Bipolar disorder (BD) is a genetically complex mental illness characterized by severe oscillations of mood and behavior. Genome-wide association studies (GWAS) have identified several risk loci that together account for a small portion of the heritability. To identify additional risk loci, we performed a two-stage meta-analysis of >9 million genetic variants in 9,784 bipolar disorder patients and 30,471 controls, the largest GWAS of BD to date. In this study, to increase power we used ~2,000 lithium-treated cases with a long-term diagnosis of BD from the Consortium on Lithium Genetics, excess controls, and analytic methods optimized for markers on the Xchromosome. In addition to four known loci, results revealed genome-wide significant associations at two novel loci: an intergenic region on 9p21.3 (rs12553324, p = 5.87×10-9; odds ratio = 1.12) and markers within ERBB2 (rs2517959, p = 4.53×10-9; odds ratio = 1.13). No significant X-chromosome associations were detected and X-linked markers explained very little BD heritability. The results add to a growing list of common autosomal variants involved in BD and illustrate the power of comparing well-characterized cases to an excess of controls in GWAS.
Der Zusammenhang zwischen kognitiver Emotionsregulation, positivem Aufmerksamkeitsbias und Resilienz

Das Verständnis von Faktoren, die die Widerstandsfähigkeit gegen Stress fördern, ist entscheidend für die Entwicklung von Stresspräventionsprogrammen und für die Verbesserung der Behandlung stressbedingter Störungen. Zum einen war es Ziel der vorliegenden Forschungsarbeit, den Einfluss der kognitiven Emotionsregulation (ER) auf die psychische Gesundheit zu untersuchen und bereits vorhandene Ergebnisse zum Einfluss der Emotionsregulation auf die Resilienz zu replizieren. Es wird zunehmend anerkannt, dass die meisten psychiatrischen Erkrankungen mit Emotionsdysregulation einhergehen und dass klinische Interventionen davon profitieren, wenn sie auf einem empirischen Verständnis der Emotionsprozesse beruhen.
Der Hauptfokus lag zudem darauf, zu untersuchen, ob es einen Zusammenhang zwischen Resilienz und einem Aufmerksamkeitsbias auf positive Informationen, einen Positivitätsbias, gibt.
In der vorliegenden Studie wurde eine Stichprobe psychisch gesunder Teilnehmer (n=229) im Alter von 18 bis 55 Jahren herangezogen, denen Bilder aus dem „International Affective Picture System“ präsentiert wurden. Dabei bekamen sie die Anweisung, negative Emotionen zu Fotos durch kognitives Umbewerten oder Distanzieren herunterzuregulieren. Die Reaktionszeiten sowie die Erregungsbewertungen wurden für die Bedingungen Neubewertung, Distanzieren, negatives passives Betrachten sowie neutrales passives Betrachten erhoben. Zudem wurde die Aufmerksamkeitsverzerrung gegenüber positiven und negativen Reizen mithilfe einer Visual Dot-Probe Aufgabe untersucht. Gemessen wurden die Reaktionszeiten der Studienteilnehmer bei Reaktionen auf einen Stimulus, der auf die Präsentation emotionaler Gesichter folgt, im Vergleich zu Reaktionen auf einen Stimulus, der an die Stelle von neutralen Gesichtern rückt. Hieraus wurden Aufmerksamkeitsverzerrungen abgeleitet.
Die anschließende Datenanalyse und statistische Auswertung konnten zeigen, dass die Neubewertung im Vergleich zum passiven Betrachten der negativen Bilder eine längere Reaktionszeit aufweist, was darauf hinweist, dass die kognitive Emotionsregulation möglicherweise anstrengendere kognitive Kontrollprozesse aktiviert. Hinsichtlich des Zusammenhangs mit der Resilienz konnte eine signifikante positive Korrelation der Reaktionszeitdifferenzen der Emotionsregulationsstrategien Distanzieren und Neubewerten mit dem Resilienz-Score beobachtet werden.
Bei den Erregungsbewertungen der Emotionsregulation zeigte sich weder ein signifikanter Effekt der Bedingung noch ein Zusammenhang mit dem Resilienz-Score.
Die Ergebnisse der Visual Dot-Probe zeigten, dass es keine Unterschiede in den Reaktionszeiten nach der Präsentation des neutralen sowie des emotionalen Stimulus gab. Auch zeigten sich keine Zusammenhänge mit der Resilienz.
Der erwartete Zusammenhang zwischen Resilienz und einem Aufmerksamkeitsbias auf positive Informationen konnte in den Ergebnissen nicht gefunden werden. In den vorliegenden Daten zeigte sich hingegen eine signifikante negative Korrelation der Fähigkeit der kognitiven Neubewertung mit der Höhe des negativen Aufmerksamkeits Bias-Scores.
Somit liefert die vorliegende Arbeit einen Hinweis darauf, dass Personen mit einem hohen Maß an Emotionsregulationskapazitäten eine höhere Aufmerksamkeitslenkung weg von emotional negativen Stimuli aufweisen.
Bezüglich der Anwendung auf klinische Stichproben besteht noch Raum zu
analysieren, ob es sich um ein stabiles Phänomen handelt, das auf diese übertragen werden kann.
Integraseinhibitoren sind bereits seit Jahren in der Initialtherapie HIV-positiver Erwachsener empfohlen, in der Schwangerschaft aber bisher aber meist nur unter besonderen Voraussetzungen (z.B. bei hoher maternaler Viruslast kurz vor Entbindung) eingesetzt.
Ziel dieser retrospektiven Studie ist es, die Effektivität und Verträglichkeit integraseinhibitorhaltiger ART bei schwangeren Patientinnen zu untersuchen und diese mit integraseinhibitorfreien Regimen zu vergleichen.
Die quantitative Viruslast bzw. die Suppression der Viruslast zum Zeitpunkt der Entbindung stellte die primäre Zielgröße der Studie dar, Indikationen für einen INSTI als Intensivierung der ART oder Gründe für einen Therapiewechsel sowie unerwünschte Wirkungen bei Mutter und Kind waren sekundäre Zielgrößen.
Hierzu wurden retrospektive Daten aus der Epidem-Datenbank des HIVCENTERs und aus den Patientinnenakten der teilnehmenden Frankfurter Schwerpunktpraxen extrahiert und in die Auswertung einbezogen.
Im Beobachtungzeitraum vom 1. Januar 2008 bis zum 01. Juni 2018 wurden n=274 Schwangerschaften erfasst, diese resultierten in n=281 Kindern (fünf Zwillinge und einmal Drillinge). INSTI-haltige Therapieregime wurden bei 52 (19%) der Schwangerschaften eingesetzt, darunter zumeist Raltegravir (92%).
Die maternale Viruslast zum Zeitpunkt der Geburt unterschied sich in beiden Therapiegruppen nicht signifikant, in beiden Subgruppen konnte eine signifikante Reduktion der Viruslast zum Zeitpunkt der Entbindung erreicht werden.
Angiogenesis, the process by which endothelial cells (ECs) form new blood vessels from existing ones, is intimately linked to the tissue’s metabolic milieu and often occurs at nutrient-deficient sites. However, ECs rely on sufficient metabolic resources to support growth and proliferation. How endothelial nutrient acquisition and usage are regulated is unknown. Here we show that these processes are instructed by Yes-associated protein 1 (YAP)/WW domain-containing transcription regulator 1 (WWTR1/TAZ)-transcriptional enhanced associate domain (TEAD): a transcriptional module whose function is highly responsive to changes in the tissue environment. ECs lacking YAP/TAZ or their transcriptional partners, TEAD1, 2 and 4 fail to divide, resulting in stunted vascular growth in mice. Conversely, activation of TAZ, the more abundant paralogue in ECs, boosts proliferation, leading to vascular hyperplasia. We find that YAP/TAZ promote angiogenesis by fuelling nutrient-dependent mTORC1 signalling. By orchestrating the transcription of a repertoire of cell-surface transporters, including the large neutral amino acid transporter SLC7A5, YAP/TAZ-TEAD stimulate the import of amino acids and other essential nutrients, thereby enabling mTORC1 activation. Dissociating mTORC1 from these nutrient inputs—elicited by the loss of Rag GTPases—inhibits mTORC1 activity and prevents YAP/TAZ-dependent vascular growth. Together, these findings define a pivotal role for YAP/TAZ-TEAD in controlling endothelial mTORC1 and illustrate the essentiality of coordinated nutrient fluxes in the vasculature.
Purpose: Trauma is the leading cause of death in children. In adults, blood transfusion and fluid resuscitation protocols changed resulting in a decrease of morbidity and mortality over the past 2 decades. Here, transfusion and fluid resuscitation practices were analysed in severe injured children in Germany.
Methods: Severely injured children (maximum Abbreviated Injury Scale (AIS) ≥ 3) admitted to a certified trauma-centre (TraumaZentrum DGU®) between 2002 and 2017 and registered at the TraumaRegister DGU® were included and assessed regarding blood transfusion rates and fluid therapy.
Results: 5,118 children (aged 1–15 years) with a mean ISS 22 were analysed. Blood transfusion rates administered until ICU admission decreased from 18% (2002–2005) to 7% (2014–2017). Children who are transfused are increasingly seriously injured. ISS has increased for transfused children aged 1–15 years (2002–2005: mean 27.7–34.4 in 2014–2017). ISS in non-transfused children has decreased in children aged 1–15 years (2002–2005: mean 19.6 to mean 17.6 in 2014–2017). Mean prehospital fluid administration decreased from 980 to 549 ml without affecting hemodynamic instability.
Conclusion: Blood transfusion rates and amount of fluid resuscitation decreased in severe injured children over a 16-year period in Germany. Restrictive blood transfusion and fluid management has become common practice in severe injured children. A prehospital restrictive fluid management strategy in severely injured children is not associated with a worsened hemodynamic state, abnormal coagulation or base excess but leads to higher hemoglobin levels.
Introduction: MDRO-colonization has been shown to impair survival in patients with hematological malignancies and solid tumors as well as in patients with liver disease. Despite the increasing spread of multidrug-resistant organisms (MDRO), its impact on patients with hepatocellular carcinoma (HCC) has not been studied. We conducted this retrospective study to analyze the impact of MDRO-colonization on overall prognosis in HCC patients.
Materials and methods: All patients with confirmed HCC diagnosed between January 2008 and December 2017 at the University Hospital Frankfurt were included in this study. HCC patients with a positive MDRO screening before or within the first 90 days after diagnosis of HCC were defined as colonized HCC patients, HCC patients with a negative MDRO screening were defined as noncolonized HCC patients.
Results: 59 (6%) colonized and 895 (94%) noncolonized HCC patients were included. Enterobacterales with extended-spectrum β-lactamase-like phenotype with or without resistance to fluoroquinolones (ESBL/ ± FQ) were the most frequently found MDRO with 59%, followed by vancomycin-resistant Enterococcus faecium with 37%. Colonized HCC patients had more severe cirrhosis and more advanced HCC stage compared to noncolonized HCC patients. Colonized HCC patients showed an impaired survival with a median OS of 189 days (6.3 months) compared to a median OS of 1001 days (33.4 months) in noncolonized HCC patients. MDRO-colonization was identified as an independent risk factor associated with survival in multivariate analysis.
Conclusion: MDRO-colonization is an independent risk factor for survival in patients with HCC highlighting the importance of regular MDRO screening, isolation measures as well as interdisciplinary antibiotic steward-ship programs to guide responsible use of antibiotic agents.
Treatment‐related complications contribute substantially to morbidity and mortality in acute myeloid leukemia (AML) patients undergoing induction chemotherapy. Although AML patients are susceptible to fluid overload (FO) (e.g., in the context of chemotherapy protocols, during sepsis treatment or to prevent tumor lysis syndrome), little attention has been paid to its role in AML patients undergoing induction chemotherapy. AML patients receiving induction chemotherapy between 2014 and 2019 were included in this study. FO was defined as ≥5% weight gain on day 7 of induction chemotherapy compared to baseline weight determined on the day of admission. We found FO in 23 (12%) of 187 AML patients undergoing induction chemotherapy. Application of >100 ml crystalloid fluids/kg body weight until day 7 of induction chemotherapy was identified as an independent risk factor for FO. AML patients with FO suffered from a significantly increased 90-day mortality rate and FO was demonstrated as an independent risk factor for 90-day mortality. Our data suggests an individualized, weight-adjusted calculation of crystalloid fluids in order to prevent FO-related morbidity and mortality in AML patients during induction chemotherapy. Prospective trials are required to determine the adequate fluid management in this patient population.
Parodontitis ist eine chronisch entzündliche nichtübertragbare Erkrankung, die alle Anteile des Zahnhalteapparates (Parodonts) betrifft und dort weitgehend irreversible Schäden verursacht. Schätzungen legen nahe, dass in Deutschland ca. 10 Mio. Menschen an einer schweren Parodontitis erkrankt sind. Parodontitis zeigt über viele Jahre zumeist wenige oder nur milde Symptome, die von den Patienten oft nicht wahrgenommen oder richtig eingeordnet werden. Fehlendes Bewusstsein kann dazu führen, dass zahnärztliche Behandlung erst in einem fortgeschrittenen Erkrankungsverlauf in Anspruch genommen wird, wenn umfangreiche Therapiemaßnahmen notwendig geworden sind und sich die Prognose für den Erhalt der Zähne verschlechtert hat. Der parodontale Screeningindex (PSI) ist ein einfaches und schnelles Instrument, mit dem die Notwendigkeit weiterführender diagnostischer Maßnahmen beurteilt werden kann. Der Index wird mittlerweile bei vielen Patienten durchgeführt. Trotzdem bleiben die Versorgungszahlen niedrig und hinter dem zurück, was für das Absenken der bestehenden Parodontitislast notwendig wäre. Jede Zahnarztpraxis muss in der Lage sein, Parodontitistherapie umzusetzen. Fachzahnärzte oder Spezialisten können die allgemeinzahnärztlichen Kollegen wesentlich bei der Behandlung von schweren Formen von Parodontitis unterstützen. Dazu ist eine Aufwertung des Faches in der universitären Ausbildung erforderlich, aber auch die zunehmende postgraduale Ausdifferenzierung von Spezialisten oder Fachzahnärzten für Parodontologie. Die neuen Behandlungsrichtlinien für die Parodontaltherapie (PAR-Therapie) erlauben die Versorgung der parodontal erkrankten Patienten auf Basis international anerkannter wissenschaftlicher Standards und verbessern damit die Rahmenbedingungen für die Parodontitistherapie in der zahnärztlichen Praxis.
hintergrund: Männer in Deutschland sterben früher als Frauen und nehmen weniger häufig Krebsvorsorgeuntersuchungen wahr.
Fragestellung: Ziel war die prospektive Evaluation einer „Movember-Gesundheitsinitiative“ am Universitätsklinikum Frankfurt (UKF) im November 2019.
Methoden: Im Rahmen der „Movember-Gesundheitsinitiative“ wurde allen männlichen Mitarbeitern des UKF ab dem 45. Lebensjahr und bei erstgradiger familiärer Vorbelastung eines Prostatakarzinoms ab dem 40. Lebensjahr im November 2019 gemäß S3-Leitlinien der Deutschen Gesellschaft für Urologie (DGU) eine Prostatakarzinom-Vorsorgeuntersuchung angeboten.
Ergebnisse: Insgesamt nahmen 14,4 % der Mitarbeiter teil. Eine familiäre Vorbelastung gaben insgesamt 14,0 % Teilnehmer an. Das mediane Alter betrug 54 Jahre. Der mediane PSA(prostataspezifisches Antigen)-Wert lag bei 0,9 ng/ml, der mediane PSA-Quotient bei 30 %. Bei 5 % (n = 6) zeigte sich ein suspekter Tastbefund in der DRU (digital-rektale Untersuchung). Nach Altersstratifizierung (≤ 50 vs. > 50 Lebensjahre) zeigten sich signifikante Unterschiede im medianen PSA-Wert (0,7 ng/ml vs. 1,0 ng/ml, p < 0,01) und der bereits zuvor durchgeführten urologischen Vorsorge (12,1 vs. 42,0 %, p < 0,01). Vier Teilnehmer (3,3 %) zeigten erhöhte Gesamt-PSA-Werte. Bei 32,2 % der Teilnehmer zeigte sich mindestens ein kontrollbedürftiger Befund. Insgesamt wurden 6 Prostatabiopsien durchgeführt. Hierbei zeigte sich in einem Fall ein intermediate-risk Prostatakarzinom (Gleason 3 + 4, pT3a, pPn1, pNx, R0).
Schlussfolgerungung: Im Rahmen der UKF-Movember-Gesundheitsinitiative 2019 konnten durch ein Vorsorgeangebot 121 Männer für eine Prostatakrebs-Vorsorge inklusive PSA-Testung gewonnen werden. Auffällige/kontrollbedürftige Befunde zeigten sich bei 32,2 %. Bei einem Mitarbeiter wurde ein therapiebedürftiges Prostatakarzinom entdeckt und therapiert.
Lifestyle interventions including meal replacement are suitable for prevention and treatment of obesity and type-2-diabetes. Since leptin is involved in weight regulation, we hypothesised that a meal replacement-based lifestyle intervention would reduce leptin levels more effectively than lifestyle intervention alone. In the international, multicentre, randomised-controlled ACOORH-trial (Almased-Concept-against-Overweight-and-Obesity-and-Related- Health-Risk), overweight or obese participants with metabolic syndrome criteria (n = 463) were randomised into two groups and received telemonitoring devices and nutritional advice. The intervention group additionally used a protein-rich, low-glycaemic meal replacement. Data were collected at baseline, after 1, 3, 6, and 12 months. All datasets providing leptin data (n = 427) were included in this predefined subanalysis. Serum leptin levels significantly correlated with sex, body mass index, weight, and fat mass at baseline (p < 0.0001). Stronger leptin reduction has been observed in the intervention compared to the control group with the lowest levels after 1 month of intervention (estimated treatment difference −3.4 µg/L [1.4; 5.4] for females; −2.2 µg/L [1.2; 3.3] for males; p < 0.001 each) and was predictive for stronger reduction of body weight and fat mass (p < 0.001 each) over 12 months. Strongest weight loss was observed after 6 months (−5.9 ± 5.1 kg in females of the intervention group vs. −2.9 ± 4.9 kg in the control group (p < 0.0001); −6.8 ± 5.3 kg vs. −4.1 ± 4.4 kg (p = 0.003) in males) and in those participants with combined leptin and insulin decrease. A meal replacement-based lifestyle intervention effectively reduces leptin which is predictive for long-term weight loss.
Besides transcription, RNA decay accounts for a large proportion of regulated gene expression and is paramount for cellular functions. Classical RNA surveillance pathways, like nonsense-mediated decay (NMD), are also implicated in the turnover of non-mutant transcripts. Whereas numerous protein factors have been assigned to distinct RNA decay pathways, the contribution of long non-coding RNAs (lncRNAs) to RNA turnover remains unknown. Here we identify the lncRNA CALA as a potent regulator of RNA turnover in endothelial cells. We demonstrate that CALA forms cytoplasmic ribonucleoprotein complexes with G3BP1 and regulates endothelial cell functions. A detailed characterization of these G3BP1-positive complexes by mass spectrometry identifies UPF1 and numerous other NMD factors having cytoplasmic G3BP1-association that is CALA-dependent. Importantly, CALA silencing impairs degradation of NMD target transcripts, establishing CALA as a non-coding regulator of RNA steady-state levels in the endothelium.
Astrozyten sind außerordentlich vielseitige Zellen, die im Gefüge des zentralen Nervensystems wichtige Rollen in der synaptischen Signalübertragung, im Neurotransmitterstoffwechsel sowie unter anderem in der Regulation der Energiesubstrataufnahme und der lokalen Durchblutung spielen. Die genannten Prozesse weisen eine ausgeprägte Abhängigkeit von lokal verfügbaren Energieäquivalenten auf. Astrozyten können als Reaktion auf verstärkte, umliegende synaptische Aktivität nicht nur anaerobe Stoffwechselwege wie z.B. Glykolyse hochregulieren, sondern weisen auch eine enorme oxidative Verstoffwechselungskapazität auf. Die Mechanismen zur Regulation des oxidativen Stoffwechsels sind z.B. in Hepatozyten oder Muskelzellen genauer beschrieben, wie vieles andere jedoch in Glia untererforscht. Einer der Signalwege zur Aktivierung des obligat aeroben Citratzyklus über das Schlüsselenzym Pyruvatdehydrogenase-Komplex (PDC) führt in Hepatozyten über die Proteinkinase Cδ (PKCδ). Diese bewerkstelligt die Aktivierung der PDC über eine Dephosphorylierung der Pyruvatdehydrogenase (PDH) durch die PDH-Phosphatase 1 (PDP1).
Die selbst in peripheren Astrozytenforsätzen zahlreich vorhandenen Mitochondrien sind bislang in ihrer Funktion und Regulation nur unzureichend untersucht worden. Daher stellte sich die Frage, ob ein solcher PKCδ-vermittelter Signalweg auch in Astrozyten (über die Aktivierung der PDH) zur Aktivierung des mitochondrialen Stoffwechsels führen könnte, insbesondere um auf einen höheren Energiebedarf reagieren zu können.
Nach zahlreichen Versuchen zum Nachweis des Vorliegens der beteiligten Enzyme in der postulierten Signalkaskade wurden primäre Astrozyten mit Glutamat stimuliert, wodurch eine erhöhte synaptische Aktivität in unmittelbarer Umgebung simuliert wurde. Mithilfe von anti-PKCδ-, anti-PDC- und phosphospezifischen anti-phospho-PDC-Antikörpern sowie der Nutzung Objekt-orientierter Bildverarbeitungsmethoden konnte eine Glutamat-induzierte, mitochondriale Translokation der PKCδ mit nachfolgender Dephosphorylierung des PDC (und damit dessen Aktivierung) gezeigt werden. Beide Prozesse, PKCδ-Translokation und PDH-Dephosphorylierung, werden durch den metabotropen Glutamatrezeptor 5 (mGluR5) vermittelt, wie sich durch dessen pharmakologische Inhibition zeigen lässt. Die mitochondriale Translokation der PKCδ und ihre Kinaseaktivität tragen dabei maßgeblich zur Aktivierung des PDC bei, sodass durch das Einwirken von Glutamat eine lokale Modulation des glialen oxidativen Stoffwechsels und der damit verbundenen Produktion von Adenosintriphosphat (ATP) ermöglicht wird. Da Astrozyten (v.a. mit ihren feinen peripheren Fortsätzen) besonders in der Nähe von Synapsen für die Funktionalität von Neuronen essentielle, ATP-verbrauchende Prozesse wie z.B. Glutamattransport aufrechterhalten müssen, ist eine solche Regulation durch Glutamat selbst als Indikator vermehrter synaptischer Aktivität naheliegend. Dabei scheint Glutamat in dualer Funktion sowohl signalgebend die Aktivität des Citratzyklus zu beeinflussen als auch nach Aufnahme in die Zelle selbst als Substrat zum anaplerotischen Einschleusen in den Citratzyklus zur Verfügung zu stehen. Mithilfe von luminometrischen ATP-Assays sowie mGluR- und isoformspezifischen PKC-Inhibitoren konnte gezeigt werden, dass einem durch Glutamat verursachten Abfall der intrazellulären ATP-Spiegel (aufgrund der damit einhergehenden Mehrbelastung der Zelle für Transportprozesse und z.B. Ionenhomöostase) durch die mGluR5/PKCδ-vermittelte Aktivierung der PDH entgegengewirkt werden könnte.
In Zusammenschau könnte der mGluR5/PKCδ/PDH-vermittelte Signalweg gewissermaßen eine Bereitschaft bieten, um für gliale Stoffwechselprozesse die Deckung des Energiebedarfs in Astrozytenfortsätzen bei Aktivität angrenzender Synapsen (im Rahmen des Modells der dreiteiligen Synapse) zu ermöglichen. Eine mögliche pathophysiologische Rolle dieses Mechanismus bei zahlreichen neurodegenerativen Erkrankungen (bei denen häufig ein gestörter Energiestoffwechsel und/oder gliale Dysfunktion vermutet wird) müsste in weiterführenden Studien untersucht werden.
Epilepsie gehört weltweit zu den häufigsten neurologischen Erkrankungen. Sie geht mit wiederholten und in unterschiedlichsten Situationen auftretenden epileptischen Anfällen einher, wodurch Betroffene deutlich in ihrer Lebensqualität eingeschränkt sind. Umfassende Patientenaufklärungen und Patientenschulungen zählen demnach zu den zentralen Bestandteilen der Behandlung. Je besser die Patienten über ihre Erkrankung informiert sind, desto höher ist das Selbstmanagement und die Compliance und demnach auch die Lebensqualität. Vor allem im Hinblick auf den plötzlichen Epilepsietod, englische Abkürzung SUDEP, zählt die Anfallsprävention und die Aufklärung zu den wichtigsten Präventionsstrategien. Das Ziel dieser Studie war es, sowohl den Wissensstand als auch die Wissenslücken der Epilepsiepatienten bezüglich Epilepsie-spezifischer Risikofaktoren sowie Morbidität und Mortalität aufzuzeigen.
Volljährigen Patienten mit diagnostizierter Epilepsie, die sich zwischen Februar 2018 und Mai 2020 an den Epilepsiezentren der Universitätskliniken in Frankfurt am Main, Greifswald und Marburg in Behandlung befanden, wurden eingeladen, an der Studie teilzunehmen. Insgesamt nahmen 238 Patienten (52% Frauen) mit einem mittleren Alter von 39,2 Jahren (Spannweite: 18-77 Jahre) an der Studie teil. Anhand von standardisierten Fragebögen und einem persönlichen Gespräch ermittelten wir die den Patienten bekannten Risiken für und durch Anfälle und erhoben den Wissensstand bezüglich frühzeitiger Mortalität. Hinsichtlich Risiken, die mit Anfällen und einer Epilepsie einhergehen, nannte die Mehrheit der Patienten (51,3%) Autofahren und Verkehrsunfälle. 23,9% der Patienten gaben weitere Gründe für einen frühzeitigen Tod an, wie z.B. Ersticken, Ertrinken oder Atem- und Herzstillstand als Folge eines Anfalls. Zudem wurden Stürze (19,7%) und allgemeine Verletzungen (17,6%) als Gründe für erhöhte Morbidität und Mortalität erwähnt. Ein Großteil der Patienten wusste, dass Alkohol (87,4%) und Schlafentzug (86,6%) Anfälle triggern können. Betrachtet man die Sterblichkeit insgesamt, gaben 52,1% der Patienten an, dass Epilepsiepatienten ein höheres Risiko haben, frühzeitig zu versterben, was hingegen von 46,2% der Patienten verneint wurde. Nur 29,4% der Patienten kannten den Status epilepticus, gerade einmal 27,3% der Patienten hatten bereits von SUDEP gehört. Etwa ein Viertel der Patienten (26,9%) gab an, von ihrem behandelnden Arzt nicht über Risikofaktoren oder Ursachen frühzeitiger Sterblichkeit informiert worden zu sein. Zudem beleuchteten wir mittels Vier-Punkt-Likert-Skala die Themenbereiche, die den Epilepsiepatienten Sorgen bereiten und sie im Alltag einschränken. Die meisten Sorgen machten sich die Patienten über die eingeschränkte Fahrtauglichkeit und Arbeitsfähigkeit, die frühzeitige Sterblichkeit dagegen zählte nicht zu den größten Sorgen.
Insgesamt stellten wir fest, dass die Patienten über ein gutes Allgemeinwissen zu ihrer Erkrankung verfügen. Allerdings konnten wir zeigen, dass Epilepsiepatienten in Bezug auf die frühzeitige Sterblichkeit, SUDEP und den Status epilepticus deutliche Wissenslücken aufweisen. Ein Großteil der Patienten gab hierbei an, von ihrem behandelnden Arzt nicht ausreichend informiert worden zu sein und wünschte sich eine bessere Aufklärung.
Bartonella henselae (B. henselae) ist ein zoonotischer humanpathogener Erreger, der die Katzenkratzkrankheit sowie andere Infektionskrankheiten verursacht. Trotz des globalen Auftretens sind epidemiologische Daten rar oder beruhen auf geringen Fallzahlen. Die aktuelle Forschung zu Bartonellen zielt mehr und mehr auf einen interdisziplinären Ansatz, der als one-health-Konzept zusammengefasst werden kann und als solcher zunehmend Daten zur Seroprävalenz mit hohen Fallzahlen benötigt.
Die Detektion von anti-B. henselae IgG-Antikörpern mittels indirektem Immunfluoreszenztest (IFT) ist die Diagnostik der Wahl für Bartonella-Infektionen. Dabei handelt es sich um ein objektträgerbasiertes Verfahren, wodurch dessen Handhabung ungeeignet für die Verarbeitung im Hochdurchsatz wird.
Im Rahmen der vorliegenden Arbeit wurde der high-throughput IFT (hpIFT) zum Nachweis von anti-B. henselae IgG-Antikörpern im 96-Well-Platten-Format entwickelt, welcher es zum ersten Mal ermöglicht, seroepidemiologische Daten mit hohen Fallzahlen zu generieren.
Für die Antigenpräparation im 96-Well-Platten-Format wurden HeLa 229-Zellen mit B. henselae [Houston-1 RSE 247 (BadA-)-Stamm] infiziert, fixiert, blockiert und permeabilisiert. Humane Seren wurden 1:320 verdünnt. Nach Inkubation der Seren folgte die Zugabe von Sekundärantikörpern. Für die genannten Schritte wurde ein einfacher, ökonomischer Workflow unter Zuhilfenahme des Pipettierroboters VIAOFLO 96/384 von Integra erstellt.
Mit dem entwickelten hpIFT wurden insgesamt 5.215 Proben verarbeitet. Alle Seren wurden bei einem cut-off-Wert von 1:320 bewertet, dabei galten Werte von ≥ 1:320 als positiv. Um die diagnostische Validität zu überprüfen, wurden zunächst 20 bekannte Seren in unterschiedlichen Titerstufen und anschließend 238 zufällig ausgewählte Blutspendeseren mit der kommerziellen objektträger-basierten IFT-Methode von Euroimmun (Produktcode: FK-219b-1010-1G) verglichen. Zur Überprüfung der Intraspezies-Kreuzreaktivität wurden verschiedene B. henselae-Stämme [Houston-1 RSE 247 (BadA-), Marseille (BadA+), Houston-1 MFE341 (BadA+), Marseille (BadA-)] und B. quintana-Stämme [JK31 (Vomp+) und 2D70(Vomp-)] als Antigen mit unterschiedlichen, in der Antikörperkonstellation bekannten Seren untereinander verglichen. Um die Interspezies-Kreuzreaktivitäten abzuklären, wurden Seren mit hohen IgG-Antikörpern gegen andere Pathogene (Anaplasma phagocytophilum, Leptospira spp., Brucella spp., Coxiella burnetii, Rickettsia typhi, Treponema pallidum, Mycoplasma pneumoniae, Epstein-Barr-Virus und B. quintana) überprüft.
Es zeigte sich, (I) dass der modifizierte hpIFT mit beinahe gleichwertiger Sensitivität (86,2 %) und Spezifität (80,6 %) verglichen mit dem kommerziell erhältlichen IFT von Euroimmun bei umfangreichen Probenmengen ein ökonomisches, vergleichsweise einfach zu bedienendes, zuverlässiges und präzises Verfahren zur Detektion von anti-B. henselae IgG-Antikörpern in humanen Seren darstellt und (II), dass die ermittelte anti-B. henselae IgG-Prävalenz von 22,3 % unter 5.215 Blutspenderinnen und Blutspendern verglichen mit weltweit publizierten Studien vergleichbarer Kohorten der letzten Jahre höher ausfällt als angenommen.
Der hier etablierte hpIFT stellt damit ein vielversprechendes Tool für die Hochdurchsatz-Serologie zum Nachweis von anti-B. henselae-Antikörpern in Prävalenzstudien dar. Eine weitere Modifizierung beispielsweise für den veterinären Bereich könnte im Sinne des one-health Ansatzes neue Möglichkeiten für epidemiologische Forschungsaspekte eröffnen.
Hintergrund und Ziel der Studie: Der größte Nachteil von Plastikstents ist ihre im Vergleich mit Metallstents hohe Verschlussrate. Das übliche Wechselintervall für biliäre Plastikstents beträgt drei Monate. Im Universitätsklinikum Frankfurt wurde das Intervall für den elektiven Austausch von Plastikstents auf sechs bis acht Wochen festgelegt.
In dieser Studie sollte untersucht werden, ob sich ein kürzeres Intervall von sechs bis acht Wochen auf die mediane vorzeitige Wechselrate (mPER) auswirkt. Hierfür wurden sowohl benigne als auch maligne Gallengangsstenosen untersucht.
Methoden: An die Anwendung verschiedener Ein- und Ausschlusskriterien auf alle seit 2007 im Universitätsklinikum Frankfurt durchgeführten ERCPs inklusive der Implantation mindestens eines Plastikstents, schloss sich die retrospektive Analyse der geeigneten Fälle und die Berechnung der jeweiligen mPER an. Hierfür wurden sowohl verschiedene Stentparameter wie die verwendete Stentgröße, -länge und -anzahl als auch patientenbezogene Parameter wie die indikationsgebende Diagnose erfasst.
Ergebnisse: Insgesamt wurden 3979 ERCPs analysiert, welche sich auf 1199 Patienten verteilen. Eine gutartige Stenose lag in 2717 (68,3 %) Fällen vor. In 1262 (31,7 %) Fällen wurde die Indikation einer Stentimplantation aufgrund einer bösartigen Stenose gestellt.
Die mediane Stentliegedauer (mSP) betrug 41 Tage (Spanne: 14–120) bei planmäßigen Stentwechseln, während sie bei vorzeitig gewechselten Stents bei 17 Tagen (Spanne: 1–75) lag.
Die mPER war bei malignen (28,1 %, Spanne: 35–50 %) im Vergleich zu benignen Strikturen (15,2 %, Spanne: 10–28 %) signifikant höher (p < 0,0001).
Das mSP war in Fällen von einem singulären Stent (34 Tage, Spanne: 1–87) im Vergleich mit Fällen multipler Stentimplantation (41 Tage, Spanne: 1–120) signifikant kürzer (p < 0,0001). Eine signifikant kürzere mSP wurde außerdem in Fällen von nur einem 7 Fr Stent (28 Tage, Spanne: 2–79) im Vergleich zu größeren Stents (34 Tage, Spanne: 1–87, p = 0,001) festgestellt.
Entsprechend war die mPER in Fällen mit einem singulären Stent (23 %) verglichen mit Fällen multiplen Stentings (16,2 %) signifikant höher (p < 0,0001). Beim Vergleich der Implantationen eines einzelnen 7-Fr-Stents (31,3 %) mit jenen größerer Stents (22,4 %) wurde ebenfalls eine signifikant erhöhte mPER der 7-Fr-Stents festgestellt (p = 0,007).
Konklusion: Diese Studie liefert erstmals umfassende Daten von Patienten, welche mit Plastikstents und kürzeren Wechselintervallen behandelt wurden.
Im direkten Vergleich der Stentparameter war bei der Implantation größerer oder multipler Stents die Einliegedauer verlängert. Dies lässt auf ein geringeres Komplikationsrisiko bei der Verwendung multipler oder größerer Stents schließen, welches am ehesten auf die dadurch optimierte Überbrückung der Stenosen und daraus resultierende Drainage zurückzuführen ist.
Weiter lassen die Ergebnisse vermuten, dass ein verkürztes Wechselintervall unter drei Monaten bei benignen biliären Stenosen nicht zu einer klinisch relevanten Reduktion der Komplikationsrate führt.
Zudem war die bei malignen Stenosen beobachtete Rate vorzeitiger Stentwechsel deutlich größer als die der benignen Stenosen. Sollte in diesem Setting ein Wechsel von einem Plastikstent auf einen SEMS geplant sein, ist ein verkürztes Wechselintervall von drei bis vier Wochen in Erwägung zu ziehen. Bei fehlender Möglichkeit der Implantation eines SEMS, ist die Verwendung multipler und größerer Stents das Mittel der Wahl.
Abschließend lässt sich Folgendes festhalten. Ein kürzeres Austauschintervall scheint bei benignen und malignen Stenosen nicht zu einer klinisch bedeutsamen Verringerung der mPER zu führen. Sofern es die Untersuchungsbedingungen zulassen, sollten große und multiple Stents für die Behandlung von Gallengangsstenosen bevorzugt werden.
Der Nutzen von DJ-Kathetern ist unverzichtbar für die moderne Urologie. Sie sind essenziell, um bei einer Vielzahl von Erkrankungen einen adäquaten Harnabfluss und dementsprechend eine gute Nierenfunktion gewährleisten zu können. Dabei können sie für eine temporäre oder dauerhafte Schienung genutzt werden. Als einliegende Fremdkörper können sie, insbesondere bei langfristiger Nutzung, leicht durch Bakterien und Pilze kolonisiert werden und beherbergen somit ein erhöhtes Risiko für Harnwegsinfekte.
Das Hauptziel dieser Arbeit ist folglich, durch das Auffinden von protektiven oder prädisponierenden Faktoren für die Entwicklung von fieberhaften Harnwegsinfekten in der Zukunft Mortalität, Resistenzbildung und Kosten sowohl bei DJ-Katheter-Dauerversorgung als auch generell senken zu können. Zusätzlich soll untersucht werden, ob verschiedene Antibiotika-Regime einen Einfluss auf die Entwicklung postoperativer, fieberhafter Harnwegsinfekte haben.
Um dies feststellen zu können, wurden in dieser Studie 100 Patientinnen und Patienten eingeschlossen, die von 2013 bis 2018 in der Klinik für Urologie des Universitätsklinikums Frankfurt zum DJ-Wechsel bei DJ-Katheter-Dauerversorgung vorstellig wurden. Anschließend wurden verschiedene Faktoren untersucht, die das Risiko für das Auftreten von fieberhaften Harnwegsinfekten oder die Entwicklung von Resistenzen in Krankheitserregern erhöhen oder reduzieren könnten.
Hierzu wurden insgesamt 950 DJ-Katheter-assoziierte Eingriffe (Einlage, Wechsel, Entfernung) analysiert. Der individuelle Beobachtungszeitraum betrug durchschnittlich 2,9 Jahre mit durchschnittlich 7 DJ-Katheter-Wechsel.
Gegenüber der Normalbevölkerung wies die Studienpopulation, am ehesten durch die einliegenden DJ-Katheter, eine deutlich erhöhte Prävalenz von Harnwegsinfekten auf (18,53% vs. 2,5%). Darüber hinaus konnte gezeigt werden, dass ein höheres Patientenalter mit einem gering erhöhten Risiko für die Ausbildung von resistenten Keimen im Urin korreliert (p=0,0121; OR 1,0395; KI 1,0096-1,0731 (univariate Analyse), p=0,0030; OR 1,0618 KI 1,0226-1,1077 (multivariate Analyse)). Dies korrelierte jedoch nicht mit einem erhöhten Risiko für fieberhafte Harnwegsinfekte. Darüber hinaus konnte festgestellt werden, dass die operative Manipulation unter empirischer oder testgerechter, antibiotischer Therapie nicht mit einem hohen Risiko für die Entstehung fieberhafter Harnwegsinfekte einhergeht. In den analysierten Daten fanden sich insgesamt zu wenige fieberhafte Infekte (n=72; 7,58%), um das Ziel begünstigender oder protektiver Faktoren für die Entstehung von fieberhaften Harnwegsinfekten adäquat zu untersuchen. Aus dem gleichen Grund konnten aus den Daten auch keine Hinweise für das optimale Antibiotika-Regime abgeleitet werden. In fast 70% der untersuchten Fälle wurde eine periinterventionelle single-shot Antibiose angewendet, weswegen diese als Infektionsprophylaxe einen adäquaten Schutz darzustellen scheint.
Generell kann davon ausgegangen werden, dass die DJ-Katheter-Dauerversorgung, trotz der möglichen mikrobiellen Besiedlung mit eventuellen Komplikationen wie Bakteriurie oder systemischen Infektionen, ein sicheres Verfahren ist. Manipulationen oder operativer Wechsel der DJ-Katheter stellen, trotz Präsenz der Keime, keine riskanten Manöver dar, sofern diese adäquat und unter entsprechender, antibiotischer Therapie durchgeführt werden.
Weitere, noch größere und insbesondere prospektive, randomisierte Studien sind zu empfehlen und könnten die Ergebnisse weiter bestätigen und erweitern, vor allem in Bezug auf die Überlegenheit verschiedener antibiotischer Regime in der Vermeidung einer Resistenzbildung.
Background: With the rise of single-cell RNA sequencing new bioinformatic tools have been developed to handle specific demands, such as quantifying unique molecular identifiers and correcting cell barcodes. Here, we benchmarked several datasets with the most common alignment tools for single-cell RNA sequencing data. We evaluated differences in the whitelisting, gene quantification, overall performance, and potential variations in clustering or detection of differentially expressed genes. We compared the tools Cell Ranger version 6, STARsolo, Kallisto, Alevin, and Alevin-fry on 3 published datasets for human and mouse, sequenced with different versions of the 10X sequencing protocol.
Results: Striking differences were observed in the overall runtime of the mappers. Besides that, Kallisto and Alevin showed variances in the number of valid cells and detected genes per cell. Kallisto reported the highest number of cells; however, we observed an overrepresentation of cells with low gene content and unknown cell type. Conversely, Alevin rarely reported such low-content cells. Further variations were detected in the set of expressed genes. While STARsolo, Cell Ranger 6, Alevin-fry, and Alevin produced similar gene sets, Kallisto detected additional genes from the Vmn and Olfr gene family, which are likely mapping artefacts. We also observed differences in the mitochondrial content of the resulting cells when comparing a prefiltered annotation set to the full annotation set that includes pseudogenes and other biotypes.
Conclusion: Overall, this study provides a detailed comparison of common single-cell RNA sequencing mappers and shows their specific properties on 10X Genomics data.
WIPI2 is a member of the human WIPI protein family (seven-bladed b-propeller proteins binding phosphatidylinositols, PROPPINs), which play a pivotal role in autophagy and has been implicated in the pathogenesis of several neurological conditions. The homozygous WIPI2 variant c.745G>A; p.(Val249Met) (NM_015610.4) has recently been associated with a neurodevelopmental disorder in a single family. Using exome sequencing and Sanger segregation analysis, here, two novel homozygous WIPI2 variants [c.551T>G; p.(Val184Gly) and c.724C>T; p.(Arg242Trp) (NM_015610.4)] were identified in four individuals of two consanguineous families. Additionally, follow-up clinical data were sought from the previously reported family. Three non-ambulant affected siblings of the first family harbouring the p.(Val184Gly) missense variant presented with microcephaly, profound global developmental delay/intellectual disability, refractory infantile/childhood-onset epilepsy, progressive tetraplegia with joint contractures and dyskinesia. In contrast, the proband of the second family carrying the p.(Arg242Trp) missense variant, similar to the initially reported WIPI2 cases, presented with a milder phenotype, encompassing moderate intellectual disability, speech and visual impairment, autistic features, and an ataxic gait. Brain MR imaging in five patients showed prominent white matter involvement with a global reduction in volume, posterior corpus callosum hypoplasia, abnormal dentate nuclei and hypoplasia of the inferior cerebellar vermis. To investigate the functional impact of these novel WIPI2 variants, we overexpressed both in WIPI2-knockout HEK293A cells. In comparison to wildtype, expression of the Val166Gly WIPI2b mutant resulted in a deficient rescue of LC3 lipidation whereas Arg224Trp mutant increased LC3 lipidation, in line with the previously reported Val231Met variant. These findings support a dysregulation of the early steps of the autophagy pathway. Collectively, our findings provide evidence that biallelic WIPI2 variants cause a neurodevelopmental disorder of variable severity and disease course. Our report expands the clinical spectrum and establishes WIPI2-related disorder as a congenital disorders of autophagy.
Background: Bacterial burden as well as duration of bacteremia influence the outcome of patients with bloodstream infections. Promptly decreasing bacterial load in the blood by using extracorporeal devices in addition to anti-infective therapy has recently been explored. Preclinical studies with the Seraph® 100 Microbind® Affinity Blood Filter (Seraph® 100), which consists of heparin that is covalently bound to polymer beads, have demonstrated an effective binding of bacteria and viruses. Pathogens adhere to the heparin coated polymer beads in the adsorber as they would normally do to heparan sulfate on cell surfaces. Using this biomimetic principle, the Seraph® 100 could help to decrease bacterial burden in vivo.
Methods: This first in human, prospective, multicenter, non-randomized interventional study included patients with blood culture positive bloodstream infection and the need for kidney replacement therapy as an adjunctive treatment for bloodstream infections. We performed a single four-hour hemoperfusion treatment with the Seraph® 100 in conjunction with a dialysis procedure. Post procedure follow up was 14 days.
Results: Fifteen hemodialysis patients (3F/12 M, age 74.0 [68.0–78.5] years, dialysis vintage 28.0 [11.0–45.0] months) were enrolled. Seraph® 100 treatment started 66.4 [45.7–80.6] hours after the initial positive blood culture was drawn. During the treatment with the Seraph® 100 with a median blood flow of 285 [225–300] ml/min no device or treatment related adverse events were reported. Blood pressure and heart rate remained stable while peripheral oxygen saturation improved during the treatment from 98.0 [92.5–98.0] to 99.0 [98.0–99.5] %; p = 0.0184. Four patients still had positive blood culture at the start of Seraph® 100 treatment. In one patient blood cultures turned negative during treatment. The time to positivity (TTP) was increased between inflow and outflow blood cultures by 36 [− 7.2 to 96.3] minutes. However, overall TTP increase was not statistical significant.
Conclusions: Seraph® 100 treatment was well tolerated. Adding Seraph® 100 to antibiotics early in the course of bacteremia might result in a faster resolution of bloodstream infections, which has to be evaluated in further studies.
Purpose: The role of obesity in glioblastoma remains unclear, as previous analyses have reported contradicting results. Here, we evaluate the prognostic impact of obesity in two trial populations; CeTeG/NOA-09 (n = 129) for MGMT methylated glioblastoma patients comparing temozolomide (TMZ) to lomustine/TMZ, and GLARIUS (n = 170) for MGMT unmethylated glioblastoma patients comparing TMZ to bevacizumab/irinotecan, both in addition to surgery and radiotherapy.
Methods: The impact of obesity (BMI ≥ 30 kg/m2) on overall survival (OS) and progression-free survival (PFS) was investigated with Kaplan–Meier analysis and log-rank tests. A multivariable Cox regression analysis was performed including known prognostic factors as covariables.
Results: Overall, 22.6% of patients (67 of 297) were obese. Obesity was associated with shorter survival in patients with MGMT methylated glioblastoma (median OS 22.9 (95% CI 17.7–30.8) vs. 43.2 (32.5–54.4) months for obese and non-obese patients respectively, p = 0.001), but not in MGMT unmethylated glioblastoma (median OS 17.1 (15.8–18.9) vs 17.6 (14.7–20.8) months, p = 0.26). The prognostic impact of obesity in MGMT methylated glioblastoma was confirmed in a multivariable Cox regression (adjusted odds ratio: 2.57 (95% CI 1.53–4.31), p < 0.001) adjusted for age, sex, extent of resection, baseline steroids, Karnofsky performance score, and treatment arm.
Conclusion: Obesity was associated with shorter survival in MGMT methylated, but not in MGMT unmethylated glioblastoma patients.
Hintergrund: Depressionen sind häufige, schwere und oft lebensbedrohliche Erkrankungen, bei denen es – trotz sehr guter Behandlungsmethoden – Versorgungslücken gibt. Hierzu tragen Vorbehalte gegen eine leitlinienkonforme pharmako- und/oder psychotherapeutische Behandlung bei. Ziel der Arbeit ist es zu ermitteln, in welchen soziodemographischen Bevölkerungssegmenten diese Vorbehalte besonders ausgeprägt sind.
Methodik: Die Untersuchung basiert auf Online-Befragungen der deutschen Bevölkerung aus den Jahren 2021, 2020 und 2019, darunter 1656 Personen (2021), 1775 Personen (2020) und 1729 Personen (2019) ohne Depressionserfahrungen. Mit einer CHAID-Analyse wurde geprüft, in welchen Bevölkerungssegmenten die Vorbehalte gegen eine leitliniengerechte Behandlung besonders groß sind.
Ergebnisse: Vorbehalte gegen Pharmakotherapie hatten insgesamt 69,8 % der Befragten ohne Depressionserfahrungen. Am größten waren die Vorbehalte unter jüngeren Personen (< 40 Jahre); hier lag der Anteil bei 74,2 %. Vorbehalte gegen Psychotherapie äußerten 31,4 % ohne Depressionserfahrungen; unter Frauen mit geringerer Schulbildung hatten 40,5 % Vorbehalte gegen eine Psychotherapie; unter Männern mit geringerer Schulbildung waren es 39,1 %. Vorbehalte gegen beide Behandlungsformen zeigten 27,7 %. Am größten waren die Vorbehalte unter Männern mit Schulbildung unterhalb der Hochschulreife (34,1 %). Die Ergebnisse sind signifikant (χ2-Test, p < 0,05).
Diskussion: Eine allgemeine Informationsstrategie wäre geeignet, Vorbehalten gegen Pharmakotherapie und Psychotherapie gleichermaßen zu verringern. Für eine spezifische Informationsstrategie müssen die Botschaften hinsichtlich Inhalt und Kommunikationskanälen so gestaltet werden, dass die jüngere Zielgruppe zuverlässig erreicht wird.
Although vaccination campaigns are currently being rolled out to prevent coronavirus disease (COVID-19), antivirals will remain an important adjunct to vaccination. Antivirals against coronaviruses do not exist, hence global drug repurposing efforts have been carried out to identify agents that may provide clinical benefit to patients with COVID-19. Itraconazole, an antifungal agent, has been reported to have activity against animal coronaviruses. Using cell-based phenotypic assays, the in vitro antiviral activity of itraconazole and 17-OH itraconazole was assessed against clinical isolates from a German and Belgian patient infected with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Itraconazole demonstrated antiviral activity in human Caco-2 cells (EC50 = 2.3 µM; 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay). Similarly, its primary metabolite, 17-OH itraconazole, showed inhibition of SARS-CoV-2 activity (EC50 = 3.6 µM). Remdesivir inhibited viral replication with an EC50 = 0.4 µM. Itraconazole and 17-OH itraconazole resulted in a viral yield reduction in vitro of approximately 2-log10 and approximately 1-log10, as measured in both Caco-2 cells and VeroE6-eGFP cells, respectively. The viral yield reduction brought about by remdesivir or GS-441524 (parent nucleoside of the antiviral prodrug remdesivir; positive control) was more pronounced, with an approximately 3-log10 drop and >4-log10 drop in Caco-2 cells and VeroE6-eGFP cells, respectively. Itraconazole and 17-OH itraconazole exert in vitro low micromolar activity against SARS-CoV-2. Despite the in vitro antiviral activity, itraconazole did not result in a beneficial effect in hospitalized COVID-19 patients in a clinical study (EudraCT Number: 2020-001243-15).
Complexome profiling is an emerging ‘omics’ approach that systematically interrogates the composition of protein complexes (the complexome) of a sample, by combining biochemical separation of native protein complexes with mass-spectrometry based quantitation proteomics. The resulting fractionation profiles hold comprehensive information on the abundance and composition of the complexome, and have a high potential for reuse by experimental and computational researchers. However, the lack of a central resource that provides access to these data, reported with adequate descriptions and an analysis tool, has limited their reuse. Therefore, we established the ComplexomE profiling DAta Resource (CEDAR, www3.cmbi.umcn.nl/cedar/), an openly accessible database for depositing and exploring mass spectrometry data from complexome profiling studies. Compatibility and reusability of the data is ensured by a standardized data and reporting format containing the “minimum information required for a complexome profiling experiment” (MIACE). The data can be accessed through a user-friendly web interface, as well as programmatically using the REST API portal. Additionally, all complexome profiles available on CEDAR can be inspected directly on the website with the profile viewer tool that allows the detection of correlated profiles and inference of potential complexes. In conclusion, CEDAR is a unique, growing and invaluable resource for the study of protein complex composition and dynamics across biological systems.
Ein persistierender Ductus arteriosus (PDA) stellt bei Frühgeborenen mit <1500g Geburtsgewicht einen relevanten Faktor für eine Erhöhung der Morbidität und Mortalität dar. Aufgrund fehlender Evidenz konnte bisher keine allgemeine Therapieempfehlung nach frustranem medikamentösem Verschlussversuch erarbeitet werden, sowohl eine Ligatur im Rahmen eines operativen Eingriffs, als auch eine Katheterintervention sind möglich. In der vorliegenden Arbeit wurde retrospektiv die Auswirkung dieser verschiedenen Therapiemethoden auf die Morbidität und Mortalität bei Frühgeborenen <1500g Geburtsgewicht mit hämodynamisch signifikantem persistierendem Ductus arteriosus untersucht. Insbesondere wurde geprüft, ob eine Katheterintervention das Risiko einer Bronchopulmonalen Dysplasie (BPD) oder des Versterbens der Kinder, sowie andere häufige Folgeerkrankungen des Frühgeborenen erhöht. Analysiert wurden dafür alle 469 stationär behandelten Kinder des Schwerpunktes Neonatologie, die zwischen dem 01.01.2013 und dem 31.12.2019 in der Uniklinik Frankfurt am Main mit einem Geburtsgewicht von <1500g geboren oder innerhalb der ersten 13 Lebenstage aus einem anderen Krankenhaus übernommen wurden. Diese wurden in 4 Gruppen eingeteilt: Kinder ohne hämodynamisch signifikanten PDA (n=341), Kinder die erfolgreich medikamentös therapiert wurden (n=92), solche, die eine Ligatur erhielten (n=16) und zuletzt solche, die interventionell behandelt wurden (n=12). Die Daten der medikamentös behandelten Kinder wurden in dieser Untersuchung nicht berücksichtigt. Es wurden sowohl verschiedene Geburtsparameter um eine Vergleichbarkeit der Gruppen zu untersuchen, als auch relevante Parameter des Outcomes ausgewertet. Es zeigte sich, dass Kinder ohne PDA zum Zeitpunkt der Geburt ein höheres Gestationsalter hatten als solche mit PDA und daraus resultierend eine geringere Intensivmedizinische Betreuung und geringere Morbidität aufwiesen. Im Vergleich der Interventionskinder mit einer Matched-pair Gruppe aus 24 Kindern der Kontrollgruppe ohne PDA (± 8 Schwangerschaftstage bei Geburt und ± 410 g Geburtsgewicht) konnten keine Unterschiede in Mortalität oder Morbidität festgestellt werden. Kinder nach Intervention zeigten kein signifikant schlechteres Outcome (Bronchopulmonale Dysplasie, Intraventrikuläre Hämorrhagie, operativ versorgte nekrotisierende Enterokolitis, therapiebedürftige Retinopathia praematurorum, Periventrikuläre Leukomalazie) im Vergleich zu Kindern nach Ligatur. Eine schwere Bronchopulmonale Dysplasie trat mit einem Relativen Risiko von 2,25 seltener auf. Diese Ergebnisse lassen den Schluss zu, dass die Interventionelle Therapie des hsPDA ein ähnlich gutes Outcome im Vergleich zur Ligatur aufweist und sogar einen Vorteil für die Kinder bringt. Aufgrund der geringen Fallzahlen muss die Relevanz diese Ergebnisse kritisch hinterfragt werden. Eine mögliche randomisierte kontrollierte Studie könnte diese bestätigen.
Die Heparininduzierte Thrombozytopenie (HIT) ist eine lebensbedrohliche Komplikation der Heparintherapie. Diese wird verursacht durch die Bildung von Antikörpern gegen einen Komplex aus Plättchenfaktor 4 (PF4) und Heparin. Orthopädische und herzchirurgische Patienten weisen deutlich höhere Serokonversionsraten auf (15-75 %) als andere Patientenkohorten, z.B. internistische Patienten (<1 %), ohne dabei in allen Fällen mit den klinischen Zeichen einer HIT, d.h. Thrombozytopenie sowie arteriellen und venösen thromboembolischen Komplikationen, einherzugehen. Der Grund für diese Beobachtung ist unklar. Es ist bekannt, dass Glykosaminoglykane und sulfatierte Polysaccaride ein Epitop auf dem Plättchenfaktor 4 induzieren können, gegen das eine Sensibilisierung erfolgen kann. Es wird vermutet, dass Glykosaminoglykane, d.h. Chondroitinsulfate, die bei großen orthopädischen Eingriffen freigesetzt werden, die hohen Serokonversionsraten bei orthopädischen Patienten erklären könnten.
Das Ziel der vorliegenden Studie war es, die Inzidenz von HIT-Antikörpern bei Patienten mit orthopädischen Eingriffen zu untersuchen, bei denen perioperativ keine Heparinexposition erfolgte. Die Antikoagulation zur Thromboseprophylaxe im untersuchten Patientenkollektiv wurde postoperativ mit dem zur orthopädischen Thromboseprophylaxe zugelassenen, direkten oralen Faktor Xa Inhibitor Rivaroxaban und nicht mit niedermolekularem Heparin durchgeführt, welches eine HIT-Antikörperbildung hätte induzieren können.
Material und Methoden: Zur HIT Diagnostik wurde jeweils präoperativ und mindestens 5 Tage postoperativ ein immunologischer Chemilumineszenztest (HemosIL® AcuStar HIT-IgG(PF4-H)) und ein funktioneller Heparin-induzierter Plättchenaktivierungstest (HIPA) durchgeführt, um eine Serokonversion zu detektieren. Ausschlusskriterium war eine Heparinexposition innerhalb der letzten 3 Monate sowie während und nach der Operation.
Ergebnisse: 107 konsekutive Patienten, 65 (60,7 %) weibliche und 42 (39,3 %) männliche Patienten, bei denen eine operative totale Hüftgelenk- (n= 66; 61,6 %) oder Kniegelenkendoprothese (n= 41; 38,3 %) durchgeführt wurde, wurden in die Studie eingeschlossen. Vor und auch nach der Operation konnte weder im durchgeführten Immunoassay noch im funktionellen HIPA eine Serokonversion mit der Bildung von anti-PF-4/Heparin Antikörpern festgestellt werden.
Diskussion: Große orthopädische Operationen allein nicht auszureichen, um die hohen HIT-Antikörper Serokonversionsraten in Abwesenheit von Heparin bei diesen Patienten zu erklären. Heparin scheint für eine Sensibilisierung unerlässlich zu sein. Aber auch andere Risikofaktoren für die Bildung von HIT-Antikörpern gegen den Komplex aus PF4 und Heparin bzw. Polyanionen, wie z.B. bakterielle Infektionen, Ausmaß des Gewebetraumas, Freisetzung von Nukleinsäuren, könnten eine Rolle spielen. Da das antigene Epitop nur bei optimalen stöchiometrischen Ratios von PF4 und Heparin/Glykosaminoglykanen induziert wird, ist es vorstellbar, dass einige der genannten Faktoren zusammenwirken könnten, um die Immunreaktion zu ermöglichen. Auf der Basis unserer Studie kann zwar das Konzept der ‘‘spontanen’’ HIT nicht erklärt werden. Auf Basis der bestehenden Daten kann es aber auch nicht verworfen werden.
Erythropoietin (Epo) is a crucial hormone regulating red blood cell number and consequently the hematocrit. Epo is mainly produced in the kidney by interstitial fibroblast-like cells. Previously, we have shown that in cultures of the immortalized mouse renal fibroblast-like cell line FAIK F3-5, sphingosine 1-phosphate (S1P), by activating S1P1 and S1P3 receptors, can stabilize hypoxia-inducible factor (HIF)-2α and upregulate Epo mRNA and protein synthesis. In this study, we have addressed the role of intracellular iS1P derived from sphingosine kinases (Sphk) 1 and 2 on Epo synthesis in F3-5 cells and in mouse primary cultures of renal fibroblasts. We show that stable knockdown of Sphk2 in F3-5 cells increases HIF-2α protein and Epo mRNA and protein levels, while Sphk1 knockdown leads to a reduction of hypoxia-stimulated HIF-2α and Epo protein. A similar effect was obtained using primary cultures of renal fibroblasts isolated from wildtype mice, Sphk1−/−, or Sphk2−/− mice. Furthermore, selective Sphk2 inhibitors mimicked the effect of genetic Sphk2 depletion and also upregulated HIF-2α and Epo protein levels. The combined blockade of Sphk1 and Sphk2, using Sphk2−/− renal fibroblasts treated with the Sphk1 inhibitor PF543, resulted in reduced HIF-2α and Epo compared to the untreated Sphk2−/− cells. Exogenous sphingosine (Sph) enhanced HIF-2α and Epo, and this was abolished by the combined treatment with the selective S1P1 and S1P3 antagonists NIBR-0213 and TY52156, suggesting that Sph was taken up by cells and converted to iS1P and exported to then act in an autocrine manner through S1P1 and S1P3. The upregulation of HIF-2α and Epo synthesis by Sphk2 knockdown was confirmed in the human hepatoma cell line Hep3B, which is well-established to upregulate Epo production under hypoxia. In summary, these data show that sphingolipids have diverse effects on Epo synthesis. While accumulation of intracellular Sph reduces Epo synthesis, iS1P will be exported to act through S1P1+3 to enhance Epo synthesis. Furthermore, these data suggest that selective inhibition of Sphk2 is an attractive new option to enhance Epo synthesis and thereby to reduce anemia development in chronic kidney disease.
Background: Atypical EGFR mutations occur in 10%-30% of non-small-cell lung cancer (NSCLC) patients with EGFR mutations and their sensitivity to classical epidermal growth factor receptor (EGFR)-tyrosine kinase inhibitors (TKI) is highly heterogeneous. Patients harboring one group of uncommon, recurrent EGFR mutations (G719X, S768I, L861Q) respond to EGFR-TKI. Exon 20 insertions are mostly insensitive to EGFR-TKI but display sensitivity to exon 20 inhibitors. Clinical outcome data of patients with very rare point and compound mutations upon systemic treatments are still sparse to date.
Patients and methods: In this retrospective, multicenter study of the national Network Genomic Medicine (nNGM) in Germany, 856 NSCLC cases with atypical EGFR mutations including co-occurring mutations were reported from 12 centers. Clinical follow-up data after treatment with different EGFR-TKIs, chemotherapy and immune checkpoint inhibitors were available from 260 patients. Response to treatment was analyzed in three major groups: (i) uncommon mutations (G719X, S7681, L861Q and combinations), (ii) exon 20 insertions and (iii) very rare EGFR mutations (very rare single point mutations, compound mutations, exon 18 deletions, exon 19 insertions).
Results: Our study comprises the largest thus far reported real-world cohort of very rare EGFR single point and compound mutations treated with different systemic treatments. We validated higher efficacy of EGFR-TKI in comparison to chemotherapy in group 1 (uncommon), while most exon 20 insertions (group 2) were not EGFR-TKI responsive. In addition, we found TKI sensitivity of very rare point mutations (group 3) and of complex EGFR mutations containing exon 19 deletions or L858R mutations independent of the combination partner. Notably, treatment responses in group 3 (very rare) were highly heterogeneous. Co-occurring TP53 mutations exerted a non-significant trend for a detrimental effect on outcome in EGFR-TKI-treated patients in groups 2 and 3 but not in group 1.
Conclusions: Based on our findings, we propose a novel nNGM classification of atypical EGFR mutations.
Nowadays, several options are available to treat patients with conductive or mixed hearing loss. Whenever surgical intervention is not possible or contra-indicated, and amplification by a conventional hearing device (e.g., behind-the-ear device) is not feasible, then implantable hearing devices are an indispensable next option. Implantable bone-conduction devices and middle-ear implants have advantages but also limitations concerning complexity/invasiveness of the surgery, medical complications, and effectiveness. To counsel the patient, the clinician should have a good overview of the options with regard to safety and reliability as well as unequivocal technical performance data. The present consensus document is the outcome of an extensive iterative process including ENT specialists, audiologists, health-policy scientists, and representatives/technicians of the main companies in this field. This document should provide a first framework for procedures and technical characterization to enhance effective communication between these stakeholders, improving health care.
Immune-mediated inflammatory diseases (IMIDs), such as rheumatoid arthritis (RA), psoriatic arthritis (PsA), and psoriasis (Ps), represent autoinflammatory and autoimmune disorders, as well as conditions that have an overlap of both categories. Understanding the underlying pathogeneses, making diagnoses, and choosing individualized treatments remain challenging due to heterogeneous disease phenotypes and the lack of reliable biomarkers that drive the treatment choice. In this review, we provide an overview of the low-molecular-weight metabolites that might be employed as biomarkers for various applications, e.g., early diagnosis, disease activity monitoring, and treatment-response prediction, in RA, PsA, and Ps. The literature was evaluated, and putative biomarkers in different matrices were identified, categorized, and summarized. While some of these candidate biomarkers appeared to be disease-specific, others were shared across multiple IMIDs, indicating common underlying disease mechanisms. However, there is still a long way to go for their application in a routine clinical setting. We propose that studies integrating omics analyses of large patient cohorts from different IMIDs should be performed to further elucidate their pathomechanisms and treatment options. This could lead to the identification and validation of biomarkers that might be applied in the context of precision medicine to improve the clinical outcomes of these IMID patients.
(1) Intravenous thrombolysis with recombinant tissue plasminogen activator (rt-PA) in patients with acute ischemic stroke is limited because of several contraindications. In routine clinical practice, patients with a recent stroke are typically not treated with rt-PA in case of a recurrent ischemic event. The same applies to its use in the context of pulmonary artery embolism and myocardial infarction with a recent stroke. In this translational study, we evaluated whether rt-PA treatment after experimental ischemic stroke with or without additional hyperglycemia increases the risk for hemorrhagic transformation (HT) and worsens functional outcome regarding the old infarct area. (2) In total, 72 male C57BL/6N mice were used. Ischemic stroke (index stroke) was induced by transient middle cerebral artery occlusion (tMCAO). Mice received either rt-PA or saline 24 h or 14 days after index stroke to determine whether a recent ischemic stroke predisposes to HT. In addition to otherwise healthy mice, hyperglycemic mice were analyzed to evaluate diabetes as a second risk factor for HT. Mice designated to develop hyperglycemia were pre-treated with streptozotocin. (3) The neurological outcome in rt-PA and saline-treated normoglycemic mice did not differ significantly, either at 24 h or at 14 days. In contrast, hyperglycemic mice treated with rt-PA had a significantly worse neurological outcome (at 24 h, p = 0.02; at 14 days, p = 0.03). At 24 h after rt-PA or saline treatment, HT scores differed significantly (p = 0.02) with the highest scores within hyperglycemic mice treated with rt-PA, where notably only small petechial hemorrhages could be detected. (4) Thrombolysis after recent ischemic stroke does not increase the risk for HT or worsen the functional outcome in otherwise healthy mice. However, hyperglycemia as a second risk factor leads to neurological deterioration after rt-PA treatment, which cannot be explained by an increase of HT alone. Direct neurotoxic effects of rt-PA may play a role.
Background: Assessment of the effect of subgingival instrumentation (SI) on systemic inflammation in periodontitis grades B (BP) and C (CP). Methods: In this prospective cohort study, eight BP and 46 CP patients received SI. Data were collected prior to and 12 weeks after SI. Blood was sampled prior to, one day, 6, and 12 weeks after SI. Neutrophil elastase (NE), C-reactive protein (CRP), leukocyte count, lipopolysaccharide binding protein, interleukin 6 (IL-6) and IL-8 were assessed. Results: Both groups showed significant clinical improvement. NE was lower in BP than CP at baseline and 1 day after SI, while CRP was lower in BP than CP at baseline (p < 0.05). NE and CRP had a peak 1 day after SI (p < 0.05). Between-subjects effects due to CP (p = 0.042) and PISA (p = 0.005) occurred. Within-subjects NE change was confirmed and modulated by grade (p = 0.017), smoking (p = 0.029), number of teeth (p = 0.033), and PISA (p = 0.002). For CRP between-subjects effects due to BMI (p = 0.008) were seen. Within-subjects PISA modulated the change of CRP over time (p = 0.017). Conclusions: In untreated CP, NE and CRP were higher than in BP. SI results in better PPD and PISA reduction in BP than CP. Hintergrund: Ein adäquates Gerinnungsmanagement ist für antikoagulierte Patient*innen von zentraler Bedeutung. Gleichzeitig stellt dies sowohl für die Hausarztpraxis als auch für die Patient*innen eine große Herausforderung dar. Orale Antikoagulantien (OAK) werden zur Primär- und Sekundärprophylaxe von thromboembolischen Erkrankungen eingesetzt. Häufigste Nebenwirkungen sind Blutungskomplikationen, von einer harmlosen Bagatellblutung bis hin zur gefürchteten vital bedrohlichen intrazerebralen Blutung reichend. Zur Optimierung des Gerinnungsmanagements in der hausärztlichen Versorgung und Reduktion gerinnungsassoziierter Komplikationen bei Betroffenen wurde im Rahmen der PICANT-Studie (Primary Care Management for Optimized Antithrombotic Treatment) ein hausarztpraxisbasiertes Case Management mit einem engmaschigen Monitoring von Patient*innen durch das Praxisteam – bestehend aus Medizinischer Fachangestellten (MFA) und Hausärzt*innen – untersucht. Die stärke Einbindung der MFA in die Betreuung von Patient*innen mit einer Langzeitindikation für eine gerinnungshemmende Medikation stellt eine Weiterentwicklung des Gerinnungsmanagements in der hausärztlichen Versorgung dar. Diese Arbeit beschreibt die Sichtweise und Erfahrungen von MFA in ihrer neuen Rolle als Case Manager*innen (CM) für ein hausärztliches Gerinnungsmanagement. Zudem soll durch die zusätzliche Befragung beteiligter Hausärzt*innen und Patient*innen die Rolle der MFA aus den drei unterschiedlichen Perspektiven beleuchtet und miteinander verglichen werden. Durch die Erfahrungsberichte von MFA, Hausärzt*innen und Patient*innen zum hausarztpraxisbasiertem Case Management soll diese Arbeit einen wertvollen Beitrag zu den Diskussionen um neue Versorgungs- und Delegationskonzepte im Gerinnungsmanagement leisten.
Methoden: Im Anschluss an die cluster-randomisierte PICANT-Studie wurden leitfadengestützte Telefoninterviews mit 15 MFA, 15 Hausärzt*innen und 25 Patient*innen der Interventionsgruppe geführt. Nach Datenerhebung und wörtlicher Transkription erfolgte die Auswertung nach der Methode der inhaltlich strukturierenden Inhaltsanalyse nach Kuckartz mithilfe der Auswertungssoftware MAXQDA 12.
Ergebnisse: Die Mehrheit der MFA empfand die Erweiterung ihres Verantwortungsbereiches und die damit einhergehende stärkere Einbindung in die Patientenbetreuung als persönliche Bereicherung. Sie gaben an, dass sie neben vertieften Kenntnissen zum Gerinnungsmanagement v.a. auch an Selbstsicherheit im Umgang mit antikoagulierten Patient*innen dazugewinnen konnten. In der Rolle der CM betrachteten sie sich als federführende Studienorganisatorinnen, Vertrauenspersonen für Erkrankte, kompetente Ansprechpartnerinnen und Vermittlerinnen zwischen Hausärzt*innen und Patient*innen. Auch die beteiligten Hausärzt*innen sahen die MFA als treibende Kräfte der Studienorganisation und erkannten, dass sie einen Wissenszuwachs erzielen und damit die Rolle einer kompetenten Ansprechpartnerin für Patient*innen einnehmen konnten. Demgegenüber waren die MFA aus Patientensicht vorwiegend für die Studienorganisation verantwortlich und wurden eher weniger als kompetente Ansprechpartnerinnen bei Fragen zum Gerinnungsmanagement gesehen.
Schlussfolgerung: Unter den Befragten zeigte sich ein unterschiedliches Rollenverständnis. Während MFA bereit waren, ihren Kompetenzbereich zu erweitern und eine Aufwertung ihrer beruflichen Funktion beschrieben, wurden die neuen Rollen von der Ärzteschaft und den Patient*innen nur bedingt wahrgenommen. Insbesondere für den Großteil der Erkrankten blieb der Hausarzt / die Hausärztin die einzige Anlaufstelle bei wichtigen medizinischen Themen. Damit besteht weiterer Forschungsbedarf hinsichtlich teambasierter Interventionen, einschließlich der Frage, wie der Kompetenzgewinn der MFA und die damit verbundene Aufwertung ihrer beruflichen Rolle noch stärker ins Bewusstsein der Ärzteschaft und Patient*innen gelangen kann. Schließlich erfordert die erfolgreiche Implementierung eines Case Managements mit stärkerem Einbezug von MFA die Umgestaltung vorhandener Handlungs- und Rollenmuster und die enge Zusammenarbeit aller Beteiligten.
Hintergrund: Das Dravet-Syndrom (DS) ist ein seltenes, in der frühen Kindheit beginnendes, therapierefraktäres Epilepsiesyndrom, das mit einer hohen Morbidität und Mortalität verbunden ist.
Fragestellung: Ziele der Querschnittsstudie „Dravet syndrome caregiver survey“ (DISCUSS) sind die Identifizierung und Beschreibung der Faktoren, die einen Einfluss auf die Krankheitslast von Patienten mit DS und ihre Betreuer haben können. Die Ergebnisse der deutschen Kohorte werden vorgestellt.
Material und Methoden: Die Datenerhebung erfolgte durch eine anonyme Befragung von Eltern. Die Ergebnisse wurden für die verschiedenen Altersgruppen statistisch ausgewertet.
Ergebnisse: Der Fragebogen wurde von 68 Eltern der DS-Patienten mit einem durchschnittlichen Alter von 10 Jahren (Median: 9, Spanne: 1–26) ausgefüllt. Nur 3 Patienten (4,4 %) waren in den letzten 3 Monaten anfallsfrei. Insgesamt hatten 97 % der Patienten, die älter als 5 Jahre waren (n = 45), mindestens eine Komorbidität. Die zum Befragungszeitpunkt am häufigsten eingenommenen Antiepileptika waren Valproat, Kaliumbromid, Stiripentol, Clobazam und Topiramat. In der Vergangenheit wurden Natriumkanalblocker, Phenobarbital und Levetiracetam eingesetzt, aktuell fanden diese Antiepileptika nur selten Verwendung. Die Lebensqualität der Patienten war niedriger als die der Allgemeinbevölkerung. Die Erkrankung eines Familienmitglieds mit DS beeinflusst Eltern und Geschwister in hohem Maße.
Diskussion: Trotz individueller Kombinationstherapien sind die meisten Patienten mit DS nicht anfallsfrei. Insgesamt hat sich der Einsatz von beim DS wenig wirksamer Medikamente und der kontraindizierten Natriumkanalblocker zugunsten von wirksameren Medikamenten verschoben. Neue Therapie- und Versorgungskonzepte sind notwendig, um die Versorgung der Patienten mit DS zu verbessern und Eltern und Geschwister zu entlasten.
Background: Berotralstat (BCX7353) is an oral, once-daily inhibitor of plasma kallikrein in development for the prophylaxis of hereditary angioedema (HAE) attacks.
Objective: Our aim was to determine the efficacy, safety, and tolerability of berotralstat in patients with HAE over a 24-week treatment period (the phase 3 APeX-2 trial).
Methods: APeX-2 was a double-blind, parallel-group study that randomized patients at 40 sites in 11 countries 1:1:1 to receive once-daily berotralstat in a dose of 110 mg or 150 mg or placebo (Clinicaltrials.gov identifier NCT03485911). Patients aged 12 years or older with HAE due to C1 inhibitor deficiency and at least 2 investigator-confirmed HAE attacks in the first 56 days of a prospective run-in period were eligible. The primary efficacy end point was the rate of investigator-confirmed HAE attacks during the 24-week treatment period.
Results: A total of 121 patients were randomized; 120 of them received at least 1 dose of the study drug (n = 41, 40, and 39 in the 110-mg dose of berotralstat, 150-mg of dose berotralstat, and placebo groups, respectively). Berotralstat demonstrated a significant reduction in attack rate at both 110 mg (1.65 attacks per month; P = .024) and 150 mg (1.31 attacks per month; P < .001) relative to placebo (2.35 attacks per month). The most frequent treatment-emergent adverse events that occurred more with berotralstat than with placebo were abdominal pain, vomiting, diarrhea, and back pain. No drug-related serious treatment-emergent adverse events occurred.
Conclusion: Both the 110-mg and 150-mg doses of berotralstat reduced HAE attack rates compared with placebo and were safe and generally well tolerated. The most favorable benefit-to-risk profile was observed at a dose of 150 mg per day.
Although chest radiograph (CXR) is commonly used in diagnosing pediatric community acquired pneumonia (pCAP), limited data on interobserver agreement among radiologists exist. PedCAPNETZ is a prospective, observational, and multicenter study on pCAP. N = 233 CXR from patients with clinical diagnosis of pCAP were retrieved and n = 12 CXR without pathological findings were added. All CXR were interpreted by a radiologist at the site of recruitment and by two external, blinded pediatric radiologists. To evaluate interobserver agreement, the reporting of presence or absence of pCAP in CXR was analyzed, and prevalence and bias-adjusted kappa (PABAK) statistical testing was applied. Overall, n = 190 (82%) of CXR were confirmed as pCAP by two external pediatric radiologists. Compared with patients with pCAP negative CXR, patients with CXR-confirmed pCAP displayed higher C-reactive protein levels and a longer duration of symptoms before enrollment (p < .007). Further parameters, that is, age, respiratory rate, and oxygen saturation showed no significant difference. The interobserver agreement between the onsite radiologists and each of the two independent pediatric radiologists for the presence of pCAP was poor to fair (69%; PABAK = 0.39% and 76%; PABAK = 0.53, respectively). The concordance between the external radiologists was fair (81%; PABAK = 0.62). With regard to typical CXR findings for pCAP, chance corrected interrater agreement was highest for pleural effusions, infiltrates, and consolidations and lowest for interstitial patterns and peribronchial thickening. Our data show a poor interobserver agreement in the CXR-based diagnosis of pCAP and emphasized the need for harmonized interpretation standards.
Myocardial fibrosis and inflammation by CMR predict cardiovascular outcome in people living with HIV

Objectives_: The goal of this study was to examine prognostic relationships between cardiac imaging measures and cardiovascular outcome in people living with human immunodeficiency virus (HIV) (PLWH) on highly active antiretroviral therapy (HAART).
Background: PLWH have a higher prevalence of cardiovascular disease and heart failure (HF) compared with the noninfected population. The pathophysiological drivers of myocardial dysfunction and worse cardiovascular outcome in HIV remain poorly understood.
Methods: This prospective observational longitudinal study included consecutive PLWH on long-term HAART undergoing cardiac magnetic resonance (CMR) examination for assessment of myocardial volumes and function, T1 and T2 mapping, perfusion, and scar. Time-to-event analysis was performed from the index CMR examination to the first single event per patient. The primary endpoint was an adjudicated adverse cardiovascular event (cardiovascular mortality, nonfatal acute coronary syndrome, an appropriate device discharge, or a documented HF hospitalization).
Results: A total of 156 participants (62% male; age [median, interquartile range]: 50 years [42 to 57 years]) were included. During a median follow-up of 13 months (9 to 19 months), 24 events were observed (4 HF deaths, 1 sudden cardiac death, 2 nonfatal acute myocardial infarction, 1 appropriate device discharge, and 16 HF hospitalizations). Patients with events had higher native T1 (median [interquartile range]: 1,149 ms [1,115 to 1,163 ms] vs. 1,110 ms [1,075 to 1,138 ms]); native T2 (40 ms [38 to 41 ms] vs. 37 ms [36 to 39 ms]); left ventricular (LV) mass index (65 g/m2 [49 to 77 g/m2] vs. 57 g/m2 [49 to 64 g/m2]), and N-terminal pro–B-type natriuretic peptide (109 pg/l [25 to 337 pg/l] vs. 48 pg/l [23 to 82 pg/l]) (all p < 0.05). In multivariable analyses, native T1 was independently predictive of adverse events (chi-square test, 15.9; p < 0.001; native T1 [10 ms] hazard ratio [95% confidence interval]: 1.20 [1.08 to 1.33]; p = 0.001), followed by a model that also included LV mass (chi-square test, 17.1; p < 0.001). Traditional cardiovascular risk scores were not predictive of the adverse events.
Conclusions: Our findings reveal important prognostic associations of diffuse myocardial fibrosis and LV remodeling in PLWH. These results may support development of personalized approaches to screening and early intervention to reduce the burden of HF in PLWH (International T1 Multicenter Outcome Study; NCT03749343).
A webinar series that was organised by the Academy of Pharmaceutical Sciences Biopharmaceutics focus group in 2021 focused on the challenges of developing clinically relevant dissolution specifications (CRDSs) for oral drug products. Industrial scientists, together with regulatory and academic scientists, came together through a series of six webinars, to discuss progress in the field, emerging trends, and areas for continued collaboration and harmonisation. Each webinar also hosted a Q&A session where participants could discuss the shared topic and information. Although it was clear from the presentations and Q&A sessions that we continue to make progress in the field of CRDSs and the utility/success of PBBM, there is also a need to continue the momentum and dialogue between the industry and regulators. Five key areas were identified which require further discussion and harmonisation.
Purpose: Optimization of local therapies in synovial sarcoma (SS) considered unresectable at diagnosis is needed. We evaluated the effects of neoadjuvant versus adjuvant radiation versus surgery only on long-term outcomes.
Methods: Patients with macroscopic SS tumors before chemotherapy (IRS-group-III) in the trials CWS-81, CWS-86, CWS-91, CWS-96, CWS-2002-P and SoTiSaR-registry were analyzed. Local therapies were scheduled after 3 neoadjuvant chemotherapy cycles.
Results: Median age of 145 patients was 14.5 years. 106 survivors had median follow-up of 7.0 years. Tumor site was 96 extremities, 19 head–neck, 16 shoulder/hip, 14 trunk. Tumors were < 3 cm in 16, 3–5 cm in 28, 5–10 cm in 55, > 10 cm in 34 patients. In a secondary resection during chemotherapy, R0-status was accomplished in 82, R1 in 30, R2 in 21 (12 missing). Radiotherapy was administered to 115 (R0 61, R1 29, R2 20, missing 5), thereof 57 before and 52 after tumor resection. 23 were treated with surgery only. For all patients, 5 year event-free (EFS) and overall survival (OS) was 68.9% ± 7.6 (95%CI) and 79.1% ± 6.9. To establish independent significance, tumor site, size, surgical results and sequencing of local therapies were analyzed in a Cox regression analysis. Variables associated with EFS and OS are site, size and sequencing of local therapies. Variables associated with local recurrence are site, surgical results and sequencing of local therapies. The only variable associated with suffering metastatic recurrence is tumor size.
Conclusion: Differences in sequencing of local therapy procedures are independently associated with outcomes. Best local control is achieved when tumors are irradiated pre-operatively and undergo R0 or R1 resection thereafter.
Objectives: To discuss optimal management of recurrent urinary tract infections (UTIs) in women. About every second woman experiences at least one UTI in her lifetime, of those 30% experience another UTI, and 3% further recurrences. Especially young healthy women without underlying anatomical deficiencies suffer from recurrent UTIs (rUTI), which are associated with significant morbidity and reduction in quality of life.
Methods: This is a narrative review, investigating publications dealing with recurrent UTI in women. Risk factors and options for management are discussed.
Results: The increased susceptibility of women to rUTI is based on the female anatomy in addition to behavioural, genetic, and urological factors. However, why some women are more likely than others to develop and maintain rUTI remains to be clarified. Invasive characteristics of certain uropathogenic Escherichia coli that are able to form extra- and intracellular biofilms and may therefore cause delayed release of bacteria into the bladder, may play a role in this setting. Treatment recommendations for an acute episode of rUTI do not differ from those for isolated episodes. Given the nature of rUTI, different prophylactic approaches also play an important role. Women with rUTI should first be counselled to use non-antibiotic strategies including behavioural changes, anti-adhesive treatments, antiseptics, and immunomodulation, before antibiotic prophylaxis is considered. In addition to the traditional treatment and prophylactic therapies, new experimental strategies are emerging and show promising effects, such as faecal microbiota transfer (FMT), a treatment option that transfers microorganisms and metabolites of a healthy donor’s faecal matter to patients using oral capsules, enemas, or endoscopy. Initial findings suggest that FMT might be a promising treatment approach to interrupt the cycle of rUTI. Furthermore, bacteriophages, infecting and replicating in bacteria, have been clinically trialled for UTIs.
Conclusion: Due to the limitation of available data, novel treatment options require further clinical research to objectify the potential in treating bacterial infections, particularly UTIs.
Despite recent advances in the treatment of metastatic prostate cancer (PCa), resistance development after taxane treatments is inevitable, necessitating effective options to combat drug resistance. Previous studies indicated antitumoral properties of the natural compound amygdalin. However, whether amygdalin acts on drug-resistant tumor cells remains questionable. An in vitro study was performed to investigate the influence of amygdalin (10 mg/mL) on the growth of a panel of therapy-naïve and docetaxel- or cabazitaxel-resistant PCa cell lines (PC3, DU145, and LNCaP cells). Tumor growth, proliferation, clonal growth, and cell cycle progression were investigated. The cell cycle regulating proteins (phospho)cdk1, (phospho)cdk2, cyclin A, cyclin B, p21, and p27 and the mammalian target of rapamycin (mTOR) pathway proteins (phospho)Akt, (phospho)Raptor, and (phospho)Rictor as well as integrin β1 and the cytoskeletal proteins vimentin, ezrin, talin, and cytokeratin 8/18 were assessed. Furthermore, chemotactic activity and adhesion to extracellular matrix components were analyzed. Amygdalin dose-dependently inhibited tumor growth and reduced tumor clones in all (parental and resistant) PCa cell lines, accompanied by a G0/G1 phase accumulation. Cell cycle regulating proteins were significantly altered by amygdalin. A moderate influence of amygdalin on tumor cell adhesion and chemotaxis was observed as well, paralleled by modifications of cytoskeletal proteins and the integrin β1 expression level. Amygdalin may, therefore, block tumor growth and disseminative characteristics of taxane-resistant PCa cells. Further studies are warranted to determine amygdalin’s value as an antitumor drug.
There are different avenues for obtaining postgraduate doctoral/Ph.D. degrees in Germany and abroad. Depending on their interests and career plans, candidates can choose a postgraduate doctorate/Ph.D. that focuses on a career in academia or a doctorate that does not involve all elements of a Ph.D. and is obtained for the title’s sake. Germany offers this type of diversity and flexibility, whereas the USA postgraduate doctorate model presents a more structured doctorate. The current article provides insight regarding various and more flexible pathways for obtaining a postgraduate doctorate by comparing the German and the American model. The diversity of academic degrees in dentistry and medicine, such as postgraduate doctoral degrees and the higher postdoctoral degrees available in Germany for graduates interested in academia, makes educational evaluation processes and credentials recognition challenging. The lack of transparency and a systematic approach for the academic acknowledgment of the different scientific values of each doctorate type is creating confusion, primarily when German postgraduate doctorate holders pursue academic careers internationally. The current article aims to enhance the knowledge about the different academic degrees and facilitate the educational evaluations, specialty applications, and employment processes. Understanding the additional scientific value of each doctorate type offered in Germany is imperative for their credential recognition internationally.
Autism spectrum disorder (ASD) is a neurodevelopmental disorder with onset in early childhood. While highly heterogeneous, the core manifestations always include persistent difficulties in social interaction and communication, as well as a pattern of restricted interests, repetitive behaviours, and abnormal sensory processing [1]. In addition, psychiatric comorbidity is high [2], and there are genetic risk overlaps with some other mental and neurodevelopmental disorders. In the vast majority of cases, the condition persists into adulthood [3], albeit with various behavioural features and variable mental and somatic comorbidity over a given lifespan. ASD is associated with high societal, educational, and health care costs, and, in many cases, a dramatic impact on the quality of life of patients and their families. ASDs are highly heritable [4], and a multitude of genetic studies have been published. In addition, more recent reviews also emphasize the role of genetic and environmental factors in the pathophysiology of ASD [5,6], which are mediated by lasting epigenetic changes. The genetic architecture of ASD comprises common and rare variations as well as cytogenetic disturbances, such as copy number variations, translocations, inversions, and numerical chromosomal aberrations [7]. Based on the genes affected and the respective functional effects, the idea of personalised medicine is to eventually use that information for the development of targeted treatments or towards the ability to predict the response to a specific intervention, mainly pharmacological but also psychosocial, given the individual’s genetic and environmental risk constellation. The current Special Issue aims to highlight some core aspects regarding basic and applied science approaches in advancing this field of science.
Currently, psychopharmacological treatment in ASD can improve many comorbid neurodevelopmental disorders, such as attention-deficit/hyperactivity disorder or aggressive behaviour, and the core symptoms of restricted and repetitive behaviours [8,9]. No pharmacological options targeting social interaction and communication are available. Social communication and other strongly relevant targets of intervention in ASD [10], such as adaptive behaviour, cognitive and language development, or quality of life may be improved by early behavioural intervention [11]. Still, individual outcomes are highly variable, even with the same kind of psychosocial intervention approach. A better understanding of the pathophysiological mechanisms underlying this broad range of symptoms and abilities, as well as their longitudinal course, is a crucial first step towards the development of personalised treatments.
Given the heterogeneity regarding the ASD phenotype and its underlying etiology, such as diverse genetic variation and additional environmental risks with the related neurobiological mechanisms, discovering new pharmacological treatments for the condition is a huge challenge. This challenge is at the heart of this Special Issue. Here, we have collected a set of contributions providing state-of-the-art coverage, ranging from the theoretical framework, linking genetics to human behaviour and therapy, to initial practical examples of how genetics can provide valuable insights into the personalized clinical management of autistic individuals. To introduce the papers of this Special Issue, a broad summary of the many challenges related to the development of personalised medicine in ASD is given here. In the final statement from the editors, the specific contributions of the articles included in this Special Issue will be summarised.
Due to ongoing demographic changes, the need for care is increasing in Germany. The number of outpatient care services is also rising, and with it, the number of employees in outpatient care, who are also continuously becoming older. Workplace health promotion (WHP) becomes relevant in this context, as it can reduce negative strain reactions and promote employees’ health. The aim of this study was (1) to reveal implemented WHP interventions in German outpatient care services; (2) to examine the potential challenges regarding a successful implementation of WHP measures; and (3) to illuminate further requests and needs experienced by outpatient careworkers. In qualitative field research, 30 semi-structured individual interviews were conducted with German caregivers, using the problem-centered interview method. The collected data were deductively and inductively evaluated and interpreted, using qualitative content analysis according to Mayring. Outpatient caregivers reported various WHP measures known from their workplaces, such as the provision of fruit baskets, programmes to increase physical activity, or a subsidy for a personal gym. They further reported WHP, such as back training, known from other care services. However, the respondents spoke of the challenges regarding the implementation or the use of WHP interventions in general. The most frequently named barriers were a lack of time after work and interventions that were only offered in their leisure time. In the same course, the participants still needed offers to increase physical activity, joint activities, or relaxation techniques. However, respondents highlighted that they preferred the interventions to take place during working hours. This way, they would also be more likely to take advantage of the interventions. The results of this study provide an insight into various WHP measures that already exist, or that are desirable for implementation with regard to caregivers’ needs. Subjectively perceived challenges for a successful implementation of WHP measures represent the importance of adjustments in the work organization of caregivers. It becomes clear that WHP is not yet established in the ambulant care sector, although it appears to be imperative for keeping caregivers healthy. Considering the different needs of employees, the results can provide a basis for the development of needs-based health promotion measures for caregivers.
Purpose: The management of patients with suspected appendicitis remains a challenge in daily clinical practice, and the optimal management algorithm is still being debated. Negative appendectomy rates (NAR) continue to range between 10 and 15%. This prospective study evaluated the accuracy of a diagnostic pathway in acute appendicitis using clinical risk stratification (Alvarado score), routine ultrasonography, gynecology consult for females, and selected CT after clinical reassessment.
Methods: Patients presenting with suspected appendicitis between November 2015 and September 2017 from age 18 years and above were included. Decision-making followed a clear management pathway. Patients were followed up for 6 months after discharge. The hypothesis was that the algorithm can reduce the NAR to a value of under 10%.
Results: A total of 183 patients were included. In 65 of 69 appendectomies, acute appendicitis was confirmed by histopathology, corresponding to a NAR of 5.8%. Notably, all 4 NAR appendectomies had other pathologies of the appendix. The perforation rate was 24.6%. Only 36 patients (19.7%) received a CT scan. The follow-up rate after 30 days achieved 69%, including no patients with missed appendicitis. The sensitivity and specificity of the diagnostic pathway was 100% and 96.6%, respectively. The potential saving in costs can be as much as 19.8 million €/100,000 cases presenting with the suspicion of appendicitis.
Conclusion: The risk-stratified diagnostic algorithm yields a high diagnostic accuracy for patients with suspicion of appendicitis. Its implementation can safely reduce the NAR, simultaneously minimizing the use of CT scans and optimizing healthcare-related costs in the treatment of acute appendicitis.
Purpose: Classification and treatment of WHO grade II/III gliomas have dramatically changed. Implementing molecular markers into the WHO classification raised discussions about the significance of grading and clinical trials showed overall survival (OS) benefits for combined radiochemotherapy. As molecularly stratified treatment data outside clinical trials are scarce, we conducted this retrospective study.
Methods: We identified 343 patients (1995–2015) with newly diagnosed WHO grade II/III gliomas and analyzed molecular markers, patient characteristics, symptoms, histology, treatment, time to treatment failure (TTF) and OS.
Results: IDH-status was available for all patients (259 mutant, 84 IDH1-R132H-non-mutant). Molecular subclassification was possible in 173 tumors, resulting in diagnosis of 80 astrocytomas and 93 oligodendrogliomas. WHO grading remained significant for OS in astrocytomas/IDH1-R132H-non-mutant gliomas (p < 0.01) but not for oligodendroglioma (p = 0.27). Chemotherapy (and temozolomide in particular) showed inferior OS compared to radiotherapy in astrocytomas (median 6.1/12.1 years; p = 0.03) and oligodendrogliomas (median 13.2/not reached (n.r.) years; p = 0.03). While radiochemotherapy improved TTF in oligodendroglioma (median radiochemotherapy n.r./chemotherapy 3.8/radiotherapy 7.3 years; p < 0.001/ = 0.06; OS data immature) the effect, mainly in combination with temozolomide, was weaker in astrocytomas (median radiochemotherapy 6.7/chemotherapy 2.3/radiotherapy 2.0 years; p < 0.001/ = 0.11) and did not translate to improved OS (median 8.4 years).
Conclusion: This is one of the largest retrospective, real-life datasets reporting treatment and outcome in low-grade gliomas incorporating molecular markers. Current histologic grading features remain prognostic in astrocytomas while being insignificant in oligodendroglioma with interfering treatment effects. Chemotherapy (temozolomide) was less effective than radiotherapy in both astrocytomas and oligodendrogliomas while radiochemotherapy showed the highest TTF in oligodendrogliomas.
Background; Musculoskeletal disorders (MSD) are a common health problem among dentists. Dental treatment is mainly performed in a sitting position. The aim of the study was to quantify the effect of different ergonomic chairs on the sitting position. In addition, it was tested if the sitting position of experienced workers is different from a non-dental group.
Methods; A total of 59 (28 m/31f) subjects, divided into two dentist groups according to their work experience (students and dentists (9 m/11f) < 10 years, dentists (9 m/10f) ≥ 10 years) and a control group (10 m/10f) were measured. A three-dimensional back scanner captured the bare back of all subjects sitting on six dentist’s chairs of different design. Initially, inter-group comparisons per chair, firstly in the habitual and secondly in the working postures, were carried out. Furthermore, inter-chair comparison was conducted for the habitual as well as for the working postures of all subjects and for each group. Finally, a comparison between the habitual sitting posture and the working posture for each respective chair (intra-chair comparison) was conducted (for all subjects and for each group). In addition, a subjective assessment of each chair was made.
For the statistical analysis, non-parametric tests were conducted and the level of significance was set at 5%.
Results: When comparing the three subject groups, all chairs caused a more pronounced spinal kyphosis in experienced dentists. In both conditions (habitual and working postures), a symmetrical sitting position was assumed on each chair.
The inter-chair comparisons showed no differences regarding the ergonomic design of the chairs. The significances found in the inter-chair comparisons were all within the measurementerror and could, therefore, be classified as clinically irrelevant.
The intra-chair comparison (habitual sitting position vs. working sitting position) illustrated position-related changes in the sagittal, but not in the transverse, plane. These changes were only position-related (forward leaned working posture) and were not influenced by the ergonomic sitting design of the respective chair. There are no differences between the groups in the subjective assessment of each chair.
Conclusions; Regardless of the group or the dental experience, the ergonomic design of the dentist’s chair had only a marginal influence on the upper body posture in both the habitual and working sitting postures. Consequently, the focus of the dentist’s chair, in order to minimize MSD, should concentrate on adopting a symmetrical sitting posture rather than on its ergonomic design.
