Tinnitus is a symptom experienced by most people at least once in their lifetime. In most documented cases, a new onset of chronic tinnitus can be chronologically correlated with hearing loss. However, tinnitus can also occur in people with (apparently) normal hearing and remains without a traceable preceding cause. Despite the frequency of occurrence of tinnitus, the pathophysiological mechanisms are still not fully understood. A currently proposed hypothesis focuses on a "hidden" hearing loss called synaptopathy as a pathomechanism of tinnitus in normal hearing subjects. In the present study, the objective was to test whether finestructure audiometry or measurement of otoacoustic emissions can reveal possibly overlooked hearing impairment in presumed normalhearing individuals with chronic tinnitus. Thus, a hearing loss not audiologically detectable by the usual methods would supplement or replace the presumed synaptopathic pathomechanism. Another objective was to attempt to replicate the existing findings of another research group on synaptopathy as cause for tinnitus in normal hearing people. Schaette and McAlpine (2011) were able to demonstrate a significant difference in wave I amplitudes between groups of normal hearing subjects with and without chronic tinnitus by deriving clickevoked auditory brainstem potentials, thus supporting the hypothesis of synaptopathy18.
For the present study, a cohort of normal-hearing subjects consisting of a group of tinnitus subjects (N = 15) and a control group (N = 14) was tested. Manual puretone audiometry with 11 test frequencies was conducted to determine hearing performance. Inclusion criteria were defined as air conducted hearing thresholds of 10 dB HL or lower. A deviation at a test frequency of 15 dB HL or less was tolerated. Data of tinnitus characteristics, such as pitch and intensity, were collected by presentation and matching of comparative tones, quality and subjective disturbance by questionnaire. Furthermore, data was obtained from both test groups by Békésy gliding frequency audiometry (794 test frequencies), as well as DPOAE measurement (36 test frequencies) and auditory brainstem response (ABR) audiometry (derivation of early auditory evoked potentials). The results showed a correlation of the determined tinnitus comparison pitch with the frequency location of the largest deviation (impairment) from the normal hearing curve in the Békésy gliding frequency audiometry (p = 0.032). All further analyses of the finestructure hearing curve (steepness of hearing loss, slope, number of hearing loss dips) showed no statistically significant relationship between the morphology of the fine-structure hearing curve and tinnitus characteristics. Finestructure measurement revealed areas of hearing loss that were not mapped in manual puretone audiometry. These "undetected" hearing losses would have led to the exclusion of 12 of 29 subjects (41.4 %) if the finestructure hearing curve had been used as an inclusion criterion. A direct comparison of the mean finestructure hearing curves of both test groups showed a statistically significant better mean hearing performance of the tinnitus group (p < 0.05) in 3 different test frequency ranges (1.5 kHz, 3 kHz, 7 kHz) with a maximum of 4 dB HL. Analy-sis of the mean amplitudes of wave I of the ABRs showed, contrary to expectation, a weak trend toward higher amplitudes in the tinnitus group (p = 0.06). According to Schaette and McAlpine (2011), synaptopathy pathogenesis should have resulted in an opposite trend, i.e., a decrease in wave I amplitude in the tinnitus group. As a secondary finding, a weak trend between wave I amplitude and subjectively perceived disturbance of tinnitus was demonstrated (p = 0.06). Statistical analysis of the parameters determined from the DPOAE measurements did not reveal any significant differences between the tinnitus group and control group. Direct comparison of the DPOAE and finestructure hearing curves, revealed a significant difference in the differences of the frequencyspecific measurements around 2.4 kHz (p = 0.007).
The results of the study suggest that in previous studies with supposedly normal hearing tinnitus subjects there were unrecognized hearing losses that either went unrecognized by the screening by manual puretone audiometry, or subjects with previously aboveaverage hearing experienced a subtle spontaneous decrease in their hearing as tinnitus pathogenesis. This assumption is also supported by the fact that there is a significant correlation between the frequency range of the greatest hearing loss in the finestructure hearing curves and the tinnitus frequency.
The suspected pathomechanism of synaptopathy in "normal hearing" subjects with tinnitus could not be confirmed. The correlation between wave I amplitudes and subjectively perceived disturbance by tinnitus, indicated by the data of this study, should be investigated in more detail in future studies. Further research with more accurate measurement methods and larger subject groups is needed to clarify the hypothesis "Genesis of chronic subjective tinnitus without hearing loss".

• Placement of tissue expander is essential in planning the reconstruction.
• MRI-compatibility of the tissue expander is paramount for shortening the total duration of anesthesia.
• Role of routine skin biopsies to identify PD patients at additional risk for developing scalp defects should be investigated.
Nitric oxide (NO) plays an important role in the regulation of the functional integrity of the endothelium. The intracellular reaction of NO with reactive cysteine groups leads to the formation of S-nitrosothiols. To investigate the regulation of S-nitrosothiols in endothelial cells, we first analyzed the composition of the S-nitrosylated molecules in endothelial cells. Gel filtration revealed that more than 95% of the detected S-nitrosothiols had a molecular mass of more than 5000 Da. Moreover, inhibition of de novosynthesis of glutathione using N-butyl-sulfoximine did not diminish the overall cellular S-NO content suggesting that S-nitrosylated glutathione quantitatively plays only a minor role in endothelial cells. Having demonstrated that most of the S-nitrosothiols are proteins, we determined the regulation of the S-nitrosylation by pro-inflammatory and pro-atherogenic factors, such as TNFα and mildly oxidized low density lipoprotein (oxLDL). TNFα and oxLDL induced denitrosylation of various proteins as assessed by Saville-Griess assay, by immunostaining with an anti-S-nitrosocysteine antibody, and by a Western blot approach. Furthermore, the caspase-3 p17 subunit, which has previously been shown to be S-nitrosylated and thereby inhibited, was denitrosylated by TNFα treatment suggesting thatS-nitrosylation and denitrosylation are important regulatory mechanisms in endothelial cells contributing to the integrity of the endothelial cell monolayer.
Tobacco smoke-associated particulate matter emissions in a car cabin using the TAPaC platform

Zigarettenrauch enthält bis zu 5000 Inhaltsstoffe, von denen mindestens 250 gesundheits-schädlich und 98 krebserregend sind. Der freigesetzte Feinstaub erreicht hohe Konzentrati-onen in Innenräumen und ist somit besonders schädlich für Passivraucher (z.B. Kinder).
Ziel war es eine Messplattform zu etablieren, mit dessen Hilfe die Feinstaubexposition durch Zigarettenrauch im Fahrzeuginnenraum unter unterschiedlichen ventilatorischen Szenarien untersucht werden konnte. Zudem sollte der Einfluss verschiedener Tabakprodukte auf die Feinstaubkonzentration getestet werden.
Im ersten Teil dieser Dissertation wird die neuartige TAPaC Messplattform (tobacco-associated particulate matter emissions inside a car cabin: establishment of a new measuring platform) vorgestellt. Sie erlaubt die Auswirkungen verschiedener ventilatorischer Sze-narien auf die Feinstaubemission von Zigarettenrauch im Auto besser beurteilen zu können. Da niemand gesundheitsschädlichem Tabakrauch ausgesetzt wird, kann sie ohne jegliche ethische Bedenken eingesetzt werden. Die Zigaretten werden hierbei einzeln auf der Beifahrerseite verraucht. Der beim Rauchen freigesetzte Feinstaub wird auf der Fahrerseite gemessen und in PM10 (Partikel mit einem aerodynamischen Durchmesser <10 µm), PM2,5 (Partikel mit einem aerodynamischen Durchmesser <2,5 µm), und PM1 (Partikel mit einem aerodynamischen Durchmesser <1 µm) unterteilt. Hierbei konnten unter der Verwendung von 3R4F Research Cigarettes extrem hohe Feinstaubmesswerte bei geschlossenen Fenstern und ausgeschalteter Lüftung nachgewiesen werden (PM10: 1608 µg/m3, PM2,5: 1583 µg/m3, PM1: 1133 µg/m3). Diese Daten stellen Durchschnittwerte nach 10-minütiger Messung dar. Auch konnte eine Reduktion der Feinstaubkonzentration (PM10: -70,8 bis -74,4%, PM2,5: -70,6 bis -74,3%, PM1: -64,0 bis -68,0%) durch den Einsatz der Lüftung nachgewiesen werden.
Der zweite Teil dieser Dissertation befasste sich maßgeblich mit dem Einfluss unterschiedlicher ventilatorischer Szenarien auf die Feinstaubkonzentrationen im Auto. Unter Verwen-dung von drei unterschiedlichen Zigarettenprodukten (3R4F Research Cigarettes, Marlboro Red, Marlboro Gold) wurden insgesamt 7 ventilatorische Szenarien (Condition C1b–C7b) getestet. Für alle Szenarien, mit Ausnahme von C1b, war die Autolüftung auf Stufe 2/4 ge-stellt und in Richtung der Windschutzscheibe gerichtet. Die Szenarien beinhalteten: Condition 1 (C1b) Fenster geschlossen, Autolüftung aus und externer Ventilator aus, Condition 2 (C2b) Fenster 10 cm geöffnet und externer Ventilator aus, Condition 3 (C3b) Fenster 10 cm geöffnet und externer Ventilator auf höchster Stufe (3/3) an, Condition 4 (C4b) Fenster halb geöffnet und externer Ventilator aus, Condition 5 (C5b) Fenster halb geöffnet und externer Ventilator auf höchster Stufe (3/3) an, Condition 6 (C6b) Fenster vollständig geöffnet und externer Ventilator aus und Condition 7 (C7b) Fenster vollständig geöffnet und externer Ventilator auf höchster Stufe (3/3) an.
Es zeigten sich besonders hohe Feinstaubkonzentrationen bei Zigaretten, welche ohne Ventilation bei geschlossenem Fenster verraucht wurden. Unabhängig von der verwendeten Marke war die Feinstaubbelastung nach 10 min unter C1b (PM10: 1272–1697 µg/m3, PM2,5: 1253–1659 µg/m3, PM1: 964–1263 µg/m3) deutlich höher als unter C2b (PM10: 67–84 µg/m3, PM2,5: 68–83 µg/m3, PM1: 66–79 µg/m3), C3b (PM10: 100–139 µg/m3, PM2,5: 99–138 µg/m3, PM1: 95–132 µg/m3), C4b (PM10: 84–94 µg/m3, PM2,5: 84–93 µg/m3, PM1: 81–89 µg/m3), C5b (PM10: 94–120 µg/m3, PM2,5: 93–119 µg/m3, PM1: 90–114 µg/m3), C6b (PM10: 155–196 µg/m3, PM2,5: 154–195 µg/m3, PM1: 148–184 µg/m3), und C7b (PM10: 74–99 µg/m3, PM2,5: 72–97 µg/m3, PM1: 69–93 µg/m3). Ebenfalls wurden Feinstaubspitzenwerte bei 4,5 min und 10 min ausgewertet. Bei 4,5 min konnte PM10 unter C2b–C7b um 81,6–93,3% im Vergleich zu C1b reduziert werden. Bei 10 min sogar um 92,9–99,3%. Die 3R4F Zigarette hatte die höchste Feinstaubemission gefolgt von Marlboro Rod und Marlboro Gold. Zudem zeigte sich, dass die Feinstaubemission auch von den Tabakinhaltsstoffen und dessen Konzentrationen abhängig ist.
Die Etablierung der neuen Messplattform ermöglicht die Erforschung von Feinstaubexpositionen durch Zigarettenrauch im Auto ohne potentiell gesundheitliche Folgen. In Zusammenschau der Daten ließ sich nach Öffnen des Fensters und unter Einflussnahme verschiedener ventilatorischer Einstellungen eine deutliche Reduktion der Feinstaubkonzentration im Auto feststellen. Nichtsdestotrotz bleibt die Feinstaubbelastung im Autoinnenraum zu hoch und übersteigt die Richtwerte der Air Quality Guidelines aus dem Jahre 2021 der WHO. Die experimentell untersuchten Belüftungsszenarien sind somit insuffizient, da sie nicht vor der toxischen Feinstaubexposition durch Passivrauch beim Autofahren schützen können.
Hintergrund: Das genaue Wissen um die Umstände eines jeden tödlichen Arbeitsunfalls ist Voraussetzung für die Identifizierung von Unfallschwerpunkten und ermöglicht eine effektive Präventionsarbeit. Mit dieser rechtsmedizinischen Studie zum Arbeitsunfallgeschehen soll ein Beitrag dazu geleistet werden, die Zahl tödlicher Arbeitsunfälle in Deutschland zu senken.
Material und Methode: Zur Untersuchung kamen die tödlichen Arbeitsunfälle, die sich im Einzugsbereich des rechtsmedizinischen Instituts Frankfurt am Main in den Jahren von 2005 bis 2016 ereigneten. Ausgewertet wurden Obduktionsprotokolle sowie die dem Institut zur Verfügung gestellten Staatsanwaltschaftlichen Ermittlungsakten.
Ergebnisse. Es fanden sich 87 tödliche Arbeitsunfälle in dem genannten Zwölfjahreszeitraum. Die Altersstruktur reichte vom jugendlichen Alter bis in das Rentenalter. Betroffen waren zum größten Teil männliche Arbeiter (96,6%, p< 0,0001), verhältnismäßig häufig ausländischer Nationalit.t (34,5%). Die meisten Unfälle ereigneten sich in der 2. Jahreshälfte (58,6%), an Montagen (26,4%), kurz vor und nach der Mittagspause. In 3 Fällen lag die Blutalkoholkonzentration über 0,5‰. Die Baubranche (55,2%) war der unfallträchtigste Wirtschaftszweig. Der Absturz (28,7%) war der häufigste
Unfallmechanismus und das Polytrauma (39,1%) gemeinsam mit dem Schädel-Hirn-Trauma (24,1%) gemäß dem ISS die häufigste Todesursache.
Diskussion: Nach den Ergebnissen dieser Studie sollten Alter der Arbeiter sowie die Tages-, Wochen- und Jahreszeit bei der Ausführung risikoreicher Arbeiten im Baugewerbe berücksichtigt werden. Besonderes Augenmerk sollten Arbeitgeber auf die Kontrolle von Sicherheitsvorkehrungen bei Arbeiten in der Höhe sowie auf die Durchsetzung der Helmpflicht gerade auch bei ausländischen Arbeitnehmern legen.
Background: Estimating prognosis of periodontally affected teeth at the beginning of supportive periodontal care (SPC) is an important component for further treatment planning. This study aimed to evaluate tooth loss (TL) during 10 years of SPC in periodontally compromised patients and to identify tooth-related factors affecting TL.
Methods: Patients were re-examined 120 ± 12 months after accomplishment of active periodontal therapy. TL was defined as primary outcome variable and tooth-related factors (abutment status, furcation involvement [FI], tooth mobility, mean periodontal probing depth [PD], and clinical attachment level [CAL] at beginning of SPC, and initial bone loss [BL]) were estimated based on an adjusted regression analyses model.
Results: Ninety-seven patients (51 females and 46 males; mean age, 65.3 ± 11 years) lost 119 of 2,323 teeth (overall TL [OTL]: 0.12 teeth/patient/y) during 10 years of SPC. Forty of these teeth (33.6%) were lost for periodontal reasons (TLP; 0.04 teeth/patient/y). Significantly more teeth were lost due to other reasons (P <0.0001). TLP (OTL) only occurred in 5.9% (14.7%) of all teeth, when BL was at least 80%. Use as abutment tooth, FI degree III, tooth mobility degrees I and II, mean PD, and CAL positively correlated with OTL (P <0.05). For TLP, FI and tooth mobility degree III as well as mean CAL were identified as tooth-related prognostic factors (P <0.05).
Conclusions: During 10 years of SPC, most of the teeth (93.4%) of periodontally compromised patients were retained, showing the positive effect of a well-established treatment concept. Well-known tooth-related prognostic factors were confirmed.
Purpose: Total elbow arthroplasty (TEA) has evolved over the last years, with satisfactory early results, mainly not only in degenerative arthritis, but also increasingly after trauma. Outcome studies in recently published papers are mainly based on the range of motion (ROM), complication rate as well as patient-reported outcome scales and questionnaires. The purpose of this study was to add a new perspective with the “Purdue Pegboard” skill tests in a homogenous set of elderly trauma patients to contribute to a more precise objective outcome measurement in this specific population.
Methods: A retrospective review was performed on a consecutive cohort of all patients with age above 60 years that received TEA after trauma. Data from follow-up examinations over a standardized time-schedule within 2 years after TEA were included. Mayo Elbow Performance Score (MEPS), “Disability of Arm, Shoulder and Hand” (DASH) Questionnaire, ROM as well as test-scores using the Pegboard test were evaluated.
Results: Mean age was 76.0 years ± 10.3. Indications for TEA were posttraumatic arthrosis in 68.8% (n = 11) and extensive fractures that could not be reconstructed surgically in 31.3% (n = 5). The mean score of MEPS was 82.81 ± 16.63 and 29.18 ± 12.01 in the DASH. ROM presented with a mean of 109.7° ± 15.4. Patients demonstrated good, but marginally reduced test scores in the Pegboard skill tests in comparison with the healthy reference population. No relevant differences between the arm with and the arm without TEA (0.3 ± 3.6; p = 0.715) were noted after 2 years.
Conclusion: In the elderly trauma patient with complex fractures of the elbow, TEA is a good alternative to joint reconstruction using various osteosynthesis techniques. TEA is able to avoid revision surgery after open reduction and internal fixation of complex fractures. In cases of failed reconstruction, it is also a viable secondary procedure in posttraumatic arthrosis. Good outcomes in functionality and dexterity can be achieved. Skill tests like the Purdue Pegboard could add a valuable perspective in assessing functional outcomes after TEA.

Abstract
Background: Bipolar disorder (BD) is a costly, devastating and life shortening mental disorder that is often misdiagnosed, especially on initial presentation. Misdiagnosis frequently results in ineffective treatment. We investigated the utility of a biomarker panel as a diagnostic test for BD.
Methods and findings: We performed a meta-analysis of eight case-control studies to define a diagnostic biomarker panel for BD. After validating the panel on established BD patients, we applied it to undiagnosed BD patients. We analysed 249 BD, 122 pre-diagnostic BD, 75 pre-diagnostic schizophrenia and 90 first onset major depression disorder (MDD) patients and 371 controls. The biomarker panel was identified using ten-fold cross-validation with lasso regression applied to the 87 analytes available across the meta-analysis studies.
We identified 20 protein analytes with excellent predictive performance [area under the curve (AUC) ⩾ 0.90]. Importantly, the panel had a good predictive performance (AUC 0.84) to differentiate 12 misdiagnosed BD patients from 90 first onset MDD patients, and a fair to good predictive performance (AUC 0.79) to differentiate between 110 pre-diagnostic BD patients and 184 controls. We also demonstrated the disease specificity of the panel.
Conclusions: An early and accurate diagnosis has the potential to delay or even prevent the onset of BD. This study demonstrates the potential utility of a biomarker panel as a diagnostic test for BD.
Background: Marked sex differences in autism prevalence accentuate the need to understand the role of biological sex-related factors in autism. Efforts to unravel sex differences in the brain organization of autism have, however, been challenged by the limited availability of female data.
Methods: We addressed this gap by using a large sample of males and females with autism and neurotypical (NT) control individuals (ABIDE; Autism: 362 males, 82 females; NT: 409 males, 166 females; 7-18 years). Discovery analyses examined main effects of diagnosis, sex and their interaction across five resting-state fMRI (R-fMRI) metrics (voxel-level Z > 3.1, cluster-level P < 0.01, gaussian random field corrected). Secondary analyses assessed the robustness of the results to different pre-processing approaches and their replicability in two independent samples: the EU-AIMS Longitudinal European Autism Project (LEAP) and the Gender Explorations of Neurogenetics and Development to Advance Autism Research (GENDAAR).
Results: Discovery analyses in ABIDE revealed significant main effects across the intrinsic functional connectivity (iFC) of the posterior cingulate cortex, regional homogeneity and voxel-mirrored homotopic connectivity (VMHC) in several cortical regions, largely converging in the default network midline. Sex-by-diagnosis interactions were confined to the dorsolateral occipital cortex, with reduced VMHC in females with autism. All findings were robust to different pre-processing steps. Replicability in independent samples varied by R-fMRI measures and effects with the targeted sex-by-diagnosis interaction being replicated in the larger of the two replication samples – EU-AIMS LEAP.
Limitations: Given the lack of a priori harmonization among the discovery and replication datasets available to date, sample-related variation remained and may have affected replicability.
Conclusions: Atypical cross-hemispheric interactions are neurobiologically relevant to autism. They likely result from the combination of sex-dependent and sex-independent factors with a differential effect across functional cortical networks. Systematic assessments of the factors contributing to replicability are needed and necessitate coordinated large-scale data collection across studies.

Die Gattungen Nicotiana tabacum und Nicotiana rustica der Tabakpflanze sind von großer wirtschaftlicher Bedeutung. Aus ihnen wird Tabak hergestellt, der mit Alkohol zur weltweit am häufigsten konsumierten Genussdroge zählt. Aufgrund seiner Legalität wird die Toxizität trotz steigender Warnung und Aufklärung immer noch unterschätzt. Die Toxizität der Tabakpflanze ist vor allem auf das Alkaloid Nikotin zurückzuführen. Dass es selten zu einer Vergiftung durch die reine Pflanze kommt, liegt daran, dass sie optisch kaum zum Verzehr anregt. Häufiger dagegen ist eine Vergiftung durch z. B. verschluckte Zigarettenstummel, die vor allem für Kinder sehr gefährlich sein kann. Eine weitere Gefahr der Vergiftung entsteht bei der Tabakernte. Nikotin wird auch über die Haut aufgenommen und kann so zu der Green Tobacco Sickness bei Tabakplantagenarbeitern führen. Im Ernstfall existiert kein Antidot. Aktivkohle sollte so schnell wie möglich gegeben werden, um die Resorption zu vermindern. Ansonsten muss das Nikotin mit einer Magenwäsche aus dem Körper gefiltert werden. Präventiv sollten deshalb verstärkt auf die Gefahren des Tabaks aufmerksam gemacht werden.
