Hintergrund: Träger eines bestimmten Haplotyps des Gens der GTP Cyclohydrolase (GCH1) gaben im Anschluss an eine Diskektomie nach chronischer lumbaler Radikulopathie weniger Schmerzen an und hatten in experimentellen Schmerzmessungen höhere Schmerzschwellen. Die ex-vivo GCH1-Hochregulierung und BH4-Produktion nach Forskolinstimulierung war reduziert, während die Grundwerte der BH4-Konzentration nicht verändert waren. Diese Beobachtungen legen nahe, dass der Haplotyp hauptsächlich Veränderungen zeigt, wenn das GCH1-System provoziert wird. Diese Studie zielt darauf ab, (1) diese Hypothese zu testen und (2) die schmerzprotektiven Eigenschaften, die diesem Haplotyp zuvor zugeschrieben wurden in unabhängigen Rahmen zu reproduzieren. Methodik: Hierzu wurden experimentelle Schmerzmodelle mit Sensitisierung (lokale Hautentzündung, topische Capsaicinapplikation) und ohne Sensitisierung (punktuell-mechanischer Druck, stumpfer Druck, thermischer und elektrischer Schmerz) an 10 homozygoten Trägern und 22 homozygoten Nichtträgern des angeblich schmerzprotektiven GCH1-Haplotyps durchgeführt. GCH1-, iNOS-Hochregulierung und BH4-Produktion wurden ex-vivo in Leukozyten nach LPS-Stimulation für 24 Stunden ermittelt. Ergebnisse: Träger dieses GCH1-Haplotyps hatten höhere Schmerzschwellen für punktuell-mechanischen Druck (von Frey Haare) im Anschluss an eine lokale Hautentzündung (18.1 ± 11.3 vs. 9 ± 2.8 g; p = 0.005) und im kleineren Rahmen für thermischen Schmerz nach Hyperalgesieinduktion durch Capsaicincreme (35.2 ± 0.9 vs. 36.6 ± 2.4 _C; p = 0.026). Dem hingegen konnten bei den Schmerzmodellen ohne Sensitisierung keine genotypabhängigen Effekte beobachtet werden. GCH1-, BH4 und iNOS-Hochregulierung in Leukozyten nach 24-stündiger LPS-Stimulation waren vermindert bei Trägern des GCH1-Haplotyps, woraus sich schließen lässt, dass sich die Genotypgruppen im Hinblick auf die Regulationsvorgänge im Biopterinstoffwechsel unterscheiden. Schlussfolgerung: Diese Studie bestätigt die vorherigen Befunde, dass eine verminderte GCH1-Funktion oder GCH1-Induzierbarkeit als Folge von genetischen Polymorphismen einen schmerzprotektiven Effekt mit sich bringt. Als Erweiterung der Vorresultate zeigt diese Arbeit, dass sich der schmerzprotektive Effekt vorzugsweise in Schmerzmodellen mit Sensitisierung ausprägt. Dies legt eine besondere Rolle von BH4 bei spezifischen Schmerzprozessen nahe.
Einleitung: Patienten nach operativen Eingriffen haben ein hohes Risiko in Folge von nosokomialen Infektionen, Wundinfekten oder Nahtdehiszensen eine Sepsis zu entwickeln. Die Sterblichkeitsraten des septischen Schocks haben sich mit 40-60% in den letzten 20 Jahren kaum verändert. Der Zeitpunkt der Diagnose und damit der frühzeitige Beginn v.a. einer kausalen, aber auch einer frühzeitigen supportiven Therapie sind entscheidende Determinanten der Überlebenswahrscheinlichkeit. Die vorliegende Arbeit untersucht erstmalig, ob IL-22, ein Mitglied der IL-10 Familie und, zur angeborenen Immunität gehörend, zur besseren Differenzierung von Inflammation und Infektion bei Patienten im perioperativen Kontext geeignet ist. Material & Methoden: 115 Patienten wurden in einem Zeitraum zwischen November 2005 und November 2007 in drei Gruppen untersucht. Nach Genehmigung durch die Ethikkommission wurden nach entsprechender Einwilligung durch die Patienten, oder die gesetzlichen Betreuer, zum einen Patienten mit schwerer Sepsis [n= 36], elektiven großen viszeralchirurgischen Eingriffen (CAB) [n= 31] und Patienten bei orthotoper Lebertransplantation (LTX) [n= 48] rekrutiert. Es wurden Blutproben prä-, postoperativ sowie an den ersten fünf postoperativen Tagen entnommen. Die Proben wurden unmittelbar nach Abnahme kühl zentrifugiert, aliquotiert und anschließend bei -70°C gelagert. Parallel wurden Blutkulturen angefertigt und Procalcitonin, IL-6, IL-10, Lipopolysaccharid-bindendes Protein (LBP), C-reaktives Protein (CRP) und Leukozyten sowie ein Standardlabor bestimmt. Mittels ELISA in Sandwichtechnik bestimmten wir in nachfolgenden Labormessungen die IL-22 Konzentrationen. IL-6, IL-10 und LBP wurden mittels Immulite® gemessen. Die Daten der Hämodynamik wurden entsprechend einem Protokoll erhoben und in einer Datenbank gespeichert. Ergebnisse: Große, elektive viszeralchirurgische Eingriffe sowie die orthotope LTX haben keinen Einfluss auf Serumkonzentrationen von IL-22. Die Blutspiegel liegen sowohl prä-, als auch postoperativ nahe unter der Nachweisgrenze von 10pg/ml (CAB 7,3pg/ml, LTX 9,4pg/ml). Patienten mit einer schweren Sepsis hatten im Vergleich zu Patienten nach viszeralchirurgischen Eingriffen signifikant höhere IL-22 Konzentrationen im Serum (75pg/ml, p= 0,003, Mann-Whitney-Test). Bei Unterteilung nach dem Sepsisfokus hatten Patienten mit einer vom Abdomen ausgehenden Infektion einen Median von 101,6pg/ml, bei Sepsis pulmonaler Genese eine signifikant niedrigere IL-22 Konzentration von 56,9pg/ml (p= 0,037). Die Stimulation von IL-22 hält postoperativ nach Fokussanierung bei Patienten mit abdomineller Sepsis an, so dass ein Abfall in den ersten fünf postoperativen Tagen bis auf 30% des Ausgangswertes beobachtet werden kann. Auch bei den LTX Patienten zeigt sich, wenn auch nicht signifikant, dass eine bakterielle Infektion (primäre Sepsis) zu einer deutlichen Stimulation von IL-22 führt (9,4pg/ml versus 49,7pg/ml bei Sepsis). Quo ad vitam führt IL-22 bei LTX Patienten in der 1-Jahresüberlebenswahrscheinlichkeit zu einer signifikanten Reduktion des Überlebens, wenn die präoperativen IL-22 Serumspiegel über 100pg/ml liegen. Eine kausale Verknüpfung kann zum jetzigen Zeitpunkt nicht hergestellt werden. Der MELD Score (Model for end-stage liver disease) korreliert mit den erhobenen IL-22 Werten, so dass auch ein MELD > 30 mit erhöhten IL-22 Spiegeln einhergeht. Zusammenfassung: IL- 22 eignet sich im postoperativen Verlauf nach viszeralchirurgischen Eingriffen zur Detektion von Patienten mit schweren Komplikationen. Bei Erhöhung des IL-22 Serumspiegels >100pg/ml kann von einer Darmperfusionsstörung bzw. peritonealen Reizung ausgegangen werden. Unsere Daten weisen weiter darauf hin, dass bei schweren Perfusionsstörungen bis hin zu kompletter Ischämie einzelner Darmabschnitte Werte von >500pg/ml erreicht werden. Hingegen führen eine primäre Sepsis sowie Pneumonien zu einer IL-22 Stimulation mit Serumspiegeln zwischen 50 und 100pg/ml. Eine Korrelation mit anderen Markern der Inflammation war bei relativ kleinen Fallzahlen nicht zu beobachten. Lediglich auffällig erscheinen hohe IL-22 Spiegel, welche eher mit einer Leukopenie einhergehen als mit einer Leukozytose. Ist IL-22 bei Patienten vor Lebertransplantation erhöht, d.h. unabhängig einer Sepsis bei >100 pg/ml, ist die 1-Jahres Überlebenswahrscheinlichkeit signifikant erniedrigt. Dies korreliert entsprechend einer Überlebenswahrscheinlichkeit mit einem MELD>30 vor Transplantation. Hiermit ist zu prüfen, ob eine ggf. längerfristige Aktivierung der angeborenen Immunität bei Leberzirrhose vor Transplantation auf das Überleben nach Transplantation einen wesentlichen Einfluß hat. Eine entsprechende Anpassung der Therapie vor Transplantation könnte dann möglicherweise das eingeschränkte Überleben positiv beeinflussen.
Introduction: Hypothermia improves survival and neurological recovery after cardiac arrest. Pro-inflammatory cytokines have been implicated in focal cerebral ischemia/reperfusion in-jury. It is unknown whether cardiac arrest also triggers the release of cerebral inflammatory molecules, and whether therapeutic hypothermia alters this inflammatory response. This study sought to examine whether hypothermia or the combination of hypothermia with anes-thetic postconditioning with sevoflurane affect cerebral inflammatory response after cardio-pulmonary resuscitation. Methods: Thirty pigs (28 - 34kg) were subjected to cardiac arrest following temporary coro-nary artery occlusion. After 7 minutes of ventricular fibrillation and 2 minutes of basic life support, advanced cardiac life support was started according to the current AHA guidelines. Return of spontaneous circulation was achieved in 21 animals who were randomized to ei-ther normothermia at 38degreesC, hypothermia at 33degreesC or hypothermia at 33degreesC combined with se-voflurane (each group: n = 7) for 24 hours. The effects of hypothermia and the combination of hypothermia with sevoflurane on cerebral inflammatory response after cardiopulmonary resuscitation were studied using tissue samples from the cerebral cortex of pigs euthanized after 24 hours and employing quantitative RT-PCR and ELISA techniques. Results: Global cerebral ischemia following resuscitation resulted in significant upregulation of cerebral tissue inflammatory cytokine mRNA expression (mean +/- SD; interleukin (IL)-1beta 8.7 +/- 4.0, IL-6 4.3 +/- 2.6, IL-10 2.5 +/- 1.6, tumor necrosis factor (TNF)alpha 2.8 +/- 1.8, intercellular adhesion molecule-1 (ICAM-1) 4.0 +/- 1.9-fold compared with sham control) and IL-1beta protein concentration (1.9 +/- 0.6-fold compared with sham control). Hypothermia was associated with a significant (P <0.05 versus normothermia) reduction in cerebral inflammatory cytokine mRNA expression (IL-1beta 1.7 +/- 1.0, IL-6 2.2 +/- 1.1, IL-10 0.8 +/- 0.4, TNFalpha 1.1 +/- 0.6, ICAM-1 1.9 +/- 0.7-fold compared with sham control). These results were also confirmed for IL-1beta on protein level. Experimental settings employing hypothermia in combination with sevoflurane showed that the volatile anesthetic did not confer additional anti-inflammatory effects com-pared with hypothermia alone. Conclusions: Mild therapeutic hypothermia resulted in decreased expression of typical ce-rebral inflammatory mediators after cardiopulmonary resuscitation. This may confer, at least in part, neuroprotection following global cerebral ischemia and resuscitation.
Markt- und wettbewerbsorientierte Reformstrategien in den Krankenhaussystemen zahlreicher Industrieländer haben Befürchtungen vor einer kommerzialisierten Krankenhausversorgung hervorgebracht. Dieser Beitrag unterbreitet einen analytischen Interpretationsrahmen zur Erklärung der internationalen Verbreitung dieser Reformstrategien und versucht die behaupteten negativen Effekte von Kommerzialisierungsprozessen auf Versorgungsqualität und Zugänglichkeit zu untersuchen. Gestützt auf einen Vergleich eines idealtypischen Kommerzialisierungsmodells mit dem institutionellen und organisatorischen Wandel im deutschen Krankenhaussystem kommt der Beitrag zu dem Schluss, dass Kommerzialisierungsprozesse in der Krankenhausversorgung bislang noch begrenzt sind. Obwohl ein markt- und wettbewerbsbasierter Umbau der Governancestrukturen zu beobachten ist und Krankenhäuser zu einer Kommerzialisierungsstrategie gedrängt werden, lässt sich aufgrund einer unzureichenden Daten- und Forschungslage bislang nicht empirisch feststellen, ob die Kommerzialisierungsprozesse zu einer Verschlechterung der Qualität und Zugänglichkeit der Krankenhausversorgung geführt haben.
Background: Complex care management is seen as an approach to face the challenges of an ageing society with increasing numbers of patients with complex care needs. The Medical Research Council in the United Kingdom has proposed a framework for the development and evaluation of complex interventions that will be used to develop and evaluate a primary care-based complex care management program for chronically ill patients at high risk for future hospitalization in Germany. Methods and design: We present a multi-method procedure to develop a complex care management program to implement interventions aimed at reducing potentially avoidable hospitalizations for primary care patients with type 2 diabetes mellitus, chronic obstructive pulmonary disease, or chronic heart failure and a high likelihood of hospitalization. The procedure will start with reflection about underlying precipitating factors of hospitalizations and how they may be targeted by the planned intervention (pre-clinical phase). An intervention model will then be developed (phase I) based on theory, literature, and exploratory studies (phase II). Exploratory studies are planned that entail the recruitment of 200 patients from 10 general practices. Eligible patients will be identified using two ways of 'case finding': software based predictive modelling and physicians' proposal of patients based on clinical experience. The resulting subpopulations will be compared regarding healthcare utilization, care needs and resources using insurance claims data, a patient survey, and chart review. Qualitative studies with healthcare professionals and patients will be undertaken to identify potential barriers and enablers for optimal performance of the complex care management program. Discussion: This multi-method procedure will support the development of a primary care-based care management program enabling the implementation of interventions that will potentially reduce avoidable hospitalizations.

Heute werden viele Leukämiekranke geheilt. An der Goethe-Universität setzt ein Schwerpunkt für Lymphom- und Leukämieforschung deutschlandweit Akzente bei Forschung und Diagnostik.
Background: Prospective, observational studies that enroll large numbers of patients with few exclusion criteria may better reflect actual ongoing clinical experience than randomized clinical trials. Our purpose was to obtain efficacy and safety information from a cohort of subjects exposed to latanoprost/timolol fixed combination (FC) for [greater than or equal to]18 months using a prospective, observational design. Methods: In all, 577 office-based ophthalmologists in Germany switched 2339 patients with glaucoma or ocular hypertension to latanoprost/timolol FC for medical reasons. Follow-up visits were scheduled for every 6 months over 24 months; physicians followed usual care routines. Intraocular pressure (IOP), visual field status, optic nerve head findings, and adverse events were recorded. Efficacy parameters were evaluated for the per protocol (PP) population; the safety population included subjects receiving [greater than or equal to]1 drop of FC. Physicians rated efficacy, tolerability, and subject compliance at month 24. Results: Of the 2339 subjects switched to latanoprost/timolol FC (safety population), the primary reasons for switching were inadequate IOP reduction (78.2%) and desire to simplify treatment with once-daily dosing (29.4%; multiple reasons possible). In all, 1317 (56.3%) subjects completed the study, and 1028 (44.0%) were included in the PP population. Most discontinuations were due to loss to follow-up. Change in mean IOP from baseline to month 6 was -4.0 +/- 4.31 mmHg, a reduction that was maintained throughout (P<0.05 for change at all time points). By investigator assessments, optic disc parameters and visual field were stable over 24 months, and there was no relationship between IOP reduction over 24 months and development of a visual field defect. More than 90% of physicians rated latanoprost/timolol FC as "very good" or "good" for efficacy (PP population), tolerability, and compliance. The FC was safe and well tolerated. No change in iris color was reported by most subjects (83.1%) at month 24. Conclusions: Over 24 months, latanoprost/timolol FC effectively lowers IOP levels and is well tolerated in patients with glaucoma or ocular hypertension who change from their previous ocular hypotensive therapy for medical reasons. Investigator assessments found optic disc parameters and visual field to be stable throughout 24 months of follow-up.
Activation of hypoxia inducible factor 1 is a general phenomenon in infections with human pathogens

Background: Hypoxia inducible factor (HIF)-1 is the key transcriptional factor involved in the adaptation process of cells and organisms to hypoxia. Recent findings suggest that HIF-1 plays also a crucial role in inflammatory and infectious diseases. Methodology/Principal Findings: Using patient skin biopsies, cell culture and murine infection models, HIF-1 activation was determined by immunohistochemistry, immunoblotting and reporter gene assays and was linked to cellular oxygen consumption. The course of a S. aureus peritonitis was determined upon pharmacological HIF-1 inhibition. Activation of HIF-1 was detectable (i) in all ex vivo in biopsies of patients suffering from skin infections, (ii) in vitro using cell culture infection models and (iii) in vivo using murine intravenous and peritoneal S. aureus infection models. HIF-1 activation by human pathogens was induced by oxygen-dependent mechanisms. Small colony variants (SCVs) of S. aureus known to cause chronic infections did not result in cellular hypoxia nor in HIF-1 activation. Pharmaceutical inhibition of HIF-1 activation resulted in increased survival rates of mice suffering from a S. aureus peritonitis. Conclusions/Significance: Activation of HIF-1 is a general phenomenon in infections with human pathogenic bacteria, viruses, fungi and protozoa. HIF-1-regulated pathways might be an attractive target to modulate the course of life-threatening infections.
