Background: During ECMO therapy ischemia of the limbs or internal organs are potential lethal complications. This study analyzed incidence and type of ischemic complications during ECMO therapy, divided in limb, mesenteric, cardiac and neurological ischemia.
Methods: In this single-center retrospective observational study data from 348 patients treated with veno-venous, veno-arterial or veno-venous-arterial ECMO at the Asklepios Klinik Langen between April 1st 2011 and March 31st 2020 was screened. 321 patients with diagnosis of acute respiratory distress syndrome, cardiogenic or septic shock were included.
Primary outcome variable was type of ischemic complication. Further variables were serum lactate levels 24h before and immediately after diagnosis of the ischemic complication, duration of ICU and hospital stay, ECMO therapy and duration of invasive ventilation and arterial blood gas analysis on day of admission to the ICU. Age, sex, ECMO mode, diagnosis, SAPS II, SOFA score, hospital mortality, the use of renal replacement therapy and tracheotomy, the occurrence of infections during the ICU stay and the need of CPR before ECMO implantation were recorded as well.
Results: 62/321 patients (19.3%) were diagnosed with an ischemic complication. Most common areas were limbs (n=32) and mesenteric ischemia (n=21). Patients who were diagnosed with a septic shock had the highest rate of ischemic complications (36.2%). In VV mode there was a difference in survival between patients with and without ischemic complication (p=0.025). Using multivariate logistic regression, age ≥50 years (p=0.029; OR=2.793; CI 1.109 – 7.033), use of hemodialysis (p=0.003; OR=3.283; CI=1.513 – 7.124) and initial diagnosis of a septic shock (p=0.049; OR=2.144; CI=1.003 – 4.583) could be identified as predictors for ischemic complications.
Conclusions: Ischemic complications are frequent during ECMO therapy. An age of at least 50 years, the use of hemodialysis and diagnosis of a septic shock were predictors of ischemic complications. No correlation between ECMO mode and ischemic complications was found. An influence of ischemic complications on survival could be found only in patients treated with VV mode.
Hintergrund: Der eigene Körper ist das zentrale Arbeitsinstrument eines*einer Tanzpädagog*in (TP) innerhalb der Bewegungsvermittlung. Bisher fehlen Erkenntnisse über die subjektive Wahrnehmung der eigenen berufsassoziierten Gesundheit und Zufriedenheit sowie die Identifizierung gesundheitsbelastender Berufsmerkmale.
Methodik: Im Rahmen einer fragebogenbasierten Querschnittserhebung wurde eine Kohorte von TP in Deutschland zur eigenen Gesundheit und generellen Berufszufriedenheit und belastenden Aspekten im Zusammenhang mit ihrer Berufsausübung untersucht. Zusätzlich wurden allgemeine anthropometrische und soziodemographische Merkmale erfasst. Neben der Betrachtung der Gesamtkohorte wurde auf geschlechtsspezifische Unterschiede getestet. In die statistische Analyse wurden n = 232 TP (m: 51/w: 181) im Alter von 43,1 ± 11,0 Jahren eingeschlossen.
Ergebnisse: Der allgemeine Gesundheitszustand wurde von 85,3 % der Befragten mit „befriedigend“ (26,1 %) bis „sehr gut“ (14,7 %) beurteilt. 59,2 % der Tanzpädagog*innen schätzten ihre Gesundheit „gut“ (35,3 %) bis „sehr gut“ ein. Es herrschte eine hohe Zufriedenheit mit der eigenen Berufsausübung für 80 % der Teilnehmenden. Die TP fühlten sich überwiegend in der Lage (trifft „voll & ganz“ bzw. „eher zu“), mit den physischen (75,7 %) und psychischen Berufsanforderungen (70,3 %) umzugehen. Als belastende Berufsmerkmale in der Eigenwahrnehmung können neben Zukunftsängsten (51,5 %) vor allem arbeitsorganisatorische (fehlende Zeit für Familie und Freunde bei 28,4 %) und ökonomische Aspekte (Einkommensunsicherheit bei 61,0 % und fehlende Altersabsicherung bei 65,7 %) herausgestellt werden.
Diskussion: Die Berufsausübung als TP geht mit einer hohen generellen Zufriedenheit und einem positiven Empfinden des eigenen Gesundheitszustandes einher. Eine Bestätigung dieser positiven Ergebnisse durch Verletzungs- und Erkrankungsstatistiken steht noch aus. Darüber hinaus wäre eine Verbesserung arbeitsorganisatorischer und ökonomischer Aspekte wünschenswert.
Ataxia telangiectasia (A-T) is a devastating multi-system disorder characterized by progressive cerebellar ataxia and immunodeficiency. The neurological decline may be caused by multiple factors of which ongoing inflammation and oxidative stress may play a dominant role. The objective of the present investigation was to determine cerebrospinal fluid (CSF) proteins and possible low-grade inflammation and its relation to age and neurological deterioration. In the present study, we investigated 15 patients with A-T from 2 to 16 years. Our investigation included blood and CSF tests, clinical neurological examination, A-T score, and MRI findings. The albumin ratio (AR) was analyzed to determine the blood–brain-barrier function. In addition, inflammatory cytokines (IL-1α, IL-6, IL-8, IL-12 p40, IL-17A, IFN-γ, TNF-α) were measured by the multiplex cytometric bead array. We compared the results with those from an age-matched control group. Three of the A-T patients were analyzed separately (one after resection of a cerebral meningioma, one after radiation and chemotherapy due to leukemia, one after stem cell transplantation). Patient had significantly more moderate and severe side effects due to CSF puncture (vomiting, headache, need for anti-emetic drugs) compared with healthy controls. Total protein, albumin, and the AR increased with age indicating a disturbed blood barrier function in older children. There were no differences for cytokines in serum and CSF with the exception of IL-2, which was significantly higher in controls in serum. The AR is significantly altered in A-T patients, but low-grade inflammation is not detectable in serum and CSF.
Hintergrund: Ab Frühjahr 2020 kam es zur weltweiten Verbreitung von SARS-CoV‑2 mit der heute als erste Welle der Pandemie bezeichneten Phase ab März 2020. Diese resultierte an vielen Kliniken in Umstrukturierungen und Ressourcenverschiebungen. Ziel unserer Arbeit war die Erfassung der Auswirkungen der Pandemie auf die universitäre Hals-Nasen-Ohren(HNO)-Heilkunde für die Forschung, Lehre und Weiterbildung. Material und Methoden: Die Direktorinnen und Direktoren der 39 Universitäts-HNO-Kliniken in Deutschland wurden mithilfe einer strukturierten Online-Befragung zu den Auswirkungen der Pandemie im Zeitraum von März bis April 2020 auf die Forschung, Lehre und die Weiterbildung befragt. Ergebnisse: Alle 39 Direktorinnen und Direktoren beteiligten sich an der Umfrage. Hiervon gaben 74,4 % (29/39) an, dass es zu einer Verschlechterung ihrer Forschungstätigkeit infolge der Pandemie gekommen sei. Von 61,5 % (24/39) wurde berichtet, dass pandemiebezogene Forschungsaspekte aufgegriffen wurden. Von allen Kliniken wurde eine Einschränkung der Präsenzlehre berichtet und 97,5 % (38/39) führten neue digitale Lehrformate ein. Im Beobachtungszeitraum sahen 74,4 % der Klinikdirektoren die Weiterbildung der Assistenten nicht gefährdet. Schlussfolgerung: Die Ergebnisse geben einen Einblick in die heterogenen Auswirkungen der Pandemie. Die kurzfristige Bearbeitung pandemiebezogener Forschungsthemen und die Einführung innovativer digitaler Konzepte für die studentische Lehre belegt eindrücklich das große innovative Potenzial und die schnelle Reaktionsfähigkeit der HNO-Universitätskliniken, um auch während der Pandemie ihre Aufgaben in der Forschung, Lehre und Weiterbildung bestmöglich zu erfüllen.
Fasting Ramadan is known to influence patients’ medication adherence. Data on patients’ behavior to oral anticoagulant (OAC) drug intake during Ramadan is missing. We aimed to determine patient-guided modifications of OAC medication regimen during Ramadan and to evaluate its consequences. A multicenter cross-sectional study conducted in Saudi Arabia. Data were collected shortly after Ramadan 2019. Participants were patients who fasted Ramadan and who were on long-term anticoagulation. Patient-guided medication changes during Ramadan in comparison to the regular intake schedule before Ramadan were recorded. Modification behavior was compared between twice daily (BID) and once daily (QD) treatment regimens. Rates of hospital admission during Ramadan were determined. We included 808 patients. During Ramadan, 53.1% modified their intake schedule (31.1% adjusted intake time, 13.2% skipped intakes, 2.2% took double dosing). A higher frequency of patient-guided modification was observed in patients on BID regimen compared to QD regimen. During Ramadan, 11.3% of patients were admitted to hospital. Patient-guided modification was a strong predictor for hospital admission. Patient-guided modification of OAC intake during Ramadan is common, particularly in patients on BID regimen. It increases the risk of hospital admission during Ramadan. Planning of OAC intake during Ramadan and patient education on the risk of low adherence are advisable.
Der Anspruch in der Implantatprothetik, ein hohes Maß an Qualität bei der Funktionalität und Ästhetik zu erreichen, hat zur Entwicklung verschiedener Implantatsysteme geführt. Deren Komponenten sollten nicht nur belastbar, sondern zugleich möglichst gewebeverträglich sein. Große Bedeutung kommt hier dem Bindeglied zwischen enossalem Implantat und der implantatgetragenen Krone, dem Abutment, zu. Dieses macht zumeist lediglich ein kleines Segment des transmukosalen Übergangs aus, zeigt sich jedoch für multiple Risikofaktoren, die den Langzeiterfolg der Implantatversorgung bedingen, verantwortlich.
Zirkonoxidabutments bestechen durch ihre Ästhetik und sehr gute Gewebeverträglichkeit. Titanabutments sind solchen aus Zirkonoxid mechanisch vor allem durch ihr charakteristisches Verhalten im Bereich der IAV überlegen. Um deren Vorteile zu vereinen, werden Standardabutments aus Titan mit einer individualisierten Zirkonoxidsuprakonstruktion adhäsiv zu einer funktionellen Einheit verbunden. Zur Verklebung dieser Hybridabutments eignen sich moderne Befestigungskomposite. Die Belastbarkeit dieser Verbindung kann durch verschiedene In-vitro-Untersuchungen bestätigt werden. Die Verbundfestigkeit kann durch Parameter wie den Zementspalt oder die Höhe des Titanabutments divergieren. Der Prozess der Verklebung muss einen hydrolysestabilen adhäsiven Verbund im feuchten Milieu der Mundhöhle gewährleisten, ohne die physikalischen Eigenschaften der eingesetzten Komponenten zu verändern.
Die Vermeidung bakterieller Kontamination der Hybridabutments erfolgt durch die Sterilisation im Autoklav. Da die Hybridabutments der semikritischen Risikogruppe der MP zugeordnet werden konnten, wurden diese häufig vor dem klinischen Einsatz jedoch nicht sterilisiert. Die Anwendung eines Sterilisationsverfahrens begünstigt sowohl den Zustand des periimplantären Weichgewebes, als auch die Stabilität der IAV bei Hybridabutments, geht allerdings mit einer thermischen Belastung einher. Verfahren zum Lösen von Klebeverbindungen nutzen die Eigenschaft von Klebstoffen, bei kritischem Wärmeeintrag mit einer Schädigung der Polymermatrix zu reagieren. Daher wurde die Hypothese formuliert, dass die Sterilisation im Autoklav den adhäsiven Verbund schwächen und zu niedrigeren Haftwerten führen könnte.
Zur Beurteilung eines möglichen Einflusses der Sterilisation auf die mechanische Widerstandsfähigkeit der verklebten Hybridabutments wurden mehrere Befestigungskomposite in die Untersuchung miteinbezogen. Es wurden 80 Titanbasen angefertigt und adhäsiv mit Zirkonoxidsuprakonstruktionen befestigt. Bei den fünf Befestigungskompositen handelte es sich um den DTK Kleber (Bredent), G-CEM LinkAce (GC Germany), RelyX Unicem2 (3M Espe AG), Multilink Hybridabutment (Ivoclar Vivadent GmbH) und Panavia F2.0 (Kuraray Medical). Von den fünf Gruppen mit je 16 Prüfkörpern durchlief jeweils die Hälfte eine Sterilisation im Autoklav während die Kontrollgruppe bei Raumtemperatur gelagert wurde. Abschließend wurden die Prüfkörper mit einer Universalprüfmaschine in uniaxialer Richtung bis zum Versagen der Verbindung belastet. Die dabei auftretende Abzugskraft wurde aufgezeichnet und die jeweiligen Werte miteinander verglichen.
Bei der Auswertung lagen statistisch signifikante Unterschiede der Haftwerte zwischen einer Test- und Kontrollgruppe, sowie unter den Befestigungskompositen selbst vor. Die Ergebnisse zeigen eine große Varianz, entkräften aber die formulierte Hypothese, dass eine abschließende Sterilisation im Autoklav zu einer Reduktion der Haftkraft innerhalb der Hybridabutments führen könnte. Die Ergebnisse können im Gegenteil dahingehend interpretiert werden, dass die Widerstandsfähigkeit durch die Sterilisation sogar verstärkt wird. Dies stellt vor dem Hintergrund, dass die Sterilisation neben der Beseitigung einer bakteriellen Kontamination auch Debris reduziert und somit die Passung der Komponenten erhöht, eindeutige Vorteile gegenüber weniger effektiven Aufbereitungsmethoden dar. Die positiven Effekte der Sterilisation auf den Knochenerhalt und das periimplantäre Gewebe überwiegen die potentiellen Nachteile nach derzeitigem Kenntnisstand bei weitem.
Da in der vorliegenden Versuchsreihe jedoch nur die Abzugskräfte in axialer Richtung verglichen wurden und keine künstliche Alterung simuliert wurde, bedarf es weiterer Untersuchungen um die mechanische Belastbarkeit in vivo zu verifizieren.
Hintergrund: Für die Langzeitbehandlung vieler chronischen Erkrankungen im Kindesalter ist ein dauerhafter zentralvenöser Zugang notwendig, der die Applikation von Medikamenten und eine parenterale Ernährung sichert. Häufig werden hierfür sogenannte tunnelierte zentralvenöse Katheter wie Hickman-/Broviac- Katheter verwendet. Diese Kathetersysteme weisen spezifische postoperative Komplikationen auf. Katheterassoziierte Komplikationen sind von großer klinischer Relevanz, da es hierdurch zu einer verfrühten Explanation des Katheters kommen kann und die Applikation von Medikamenten nicht mehr gewährleistet ist. Es ist daher entscheidend, die Risiken postoperativer Komplikationen zu identifizieren, um entsprechende Präventions- und Verhaltensregeln zu entwickeln und die Verweildauer des tunnelierten Katheters möglichst langfristig sicherzustellen.
Ziel der vorliegenden retrospektiven Studie war neben der Untersuchung postoperativer Komplikationen tunnelierter Hickman-/Broviac- Katheter im Kindesalter die Identifikation von prädisponierenden Risikofaktoren.
Material und Methoden: In einer retrospektiven Kohortenstudie wurden in dem Zeitraum von Januar 2013 bis Juni 2017 Daten von pädiatrischen Patient:innen bis 21 Jahre mit einliegenden Hickmann-/Broviac Kathethersystemen eingeschlossen. Mögliche Risikofaktoren bezüglich katheterassoziierter Komplikationen wurden analysiert: Alter, Geschlecht, Grunderkrankung, Behandlungsindikation, das zur Implantation verwendete Gefäß, Anzahl der Lumen und die Kathetertage in situ. Die Daten wurden aus Papierakten, digitalen Krankenakten, Operationsberichten und Röntgenaufnahmen der Klinik für Kinderheilkunde und der Klinik für Kinderchirurgie und Kinderurologie des Universitätsklinikums Frankfurt in Excel (Version 2016) anonymisiert erfasst und deskriptiv und explorativ ausgewertet. Signifikante Unterschiede wurden mit Hilfe des X2-Test, des exakten Fisher-Tests, sowie des Mann- Whitney-U-Tests ermittelt. Die Durchführung der Studie wurde vom Ethikkomitee des Universitätsklinikums Frankfurt am Main genehmigt (Nummer 172-18).
Ergebnisse: Insgesamt wurden 333 Patient:innen mit 386 implantierten Kathetern (BARD Hickman-/Broviac- zentralvenöser Katheter; Bard Access Systems, Salt Lake City, UT), in die Studie eingeschlossen. Der Altersmedian der Patient:innen zum Zeitpunkt der Implantation lag bei 6.34 Jahren. Die meisten Katheter wurden wegen einer malignen Erkrankungen implantiert, die entsprechend mit einer Chemotherapie, Stammzelltransplantation und/oder Radiotherapie behandelt wurden. Die Explantation der Katheter erfolgte in der Regel mit der Beendigung der Therapie. In 22% (85 von 386 tunnelierten Kathetern) traten postoperative Komplikationen auf, mit einer kumulativen Ein-Jahres-Inzidenz von 21 % und 0.93 Komplikationen pro 1000 Kathetertagen. Am häufigsten waren Katherinfektionen (0.56 pro 1000 Kathetertagen) gefolgt von Dislokationen (0.29 pro 1000 Kathetertagen). Folgende Risikofaktoren für postoperative Komplikationen wurden identifiziert: Ein Patient:innenenalter von unter einem Lebensjahr, eine parenterale Ernährung, eine Stammzelltransplantation bei nicht malignen Grunderkrankungen, eine hämatologische Erkrankung und Tumore des Zentralen Nervensystems, eine offen chirurgische Implantation des Katheters, die V. jugularis interna als Zielgefäß und die Anwendung einlumiger Katheter. Tunnelierte Katheter, die für die Behandlung von einer Leukämie, einem Lymphom, oder eines soliden Tumors implantiert wurden, wiesen am wenigsten Komplikationen auf.
Schlussfolgerung/Ausblick: Insgesamt zeigt die vorliegende Studie eine mit der internationalen Literatur vergleichbare hohe Rate an postoperativen Komplikationen auf. Katheterinfektionen und Katheterdislokationen sind dabei die häufigsten Komplikationen und sollten daher besondere Beachtung im klinischen Alltag finden. Ferner konnten spezifische, risikobehaftete Patient:innengruppen identifiziert werden. Insbesondere Patient:innen, welche eine parenterale Ernährung erhielten, wiesen Katheterinfektionen auf. Daraus ist abzuleiten, dass insbesondere diesen Patient:innen eine engmaschige Überwachung zu kommen sollte, um frühzeitig Komplikationen zu detektieren und diesen entgegenzuwirken.
Die Gesamtzahl der untersuchten Katheter ist groß, einzelne Subgruppen fallen aber klein aus. Sie beeinflussen sich teilweise gegenseitig. In Zukunft sollten multivariate Studien für eine genauere Extrahierung einzelner Risikofaktoren erfolgen.
Background: Increasing numbers of patients surviving malignant bone tumors around the knee joint have led to an increasing importance to investigate long-term results. This study assessed the long-term results of rotationplasty after resection of malignant bone tumors regarding functional outcome and quality of life to allow better comparison with other treatment options in bone cancer treatment.
Procedure: 60 participants who underwent rotationplasty due to bone cancer took part in this multicentric questionnaire- based study. The long-term functional outcome was measured by the Musculoskeletal tumor society score (MSTS) and the Tegner activity level scale. The health-related quality of life (HRQL) was assessed by using the Short Form Health Survey (SF-36).
Results: Patients treated with rotationplasty (median follow- up of 22 years, range 10–47 years) regained a high level of activity (median MSTS score of 24). Even a return to high level sports was possible (mean Tegner activity level scale of 4). Duration of follow-up did not influence the functional outcome. HRQL scores were comparable to the general German popula tion. Concerns of psychological problems due to the unusual appearance of the rotated foot have not been confirmed.
Conclusion: Rotationplasty can be a good alternative to en- doprosthetic replacement or amputation, either as primary surgery or as a salvage procedure. Especially for growing children and very active patients rotationplasty should be considered.
Acute myeloid leukemia (AML) is a neoplastic disease of an early myeloid precursor cell in hematopoiesis. It leads to the accumulation of monoclonal cells in the bone marrow and the peripheral blood, showing a differentiation block and deregulated self-renewal. Frequently, the leukemic cells exhibit genetic aberrations with reciprocal chromosomal translocations. These translocations induce the formation of a fusion protein, that can lead to new cellular functions and a transformation into a leukemic cell. Common chromosomal translocation in AML are t(8;21) or t(15;17), which cause the formation of the fusion proteins AML1/ETO and PML/RARα and determine the leukemic phenotype of the AML.
The translocation t(6;9) leads to the formation of the fusion protein DEK/CAN and is of special interest, because of its association with mostly young patients and a very aggressive course of the disease. The fusion product induces leukemia in a small subset of hematopoietic stem cells, but its mechanism of leukemogenesis is greatly unknown.
The intention of this work was to characterize the DEK/CAN-induced AML on a molecular genetic level to gain a deeper understanding of the disease pathogenesis. Therefore, gene expression analysis with polymerase chain reaction (PCR) and microarray analysis was performed.
To detect DEK/CAN in different cell lines by PCR and real-time quantitative PCR (qPCR), specific primers and probes were designed, and a standardized workflow was established. Emphasis was placed on the optimization of RNA isolation, DNase treatment, cDNA synthesis with following PCR and qPCR, which enabled the detection of the fusion product DEK/CAN in the cell lines 32B, Phoenix and FKH-1. To quantify the fusion product DEK/CAN, the method of qPCR with absolute and relative quantification was used. Absolute quantification enabled the calculation of an exact copy number of the fusion transcript DEK/CAN with a detection limit of 50 copies/µl at a sensitivity of 10-6, which is of importance in determining the minimal residual disease (MRD) of patients with DEK/CAN-positive AML. MRD detection by qPCR is a highly sensitive diagnostic method to identify leukemic cells, even in low cell counts. This enables a thorough evaluation of the treatment response and allows an early detection of changes in the MRD level as part of the remission control.
Additionally, a microarray gene expression analysis was performed to identify alterations in relevant target genes and associated signaling pathways in DEK/CAN-positive cells.
Because of DEK/CAN’s potential to induce leukemia in a subset of hematopoietic stem cells, Sca+/Lin- cells of the bone marrow of C57Bl/6 mice were used and transfected with the gene products DEK/CAN and PML/RARα. Microarray analysis led to the identification of 16 different genes of interest, which demonstrated significant alterations of gene expression in DEK/CAN-positive cells. They were validated and quantified with TaqMan assay assisted qPCR. The elevated expression of the transcription factors TRIM25, HIF1α and ATF2, in DEK/CAN-positive cells, indicated an altered transcription factor activity and interaction with DNA in the nucleus. The localization of DEK/CAN in the nucleus emphasizes this assumption. Also, the upregulated expression of the nuclear export receptor XPO1 suggested changes in nuclear transport processes and impaired export activity in DEK/CAN-positive cells.
Furthermore, the results demonstrated changes of gene expression in genes that are involved in the JAK/STAT signaling pathway. PTPRC, the Protein Tyrosine Phosphatase Receptor Type C, functions as a direct inhibitor of JAKs (Janus Kinases) and STATs (Signal Transducers and Activators of Transcription) and their associated signaling pathway.
It was shown that the gene expression of PTPRC was significantly reduced in DEK/CAN-positive cells. This allowed the assumption, that the reduced expression of PTPRC led to a loss of inhibition and thus a consecutive hyperactivation of the JAK/STAT signaling pathway. This hypothesis was supported by an independent activation of PIM1, a target gene of STAT5 and the activation of LMO2, a direct target gene of JAK2. In addition, the transmembrane receptor CSF1R, which is directly involved in STAT activation, also showed an upregulation in gene expression.
The results of this work show an activation of the JAK/STAT signaling pathway in DEK/CAN-positive cells, which may be a key mechanism in DEK/CAN-induced leukemogenesis.
Considering treatment options in the future, the addition of targeted therapy, such as pan-JAK inhibitors, to the standard therapy, could be a chance to improve the overall survival rate and the prognosis of t(6;9)-positive AML.
Allogeneic hematopoietic cell transplantation (allo-HCT) is increasingly used in older myelofibrosis (MF) patients, but its risk/benefit ratio compared to non-transplant approaches has not been evaluated in this population. We analyzed the outcomes of allo-HCT in 556 MF patients aged ≥65 years from the EBMT registry, and determined the excess mortality over the matched general population of MF patients ≥65 years managed with allo-HCT (n = 556) or conventional drug treatment (n = 176). The non-transplant cohort included patients with intermediate-2 or high risk DIPSS from the Spanish Myelofibrosis Registry. After a median follow-up of 3.4 years, the estimated 5-year survival rate, non-relapse mortality (NRM), and relapse incidence after transplantation was 40%, 37%, and 25%, respectively. Busulfan-based conditioning was associated with decreased mortality (HR: 0.7, 95% CI: 0.5–0.9) whereas the recipient CMV+/donor CMV- combination (HR: 1.7, 95% CI: 1.2–2.4) and the JAK2 mutated genotype (HR: 1.9, 95% CI: 1.1–3.5) predicted higher mortality. Busulfan-based conditioning correlated with improved survival due to less NRM, despite its higher relapse rate when compared with melphalan-based regimens. Excess mortality was higher in transplanted patients than in the non-HCT cohort in the first year of follow-up (ratio: 1.93, 95% CI: 1.13–2.80), whereas the opposite occurred between the fourth and eighth follow-up years (ratio: 0.31, 95% CI: 0.18–0.53). Comparing the excess mortality of the two treatments, male patients seemed to benefit more than females from allo-HCT, mainly due to their worse prognosis with non-transplant approaches. These findings could potentially enhance counseling and treatment decision-making in elderly transplant-eligible MF patients.
The transcription factor vitamin D receptor (VDR) is the high affinity nuclear target of the biologically active form of vitamin D3 (1,25(OH)2D3). In order to identify pure genomic transcriptional effects of 1,25(OH)2D3, we used VDR cistrome, transcriptome and open chromatin data, obtained from the human monocytic cell line THP-1, for a novel hierarchical analysis applying three bioinformatics approaches. We predicted 75.6% of all early 1,25(OH)2D3-responding (2.5 or 4 h) and 57.4% of the late differentially expressed genes (24 h) to be primary VDR target genes. VDR knockout led to a complete loss of 1,25(OH)2D3–induced genome-wide gene regulation. Thus, there was no indication of any VDR-independent non-genomic actions of 1,25(OH)2D3 modulating its transcriptional response. Among the predicted primary VDR target genes, 47 were coding for transcription factors and thus may mediate secondary 1,25(OH)2D3 responses. CEBPA and ETS1 ChIP-seq data and RNA-seq following CEBPA knockdown were used to validate the predicted regulation of secondary vitamin D target genes by both transcription factors. In conclusion, a directional network containing 47 partly novel primary VDR target transcription factors describes secondary responses in a highly complex vitamin D signaling cascade. The central transcription factor VDR is indispensable for all transcriptome-wide effects of the nuclear hormone.
In 2004, Germany introduced a program based on voluntary contracting to strengthen the role of general practice care in the healthcare system. Key components include structured management of chronic diseases, coordinated access to secondary care, data-driven quality improvement, computerized clinical decision-support, and capitation-based reimbursement. Our aim was to determine the long-term effects of this program on the risk of hospitalization of specific categories of high-risk patients. Based on insurance claims data, we conducted a longitudinal observational study from 2011 to 2018 in Baden-Wuerttemberg, Germany. Patients were assigned to one or more of four open cohorts (in 2011, elderly, n = 575,363; diabetes mellitus, n = 163,709; chronic heart failure, n = 82,513; coronary heart disease, n = 125,758). Adjusted for key patient characteristics, logistic regression models were used to compare the hospitalization risk of the enrolled patients (intervention group) with patients receiving usual primary care (control group). At the start of the study and throughout long-term follow-up, enrolled patients in the four cohorts had a lower risk of all-cause hospitalization and ambulatory, care-sensitive hospitalization. Among patients with chronic heart failure and coronary heart disease, the program was associated with significantly reduced risk of cardiovascular-related hospitalizations across the eight observed years. The effect of the program also increased over time. Over the longer term, the results indicate that strengthening primary care could be associated with a substantial reduction in hospital utilization among high-risk patients.
Objective: To evaluate the incidence and risk factors of generalized convulsive seizure (GCS)-related fractures and injuries during video-EEG monitoring.
Methods: We analyzed all GCSs in patients undergoing video-EEG-monitoring between 2007 and 2019 at epilepsy centers in Frankfurt and Marburg in relation to injuries, falls and accidents associated with GCSs. Data were gathered using video material, EEG material, and a standardized reporting form.
Results: A total of 626 GCSs from 411 patients (mean age: 33.6 years; range 3–74 years; 45.0% female) were analyzed. Severe adverse events (SAEs) such as fractures, joint luxation, corneal erosion, and teeth loosening were observed in 13 patients resulting in a risk of 2.1% per GCS (95% CI 1.2–3.4%) and 3.2% per patient (95% CI 1.8–5.2%). Except for a nasal fracture due to a fall onto the face, no SAEs were caused by falls, and all occurred in patients lying in bed without evidence of external trauma. In seven patients, vertebral body compression fractures were confirmed by imaging. This resulted in a risk of 1.1% per GCS (95% CI 0.5–2.2%) and 1.7% per patient (95% CI 0.8–3.3%). These fractures occurred within the tonic phase of a GCS and were accompanied by a characteristic cracking noise. All affected patients reported back pain spontaneously, and an increase in pain on percussion of the affected spine section.
Conclusions: GCSs are associated with a substantial risk of fractures and shoulder dislocations that are not associated with falls. GCSs accompanied by audible cracking, and resulting in back pain, should prompt clinical and imaging evaluations.
Borrelia miyamotoi, a relapsing fever spirochete transmitted by Ixodid ticks causes B. miyamotoi disease (BMD). To evade the human host´s immune response, relapsing fever borreliae, including B. miyamotoi, produce distinct variable major proteins. Here, we investigated Vsp1, Vlp15/16, and Vlp18 all of which are currently being evaluated as antigens for the serodiagnosis of BMD. Comparative analyses identified Vlp15/16 but not Vsp1 and Vlp18 as a plasminogen-interacting protein of B. miyamotoi. Furthermore, Vlp15/16 bound plasminogen in a dose-dependent fashion with high affinity. Binding of plasminogen to Vlp15/16 was significantly inhibited by the lysine analog tranexamic acid suggesting that the protein–protein interaction is mediated by lysine residues. By contrast, ionic strength did not have an effect on binding of plasminogen to Vlp15/16. Of relevance, plasminogen bound to the borrelial protein cleaved the chromogenic substrate S-2251 upon conversion by urokinase-type plasminogen activator (uPa), demonstrating it retained its physiological activity. Interestingly, further analyses revealed a complement inhibitory activity of Vlp15/16 and Vlp18 on the alternative pathway by a Factor H-independent mechanism. More importantly, both borrelial proteins protect serum sensitive Borrelia garinii cells from complement-mediated lysis suggesting multiple roles of these two variable major proteins in immune evasion of B. miyamotoi.
Implementing an automated monitoring process in a digital, longitudinal observational cohort study

Background: Clinical data collection requires correct and complete data sets in order to perform correct statistical analysis and draw valid conclusions. While in randomized clinical trials much effort concentrates on data monitoring, this is rarely the case in observational studies- due to high numbers of cases and often-restricted resources. We have developed a valid and cost-effective monitoring tool, which can substantially contribute to an increased data quality in observational research.
Methods: An automated digital monitoring system for cohort studies developed by the German Rheumatism Research Centre (DRFZ) was tested within the disease register RABBIT-SpA, a longitudinal observational study including patients with axial spondyloarthritis and psoriatic arthritis. Physicians and patients complete electronic case report forms (eCRF) twice a year for up to 10 years. Automatic plausibility checks were implemented to verify all data after entry into the eCRF. To identify conflicts that cannot be found by this approach, all possible conflicts were compiled into a catalog. This “conflict catalog” was used to create queries, which are displayed as part of the eCRF. The proportion of queried eCRFs and responses were analyzed by descriptive methods. For the analysis of responses, the type of conflict was assigned to either a single conflict only (affecting individual items) or a conflict that required the entire eCRF to be queried.
Results: Data from 1883 patients was analyzed. A total of n = 3145 eCRFs submitted between baseline (T0) and T3 (12 months) had conflicts (40–64%). Fifty-six to 100% of the queries regarding eCRFs that were completely missing were answered. A mean of 1.4 to 2.4 single conflicts occurred per eCRF, of which 59–69% were answered. The most common missing values were CRP, ESR, Schober’s test, data on systemic glucocorticoid therapy, and presence of enthesitis.
Conclusion: Providing high data quality in large observational cohort studies is a major challenge, which requires careful monitoring. An automated monitoring process was successfully implemented and well accepted by the study centers. Two thirds of the queries were answered with new data. While conventional manual monitoring is resource-intensive and may itself create new sources of errors, automated processes are a convenient way to augment data quality.
Background: Amyloid precursor protein (APP) processing is central to Alzheimer’s disease (AD) etiology. As early cognitive alterations in AD are strongly correlated to abnormal information processing due to increasing synaptic impairment, it is crucial to characterize how peptides generated through APP cleavage modulate synapse function. We previously described a novel APP processing pathway producing η-secretase-derived peptides (Aη) and revealed that Aη–α, the longest form of Aη produced by η-secretase and α-secretase cleavage, impaired hippocampal long-term potentiation (LTP) ex vivo and neuronal activity in vivo.
Methods: With the intention of going beyond this initial observation, we performed a comprehensive analysis to further characterize the effects of both Aη-α and the shorter Aη-β peptide on hippocampus function using ex vivo field electrophysiology, in vivo multiphoton calcium imaging, and in vivo electrophysiology.
Results: We demonstrate that both synthetic peptides acutely impair LTP at low nanomolar concentrations ex vivo and reveal the N-terminus to be a primary site of activity. We further show that Aη-β, like Aη–α, inhibits neuronal activity in vivo and provide confirmation of LTP impairment by Aη–α in vivo.
Conclusions: These results provide novel insights into the functional role of the recently discovered η-secretase-derived products and suggest that Aη peptides represent important, pathophysiologically relevant, modulators of hippocampal network activity, with profound implications for APP-targeting therapeutic strategies in AD.
The duration of infectivity of SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) in living patients has been demarcated. In contrast, a possible SARS-CoV-2 infectivity of corpses and subsequently its duration under post mortem circumstances remain to be elucidated. The aim of this study was to investigate the infectivity and its duration of deceased COVID-19 (coronavirus disease) patients. Four SARS-CoV-2 infected deceased patients were subjected to medicolegal autopsy. Post mortem intervals (PMI) of 1, 4, 9 and 17 days, respectively, were documented. During autopsy, swabs and organ samples were taken and examined by RT-qPCR (real-time reverse transcription-polymerase chain reaction) for the detection of SARS-CoV-2 ribonucleic acid (RNA). Determination of infectivity was performed by means of virus isolation in cell culture. In two cases, virus isolation was successful for swabs and tissue samples of the respiratory tract (PMI 4 and 17 days). The two infectious cases showed a shorter duration of COVID-19 until death than the two non-infectious cases (2 and 11 days, respectively, compared to > 19 days), which correlates with studies of living patients, in which infectivity could be narrowed to about 6 days before to 12 days after symptom onset. Most notably, infectivity was still present in one of the COVID-19 corpses after a post-mortem interval of 17 days and despite already visible signs of decomposition. To prevent SARS-CoV-2 infections in all professional groups involved in the handling and examination of COVID-19 corpses, adequate personal safety standards (reducing or avoiding aerosol formation and wearing FFP3 [filtering face piece class 3] masks) have to be enforced for routine procedures.
The advancement of medical technology has led not only to an increase in life expectancy but also to a rise in aging-related diseases. Aging promotes metabolic disorders, in turn affecting cardiovascular health. Derailment of biological processes in the pancreas, liver, adipose tissue, and skeletal muscle impairs glucose and lipid metabolism, and mitochondrial function, triggering the development of diabetes and lipid-related disorders that inflict damage on cardiac and vascular tissues. Long noncoding RNAs (lncRNAs) regulate a wide range of biological process and are one of the key factors controlling metabolism and mitochondria. Here, we discuss the versatile function of lncRNAs involved in the metabolic regulation of glucose and lipid, and mitochondrial function, and how the dysregulation of lncRNAs induces the development of various metabolic disorders and their cardiovascular consequences.
Since 2010, an intensified ambulatory cardiology care programme has been implemented in southern Germany. To improve patient management, the structure of cardiac disease management was improved, guideline-recommended care was supported, new ambulatory medical services and a morbidity-adapted reimbursement system were set up. Our aim was to determine the effects of this programme on the mortality and hospitalisation of enrolled patients with cardiac disorders. We conducted a comparative observational study in 2015 and 2016, based on insurance claims data. Overall, 13,404 enrolled patients with chronic heart failure (CHF) and 19,537 with coronary artery disease (CAD) were compared, respectively, to 8,776 and 16,696 patients that were receiving usual ambulatory cardiology care. Compared to the control group, patients enrolled in the programme had lower mortality (Hazard Ratio: 0.84; 95% CI: 0.77–0.91) and fewer all-cause hospitalisations (Rate Ratio: 0.94; 95% CI: 0.90–0.97). CHF-related hospitalisations in patients with CHF were also reduced (Rate Ratio: 0.76; 95% CI: 0.69–0.84). CAD patients showed a similar reduction in mortality rates (Hazard Ratio: 0.81; 95% CI: 0.76–0.88) and all-cause hospitalisation (Rate Ratio: 0.94; 95% CI: 0.91–0.97), but there was no effect on CAD-related hospitalisation. We conclude that intensified ambulatory care reduced mortality and hospitalisation in cardiology patients.
