Introduction: In the large-scale case-control study EPILIFT, we investigated the dose-response relationship between lifestyle factors (weight, smoking amount, cumulative duration of different sports activities) and lumbar disc disease. Methods: In four German study regions (Frankfurt am Main, Freiburg, Halle/Saale, Regensburg), 564 male and female patients with lumbar disc herniation and 351 patients with lumbar disc narrowing (chondrosis) aged 25 to 70 years were prospectively recruited. From the regional population registers, 901 population control subjects were randomly selected. In a structured personal interview, we enquired as to body weight at different ages, body height, cumulative smoking amount and cumulative duration of different sports activities. Confounders were selected according to biological plausibility and to the change-in-estimate criterion. Adjusted, gender-stratified odds ratios with 95% confidence intervals were calculated using unconditional logistic regression analysis. Results: The results of this case-control study reveal a positive association between weight and lumbar disc herniation as well as lumbar disc narrowing among men and women. A medium amount of pack-years was associated with lumbar disc herniation and narrowing in men and women. A non-significantly lowered risk of lumbar disc disease was found in men with high levels of cumulative body building and strength training. Conclusions: According to our multi-center case-control study, body weight might be related to lumbar disc herniation as well as to lumbar disc narrowing. Further research should clarify the potential protective role of body building or strength training on lumbar disc disease.
Background: Descending inhibitory pain control contributes to the endogenous defense against chronic pain and involves noradrenergic and serotonergic systems. The clinical efficacy of antidepressants suggests that serotonin may be particularly relevant for neuropathic pain conditions. Serotonergic signaling is regulated by synthesis, metabolisms, reuptake and receptors. To address the complexity, we used inbred mouse strains, C57BL/6J, 129 Sv, DBA/2J and Balb/c, which differ in brain serotonin levels. Results: Serotonin analysis after nerve injury revealed inter-strain differences in the adaptation of descending serotonergic fibers. Upregulation of spinal cord and midbrain serotonin was apparent only in 129 Sv mice and was associated with attenuated nerve injury evoked hyperalgesia and allodynia in this strain. The increase of dorsal horn serotonin was blocked by hemisectioning of descending fibers but not by rhizotomy of primary afferents indicating a midbrain source. Para-chlorophenylalanine-mediated serotonin depletion in spinal cord and midbrain intensified pain hypersensitivity in the nerve injury model. In contrast, chronic inflammation of the hindpaw did not evoke equivalent changes in serotonin levels in the spinal cord and midbrain and nociceptive thresholds dropped in a parallel manner in all strains. Conclusion: The results suggest that chronic nerve injury evoked hypernociception may be contributed by genetic differences of descending serotonergic inhibitory control.
Markt- und wettbewerbsorientierte Reformstrategien in den Krankenhaussystemen zahlreicher Industrieländer haben Befürchtungen vor einer kommerzialisierten Krankenhausversorgung hervorgebracht. Dieser Beitrag unterbreitet einen analytischen Interpretationsrahmen zur Erklärung der internationalen Verbreitung dieser Reformstrategien und versucht die behaupteten negativen Effekte von Kommerzialisierungsprozessen auf Versorgungsqualität und Zugänglichkeit zu untersuchen. Gestützt auf einen Vergleich eines idealtypischen Kommerzialisierungsmodells mit dem institutionellen und organisatorischen Wandel im deutschen Krankenhaussystem kommt der Beitrag zu dem Schluss, dass Kommerzialisierungsprozesse in der Krankenhausversorgung bislang noch begrenzt sind. Obwohl ein markt- und wettbewerbsbasierter Umbau der Governancestrukturen zu beobachten ist und Krankenhäuser zu einer Kommerzialisierungsstrategie gedrängt werden, lässt sich aufgrund einer unzureichenden Daten- und Forschungslage bislang nicht empirisch feststellen, ob die Kommerzialisierungsprozesse zu einer Verschlechterung der Qualität und Zugänglichkeit der Krankenhausversorgung geführt haben.
Background: Haemostasis in liver surgery remains a challenge despite improved resection techniques. Oozing from blood vessels too small to be ligated necessitate a treatment with haemostats in order to prevent complications attributed to bleeding. There is good evidence from randomised trials for the efficacy of fibrin sealants, on their own or in combination with a carrier material. A new haemostatic device is Sangustop(R). It is a collagen based material without any coagulation factors. Pre-clinical data for Sangustop(R) showed superior haemostatic effect. This present study aims to show that in the clinical situation Sangustop(R) is not inferior to a carrier-bound fibrin sealant (Tachosil(R)) as a haemostatic treatment in hepatic resection. Methods: This is a multi-centre, patient-blinded, intra-operatively randomised controlled trial. A total of 126 patients planned for an elective liver resection will be enrolled in eight surgical centres. The primary objective of this study is to show the non-inferiority of Sangustop(R) versus a carrier-bound fibrin sealant (Tachosil(R)) in achieving haemostasis after hepatic resection. The surgical intervention is standardised with regard to devices and techniques used for resection and primary haemostasis. Patients will be followed-up for three months for complications and adverse events. Discussion: This randomised controlled trial (ESSCALIVER) aims to compare the new collagen haemostat Sangustop(R) with a carrier-bound fibrin sealant which can be seen as a "gold standard" in hepatic and other visceral organ surgery. If non-inferiority is shown other criteria than the haemostatic efficacy (e.g. costs, adverse events rate) may be considered for the choice of the most appropriate treatment. Within the visual cortex, it has been proposed that interhemispheric interactions serve to re-establish the continuity of the visual field across its vertical meridian (VM) by mechanisms similar to those used by intrinsic connections within a hemisphere. However, other specific functions of transcallosal projections have also been proposed, including contributing to disparity tuning and depth perception. Here, we consider whether interhemispheric connections modulate specific response properties, orientation and direction selectivity, of neurons in areas 17 and 18 of the ferret by combining reversible thermal deactivation in one hemisphere with optical imaging of intrinsic signals and single-cell electrophysiology in the other hemisphere. We found interhemispheric influences on both the strength and specificity of the responses to stimulus orientation and direction of motion, predominantly at the VM. However, neurons and domains preferring cardinal contours, in particular vertical contours, seem to receive stronger interhemispheric input than others. This finding is compatible with interhemispheric connections being involved in horizontal disparity tuning. In conclusion, our results support the view that interhemispheric interactions mainly perform integrative functions similar to those of connections intrinsic to one hemisphere. Studying the role of human parietal cortex in visuospatial attention with concurrent TMS-fMRI

Combining transcranial magnetic stimulation (TMS) with concurrent functional magnetic resonance imaging (fMRI) allows study of how local brain stimulation may causally affect activity in remote brain regions. Here, we applied bursts of high- or low-intensity TMS over right posterior parietal cortex, during a task requiring sustained covert visuospatial attention to either the left or right hemifield, or in a neutral control condition, while recording blood oxygenation-level–dependent signal with a posterior MR surface coil. As expected, the active attention conditions activated components of the well-described “attention network,” as compared with the neutral baseline. Also as expected, when comparing left minus right attention, or vice versa, contralateral occipital visual cortex was activated. The critical new finding was that the impact of high- minus low-intensity parietal TMS upon these visual regions depended on the currently attended side. High- minus low-intensity parietal TMS increased the difference between contralateral versus ipsilateral attention in right extrastriate visual cortex. A related albeit less pronounced pattern was found for left extrastriate visual cortex. Our results confirm that right human parietal cortex can exert attention-dependent influences on occipital visual cortex and provide a proof of concept for the use of concurrent TMS–fMRI in studying how remote influences can vary in a purely top–down manner with attentional demands. Recombinase-mediated cassette exchange (RMCE) exploits the possibility to unidirectionally exchange any genetic material flanked by heterotypic recombinase recognition sites (RRS) with target sites in the genome. Due to a limited number of available pre-fabricated target sites, RMCE in mouse embryonic stem (ES) cells has not been tapped to its full potential to date. Here, we introduce a universal system, which allows the targeted insertion of any given transcriptional unit into 85 742 previously annotated retroviral conditional gene trap insertions, representing 7013 independent genes in mouse ES cells, by RMCE. This system can be used to express any given cDNA under the control of endogenous trapped promoters in vivo, as well as for the generation of transposon ‘launch pads’ for chromosomal region-specific ‘Sleeping Beauty’ insertional mutagenesis. Moreover, transcription of the gene-of-interest is only activated upon Cre-recombinase activity, a feature that adds conditionality to this expression system, which is demonstrated in vivo. The use of the RMCE system presented in this work requires one single-cloning step followed by one overnight gateway clonase reaction and subsequent cassette exchange in ES cells with efficiencies of 40% in average.
Die Sicherung der Atemwege ist eine der wichtigsten Aufgaben des mit dem Atemwegsmanagement beauftragten Arztes, da eine fehlgeschlagene Intubation und sich über längere Zeit erstreckende Intubationsversuche schnell zu einer kritischen Hypoxie führen können. Gelingt eine endotracheale Intubation mittels konventioneller Larnygoskopie mit dem Macintosh-Spatel unerwartet nicht, stehen verschiedene supraglottische Atemwegshilfen wie z.B. die Larynxmaske zur Atemwegssicherung zur Verfügung. Falls sich jedoch aus verschiedenen Gründen der Einsatz eines supraglottischen Atemwegs verbietet und die Notwendigkeit einer endotrachealen Intubation besteht, muss eine andere Intubationsmethode als die konventionelle Laryngoskopie gewählt werden. Das Standardverfahren für den erwartet schwierigen Atemweg, die Intubation mit dem flexiblen Endoskop am spontan atmenden Patienten, eignet sich nicht für den unerwartet schwierigen Atemweg. Hierfür werden die Intubationslarynxmaske, Videolaryngoskope, Führungsstäbe mit Transillumination und verschiedene starre Fiberoptiken wie das Bonfils Intubationsfiberskop oder das Laryngoskop nach Bullard eingesetzt. Der Erfolg des Bonfils Intubationsfiberskops am unerwartet schwierigen Atemweg und am erwartet schwierigen Atemweg, basierend auf einer Reihe klinischer Faktoren, wurde bereits bewiesen. Es ist jedoch nicht bekannt, ob sich das Instrument für einen klar definierten schwierigen Atemweg im Sinne einer eingeschränkten Mundöffnung und eingeschränkten Beweglichkeit in der Halswirbelsäule eignet. Ziel der vorliegenden Studie war es zu untersuchen, ob sich das Bonfils Intubationsfiberskop für den Einsatz am schwierigen Atemweg, simuliert durch einen Immobilisationskragen, eignet. Nach Einwilligung der Ethikkommission wurde die Studie an 76 Patienten durchgeführt, die sich einem elektiven gynäkologischen Eingriff unterzogen. Nach der Simulation des schwierigen Atemwegs durch Anlegen eines Immobilisationskragens wurden jeweils 38 Patienten randomisiert entweder mittels direkter Laryngoskopie oder dem Bonfils Intubationsfiberskop intubiert. Die erfolgreiche Platzierung des Endotrachealtubus mit dem jeweiligen Instrument war der primäre Zielparameter der Studie. Nach Immobilisierung der Halswirbelsäule betrug die maximale Mundöffnung 2,6 cm ± 0,7 cm in der Macintosh-Gruppe und 2,6 cm ± 0,8 cm in der Bonfils-Gruppe. Mit dem Laryngoskop mit Macintosh-Spatel konnten 15/38 Patienten (39,5%) erfolgreich intubiert werden, mit dem Bonfils Intubationsfiberskop konnten 31/38 Patienten (81,6%) erfolgreich intubiert werden (P<0,05). Die benötigte Zeit bis zur erfolgreichen Platzierung des Endotrachealtubus war mit dem Laryngoskop geringer (53 ± 22 s) als mit dem Bonfils Intubationsfiberskop (64 ± 24 s), dieser Zeitunterschied besitzt jedoch weder statistische, noch klinische Relevanz. In der vorliegenden Studie konnte gezeigt werden, dass das Bonfils Intubationsfiberskop der direkten Laryngoskopie mit Macintosh-Spatel an Patienten mit eingeschränkter Mundöffnung und immobilisierter Halswirbelsäule überlegen ist.
BACKGROUND: Parkinson's disease (PD), the second most frequent neurodegenerative disorder at old age, can be caused by elevated expression or the A53T missense mutation of the presynaptic protein alpha-synuclein (SNCA). PD is characterized pathologically by the preferential vulnerability of the dopaminergic nigrostriatal projection neurons. METHODOLOGY/PRINCIPAL FINDINGS: Here, we used two mouse lines overexpressing human A53T-SNCA and studied striatal dysfunction in the absence of neurodegeneration to understand early disease mechanisms. To characterize the progression, we employed young adult as well as old mice. Analysis of striatal neurotransmitter content demonstrated that dopamine (DA) levels correlated directly with the level of expression of SNCA, an observation also made in SNCA-deficient (knockout, KO) mice. However, the elevated DA levels in the striatum of old A53T-SNCA overexpressing mice may not be transmitted appropriately, in view of three observations. First, a transcriptional downregulation of the extraneural DA degradation enzyme catechol-ortho-methytransferase (COMT) was found. Second, an upregulation of DA receptors was detected by immunoblots and autoradiography. Third, extensive transcriptome studies via microarrays and quantitative real-time RT-PCR (qPCR) of altered transcript levels of the DA-inducible genes Atf2, Cb1, Freq, Homer1 and Pde7b indicated a progressive and genotype-dependent reduction in the postsynaptic DA response. As a functional consequence, long term depression (LTD) was absent in corticostriatal slices from old transgenic mice. CONCLUSIONS/SIGNIFICANCE: Taken together, the dysfunctional neurotransmission and impaired synaptic plasticity seen in the A53T-SNCA overexpressing mice reflect early changes within the basal ganglia prior to frank neurodegeneration. As a model of preclinical stages of PD, such insights may help to develop neuroprotective therapeutic approaches.
Introduction: Acute lung injury (ALI) is an inflammatory disorder of pulmonary or extrapulmonary origin. We have previously demonstrated that netrin-1 dampens murine ALI, and in an attempt to advance this finding into future clinical practice we evaluated whether netrin-1 would reduce alveolar inflammation during porcine ALI. Methods: This was a controlled in vivo experimental study in pigs. We induced ALI through lipoploysaccharide (LPS) infusion (50 micro g/kg) for 2 hours. Following this, we exposed animals to either vehicle, intravenous netrin-1 (netrin-1 i.v.) or inhaled netrin-1 (netrin-1 inh.). Serum samples and bronchoalveolar lavage (BAL) were obtained to determine levels of tumor necrosis factor-alpha (TNF-alpha), interleukin (IL)-1beta, interleukin-6 and interleukin-8 at baseline and 6 hours following treatment. Myeloperoxidase activity (MPO) and protein levels were determined in the BAL, and tissue samples were obtained for histological evaluation. Finally, animals were scanned with spiral CT. Results: Following LPS infusion, animals developed acute pulmonary injury. Serum levels of TNF-alpha and IL-6 were significantly reduced in the netrin-1 i.v. group. BAL demonstrated significantly reduced cytokine levels 6 hours post-netrin-1 treatment (TNF-alpha: vehicle 633 ± 172 pg/ml, netrin-1 i.v. 84 ± 5 pg/ml, netrin-1 inh. 168 ± 74 pg/ml; both P < 0.05). MPO activity and protein content were significantly reduced in BAL samples from netrin-1-treated animals. Histological sections confirmed reduced inflammatory changes in the netrin-1-treated animals. Computed tomography corroborated reduced pulmonary damage in both netrin-1-treated groups. Conclusions: We conclude that treatment with the endogenous anti-inflammatory protein netrin-1 reduces pulmonary inflammation during the initial stages of ALI and should be pursued as a future therapeutic option.
Plasticity resembling spike-timing dependent synaptic plasticity: the evidence in human cortex

Spike-timing dependent plasticity (STDP) has been studied extensively in a variety of animal models during the past decade but whether it can be studied at the systems level of the human cortex has been a matter of debate. Only recently newly developed non-invasive brain stimulation techniques such as transcranial magnetic stimulation (TMS) have made it possible to induce and assess timing dependent plasticity in conscious human subjects. This review will present a critical synopsis of these experiments, which suggest that several of the principal characteristics and molecular mechanisms of TMS-induced plasticity correspond to those of STDP as studied at a cellular level. TMS combined with a second phasic stimulation modality can induce bidirectional long-lasting changes in the excitability of the stimulated cortex, whose polarity depends on the order of the associated stimulus-evoked events within a critical time window of tens of milliseconds. Pharmacological evidence suggests an NMDA receptor mediated form of synaptic plasticity. Studies in human motor cortex demonstrated that motor learning significantly modulates TMS-induced timing dependent plasticity, and, conversely, may be modulated bidirectionally by prior TMS-induced plasticity, providing circumstantial evidence that long-term potentiation-like mechanisms may be involved in motor learning. In summary, convergent evidence is being accumulated for the contention that it is now possible to induce STDP-like changes in the intact human central nervous system by means of TMS to study and interfere with synaptic plasticity in neural circuits in the context of behavior such as learning and memory. Disruptive behaviour disorders are reflected by a great variety of symptoms ranging from impulsive-hot tempered quarrels to purposeful and goal directed acts of cruelty. A growing body of data indicates that there are neurobiological factors that increase the risk for developing disruptive behaviour disorders. In this review, we give a broad overview of recent studies investigating physiological, neural, genetic factors, and specific neurotransmitter systems. We also discuss the impact of psychosocial risk and consider the effects of gene-environment interactions. Due to the heterogeneity of disruptive behaviour disorders, it is concluded that specific subtypes of disruptive behaviour should be considered both in terms their biological basis and in regard to specific treatment needs.

Tubular carbonate concretions of up to 1 m in length and perpendicular to bedding, occur abundantly in the Upper Pliensbachian (upper Amaltheus margaritatus Zone, Gibbosus Subzone) in outcrops (Fontaneilles section) in the vicinity of Rivière-sûr-Tarn, southern France. Stable isotope analyses of these concretions show negative delta 13C values that decrease from the rim to the center from - 18.8‰ to - 25.7‰ (V-PDB), but normal marine delta 18 O values (- 1.8‰). Carbon isotope analyses of Late Pliensbachian bulk carbonate (matrix) samples from the Fontaneilles section show clearly decreasing C-isotope values across the A. margaritatus Zone, from +1‰ to - 3‰ (V-PDB). Isotope analyses of coeval belemnite rostra do not document such a negative C-isotope trend with values remaining stable around +2‰ (V-PDB). Computer tomographic (CT) scanning of the tubular concretions show multiple canals that are lined or filled entirely with pyrite. Previously, the formation of these concretions with one, two, or more central tubes, has been ascribed to the activity of an enigmatic organism, possibly with annelid or arthropod affinities, known asTisoa siphonalis. Our results suggest tisoan structures are abiogenic. Based on our geochemical analyses and sedimentological observations we suggest that these concretions formed as a combination of the anaerobic oxidation of methane (AOM) and sulfate reduction within the sediment. Fluids rich in methane and/or hydrocarbons likely altered local bulk rock carbon isotope records, but did not affect the global carbon cycle. Interestingly, Tisoa siphonalis has been described from many locations in the Grands Causses Basin in southern France, and from northern France and Luxemburg, always occurring at the same stratigraphic level. Upper Pliensbachian authigenic carbonates thus possibly cover an area of many thousand square kilometers. Greatly reduced sedimentation rates are needed to explain the stabilization of the sulfate-methane transition zone in the sedimentary column in order for the tubular concretions to form. Late Pliensbachian cooling, reducing run-off, and/or the influx of colder water and more vigorous circulation could be responsible for a halt in sedimentation. At the same time (thermogenic) methane may have destabilized during a major phase of Late Pliensbachian sea level fall. As such Tisoa siphonalis is more than a geological curiosity, and its further study could prove pivotal in understanding Early Jurassic paleoenvironmental change.
Background: Abnormalities of 11q23 involving the MLL gene are found in approximately 10% of human leukemias. To date, nearly 100 different chromosome bands have been described in rearrangements involving 11q23 and 64 fusion genes have been cloned and characterized at the molecular level. In this work we present the identification of a novel MLL fusion partner in a pediatric patient with de novo biphenotypic acute leukemia. Methods: Cytogenetics, fluorescence in situ hybridization (FISH), molecular studies (RT-PCR and LDI-PCR), and bioinformatic sequence analysis were used to characterize the CT45A2 gene as novel MLL fusion partner in pediatric acute leukemia. Results: Fluorescence in situ hybridization of the patient G-banded metaphases demonstrated a cryptic insertion of 11q23 in Xq26.3 involving the MLL gene. Breakpoint fusion analysis revealed that a DNA fragment of 653 kb from 11q23, containing MLL exons 1-9 in addition to 16 other 11q23 genes, was inserted into the upstream region of the CT45A2 gene located at Xq26.3. In addition, a deletion at Xq26.3 encompassing the 3' region of the DDX26B gene (exons 9-16) and the entire CT45A1 gene was identified. RNA analysis revealed the presence of a novel MLL-CT45A2 fusion transcript in which the first 9 exons of the MLL gene were fused in-frame to exon 2 of the CT45A2 gene, resulting in a spliced MLL fusion transcript with an intact open reading frame. The resulting chimeric transcript predicts a fusion protein where the N-terminus of MLL is fused to the entire open reading frame of CT45A2. Finally, we demonstrate that all breakpoint regions are rich in long repetitive motifs, namely LINE/L1 and SINE/Alu sequences, but all breakpoints were exclusively identified outside these repetitive DNA sequences. Conclusion: We have identified CT45A2 as a novel spliced MLL fusion partner in a pediatric patient with de novo biphenotypic acute leukemia, as a result of a cryptic insertion of 11q23 in Xq26.3. Since CT45A2 is the first Cancer/Testis antigen family gene found fused with MLL in acute leukemia, future studies addressing its biologic relevance for leukemogenesis are warranted.
Background: The enhancer of zeste homolog 2 (EZH2) gene exerts oncogene-like activities and its (over)expression has been linked to several human malignancies. Here, we studied a possible association between EZH2 expression and prognosis in patients with renal cell carcinoma (RCC). Methods: EZH2 protein expression in RCC specimens was analyzed by immunohistochemistry using a tissue microarray (TMA) containing RCC tumor tissue and corresponding normal tissue samples of 520 patients. For immunohistochemical assessment of EZH2 expression, nuclear staining quantity was evaluated using a semiquantitative score. The effect of EZH2 expression on cancer specific survival (CSS) was assessed by univariate and multivariate Cox regression analyses. Results: During follow-up, 147 patients (28%) had died of their disease, median follow-up of patients still alive was 6.0 years (range 0 - 16.1 years). EZH2 nuclear staining was present in tumor cores of 411 (79%) patients. A multivariate Cox regression analysis revealed that high nuclear EZH2 expression was an independent predictor of poor CSS (>25-50% vs. 0%: HR 2.72, p = 0.025) in patients suffering from non-metastatic RCC. Apart from high nuclear EZH2 expression, tumor stage and Fuhrman's grading emerged as significant prognostic markers. In metastatic disease, nuclear EZH2 expression and histopathological subtype were independent predictive parameters of poor CSS (EZH2: 1-5%: HR 2.63, p = 0.043, >5-25%: HR 3.35, p = 0.013, >25%-50%: HR 4.92, p = 0.003, all compared to 0%: HR 0.36, p = 0.025, respectively). Conclusions: This study defines EZH2 as a powerful independent unfavourable prognostic marker of CSS in patients with metastatic and non-metastatic RCC.
If insufficiently treated, Lyme borreliosis can evolve into an inflammatory disorder affecting skin, joints, and the CNS. Early innate immunity may determine host responses targeting infection. Thus, we sought to characterize the immediate cytokine storm associated with exposure of PBMC to moderate levels of live Borrelia burgdorferi. Since Th17 cytokines are connected to host defense against extracellular bacteria, we focused on interleukin (IL)-17 and IL-22. Here, we report that, despite induction of inflammatory cytokines including IL-23, IL-17 remained barely detectable in response to B. burgdorferi. In contrast, T cell-dependent expression of IL-22 became evident within 10 h of exposure to the spirochetes. This dichotomy was unrelated to interferon-gamma but to a large part dependent on caspase-1 and IL-1 bioactivity derived from monocytes. In fact, IL-1beta as a single stimulus induced IL-22 but not IL-17. Neutrophils display antibacterial activity against B. burgdorferi, particularly when opsonized by antibodies. Since neutrophilic inflammation, indicative of IL-17 bioactivity, is scarcely observed in Erythema migrans, a manifestation of skin inflammation after infection, protective and antibacterial properties of IL-22 may close this gap and serve essential functions in the initial phase of spirochete infection.
Background: Familial hemophagocytosis (FHL) is a rare disease associated with defects in proteins involved in CD8+ T-cell cytotoxicity. Hyperactivation of immune cells results in a perilous, Th1-driven cytokine storm. We set out to explore the regulation of cytokines in an FHL patient who was clinically stable on low-dose immunosuppressive therapy after bone marrow transplantation over a six-month period. During this period, chimerism analyses showed that the fraction of host cells was between 1 and 10%. Both parents of the patient as well as healthy volunteers were studied for comparison. Methods/Principal Findings: Using ELISA, quantitative real-time PCR, and clinical laboratory methods, we investigated constitutive and inducible cytokines, polymorphisms, and clinical parameters in whole blood and whole blood cultures. Although routine laboratory tests were within the normal range, the chemokines IP-10 and IL-8 as well as the cytokine IL-27p28 were increased up to 10-fold under constitutive and stimulated conditions compared to healthy controls. Moreover, high levels of IFNgamma and TNFalpha were produced upon stimulation. Unexpectedly, IFNgamma induction of IL-18 binding protein (IL-18BP) was markedly reduced (1.6-fold vs 5-fold in controls). The patient's mother featured intermediately increased cytokine levels, whereas levels in the father were similar to those in the controls. Conclusions/Significance: Since IL-18 plays a major role in perpetuating hemophagocytosis, the failure of IFNgamma to induce IL-18BP may constitute a fundamental pathogenetic mechanism. Furthermore, increased production of IL-8 and IL-27 appears to be associated with this disease. Such dysregulation of cytokines was also found in the heterozygous parents, providing a novel insight into genotype-phenotype correlation of FHL which may encourage future research of this rare disease.
Purpose. The aim of this prospective longitudinal clinical pilot study was the evaluation of the effect on the Oral Health Impact Profile (OHIP) and patient-centered results of the envelope technique for Connective Tissue Graft (CTG). Methods. Sixteen patients (11 females) 24 to 71 years of age (42.6±11.1) received CTG that had been harvested from the palate and grafted using the envelope technique. Prior to and 3 months after surgery, all patients were examined clinically, completed the OHIP-G49 questionnaire, and were asked to judge the results of surgery. Results. Mean baseline recession depth of 2.5±0.8 mm was reduced by 1.2±0.9 mm (P<.001). Root coverage amounted to 48±39%. In 5 of 16 defects complete root coverage was achieved. Pain at the donor site was more pronounced than at recipient site regarding prevalence (8/6; P=.007), intensity (2.1±2.3/1.1±1.9 [visual analogue scale]; P=.016), and duration (1.4±2.3/0.8±1.4 days; P=.042). Baseline OHIP (15.7±12.1) was decreased by 3.6±8.5 three months after surgery (P=.139). Thirteen patients (81%) would undergo CTG surgery for similar reasons again. Conclusions. Root coverage using CTG according to the envelope technique provided improvement of OHIP as early as 3 months after surgery. Over all, patients were reasonably satisfied with the surgical technique and its results.
Die Gamma-Glutamyl-Carboxylase spielt eine Schlüsselrolle im menschlichen Organismus, da sie für die posttranslationale Modifikation sämtlicher Vitamin-K-abhängiger Proteine verantwortlich ist. Ein hereditärer Mangel aller Vitamin-K-abhängigen Faktoren aufgrund eines Defekts in der Gamma-Glutamyl-Carboxylase (VKCFD1) ist sehr selten und geht mit einer gesteigerten Blutungsneigung einher. Aktuelle Arbeiten zeigen eine weitere Manifestation von Mutationen im Gamma-Glutamyl-Carboxylase-Gen, die ein dermatologisches Krankheitsbild, ähnlich der Pseudoxanthoma elasticum, darstellt. In diesen Fällen zeigen die Patienten sowohl dermatologische Effloreszenzen im Sinne der PXE als auch eine Verminderung der Vitamin-K-abhängigen Faktoren. Es wird vermutet, dass die PXE-ähnlichen Hautveränderungen auf eine Funktionsstörung des Matrix-Gla-Proteins zurückzuführen sein könnten. In der vorliegenden Arbeit wurde eine Mutationsdiagnostik bei vier Patienten mit kongenitalem Mangel aller Vitamin-K-abhängigen Gerinnungsfaktoren durchgeführt. Insgesamt wurden dabei fünf Mutationen nachgewiesen, davon vier Missense-Mutationen und eine Stopp-Mutation. Zwei der Patienten wiesen eine „compound Heterozygotie“ auf. Bei den gefunden Mutationen wurden drei im Rahmen dieser Arbeit erstmals aufgezeigt, die anderen zwei waren in der Literatur bereits beschrieben. Bei einer der Mutationen konnte ein „Founder“-Effekt in Bezug auf einen anderen Patienten aus der Literatur nachgewiesen werden. Sollte zukünftig diese Mutation in Deutschland vorkommen und sich auch auf die „Founder“-Mutation zurückführen lassen, böte sich dieser Mechanismus als eine Erklärung für das gehäufte Vorkommen der VKCFD Typ 1 in Deutschland an. Ein Hinweis auf eine PXE-like Disorder konnte bei keinem der Patienten nachgewiesen werden. Zusätzlich wurde die genetische Variabilität des Gamma-Glutamyl-Carboxylase-Gens in der Normalbevölkerung untersucht. Für die Untersuchungen wurde zunächst die DNA einer phänotypisch unauffälligen Kontrollgruppe, bestehend aus 200 gesunden Blutspendern, auf Genvariationen im GGCX-Gen analysiert. Nach der Isolierung und der Amplifikation der DNA wurde mit sämtlichen Proben zunächst ein Mutations-Screening mittels dHPLC durchgeführt. Auffällige Abschnitte und solche, bei denen häufige und/oder mehrere Polymorphismen bereits bekannt waren, wurden sequenziert. Auf diese Weise wurden acht Polymorphismen in den codierenden bzw. die Exone flankierenden Genabschnitten nachgewiesen. Davon wurden zwei erstmals beschrieben (c.43+33T>A; c.1288-38T>C). Schließlich sollte mit dieser Arbeit versucht werden zu klären, wie eine möglichst effiziente Diagnostik von Mutationen bzw. Polymorphismen des GGCX-Gens möglich ist. In unseren Untersuchungen wurde sowohl mit der dHPLC (Wave-System) als Screening-Methode gearbeitet als auch Genabschnitte sequenziert. Mittels der dHPLC kann ein hohes Probenaufkommen in kurzer Zeit mit vergleichsweise geringem Aufwand und vergleichsweise geringeren Kosten (ca. 30 € pro Genuntersuchung vs. Komplettsequenzierung ca. 300 €) bewältigt werden. Bei Genabschnitten mit vielen oder häufigen Polymorphismen ist dies ineffektiv, weil es nur das Vorhandensein einer Veränderung, nicht jedoch die Veränderung selbst beschreiben kann. Daher empfiehlt es sich, beide Methoden zu kombinieren, welches sich als die effizienteste und rationalste Methode für die Mutationsdiagnostik darstellt. Die Ergebnisse der vorliegenden Arbeit sind von grundsätzlichem Interesse für die biomedizinische Forschung. Eventuell wird es damit dann möglich sein, Krankheitsentstehungen und ihre Verläufe bei Mutationen der Gamma-Glutamyl-Carboxylase besser zu verstehen und damit eine noch effektivere und nebenwirkungsärmere Therapie einzusetzen.
In der vorliegenden Arbeit wurde in einer Fall-Kontroll-Studie mit 483 Fällen und 300 Kontrollen das Risiko für die berufsbedingte Supraspinatussehnenruptur durch Arbeiten auf oder über Schulterniveau untersucht. Dabei ergab sich eine statistisch signifikante Risikoerhöhung durch das Arbeiten auf oder über Schulterniveau. Eine Dosis-Antwortbeziehung zeigte einen Zusammenhang der Ereignisse. Am stärksten war die Risikoerhöhung bei Patienten, die lebenslang kumulativ mindestens 3195 Stunden auf oder über Schulterniveau arbeiteten. Die adjustierte Odds Ratio dafür betrug 2,1 (CI 1,2-3,7), das Ergebnis ist statistisch signifikant. Neben dem beruflichen Arbeiten auf oder über Schulterniveau gibt es andere Faktoren, die die Entstehung einer Rotatorenmanschettenruptur begünstigen. Dabei sind Alter, Heben/Tragen von Lasten über 20 kg und Arbeiten mit handgeführten vibrierenden Werkzeugen zu nennen. Deshalb wurden die Daten hinsichtlich dieser Faktoren adjustiert. Auch einige Sportaktivitäten können das Risiko für die Rotatorenmanschettenruptur erhöhen. Deshalb wurden die Daten auch für Geräteturnen, Kugelstoßen, Speer- und Hammerwerfen, Ringen und Tennis adjustiert, und die sportliche Anstrengung wurde als Confounder behandelt. Die Ergebnisse stehen im Einklang mit Ergebnissen früherer Studien, die ebenfalls eine Risikoerhöhung für die Entstehung der Rotatorenmanschettenruptur durch Arbeiten auf oder über Schulterniveau zeigen. Eine genaue Dosis, die zu dieser Risikoerhöhung führt, wurde in der Literatur bisher nicht genannt. Die vorliegende Arbeit liefert die Evidenz für die Annahme eines erhöhten Risikos der Ruptur der Sehne des M. supraspinatus bei Arbeiten auf oder über Schulterniveau. Weiterhin besteht ein Potential für eine primäre Prävention, indem Arbeiten auf oder über Schulterniveau möglichst ganz vermieden wird. Daher sollte die Ruptur der Sehne des M. supraspinatus als eigene Berufskrankheit in die Liste der Berufskrankheiten aufgenommen und die Betroffenen bei kumulativem Arbeiten von mindestens 3200 Stunden auf oder über Schulterniveau entschädigt werden.
Zur Korrektur der Myopie bietet die refraktive Chirurgie prinzipiell die Möglichkeiten entweder die Kornea zu modulieren oder eine phake Intraokularlinse einzusetzen. Bei der Beseitigung einer hochgradigen Myopie von über 6 dpt haben sich jedoch die pIOL durchgesetzt. Besonders irisfixierte Linsen weisen sehr gute Refraktionsergebnisse bei einem geringen Komplikationsprofil auf. Zwei Modelle der Artisan-Linse (optische Zone 5 mm n = 48, optische Zone 6 mm n = 264) wurden in 312 Augen von 176 hochgradig myopen Probanden in der Universitätsaugenklinik Frankfurt am Main eingesetzt. Die Untersuchung und Auswertung erfolgte hinsichtlich der Refraktionsqualität und Komplikationsrate. Zur Beurteilung der Refraktion wurden die Standardparameter der refraktiven Chirurgie (Sicherheit, Wirksamkeit, Vorhersagbarkeit und Stabilität) gemessen und analysiert. Bei Erhebung und Analyse des Komplikationsprofils wurde besonderes Augenmerk auf den Verlust der EZD gerichtet. Die Untersuchungen wurden präoperativ, nach einem Tag, einer Woche, einem Monat und anschließend im Jahresrhythmus durchgeführt. Der präoperative mittlere BKSM von 0,82 wurde bei keiner Folgeuntersuchung unterschritten, so dass der Sicherheitsindex stets über 1 betrug. Kein Patient verlor mehr als eine Zeile beim BKSM. Ein UKSM von 0,8 oder mehr wurde nach einem Jahr in 78%, nach zwei Jahren in 81%, nach drei Jahren in 76%, nach vier Jahren in 60% und nach fünf Jahren in 65% der Fälle beobachtet. Im Vergleich dazu betrug der präoperative BKSM bei 78% der Augen 0,8 oder mehr. Die Vorhersagbarkeit erwies sich innerhalb des gesamten Beobachtungszeitraums als sehr hoch. Bei vier der fünf Jahreskontrollen lagen alle Augen (98% bei der 3-Jahresuntersuchung) innerhalb einer Spannweite von ± 2 dpt und stets mindestens 86% der Augen innerhalb von ± 1 dpt der angestrebten refraktiven Korrektur. Das mittlere sphärische Äquivalent betrug präoperativ -11,24 ±4,46 dpt. Einen Monat nach der Implantation der irisfixierten Linse ergab sich ein Wert von -0,54 ±0,55 dpt. Es kam zu keinen größeren Veränderungen der Refraktion während des Nachbeobachtungszeitraums (Minimum: -0,78 dpt nach fünf Jahren; Maximum: -0,41 dpt nach einem Jahr). Im Mittel betrug die präoperative Endothelzellzahl 2751 ±331 Zellen/mm2 (2004 bis 3508) und verringerte sich durchschnittlich um 2,6% / Jahr auf 2387 ±322 Zellen/mm2 (1605 bis 3164) nach fünf Jahren. Bei Auswertung der EZD der konsekutiven Kohorte wurde an zwei Augen ein Verlust von über 30% registriert (34% bzw. 38%). An keinem Auge wurde ein intrastromales Korneaödem oder eine Trübung der Hornhaut festgestellt. Eine signifikante Assoziation zwischen Pigmentdispersion und erhöhtem intraokularem Druck konnte beobachtet werden (p = 0,047). Schwere Nebenwirkungen wie eine chronische Entzündung oder Glaukom sind nicht aufgetreten. An 9 Augen mußte eine Reenklavation erfolgen, ursächlich hierfür war in einem Fall eine Dezentrierung, in einem anderen eine Iritis und in den übrigen Fällen zu wenig eingehaktes Irisgewebe in den Linsenhaptiken. Eine Explantation war an keinem Auge indiziert. Um ein besseres Refraktionsergebnis zu erzielen, wurde in 21 Augen mittels LASIK und in 9 Augen mittels LRI nachkorrigiert. Diese Langzeitergebnisse der irisfixierten Linse zur Korrektur einer hochgradigen Myopie erweisen sich hinsichtlich Sicherheit, Wirksamkeit, Vorhersagbarkeit und Stabilität als sehr gut. Im Vergleich zum natürlichen Verlust der EZD ist dieser nach Einsatz der pIOL signifikant erhöht, so dass bei jüngeren Patienten strengere Ausschlusskriterien eingeführt werden sollten. Insgesamt erweist sich aber die irisfixierte Linse als eine sehr sichere und komplikationsarme Option zur Beseitigung einer hochgradigen Myopie.
One of the earliest and most striking observations made about HIV is the extensive genetic variation that the virus has within individual hosts, particularly in the hypervariable regions of the env gene which is divided into 5 variable regions (V1-V5) and 5 more constant (C1-C5) regions. HIV evolves at any time over the course of an individual’s infection and infected individuals harbours a population of genetically related but non-identical viruses that are under constant change and ready to adapt to changes in their environment. These genetically heterogeneous populations of closely related genomes are called quasispecies [65]. Tuberculosis or tubercle forming disease is an acute and/or chronic bacterial infection that primarily attacks the lungs, but which may also affect the kidneys, bones, lymph nodes, and brain. The disease is caused by Mycobacterium tuberculosis (MTB), a slow growing rod-shaped, acid fast bacterium. It is transmitted from person to person through inhalation of bacteria-carrying air droplets. Worldwide, one person out of three is infected with Mycobacterium tuberculosis – two billion people in total. TB currently holds the seventh place in the global ranking of causes of death [73]. In 2008, there were an estimated 9.4 (range, 8.9–9.9 million) million incident cases (equivalent to 139 cases per 100 000 population) of TB globally [75]. A complex biological interplay occurs between M. tuberculosis and HIV in coinfected host that results in the worsening of both pathologies. HIV promotes progression of M. tuberculosis either by endogenous reactivation or exogenous reinfection [77, 78] and, the course of HIV-1 infection is accelerated subsequent to the development of TB [80]. Active TB is associated with an increase in intra-patient HIV-1 diversity both systemically and at the infected lung sites [64,122]. The sustainability or reversal of the HIV-1 quasispecies heterogeneity after TB treatment is not known. Tetanus toxoid vaccinated HIV-1 infected patients developed a transient increase in HIV-1 heterogeneity which was reversed after few weeks [121]. Emergence of a heterogeneous HIV-1 population within a patient may be one of the mechanisms to escape strong immune or drug pressure [65,128]. The existence of better fitting and/or immune escape HIV-variants can lead to an increase in HIV-1 replication [129,130]. It might be that TB favourably selected HIV-1 variants which are sources for consistent HIV-1 replication. Understanding the mechanisms underlying the impacts of TB on HIV-1 is essential for the development of effective measures to reduce TB related morbidity and mortality in HIV-1 infected individuals. In the present study we studied whether the increase in HIV-1 quasispecies diversity during active TB is reversed or preserved throughout the course of antituberculous chemotherapy. For this purpose Two time point HIV-1 quasispecies were evaluated by comparing HIV-1 infected patients with active tuberculosis (HIV-1/TB) and HIV-1 infected patients without tuberculosis (HIV-1/non TB). Plasma samples were obtained from the Frankfurt HIV cohort and HIV-1 RNA was isolated. C2V5 env was amplified by PCR and molecular cloning was performed. Eight to twenty five clones were sequenced from each patient. Various phylogenetic analyses were performed including tree inferences, intra-patient viral diversity and divergence, selective pressure, co-receptor usage prediction and two time point identity of quasispecies comparison using Mantel’s test. We found out from this study that: 1) Active TB sustains HIV-1 quasispecies diversity for longer period 2. Active TB increases the rate of HIV-1 divergence 3) TB might slow down evolution of X4 variants And we concluded that active TB has an impact on HIV-1 viral diversity and divergence over time. The influence of active TB on longitudinal evolution of HIV- 1 may be predominant for R5 viruses. The use of CCR5-coreceptor inhibitors for HIV-1/TB patients as therapeutic approach needs further investigation.
Background: New drugs are constantly sought after to improve the survival of patients with malignant gliomas. The ideal substance would selectively target tumor cells without eliciting toxic side effects. Here, we report on the anti-proliferative, anti-migratory, and anti-invasive properties of the natural, nontoxic compound Curcumin observed in five human glioblastoma (GBM) cell lines in vitro. Methods: We used monolayer wound healing assays, modified Boyden chamber trans-well assays, and cell growth assays to quantify cell migration, invasion, and proliferation in the absence or presence of Curcumin at various concentrations. Levels of the transcription factor phospho-STAT3, a potential target of Curcumin, were determined by sandwich-ELISA. Subsequent effects on transcription of genes regulating the cell cycle were analyzed by quantitative real-time PCR. Effects on apoptosis were determined by caspase assays. Results: Curcumin potently inhibited GBM cell proliferation as well as migration and invasion in all cell lines contingent on dose. Simultaneously, levels of the biologically active phospho-STAT3 were decreased and correlated with reduced transcription of the cell cycle regulating gene c-Myc and proliferation marking Ki-67, pointing to a potential mechanism by which Curcumin slows tumor growth. Conclusions: Curcumin is part of the diet of millions of people every day and is without known toxic side effects. Our data show that Curcumin bears anti-proliferative, anti-migratory, and anti-invasive properties against GBM cells in vitro. These results warrant further in vivo analyses and indicate a potential role of Curcumin in the treatment of malignant gliomas.
Thema der vorliegenden Studie ist die Evaluation der Betazellfunktion bei Patienten mit Diabetes mellitus Typ 2, für deren Bestimmung es bisher keine einfach anwendbare Methode gibt. Die Clamp-Methode erfordert eine kontinuierliche Überwachung und ist daher nicht geeignet für die Anwendung in der täglichen Klinikroutine. Nateglinide ist ein Derivat der Aminosäure D-Phenylalanin, das durch direkte Wirkung an den Betazellen der Langerhanns’schen Inseln des Pankreas die Insulinsekretion vor allem in der frühen Phase der Freisetzung stimuliert. Ziel der Arbeit war die Überprüfung eines Nateglinide-stimulierten oralen Glucosetole-ranztests als Instrument zur ambulanten Bestimmung der stimulierbaren Betazellkapa-zität sowie sekundär die Bestimmung von Grenzwerten für Blutzucker und Insulinsek-retion für die Entscheidung der Therapieeskalation einer oralen Therapie hin zu einer Insulin(unterstützten) Therapie. Im Rahmen dieser Studie führten wir an 30 gesunden Kontrollpersonen und 45 Patien-ten mit Diabetes mellitus Typ 2, die entweder diätetisch oder mit oralen Antidiabetika behandelt wurden, einen modifizierten oralen Glucosetoleranztest (50g) von dreistün-diger Dauer durch, der mit der Gabe von 120mg Nateglinide kombiniert wurde. Es erfolgte die Bestimmung der Basalwerte von Glucose, Insulin und C-Peptid sowie der Spiegel nach 30, 60, 120 und 180 Minuten. Aus den Ergebnissen wurden der HOMA-IR, HOMA-ß-cell-Index sowie die AUC von Insulin, C-Peptid und Glucose bestimmt. Die gesunden Kontrollpersonen wiesen zu allen Testzeitpunkten signifikant niedrigere Glucosewerte auf als die Patienten mit Typ-2-Diabetes (Maximum Glucose: 125,05±25,97 vs. 234,78±64,58mg/dl, p<0,001 und AUC Glucose: 15197,25±2798,9 vs. 34022,05±11142,17mg/dl, p=0,02), die zudem ihr Blutzuckermaximum zu einem späteren Testzeitpunkt erreichten (35,25±17,61 vs. 63,33±31,76 min, p<0,001). Zum Testzeitpunkt 120 Minuten konnte bei 18 Patienten mit Typ-2-Diabetes ein norm-wertiger Blutglucosespiegel <120mg/dl bestimmt werden, 7 hatten Werte vereinbar mit einer IGT und 20 wiesen diabetische Blutglucosespiegel >200mg/dl auf. Es fiel eine signifikante Korrelation vom Zeitpunkt der maximalen Insulinsekretion und dem Blutglucosemaximum auf. Je später der Zeitpunkt der maximalen Insulin- bzw. C-Peptid-Sekretion erreicht wird, desto höher ist die maximale Blutglucosekonzentration. Adipöse Patienten (BMI >30kg/m2) hatten während des gesamten Testverlaufs höhere Blutglucosewerte und eine geringere stimulierbare Insulinsekretionsleistung als die Patienten mit einem niedrigeren BMI. Die untersuchten Patienten mit Typ-2-Diabetes lassen sich in drei Gruppen unterteilen: eine mit vorherrschendem Insulinsekretionsdefizit (Maximum C-Peptid <4ng/ml, n=6), eine mit führender Insulinresistenz (HOMA-IR>2,5, n=16) und eine Gruppe mit einer Kombination aus beiden Störungen (n=3). Die Patienten mit kombinierter Störung hat-ten einen höheren HbA1c als die anderen Gruppen (10,2%±2,38 vs. 6,66%±0,46 und 6,91%±1,24, n.s.) und wiesen über den gesamten Testverlauf höhere Blutglucosewerte auf. Mit dem Ziel, eine Hilfestellung bei der Entscheidung einer Therapieintensivierung bzw. –umstellung auf eine Insulintherapie zu geben, wurde ein Score aufgestellt, der die Parameter Blutzuckerspiegel zum Testzeitpunkt 120 min, maximaler C-Peptidspiegel, HOMA-IR sowie den Steigerungsfaktor von basalem zu maximalem C-Peptidspiegel umfasst. Beurteilt anhand der dort erreichten Punktzahl zeigen einerseits 23 der unter-suchten Patienten ein suffizientes Ansprechen auf die orale antihyperglykämische The-rapie, was die Fortführung der oralen Diabetestherapie unterstützt. Andererseits er-reichten auch 6 Patienten einen Punktebereich, bei dem von einer unzureichenden Betazellsekretion auszugehen ist und somit ein hohes Risiko für ein Sekundärversagen der oralen Diabetestherapie besteht, so dass eine Therapieumstellung auf eine Insu-lin(unterstützte) Therapie empfohlen wird. Basierend auf der limitierten Anzahl an Daten, die wir erhoben haben, empfehlen wir die Umstellung auf eine Insulintherapie, wenn im Nateglinide-Test mindestens drei der folgenden vier Punkte zutreffen: • Blutglucose zum Zeitpunkt 120min >200mg/dl • HOMA-IR >4 • Maximales C-Peptid <4ng/ml • Stimulierte C-Peptid-Steigerung <2fach Die hiesige Studie zeigt, dass der Nateglinide-Test ein in der täglichen Klinikroutine praktikables Instrument zur Evaluation der residualen Betazellfunktion darstellt. Er er-möglicht die gleichzeitige Beurteilung der noch vorhandenen Insulinsekretionskapazität und der Insulinresistenz bei Patienten mit Typ-2-Diabetes. In Kombination mit dem vorgestellten Score kann er helfen, eine Progression des Diabetes mellitus Typ 2 zu verhindern, indem der Zeitpunkt für den Beginn einer Insulintherapie genauer definiert wird.
Background: Complex care management is seen as an approach to face the challenges of an ageing society with increasing numbers of patients with complex care needs. The Medical Research Council in the United Kingdom has proposed a framework for the development and evaluation of complex interventions that will be used to develop and evaluate a primary care-based complex care management program for chronically ill patients at high risk for future hospitalization in Germany. Methods and design: We present a multi-method procedure to develop a complex care management program to implement interventions aimed at reducing potentially avoidable hospitalizations for primary care patients with type 2 diabetes mellitus, chronic obstructive pulmonary disease, or chronic heart failure and a high likelihood of hospitalization. The procedure will start with reflection about underlying precipitating factors of hospitalizations and how they may be targeted by the planned intervention (pre-clinical phase). An intervention model will then be developed (phase I) based on theory, literature, and exploratory studies (phase II). Exploratory studies are planned that entail the recruitment of 200 patients from 10 general practices. Eligible patients will be identified using two ways of 'case finding': software based predictive modelling and physicians' proposal of patients based on clinical experience. The resulting subpopulations will be compared regarding healthcare utilization, care needs and resources using insurance claims data, a patient survey, and chart review. Qualitative studies with healthcare professionals and patients will be undertaken to identify potential barriers and enablers for optimal performance of the complex care management program. Discussion: This multi-method procedure will support the development of a primary care-based care management program enabling the implementation of interventions that will potentially reduce avoidable hospitalizations.
Das Ziel dieser retrospektiven Untersuchung ist, die medikamentöse Therapie des neonatalen Entzugsyndroms mittels Clonidin und Chloralhydrat mit der sehr häufig angewendeten Kombinationstherapie aus Morphin und Phenobarbital zu vergleichen. Im dem Zeitraum zwischen 1998 und 2008 wurden in unserer Klinik 133 Neugeborene mit einem neonatalen Entzugssyndrom behandelt. Bei allen Patienten handelte es sich um Neugeborene drogenabhängiger Mütter, welche während der Schwangerschaft mit Methadon substituiert wurden. Bei 29 Patienten wurde eine Therapie mit Clonidin und Chloralhydrat und bei 64 Patienten eine Entzugstherapie mittels Morphin und Phenobarbital durchgeführt. In der Clonidingruppe konnte eine signifikant kürzere Behandlungsdauer (Median: 14 d vs. 35 d) festgestellt werden. Entsprechend war auch die gesamte Aufenthaltsdauer in der Clonidingruppe deutlich kürzer (Median: 32 d vs. 44 d). Außerdem konnte in der Clonidingruppe ein deutlich milderer Verlauf der Entzugssymptomatik festgestellt werden.
Background: Prospective, observational studies that enroll large numbers of patients with few exclusion criteria may better reflect actual ongoing clinical experience than randomized clinical trials. Our purpose was to obtain efficacy and safety information from a cohort of subjects exposed to latanoprost/timolol fixed combination (FC) for [greater than or equal to]18 months using a prospective, observational design. Methods: In all, 577 office-based ophthalmologists in Germany switched 2339 patients with glaucoma or ocular hypertension to latanoprost/timolol FC for medical reasons. Follow-up visits were scheduled for every 6 months over 24 months; physicians followed usual care routines. Intraocular pressure (IOP), visual field status, optic nerve head findings, and adverse events were recorded. Efficacy parameters were evaluated for the per protocol (PP) population; the safety population included subjects receiving [greater than or equal to]1 drop of FC. Physicians rated efficacy, tolerability, and subject compliance at month 24. Results: Of the 2339 subjects switched to latanoprost/timolol FC (safety population), the primary reasons for switching were inadequate IOP reduction (78.2%) and desire to simplify treatment with once-daily dosing (29.4%; multiple reasons possible). In all, 1317 (56.3%) subjects completed the study, and 1028 (44.0%) were included in the PP population. Most discontinuations were due to loss to follow-up. Change in mean IOP from baseline to month 6 was -4.0 +/- 4.31 mmHg, a reduction that was maintained throughout (P<0.05 for change at all time points). By investigator assessments, optic disc parameters and visual field were stable over 24 months, and there was no relationship between IOP reduction over 24 months and development of a visual field defect. More than 90% of physicians rated latanoprost/timolol FC as "very good" or "good" for efficacy (PP population), tolerability, and compliance. The FC was safe and well tolerated. No change in iris color was reported by most subjects (83.1%) at month 24. Conclusions: Over 24 months, latanoprost/timolol FC effectively lowers IOP levels and is well tolerated in patients with glaucoma or ocular hypertension who change from their previous ocular hypotensive therapy for medical reasons. Investigator assessments found optic disc parameters and visual field to be stable throughout 24 months of follow-up.
Background: FibroTest (FT) is the most frequently used serum fibrosis marker and consists of an algorithm of five fibrosis markers (alfa2-macroglobulin, apolipoproteinA1, haptoglobin, GGT, bilirubin). The Enhanced Liver Fibrosis (ELF) test consists of an algorithm of three fibrosis markers (hyaluronic acid, amino-terminal propeptide-of-type-III-collagen, tissue-inhibitor of matrix-metaloproteinase-1). While a systematic review has shown comparable results for both individual markers, there has been no direct comparison of both markers. Methods: In the present study, the ELF-test was analyzed retrospectively in patients with chronic liver disease, who received a liver biopsy, transient elastography (TE) and the FibroTest using histology as the reference method. Histology was classified according to METAVIR and the Ludwig's classification (F0-F4) for patients with chronic hepatitis C and B virus (HCV, HBV) infection and primary biliary cirrhosis (PBC), respectively. Results: Seventy-four patients were analysed: 36 with HCV, 10 with HBV, and 28 with PBC. The accuracy (AUROC) for the diagnosis of significant fibrosis (F[greater than or equal to]2) for ELF and FibroTest was 0.78 (95%CI:0.67-0.89) and 0.69 (95%-CI:0.57-0.82), respectively (difference not statistically significant, n.s.). The AUROC for the diagnosis of liver cirrhosis was 0.92 (95%CI:0.83-1,00), and 0.91 (95%CI:0.83-0.99), respectively (n.s.). For 66 patients with reliable TE measurements the AUROC for the diagnosis of significant fibrosis (cirrhosis) for TE, ELF and FT were 0.80 (0.94), 0.76 (0.92), and 0.67 (0.91), respectively (n.s.). Conclusion: FibroTest and ELF can be performed with comparable diagnostic accuracy for the non-invasive staging of liver fibrosis. Serum tests are informative in a higher proportion of patients than transient elastography.
Epoxyeicotrienoic acids (EETs) are cytochrome P450-dependent anti-hypertensive and anti-inflammatory derivatives of arachidonic acid, which are highly abundant in the kidney and considered reno-protective. EETs are degraded by the enzyme soluble epoxide hydrolase (sEH) and sEH inhibitors are considered treatment for chronic renal failure (CRF). We determined whether sEH inhibition attenuates the progression of CRF in the 5/6-nephrectomy model (5/6-Nx) in mice. 5/6-Nx mice were treated with a placebo, an ACE-inhibitor (Ramipril, 40 mg/kg), the sEH-inhibitor cAUCB or the CYP-inhibitor fenbendazole for 8 weeks. 5/6-Nx induced hypertension, albuminuria, glomerulosclerosis and tubulo-interstitial damage and these effects were attenuated by Ramipril. In contrast, cAUCB failed to lower the blood pressure and albuminuria was more severe as compared to placebo. Plasma EET-levels were doubled in 5/6 Nx-mice as compared to sham mice receiving placebo. Renal sEH expression was attenuated in 5/6-Nx mice but cAUCB in these animals still further increased the EET-level. cAUCB also increased 5-HETE and 15-HETE, which derive from peroxidation or lipoxygenases. Similar to cAUCB, CYP450 inhibition increased HETEs and promoted albuminuria. Thus, sEH-inhibition failed to elicit protective effects in the 5/6-Nx model and showed a tendency to aggravate the disease. These effects might be consequence of a shift of arachidonic acid metabolism into the lipoxygenase pathway.

Ob mit dem Skalpell, durch scharfsinnige Beobachtung oder Hightech-Laboranalytik: Spezialisierte Rechtsmediziner können einen Tathergang anhand von Blutspurenverteilungsmustern rekonstruieren, Toxikologen messen im Blut betäubende oder giftige Substanzen, Molekularbiologen ordnen Blutspuren über DNA-Profi le Personen zu und versuchen, mit molekulardiagnostischen Methoden unklare Todesursachen aufzuklären. Zwei konstruierte Todesfälle gewähren einen forensischen Blick auf das Blut.
Brustkrebs ist die häufigste Krebserkrankung und Todesursache bei Frauen. Die Forschung der letzten Jahrzehnte hat gezeigt, dass es sich dabei nicht um eine einzelne, immer gleich verlaufende Erkrankung handelt. Vielmehr geht man heute davon aus, dass Brustkrebs eine heterogene Erkrankung mit verschiedenen Subtypen darstellt. Sie lassen sich klinisch und molekular deutlich von einander unterscheiden. Wichtiges Ziel der modernen Forschung und ihrer Methoden ist daher die Entwicklung einer individuellen Therapie für jede einzelne Patientin.

Heute werden viele Leukämiekranke geheilt. An der Goethe-Universität setzt ein Schwerpunkt für Lymphom- und Leukämieforschung deutschlandweit Akzente bei Forschung und Diagnostik.
Schlangengifte enthalten einen ganzen Arzneischrank voller hochwirksamer Stoffe, die binnen kurzer Zeit zu Schock, Lähmung oder unstillbaren Blutungen führen können. Forscher interessieren sich vor allem für Verbindungen, welche die Blutgerinnung hemmen, denn sie könnten pharmakologisch interessante Leitstrukturen für neue Wirkstoffe sein, die das Risiko eines Herzinfarkts, Gehirnschlags und anderer Thrombosen mindern.
Introduction: Purpura fulminans (PF) is a devastating complication of uncontrolled systemic inflammation, associated with high incidence of amputations, skin grafts and death. In this study, we aimed to clarify the clinical profile of pediatric patients with PF who improved with protein C (PC) treatment, explore treatment effects and safety, and to refine the prognostic significance of protein C plasma levels. Methods: In Germany, patients receiving protein C concentrate (Ceprotin(R), Baxter AG, Vienna, Austria) are registered. The database was used to locate all pediatric patients with PF treated with PC from 2002 to 2005 for this National, retrospective, multi-centered study. Results: Complete datasets were acquired in 94 patients, treated in 46 centers with human, non-activated protein C concentrate for purpura fulminans. PC was given for 2 days (median, range 1-24 days) with a median daily dose of 100 IU/kg. Plasma protein C levels increased from a median of 27% to a median of 71% under treatment. 22.3% of patients died, 77.7% survived to discharge. Skin grafts were required in 9.6%, amputations in 5.3%. PF recovered or improved in 79.8%, remained unchanged in 13.8% and deteriorated in 6.4%. Four adverse events occurred in 3 patients, none classified as severe. Non-survivors had lower protein C plasma levels (P < 0.05) and higher prevalence of coagulopathy at admission (P < 0.01). Time between admission and start of PC substitution was longer in patients who died compared to survivors (P = 0.03). Conclusions: This retrospective dataset shows that, compared to historic controls, only few pediatric patients with PF under PC substitution needed dermatoplasty and/or amputations. Apart from epistaxis, no bleeding was observed. Although the data comes from a retrospective study, the evidence we present suggests that PC had a beneficial impact on the need for dermatoplasty and amputations, pointing to the potential value of carrying out a prospective randomised controlled trial.
Although autism spectrum disorders (ASDs) have a substantial genetic basis, most of the known genetic risk has been traced to rare variants, principally copy number variants (CNVs). To identify common risk variation, the Autism Genome Project (AGP) Consortium genotyped 1558 rigorously defined ASD families for 1 million single-nucleotide polymorphisms (SNPs) and analyzed these SNP genotypes for association with ASD. In one of four primary association analyses, the association signal for marker rs4141463, located within MACROD2, crossed the genome-wide association significance threshold of P < 5 × 10−8. When a smaller replication sample was analyzed, the risk allele at rs4141463 was again over-transmitted; yet, consistent with the winner's curse, its effect size in the replication sample was much smaller; and, for the combined samples, the association signal barely fell below the P < 5 × 10−8 threshold. Exploratory analyses of phenotypic subtypes yielded no significant associations after correction for multiple testing. They did, however, yield strong signals within several genes, KIAA0564, PLD5, POU6F2, ST8SIA2 and TAF1C.
Background: Thymostimulin is a thymic peptide fraction with immune-mediated cytotoxicity against hepatocellular carcinoma (HCC) in vitro and palliative efficacy in advanced HCC in two independent phase II trials. The aim of this study was to assess the efficacy of thymostimulin in a phase III trial. Methods: The study was designed as a prospective randomised, placebo-controlled, double-blind, multicenter clinical phase III trial. Between 10/2002 and 03/2005, 135 patients with locally advanced or metastasised HCC (Karnofsky [greater than or equal to]60% / Child-Pugh [less than or equal to]12) were randomised to receive thymostimulin 75 mg s.c. 5x/week or placebo stratified according to liver function. Primary endpoint was twelve-month survival, secondary endpoints overall survival (OS), time to progression (TTP), tumor response, safety and quality of life. A subgroup analysis according to liver function, KPS and tumor stage (Okuda, CLIP and BCLC) formed part of the protocol. Current Controlled Trials ISRCTN64487365. Results: Twelve-month survival was 28% [95%CI 17-41; treatment] and 32% [95%CI 19-44; control] with no significant differences in median OS (5.0 [95% CI 3.7-6.3] vs. 5.2 [95% CI 3.5-6.9] months; p=0.87, HR=1.04 [95% CI 0.7-1.6]) or TTP (5.3 [95%CI 2.0-8.6] vs. 2.9 [95%CI 2.6-3.1] months; p=0.60, HR=1.13 [95% CI 0.7-1.8]). Adjustment for liver function, Karnofsky status or tumor stage did not affect results. While quality of life was similar in both groups, fewer patients on thymostimulin suffered from accumulating ascites and renal failure. Conclusions: In our phase III trial, we found no evidence of any benefit to thymostimulin in the treatment of advanced HCC and there is therefore no justification for its use as single-agent treatment. The effect of thymostimulin on hepato-renal function requires further confirmation. trial registration: Current Controlled Trials ISRCTN64487365
Background: Reactive oxygen species (ROS) and reactive nitrogen species (RNS) are produced during hemorrhagic shock and resuscitation (H/R), which may contribute to multiple organ failure. The AIM of this study was to test the hypothesis that green tea (Camellia sinenesis) extract containing 85% polyphenols decreases injury after H/R in rats by scavenging ROS and RNS. Method: S: Female Sprague Dawley rats were given 100 mg polyphenol extract/kg body weight or vehicle 2 h prior to hemorrhagic shock. H/R was induced by two protocols: 1) withdrawal of blood to a mean arterial pressure of 40 mm Hg followed by further withdrawals to decrease blood pressure progressively to 28 mm Hg over 1 h (severe), and 2) withdrawal of blood to a sustained hypotension of 40 mm Hg for 1 h (moderate). Rats were then resuscitated over 1 h with 60% of the shed blood volume plus twice the shed blood volume of lactated Ringer's solution. Serum samples were collected at 10 min and 2 h after resuscitation. At 2 or 18 h, livers were harvested for cytokine and 3-nitrotyrosine quantification, immunohistochemical detection of 4-hydroxynonenol (4-HNE) and inducible nitric oxide synthase (iNOS) protein expression. Results: After severe H/R, 18-h survival increased from 20% after vehicle to 70% after polyphenols (p<0.05). After moderate H/R, survival was greater (80%) and not different between vehicle and polyphenols. In moderate H/R, serum alanine aminotransferase (ALT) increased at 10 min and 2 h postresuscitation to 345 and 545 IU/L, respectively. Polyphenol treatment blunted this increase to 153 and 252 IU/L at 10 min and 2 h (p<0.01). Polyphenols also blunted increases in liver homogenates of TNFalpha (7.0 pg/mg with vehicle vs. 4.9 pg/mg with polyphenols, p<0.05), IL-1beta (0.80 vs. 0.37 pg/mg, p<0.05), IL-6 (6.9 vs. 5.1 pg/mg, p<0.05) and nitrotyrosine (1.9 pg/mg vs. 0.6 pg/mg, p<0.05) measured 18 h after H/R. Hepatic 4-HNE immunostaining indicative of lipid peroxidation also decreased from 4.8% after vehicle to 1.5% after polyphenols (p<0.05). By contrast, polyphenols did not block increased iNOS expression at 2 h after H/R. CONCLUSION: Polyphenols decrease ROS/RNS formation and are beneficial after hemorrhagic shock and resuscitation.
Background: Because Endomyocardial Biopsy has low sensitivity of about 20%, it can be performed near to myocardium that presented as Late Gadolinium Enhancement (LGE) in cardiovascular magnetic resonance (CMR). However the important issue of comparing topography of CMR and histological findings has not yet been investigated. Thus the current study was performed using an animal model of myocarditis. Results: In 10 male Lewis rats Experimental Autoimmune myocarditis was induced, 10 rats served as control. On day 21 animals were examined by CMR to compare topographic distribution of LGE to histological inflammation. Sensitivity, specificity, positive and negative predictive values for LGE in diagnosing myocarditis were determined for each segment of myocardium. Latter diagnostic values varied widely depending on topographic distribution of LGE and inflammation as well as on the used CMR sequence. Sensitivity of LGE was up to 76% (left lateral myocardium) and positive predictive values were up to 85% (left lateral myocardium), whereas sensitivity and positive predictive value dropped to 0 - 33% (left inferior myocardium). Conclusions: Topographic distribution of LGE and histological inflammation seem to influence sensitivity, specifity, positive and negative predictive values. Nevertheless, positive predictive value for LGE of up to 85% indicates that Endomyocardial Biopsy should be performed "MR-guided". LGE seems to have greater sensitivity than Endomyocardial Biopsy for the diagnosis of myocarditis.
Die vorliegende Dissertationsschrift befasst sich mit der molekulargenetischen Analyse zweier Basalganglienerkrankungen. Zum einen wurden Patienten mit M. Parkinson genetisch untersucht, zum anderen Patienten mit autosomal dominanter zervikaler Torsionsdystonie. Die Aufgabe bestand in der passenden Wahl der Methode zur jeweiligen humangenetischen Fragestellung. Der erste Teil handelte von der Suche der krankheitsverursachenden Mutation für die autosomal dominante zervikale Torsionsdystonie mit Spätmanifestation auf Chromosom 18p (Kandidatenlokus DYT7). Die erkrankte Familie deutscher Herkunft zeigt dystone Symptome mit Betonung auf kraniozervikale und brachiale Körperabschnitte und ist somit die weltweit einzige bekannte Familie mit Vererbung dieser ansonsten sporadisch auftretenden Erkrankung. Die PCR-Sequenzierung der Kandidatengene ZFP161, LOC390828, NDUFV2 und PTPRM auf dem DYT7 Lokus erbrachte bei den sieben erkrankten Familienmitgliedern im Vergleich zu nicht verwandten Kontrollen (Ehepartner und 96 Kontrollen der Blutbank) keinen Aminosäureaustausch, der ausschließlich bei den erkrankten Probanden zu finden war. Technisch konzentrierte sich diese Untersuchung auf die Amplifizierung und anschließende Sequenzierung jedes einzelnen Exons in den zu untersuchenden Proben, und die Bestätigung einer putativen Mutation mittels Verdau der PCR-Produkte durch Restriktionsendonukleasen. Die Auswahl der Kandidatengene erfolgte aufgrund der Annahme pathobiochemischer Mechanismen, die durch andere Formen der vererbten Torsionsdystonie oder zellbiologische Experimente als krankheitsverursachend gelten. Auch wenn keine Mutation gefunden wurde, so konnten bereits bekannte und neue single nucleotide polymorphisms (SNP) etabliert werden. Die zweite Thematik befasste sich mit der Frage, ob das bereits bekannte Parkinson-Gen UCH-L1 auf dem PARK5 Lokus krankheitsverursachend für den autosomal dominanten M. Parkinson in einer spanischen Familie ist. Diese parametrische Kopplungsanalyse wurde mithilfe der heißen Polyacrylamid-Gelelektrophorese (PAGE) durchgeführt. Dabei konnte in allen Patienten und den Verwandten ersten Grades über Analyse der Mikrosatelliten nördlich und südlich der Kandidatenregion (UCH-L1) in einem Bereich sehr niedriger Rekombinationswahrscheinlichkeit eine Haplotypisierung erfolgen. Die Haplotypisierung zeigte nicht die erforderliche Identifizierung eines Krankheitsallels bei allen betroffenen Probanden. Somit ist hier neben der einzig bekannten deutschen PARK5 Familie keine weitere Familie mit UCH-L1 Mutation bestätigt worden. Dementsprechend ist die Ätiologie dieser Erkrankung in dieser Familie noch unklar, was aber der Bedeutung des Ubiquitin-Proteasom Systems in der Parkinson-Entität keinen Abbruch getan hat. Da alle anderen autosomal dominanten Parkinson-Loci ausgeschlossen sind, muss sich die Ursache für den M. Parkinson in dieser Familie in einem heute noch unbekannten Gen befinden. Weitere Untersuchungen im Rahmen eines Genomscans sind aufgrund der geringen Fallzahl nicht möglich. Die letzte Aufgabe dieser Arbeit bestand in der Durchführung einer Assoziationsstudie mit den putativen PINK1 (PARK6) Interaktoren NME4 und MTIF3 für den mehrheitlich sporadisch auftretenden M. Parkinson. Dabei wurden in zwei unabhängigen Studiengruppen mit insgesamt 453 sporadischen Parkinsonpatienten und 370 Kontrollen jeweils zwei SNPs auf gekoppelte Vererbung mit der Erkrankung untersucht. Der Unterschied zwischen den Testgruppen bestand im Studiendesign, da zum einen mit den Patienten nicht verwandte Kontrollen und zum anderen verwandte Kontrollen verwendet wurden. Die mit beiden Studientypen normalerweise auftretenden Probleme durch Stratifikation bzw. erniedrigte statistische Power konnten durch Kombination der Studien ausgeglichen werden. Das Methodenspektrum umfasste PCR und Restriktionsverdau, was zum Auffinden eines Kopplungsungleichgewichts für das Gen MTIF3 führte. Ein heterozygoter Basenaustausch für den Polymorphismus rs7669 erhöht signifikant das Relative Risiko an M. Parkinson zu erkranken, wohingegen der homozygote Basenaustausch das Krankheitsrisiko des Trägers signifikant erniedrigt. Bezüglich des Relativen Risikos wurde der Effekt der molekularen Heterosis nachgewiesen. Bei diesem mitochondrial lokalisierten Gen handelt es sich um einen Initiator der mitochondrialen Translation. Demzufolge besteht hier Einfluss auf die Homöostase und somit Funktionalität der Atmungskettenkomplexe, die als bedeutend für die Pathogenese des M. Parkinson angesehen werden. Die Verbindung zum mitochondrial lokalisierten PINK1 besteht aufgrund seiner Kinase-Aktivität in der An- und Abschaltung des mitochondrialen Translations - Initiationsfaktors. Aber auch die Wichtigkeit von NME4 konnte in dieser Studie trotz fehlender Assoziation nicht ausgeschlossen werden, da vorangehende experimentelle Ergebnisse dieses Protein bereits in den PINK1 Signalweg zuordnen konnten. MTIF3 könnte wohlmöglich ein wichtiger genetische Risikofaktor für den idiopathischen M. Parkinson sein. Es bleibt abzuwarten, ob zukünftige genetische und zellbiologische Experimente die Wichtigkeit, die in diesem Protein zu liegen scheint, bestätigen können.
Einführung Seit Einführung der Diffusionstensorbildgebung- (DTI) basierten Traktographie von zerebralen Bahnsystemen besteht der Verdacht einer zu dünnen Ausdehnung der Faserbahnen in der unmittelbaren Nachbarschaft von zerebralen Läsionen. Der gegenüber der tatsächlichen Ausdehnung verminderte Durchmesser verjüngt sich zusätzlich mit zunehmendem Abstand von dem sog. seed-Volume (“seed-VOI”). Die unterrepräsentierte Ausdehnung der Faserbahnen stellt in der neurochirurgischen Operationsplanung und intraoperativen Neuronavigation ein erhebliches Problem bei der Beurteilung der Resektionsgrenzen von Tumoren bzw. der Grenze dringlich zu erhaltender eloquenter Faserbahnen dar. Mit einem zusätzlichen, auf die Läsion fokussierten Traktographie-Algorithmus – Lesion-based Fibertracking (LBFT) – soll die Auswertbarkeit von Faserbahnen in der Umgebung von intrazerebralen Läsionen verbessert werden. Der Algorithmus von LBFT wird vorgestellt und das Verfahren anhand der Darstellung von Bahnen des Tractus corticospinalis (TCS) mit dem Standardverfahren verglichen. Methode In 40 Patienten mit intrazerebralen Läsionen in Nachbarschaft zu kortikospinalen Bahnen (Pyramidenbahn) wurde eine Diffusionstensor-bildgebung und fMRT basierte Faserbahndarstellung des Tractus corticospinalis auf Grundlage eines „tensor-deflection-Algorithmus“ (TEND) durchgeführt. Hierfür wurden Bahnen von den kortikalen motorischen Repräsentationen der Hand, des Fußes und der Zunge zum Hirnstamm visualisiert. Im Standardverfahren wird ein würfelförmiges Volumen – das sog. seed-Volume oder Ursprungsvolumen – im Gyrus praecentralis entsprechend der anatomischen und funktionellen Bildgebung definiert. Ein zweites würfelförmiges Volumen, lokalisiert im Hirnstamm selektiert ausschließlich Fasern welche durch beide Volumen verlaufen. Die resultierenden Fasern werden bezüglich ihres Verlaufes durch typische anatomische Landmarken kontrolliert und ggf. korrigiert. Anschließend wird das Faserbündel mittels einer Oberflächenrekonstruktionstechnik („surface rendering“) dreidimensional rekonstruiert (iPlan 2.5Cranial, BrainLab®, Feldkirchen, Germany). Für das neue Verfahren des LBFT wird die Region definiert, in welcher die Faserbahn des Standardverfahrens der Läsion am nächsten kommt und hier, um die Faserbahn des Standardverfahrens, ein neues seed-Volume platziert, welches das Standardfaserbündel um 10 mm überragt. Traktographie und Segmentierung werden analog dem Standardverfahren durchgeführt. Fasern, die nicht den Gyrus praecentralis erreichen oder nicht durch den Pedunculus cerebri verlaufen, werden eliminiert. Die Faserzahl, die Größe der Faserbahnen und die Größendifferenz zwischen den Bahnen des Standardverfahrens und LBFT werden verglichen und das Verfahren auf inter- und intra-rater Reliabilität geprüft. Ergebnisse Das Standardverfahren und LBFT waren in allen 40 Patienten durchführbar. Die Faserzahl bei LBFT erhöhte sich signifikant gegenüber dem Standardverfahren um 383,27% (p<0,0001). Der maximale Durchmesser in der Ebene, in welcher das Faserbündel der Läsion am nächsten kommt, sowie der Durchmesser in Richtung der Läsion erhöhen sich signifikant um 171,75 % bzw. 196,45 % (jeweils p<0.0001). Daraus folgt eine durchschnittliche Zunahme des Durchmessers in Richtung der Läsion um 4.48mm (± 2.35). Fazit Die Fehleinschätzung des Durchmessers und der Distanz des TCS zu subkortikalen Läsionen bei Anwendung des Standardverfahrens DTI-basierter Traktographie stellt ein erhebliches Problem in der funktionellen neurochirurgischen Operationsplanung und intraoperativen Neuronavigation dar. Durch den zusätzlichen Schritt des LBFT kann die Fehleinschätzung korrigiert und der in vorhergehenden Studien eingeforderte Sicherheitsabstand standardisiert robust und reliabel realisiert werden.
Central elements of the Bologna declaration have been implemented in a huge variety of curricula in humanities, social sciences, natural sciences and engineering sciences at German universities. Overall the results have been nothing less than disastrous. Surprisingly, this seems to be the perfect time for German universities to talk about introducing a curriculum that is fully compatible with the Bologna declaration for medical education as well. However, German medical education does not have problems the Bologna declaration is intended to solve, such as quality, mobility, internationalization and employability. It is already in the Post-Bologna age.
Background: The integration of the non-cross-resistant chemotherapeutic agents capecitabine and vinorelbine into an intensified dose-dense sequential anthracycline- and taxane-containing regimen in high-risk early breast cancer (EBC) could improve efficacy, but this combination was not examined in this context so far. Methods: Patients with stage II/IIIA EBC (four or more positive lymph nodes) received post-operative intensified dose-dense sequential epirubicin (150mg/m2 every 2 weeks) and paclitaxel (225mg/m2 every 2 weeks) with filgrastim and darbepoetin alfa, followed by capecitabine alone (dose levels 1 and 3) or with vinorelbine (dose levels 2 and 4). Capecitabine was given on days 1-14 every 21 days at 1000 or 1250 mg/m2 twice daily (dose levels 1/2 and 3/4, respectively). Vinorelbine 25 mg/m2 was given on days 1 and 8 of each 21-day course (dose levels 2 and 4). Results: Fifty-one patients were treated. There was one dose-limiting toxicity (DLT) at dose level 1. At dose level 2 (capecitabine and vinorelbine), five of 10 patients experienced DLTs. Therefore evaluation of vinorelbine was abandoned and dose level 3 (capecitabine monotherapy) was expanded. Hand-foot syndrome and diarrhoea were dose limiting with capecitabine 1250 mg/m2 twice daily. At 35.2 months' median follow-up, the estimated 3-year relapse-free and overall survival rates were 82% and 91%, respectively. Administration of capecitabine monotherapy after sequential dose-dense epirubicin and paclitaxel is feasible in node-positive EBC, while the combination of capecitabine and vinorelbine as used here caused more DLTs. Hereditary angioedema (C1 inhibitor deficiency, HAE) is associated with intermittent swellings which are disabling and may be fatal. Effective treatments are available and these are most useful when given early in the course of the swelling. The requirement to attend a medical facility for parenteral treatment results in delays. Home therapy offers the possibility of earlier treatment and better symptom control, enabling patients to live more healthy, productive lives. This paper examines the evidence for patient-controlled home treatment of acute attacks ('self or assisted administration') and suggests a framework for patients and physicians interested in participating in home or self-administration programmes. It represents the opinion of the authors who have a wide range of expert experience in the management of HAE.
Background: The human chromosomal region 9p21.3 has been shown to be strongly associated with Coronary Heart Disease (CHD) in several Genome-wide Association Studies (GWAS). Recently, this region has also been shown to be associated with Aggressive Periodontitis (AgP), strengthening the hypothesis that the established epidemiological association between periodontitis and CHD is caused by a shared genetic background, in addition to common environmental and behavioural risk factors. However, the size of the analyzed cohorts in this primary analysis was small compared to other association studies on complex diseases. Using our own AgP cohort, we attempted to confirm the described associations for the chromosomal region 9p21.3. Methods: We analyzed our cohort consisting of patients suffering from the most severe form of AgP, generalized AgP (gAgP) (n = 130) and appropriate periodontally healthy control individuals (n = 339) by genotyping four tagging SNPs (rs2891168, rs1333042, rs1333048 and rs496892), located in the chromosomal region 9p21.3, that have been associated with AgP. Results: The results confirmed significant associations between three of the four SNPs and gAgP. The combination of our results with those from the study which described this association for the first time in a meta-analysis of the four tagging SNPs produced clearly lower p-values compared with the results of each individual study. According to these results, the most plausible genetic model for the association of all four tested SNPs with gAgP seems to be the multiplicative one. Conclusion: We positively replicated the finding of an association between the chromosomal region 9p21.3 and gAgP. This result strengthens support for the hypothesis that shared susceptibility genes within this chromosomal locus might be involved in the pathogenesis of both CHD and gAgP.
Die Notwendigkeit der ipsilateralen Adrenalektomie als obligater Bestandteil einer Tumornephrektomie beim Nierenzellkarzinom wurde in der Literatur kontrovers diskutiert. Das Ziel der vorliegenden Studie war es, durch einen Vergleich von tumornephrektomierten Patienten mit und ohne Nebennierenbefall Parameter zu ermitteln, die eine präoperative Abschätzung des Nebennierenbefalls erlauben, und somit als Entscheidungshilfe zur Durchführung der Adrenalektomie im Rahmen einer Tumornephrektomie dienen können. In unserer Studie wurden insgesamt 250 Patienten mit Nierenzellkarzinom erfasst, die zwischen 1992 und 2001 in der Klinik für Urologie und Kinderurologie Prof. Dr. med. Dietger Jonas, Johann Wolfgang Goethe – Universität Frankfurt am Main operiert wurden. Dabei wurde in allen Fällen die radikale Tumornephrektomie nach Robson inklusive ipsilateraler Adrenalektomie durchgeführt. Alle Operationspräparate wurden im Senckenbergischen Institut für Pathologie Prof. Dr. med. M.-L. Hansmann, Frankfurt am Main untersucht und nach der 6. Auflage der TNM-Klassifikation in der von der UICC empfohlenen Weise eingestuft. In 9 der 250 Fälle (3,6 %) wurde ein Nebennierenbefall festgestellt. Statistisch signifikante Ergebnisse konnten für folgende Parameter ermittelt werden: · Präoperativer CT-Befund In der präoperativ durchgeführten CT wurden von 235 als unauffällig eingestuften Fällen 5 pathologische Nebennierenbefunde nicht erkannt (2,13 %). Von 15 als auffällig eingestuften Fällen wurden jedoch 4 als richtig pathologisch erkannt (26,6 %). Es ergaben sich für den Nachweis eines Nebennierenbefalls im CT eine Sensitivität von 44 %, eine Spezifität von 95,4 %, ein positiver prädiktiver Wert von 26,6 %, und ein negativer prädiktiver Wert von 97,8 %. · Nierentumorgröße Es zeigte sich ein Nebennierenbefall erst ab einer Tumorgröße von > 5 cm mit einer Wahrscheinlichkeit von 5,8 % (p = 0,035). · pTNM-Stadium Ein Nebennierenbefall ließ sich erst ab Tumorstadium pT 3a erkennen. Bei pT1- oder pT2-Tumoren war kein Nebennierenbefall festgestellt worden. Bei Patienten ohne Lymphknotenmetastasierung konnte ein Tumorbefall der Nebenniere in 2 von 224 Fällen (0,89 %) gefunden werden. Bei den Patienten mit Lymphknotenmetastasierung lag der Nebennierenbefall bei 3 von 12 Fällen (25 %) für die Nierentumoren mit pN1 und bei 4 von 14 Fällen (28,6 %) bei Nierentumoren mit pN2. Bei Patienten ohne Fernmetastasierung (230 Fälle) ließ sich in 3 Fällen (1,3 %) ein Nebennierenbefall aufweisen; bei vorliegender Fernmetastasierung (20 Fälle) lag der Anteil der befallenen Nebennieren mit 6 Fällen bei 30 %. · Tumorgrading Ein Nebennierenbefall war bei keinem der G1-Tumoren festgestellt worden. Bei 4,4 % der G2-Tumoren (6/136 Fällen) und bei 12,5 % der G3-Tumoren (3/24 Fällen) konnte ein Nebennierenbefall nachgewiesen werden. Da das Grading nur am Nephrektomiepräparat vorgenommen werden kann, spielt es als prädiktiver Parameter keine Rolle. · Infiltration benachbarter Strukturen 0,5 % (1/200 Fällen) der Patienten ohne und 16 % (8/50 Fällen) der Patienten mit einem Tumorthrombus in der V. renalis zeigten auch einen Nebennierenbefall (p = 0,000012). 2,9 % (7/240 Fällen) der Patienten ohne und 20 % (2/10 Fällen) der Patienten mit einer Infiltration in die V. cava zeigten einen Nebennierenbefall (p = 0,044). 14,5 % (9/62 Fällen) der Patienten mit Tumorinfiltration in die Nierenkapsel zeigten einen Nebennierenbefall; keiner der Patienten ohne Nierenkapselinfiltration zeigte einen Nebennierenbefall (p = 0,000002). Zusammenfassend lässt sich festhalten, dass eine ipsilaterale Adrenalektomie heutzutage keine obligate Komponente der Tumornephrektomie sein muß, sondern nur durchgeführt werden sollte, wenn folgende präoperative Bedingungen vorliegen: 1. auffällige präoperative bildgebende Beurteilung der Nebenniere durch Sonographie, Computertomographie oder ggf. Magnetresonanztomographie 2. Größe des Nierentumors > 5 cm unabhängig von der Pollokalisation 3. . Primärtumorstadium >= cT3a 4. CT-graphischer Nachweis von Lymphknoten- und/oder Fernmetastasen 5. CT-graphischer Nachweis einer vaskulären Invasion 6. CT-graphischer Nachweis einer Nierenkapselinfiltration 7. auffälliger intraoperativer Befund
On demand treatment and home therapy of hereditary angioedema in Germany - the Frankfurt experience

Background: Manifestation of acute edema in hereditary angioedema (HAE) is characterized by interindividual and intraindividual variability in symptom expression over time. Flexible therapy options are needed. Methods: We describe and report on the outcomes of the highly individualized approach to HAE therapy practiced at our HAE center in Frankfurt (Germany). Results: The HAE center at the Frankfurt University Hospital currently treats 450 adults with HAE or AAE and 107 pediatric HAE patients with highly individualized therapeutic approaches. 73.9% of the adult patients treat HAE attacks by on-demand therapy with pasteurized pd C1-INH concentrate, 9.8% use additional prophylaxis with attenuated androgens, 1% of the total patient population in Frankfurt has been treated with Icatibant up to now. In addition adult and selected pediatric patients with a high frequency of severe attacks are instructed to apply individual replacement therapy (IRT) with pasteurized pd C1-INH concentrate. Improvement on Quality of Life items was shown for these patients compared to previous long-term danazol prophylaxis. Home treatment of HAE patients was developed in the Frankfurt HAE center in line with experiences in hemophilia therapy and has so far been implemented over a period of 28 years. At present 248 (55%) of the adult patients and 26 (24%) of the pediatric patients are practicing home treatment either as on demand or IRT treatment. Conclusions: In conclusion, the individualized home therapies provided by our HAE center, aim to limit the disruption to normal daily activities that occurs for many HAE patients. Furthermore, we seek to optimize the economic burden of the disease while offering a maximum quality of life to our patients.
Background We published the Canadian 2003 International Consensus Algorithm for the Diagnosis, Therapy, and Management of Hereditary Angioedema (HAE; C1 inhibitor [C1-INH] deficiency) and updated this as Hereditary angioedema: a current state-of-the-art review: Canadian Hungarian 2007 International Consensus Algorithm for the Diagnosis, Therapy, and Management of Hereditary Angioedema. Objective To update the International Consensus Algorithm for the Diagnosis, Therapy and Management of Hereditary Angioedema (circa 2010). Methods The Canadian Hereditary Angioedema Network (CHAEN)/Reseau Canadien d'angioedeme hereditaire (RCAH) (www.haecanada.com) and cosponsors University of Calgary and the Canadian Society of Allergy and Clinical Immunology (with an unrestricted educational grant from CSL Behring) held our third Conference May 15th to 16th, 2010 in Toronto Canada to update our consensus approach. The Consensus document was reviewed at the meeting and then circulated for review. Results This manuscript is the 2010 International Consensus Algorithm for the Diagnosis, Therapy and Management of Hereditary Angioedema that resulted from that conference. Conclusions Consensus approach is only an interim guide to a complex disorder such as HAE and should be replaced as soon as possible with large phase III and IV clinical trials, meta analyses, and using data base registry validation of approaches including quality of life and cost benefit analyses, followed by large head-to-head clinical trials and then evidence-based guidelines and standards for HAE disease management.
To date it is not clear at which stage of differentiation mature T cell leukaemia/lymphoma is initiated. Previous studies in our group showed that mature T cells are relatively resistant to transformation. We wanted to further investigate the transformation potential of NPM-ALK, p21SNFT and the viral oncoprotein Tax on mature T cells. First, we analyzed the effects on T cell growth in vitro after transducing human T cell lines with gammaretroviral vectors encoding these genes. No growth or proliferation promoting effect of all three genes was observed. In the second part of the project, we transduced murine, mature T cells and/or haematopoietic stem cells (HPCs/HSCs) and transplanted these cells into Rag-1 deficient recipients. All mice transplanted with NPM-ALK transduced monoclonal mature T cells (OT-1) developed leukaemia/lymphoma. In contrast, only few NPM-ALK transduced polyclonal T cell and HPC/HSC transplanted mice developed leukaemia/lymphoma. From the p21SNFT group, only two mice transplanted with transduced OT-1 T cells developed leukaemia/lymphoma, which showed high eGFP and interestingly CD19 expression. No malignancies were observed in Tax transplanted animals so far. Furthermore, the recipients do not show any eGFP marking in the periphery. In conclusion, our results show that compared to polyclonal T cells, monoclonal T cells are transformable after gammaretroviral transfer of NPM-ALK and p21SNFT.
Background: The potential anti-cancer effects of mammalian target of rapamycin (mTOR) inhibitors are being intensively studied. To date, however, few randomised clinical trials (RCT) have been performed to demonstrate anti-neoplastic effects in the pure oncology setting, and at present, no oncology endpoint-directed RCT has been reported in the high-malignancy risk population of immunosuppressed transplant recipients. Interestingly, since mTOR inhibitors have both immunosuppressive and anti-cancer effects, they have the potential to simultaneously protect against immunologic graft loss and tumour development. Therefore, we designed a prospective RCT to determine if the mTOR inhibitor sirolimus can improve hepatocellular carcinoma (HCC)-free patient survival in liver transplant (LT) recipients with a pre-transplant diagnosis of HCC. Methods: The study is an open-labelled, randomised, RCT comparing sirolimus-containing versus mTOR-inhibitor-free immunosuppression in patients undergoing LT for HCC. Patients with a histologically confirmed HCC diagnosis are randomised into 2 groups within 4-6 weeks after LT; one arm is maintained on a centre-specific mTOR-inhibitor-free immunosuppressive protocol and the second arm is maintained on a centre-specific mTOR-inhibitor-free immunosuppressive protocol for the first 4-6 weeks, at which time sirolimus is initiated. A 3-year recruitment phase is planned with a 5-year follow-up, testing HCC-free survival as the primary endpoint. Our hypothesis is that sirolimus use in the second arm of the study will improve HCC-free survival. The study is a non-commercial investigator-initiated trial (IIT) sponsored by the University Hospital Regensburg and is endorsed by the European Liver and Intestine Transplant Association; 13 countries within Europe, Canada and Australia are participating. Discussion: If our hypothesis is correct that mTOR inhibition can reduce HCC tumour growth while simultaneously providing immunosuppression to protect the liver allograft from rejection, patients should experience less post-transplant problems with HCC recurrence, and therefore could expect a longer and better quality of life. A positive outcome will likely change the standard of posttransplant immunosuppressive care for LT patients with HCC. Einleitung: Schwer verletzte Patienten nach Trauma (ISS > 16) sind häufig in Folge der Verletzungen über mehrere Tage beatmet. 40% dieser Patienten weisen eine Lungenkontusion auf. Mit zunehmender Beatmungsdauer steigt das Risiko einer Ventilator-assoziierten Pneumonie (VAP). Zeitgleich findet eine Reparation des Lungengewebes statt. Eine zeitnahe antiinfektive Therapie bei Verdacht auf eine VAP zu initiieren ist schwierig. Derzeit existiert kein validierter Parameter oder Score, der eine sichere Diskriminierung zwischen Infektion und Inflammation zulässt. Triggering receptor on myeloid cells (TREM-1) ist ein Rezeptor des angeborenen Immunsystems und wurde im Jahr 2000 erstmalig beschrieben. Sein löslicher Anteil, sTREM-1, ist in der bronchoalveolären Lavage (BAL) bei Patienten mit Pneumonie signifikant erhöht (> 200pg/ml). Es liegen keine Daten zu sTREM-1 bei Patienten nach Lungenkontusion vor. Unklar ist, ob sTREM-1 als Pneumonie-Marker nach Lungenkontusion geeignet ist. Material & Methoden: Nach Zustimmung der Ethikkommission und Einwilligung durch einen Angehörigen wurden prospektiv 42 Patienten mit Thoraxtrauma rekrutiert. Am ersten (im Median 15h nach dem Trauma) und an den Behandlungstagen zwei, drei, fünf, sechs und sieben wurden bei allen Patienten über den Tubus mit einem Aero-Jet Katheter BAL (20ml Spülung) gewonnen und zeitgleich Serumproben entnommen. Die Messung der sTREM-1-Konzentration erfolgte mittels Sandwich-ELISA in Doppelbestimmung (Quantikine sTREM-1 Immunoassay; Firma R&D Systems). Die Serum-Konzentrationen der Interleukine (IL) 6 und 10 sowie des Lipopolysaccharid bindenden Proteins (LBP) wurden mittels Immulite® bestimmt. Die Diagnose Pneumonie wurde retrospektiv mittels Clinical Pulmonary Infection Score (CPIS) gestellt: CPIS > 6 Pneumonie, ≤ 6 keine Pneumonie. Ergebnisse & Diskussion: 15 Stunden nach Trauma wurde der sTREM-1 Spiegel in der BAL, bei im Verlauf pulmonal klinisch unauffälligen Patienten, im Median mit 219pg/ml bestimmt. Im Weiteren stieg sTREM-1 im Median nach 24h auf 575pg/ml an und zeigte ähnliche Konzentrationen im Beobachtungszeitraum. Der Schweregrad der Lungenkontusion korreliert mit der Höhe des sTREM-1-Spiegels in der BAL 40h nach Trauma. Patienten mit schwerer Lungenkontusion (im Median 2240pg/ml) haben signifikant höhere Werte gegenüber Patienten ohne Kontusion (Median 217pg/ml), oder geringer Kontusion (Median 339pg/ml). Am Tag der Diagnosestellung Pneumonie (CPIS > 6, n= 9) zeigten die betroffenen Patienten einen signifikant erhöhten sTREM-1-Spiegel in der BAL (Median 2145pg/ml, p < 0,05) im Vergleich zum Tag vor der Pneumonie (Median 588pg/ml). Wird der cut off für sTREM-1 bei 800pg/ml festgelegt ergibt sich eine Sensitivität von 87% und eine Spezifität von 38%. Eine positive BAL weist im Vergleich zu einer negativen BAL signifikant höhere sTREM-1-Konzentrationen (Median 1492pg/ml vs. 971pg/ml, p < 0,05) auf. Die Sensitivität (85%) ist hoch, die Spezifität (51%) gering. Somit ist sTREM-1 nicht nur durch eine Infektion, sondern auch durch eine Gewebeschädigung mit Einblutung und Inflammation stimulierbar. sTREM-1 ist durch die kontusionsbedingte Stimulation in der ersten Woche nach Trauma ungeeignet, um sicher zwischen einer Pneumonie und einer kontusionsbedingten Inflammation zu unterscheiden. Zytokine und akute Phase Proteine (IL-6, LBP, Procalcitonin) sind bekanntermaßen ebenfalls nicht zur sicheren Diskriminierung einer Infektion geeignet. In Kombination mit sTREM-1 lassen sich jedoch zur Diagnosestellung einer Pneumonie vergleichbare Werte für Sensitivität und Spezifität erreichen wie mittels CPIS Score, wobei der CPIS nur retrospektiv ermittelt werden kann. Die Laborparameter liegen bereits am Tag des Verdachts auf eine Infektion vor. Die klinische Entscheidung zur Initiierung einer Antiinfekitvatherapie korrelierte weder mit dem CPIS noch mit den Inflammationsparametern. Drei von neun Patienten erhielten trotz steigenden Entzündungszeichen und einem CPIS > 6 keine Antiinfektiva. In der Konsequenz könnte eine Kombination aus IL-6 und LBP im Serum, sTREM-1 in der BAL und klinischen Parameter des CPIS eine sensitive und spezifische Entscheidungshilfe für eine antiinfektive Therapie bei Polytrauma und Verdacht auf eine VAP werden.
Introduction: It has been proposed that individual genetic variation contributes to the course of severe infections and sepsis. Recent studies of single nucleotide polymorphisms (SNPs) within the endotoxin receptor and its signaling system showed an association with the risk of disease development. This study aims to examine the response associated with genetic variations of TLR4, the receptor for bacterial LPS, and a central intracellular signal transducer (TIRAP/Mal) on cytokine release and for susceptibility and course of severe hospital acquired infections in distinct patient populations. Methods: Three intensive care units in tertiary care university hospitals in Greece and Germany participated. 375 and 415 postoperative patients and 159 patients with ventilator associated pneumonia (VAP) were included. TLR4 and TIRAP/Mal polymorphisms in 375 general surgical patients were associated with risk of infection, clinical course and outcome. In two prospective studies, 415 patients following cardiac surgery and 159 patients with newly diagnosed VAP predominantly caused by Gram-negative bacteria were studied for cytokine levels in-vivo and after ex-vivo monocyte stimulation and clinical course. Results: Patients simultaneously carrying polymorphisms in TIRAP/Mal and TLR4 and patients homozygous for the TIRAP/Mal SNP had a significantly higher risk of severe infections after surgery (odds ratio (OR) 5.5; confidence interval (CI): 1.34 - 22.64; P = 0.02 and OR: 7.3; CI: 1.89 - 28.50; P < 0.01 respectively). Additionally we found significantly lower circulating cytokine levels in double-mutant individuals with ventilator associated pneumonia and reduced cytokine production in an ex-vivo monocyte stimulation assay, but this difference was not apparent in TIRAP/Mal-homozygous patients. In cardiac surgery patients without infection, the cytokine release profiles were not changed when comparing different genotypes. Conclusions: Carriers of mutations in sequential components of the TLR signaling system may have an increased risk for severe infections. Patients with this genotype showed a decrease in cytokine release when infected which was not apparent in patients with sterile inflammation following cardiac surgery.

Blood vessels form de novo through the tightly regulated programs of vasculogenesis and angiogenesis. Both processes are distinct but one of the steps they share is the formation of a central lumen, when groups of cells organized as vascular cords undergo complex changes to achieve a tube-like morphology. Recently, a protein termed epidermal growth factor-like domain 7 (EGFL7) was described as a novel endothelial cell-derived factor involved in the regulation of the spatial arrangement of cells during vascular tube assembly. With its impact on tubulogenesis and vessel shape EGFL7 joined the large family of molecules governing blood vessel formation. Only recently, the molecular mechanisms underlying EGFL7's effects have been started to be elucidated and shaping of the extracellular matrix (ECM) as well as Notch signaling might very well play a role in mediating its biological effects. Further, findings in knock-out animal models suggest miR-126, a miRNA located within the egfl7 gene, has a major role in vessel development by promoting VEGF signaling, angiogenesis and vascular integrity. This review summarizes our current knowledge on EGFL7 and miR-126 and we will discuss the implications of both bioactive molecules for the formation of blood vessels.
Background: Up to the 1950s, there was an ongoing debate about the diversity of hereditary optic neuropathies, in particular as to whether all inherited optic atrophies can be ascribed to Leber's hereditary optic neuropathy (LHON) or represent different disease entities. In 1954 W. Jaeger published a detailed clinical and genealogical investigation of a large family with explicit autosomal dominant segregation of optic atrophy thus proving the existence of a discrete disease different from LHON, which is nowadays known as autosomal dominant optic atrophy (ADOA). Since the year 2000 ADOA is associated with genomic mutations in the OPA1 gene, which codes for a protein that is imported into mitochondria where it is required for mitochondrial fusion. Interestingly enough, the underlying mutation in this family has not been identified since then. Results: We have reinvestigated this family with the aim to identify the mutation and to further clarify the underlying pathomechanism. Patients showed a classical non-syndromic ADOA. The long term deterioration in vision in the two teenagers examined 50 years later is of particular note 5/20 to 6/120. Multiplex ligation probe amplification revealed a duplication of the OPA1 exons 7-9 which was confirmed by long distance PCR and cDNA analysis, resulting in an in-frame duplication of 102 amino acids. Segregation was verified in 53 available members of the updated pedigree and a penetrance of 88% was calculated. Fibroblast cultures from skin biopsies were established to assess the mitochondrial network integrity and to qualitatively and quantitatively study the consequences of the mutation on transcript and protein level. Fibroblast cultures demonstrated a fragmented mitochondrial network. Processing of the OPA1 protein was altered. There was no correlation of the OPA1 transcript levels and the OPA1 protein levels in the fibroblasts. Intriguingly an overall decrease of mitochondrial proteins was observed in patients' fibroblasts, while the OPA1 transcript levels were elevated. Conclusions: The thorough study of this family provides a detailed clinical picture accompanied by a molecular investigation of patients' fibroblasts. Our data show a classic OPA1-associated non-syndromic ADOA segregating in this family. Cell biological findings suggest that OPA1 is regulated by post-translational mechanisms and we would like to hypothesize that loss of OPA1 function might lead to impaired mitochondrial quality control. With the clinical, genetic and cell biological characterisation of a family described already more than 50 years ago, we span more than half a century of research in optic neuropathies.
Background: Vaccinia virus strain Lister Elstree (VACV) is a test virus in the DVV/RKI guidelines as representative of the stable enveloped viruses. Since the potential risk of laboratory-acquired infections with VACV persists and since the adverse effects of vaccination with VACV are described, the replacement of VACV by the modified vaccinia Ankara strain (MVA) was studied by testing the activity of different chemical biocides in three German laboratories. Methods: The inactivating properties of different chemical biocides (peracetic acid, aldehydes and alcohols) were tested in a quantitative suspension test according to the DVV/RKI guideline. All tests were performed with a protein load of 10% fetal calf serum with both viruses in parallel using different concentrations and contact times. Residual virus was determined by endpoint dilution method. Results: The chemical biocides exhibited similar virucidal activity against VACV and MVA. In three cases intra-laboratory differences were determined between VACV and MVA - 40% (v/v) ethanol and 30% (v/v) isopropanol are more active against MVA, whereas MVA seems more stable than VACV when testing with 0.05% glutardialdehyde. Test accuracy across the three participating laboratories was high. Remarkably inter-laboratory differences in the reduction factor were only observed in two cases. Conclusions: Our data provide valuable information for the replacement of VACV by MVA for testing chemical biocides and disinfectants. Because MVA does not replicate in humans this would eliminate the potential risk of inadvertent inoculation with vaccinia virus and disease in non-vaccinated laboratory workers.
Das West-Nil-Virus (WNV) gehört zur Gruppe des Japan-Enzephalitis-Serokomplexes der Flaviviren, welches in Afrika, Asien und Mittleren Osten sowie in Süd- und Osteuropa seine Verbreitung findet. Das unerwartete Auftreten des Erregers in den USA (New York, 1999) bedingte eine große Zahl an transfusions- sowie transplantations-assoziierten WNV-Infektionen. Derzeit liegen keine Erkenntnisse über die Verbreitung des WNV in der deutschen Blutspenderpopulation vor. Aus diesem Grund war das Ziel dieser Arbeit die Bestimmung der Prävalenz und Inzidenz des Erregers zur Einschätzung der Gefahr von transfusions-assoziierten WNV-Infektionen. Zunächst wurde zur Bestimmung der Prävalenz des Erregers die Sensitivität und Spezifität verschiedener ELISAs (Panbio, Focus und Euroimmun) für die WNV-Antikörpertestung bestimmt. Es zeigte sich, dass WNV starke Kreuzreaktionen mit dem für unsere Breiten relevanten Frühsommer-Meningoenzephalitis-Virus (FSME) aufweist. Aus diesem Grund wurde zusätzlich ein FSME-ELISA sowie als Bestätigungstest ein Neutralisationstest verwendet und ein Testalgorithmus entwickelt. Für die Bestimmung der Inzidenz wurde auf den Grundlagen von Hadfield eine WNV-Reverse Transkriptase-PCR (RT-PCR) entwickelt und etabliert. Die 95% Nachweisgrenze wurde nach der Entwicklung der RT-PCR auf 54Kop./ml WNV-RNA bezogen auf eine Einzelprobe in einem Minipool bestehend aus acht Blutspenden bestimmt. Das Konfidenzintervall beträgt (CI): 42; 6 - 79; 92 Kopien/ml. Zudem wurde die Sensitivität der automatischen Extraktion mittels des auf magnetischen Festphasen beruhenden Separationsmoduls I mit der WNV-RT-PCR ermittelt. Die 95% Nachweisgrenze lag bei 225Kop./ml WNVRNA((CI): 168; 5 - 314; 3 ) bezogen auf eine Einzelprobe in einem Minipool bestehend aus acht Blutspenden. Aufgrund der etwas schlechteren Sensitivität wurde für die Bestimmung der Inzidenz die manuelle Extraktion durchgeführt. Zudem wurde das kommerzielle WNV Procleix Assay der Firma Chiron verwendet, um alle bisher in Europa aufgetretenen WNV-Stämme zu erfassen. Im Sommer 2004 wurde zunächst die Prävalenz des WNV bestimmt. Hierfür wurden 14:000 aus Hessen stammende Blutspenden von gesunden Spendern untersucht. Initial zeigte sich eine hohe Rate an anti-WNV reaktiven (5; 9 %) Blutspenden, wobei nur 0; 03% der Blutspenden im Neutralisationstest als Anti-WNV positiv bestätigt worden. 0; 15% der bestätigten anti-WNV positiven Blutspenden waren hierbei am ehesten mit Reisen in WNV-Epidemiegebiete assoziiert. Die Bestimmung der Inzidenz erfolgt im darauf folgendem Jahr durch die Untersuchung von 10:976 Blutspenden zusammengefasst in 1:372 Minipools mittels WNV RT-PCR. Von 1:372 Minipools wurden 1:247 Minipools mit dem Procleix WNV Assay untersucht. Beide Untersuchungsmethoden konnten keine WNV-positive Blutspende nachweisen. Insgesamt scheint das WNV -wenn überhaupt- nur in verschwindend geringem Maße in der deutschen Blutspenderpopulation zu exisitieren. Somit besteht derzeit keine Gefahr für transfusions-assoziierte WNV-Infektionen. Dennoch wurden importierte WNV-Infektionen aus Endemiegebieten in Deutschland beschrieben. Durch weitere Veränderungen der klimatischen Gegebenheiten wäre die Einschleppung des WNV nach Deutschland in Zukunft durchaus denkbar. Hierfür steht nun eine für das Spenderscreening taugliche Real-time PCR zür Verfügung, so dass jederzeit ein Blutspenderscreening auf WNV eingeführt werden kann.
Für eine erfolgreiche Gentherapie ist zunächst ein effizientes Gentransfersystem nötig, das das Transgen in möglichst vielen Zellen einbaut und es aktiv hält. Damit sich dann der Anteil der geschützten Zellen vergrößert, muss eine Selektivität der genmodifizierten Zellen gegenüber den nativen Zellen gegeben sein, wobei die Sicherheit nicht außer Acht gelassen werden darf, da ein ungünstiger Einbau des Transgens eine Insertionsmutagenese und somit Tumoren induzieren kann. Der durch die Arbeitsgruppe von Laer entwickelte retrovirale Vektor M87o codiert den membranständigen Fusionsinhibitor maC46 (membran-anchored C-Peptid 46), der den Eintritt von HIV (Human Immunodeficiency Virus) in die Zielzelle effektiv verhindert. Diese Gentherapie mit M87o wurde in einer klinischen Studie an T-Lymphozyten von 10 weit fortgeschrittenen AIDS (Acquired Immune Deficiency Syndrome)-Patienten durchgeführt, wobei die Therapie gut verträglich war und keine Toxizität zeigte. Allerdings hatten die Patienten auch keinen klaren Vorteil von der Therapie. In der vorliegenden Arbeit wurden SIN Vektoren (Self-inactivating Vektoren) in 5 verschiedenen Konstruktionen getestet, um die optimale Vektordesign zu ermitteln und eine langfristige hohe Expression zu ermöglichen. Da die SIN Vektoren im Vergleich zu konventionellen gammaretroviralen Vektoren ein geringeres Risiko bezüglich der Insertionsmutagenese aufweisen, stellen sie ein sichereres Vektorsystem dar. Um eine bessere Transgenexpression zu erzielen, wurde in den SIN Vektoren entweder ein zellulärer Promotor oder ein viraler SFFV (spleen focus forming virus) als internen Promotor verwendet. Zusätzliche regulatorische Elemente, wie wPRE (Woodchuck Posttranscriptional Regulatory Element), cHS4 (chicken Hypersensitive Site) Insulator und SAR (Scaffold Attachment Region) Element wurden dann in unterschiedlichen Kombinationen zu stärkeren und langanhaltenden Expressionen integriert, wobei wPRE die RNA Prozessierung verbessert und somit die RNA Stabilität erhöht und SAR und cHS4 Insulator dem Silencing entgegenwirken und so die Expression aufrechterhalten. Diese fünf SIN Konfigurationen wurden untereinander und mit dem klassischen gammaretroviralen Vektor M87o bezüglich des Titers, der Expressionsstärke und der Langzeit-Genexpression verglichen. Dazu wurden zunächst humane T-Zelllinien PM-1 und primäre humane T-Zellen als Testzellen verwendet. Die Versuche wurden dann mit murinen T-Zellen wiederholt, die in die immundefiziten Mäuse transplantiert wurden, um die Genexpression in vivo weiter zu verfolgen. Die SIN Konstrukte zeigten jedoch eine deutlich schwächere Expression als die LTR (Long Terminal Repeat)-getriebene Vektoren und nur ein Konstrukt mit dem viralen Promotor und wPRE zeigte eine annähernd so hohe Expression wie die konventionellen Vektoren. Während der virale SFFV Promotor eine höhere Expressionsstärke gegenüber dem zellulären EF1α (Elongationsfaktor 1 alpha) Promotor zeigte, hatte der cHS4 Insulator nur geringfügige Einflüsse sowohl auf den Titer als auch auf die Expressionsstärke. Der Vektor mit dem SAR-Element zeigte zwar die geringsten Titer und Expressionsstärke, aber in Langzeitbeobachtung wies er sowohl in vitro als auch in vivo eine relativ konstante Anzahl von transgenpositiven Zellen auf. SIN Vektoren, in denen mit einer Kombination von wPRE und SAR-Element die RNA Prozessierung verbessert und das methylationsbedingte Silencing verhindert wird, könnten eine weitere Optimierungsmöglichkeit des Gentransfersystems bei der Gentherapie darstellen.
Hintergrund: Epoxyeicosatriensäuren (Epoxyeicotrienoic acids, EETs) sind antihypertensiv, anti-inflammatorisch, anti-proliferativ und pro-fibrinolytisch wirksam. Die lösliche Epoxid-Hydrolase (soluble epoxid hydrolase, sEH) wandelt EETs in ihre korrespondierenden, weniger aktiven Diole (DHETEs) um. Das legt die Vermutung nahe, dass eine Steigerung der EET-Spiegel durch sEH-Inhibition eine Strategie zur Verhinderung von Endorganschäden sein kann. Wir untersuchten den Effekt der sEH-Inhibition im Modell der chronischen Niereninsuffizienz bei der Maus. Methoden: Es wurde eine 5/6 Nephrektomie (5/6 Nx) an männlichen SV129 Mäusen durchgeführt um eine chronisch progressive Niereninsuffizienz zu induzieren. Postoperativ wurden die Tiere in Gruppen randomisiert und mit Placebo (Leitungswasser), einem ACE-Hemmer (Ramipril, 40 mg/kgKG/d), dem sEH-Inhibitor 1471 (15 mg/kgKG/d) und dem CYP-Inhibitor Fenbendazol (15 mg/kgKG/) für 8 Wochen behandelt. Ergebnisse: Verglichen mit den sham-operierten Mäusen induzierte die 5/6 Nx einen arteriellen Hypertonus, eine Proteinurie, Glomerulosklerose und eine erhöhte Sterblichkeit. Wie erwartet führte die Gabe von Ramipril zu einer Blutdrucksenkung, einer verminderten Albuminurie und einer verbesserten Überlebensrate. Fenbendazol senkte die Plasma- Konzentration von einigen EETs, hatte aber keinen Einfluss auf die Progression der Niereninsuffizienz. Überraschenderweise kam es unter sEH-Inhibitor-Therapie zu keiner Blutdrucksenkung in diesem Modell. Im Gegenteil, die Proteinurie war sogar noch verstärkt bei den 5/6 Nx-Tieren unter 1471-Therapie verglichen mit der Placebo-Gruppe. Die Plasma-EET-Spiegel waren >2 mal höher in den 5/6 Nx-Mäusen als in der korrespondierenden sham-Gruppe. Die renale sEH-Expression war in den 5/6 Nx-Mäusen vermindert, eine medikamentöse sEH-Inhibition steigerte die EET-Spiegel in diesen Tieren noch weiter. Bemerkenswert war, dass, ausschließlich in den 5/6 Nx-Gruppen, eine sEH-Inhibition und eine CYP-Inhibition zu einer gesteigerten Konzentration von 5-Hydroxyeicosatriensäure (5-HETE), 12-HETE und 15-HETE führte, welche Lipid Peroxidations- und Lipoxygenase-Produkte sind. Fazit: Im Modell der 5/6 Nephrektomie bei der Maus führt die Hemmung der sEH zu keiner Blutdrucksenkung, sondern zu einer Verschlechterung der Proteinurie. Ursächlich hierfür sind vermutlich die durch die Niereninsuffizienz hervorgerufe Akkumulation von EETs sowie eine Verschiebung des Arachidonsäure-Metabolismus hin zu Lipoxgenase-Produkten mit nierenschädigender Wirkung.
Die Wahrnehmung unserer Umwelt erfolgt über verschiedene Sinnesmodalitäten, deren Informationen bevorzugt in bestimmten Hirnarealen verarbeitet werden und sich schließlich zu einem kohärenten Bild ergänzen. Wie diese Prozesse ablaufen und durch welche Vorgänge die Wahrnehmungen einen einheitlichen Sinneseindruck formen, sind fundamentale Fragen der kognitiven Neurowissenschaft. Ziel der vorliegenden Studie war es, diese sensorischen Verarbeitungsprozesse genauer zu untersuchen und bisherige Ergebnisse zu bestätigen sowie neue Erkenntnisse bezüglich audio-haptischer und audio-visuo-haptischer Integration zu gewinnen. Weiterhin sollten angemessene Kriterien für die Auswertung der Daten untersucht werden mit dem Schwerpunkt sinnvoller Kontrollbedingungen. Sechzehn Probanden wurden hierzu mittels fMRT während uni-, bi- und trimodaler Stimulation untersucht. Die Stimulation bestand aus der Darbietung von schwarz-weißen Tierbildern, Tierstimmen und Tierfiguren, die einzeln oder in unterschiedlichen Kombinationen präsentiert wurden. Weiterhin gab es eine motorische Kontrollbedingung ohne sensorische Stimulation. Die unisensorischen Ergebnisse fanden sich in den modalitätsspezifischen Kortexarealen und bestätigten zum großen Teil bisherige Erkenntnisse und die Existenz der Was- und Wo-Pfade. Bimodale audio-visuelle Stimulation führte zu Aktivierungen im Temporallappen um den STS/STG, welcher bei audio-visueller Integration von großer Bedeutung ist. Aktivierungen im Gyrus cinguli, Gyrus fusiformis und Precuneus spielen anscheinend eine Rolle bei der Präsentation von familiären Stimuli und kongruenten Darbietungskombinationen. Auf Ebene früher visueller und auditorischer Leitungsbahnen zeigte sich ebenfalls audio-visuelle Interaktion. Visuo-haptische Integration scheint sich nach den vorliegenden Daten im LOtv, IPS und ventralen Zerebellum abzuspielen. LOtv und IPS sind vermutlich insbesondere für die Verarbeitung von geometrischen Formen von Bedeutung. Im ventralen Zerebellum kommt es durch kortiko-zerebelläre Verschaltungen zur Beteiligung bei visuo-haptischer Informationsverarbeitung. Die Aktivationsmuster bei audio-haptischer Stimulation deuteten auf Verarbeitung bilateral im temporo-parietalen Bereich hin in Temporallappen, Parietallappen und der Insula. Die Aktivierungen kommen vermutlich durch Feedforward-Projektionen und Konnektivität zwischen auditorischem und somatosensorischem Kortex zustande, in der Insula durch Konvergenz der auditorischen und somatosensorischen Was-Informationen. Trimodale Stimulation zeigte bei Anwendung des Max-Kriteriums, bei welchem die audio-visuo-haptische Aktivität stärker als die einzelnen unisensorischen Signalantworten ausfallen sollte, ein Areal im linken temporo-parietalen Bereich. Bei weniger strenger Auswertung wurde eine weitere Region im rechten Zerebellum nachgewiesen. Beide konnten der Erweiterung des Max-Kriteriums zum Vergleich zwischen tri- und bisensorischer Aktivität nicht standhalten. Beim Affen existiert eine superior temporale polysensorische Region, welche auf visuelle, auditorische und haptische Reize reagiert. Ob ein integratives trisensorisches Areal beim Menschen existiert, sollte in weiteren Studien untersucht werden. Vielleicht liegt eine ausgeprägte interindividuelle Variabilität der Lokalisation des Konvergenzpunktes vor. Eine andere Möglichkeit ist, dass die Verarbeitung im Gehirn vermehrt bisensorisch gegliedert verläuft. Um dies in zukünftigen Studien besser zu untersuchen, könnte es von Vorteil sein simple sensorische Stimuli zu verwenden. Eine Reizdegradierung hätte verminderte Signalamplituden zur Folge, was multisensorische Interaktionen mit nachfolgendem Signalanstieg besser beurteilen ließe. Darüber hinaus könnte in den Studien fMRT-Adaptation angewendet werden. Über die repetitive Stimulusdarbietung kommt es ebenfalls zu verminderter BOLD-Signalantwort. Die vorliegende Studie hat weiterhin die Notwendigkeit der Einführung von Kontrollbedingungen demonstriert. Neben unisensorischen sollten bisensorische Kontrollbedingungen zur Beurteilung trisensorischer Aktivität herangezogen werden. Eine Kontrollbedingung zur Beurteilung des Ruhezustandes ohne sensorischen Reiz sollte ebenfalls vorhanden sein. Prinzipiell scheint das Max-Kriterium zur Detektion multisensorischer Integration angemessen zu sein.
Influenza A (H1N1) 2009 : impact on Frankfurt in due consideration of health care and public health

Background: In April 2009 a novel influenza A H1N1/2009 virus was identified in Mexico and in the United States which quickly spread around the world. Most of the countries established infection surveillance systems in order to track the number of (laboratory-confirmed) H1N1 cases, hospitalizations and deaths. Methods: The impact of the emergence of the novel pandemic (H1N1) 2009 virus on Frankfurt was statistically evaluated by the Health Protection Authority, City of Frankfurt am Main. Vaccination rates of the health care workers (HCWs) of the University Hospital Frankfurt were measured by the Occupational Health Service. Results: Although the virulence of pandemic (H1N1) 2009 seems to be comparable with seasonal influenza, a major patient load and wave of hospital admissions occurred in the summer of 2009. Even though the 2009 vaccination rate of the University Hospital Frankfurt (seasonal influenza [40.5%], swine flu [36.3%]) is better than the average annual uptake of influenza vaccine in the German health care system (approximately 22% for seasonal and 15% for swine flu), vaccination levels remain insufficient. However, physicians were significantly (p < 0.001) more likely to have been vaccinated against swine flu and seasonal influenza than nurses. Conclusions: The outbreak of the pandemic (H1N1) 2009 in April 2009 provided a major challenge to health services around the world. Nosocomial transmission of H1N1/2009 has been documented. Present experience should be used to improve pandemic preparedness plans and vaccination programs ought to target as many HCWs as possible.
Due to an increasing awareness of the potential hazardousness of air pollutants, new laws, rules and guidelines have recently been implemented globally. In this respect, numerous studies have addressed traffic-related exposure to particulate matter using stationary technology so far. By contrast, only few studies used the advanced technology of mobile exposure analysis. The Mobile Air Quality Study (MAQS) addresses the issue of air pollutant exposure by combining advanced high-granularity spatial-temporal analysis with vehicle-mounted, person-mounted and roadside sensors. The MAQS-platform will be used by international collaborators in order 1) to assess air pollutant exposure in relation to road structure, 2) to assess air pollutant exposure in relation to traffic density, 3) to assess air pollutant exposure in relation to weather conditions, 4) to compare exposure within vehicles between front and back seat (children) positions, and 5) to evaluate "traffic zone"- exposure in relation to non-"traffic zone"-exposure. Primarily, the MAQS-platform will focus on particulate matter. With the establishment of advanced mobile analysis tools, it is planed to extend the analysis to other pollutants including including NO2, SO2, nanoparticles, and ozone.
Background: Hemorrhagic shock/resuscitation is associated with aberrant neutrophil activation and organ failure. This experimental porcine study was done to evaluate the effects of Fas-directed extracorporeal immune therapy with a leukocyte inhibition module (LIM) on hemodynamics, neutrophil tissue infiltration, and tissue damage after hemorrhagic shock/resuscitation. Methods: In a prospective controlled double-armed animal trial 24 Munich Mini Pigs (30.3 +/- 3.3 kg) were rapidly haemorrhaged to reach a mean arterial pressure (MAP) of 35 +/- 5 mmHg, maintained hypotensive for 45 minutes, and then were resuscitated with Ringer's solution to baseline MAP. With beginning of resuscitation 12 pigs underwent extracorporeal immune therapy for 3 hours (LIM group) and 12 pigs were resuscitated according to standard medical care (SMC). Haemodynamics, haematologic, metabolic, and organ specific damage parameters were monitored. Neutrophil infiltration was analyzed histologically after 48 and 72 hours. Lipid peroxidation, and apoptosis were specifically determined in lung, bowel, and liver. Results: In the LIM group, neutrophil counts were reduced versus SMC during extracorporeal immune therapy. After 72 hours, the haemodynamic parameters MAP and cardiac output (CO) were significantly better in the LIM group. Histological analyses showed reduction of shock-related neutrophil tissue infiltration in the LIM group, especially in the lungs. Lower amounts of apoptotic cells and lipid peroxidation were found in organs after LIM treatment. Conclusions: Transient Fas-directed extracorporeal immune therapy may protect from posthemorrhagic neutrophil tissue infiltration and tissue damage.
Objective: To summarise the benefits and harms of treatments for women with gestational diabetes mellitus. Design: Systematic review and meta-analysis of randomised controlled trials. Data sources: Embase, Medline, AMED, BIOSIS, CCMed, CDMS, CDSR, CENTRAL, CINAHL, DARE, HTA, NHS EED, Heclinet, SciSearch, several publishers’ databases, and reference lists of relevant secondary literature up to October 2009. Review methods: Included studies were randomised controlled trials of specific treatment for gestational diabetes compared with usual care or "intensified" compared with "less intensified" specific treatment. Results: Five randomised controlled trials matched the inclusion criteria for specific versus usual treatment. All studies used a two step approach with a 50 g glucose challenge test or screening for risk factors, or both, and a subsequent 75 g or 100 g oral glucose tolerance test. Meta-analyses did not show significant differences for most single end points judged to be of direct clinical importance. In women specifically treated for gestational diabetes, shoulder dystocia was significantly less common (odds ratio 0.40, 95% confidence interval 0.21 to 0.75), and one randomised controlled trial reported a significant reduction of pre-eclampsia (2.5 v 5.5%, P=0.02). For the surrogate end point of large for gestational age infants, the odds ratio was 0.48 (0.38 to 0.62). In the 13 randomised controlled trials of different intensities of specific treatments, meta-analysis showed a significant reduction of shoulder dystocia in women with more intensive treatment (0.31, 0.14 to 0.70). Conclusions: Treatment for gestational diabetes, consisting of treatment to lower blood glucose concentration alone or with special obstetric care, seems to lower the risk for some perinatal complications. Decisions regarding treatment should take into account that the evidence of benefit is derived from trials for which women were selected with a two step strategy (glucose challenge test/screening for risk factors and oral glucose tolerance test).
Background: Previously, we showed that glioma pathogenesis related protein (GliPR) is induced in CEM T cells upon HIV-1 infection in vitro. To examine whether GliPR plays a role as HIV dependency factor (HDF), we tested the effect of GliPR suppression by siRNA on HIV-1 replication. Results: Induction of GliPR expression by HIV-1 was confirmed in P4-CCR5 cells. When GliPR was suppressed by siRNA, HIV-1 replication was significantly reduced as measured by HIV-1 transcript levels, HIV-1 p24 protein levels, and HIV-1 LTR-driven reporter gene expression, suggesting that GliPR is a cellular co-factor of HIV-1. Microarray analysis of uninfected HeLa cells following knockdown of GliPR revealed, among a multitude of gene expression alterations, a down-regulation of syndecan-1, syndecan-2, protein kinase C alpha (PRKCA), the catalytic subunit beta of cAMP-dependent protein kinase (PRKACB), nuclear receptor co-activator 3 (NCOA3), and cell surface protein CD59 (protectin), all genes having relevance for HIV-1 pathology. Conclusions: The up-regulation of GliPR by HIV-1 and the early significant inhibition of HIV-1 replication mediated by knockdown of GliPR reveal GliPR as an important HIV-1 dependency factor (HDF), which may be exploited for HIV-1 inhibition.
Background: Autism spectrum disorders (ASDs) are a group of highly heritable neurodevelopmental disorders which are characteristically comprised of impairments in social interaction, communication and restricted interests/behaviours. Several cell adhesion transmembrane leucine-rich repeat (LRR) proteins are highly expressed in the nervous system and are thought to be key regulators of its development. Here we present an association study analysing the roles of four promising candidate genes - LRRTM1 (2p), LRRTM3 (10q), LRRN1 (3p) and LRRN3 (7q) - in order to identify common genetic risk factors underlying ASDs.
Methods: In order to gain a better understanding of how the genetic variation within these four gene regions may influence susceptibility to ASDs, a family-based association study was undertaken in 661 families of European ancestry selected from four different ASD cohorts. In addition, a case-control study was undertaken across the four LRR genes, using logistic regression in probands with ASD of each population against 295 ECACC controls.
Results: Significant results were found for LRRN3 and LRRTM3 (P < 0.005), using both single locus and haplotype approaches. These results were further supported by a case-control analysis, which also highlighted additional SNPs in LRRTM3.
Conclusions: Overall, our findings implicate the neuronal leucine-rich genes LRRN3 and LRRTM3 in ASD susceptibility.
Background: Initiated by a clinical case of critical endotracheal tube (ETT) obstruction, we aimed to determine factors that potentially contribute to the development of endotracheal tube obstruction by its inflated cuff. Prehospital climate and storage conditions were simulated. Methods: Five different disposable ETTs (6.0, 7.0, and 8.0 mm inner diameter) were exposed to ambient outside temperature for 13 months. In addition, every second of these tubes was mechanically stressed by clamping its cuffed end between the covers of a metal emergency case for 10 min. Then, all tubes were heated up to normal body temperature, placed within the cock of a syringe, followed by stepwise inflation of their cuffs to pressures of 3 kPa and >=12 kPa, respectively. The inner lumen of the ETT was checked with the naked eye for any obstruction caused by the external cuff pressure. Results: Neither in tubes that were exposed to ambient temperature (range: -12°C to +44°C) nor in those that were also clamped, visible obstruction by inflated cuffs was detected at any of the two cuff pressure levels. Conclusions: We could not demonstrate a critical obstruction of an ETT by its inflated cuff, neither when the cuff was over-inflated to a pressure of 12 kPa or higher, nor in ETTs that had been exposed to unfavorable storage conditions and significant mechanical stress.
The human immunodeficiency virus type 1 (HIV-1) coreceptor use and viral evolution were analyzed in blood samples from an HIV-1 infected patient undergoing allogeneic stem cell transplantation (SCT). Coreceptor use was predicted in silico from sequence data obtained from the third variable loop region of the viral envelope gene with two software tools. Viral diversity and evolution was evaluated on the same samples by Bayesian inference and maximum likelihood methods. In addition, phenotypic analysis was done by comparison of viral growth in peripheral blood mononuclear cells and in a CCR5 (R5)-deficient T-cell line which was controlled by a reporter assay confirming viral tropism. In silico coreceptor predictions did not match experimental determinations that showed a consistent R5 tropism. Anti-HIV directed antibodies could be detected before and after the SCT. These preexisting antibodies did not prevent viral rebound after the interruption of antiretroviral therapy during the SCT. Eventually, transplantation and readministration of anti-retroviral drugs lead to sustained increase in CD4 counts and decreased viral load to undetectable levels. Unexpectedly, viral diversity decreased after successful SCT. Our data evidence that only R5-tropic virus was found in the patient before and after transplantation. Therefore, blocking CCR5 receptor during stem cell transplantation might have had beneficial effects and this might apply to more patients undergoing allogeneic stem cell transplantation. Furthermore, we revealed a scenario of HIV-1 dynamic different from the commonly described ones. Analysis of viral evolution shows the decrease of viral diversity even during episodes with bursts in viral load.
Background: The importance of the Notch signaling in the development of glomerular diseases has been recently described. Therefore we analyzed in podocytes the expression and activity of ADAM10, one important component of the Notch signaling complex. Methods: By Western blot, immunofluorescence and immunohistochemistry analysis we characterized the expression of ADAM10 in human podocytes, human urine and human renal tissue. Results: We present evidence, that differentiated human podocytes possessed increased amounts of mature ADAM10 and released elevated levels of L1 adhesion molecule, one well known substrate of ADAM10. By using specific siRNA and metalloproteinase inhibitors we demonstrate that ADAM10 is involved in the cleavage of L1 in human podocytes. Injury of podocytes enhanced the ADAM10 mediated cleavage of L1. In addition, we detected ADAM10 in urinary podocytes from patients with kidney diseases and in tissue sections of normal human kidney. Finally, we found elevated levels of ADAM10 in urinary vesicles of patients with glomerular kidney diseases. Conclusions: The activity of ADAM10 in human podocytes may play an important role in the development of glomerular kidney diseases.
Background: B. burgdorferi sensu lato (sl) is the etiological agent of Lyme borreliosis in humans. Spirochetes have adapted themselves to the human immune system in many distinct ways. One important immune escape mechanism for evading complement activation is the binding of complement regulators Factor H (CFH) or Factor H-like protein1 (FHL-1) to Complement Regulator-Acquiring Surface Proteins (CRASPs). Results: We demonstrate that B. garinii OspA serotype (ST4) PBi resist complement-mediated killing by binding of FHL-1. To identify the primary ligands of FHL-1 four CspA orthologs from B. garinii ST4 PBi were cloned and tested for binding to human CFH and FHL-1. Orthologs BGA66 and BGA71 were found to be able to bind both complement regulators but with different intensities. In addition, all CspA orthologs were tested for binding to mammalian and avian CFH. Distinct orthologs were able to bind to CFH of different animal origins. Conclusions: B. garinii ST4 PBi is able to evade complement killing and can bind FHL-1 to membrane expressed proteins. Recombinant proteins BGA66 can bind FHL-1 and human CFH, while BGA71 can bind only FHL-1. All recombinant CspA orthologs from B. garinii ST4 PBi can bind CFH from different animal origins. This partly explains the wide variety of animals that can be infected by B. garinii.
Introduction: Hypothermia improves survival and neurological recovery after cardiac arrest. Pro-inflammatory cytokines have been implicated in focal cerebral ischemia/reperfusion in-jury. It is unknown whether cardiac arrest also triggers the release of cerebral inflammatory molecules, and whether therapeutic hypothermia alters this inflammatory response. This study sought to examine whether hypothermia or the combination of hypothermia with anes-thetic postconditioning with sevoflurane affect cerebral inflammatory response after cardio-pulmonary resuscitation. Methods: Thirty pigs (28 - 34kg) were subjected to cardiac arrest following temporary coro-nary artery occlusion. After 7 minutes of ventricular fibrillation and 2 minutes of basic life support, advanced cardiac life support was started according to the current AHA guidelines. Return of spontaneous circulation was achieved in 21 animals who were randomized to ei-ther normothermia at 38degreesC, hypothermia at 33degreesC or hypothermia at 33degreesC combined with se-voflurane (each group: n = 7) for 24 hours. The effects of hypothermia and the combination of hypothermia with sevoflurane on cerebral inflammatory response after cardiopulmonary resuscitation were studied using tissue samples from the cerebral cortex of pigs euthanized after 24 hours and employing quantitative RT-PCR and ELISA techniques. Results: Global cerebral ischemia following resuscitation resulted in significant upregulation of cerebral tissue inflammatory cytokine mRNA expression (mean +/- SD; interleukin (IL)-1beta 8.7 +/- 4.0, IL-6 4.3 +/- 2.6, IL-10 2.5 +/- 1.6, tumor necrosis factor (TNF)alpha 2.8 +/- 1.8, intercellular adhesion molecule-1 (ICAM-1) 4.0 +/- 1.9-fold compared with sham control) and IL-1beta protein concentration (1.9 +/- 0.6-fold compared with sham control). Hypothermia was associated with a significant (P <0.05 versus normothermia) reduction in cerebral inflammatory cytokine mRNA expression (IL-1beta 1.7 +/- 1.0, IL-6 2.2 +/- 1.1, IL-10 0.8 +/- 0.4, TNFalpha 1.1 +/- 0.6, ICAM-1 1.9 +/- 0.7-fold compared with sham control). These results were also confirmed for IL-1beta on protein level. Experimental settings employing hypothermia in combination with sevoflurane showed that the volatile anesthetic did not confer additional anti-inflammatory effects com-pared with hypothermia alone. Conclusions: Mild therapeutic hypothermia resulted in decreased expression of typical ce-rebral inflammatory mediators after cardiopulmonary resuscitation. This may confer, at least in part, neuroprotection following global cerebral ischemia and resuscitation.
Die Rolle von Antiepileptika im Zusammenhang mit dem Auftreten letaler kardialer Arrhythmien wird kontrovers diskutiert. Zum aktuellen Zeitpunkt ist bei Patienten mit Epilepsie die Ursache für die im Vergleich zur Normalbevölkerung erhöhte Mortalität nicht hinreichend geklärt. Die Ätiologie und Pathophysiologie des Sudden unexpected death in epilepsy patients (SUDEP) ist nur teilweise bekannt, wodurch die Durchführung präventiver Maßnahmen nur bedingt möglich ist. Es konnten bestimmte Risikofaktoren für das Auftreten eines SUDEP identifiziert werden, wobei unter anderem die antikonvulsive Polytherapie einen unabhängigen Risikofaktor darstellt. Bezüglich der pathogenetischen Mechanismen spielen neben der Entstehung eines neurogenen pulmonalen Ödems und einer zentralen Apnoe auch kardiale Arrhythmien eine bedeutende Rolle. Valproinsäure als ein sehr häufig eingesetztes Antiepileptikum wird verdächtigt, eine Verlängerung der QT-Zeit im Elektrokardiogramm (EKG) zu bewirken. Eine Verlängerung der Plateauphase des Aktionspotentials führt zu einer instabilen Repolarisation, wodurch einfallende frühe Nachdepolarisationen bereits wieder ein Aktionspotential auslösen können. Ein erworbenes Long-QT-Syndrom (LQTS), zu dem auch die medikamentenbedingte QT-Verlängerung zählt, birgt das Risiko, durch die Auslösung von Herzrhythmusstörungen im Sinne von Torsade de Pointes an einem plötzlichen Herztod zu versterben. Im Rahmen dieser prospektiven Studie wurden 50 Kinder mit Epilepsie unter der Therapie mit Valproinsäure bezüglich des Auftretens kardialer Nebenwirkungen untersucht. Das Studienprotokoll beinhaltete ein standardisiertes EKG mit Bestimmung der Zeiten, Lagetypen, Hypertrophie- und Schädigungszeichen. Parallel wurde die Dosierung von Valproinsäure und deren Plasmaspiegel bestimmt. Ziel der vorliegenden Arbeit war es herauszufinden, ob die antikonvulsive Therapie mit Valproinsäure durch die Auslösung eines medikamentenbedingten LQTS zu einem erhöhten kardialen SUDEP-Risko bei pädiatrischen Epilepsiepatienten führen könnte. Die 25 weiblichen und 25 männlichen Patienten im mittleren Alter von 12,2 Jahren wurden wegen verschiedener Epilepsieformen behandelt. Sie nahmen Valproinsäure durchschnittlich über eine Zeitdauer von 5,8 Jahren ein (Range 3 Monate bis 32 Jahre), wobei 58% eine Monotherapie, 26% in Zweier- und 16% eine Dreier-56 Kombinationstherapie erhielten. Die durchschnittliche Dosierung lag bei 26,6 mg/ kgKG/ Tag (Range 10,5 bis 63,8 mg/ kgKG/ Tag), der im Plasma gemessene Valproinsäure-Spiegel zwischen 19,7 und 147,4 μg/ml (Mittelwert 88,8 ± 24,3 μg/ml; Median 91 μg/ml). Die gemessenen EKG-Zeiten lagen alle im altersentsprechenden Normbereich: --> PR-Intervall: Range 100-170 ms, Median: 124 ms, Mittelwert: 126,7 ± 18,5 ms --> QRS-Dauer: Range 56-108 ms, Median 79 ms, Mittelwert 79,94 ± 11,3 ms --> QT-Zeit: Range 264-400 ms, Median 348 ms, Mittelwert 343,34 ± 35,9 ms -->QTc-Zeit (Normwert < 440 ms bei Jungen und < 460 ms bei Mädchen): Range 377-433 ms, Median 403 ms, Mittelwert 402,9 ± 11,1 ms. Bezüglich der Lagetypen fanden sich bei 42% ein Indifferenz-, bei 46% ein Steil- und bei 8% ein Linkstyp. Bei einem Patienten mit Tuberöser Sklerose lag ein Rechtstyp vor. Dieser wies einen vollständigen Rechtsschenkelblock auf, welcher bei Zustand nach reseziertem Rhabdomyom des rechten Ventrikels durch die operative Durchtrennung des Reizleitungssystems verursacht wurde. Den Ergebnissen dieser Untersuchung zufolge lassen sich bei Kindern unter Therapie mit Valproinsäure keine Veränderungen im EKG, insbesondere keine QT-Verlängerungen, als möglicher Risikofaktor für eine kardiale Arrhythmie nachweisen. Aus diesen Daten lässt sich schlussfolgern, dass dieses Antiepileptikum nicht das Risiko erhöht, an einem QT-bedingten SUDEP zu versterben. Trotz eines erhöhten Risikos unter einer antiepileptischen Polytherapie kann bisher nicht von einer erhöhten Inzidenz durch die Therapie mit einem bestimmten Antikonvulsivum ausgegangen werden. Weitere Untersuchungen sind notwendig, um die Rolle der einzelnen Pharmaka in diesem Zusammenhang besser einschätzen zu können. Bisher ist davon auszugehen, dass eine gute Anfallskontrolle das SUDEP-Risiko entscheidend verringert. Bis der Pathomechanismus des SUDEP detaillierter geklärt ist, gelten eine Optimierung der antiepileptischen Therapie und eine Vermeidung von bisher bekannten Risikofaktoren als sinnvollste präventive Maßnahmen. Abschließend ist festzuhalten, dass sich im Rahmen dieser Untersuchung bei Kindern unter antikonvulsiver Therapie mit Valproinsäure kein Hinweis auf QT-Verlängerungen als möglicher Risikofaktor für einen SUDEP fand.
Zahlreiche Studien haben gezeigt, dass sich trotz der Verwendung von Dentinadhäsiven in deninbegrenzten Klasse-II-Kavitäten keine randspaltfreien Füllungen erzielen lassen. Die Ergebnisse dieser in-vitro-Studie zeigen dies ebenfalls, dass mit Dentinadhäsiven der fünften und sechsten Generation keine komplett spaltfreien Kompositfüllungen im Dentin hergestellt werden können. Zu diesem Schluss kam auch Manhart, J. et al 2000. Selbst bei Anwendung moderner Dentinadhäsivsysteme unter Einhaltung aller Verarbeitungsvorschriften lässt sich das Zustandekommen einer Komposit-Dentinhaftung in Klasse-II-Kavitäten nicht mit Sicherheit vorhersagen. Bei allen getesteten Dentinadhäsiven wurde an den Kavitätenrändern Farbstoffpenetration nachgewiesen. Die Farbstoffpenetration zeigte an den Kavitäten die Tendenz von approximal nach pulpal geringer zu werden. Optibond Solo mit Füllstoffgehalt erreichte in der Farbstoffpenetration am zervikalenFüllungsrand mit (54,54 %) und ohne (31,89 %) Liquorsimulation die besten Resultate. Auch am oralen Füllungsrand hatte Optibond Solo mit (36,38 %) und ohne (37,09 %) Liquorfluss die besten Ergebnisse. An der vestibulären Kavitätengrenze waren Optibond Solo und Syntac Sprint an der Spitze: mit Liquorsimulation Syntac Sprint 32,68 %, Optibond Solo 38,42 %; ohne Liquorsimulation Syntac Sprint 35,97 %, Optibond Solo 35,3 %. Alle Ränder zusammen betrachtet, wies ebenfalls Optibond Solo die geringste Farbstoffpenetration auf: mit Liquor 43,2 %, ohne Liquor 34,72 %. Das wasserhaltige Syntac SC belegte den letzten Platz bei Auswertung aller Ränder zusammen mit Liquor 82,89 %, ohne Liquorsimulation war hingegen Etch & Prime 3.0 absolutes Schlusslicht (80,03 %). Das schlechteste Ergebnis erreichte Syntac SC ebenfalls am vestibulären Füllungsrand ohne (75,59 %) und mit Liquorsimulation (81,16 %), sowie zervikal mit Liquor (95,32 %). Syntac SC konnte genauso mit der angewandten Moist-bonding-Technik in der REM (20 % "perfekte Ränder" vor und 8 % nach Thermocycling) nur den letzten Platz einnehmen. Das acetonbasierende Adhäsiv Syntac Sprint erlangte in der REM sowohl vor (77 %) als auch nach (76 %) Temperaturwechselbadbelastung den höchsten Anteil an "perfekten Rändern", gefolgt von Optibond Solo mit 65 % vor und 50 % nach Thermocycling. Die Temperaturwechselbadbelastung hatte Einfluss auf die Qualität der Randadaptation. Bei allen untersuchten Dentinadhäsiven reduzierte sich der prozentuale Anteil an perfekten Rändern nach Thermocycling im Vergleich zuvor. Die Qualität des Zustandekommens eines akzeptablen Füllungsrandes ist von der chemischen Zusammensetzung, insbesondere dem enthaltenen Lösungsmittel, dem Füllstoffgehalt und der Dentinfeuchtigkeit abhängig. Die Durchführung der Simulation des Liquors führte am zervikalen Rand bei Syntac SC (ohne: 92,54 %; mit: 95,32 %), Optibond Solo (ohne: 31,89 %; mit: 54,55 %) und Scotchbond 1 (ohne: 63,44 %; mit: 83,24 %) zur erhöhten Farbstoffpenetration - im Falle von Etch & Prime 3.0 (ohne: 93,11 %; mit: 86,42 %), Syntac Sprint (ohne: 81,98 %; mit: 73,75 %) und Prime & Bond NT (ohne: 88,86 %; mit: 81,01 %) jedoch zu einer verringerten Penetration des Methylenblaus.
Intracarotid administration of human bone marrow mononuclear cells in rat photothrombotic ischemia

Background: Increasing evidence suggests that cell therapy improves functional recovery in experimental models of stroke and myocardial infarction. So far only small pilot trials tested the effects of cell therapy in stroke patients, whereas large clinical trials were conducted in patients with ischemic heart disease. To investigate the therapeutic benefit of cell therapy to improve the recovery after stroke, we determined the efficacy of bone marrow derived mononuclear cells, which were shown to improve the recovery in experimental and clinical acute myocardial infarction studies, in a rat stroke model. Methods: Adult male Wistar rats were randomly assigned to receive either five million human bone marrow mononuclear cells (hBMC) or placebo intraarterially 3 days after photothrombotic ischemia. For immunosuppression the animals received daily injections of cyclosporine throughout the experiment, commencing 24 hours before the cell transplantation. A battery of behavioural tests was performed before and up to 4 weeks after ischemia. Results: Body temperature and body weight revealed no difference between groups. Neurological deficits measured by the Rotarod test, the adhesive-removal test and the cylinder test were not improved by hBMC transplantation compared to placebo. Conclusions: This study demonstrates that hBMC do not improve functional recovery when transplanted intraaterially 3 days after the onset of focal cerebral ischemia. A possible reason for the failed neurological improvement after cell therapy might be the delayed treatment initiation compared to other experimental stroke studies that showed efficacy of bone marrow mononuclear cells.
Vascularization of the vertebrate brain takes place during embryonic development from a preformed perineural vascular plexus. As a consequence of the intimate contact with neuroectodermal cells the vessels, which are entering the brain exclusively via sprouting angiogenesis, acquire and maintain unique barrier properties known as the blood-brain barrier (BBB). The endothelial BBB depends upon the close association of endothelial cells with pericytes, astrocytes, neurons and microglia, which are summarized in the term neuro-vascular unit. Although it is known since decades that the CNS tissue provides the cues for BBB induction and differentiation in endothelial cells, the molecular mechanism remained obscure. Only recently, the canonical Wnt/beta-catenin pathway and the Wnt7a/7b growth factors have been implicated in brain angiogenesis on the one hand and in BBB induction on the other. This breakthrough in understanding the differentiation of the brain vasculature prompted us to review these findings embedded in the emerging concepts of Wnt signaling in the vasculature. In particular, interactions with other pathways that are crucial for vascular development such as VEGF, Notch, angiopoietins and Sonic hedgehog are discussed. Finally, we considered the potential role of the Wnt pathway in vascular brain pathologies in which BBB function is hampered, as for example in glioma, stroke and Alzheimer's disease.
