muss noch in zukünftigen Studien weiter erforscht werden.
Das therapeutische Potential von HpbE oder HpbE-behandelten Makrophagen wurde in einem Maus Model mit HDM-induzierter allergischer Atemwegsentzündung in vivo gezeigt. Eine intranasale Behandlung mit HpbE vor HDM-Sensibilisierung und -Provokation führte zu einer Umprogrammierung des AA-Stoffwechsels und verhinderte die Allergie-induzierte Eosinophilie, Zellinfiltration, Atemwegsentzündung und Schleimproduktion. Die Modulation der Typ-2-Entzündung durch HpbE wurde vor allem durch COX-2-Metabolite vermittelt, die von HpbE-stimulierten Makrophagen freigesetzt wurden. Dies zeigte sich insbesondere darin, dass der Transfer von HpbE-stimulierten Wildtyp- aber nicht COX-2-defizienten Makrophagen vor Provokation die Granulozyten Rekrutierung und Typ-2-Entzündung während der HDM-induzierten Allergie in vivo abschwächte.
Mittels eines Maus Models für die allergische Atemwegsentzündung in unterschiedlichen Altersstufen (Neugeboren, Jungtier und Erwachsen) zeigte dieses Forschungsprojekt, dass das Alter der Sensibilisierung eine Schlüsselrolle bei der Produktion von LTs, der Expression von LT-Synthese Enzymen sowie von Faktoren, die zu strukturellen Veränderungen in den Atemwegen führen, spielt. Hier haben wir auch festgestellt, dass der Mechanismus hinter der LT-Produktion und dem Atemwegs-Remodeling im Epithel von ausgewachsenen sensibilisierten Mäusen die Aktivierung der Faktoren sPLA2X, TGM2 und Wnt5a beinhaltet.
Des Weiteren zeigte unsere Studie, dass eine Wechselwirkung zwischen entzündetem Atemwegsepithel und Alveolar-ähnlichen Makrophagen die Synthese von LTs fördern kann. Der vorgeschlagene Mechanismus startet mit der Sekretion von Wnt5a durch das entzündete Atemwegsepithel, welches die Expression von TGM2 in Makrophagen aktiviert und die Produktion von entzündungsfördernden LTs induziert, wodurch die Rolle der Makrophagen in entzündeten Atemwegen bei Erwachsenen weiter unterstützt wird. Die Relevanz der entdeckten Kaskade konnte auch in Geweben von Patienten mit chronischer Rhinosinusitis und Nasenpolypen (CRSwNP) bestätigt werden. Hohe Konzentrationen von LT Enzymen (5-LO, LTC4S LTA4H), sPLA2-X, TGM2 und Wnt5a wurden in humanen Nasenpolyp Geweben beobachtet, und hohe Konzentrationen von CysLTs wurden in Nasenpolyp Sekreten dieser Patienten gemessen. Dies lässt vermuten, dass die Expression von Atemwegs Remodeling-Faktoren, LT-Synthese Enzymen und die LT Synthese steroidresistent sind. Daher könnte diese entzündliche Kaskade ein alternatives therapeutisches Ziel für die Behandlung von Asthma darstellen, speziell bei Patienten mit steroidresistenten Formen von Atemwegsentzündungen.
Basierend auf den möglichen therapeutischen Anwendungen von HpbE haben wir begonnen, an der Charakterisierung der im HpbE vorhandenen immunmodulatorischen Wirkstoffe zu arbeiten. Glutamatdehydrogenase (GDH) und Ferritin wurden als potenzielle immunmodulatorische Komponenten von HpbE identifiziert. Es ist jedoch weitere Arbeit erforderlich, um diese in HpbE vorhandenen Proteine rekombinant herzustellen und den Wirkungsmechanismus im Bezug auf die Typ-2-Entzündung weiter aufzuklären.
A non-fundamentalist return to origin: the new Islamic reformers’ methodology of (re)interpretation

Focusing on some contemporary Islamic reformers’ solutions, in particular, Abolkarim Soroush, Mohsen Kadivar, and Fazlur Rahman, to concrete issues in Muslim societies, this article examines two different methodological strategies of alternative readings of the Sunna: an archeological one and a genealogical one. In the archeological perspective, the holy text has been considered as a repository of answers to all sorts of questions. Through a pathological analysis, this view suggests solutions to correct distortions and looks for new windows seeking an original interpretation of the Qur’an. The genealogical view, on the other hand, puts aside this pathology and instead insists on the idea of the contingency of any interpretation. Regardless of accuracy and validity, according to the genealogical view, all interpretations have addressed temporal and contextual questions. What is important in this perspective is not returning to an original source for finding the exact message of Allah, or correcting previous interpretations, but recurrently referring to an open-ended text in order to explore the futures of the Qur’an.
Baseline presence of NAFLD predicts weight loss after gastric bypass surgery for morbid obesity

Background. Bariatric surgery is a widely used treatment for morbid obesity. Prediction of postoperative weight loss currently relies on prediction models, which mostly overestimate patients’ weight loss. Data about the influence of Non-alcoholic fatty liver disease (NAFLD) on early postoperative weight loss are scarce. Methods. This prospective, single-center cohort study included 143 patients receiving laparoscopic gastric bypass surgery (One Anastomosis-Mini Gastric Bypass (OAGB-MGB) or Roux-en-Y Gastric Bypass (RYGB)). Liver biopsies were acquired at surgery. NAFLD activity score (NAS) assigned patients to “No NAFLD”, “NAFL” or “NASH”. Follow up data were collected at 3, 6 and 12 months. Results. In total, 49.7% of patients had NASH, while 41.3% had NAFL. Compared with the No NAFLD group, NAFL and NASH showed higher body-mass-index (BMI) at follow-up (6 months: 31.0 kg/m2 vs. 36.8 kg/m2 and 36.1 kg/m2, 12 months: 27.0 kg/m2 vs. 34.4 and 32.8 kg/m2) and lower percentage of total body weight loss (%TBWL): (6 months: 27.1% vs. 23.3% and 24.4%; 12 months: 38.5% vs. 30.1 and 32.6%). Linear regression of NAS points significantly predicts percentage of excessive weight loss (%EWL) after 6 months (Cologne-weight-loss-prediction-score). Conclusions. Histopathological presence of NAFLD might lead to inferior postoperative weight reduction after gastric bypass surgery. The mechanisms underlying this observation should be further studied.
O artigo trata da “Filosofia da Nova Música” de Theodor Adorno e tem como problema de investigação saber quais são os valores estéticos apresentados pelo autor para identificar Schoenberg como o representante do progresso musical. Como objetivos específicos para esse trabalho foram definidos: a identificação das principais características estéticas nas obras musicais de Stravinsky e de Schoenberg e apontar alguns fundamentos filosóficos para diferenciar progresso e regressão na estética musical de Adorno. Ao caracterizar sua concepção estética como filosofia da arte, Adorno toma a produção e a recepção como expressões de uma relação dialética com o meio social e tece críticas especialmente a indústria cultural e a arte burguesa que tinham como propósito agradar o ouvido e permitir que os produtores e receptores estabelecessem uma relação de troca no mercado capitalista. Por isso, sua estética está assentada na crítica e na possibilidade de criação dissonante e autônoma, análises que foram realizadas no âmbito desta investigação que tem como foco a questão da música enquanto objeto estético de progresso ou regressão da audição.
Background: Previously, we used inhibitors blocking BET bromodomain binding proteins (BRDs) in Ewing sarcoma (EwS) and observed that long term treatment resulted in the development of resistance. Here, we analyze the possible interaction of BRD4 with cyclin-dependent kinase (CDK) 9. Methods: Co-immunoprecipitation experiments (CoIP) to characterize BRD4 interaction and functional consequences of inhibiting transcriptional elongation were assessed using drugs targeting of BRD4 or CDK9, either alone or in combination. Results: CoIP revealed an interaction of BRD4 with EWS-FLI1 and CDK9 in EwS. Treatment of EwS cells with CDKI-73, a specific CDK9 inhibitor (CDK9i), induced a rapid downregulation of EWS-FLI1 expression and block of contact-dependent growth. CDKI-73 induced apoptosis in EwS, as depicted by cleavage of Caspase 7 (CASP7), PARP and increased CASP3 activity, similar to JQ1. Microarray analysis following CDKI-73 treatment uncovered a transcriptional program that was only partially comparable to BRD inhibition. Strikingly, combined treatment of EwS with BRD- and CDK9-inhibitors re-sensitized cells, and was overall more effective than individual drugs not only in vitro but also in a preclinical mouse model in vivo. Conclusion: Treatment with BRD inhibitors in combination with CDK9i offers a new treatment option that significantly blocks the pathognomonic EWS-ETS transcriptional program and malignant phenotype of EwS.
The complexity of atmospherical processes has always yielded a multitude of ways of knowing about the weather. What has been lacking in the historiography of meteorology so far is a way to formulate differences between forms of knowledge in a way that does not privilege modern scientific structures, but focuses instead on the epistemological category of causality. Using causality as ground of comparison for different knowledge claims, I shall argue, may enable researchers to investigate meteorological knowledge across time periods, perhaps even geographical regions, in a more symmetrical manner. This review demonstrates this approach as a means to organize a large set of historical meteorological writings from German countries between 1750 and 1850. Three distinct forms of knowledge (Semiotics, Physics, and Organics of the weather) during that time and in that region are suggested and will be described. While a bibliography with a national perspective from the 1880s was the basis for the selection of historical sources, such a setup proved awkward even to contemporaries. In addition, the bibliography came with a number of biases and shortcomings that will be critically reviewed.
Spatial and temporal processes shaping microbial communities are inseparably linked but rarely studied together. By Illumina 16S rRNA sequencing, we monitored soil bacteria in 360 stations on a 100 square meter plot distributed across six intra-annual samplings in a rarely managed, temperate grassland. Using a multi-tiered approach, we tested the extent to which stochastic or deterministic processes influenced the composition of local communities. A combination of phylogenetic turnover analysis and null modeling demonstrated that either homogenization by unlimited stochastic dispersal or scenarios, in which neither stochastic processes nor deterministic forces dominated, explained local assembly processes. Thus, the majority of all sampled communities (82%) was rather homogeneous with no significant changes in abundance-weighted composition. However, we detected strong and uniform taxonomic shifts within just nine samples in early summer. Thus, community snapshots sampled from single points in time or space do not necessarily reflect a representative community state. The potential for change despite the overall homogeneity was further demonstrated when the focus shifted to the rare biosphere. Rare OTU turnover, rather than nestedness, characterized abundance-independent β-diversity. Accordingly, boosted generalized additive models encompassing spatial, temporal and environmental variables revealed strong and highly diverse effects of space on OTU abundance, even within the same genus. This pure spatial effect increased with decreasing OTU abundance and frequency, whereas soil moisture – the most important environmental variable – had an opposite effect by impacting abundant OTUs more than the rare ones. These results indicate that – despite considerable oscillation in space and time – the abundant and resident OTUs provide a community backbone that supports much higher β-diversity of a dynamic rare biosphere. Our findings reveal complex interactions among space, time, and environmental filters within bacterial communities in a long-established temperate grassland.
Maintaining biodiversity and ecosystem function is critical on national and global scales. However, while only a fraction of the global biodiversity is known, its current decline is unprecedented, making biodiversity hotspots a conservation priority. The Sierra Gorda Biodiversity Reserve (SGBR) in Central Mexico is known for its rich biodiversity. It is an example of the juxtaposition between species discovery and extinction: aquatic species richness is mostly unknown as no efforts have investigated aquatic communities so far, but are already anthropogenically stressed. We hypothesized that invasive species are already well established in various protected areas and investigated this by assessing the threat of invasive species that are already established within the SGBR on the native biodiversity. By combining field sampling with peer-reviewed literature and local reports, we identify the presence of various non-native species in SGBR. Among these non-native species identified were opportunistic predatory fish and potentially-pathogen transmitting molluscs, but also, a habitat engineer capable of modifying ecosystem functions. Moreover, we highlight that these species were introduced despite legislation and without any knowledge among authorities. As a result, we underline the necessity to describe native species, control invasive and prevent the introduction of further non-native species. If accelerated action is not taken, we risk losing a considerable amount of described and unknown freshwater biota. Neuronal nitric oxide synthase (NOS-I) impacts on fear/anxiety-like behavior in animals. In humans, the short (S) allele of a functional promotor polymorphism of NOS1 (NOS1 ex1f-VNTR) has been shown to be associated with higher anxiety and altered fear conditioning in healthy subjects in the amygdala and hippocampus (AMY/HIPP). Here, we explore the role of NOS1 ex1f-VNTR as a pathophysiological correlate of panic disorder and agoraphobia (PD/AG). In a sub-sample of a multicenter cognitive behavioral therapy (CBT) randomized controlled trial in patients with PD/AG (n = 48: S/S-genotype n=15, S/L-genotype n=21, L/L-genotype n=12) and healthy control subjects, HS (n = 34: S/S-genotype n=7, S/L-genotype n=17, L/L-genotype=10), a differential fear conditioning and extinction fMRI-paradigm was used to investigate how NOS1 ex1f-VNTR genotypes are associated with differential neural activation in AMY/HIPP. Prior to CBT, L/L-allele carriers showed higher activation than S/S-allele carriers in AMY/HIPP. A genotype × diagnosis interaction revealed that the S-allele in HS was associated with a pronounced deactivation in AMY/HIPP, while patients showed contrary effects. The interaction of genotype × stimulus type (CS+, conditioned stimulus associated with an aversive stimulus vs. CS-, unassociated) showed effects on differential learning in AMY/HIPP. All effects were predominately found during extinction. Genotype associated effects in patients were not altered after CBT. Low statistical power due to small sample size in each subgroup is a major limitation. However, our findings provide first preliminary evidence for dysfunctional neural fear conditioning/extinction associated with NOS1 ex1f-VNTR genotype in the context of PD/AG, shedding new light on the complex interaction between genetic risk, current psychopathology and treatment-related effects.
Das Gehirn weist in mehreren Bereichen anatomische Asymmetrien zwischen beiden Hemisphären auf, so auch in Bereichen der Hörrinde. Zudem ist bereits langjährig bekannt, dass menschliche Sprache vorrangig in der linken Gehirnhälfte, d.h. linksseitig lateralisiert, verarbeitet wird. Daraus folgend stellt sich die Frage, ob dies eine besondere Spezialisierung ist, oder ob es noch weitere lateralisierte Hirnfunktionen gibt. Viele akustische Signale haben dabei frequenzmodulierte (FM) Komponenten, die im Hörsystem für die Erkennung nach Parametern wie Richtung und Dauer der Modulation analysiert werden müssen. Ob die Analyse von FM-Komponenten oder einzelner Reizparameter im Gehirn lateralisiert stattfindet, wurde in der Literatur meist mit bildgebenden Verfahren untersucht.
Für das Erkennen und Unterscheiden der Modulationsrichtung weist eine Vielzahl von Studien auf eine erhöhte Aktivität in der rechten Hörrinde hin. Für die Analyse von Stimulusdauern ist es bisher allerdings noch unklar bzw. umstritten, ob diese lateralisiert erfolgt. Für die Untersuchung der Lateralisierung einfacher Sprachkomponenten werden häufig Konsonant-Vokal-Silben (CV-Silben) verwendet. In einer Vielzahl von Studien konnte eine linkslastige Lateralisierung, wie bei der Spracherkennung, gezeigt werden.
In der vorliegenden Arbeit wurde nun untersucht, ob ein eindeutigeres Muster von Lateralisierung zu finden ist, wenn diese in Wahrnehmungsexperimenten, untersucht wird. Dabei wurde ein zu untersuchender Teststimulus (FM-/CV-Stimulus) auf einem Ohr mit einem kontralateralen breitbandigen Rauschen auf dem anderen Ohr gleichzeitig präsentiert. Durch die Struktur der Hörbahn kann dabei davon ausgegangen werden, dass in einer Hemisphäre des Vorderhirns vorrangig Informationen aus dem kontralateralen Ohr verarbeitet und Informationen aus dem ipsilateralen Ohr unterdrückt werden und sich somit Rückschlüsse auf die Funktion/Beteiligung einer Hemishpäre ziehen lassen. Das Rauschen diente dabei zur unspezifischen Aktivierung der gegenüberliegenden Hemisphäre.
Die Lateralisierung wurde systematisch für unterschiedlich komplexe Reize untersucht. Dazu wurden in zwei Versuchsreihen Unterscheidungsexperimente durchgeführt, die sich in mehrere Messungen (mit mehreren Durchläufen) mit unterschiedlichen Parametereinstellungen gliederten. Pro Durchlauf musste sich die Versuchsperson immer zwischen zwei Antwortmöglichkeiten entscheiden (2-AFC-Verfahren). Der Schalldruckpegel des Rauschens war dabei für alle Messungen konstant. Der Schalldruckpegel der Teststimuli blieb zwar während einer Messung konstant, wurde jedoch innerhalb eines Experimentes von Messung zu Messung reduziert.
In einer gemeinsamen Analyse wurden jeweils die Fehlerraten und Reaktionszeiten beider Ohren, getrennt nach Seite und FM-/ CV-Stimulus, miteinander verglichen, um so auf eine mögliche Lateralisierung schließen zu können. Damit die Daten der Versuchspersonen bei vergleichbarer Schwierigkeit analysiert werden konnten, wurde als Vergleichswert zwischen allen Versuchspersonen der Schalldruckpegel der ersten Messung mit einer Fehlerrate von mindestens 15,0 % gewählt (15 %-Kriterium). Um auszuschließen, dass das Hörvermögen der Versuchspersonen Unterschiede zwischen beiden Ohren aufweist, wurde vor jeder Messung der „Punkt subjektiver Gleichheit“ für die Lautstärke-wahrnehmung zwischen linkem und rechten Ohr bestimmt.
In der ersten Versuchsreihe wurde dabei die Verarbeitung der Modulationsrichtung und der Stimulusdauer von FM-Stimuli untersucht. Es zeigte sich für beide Experimente, dass ein sinkender Schalldruckpegel des FM-Stimulus zu einer steigenden Fehlerrate führte. Unter Anwendung des 15 %-Kriteriums waren die Fehlerraten für die Unterscheidung der Modulationsrichtung signifikant geringer, wenn der FM-Stimulus auf dem linken Ohr präsentiert wurde. Dies ist ein deutlicher Hinweis für eine rechtslastige Lateralisierung.
Für die Unterscheidung der Stimulusdauer gab es dagegen keinen signifikanten Unterschied zwischen den Fehlerraten beider Ohren. Somit muss davon ausgegangen werden, dass beide Hemisphären für diese Aufgabe benötigt werden und eine bilaterale Verarbeitung stattfindet. In den Reaktionszeiten konnten in beiden Experimente keine signifikanten Unterschiede gezeigt werden. Die Unterscheidung der Modulationsrichtung wurde dabei von allen Versuchspersonen als einfacher eingestuft als die Unterscheidung der Stimulusdauer, was sich auch in niedrigeren Antwortschnelligkeit und Fehlerraten bei vergleichbaren Schalldruckpegeln zeigte.
In der zweiten Versuchsreihe wurde als Referenzmessung nochmals die Unterscheidung der Modulationsrichtungen von FM-Stimuli durchgeführt. Anschließend wurde die Unterscheidung von „da“ und „ga“ untersucht. Diese CV-Silben differieren ausschließlich in der FM-Komponente. Die Untercheidung von CV-Silben ohne Unterschied in der FM-Komponente wurde mittels „ta“ und „ka“ getestet. Für alle drei Experimente zeigte sich, dass ein geringerer Schalldruckpegel des FM- oder CV-Stimulus zu einer steigenden Fehlerrate führte. Unter Anwendung des 15 %-Kriteriums zeigte sich für die Unterscheidung der Modulationsrichtung ein Trend zu niedrigeren Fehlerraten bei der Präsentation des FM-Stimulus auf dem linken im Vergleich mit dem rechten Ohr. In den Reaktionszeiten konnten keine signifikanten Unterschiede gezeigt werden.
Für die Unterscheidung von „da“ und „ga“ ließ sich unter Anwendung des 15 %-Kriteriums in den Fehlerraten und Reaktionszeiten kein Vorteil eines Ohres nachweisen. Dagegen zeigten sich klare Unterschiede bei einzelnen Versuchspersonen. So waren die Fehlerraten für Versuchspersonen, die vorwiegend „da“ erkannt bzw. gehört hatten signifikant höher, wenn der CV-Stimulus auf dem rechten Ohr präsentiert wurde, für „ga“-Hörer war das Gegenteil der Fall. In den Reaktionszeiten konnte kein signifikanter Zusammenhang nachgewiesen werden. Somit ließ sich zeigen, dass je nach Strategie der Versuchsperson bzw. deren individueller Wahrnehmung der CV-Silben, Unterschiede in der Lateralisierung erreicht werden können.
Für die Unterscheidung von „ta“ und „ka“ zeigten sich unter Anwendung des 15 %-Kriteriums signifikant niedrigere Fehlerraten und Reaktionszeiten, wenn der CV-Stimulus auf dem linken Ohr präsentiert wurde. Dies weist deutlich auf eine rechtslastige Lateralisierung hin. Vergleicht man alle drei Experimente ließ sich zudem zeigen, dass die Unterscheidung der Modulationsrichtung einfacher war als die Unterscheidung verschiedener CV-Stimuli. Dabei war die Unterscheidung von „da“ und „ga“ für die Versuchspersonen schwieriger als die Unterscheidung von „ta“ und „ka“. Allerdings konnte in den Lateralisierungsdaten kein direkter Zusammenhang zwischen den FM- und „da“-/„ga“-Stimuli gezeigt werden.
Zusammenfassend konnte in allen fünf Experimenten eine verschieden stark lateralisierte Verarbeitung von akustischen Stimuli bei gleichzeitigem kontralateralen Rauschen gezeigt werden. Der Vorteil eines Ohres (bzw. einer Hemisphäre) war sowohl von der Aufgabe als auch vom Stimulustyp abhängig. Dabei gab es zum Teil starke Unterschiede in der Effektstärke und dem Grad der Lateralisierung zwischen den einzelnen Versuchspersonen. Insgesamt konnte gezeigt werden, dass sich die hier angewendete psychophysische Methode gut eignet, um Ergebnisse zur Lateralisierung von akustischen Stimuli zu gewinnen und somit die Verhaltensrelevanz von Ergebnissen aus Studien mit bildgebenden Verfahren zu überprüfen.
Previous studies in developing Xenopus and zebrafish reported that the phosphate transporter slc20a1a is expressed in pronephric kidneys. The recent identification of SLC20A1 as a monoallelic candidate gene for cloacal exstrophy further suggests its involvement in the urinary tract and urorectal development. However, little is known of the functional role of SLC20A1 in urinary tract development. Here, we investigated this using morpholino oligonucleotide knockdown of the zebrafish ortholog slc20a1a. This caused kidney cysts and malformations of the cloaca. Moreover, in morphants we demonstrated dysfunctional voiding and hindgut opening defects mimicking imperforate anus in human cloacal exstrophy. Furthermore, we performed immunohistochemistry of an unaffected 6-week-old human embryo and detected SLC20A1 in the urinary tract and the abdominal midline, structures implicated in the pathogenesis of cloacal exstrophy. Additionally, we resequenced SLC20A1 in 690 individuals with bladder exstrophy-epispadias complex (BEEC) including 84 individuals with cloacal exstrophy. We identified two additional monoallelic de novo variants. One was identified in a case-parent trio with classic bladder exstrophy, and one additional novel de novo variant was detected in an affected mother who transmitted this variant to her affected son. To study the potential cellular impact of SLC20A1 variants, we expressed them in HEK293 cells. Here, phosphate transport was not compromised, suggesting that it is not a disease mechanism. However, there was a tendency for lower levels of cleaved caspase-3, perhaps implicating apoptosis pathways in the disease. Our results suggest SLC20A1 is involved in urinary tract and urorectal development and implicate SLC20A1 as a disease-gene for BEEC.
Hypertonie stellt in der westlichen Welt die Haupttodesursache dar, obwohl sie im Hinblick auf die pharmakologischen Therapieoptionen gut behandelbar ist. Hauptursächlich ist hierfür eine, vor allem durch eine Non-Adhärenz (u.a. bedingt durch den asymptomatischen Charakter) und in selteneren Fällen eine aufgrund einer therapieresistente Hypertonie (TRH) verursachte, unzureichende Blutdruckkontrolle. Eine Möglichkeit zur Überprüfung der Therapietreue in der antihypertensiven Therapie ist der qualitative Nachweis der Arzneistoffe im Blut oder Urin. Unklar ist, inwiefern die Substanzen in einer biologischen Probe innerhalb des Dosierungsintervalls oder darüber hinaus nachweisbar sind und es zu einer Falschbeurteilung kommen kann.
Daher wurde eine quantitative chromatographisch-tandem-massenspektrometrische Methode für das Therapeutische Drug Monitoring von blutdrucksenkenden Arzneistoffen entwickelt und analytisch vollständig validiert. Bei 38 Patienten mit überwachter Medikamenteneinnahme wurde die Aussagekraft der Methode hinsichtlich der Bestätigung einer Adhärenz zunächst mittels zweier Wirkstoffkonzentrationen im Serum (Tal- und Spitzenspiegel) überprüft. Zur Bewertung der Konzentrationen wurden zwei Konzepte evaluiert. Einerseits wurde die untere Grenze des therapeutischen Referenzbereiches (TRR, Literaturdaten) und andererseits die mittels Rechenmodell (Daten pharmakokinetischer Studien) individuell ermittelte, dosisbezogene Arzneimittelkonzentration (DRC) evaluiert. In einem zweiten Studienansatz wurde diese neue quantitative Methode an einem Kollektiv von 36 ambulanten Patienten (ohne überwachte Medikamenteneinnahme) angewendet und zur Überprüfung der Aussagekraft mit Ergebnissen des etablierten Urinscreenings verglichen.
Die gemessenen Wirkstoffkonzentrationen von Atenolol (64 bis 564 ng/ml), Bisoprolol (2,5 bis 53 ng/ml), Metoprolol (5,8 bis 110 ng/ml), Nebivolol (0,32 bis 3,4 ng/ml), Hydrochlorothiazid (15 bis 606 ng/ml), Furosemid (22 ng/ml), Torasemid (17 bis 1829 ng/ml), Canrenon (25 bis 221 ng/ml), Amlodipin (2,4 bis 35 ng/ml), Lercanidipin (0,24 bis 21 ng/ml), Candesartan (6,0 bis 268 ng/ml), Telmisartan (22 bis 375 ng/ml) und Valsartan (115 bis 7962 ng/ml) haben gezeigt, dass die quantitative Analyse von Antihypertensiva in Serumproben und deren Auswertung auf Basis der individuell berechneten unteren DRC in der Beurteilung einer Adhärenz vielversprechend ist.
Die Auswertung auf Basis der unteren Grenze des TRR signalisierte bei den stationären Patienten (überwachte Einnahme) innerhalb der Substanzklasse der Diuretika (ohne Torasemid) bei 16,7 %, der β-Blocker bei 29,4 %, der Calciumkanal-Blocker bei 14,8 % und der AT1-Antagonisten bei 25 % fälschlicherweise eine Non-Adhärenz.
Die rein qualitative Urinanalyse zeigte im Falle der β-Blocker Atenolol, Bisoprolol und des Diuretikums HCT aufgrund einer hohen Bioverfügbarkeit, einer langen Halbwertszeit oder einer überwiegend renalen Ausscheidung der Muttersubstanz ein Nachweisfenster über das Dosierungsintervall hinaus, was bedingt, dass einige Patienten fälschlicherweise als adhärent gewertet wurden. Ein anderes Problem zeigte sich bei einigen Patienten, die mit dem AT1-Antagonist Candesartan oder dem Calciumkanal-Blocker Lercanidipin behandelt wurden und die als non-adhärent eingestuft wurden. Eine geringe Bioverfügbarkeit, eine hohe Metabolisierungsrate oder geringe renale Ausscheidung der unveränderten Arzneistoffe lies auf eine mangelhafte Nachweisbarkeit innerhalb des Dosierungsintervalls und damit auf eine eingeschränkte Beurteilbarkeit schließen.
Aus den Untersuchungen ergibt sich, dass es bei Anwendung qualitativer Nachweismethoden aufgrund besonderer Pharmakokinetiken einzelner antihypertensiver Wirkstoffe zur Fehleinschätzung der Adhärenz kommen kann. Die neu entwickelte Methodik in Form einer quantitativen Serumanalyse ist unter Verwendung patientenindividueller Bewertungskriterien bei dieser Fragestellung überlegen.
With obesity having doubled in the last decade, hypertension is on the rise. In one-third of hypertensive patients the metabolic syndrome is present. This might be one factor for the increasing number of prescriptions for angiotensin receptor blockers and calcium-channel blockers besides a more favorable risk-to-benefit ratio. The aim of the present study was to evaluate a therapeutic drug monitoring (TDM) method for assessment of adherence based on cut-offs in inpatients and to compare it to an established urine drug screening in outpatients. A method for quantification of calcium-channel blockers and angiotensin receptor blockers using high-performance liquid chromatography-tandem mass spectrometric analysis (LC-MS/MS) was developed and validated. The method was applied to serum samples of 32 patients under supervised medication to establish cut-off values for adherence assessment based on dose-related concentrations (DRC, calculated from pharmacokinetic data). Furthermore, corresponding urine and blood samples of 42 outpatients without supervised medication were analysed and the results compared with regard to adherence assessment. All serum concentrations measured for amlodipine (n = 40), lercanidipine (n = 14), candesartan (n = 10), telmisartan (n = 4) and valsartan (n = 10) in inpatients were above the patient specific lower DRC confirming adherence. Of 42 outpatients the identification of analytes in urine as well as the quantification in serum exhibited differing results. According to urinalysis, adherence was demonstrated in only 87.0% of prescriptions, compared to 91.3% for serum analyses. Differences were observed for amlodipine, lercanidipine and candesartan which can be explained by a higher specificity of the serum analysis approach due to pharmacokinetics. The present study confirms that assessing adherence based on serum drug concentrations with individually calculated lower DRCs is more accurate than using qualitative urine analysis. In particular, drugs with low bioavailability, low renal excretion or high metabolism rate such as lercanidipine and candesartan may lead to underestimation of adherence via urine analysis.
The coronavirus disease 2019 COVID-19 pandemic is rapidly spreading worldwide and is becoming a major public health crisis. Increasing evidence demonstrates a strong correlation between obesity and the COVID-19 disease. We have summarized recent studies and addressed the impact of obesity on COVID-19 in terms of hospitalization, severity, mortality, and patient outcome. We discuss the potential molecular mechanisms whereby obesity contributes to the pathogenesis of COVID-19. In addition to obesity-related deregulated immune response, chronic inflammation, endothelium imbalance, metabolic dysfunction, and its associated comorbidities, dysfunctional mesenchymal stem cells/adipose-derived mesenchymal stem cells may also play crucial roles in fueling systemic inflammation contributing to the cytokine storm and promoting pulmonary fibrosis causing lung functional failure, characteristic of severe COVID-19. Moreover, obesity may also compromise motile cilia on airway epithelial cells and impair functioning of the mucociliary escalators, reducing the clearance of severe acute respiratory syndrome coronavirus (SARS-CoV-2). Obese diseased adipose tissues overexpress the receptors and proteases for the SARS-CoV-2 entry, implicating its possible roles as virus reservoir and accelerator reinforcing violent systemic inflammation and immune response. Finally, anti-inflammatory cytokines like anti-interleukin 6 and administration of mesenchymal stromal/stem cells may serve as potential immune modulatory therapies for supportively combating COVID-19. Obesity is conversely related to the development of COVID-19 through numerous molecular mechanisms and individuals with obesity belong to the COVID-19-susceptible population requiring more protective measures.
Human placentation is a highly invasive process. Deficiency in the invasiveness of trophoblasts is associated with a spectrum of gestational diseases, such as preeclampsia (PE). The oncogene B-cell lymphoma 6 (BCL6) is involved in the migration and invasion of various malignant cells. Intriguingly, its expression is deregulated in preeclamptic placentas. We have reported that BCL6 is required for the proliferation, survival, fusion, and syncytialization of trophoblasts. In the present work, we show that the inhibition of BCL6, either by its gene silencing or by using specific small molecule inhibitors, impairs the migration and invasion of trophoblastic cells, by reducing cell adhesion and compromising the dynamics of the actin cytoskeleton. Moreover, the suppression of BCL6 weakens the signals of the phosphorylated focal adhesion kinase, Akt/protein kinase B, and extracellular regulated kinase 1/2, accompanied by more stationary, but less migratory, cells. Interestingly, transcriptomic analyses reveal that a small interfering RNA-induced reduction of BCL6 decreases the levels of numerous genes, such as p21 activated kinase 1, myosin light chain kinase, and gamma actin related to cell adhesion, actin dynamics, and cell migration. These data suggest BCL6 as a crucial player in the migration and invasion of trophoblasts in the early stages of placental development through the regulation of various genes associated with the migratory machinery.
Background: Body dysmorphic disorder (BDD) is characterized by an excessive preoccupation with one or more perceived flaws in one’s own appearance. Previous studies provided evidence for deficits in configural and holistic processing in BDD. Preliminary evidence suggests abnormalities at an early stage of visual processing. The present study is the first examining early neurocognitive perception of the own face in BDD by using electroencephalography (EEG). We investigated the face inversion effect, in which inverted (upside-down) faces are disproportionately poorly processed compared to upright faces. This effect reflects a disruption of configural and holistic processing, and in consequence a preponderance of featural face processing.
Methods: We recorded face-sensitive event-related potentials (ERPs) in 16 BDD patients and 16 healthy controls, all unmedicated. Participants viewed upright and inverted (upside-down) images of their own face and an unfamiliar other face, each in two facial emotional expressions (neutral vs. smiling). We calculated the early ERP components P100, N170, P200, N250, and the late positive component (LPC), and compared amplitudes among both groups.
Results: In the early P100, no face inversion effects were found in both groups. In the N170, both groups exhibited the common face inversion effects, with significantly larger N170 amplitudes for inverted than upright faces. In the P200, both groups exhibited larger inversion effects to other (relative to own) faces, with larger P200 amplitudes for other upright than inverted faces. In the N250, no significant group differences were found in face processing. In the LPC, both groups exhibited larger inversion effects to other (relative to own) faces, with larger LPC amplitudes for other inverted than upright faces. These overall patterns appeared to be comparable for both groups. Smaller inversion effects to own (relative to other) faces were observed in none of these components in BDD, relative to controls.
Conclusions: The findings suggest no evidence for abnormalities at all levels of early face processing in our observed sample of BDD patients. Further research should investigate the neural substrates underlying BDD symptomatology.
Movies are brilliant choices to be subjects of discourse analysis since they bear resemblance to real-life phenomena. Lion of The Desert is one of the movies that actualizes the use of the English language as the dialogue and presents Islamic historical values as its content. Among a myriad of sub-disciplines of discourse analysis, this paper attempts to investigate speech act phenomena in the utterances of Omar Mukhtar, the main character of the movie. The discourse analysis is conducted on his utterances in order to extract the types of speech acts he employs. Primary data sources include the movie video file and its script. We execute several procedural steps of extracting the data, commencing with watching the movie while reading its script; re-watching it to identify the aspects like voice, intonation, and mimics; interpreting, and classifying the types of speech acts in accordance with the classification procedure of John R. Searle’s speech act theory. The findings revealed four types of speech acts, namely, representative, directive, commissive, and expressive, being identified and classified in Mukhtar’s utterances. The most frequently used type of speech act was representative, which is performed in 56 utterances, followed by a directive act which appears in 53 utterances. Commissive and expressive speech acts emerge in 9 and 7 utterances respectively.

Jahrhunderts. Gemäß der biedermeierlich gestimmten, belehrenden Funktion des Textes steht kindliche Allmachtsfantasie im Mittelpunkt des Geschehens. Die Haltung des ›Mehr-mehr‹ überschreitet indes die Grenzen der Moralerzählung. Entgegen der abschreckenden Funktion scheint vielmehr der kleine Häwelmann in der Verschränkung von Norm-Übertritt und Eskapismus ein ›modernes‹ Kind seiner Entstehungszeit zu sein und durchaus mit den Figuren des Struwwelpeters vergleichbar, die der Arzt und Kinderpsychiater Heinrich Hoffmann 1845 entworfen hat...
The role of radiation therapy in the treatment of (colo)-rectal cancer has changed dramatically over the past decades. Introduced with the aim of reducing the high rates of local recurrences after conventional surgery, major developments in imaging, surgical technique, systemic therapy and radiation delivery have now created a much more complex environment leading to a more personalized approach. Functional aspects including reduction of acute or late treatment-related side effects, sphincter or even organ-preservation and the unsolved problem of still high distant failure rates have become more important while local recurrence rates can be kept low in the vast majority of patients. This review summarizes the actual role of radiation therapy in different subgroups of patients with rectal cancer, including the current standard approach in different subgroups as well as recent developments focusing on neoadjuvant treatment intensification and/or non-operative treatment approaches aiming at organ-preservation.
Background: About 2000 children and adolescents under the age of 18 are diagnosed with cancer each year in Germany. Because of current medical treatment methods, a high survival rate can be reached for many types of the disease. Nevertheless, patients face a number of long-term effects related to the treatment. As a result, physical and psychological consequences have increasingly become the focus of research in recent years. Social dimensions of health have received little attention in health services research in oncology so far. Yet, there are no robust results that allow an estimation of whether and to what extent the disease and treatment impair the participation of children and adolescents and which factors mediate this effect. Social participation is of great importance especially because interactions with peers and experiences in different areas of life are essential for the development of children and adolescents.
Methods: Data are collected in a longitudinal, prospective, observational multicenter study. For this purpose, all patients and their parents who are being treated for cancer in one of the participating clinics throughout Germany will be interviewed within the first month after diagnosis (t1), after completion of intensive treatment (t2) and half a year after the end of intensive treatment (t3) using standardized questionnaires. Analysis will be done by descriptive and multivariate methods.
Discussion: The results can be used to identify children and adolescents in high-risk situations at an early stage in order to be able to initiate interventions tailored to the needs. Such tailored interventions will finally reduce the risk of impairments in the participation of children and adolescents and increase quality of life.
