Electrical stimulation (EStim) has been shown to promote bone healing and regeneration both in animal experiments and clinical treatments. Therefore, incorporating EStim into promising new bone tissue engineering (BTE) therapies is a logical next step. The goal of current BTE research is to develop combinations of cells, scaffolds, and chemical and physical stimuli that optimize treatment outcomes. Recent studies demonstrating EStim’s positive osteogenic effects at the cellular and molecular level provide intriguing clues to the underlying mechanisms by which it promotes bone healing. In this review, we discuss results of recent in vitro and in vivo research focused on using EStim to promote bone healing and regeneration and consider possible strategies for its application to improve outcomes in BTE treatments. Technical aspects of exposing cells and tissues to EStim in in vitro and in vivo model systems are also discussed.
Aims: SARS-CoV-2 infection is associated with adverse outcomes in patients with cardiovascular disease. Here, we analyzed whether specific biomarkers predict the clinical course of COVID-19 in patients with cardiovascular comorbidities. Methods and results: We enrolled 2147 patients with SARS-CoV-2 infection which were included in the Lean European Open Survey on SARS-CoV‑2 (LEOSS)-registry from March to June 2020. Clinical data and laboratory values were collected and compared between patients with and without cardiovascular comorbidities in different clinical stages of the disease. Predictors for mortality were calculated using multivariate regression analysis. We show that patients with cardiovascular comorbidities display significantly higher markers of myocardial injury and thrombo-inflammatory activation already in the uncomplicated phase of COVID-19. In multivariate analysis, elevated levels of troponin [OR 1.54; (95% CI 1.22–1.96), p < 0.001)], IL-6 [OR 1.69 (95% CI 1.26–2.27), p < 0.013)], and CRP [OR 1.32; (95% CI 1.1–1.58), p < 0.003)] were predictors of mortality in patients with COVID-19. Conclusion: Patients with cardiovascular comorbidities show elevated markers of thrombo-inflammatory activation and myocardial injury, which predict mortality, already in the uncomplicated phase of COVID-19. Starting targeted anti-inflammatory therapy and aggressive anticoagulation already in the uncomplicated phase of the disease might improve outcomes after SARS-CoV-2 infection in patients with cardiovascular comorbidities.
Background: From a global viewpoint, endometrial cancer belongs to the most common female cancers. Despite the heavy burden of diseases and numerous unanswered questions, no detailed pictures of the global structure of endometrial cancer research are available so far. Therefore, this malignancy was reviewed using the New Quality and Quantity Indices in Science (NewQIS) protocol.
Methods: Using NewQIS, we identified endometrial carcinoma related research published in the Web of Science from 1900–2015 (P1) and from 2016–2020 (P2). Item analysis was performed with regard to research activity. Also, semi-qualitative aspects and socio-economic benchmarks were visualized using density equalizing mapping.
Results: In total, 9,141 from 1900–2015 and 4,593 from 2016–2020 endometrial cancer related studies were identified with the USA having the largest numbers of publications, citations, institutions, as well as the highest country-specific h-Index concerning endometrial cancer research in both periods. In contrast to other fields of cancer research, the two East Asian countries Japan and China followed concerning total research activities until 2015. From 2016 until 2020, China was found in short distance to the USA and was ranked second. In the socio-economic analysis, European countries were in prominent positions. Greece published 579.83 endometrial carcinoma-related articles per billion US-$ GDP, Finland (527.29), Sweden (494.65), Israel (493.75), and Norway (367.85) followed in the ranking. Density equalizing mapping visualized that large parts of Africa, Asia and South America with a high burden of disease played almost no visible role in the endometrial cancer research activities.
Conclusions: Endometrial cancer research activity is continuously increasing from a global viewpoint. However, the majority of original articles is published by authors based in high-income countries. Together with the finding that the research field of public health does only play a minimal role, our study points to the necessity that global health aspects should be introduced to endometrial cancer research.
Unter Endometriose versteht man das Auftreten von endometrialen Zellen und Zellverbänden außerhalb des Cavum uteri, welche dem hormonellen Zyklus unterliegen und zu rezidivierenden Beschwerden führen können. Die Inzidenz wird je nach Quelle mit 2–15 % der Frauen im reproduktionsfähigen Alter angegeben. Schmerzen und Fertilitätseinschränkung sind die führenden Symptome. Unter Kinderwunschpatientinnen liegt die Inzidenz bei 20–48 %.
Humanized mouse models have become increasingly valuable tools to study human hematopoiesis and infectious diseases. However, human T cell differentiation remains inefficient. We generated mice expressing human interleukin (IL-7), a critical growth and survival factor for T cells, under the control of murine IL-7 regulatory elements. After transfer of human cord blood-derived hematopoietic stem and progenitor cells, transgenic mice on the NSGW41 background, termed NSGW41hIL7, showed elevated and prolonged human cellularity in the thymus while maintaining physiological ratios of thymocyte subsets. As a consequence, numbers of functional human T cells in the periphery were increased without evidence for pathological lymphoproliferation or aberrant expansion of effector or memory-like T cells. We conclude that the novel NSGW41hIL7 strain represents an optimized mouse model for humanization to better understand human T cell differentiation in vivo and to generate a human immune system with a better approximation of human lymphocyte ratios.
Enhanced LTP of population spikes in the dentate gyrus of mice haploinsufficient for neurobeachin

Deletion of the autism candidate molecule neurobeachin (Nbea), a large PH-BEACH-domain containing neuronal protein, has been shown to affect synaptic function by interfering with neurotransmitter receptor targeting and dendritic spine formation. Previous analysis of mice lacking one allele of the Nbea gene identified impaired spatial learning and memory in addition to altered autism-related behaviours. However, no functional data from living heterozygous Nbea mice (Nbea+/−) are available to corroborate the behavioural phenotype. Here, we explored the consequences of Nbea haploinsufficiency on excitation/inhibition balance and synaptic plasticity in the intact hippocampal dentate gyrus of Nbea+/− animals in vivo by electrophysiological recordings. Based on field potential recordings, we show that Nbea+/− mice display enhanced LTP of the granule cell population spike, but no differences in basal synaptic transmission, synapse numbers, short-term plasticity, or network inhibition. These data indicate that Nbea haploinsufficiency causes remarkably specific alterations to granule cell excitability in vivo, which may contribute to the behavioural abnormalities in Nbea+/− mice and to related symptoms in patients.
Entorhinal-retrosplenial circuits for allocentric-egocentric transformation of boundary coding

Spatial navigation requires landmark coding from two perspectives, relying on viewpoint-invariant and self-referenced representations. The brain encodes information within each reference frame but their interactions and functional dependency remains unclear. Here we investigate the relationship between neurons in the rat's retrosplenial cortex (RSC) and entorhinal cortex (MEC) that increase firing near boundaries of space. Border cells in RSC specifically encode walls, but not objects, and are sensitive to the animal’s direction to nearby borders. These egocentric representations are generated independent of visual or whisker sensation but are affected by inputs from MEC that contains allocentric spatial cells. Pharmaco- and optogenetic inhibition of MEC led to a disruption of border coding in RSC, but not vice versa, indicating allocentric-to-egocentric transformation. Finally, RSC border cells fire prospective to the animal’s next motion, unlike those in MEC, revealing the MEC-RSC pathway as an extended border coding circuit that implements coordinate transformation to guide navigation behavior.
Die Therapie langstreckiger Knochendefekte stellt auch weiterhin eine große Herausforderung dar. Dies beruht unter anderem darauf, dass der therapeutische Goldstandard - die Verwendung von autogener Knochensubstanz aus dem Beckenkamm - neben der begrenzten Verfügbarkeit vor allem Komplikationen im Bereich der Entnahmestelle mit sich bringen kann. Es wurde bisher aber noch kein durchschlagendes Ergebnis in der Entwicklung neuer Scaffolds zum Einsatz bei langstreckigen Knochendefekten erreicht. Dies kann eine Vielzahl an Ursachen haben, die sich von der verwendeten Ausgangssubstanz, bis hin zum verwendeten Design erstrecken können. Neben dem Ausgangsmaterial spielen vor
allem die Formgebung und physikalische Eigenschaften, wie Porosität und Mikroarchitektur, eine wichtige Rolle.
Ein aktueller Ansatz zur Nutzung als alternatives Knochenersatzmaterial ist das Knochen-Tissue-Engineering. Hierbei werden körpereigene, knochen-regenerative Zellen mit einem dreidimensionalen Gerüststoff (Knochenersatzmaterial oder -scaffold) kombiniert und in den Knochendefekt implantiert. In dieser Arbeit wurde der Fokus auf die Designentwicklung eines neuen Kochenersatz-Scaffolds gelegt. Nach Vorbild schon vorgestellter Knochenersatzdesigns und unter Berücksichtigung einer Grundstruktur, die auch Phasen der Knochenheilung wie die Frakturhämatomausbreitung und initiale Nährstoffversorgung einbeziehen sollte, wurden mehrere Designs (Raster, Tempel, Zwiebel) entwickelt. Mithilfe des additiv extrusionsbasierten Schmelzschichtverfahrens (Fused Filament Fabrication) wurden die in Computer-Aided Design entworfenen Scaffolds realisiert.
Dieser Ansatz beinhaltet, unter Verwendung des resorbierbaren und biokompatiblen Trägerpolymers Polylaktat, mehrstufige Designs, die kleine biologisch funktionelle Einheiten in eine tragende, kompressionsfeste Rahmenstruktur einbetten. Hierdurch entsteht einerseits die nötige mechanische Belastbarkeit und andererseits eine offene Architektur mit Poren, die Diffusion von Sauerstoff und
Nährstoffen in die inneren Bereiche des Implantats ermöglicht. Es wurden verschiedene Designs entwickelt, gedruckt und mechanisch sowie in vitro in den Kernbereichen Zelladhäsion, Zellaktivität und osteogene Differenzierung nach Besiedelung mit Saos-2-Zellen charakterisiert.
Ein weiterer Entwicklungsschritt stellte das Einführen eines neuartigen, innerhalb der Designs kompatiblen Baukastensystems dar. Hierdurch wird nicht nur die Anpassbarkeit an den Knochendefekt verbessert, es sind auch weitere Funktionen ergänzbar und die unterschiedlichen Designs untereinander kombinierbar.
Die Ergebnisse dieser Dissertationsarbeit dienen als Basis für einen völlig neuen Ansatz von Knochenersatzmaterialien mit positiven biologischen sowie biophysikalischen Eigenschaften.
Purpose: Acute elbow dislocations are complex injuries that predispose to chronic instability and pain. The ideal treatment strategy is part of controversial discussion and evidence-based recommendations for the treatment could not be concluded from the literature. The purpose of the present study was to assess current epidemiological data, injury pattern, and the changing trend for treatment.
Methods: This study presents a retrospective review of 72 patients ≥ 18 years of age who were treated in our level I trauma centre with acute elbow dislocations from 2014 to 2018. The data were acquired by analysis of the institution’s database, and radiological examinations.
Results: The average age of the patients was 48.5 years (range 18–86). The ratio of male to female patients was 1.9:1. A fall onto the outstretched arm (42%) was the most common injury mechanism. By classification, 42% of the elbow dislocations were simple, and 58% complex. A total of 85% of patients underwent surgery including 73% of the simple elbow dislocations due to remaining instability or non-congruency of the reduced elbow. The indication for surgical treatment correlated merely with the grade of instability and displacement, but not with age.
Conclusion: Acute elbow dislocations need identification of the precise injury pattern and instability after reduction of the elbow joint. To achieve a congruent and stable joint, we recommend primary surgical repair as first-line treatment for patients with unstable simple and complex elbow dislocation independent of age.
Purpose: Despite the high number of patients with phalangeal fractures, evidence-based recommendations for the treatment of specific phalangeal fractures could not be concluded from the literature. The purpose of the present study was to assess current epidemiological data, classification of the fracture type, and mode of treatment.
Methods: This study presents a retrospective review of 261 patients with 283 phalangeal fractures ≥ 18 years of age who were treated in our level I trauma centre between 2017 and 2018. The data were obtained by the analysis of the institution’s database, and radiological examinations.
Results: The average age of the patients was 40.4 years (range 18–98). The ratio of male to female patients was 2.7:1. The two most typical injury mechanisms were crush injuries (33%) and falls (23%). Most phalangeal fractures occurred in the distal phalanx (P3 43%). The 4th ray (D4 29%) was most frequently affected. The P3 tuft fractures, and the middle phalanx (P2) base fractures each accounted for 25% of fracture types. A total of 74% of fractures were treated conservatively, and 26% required surgery, with Kirschner wire(s) (37%) as the preferred surgical treatment. The decision for surgical treatment correlated with the degree of angular and/or rotational deformity, intraarticular step, and sub-/luxation of specific phalangeal fractures, but not with age and gender.
Conclusions: Our findings demonstrated the popularity of conservative treatment of phalangeal fractures, while surgery was only required in properly selected cases. The correct definition of precise fracture pattern in addition to topography is essential to facilitate treatment decision-making.

Abstract
Objective: This retrospective study examined patients with probable Lennox-Gastaut syndrome (LGS) identified from German healthcare data.
Methods: This 10-year study (2007–2016) assessed healthcare insurance claims information from the Vilua Healthcare research database. A selection algorithm considering diagnoses and drug prescriptions identified patients with probable LGS. To increase the sensitivity of the identification algorithm, two populations were defined: all patients with probable LGS (broadly defined) and only those with a documented epilepsy diagnosis before 6 years of age (narrowly defined). This specific criterion was used as LGS typically has a peak seizure onset between age 3 and 5 years. Primary analyses were prevalence and demographics; secondary analyses included healthcare costs, hospitalization rate and length of stay (LOS), medication use, and mortality.
Results: In the final year of the study, 545 patients with broadly defined probable LGS (mean [range] age: 31.4 [2–89] years; male: 53%) were identified. Using the narrowly defined probable LGS definition, the number of patients was reduced to 102 (mean [range] age: 7.4 [2–14] years; male: 52%). Prevalence of broadly defined and narrowly defined probable LGS was 39.2 and 6.5 per 100,000 people. During the 10-year study, 208 patients with narrowly defined probable LGS were identified and followed up for 1379 patient-years. The mean annual cost of healthcare was €22,787 per patient-year (PPY); greatest costs were attributable to inpatient care (33%), home nursing care (13%), and medication (10%). Mean annual healthcare costs were significantly greater for those with prescribed rescue medication (45% of patient-years) versus those without (€33,872 vs. €13,785 PPY, p < 0.001). Mean (standard deviation [SD]) annual hospitalization rate was 1.6 (2.0) PPY with mean (SD) annual LOS of 22.7 (46.0) days. Annual hospitalization rate was significantly greater in those who were prescribed rescue medication versus those who were not (2.2 [2.3] vs. 1.1 [1.6] PPY, p < 0.001). The mean (SD) number of different medications prescribed was 11.3 (7.3) PPY and 33.8 (17.0) over the entire observable time per patient (OET); antiepileptic drugs only accounted for 2.1 (1.1) of the medications prescribed PPY and 3.8 (2.0) OET. Over the 10-year study period, mortality in patients with narrowly defined probable LGS was significantly higher than the matched control population (six events [2.88%] vs. one event [0.01%], p < 0.001).
Conclusion: Annual healthcare costs incurred by patients with probable LGS in Germany were substantial, and mostly attributable to inpatient care, home nursing care, and medication. Patients prescribed with rescue medication incurred significantly greater costs than those who were not. Patients with narrowly defined probable LGS had a higher mortality rate versus control populations.
Respiratory complex I catalyzes electron transfer from NADH to ubiquinone (Q) coupled to vectorial proton translocation across the inner mitochondrial membrane. Despite recent progress in structure determination of this very large membrane protein complex, the coupling mechanism is a matter of ongoing debate and the function of accessory subunits surrounding the canonical core subunits is essentially unknown. Concerted rearrangements within a cluster of conserved loops of central subunits NDUFS2 (β1-β2S2 loop), ND1 (TMH5-6ND1 loop) and ND3 (TMH1-2ND3 loop) were suggested to be critical for its proton pumping mechanism. Here, we show that stabilization of the TMH1-2ND3 loop by accessory subunit LYRM6 (NDUFA6) is pivotal for energy conversion by mitochondrial complex I. We determined the high-resolution structure of inactive mutant F89ALYRM6 of eukaryotic complex I from the yeast Yarrowia lipolytica and found long-range structural changes affecting the entire loop cluster. In atomistic molecular dynamics simulations of the mutant, we observed conformational transitions in the loop cluster that disrupted a putative pathway for delivery of substrate protons required in Q redox chemistry. Our results elucidate in detail the essential role of accessory subunit LYRM6 for the function of eukaryotic complex I and offer clues on its redox-linked proton pumping mechanism.
Identifying co-expression of lipid species is challenging, but indispensable to identify novel therapeutic targets for breast cancer treatment. Lipid metabolism is often dysregulated in cancer cells, and changes in lipid metabolism affect cellular processes such as proliferation, autophagy, and tumor development. In addition to mRNA analysis of sphingolipid metabolizing enzymes, we performed liquid chromatography time-of-flight mass spectrometry analysis in three breast cancer cell lines. These breast cancer cell lines differ in estrogen receptor and G-protein coupled estrogen receptor 1 status. Our data show that sphingolipids and non-sphingolipids are strongly increased in SKBr3 cells. SKBr3 cells are estrogen receptor negative and G-protein coupled estrogen receptor 1 positive. Treatment with G15, a G-protein coupled estrogen receptor 1 antagonist, abolishes the effect of increased sphingolipid and non-sphingolipid levels in SKBr3 cells. In particular, ether lipids are expressed at much higher levels in cancer compared to normal cells and are strongly increased in SKBr3 cells. Our analysis reveals that this is accompanied by increased sphingolipid levels such as ceramide, sphingadiene-ceramide and sphingomyelin. This shows the importance of focusing on more than one lipid class when investigating molecular mechanisms in breast cancer cells. Our analysis allows unbiased screening for different lipid classes leading to identification of co-expression patterns of lipids in the context of breast cancer. Co-expression of different lipid classes could influence tumorigenic potential of breast cancer cells. Identification of co-regulated lipid species is important to achieve improved breast cancer treatment outcome.
Suspected Blood Indicator (SBI) ist ein Modus in der RAPID® Reader 6 Software des Kapselendoskopie (KE) Systems, PillCam® SB2 (Given Imaging Ltd., seit 2015 Medtronic GmbH), welcher den Behandler bei der Auffindung potenzieller Dünndarmblutungen unterstützt. Ziel dieser Arbeit ist es, die Sensitivität sowie die Spezifität des SBI-Modus der zweiten Kapselgeneration (PillCam® SB2) zu evaluieren.
Es handelt sich um eine retrospektive Studie, bei der KE-Aufnahmen von insgesamt 199 Patienten, die sich in der Zeit von Juni 2008 bis März 2013 in der Universitätsklinik Frankfurt einer KE-Untersuchung unterzogen hatten, beurteilt wurden. Die Aufnahmen wurden von erfahrenen Behandlern konventionell durchgesehen und auf blutende sowie potenziell blutende Läsionen untersucht. Gleichzeitig wurden die SBI-Markierungen in diesen Aufnahmen von einem unabhängigen Mitarbeiter gezählt und die Ergebnisse den Befunden und Diagnosen der erfahrenen Behandler gegenüberge-stellt.
Das Patientenkollektiv bestand aus 64 weiblichen und 135 männlichen Patienten (mitt-leres Alter 59 Jahre, Spannweite 12–91 Jahre). Die häufigsten Indikationen zur KE-Untersuchung waren in 97/199 Fällen eine mittlere gastrointestinale Blutung und in 50/199 Fällen eine Eisenmangelanämie. In 157/199 Fällen hatten die erfahrenen Be-handler keine Blutung feststellen können. In 137/199 Aufnahmen setzte SBI jedoch mindestens eine Markierung (durchschnittlich 18,4 Markierungen pro Aufnahme) und in 20/199 Aufnahmen gab es keine Markierung. In 13/199 Untersuchungen stellten die Behandler wenig Blut fest, SBI fand hier ebenfalls Blut und setzte durchschnittlich 36,1 Markierungen. In 29 Aufnahmen wurde von dem Behandler viel Blut festgestellt, mit durchschnittlich 46,7 Markierungen pro Aufnahme. SBI setzte Markierungen in insge-samt 179 Aufnahmen, nur in 42 davon hatten auch die erfahrenen Behandler Blutun-gen festgestellt. Alle aktiven Blutungen wurden von SBI erkannt mit einer Sensitivität von 100 % und einer Spezifität von 13 %. Bei der ROC-Analyse wurde der optimale Grenzwert für die Bestimmung einer aktiven Blutung bei 51 SBI-Markierungen (viel Blut) und bei 31 SBI-Markierungen für kleinere Läsionen (wenig Blut) festgesetzt.
Zusammenfassend kann man sagen, dass der SBI-Modus eine hervorragende Hilfe zum Ausschluss einer aktiven Blutung ist. Die hohe Sensitivität Blutungen zu erkennen, kann im Klinikalltag von weniger geübten Mitarbeitern dazu genutzt werden, Patienten oh-ne aktive Blutung schneller zu erkennen und dadurch früher entlassen zu können. Für das Auffinden aktiver Blutungsquellen oder anderer Läsionen sowie für eine genaue Diagnosestellung ist jedoch eine Inspektion der Aufnahme durch einen erfahrenen Behandler weiterhin unabdingbar.
Background: A prototype of a noninvasive glucometer combining skin excitation by a mid-infrared quantum cascade laser with photothermal detection was evaluated in glucose correlation tests including 100 volunteers (41 people with diabetes and 59 healthy people).
Methods: Invasive reference measurements using a clinical glucometer and noninvasive measurements at a finger of the volunteer were simultaneously recorded in five-minute intervals starting from fasting glucose values for healthy subjects (low glucose values for diabetes patients) over a two-hour period. A glucose range from >50 to <350 mg/dL was covered. Machine learning algorithms were used to predict glucose values from the photothermal spectra. Data were analyzed for the average percent disagreement of the noninvasive measurements with the clinical reference measurement and visualized in consensus error grids.
Results: 98.8% (full data set) and 99.1% (improved algorithm) of glucose results were within Zones A and B of the grid, indicating the highest accuracy level. Less than 1% of the data were in Zone C, and none in Zone D or E. The mean and median percent differences between the invasive as a reference and the noninvasive method were 12.1% and 6.5%, respectively, for the full data set, and 11.3% and 6.4% with the improved algorithm.
Conclusions: Our results demonstrate that noninvasive blood glucose analysis combining mid-infrared spectroscopy and photothermal detection is feasible and comparable in accuracy with minimally invasive glucometers and finger pricking devices which use test strips. As a next step, a handheld version of the present device for diabetes patients is being developed.
Evaluation of a rapid turn-over, fully-automated ADAMTS13 activity assay: a method comparison study

Thrombotic thrombocytopenic purpura (TTP) is a life-threatening thrombotic microangiopathy caused by severely reduced activity of the von-Willebrand factor-cleaving protease ADAMTS13, mainly caused by anti-ADAMTS-13 antibodies. Although several test systems for ADAMTS13 measurement exist, long turn-around times hamper the usability in daily practice. We performed a method comparison study for two commercially available ADAMTS13 assays and evaluated the agreement between the fully-automated rapid turn-over HemosIL AcuStar ADAMTS13 Activity assay and the manually performed TECHNOZYM ADAMTS-13 Activity assay. Twenty-four paired test samples derived from 10 consecutively recruited patients (n = 8, acquired TTP; n = 1, atypical hemolytic uremic syndrome; n = 1, control), of which nine test samples were collected in case of clinically apparent TTP and 13 samples were collected from TTP patients in clinical remission were included. Overall correlation between the TECHNOZYM and AcuStar assay was good with a Pearson R of 0.93 (p < 0.001). Agreement between the assays assessed with the Passing–Bablok analysis showed high agreement with an Intercept of − 2.56 (95% confidence interval [CI], − 5.07 to − 0.86) and Slope of 1.04 (95% CI 0.84–1.17). The absolute mean bias was 2.54% (standard difference [SD], 6.38%; 95% CI to 10.0–15.05%). Intra-method reliability was high with an absolute mean bias of − 0.13% (SD 3.21%; 95% CI to 6.42–6.16%). The observer agreement for categorial thresholds (> or < 10% ADAMTS3 activity) was kappa = 0.82 (95% CI 0.59–1.0). Conclusively, overall agreement between the testing methods was sufficient and we support previously published data suggesting the AcuStar assay being a valuable and accurate tool for ADAMTS13 activity testing and TTP diagnostics.
Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) can spread from symptomatic patients with COVID-19, but also from asymptomatic individuals. Therefore, robust surveillance and timely interventions are essential for the control of virus spread within the community. In this regard the frequency of testing and speed of reporting, but not the test sensitivity alone, play a crucial role. In order to reduce the costs and meet the expanding demands in real-time RT-PCR (rRT-PCR) testing for SARS-CoV-2, complementary assays, such as rapid antigen tests, have been developed. Rigorous analysis under varying conditions is required to assess the clinical performance of these tests and to ensure reproducible results. We evaluated the sensitivity and specificity of a recently licensed rapid antigen test using 137 clinical samples in two institutions. Test sensitivity was between 88.2-89.6% when applied to samples with viral loads typically seen in infectious patients. Of 32 rRT-PCR positive samples, 19 demonstrated infectivity in cell culture, and 84% of these samples were reactive with the antigen test. Seven full-genome sequenced SARS-CoV-2 isolates and SARS-CoV-1 were detected with this antigen test, with no cross-reactivity against other common respiratory viruses. Numerous antigen tests are available for SARS-CoV-2 testing and their performance to detect infectious individuals may vary. Head-to-head comparison along with cell culture testing for infectivity may prove useful to identify better performing antigen tests. The antigen test analyzed in this study is easy-to-use, inexpensive, and scalable. It can be helpful in monitoring infection trends and thus has potential to reduce transmission.
Standard monitoring of heart rate, blood pressure and arterial oxygen saturation during endoscopy is recommended by current guidelines on procedural sedation. A number of studies indicated a reduction of hypoxic (art. oxygenation < 90% for > 15 s) and severe hypoxic events (art. oxygenation < 85%) by additional use of capnography. Therefore, U.S. and the European guidelines comment that additional capnography monitoring can be considered in long or deep sedation. Integrated Pulmonary Index® (IPI) is an algorithm-based monitoring parameter that combines oxygenation measured by pulse oximetry (art. oxygenation, heart rate) and ventilation measured by capnography (respiratory rate, apnea > 10 s, partial pressure of end-tidal carbon dioxide [PetCO2]). The aim of this paper was to analyze the value of IPI as parameter to monitor the respiratory status in patients receiving propofol sedation during PEG-procedure. Patients reporting for PEG-placement under sedation were randomized 1:1 in either standard monitoring group (SM) or capnography monitoring group including IPI (IM). Heart rate, blood pressure and arterial oxygen saturation were monitored in SM. In IM additional monitoring was performed measuring PetCO2, respiratory rate and IPI. Capnography and IPI values were recorded for all patients but were only visible to the endoscopic team for the IM-group. IPI values range between 1 and 10 (10 = normal; 8–9 = within normal range; 7 = close to normal range, requires attention; 5–6 = requires attention and may require intervention; 3–4 = requires intervention; 1–2 requires immediate intervention). Results on capnography versus standard monitoring of the same study population was published previously. A total of 147 patients (74 in SM and 73 in IM) were included in the present study. Hypoxic events occurred in 62 patients (42%) and severe hypoxic events in 44 patients (29%), respectively. Baseline characteristics were equally distributed in both groups. IPI = 1, IPI < 7 as well as the parameters PetCO2 = 0 mmHg and apnea > 10 s had a high sensitivity for hypoxic and severe hypoxic events, respectively (IPI = 1: 81%/81% [hypoxic/severe hypoxic event], IPI < 7: 82%/88%, PetCO2: 69%/68%, apnea > 10 s: 84%/84%). All four parameters had a low specificity for both hypoxic and severe hypoxic events (IPI = 1: 13%/12%, IPI < 7: 7%/7%, PetCO2: 29%/27%, apnea > 10 s: 7%/7%). In multivariate analysis, only SM and PetCO2 = 0 mmHg were independent risk factors for hypoxia. IPI (IPI = 1 and IPI < 7) as well as the individual parameters PetCO2 = 0 mmHg and apnea > 10 s allow a fast and convenient conclusion on patients’ respiratory status in a morbid patient population. Sensitivity is good for most parameters, but specificity is poor. In conclusion, IPI can be a useful metric to assess respiratory status during propofol-sedation in PEG-placement. However, IPI was not superior to PetCO2 and apnea > 10 s.
Post-exercise hypotension (PEH) is the phenomenon of lowered blood pressure after a single bout of exercise. Only a fraction of people develops PEH but its occurrence correlates well with long-term effects of sports on blood pressure. Therefore, PEH has been suggested as a suitable predictor for the effectivity of exercise as therapy in hypertension. Local vascular bioactive lipids might play a potential role in this context. We performed a cross-over clinical pilot study with 18 healthy volunteers to investigate the occurrence of PEH after a single short-term endurance exercise. Furthermore, we investigated the plasma lipid profile with focus on arachidonic acid (AA)-derived metabolites as potential biomarkers of PEH. A single bout of ergometer cycling induced a significant PEH in healthy volunteers with the expected high inter-individual variability. Targeted lipid spectrum analysis revealed significant upregulation of several lipids in the direct post-exercise phase. Among these changes, only 15- hydroxyeicosatetranoic acid (HETE) correlated significantly with the extent of PEH but in an AA-independent manner, suggesting that 15-HETE might act as specific PEH-marker. Our data indicate that specific lipid modulation might facilitate the identification of patients who will benefit from exercise activity in hypertension therapy. However, larger trials including hypertonic patients are necessary to verify the clinical value of this hypothesis.
Cancer is the major cause of death besides cardiovascular disease. Leukaemia represents the most prevalent malignancy in children with a frequency of 30 % and is one of the ten leading types of cancer in adults. Philadelphia Chromosome-positive B-ALL (Ph+ B-ALL) is driven by the cytogenetic aberration of the reciprocal chromosomal translocation t(9;22)(q34;q11) leading to the formation of the Philadelphia chromosome with a BCR-ABL1 fusion gene. This fusion gene encodes a BCR-ABL1 oncoprotein which is characterized by a constitutively enhanced tyrosine kinase activity promoting amplified proliferation, differentiation arrest and resistance to cell death. Ph+ B-ALL is considered the most aggressive ALL subtype with a long-term survival rate in the range of only 30 % despite intensive standard of care including chemotherapy in combination with a tyrosine kinase inhibitor (TKI) followed by allogeneic stem cell transplantation after remission for clinically fit patients.
The efficacy of chemotherapy has long been mainly attributed to tumour cell toxicity while immune modulating effects have been overlooked, especially in light of known immunosuppressive properties. Accumulative evidence, however, emphasizes the ability of chemotherapeutic agents, including TKIs, to normalise or re-educate a dysfunctional tumour microenvironment (TME) resulting in enhanced anti-tumour immunity. One of the underlying mechanisms of immune modulation is the induction of immunogenic cell death (ICD). ICD is an anti-tumour agent-induced cell death modality determined by the capacity to convert cancer cells into anti-cancer vaccines. The induction of ICD relies on the release of damage-associated molecular patterns (DAMPs) from dying tumour cells succumbing to ICD. Translocation of CALR to the cell surface, extracellular secretion of ATP and release of HMGB1 from the nucleus are key hallmarks of ICD that mediate anti-tumour immunity upon binding to antigen presenting cells resulting in a tumour antigen-specific immune response. Besides these molecular determinants, ICD is functionally defined by the inhibition of tumour growth in a vaccination assay in which mice are injected with tumour cells exposed to the potential ICD inducer in-vitro and then re-challenged with live tumour cells of the same cancer type. Both molecular and functional criteria determine the gold standard approach to assess ICD. By increasing the immunogenicity of cancer cells, ICD contributes to the restoration of immunosurveillance as an essential feature of tumour rejection, which is clinically reflected by improved therapeutic efficacy and disease outcome in patients. Therefore, identifying novel ICD inducers is an objective of interest in the context of cancer therapy.
In respect of these considerations, the aim addressed in the present work is the examination of the second-generation TKI Nilotinib for the ability to induce ICD. The thesis is set in the context of the group's research on the role of Gas6/TAM signalling within the TME regarding the pathogenesis of acute leukaemia. In in-vivo experiments of our research group it has been consistently observed that the use of Nilotinib enhances the anti-leukaemic immunity mediated by a deletion of Gas6. Against the background of increasing importance of chemotherapeutic agents as potent modulators of a dysregulated TME, it was hypothesized that Nilotinib may synergize with a Gas6-deficient environment by inducing ICD in Ph+ B-ALL cells.
In growth inhibition and Annexin V/Propidium iodide cell death assays Nilotinib was shown to induce cell death in concentration-dependent manner that occurs bimodally in terms of cell death modality ranging between apoptosis and necrosis. By ICD marker analysis, comprising flow-cytometric detection of CALR exposure, chemoluminescence-based ATP measurement and immunoblotting for HMGB1, it was found that Nilotinib-induced cell death is not accompanied by CALR exposure and ATP secretion, but is associated with the release of HMGB1. In macrophages co-culture experiments with Nilotinib-treated leukaemic cells, no relevant shift in terms of macrophages activation and polarisation was observed in either a juxtacrine or paracrine setup. In consistency with the results obtained in the in-vitro experiments, Nilotinib was not potent to elicit a protective immune response in mice within a vaccination assay.
Conclusively, Nilotinib was identified to not qualify as bona fide ICD inducer. The role of Nilotinib-induced cell death and HMGB1 release are proposed as objective for further investigation concerning the synergistic interplay between Nilotinib and a Gas6-deficient environment. Efforts addressing exploration and optimisation of the immunological potential of chemotherapeutic agents are a promising approach aimed at providing cancer patients with the best possible treatment in future.
Inhibitors against the NS3-4A protease of hepatitis C virus (HCV) have proven to be useful drugs in the treatment of HCV infection. Although variants have been identified with mutations that confer resistance to these inhibitors, the mutations do not restore replicative fitness and no secondary mutations that rescue fitness have been found. To gain insight into the molecular mechanisms underlying the lack of fitness compensation, we screened known resistance mutations in infectious HCV cell culture with different genomic backgrounds. We observed that the Q41R mutation of NS3-4A efficiently rescues the replicative fitness in cell culture for virus variants containing mutations at NS3-Asp168. To understand how the Q41R mutation rescues activity, we performed protease activity assays complemented by molecular dynamics simulations, which showed that protease-peptide interactions far outside the targeted peptide cleavage sites mediate substrate recognition by NS3-4A and support protease cleavage kinetics. These interactions shed new light on the mechanisms by which NS3-4A cleaves its substrates, viral polyproteins and a prime cellular antiviral adaptor protein, the mitochondrial antiviral signaling protein MAVS. Peptide binding is mediated by an extended hydrogen-bond network in NS3-4A that was effectively optimized for protease-MAVS binding in Asp168 variants with rescued replicative fitness from NS3-Q41R. In the protease harboring NS3-Q41R, the N-terminal cleavage products of MAVS retained high affinity to the active site, rendering the protease susceptible for potential product inhibition. Our findings reveal delicately balanced protease-peptide interactions in viral replication and immune escape that likely restrict the protease adaptive capability and narrow the virus evolutionary space.
Die Extrakorporale Membranoxygenierung (ECMO) ist ein extrakorporales Organersatzverfahren, welches heutzutage insbesondere als Lungenersatzverfahren oder als kreislaufunterstützendes Verfahren zum Einsatz kommt. Seit dem ersten Einsatz 1971 im Rahmen eines posttraumatischen „Acute Respiratory Distress Syndrom“ (ARDS) ist die Zahl der ECMO-Behandlungen im kardiopulmonalen Bereich massiv gestiegen. Dennoch wird der Nutzen der ECMO kontrovers diskutiert. Die aktuellste randomisiert kontrollierte Studie aus dem Jahre 2018 konnte keinen signifikanten Nachweis erbringen, dass die Therapie des ARDS mittels einer ECMO einen Überlebensvorteil bringt. Randomisiert kontrollierte Studien bei Patienten mit kardiogenem Schock liegen zum gegenwärtigen Zeitpunkt nicht vor und für den Einsatz bei septischem Schock ist die Datenlage und Evidenz schwach. Angesichts der ungeklärten Evidenz und weil die ECMO ein komplexes, kostenintensives und mit Risiken verbundenes Verfahren ist, scheint eine sorgfältige Indikationsstellung unverzichtbar zu sein. Das Ziel unserer Studie war daher die Analyse der Mortalität bei Patienten mit Lungenversagen, kardiogenem Schock und septischem Schock. Darüber hinaus sollten mit der Mortalität assoziierte prognostische Variablen identifiziert werden. Des Weiteren sollte untersucht werden, ob sich mit steigender Fallzahl eine Lernkurve beobachten lässt.
Zu diesem Zwecke führten wir eine retrospektive Kohortenstudie an 131 Patienten durch, die zwischen April 2011 und Juli 2016 in der Asklepios Klinik Langen mit einer ECMO behandelt wurden. Die Patienten wurden hierbei in die drei oben genannten Gruppen kategorisiert. Mithilfe logistischer Regression erfolgte die Identifizierung prognostischer Variablen. Eine Lernkurve wurde anhand des „non-risk-adjusted cumulative observed minus expected failure graph“-Verfahrens erstellt.
Die Analyse der Daten ergab eine Gesamtmortalität von 56%. Die Mortalität bei Patienten mit Lungenversagen, kardiogenem Schock und septischem Schock betrug 54%, 59% bzw. 58%. Bei Patienten mit Lungenversagen waren das Alter, das Jahr und weniger als 20 Fälle pro Jahr signifikant mit der Mortalität assoziiert. Bei Patienten mit kardiogenem Schock waren mit der Mortalität assoziierte Variablen das Alter, der SAPS II-Score, der pH-Wert, der Serumlaktatwert prä-ECMO, der Basenüberschuss prä-ECMO sowie eine Hyperlipidämie. Bei Patienten mit septischem Schock konnten keine mit der Mortalität assoziierten Variablen identifiziert werden. In der multivariaten Regressionsanalyse nach Modellabbau zeigten sich in der gesamten Kohorte das Alter, das prä-ECMO Serumlaktat sowie weniger als 20 Fälle pro Jahr als signifikant mit der Mortalität assoziierte Variablen. Patienten, die vor 2014 behandelt wurden, also in einem Zeitraum mit weniger als 20 ECMO-Fällen pro Jahr, hatten eine signifikant höhere Mortalität als Patienten die im Intervall 2014-2016 behandelt wurden. Mit steigender Fallzahl konnte eine Lernkurve nachgewiesen werden, die einen Wendepunkt im Jahr 2014 zeigte. Der im Jahr 2014 beginnende Abwärtstrend signalisierte eine geringere Sterblichkeitsrate als erwartet.
Zusammenfassend ist die Mortalität in allen Indikationen weiter hoch. Die Fallzahl und damit auch die Erfahrung scheint eine wichtige Rolle in Bezug auf die Mortalität zu spielen. Des Weiteren unterstützen unsere Ergebnisse frühere Studien, die einen Einfluss des Alters und des prä-ECMO Laktat-Wertes auf die Mortalität zeigen konnten.
Background In the pandemic, testing for SARS-CoV-2 by RT-PCR in one of the pillars on which countermeasures are based. Factors limiting the output of laboratories interfere with the effectiveness of public health measures. Conserving reagents by pooling samples in low-probability settings is proposed, but may cause dilution and loss of sensitivity.
Methods We tested an alternate approach (FACT) by simultaneously incubating multiple respiratory swabs in a single tube. This protocol was evaluated by serial incubation of a respiratory swab in up to 10 tubes. The analytics validity of this concept was demonstrated in a five-sample mini pool set-up. It was consequently applied in the testing of 50 symptomatic patients (five-sample pools) as well as 100 asymptomatic residents of a nursing home (ten-sample pools).
Results Serial incubation of a respiratory swab in up to 10 tubes did not lead to a significant decline in viral concentration. The novel FACT-protocol did not cause a false negative result in a five-sample mini-pool setup, with non-significantly differing Ct values between single sample and mini-pool NAT. In two routine applications, all mini pools containing positive patient samples were correctly identified.
Conclusions Our proposed FACT-protocol did not cause a significant loss in analytic or diagnostic sensitivity compared to single sample testing in multiple setups. It reduced the amount of reagents needed by up to 40%, and also reduced hands-on time. This method could enhance testing efficiency, especially in groups with a low pretest-probability, such as systemically relevant professional groups.
Fallzahlaufkommen und Qualitätsindikatoren bei der Versorgung des abdominellen Bauchaortenaneurysmas

Hintergrund: Der MTL30 (Mortalität, Transfer, Liegezeit) wurde als Surrogatparameter zur Evaluation der Qualität potenziell komplikationsträchtiger viszeralchirurgischer Eingriffe vorgeschlagen.
Zielsetzung: Es wurde überprüft, inwieweit sich der MTL30 zu den Ergebnissen des Bauchaortenaneurysma(AAA)-Registers des Deutschen Instituts für Gefäßmedizinische Gesundheitsforschung (DIGG) der Deutschen Gesellschaft für Gefäßchirurgie und Gefäßmedizin (DGG) und zum Fallaufkommen der Kliniken korrelieren lässt.
Material und Methoden: Insgesamt 14.282 Patienten wurden endovaskulär (EVAR) und 3923 Patienten offen (OAR) elektiv wegen eines AAA versorgt. Bestimmt wurden Fallaufkommen der behandelnden Kliniken, Klinikletalität, Liegezeit und Verlegung in ein anderes Akutkrankenhaus 30 Tage nach dem Indexeingriff.
Ergebnisse: Die Klinikletalität machte bei EVAR 1,3 %, bei OAR 4,9 % aus (p = 0,000), der MTL30 5,0 % vs. 14,4 % (p = 0,000). Für EVAR ließ sich keine Beziehung zwischen Fallaufkommen und Klinikletalität (Quintile 1: 1,0 %; Quintile 5: 1,3 %) sowie Fallaufkommen und MTL30 (Quintile 1: 5,3 %; Quintile 5: 5,3 %) nachweisen. Auch bei OAR bestand keine signifikante Beziehung zwischen Fallaufkommen und Klinikletalität (Quintile 1: 5,8 %, Quintile 5: 3,5 %; p = 0,505) und Fallaufkommen und MTL30 (Quintile 1: 16,4 %, Quintile 5: 12,2 %, p = 0,110). Bei einer Klinikletalität von 7,2 (5–10) % betrug der MTL30 bei OAR 17,6 %. Sowohl bei EVAR als auch bei OAR korrelierte die stationäre Aufenthaltsdauer signifikant mit Klinikletalität und MTL30.
Diskussion: Eine eindeutige Beziehung zwischen Krankenhausfallaufkommen und Klinikletalität ließ sich im AAA-Register des DIGG nicht aufzeigen. Das gleiche galt für den MTL30. Ob demnach der MTL30 gegenüber der Erfassung von Klinikletalität und stationärer Liegezeit als Qualitätsparameter einen Zusatznutzen bietet, muss offenbleiben.
Treatment of large bone defects is one of the great challenges in contemporary orthopedic and traumatic surgery. Grafts are necessary to support bone healing. A well-established allograft is demineralized bone matrix (DBM) prepared from donated human bone tissue. In this study, a fibrous demineralized bone matrix (f-DBM) with a high surface-to-volume ratio has been analyzed for toxicity and immunogenicity. f-DBM was transplanted to a 5-mm, plate-stabilized, femoral critical-size-bone-defect in Sprague-Dawley (SD)-rats. Healthy animals were used as controls. After two months histology, hematological analyses, immunogenicity as well as serum biochemistry were performed. Evaluation of free radical release and hematological and biochemical analyses showed no significant differences between the control group and recipients of f-DBM. Histologically, there was no evidence of damage to liver and kidney and good bone healing was observed in the f-DBM group. Reactivity against human HLA class I and class II antigens was detected with mostly low fluorescence values both in the serum of untreated and treated animals, reflecting rather a background reaction. Taken together, these results provide evidence for no systemic toxicity and the first proof of no basic immunogenic reaction to bone allograft and no sensitization of the recipient.
Flat Panel CT Pooled Blood Volume-Mappen vor mechanischer Rekanalisation beim akuten Schlaganfall

In der Akutdiagnostik des Schlaganfalles sind die Multislice Computertomographie (MSCT) sowie gegebenenfalls die Kernspintomographie die radiologischen Methoden der Wahl zunächst zur Differenzierung eines ischämischen oder hämorrhagischen Geschehens sowie im Falle einer Ischämie zur Darstellung des Gefäßverschlusses und der Perfusionssituation. Mit Hilfe von Flat Panel Detektoren (flat panel detector computed tomography (FDCT)) in Angiographie-Einheiten konnten zunächst Schnittbilder des Kopfes ähnlich denen einer konventionellen MSCT angefertigt werden, womit ein Blutungsausschluss möglich ist. Des Weiteren wurden sogenannte „Pooled Blood Volume“ (PBV)-Karten entwickelt, welche konzipiert wurden, um das Areal mit vermindertem zerebralem Blutvolumen (cerebral blood volume = CBV) und somit annäherungsweise den Infarktkern darzustellen.
Innerhalb der letzten 10 Jahre hat sich die mechanische Rekanalisation als Therapie des akuten Verschlusses proximaler zerebraler Arterien durchgesetzt. Häufig müssen Patienten für die Intervention aus peripheren Krankenhäusern in Schlaganfall-Zentren verlegt werden, sodass eine Aktualisierung der zerebralen Perfusionsparameter zur Darstellung der Progression der Ischämie und zur Prognose-Abschätzung erstrebenswert ist. Gäbe es die Möglichkeit einer solchen Bildgebung innerhalb der Angiographie- und Interventions-Einheit, so könnte wertvolle Zeit, welche sonst für den innerklinischen Transport, Umlagerung des Patienten etc. benötigt wird, eingespart werden. Auch bei schwer betroffenen Patienten, welche direkt in das jeweilige Schlaganfall-Zentrum eingeliefert werden, könnte somit die Zeit zwischen Ankunft in der Klinik bis zur Gefäß-Rekanalisation bedeutsam verkürzt werden.
In der vorliegenden Studie wurde die Zuverlässigkeit der PBV-Karten hinsichtlich der Abschätzung des späteren Infarktareales untersucht. Hierfür wurden bei 29 aufeinanderfolgenden Patienten mit akuten intrakraniellen Verschlüssen der Arteria carotis interna oder Arteria cerebri media das präinterventionelle Volumen der in den Quellbildern der PBV-Karten dargestellten Minderperfusion mit dem finalen Infarktvolumen, wie es sich in den postinterventionellen konventionellen MSCT-Kontrollen darstellte, verglichen. Außerdem wurde durch Bestimmung der Hounsfield-Einheiten die Stärke der Minderperfusion in dem Areal der PBV-Veränderung gemessen und mit der gesunden Gegenseite verglichen. Die mechanische Rekanalisation war bei 26 der Patienten erfolgreich.
Insgesamt war das mediane präinterventionelle PBV-Defizit 9-mal größer als das mediane finale Infarktvolumen (86,4 ml (10,3; 111,6) versus 9,6 ml (3,6; 36,8)). Dieses Ergebnis basierte insbesondere auf der Subgruppe der erfolgreich rekanalisierten Patienten (PBV Defizit: 87,5 ml (10,6; 115,1), finales Infarktvolumen: 8,7 ml (3,6; 29)). Im Falle einer frustranen Intervention wurde das finale Infarktvolumen eher unterschätzt (PBV Defizit: 86,4 ml (5,9; -), finales Infarktvolumen: 116,4 ml (3,5; -)). Es gab keinen signifikanten Unterschied zwischen der HU Ratio im Bereich des später infarzierten Gewebes (0,45 (0,29; 0,51)) und der umgebenden minderperfundierten „Penumbra“ (0,4 (0,33; 0,5)) (p=0,679). Die HU Ratio zeigte eine signifikante negative Korrelation mit dem Volumen der PBV-Läsion (r= -0,448, p= 0,008).
Zusammenfassend wurde aus den Ergebnissen dieser Studie geschlossen, dass die FDCT PBV-Karten nicht zuverlässig das finale Infarktvolumen prognostizieren und daher keinen Einfluss auf die akute Therapieentscheidung haben sollten. Ursächlich sind technische und methodische Limitationen sowie die Art des untersuchten Perfusionsparameters.
NIMA (never-in-mitosis gene A)-related kinase 1 (Nek1) is shown to impact on different cellular pathways such as DNA repair, checkpoint activation, and apoptosis. Its role as a molecular target for radiation sensitization of malignant cells, however, remains elusive. Stably transduced doxycycline (Dox)-inducible Nek1 shRNA HeLa cervix and siRNA-transfected HCT-15 colorectal carcinoma cells were irradiated in vitro and 3D clonogenic radiation survival, residual DNA damage, cell cycle distribution, and apoptosis were analyzed. Nek1 knockdown (KD) sensitized both cell lines to ionizing radiation following a single dose irradiation and more pronounced in combination with a 6 h fractionation (3 × 2 Gy) regime. For preclinical analyses we focused on cervical cancer. Nek1 shRNA HeLa cells were grafted into NOD/SCID/IL-2Rγc−/− (NSG) mice and Nek1 KD was induced by Dox-infused drinking water resulting in a significant cytostatic effect if combined with a 6 h fractionation (3 × 2 Gy) regime. In addition, we correlated Nek1 expression in biopsies of patients with cervical cancer with histopathological parameters and clinical follow-up. Our results indicate that elevated levels of Nek1 were associated with an increased rate of local or distant failure, as well as with impaired cancer-specific and overall survival in univariate analyses and for most endpoints in multivariable analyses. Finally, findings from The Cancer Genome Atlas (TCGA) validation cohort confirmed a significant association of high Nek1 expression with a reduced disease-free survival. In conclusion, we consider Nek1 to represent a novel biomarker and potential therapeutic target for drug development in the context of optimized fractionation intervals.
Summary We introduce fsbrain, an R package for the visualization of neuroimaging data. The package can be used to visualize vertex-wise and region-wise morphometry data, parcellations, labels and statistical results on brain surfaces in three dimensions (3D). Voxel data can be displayed in lightbox mode. The fsbrain package offers various customization options and produces publication quality plots which can be displayed interactively, saved as bitmap images, or integrated into R notebooks.

Purpose of Review: Attention deficit hyperactivity disorder (ADHD) shows high heritability in formal genetic studies. In our review article, we provide an overview on common and rare genetic risk variants for ADHD and their link to clinical practice.
Recent findings: The formal heritability of ADHD is about 80% and therefore higher than most other psychiatric diseases. However, recent studies estimate the proportion of heritability based on singlenucleotide variants (SNPs) at 22%. It is a matter of debate which genetic mechanisms explain this huge difference. While frequent variants in first mega-analyses of genome-wideassociation study data containing several thousand patients give the first genome-wide results, explaining only little variance, the methodologically more difficult analyses of rare variants are still in their infancy. Some rare genetic syndromes show higher prevalence for ADHD indicating a potential role for a small number of patients. In contrast, polygenic risk scores (PRS) could potentially be applied to every patient. We give an overview how PRS explain different behavioral phenotypes in ADHD and how they could be used for diagnosis and therapy prediction.
Summary: Knowledge about a patient’s genetic makeup is not yet mandatory for ADHD therapy or diagnosis. PRS however have been introduced successfully in other areas of clinical medicine, and their application in psychiatry will begin within the next years. In order to ensure competent advice for patients, knowledge of the current state of research is useful forpsychiatrists.
Die Sonografie wird als primäre Untersuchungsmethode zur Abklärung vielfältiger Krankheitsbilder eingesetzt und hat sich auch in Leitlinien etabliert. Die Rolle der Sonografie in der Geriatrie ist weniger bekannt und nicht systematisch untersucht.
Ziel der Studie war die Evaluierung der Sonografie als routinemäßig eingesetztes Verfahren und erweiterte körperliche Untersuchung bei geriatrischen Patienten in der medizinischen Akutversorgung. Alle sogenannten geriatrischen Assessments und auch die Sonografie als Screening-Untersuchung erfolgten unabhängig von der Symptomatik des geriatrischen Patienten. Die Befunde wurden mit einem hand-held ultrasound device (HHUSD) und einem high-end ultrasound (HEUS) erhoben und dann verglichen. Die HEUS-Ergebnisse wurden als Goldstandard angesehen. Die Untersuchungen mit HEUS erfolgten meist zu einem späteren Zeitpunkt als mit HHUSD; in der Regel in einem Zeitraum von bis zu sieben Tagen.
Es handelt sich um eine prospektive Studie, die 86 Patienten in einem Zeitraum von 10 Monaten eingeschlossen hat. Die Untersucherin und Doktorandin stellte Folgendes dar: Das Abdomen und die basalen Abschnitte des Thorax sowie die Schilddrüse. Zusätzlich erfolgte eine elastografische Untersuchung der Leber mittels FibroScan.
Die Ultraschalluntersuchung war bei 22/86 (25,6 %) Patienten aufgrund der Symptomatik indiziert und erfolgte bei 64/86 (74,4 %) Patienten als Screening, also ohne klinische Fragestellung. Die Indikationen der Untersuchungen waren: Tumorsuche (8/86 (9,3 %)), Anämie (5/86 (5,8 %)), Leberwert-Erhöhung (5/86 (5,8 %)), Dyspnoe (5/86 (5,8 %)), Frage nach Milzpathologien (2/86 (2,3 %)), Gewichtsverlust (1/86 (1,2 %)), Infektfokussuche (1/86 (1,2 %)), Durchfall (1/86 (1,2 %)), Frage nach intraabdominalem Hämatom (1/86 (1,2 %)) und Kontrolle bei bekanntem Bauchaortenaneurysma (1/86 (1,2 %)). Bei einigen Patienten kamen mehrere Fragestellungen vor.
Die Befunde, die am häufigsten gefunden wurden, waren: Cholezystolithiasis (28/86 (32,6 %); mit HHUSD 5 falsch negative Ergebnisse), rechtsseitiger Pleuraerguss (27/86 (31,4 %); 5 falsch positive Ergebnisse mit HHUSD), Schilddrüsenknoten (26/86 (30,2 %); 2 falsch negative und 1 falsch positiver Befunde mit HHUSD), Nierenzysten (24/86 (27,9 %); 6 falsch negative Ergebnisse mit HHUSD) und Fettleber (23/86 (26,7 %); 3 falsch negative Befunde mit HHUSD). Von 64/86 (74,4 %) untersuchten Patienten ergab sich bei 8/86 (9,3 %) eine therapeutische Konsequenz. Die wichtigsten Befunde, die mit HHUSD übersehen wurden waren: 2 zystische Formationen im Pankreas (2/86 (2,3 %); mit HEUS insgesamt (6/86 (7,0 %)), eine Lebezirrhose (mit HEUS 2/86 (2,3 %)), eine Choledocholithiasis (1/86 (1,2 %); mit HEUS 2/86 (2,3 %)) und ein Lungeninfiltrat (1/86 (1,2 %); mit HEUS 2/86 (2,3 %)). Alle übersehenen Befunde hatten keine dringende therapeutische Konsequenz, so dass die point of care Sonografie (POCUS) hier ausreichend war, um die „ja/nein“-Fragen zu klären.
Bei 65/86 (75,6 %) Patienten war eine Messung der Lebersteifigkeit mittels FibroScan erfolgreich.
Es gab keine Interobserver-Variabilität. Die Untersucherin war eine Fachärztin für Innere Medizin aber keine DEGUM-ausgebildete Ultraschallerin.
Insgesamt zeigte sich eine ausreichende bis gute Übereinstimmung der erhobenen Befunde zwischen HHUSD und HEUS. Die POCUS-Untersuchung lieferte in den meisten Fällen eine Antwort auf die häufigsten Fragestellungen in der Geriatrie (wie Hydratationsstatus, Harnverhalt). Weiterhin konnte die Behandlung bei zufällig gescreenten Patienten optimiert werden. Zur Bestimmung der Organgröße konnte ebenfalls eine gute Korrelation der beiden Geräte festgestellt werden, so dass sich die alleinige Untersuchung mit HHUSD in diesen Fällen ausreichend gezeigt hätte. Ein routinemäßiger Einsatz der Sonografie in der Geriatrie muss in weiteren Studien untersucht werden. Da es sich bei geriatrischen Patienten um multimorbide Patienten mit entsprechend angepassten therapeutischen Zielen handelt, hatten mehrere der erhobenen Befunde in dieser Studie zum Zeitpunkt der geriatrischen Behandlung keine therapeutische Konsequenz.
Bei der Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS) handelt es sich um eine hochprävalente Erkrankung, die bisher insbesondere im Erwachsenenalter nur unzureichend berücksichtigt wurde. Neben den Kernsymptomen bestehend aus Einschränkungen in der Aufmerksamkeit sowie einem erhöhten Maß an Hyperaktivität und Impulsivität gilt es, Komorbiditäten und Beeinträchtigungen zu berücksichtigen, die mit dieser Erkrankung einhergehen. In früheren Studien konnte bereits nachgewiesen werden, dass sowohl Kinder als auch Erwachsene mit einer ADHS vermehrte Unfälle und infolgedessen eine erhöhte Rate an stationären Behandlungen aufweisen. Zusätzlich besteht bei ADHS-Betroffenen ein höheres Risiko, frühzeitig zu versterben, wobei Unfälle als unnatürliche Todesursache den häufigsten Grund darstellen. Bisher existieren auf internationaler Ebene einige Studien, die sich mit den Zusammenhängen von adulter ADHS und Unfallraten beschäftigten. Eine differenzierte Betrachtung, die eine deutsche Population einschließt und den Einfluss des Geschlechts auf das Unfallgeschehen bei ADHS-Betroffenen untersucht, wurde bisher nicht realisiert. Aus diesen Gründen führten wir eine Querschnittsstudie auf unfallchirurgischen Stationen an zwei Kliniken in Frankfurt am Main durch. Im Rahmen der Studie sollte die Prävalenz von adulter ADHS bei stationär betreuten Unfallopfern ermittelt und mögliche unfallcharakteristische Unterschiede zwischen den Geschlechtern der ADHS-Positivkohorte erfasst werden.
Mithilfe der etablierten Adult ADHD Self-Report Scale (ASRS v1.1) der WHO wurden alle Unfallopfer, die den Einschlusskriterien entsprachen, auf adulte ADHS gescreent. Bei der Auswertung des ASRS verwendeten wir zwei unterschiedliche Methoden, die bereits in früheren Studien zur Anwendung kamen. Durch einen eigens erstellten Unfallfragebogen konnten Unfallcharakteristika sowie psychische Komorbiditäten der ADHSPositivkohorte erfasst werden. Zusätzlich erhoben wir zum Vergleich die Unfallcharakteristika bei einer nicht von ADHS betroffenen Kontrollgruppe. Bei einem Teil der positiv gescreenten Patienten wurde eine ausführliche Testung in der ADHS Spezialambulanz des Universitätsklinikums Frankfurt am Main vorgenommen. Mittels diagnostischer Interviews und weiteren Selbstauskunftsfragebögen (DIVA, HASE) konnte eine definitive ADHS-Diagnose gestellt und das positive Screeningergebnis verifiziert werden.
Wir stellten fest, dass die Prävalenz adulter ADHS unter Unfallopfern mit 6,89 % bzw. 8,68 % in Abhängigkeit der ASRS Auswertungskriterien im Vergleich zu der Prävalenz der adulten ADHS in der Allgemeinbevölkerung erhöht war. Des Weiteren gab es mehr Männer mit einem positiven ADHS-Screeningergebnis und einem Unfallgeschehen. Es konnten keine signifikanten geschlechtsspezifischen Unterschiede in den Unfallcharakteristika und Unfallorten in der ADHS Positivkohorte festgestellt werden. Auch in dem Vorhandensein psychischer Komorbiditäten unterschieden sich die beiden Geschlechtergruppen der ADHS-Positivkohorte nicht. Unabhängig der Geschlechterzugehörigkeit wurden die meisten Unfälle der ADHS-Positivkohorte im Straßenverkehr verzeichnet und als selbstverschuldet eingestuft. Es konnten signifikant mehr Unfälle außerhalb des Straßenverkehrs in der nicht von ADHS betroffenen Kontrollgruppe im Vergleich zu der ADHS Positivkohorte nachgewiesen werden. Insgesamt war nur bei 14 % aller positiv gescreenten Patienten eine ADHS-Erkrankung bereits bekannt und diagnostiziert worden.
Auf Grundlage der Ergebnisse dieser Studie lässt sich schlussfolgern, dass Menschen mit einer ADHS-Symptomatik vermehrt auf unfallchirurgischen Stationen anzutreffen sind und im Einklang mit früheren Forschungsergebnissen ein erhöhtes Unfall- und Verletzungsrisiko aufweisen. Darüber hinaus folgern wir aus unseren Ergebnissen, dass sich Männer und Frauen mit einer ADHS-Erkrankung hinsichtlich Symptomatik, Komorbidität und spezifischer Unfallcharakteristika ähnlicher sein könnten, als dass sie sich in diesen Bereichen unterscheiden. Im Hinblick auf die geringen vorbestehenden ADHS-Diagnosen in der Positivkohorte erscheint eine Etablierung eines Screeningsystems bei Patienten sinnvoll, die im Rahmen von Unfallgeschehen gehäuft stationär betreut werden müssen. Da ADHS-Betroffene ein erhöhtes Risiko für multiple Unfälle zu haben scheinen, könnte unter diesen Voraussetzungen eine adulte ADHS-Symptomatik früher erkannt und negative Folgen wie eine erhöhte Unfallrate reduziert werden. Methodologische Limitierungen bestanden in unserer Erhebung durch eine geringe Anzahl an ausführlichen Testungen zur Verifizierung der ADHS-Diagnose sowie dem Informationsgewinn auf der Basis einer Selbstauskunft. Um Ergebnisse aussagekräftiger zu gestalten und auch kleinere Unfälle zu erfassen, erscheint eine Ausweitung der Testung in Ambulanzen sinnvoll.
Background: Fabry disease (FD), the second most prevalent lysosomal storage disorder, is classified as a rare disease. It often leads to significant quality of life impairments and premature death. Many cases remain undiagnosed due to the rarity and heterogeneity. Further, costs related to treatment often constitute a substantial financial burden for patients and health systems. While its epidemiology is still unclear, newborn screenings suggest that its actual prevalence rate is significantly higher than previously suspected. Methods: Based on well-established methodologies, this study gives an overview about the background of the development of FD-related research and provides a critical view of future needs. Results: On the grounds of benchmarking findings, an increasing research activity on FD can be observed. Most publishing countries are the USA, some European countries, Japan, Taiwan, and South Korea. In general, high-income countries publish comparably more on FD than low- or middle-income economies. The countries' financial and infrastructural background are unveiled as crucial factors for the FD research activity. Conclusions: Overall, there is a need to foster FD research infrastructure in developing and emerging countries with focus on cost-intensive genetic research that is independent from economic interests of big pharmaceutical companies.
Obstructive Sleep Apnea is emerging as a global health epidemic, particularly due to the obesity pandemic. However, comprehensive prevalence data are still lacking and global OSA research has not yet been structurally evaluated. Using the latest comprehensive age/gender-specific BMI and obesity data, a global landscape estimating the risk/burden of OSA was created. Results were presented in relation to an in-depth analysis of OSA research and countries’ socioeconomic/scientific background. While the USA, Canada, and Japan are the highest publishing countries on OSA, Iceland, Greece, and Israel appeared at the forefront when relating the scientific output to socioeconomic parameters. Conversely, China, India, and Russia showed relatively low performances in these relations. Analysis of the estimated population at risk (EPR) of OSA showed the USA, China, India, and Brazil as the leading countries. Although the EPR and OSA research correlated strongly, major regional discrepancies between the estimated demand and actual research performances were identified, mainly in, but not limited to, developing nations. Our study highlights regional challenges/imbalances in the global activity on OSA and allows targeted measures to mitigate the burden of undiagnosed/untreated OSA. Furthermore, the inclusion of disadvantaged countries in international collaborations could stimulate local research efforts and provide valuable insights into the regional epidemiology of OSA.
Introduction: Recommendations for venous thromboembolism and deep venous thrombosis (DVT) prophylaxis using graduated compression stockings (GCS) is historically based and has been critically examined in current publications. Existing guidelines are inconclusive as to recommend the general use of GCS.
Patients/Methods: 24 273 in-patients (general surgery and orthopedic patients) undergoing surgery between 2006 and 2016 were included in a retrospectively analysis from a single center. From January 2006 to January 2011 perioperative GCS was employed additionally to drug prophylaxis and from February 2011 to March 2016 patients received drug prophylaxis alone. According to german guidelines all patients received venous thromboembolism prophylaxis with weight-adapted LMWH. Risk stratification (low risk, moderate risk, high risk) was based on the guideline of the American College of Chest Physicians. Data analysis was performed before and after propensity matching (PM). The defined primary endpoint was the incidence of symptomatic or fatal pulmonary embolism (PE). A secondary endpoint was the incidence of deep venous thromboembolism (DVT).
Results: After risk stratification (low risk n = 16 483; moderate risk n = 4464; high risk n = 3326) a total of 24 273 patient were analyzed. Before to PM the relative risk for the occurrence of a PE or DVT was not increased by abstaining from GCS. After PM two groups of 11 312 patients each, one with and one without GCS application, were formed. When comparing the two groups, the relative risk (RR) for the occurrence of a pulmonary embolism was: Low Risk 0.99 [CI95% 0.998–1.000]; Moderate Risk 0.999 [CI95% 0.95–1.003]; High Risk 0.996 [CI95% 0.992–1.000] (p > 0.05). The incidence of PE in the total group LMWH alone was 0.1% (n = 16). In the total group using LMWH + GCS, the incidence was 0.3% (n = 29). RR after PM was 0.999 [CI95% 0.998–1.00].
Conclusion: In comparison to prior studies with only small numbers of patients our trial shows in a large group of patients with moderate and high risk developing VTE we can support the view that abstaining from GCS-use does not increase the incidence of symptomatic or fatal PE and symptomatic DVT.
SARS-CoV-2 infections are rapidly spreading around the globe. The rapid development of therapies is of major importance. However, our lack of understanding of the molecular processes and host cell signaling events underlying SARS-CoV-2 infection hinder therapy development. We employed a SARS-CoV-2 infection system in permissible human cells to study signaling changes by phospho-proteomics. We identified viral protein phosphorylation and defined phosphorylation-driven host cell signaling changes upon infection. Growth factor receptor (GFR) signaling and downstream pathways were activated. Drug-protein network analyses revealed GFR signaling as key pathway targetable by approved drugs. Inhibition of GFR downstream signaling by five compounds prevented SARS-CoV-2 replication in cells, assessed by cytopathic effect, viral dsRNA production, and viral RNA release into the supernatant. This study describes host cell signaling events upon SARS-CoV-2 infection and reveals GFR signaling as central pathway essential for SARS-CoV-2 replication. It provides with novel strategies for COVID-19 treatment.
Background: Point of care devices for performing targeted coagulation substitution in patients who are bleeding have become increasingly important in recent years. New on the market is the Quantra. It is a device that uses sonorheometry, a sonic estimation of elasticity via resonance, which is a novel ultrasound-based technology that measures viscoelastic properties of whole blood. Several studies have already shown the comparability of the Quantra with devices already established on the market, such as the rotational thromboelastometry (ROTEM) device.
Objective: In contrast to existing studies, this study is the first prospective interventional study using this new system in a cardiac surgical patient cohort. We will investigate the noninferiority between an already existing coagulation algorithm based on the ROTEM/Multiplate system and a new algorithm based on the Quantra system for the treatment of coagulopathic cardiac surgical patients.
Methods: The study is divided into two phases. In an initial observation phase, whole blood samples of 20 patients obtained at three defined time points (prior to surgery, after completion of cardiopulmonary bypass, and on arrival in the intensive care unit) will be analyzed using both the ROTEM/Multiplate and Quantra systems. The obtained threshold values will be used to develop a novel algorithm for hemotherapy. In a second intervention phase, the new algorithm will be tested for noninferiority against an algorithm used routinely for years in our department.
Results: The main objective of the examination is the cumulative loss of blood within 24 hours after surgery. Statistical calculations based on the literature and in-house data suggest that the new algorithm is not inferior if the difference in cumulative blood loss is <150 mL/24 hours.
Conclusions: Because of the comparability of the Quantra sonorheometry system with the ROTEM measurement methods, the existing hemotherapy treatment algorithm can be adapted to the Quantra device with proof of noninferiority.

Hepatic inflammasome activation as origin of Interleukin-1α and Interleukin-1β in liver cirrhosis

In der vorliegenden Untersuchung wurde das bovine, Hydroxylapatit-basierte, Knochenersatzmaterial Hypro-Oss® zunächst ex vivo überprüft, anschließend subkutan in den interskapulären Bereich von 12 weiblichen Wistar-Ratten (Testgruppe) eingebracht; bei 12 weiteren Tieren erfolgte eine Sham-Operation ohne Einbringung von Biomaterial (Kontrollgruppe). Anschließend wurde die Gewebereaktion über 30 Tage beobachtet und die Explantate jeweils nach Tag 3, 15 und 30 histologisch und histomorphometrisch untersucht.
Die histologische Analyse zeigte innerhalb des Beobachtungszeitraums von 30 Tagen eine störungsfreie Eingliederung der Hypro-Oss®-Granula in das umliegende Gewebe. Bereits 3 Tage nach Einbringung des Biomaterials waren mononukleäre Zellen erkennbar, die bis Tag 30 weiter zunahmen. Ab diesem Zeitpunkt zeigten sich auch TRAP-positive, CD-68-negative Multinukleäre Zellen, die das Ergebnis einer Fusion von Makrophagen sind und eine Fremdkörperreaktion indizierten. Nach 30 Tagen zeigten sich die Granula histologisch stabil integriert ohne Anzeichen einer immunologischen Abstoßungsreaktion.
Die CD-68-Expression der aufgefundenen Makrophagen und mehrkernigen Riesenzellen bildete ein Kriterium zur Unterscheidung der MNGCs von Osteoklasten, die ebenfalls mehrkernig sind, aber dieses Cluster of Differentiation nicht tragen. Dies charakterisiert die vorgefundenen MNGCs als Fremdkörper-Riesenzellen, da sie ebenso wie die pathologischen Riesenzellen vom Typ Langerhans CD-68 exprimieren.
Dieses Bild bestätigte sich für die Hypro-Oss®-Gruppe in der histomorphometrischen Betrachtung über eine kontinuierliche Zunahme von überwiegend CD-68-positiven Makrophagen bis zum Tag 30, während sie für die Kontrollgruppe über die gesamte Zeit rückläufig waren. Die Multinukleären Zellen erreichten dagegen bereits an Tag 15 ihren Höhepunkt, während in der Kontrollgruppe über den gesamten Beobachtungszeitraum erwartungsgemäß keine MNGCs gefunden wurden.
Der hoch signifikante Anstieg der MNGC-Zahl der Testgruppe bis Tag 15 korreliert positiv mit den Vaskularisationsdaten, was darauf hindeutet, dass die Multinukleären Zellen durch die Einbringung des Biomaterials induziert wurden und über die Sekretion des Signalmoleküls VEGF einen wesentlichen Faktor für die Blutgefäßbildung bilden.
Eine Auffälligkeit hat sich jedoch in Bezug auf das Alleinstellungsmerkmal von Hypro-Oss® gezeigt, welches bei der Aufreinigung nicht erhitzt wird. Diverse Studien haben einen Zusammenhang der Höhe der Sintertemperatur mit der Bildung von MNGCs nachgewiesen, wonach für Hypro-Oss® eine geringe Induzierung von MNGCs zu erwarten gewesen wäre als für vergleichbare, höher erhitzte bovine Knochenersatzmaterialien. Dagegen zeigten die Vaskularisationsdaten unserer Untersuchung für Hypro-Oss® im Vergleich zu 2 anderen bovinen Knochenersatzmaterialien (Bio-Oss® und BEGO OSS®) jedoch signifikant höhere Werte für die Blutgefäßbildung als dies aus der Korrelation von Sintertemperatur mit der Anzahl Multinukleärer Riesenzellen zu erwarten gewesen wäre.
Aufgrund der relativ kurzen Dauer der Beobachtung lassen sich keine belastbaren Ergebnisse in Bezug auf den zu erwartenden Materialabbau und die ossäre Integration von Hypro-Oss® feststellen, welche einer längerfristigen Analyse bedürften als es in dieser Untersuchung möglich war. Es gibt aber klinische Erfahrungsberichte23 hinsichtlich Handling, Heilungsverlauf und Materialintegration von Hypro-Oss® bei Sinusbodenelevation und Guided Bone Regeneration, die auch in der Langfristbetrachtung positive Ergebnisse zeigten. Offen bleibt, ob nicht eine physiologische Wundheilung nur mittels Makrophagen einer pathologischen Wundheilung unter Mitwirkung Multinukleärer Riesenzellen überlegen ist: zumindest robustere Knochenersatzmaterialien wie z.B. das hier untersuchte Hypro-Oss® scheinen dabei weniger sensibel auf Multinukleäre Riesenzellen zu reagieren.
Purpose: We evaluated efficacy and safety profile of patients with anticoagulation therapy (AT) undergoing holmium laser enucleation of the prostate (HoLEP).
Methods: Within our prospective institutional database (11/2017 to 11/2019), we analyzed functional outcomes and 30-day complication rates of HoLEP patients according to Clavien–Dindo classification (CLD), stratified according to specific AT vs. no AT. Further analyses consisted of uni- and multivariate logistic regression models (LRM) predicting complications.
Results: Of 268 patients undergoing HoLEP, 104 (38.8%) received AT: 25.7% were treated with platelet aggregation inhibitors (PAI), 8.2% with new oral anticoagulants (NOAC) and 4.9% with AT-combinations or coumarins bridged with low molecular weight heparins (LMWH/combination). Patients receiving AT were significantly more comorbid (p < 0.01). Pre- and postoperative maximal flow rates, residual void urine and IPSS at 3 months after surgery were invariably improved after HoLEP for patients with/ without AT. Overall complication rate was 19.5% in patients with no AT vs. 26.1% vs. 27.3 vs. 46.2%, respectively, in patients with PAI, NOAC and LMWH/combination (p < 0.01). Major complications (CLD ≥ 3b) occurred in 6.1% of no AT patients vs. 4.3% vs. 4.5 vs. 0% in patients with PAI, NOAC and LMWH/combination, respectively (p < 0.01). In multivariate LRM, AT was not significantly associated with higher complication rates, whereas high ASA status (OR 2.2, p = 0.04), age (OR 1.04, p = 0.02) and bioptical or incidental prostate cancer (OR 2.5, p = 0.01) represented independent risk factors.
Conclusion: Despite higher overall complication rates in AT patients, major complications were not more frequent in AT patients. HoLEP is safe and effective in anticoagulated patients.
Previous studies reported on the safety and applicability of mesenchymal stem/stromal cells (MSCs) to ameliorate pulmonary inflammation in acute respiratory distress syndrome (ARDS). Thus, multiple clinical trials assessing the potential of MSCs for COVID-19 treatment are underway. Yet, as SARS-inducing coronaviruses infect stem/progenitor cells, it is unclear whether MSCs could be infected by SARS-CoV-2 upon transplantation to COVID-19 patients. We found that MSCs from bone marrow, amniotic fluid, and adipose tissue carry angiotensin-converting enzyme 2 and transmembrane protease serine subtype 2 at low levels on the cell surface under steady-state and inflammatory conditions. We did not observe SARS-CoV-2 infection or replication in MSCs at steady state under inflammatory conditions, or in direct contact with SARS-CoV-2-infected Caco-2 cells. Further, indoleamine 2,3-dioxygenase 1 production in MSCs was not impaired in the presence of SARS-CoV-2. We show that MSCs are resistant to SARS-CoV-2 infection and retain their immunomodulation potential, supporting their potential applicability for COVID-19 treatment.

Abstract
Cellular iron, at the physiological level, is essential to maintain several metabolic pathways, while an excess of free iron may cause oxidative damage and/or provoke cell death. Consequently, iron homeostasis has to be tightly controlled. Under hypoxia these regulatory mechanisms for human macrophages are not well understood. Hypoxic primary human macrophages reduced intracellular free iron and increased ferritin expression, including mitochondrial ferritin (FTMT), to store iron. In parallel, nuclear receptor coactivator 4 (NCOA4), a master regulator of ferritinophagy, decreased and was proven to directly regulate FTMT expression. Reduced NCOA4 expression resulted from a lower rate of hypoxic NCOA4 transcription combined with a micro RNA 6862-5p-dependent degradation of NCOA4 mRNA, the latter being regulated by c-jun N-terminal kinase (JNK). Pharmacological inhibition of JNK under hypoxia increased NCOA4 and prevented FTMT induction. FTMT and ferritin heavy chain (FTH) cooperated to protect macrophages from RSL-3-induced ferroptosis under hypoxia as this form of cell death is linked to iron metabolism. In contrast, in HT1080 fibrosarcome cells, which are sensitive to ferroptosis, NCOA4 and FTMT are not regulated. Our study helps to understand mechanisms of hypoxic FTMT regulation and to link ferritinophagy and macrophage sensitivity to ferroptosis.
Einleitung: Die akute Tonsillitis gehört zu den Infektionen der oberen Atemwege und ist eine sehr häufige Erkrankung in einer Kinder- und Jugendarztpraxis. Ziel unserer Untersuchung war es, das virale und bakterielle Erregerspektrum der akuten Tonsillitis, ihre saisonale Verteilung, ihre Altersverteilung, die klinische Symptomatik und den Einfluss einer Rauchexposition zu untersuchen. Gleichzeitig sollte erneut die Sensitivität und Spezifität des angewandten StrepA ST überprüft werden.
Methoden: In drei Kinder- und Jugendarztpraxen im Rhein-Main Gebiet wurden zwischen April 2009 und Mai 2010 insgesamt 1720 Patienten mit akuten Halsschmerzen untersucht. Mit einem Anamnesebogen wurden Alter, klinische Symptomatik und die Rauchexposition erfasst. Bei allen Patienten wurde ein StrepA ST durchgeführt. In einer Praxis (Praxis 1) wurden bei 306 Patienten zusätzlich ein Abstrich für eine bakterielle Kultur und für eine Multiplex PCR auf Viren durchgeführt.
Resultate: In 84% der Fälle tritt die GAS Tonsillitis im Alter zwischen 2 und 12 Jahren auf. Es konnte keine saisonale Häufung der GAS nachgewiesen werden. Mit 64 (42%) StrepA ST positiven Patienten von 152 Raucher-Familien und 74 (37%) von 200 Nichtraucher-Raucher-Familien zeigt sich kein signifikant erhöhtes Risiko an einer GAS Tonsillitis zu erkranken, wenn mindestens ein Elternteil raucht. Bei 306 Rachenabstrichen konnten 145 (47,5%) mal Streptokokken nachgewiesen werden. Davon waren mit 133 vorwiegend GAS (92%). Die anderen Streptokokken der Gruppen C (4,8%), G (2,1%) und B (1,4%) kommen deutlich seltener vor und spielen eine untergeordnete Rolle. Die Sensitivität und Spezifität des StrepA ST war mit 89,9% und 94,1% ausgezeichnet. Bei 306 Tonsillitiden gelang bei 110 Patienten (35,8%) ein Virusnachweis. Wie erwartet fanden sich doppelt so viele Virusnachweise (46%) bei Patienten ohne GAS Nachweis als Ko-Infektionen bei einer GAS (24%). Der Anteil der Entero-/Rhinoviren unter den nachgewiesenen Viren war mit 54% am höchsten. Adenoviren waren mit 15% und Influenza- 9% die nächst häufigen Viren, Para- und Coronaviren bildeten kleinere Gruppen. Während Entero-/Rhinoviren ganzjährig vorkommen sind Influenzaviren eher in der kalten Jahreszeit für akute Tonsillitiden verantwortlich. Die Rolle der Ko-Infektion in der Entstehung und im Verlauf der akuten Tonsillitis muss in weiteren Untersuchungen erforscht werden.
Background: Breast cancer is the leading cause of cancer-related deaths in women, demanding new treatment options. With the advent of immune checkpoint blockade, immunotherapy emerged as a treatment option. In addition to lymphocytes, tumor-associated macrophages exert a significant, albeit controversial, impact on tumor development. Pro-inflammatory macrophages are thought to hinder, whereas anti-inflammatory macrophages promote tumor growth. However, molecular markers to identify prognostic macrophage populations remain elusive. Methods: We isolated two macrophage subsets, from 48 primary human breast tumors, distinguished by the expression of CD206. Their transcriptomes were analyzed via RNA-Seq, and potential prognostic macrophage markers were validated by PhenOptics in tissue microarrays of patients with invasive breast cancer. Results: Normal human breast tissue contained mainly CD206+ macrophages, while increased relative amounts of CD206− macrophages were observed in tumors. The presence of CD206+ macrophages correlated with a pronounced lymphocyte infiltrate and subsets of CD206+ macrophages, expressing SERPINH1 and collagen 1, or MORC4, were unexpectedly associated with improved survival of breast cancer patients. In contrast, MHCIIhi CD206− macrophages were linked with a poor survival prognosis. Conclusion: Our data highlight the heterogeneity of tumor-infiltrating macrophages and suggest the use of multiple phenotypic markers to predict the impact of macrophage subpopulations on cancer prognosis. We identified novel macrophage markers that correlate with the survival of patients with invasive mammary carcinoma.
The interleukin (IL)-1 family has been described for its numerous involvement in the regulation of inflammatory processes. Certain members are able to induce inflammation, whereas others have the capacity to inhibit inflammation. The newly discovered IL-1 family member IL-38 shows interesting and innovative properties. While most of these cytokines are pro-inflammatory mediators, IL-38 appears to enter the smaller circle of anti-inflammatory mediators. As a pattern, IL-38 appears to suppress IL-17-driven chronic or auto-inflammation by working as receptor antagonist. These properties, as well as its beneficial effects in models of inflammatory and autoimmune diseases suggest the possibility of IL-38-based therapies. Nevertheless, its role in the resolution of acute inflammation, thereby preventing chronic inflammation, remains unclear.
The first part of my thesis elucidated the role of IL-38 in the resolution of inflammation. I found that the complete absence of IL-38 in IL-38 KO mice leads to a delayed resolution of inflammation in the zymosan-induced peritonitis mouse model, compared to WT mice. This was marked by a persistent neutrophilia and a lower production of pro-resolving mediators during the resolution phase, such as TGFβ1 production from macrophages following efferocytosis of apoptotic cells. Reduced TGFβ1 production from macrophages coincided with reduced levels of regulatory T cells (Tregs), which are known to promote the resolution of inflammation. Unexpectedly, the TGFβ1 production capacity of macrophages did not influence the induction of Tregs from naïve T cells. Rather, IL-38 KO mice had an accumulation of Tregs in the thymus compared to WT mice. This was caused by an impairment of CD62L expression at the surface of Tregs, which is required for Tregs migration outside of the thymus. Higher Treg numbers in the thymus correlated with lower level of Tregs in peripheral lymphoid organs. Importantly, CD62L expression at the surface of IL-38 KO Tregs in the thymus was restored by injecting IL-38 i.p. for 24h. These data indicate a potential key function of IL-38 in the regulation of Treg migration, which is triggered in many cases of autoimmunity.
The second part of my thesis was to study the role of IL-38 in experimental autoimmune encephalomyelitis (EAE) development, given that EAE is IL-17-dependent. Unexpectedly, IL-38-deficient mice showed strongly reduced clinical scores and histological markers of EAE. This came with reduced inflammatory cell infiltrates, as well as reduced expression of inflammatory markers in the spinal cord. IL-38 mRNA was detected in the spinal cord, mainly by resident and infiltrated phagocytes, but also by other cells, such as ependymal cells. IL-38 was upregulated upon pro-inflammatory stimulation of bone marrow-derived macrophages, and its presence was necessary for a complete activation of inflammatory macrophages. My data suggest an alternative cell-intrinsic role of IL-38 in macrophages to promote inflammation in the central nervous system.
In the last part of my thesis, I initiated a project on the function of IL-38 in B cell physiology and antibody production, given the fact that IL-38 is expressed by B cells. I generated preliminary data showing that the absence of IL-38 in mice decreased antibody production. Furthermore, I showed that IL-38 is particularly expressed by plasma cells in human tonsils. This project remains open and further studies will be conducted to investigate how IL-38 regulates antibody production, both in physiological and autoimmune settings. Understanding the role of IL-38 in autoantibody production could lead to original and innovative therapy for patients suffering from auto-inflammatory disease.
In summary, the different projects of my thesis provide evidence that the pro-resolving function of IL-38 may be indirectly linked to the retention of Tregs in the thymus. Moreover, a possible intracellular role of IL-38 within macrophages was described showing opposite properties in the regulation of inflammation. This function could be causatively involved in EAE development. However, further studies remain to be done to find the mechanism of action by which IL-38 regulates Tregs egression and how it influences the EAE development. Complete understanding of the IL-38 biology and differentiation between its extra- vs potential intracellular functions could make it a promising therapeutic target for chronic inflammatory or autoimmune diseases.
Bipolar disorder (BD) and major depressive disorder (MDD) are severe mood disorders that belong to the most debilitating diseases worldwide. Differentiating both mood disorders often poses a major clinical challenge, leading to frequent misdiagnoses. Objective biomarkers able to differentiate individuals with BD and MDD therefore represent a psychiatric research field of utmost importance. Recent studies have applied resting-state fMRI paradigms and found promising results differentiating both disorders based on the acquired data. However, most of these studies have focused their efforts on acutely depressed patients. Thus, it remains unclear whether the aberrations remain in a symptomless disease state.
The here presented study addresses these issues by evaluating the ability to differentiate both disorders from one another by conducting a between-group comparison of functional brain network connectivity (FNC) obtained from resting-state fMRI data. Data were collected from 20 BD, 15 MDD patients and 30 age- and gender-matched healthy controls (HC). Graph theoretical analyses were applied to detect differences in functional network organization between the groups on a global and regional network level.
Network analysis detected frontal, temporal and subcortical nodes in emotion regulation areas such as the limbic system and associated regions exhibiting significant differences in network integration and segregation in BD compared to MDD patients and HC. Participants with MDD and HC only differed in frontal and insular network centrality.
These results indicate that a significantly altered brain network topology in the limbic system might be a trait marker specific to BD. Brain network analysis in these regions may therefore be used to differentiate euthymic BD not only from HC but also from patients with MDD.
Despite the success of immune checkpoint blockade in cancer, the number of patients that benefit from this revolutionary treatment option remains low. Therefore, efforts are being undertaken to sensitize tumors for immune checkpoint blockade, which includes combining immune checkpoint blocking agents such as anti-PD-1 antibodies with standard of care treatments. Here we report that a combination of chemotherapy (doxorubicin) and immune checkpoint blockade (anti-PD-1 antibodies) induces superior tumor control compared to chemotherapy and immune checkpoint blockade alone in the murine autochthonous polyoma middle T oncogene-driven (PyMT) mammary tumor model. Using whole transcriptome analysis, we identified a set of genes that were upregulated specifically upon chemoimmunotherapy. This gene signature and, more specifically, a condensed four-gene signature predicted favorable survival of human mammary carcinoma patients in the METABRIC cohort. Moreover, PyMT tumors treated with chemoimmunotherapy contained higher levels of cytotoxic lymphocytes, particularly natural killer cells (NK cells). Gene set enrichment analysis and bead-based ELISA measurements revealed increased IL-27 production and signaling in PyMT tumors upon chemoimmunotherapy. Moreover, IL-27 signaling improved NK cell cytotoxicity against PyMT cells in vitro. Taken together, our data support recent clinical observations indicating a benefit of chemoimmunotherapy compared to monotherapy in breast cancer and suggest potential underlying mechanisms.
Current research on medical biomaterials have shown that the physical and chemical characteristics of biomaterials determine the body inflammatory cellular reaction after their implantation. The aim of this study was to evaluate the individual effects of the physical characteristics over the initial biomaterial-cellular interaction and the inflammatory cellular reaction. For this purpose, an equine-derived collagen hemostatic sponge (E-CHS) was modified by pressing and evaluated using ex vivo, in vitro and in vivo methods.
The E-CHS was pressed by applying constant pressure (6.47± 0.85 N) for 2 min using a sterile stainless-steel cylinder and cut in segments of 1cm2. Subsequently, E-CHS and the pressed equine-derived collagen hemostatic sponge (P-E-CHS) were studied as two independent biomaterials and compared to a control group (CG).
A blood concentrate containing inflammatory cells known as platelet rich fibrin (PRF) was used to mimic the initial biomaterial-cell interaction and to measure the absorption coefficient of the biomaterials to liquid PRF (iPAC). Additionally, the biomaterials were cultivated together with PRF for 3 and 6 days to measure the induction of pro-inflammatory cytokines (TNF-α and IL-8). The results were obtained through enzyme-linked immunosorbent assay (ELISA) and histological methods. PRF cultivated without biomaterials served as the CG. Additionally, the biomaterials were evaluated in vivo using a subcutaneous model in Wistar rats and compared to sham operated animals (CG) representing physiologic wound healing. After 3, 15 and 30 days, the explanted samples were evaluated using histochemical and immunohistochemical (IHC) staining using the following markers: CD68 (pan macrophages), CCR7 (pro-inflammatory macrophages, M1), CD206 (pro-wound healing macrophages, M2) and α-Smooth Muscle Actin (α-SMA; vessel identification).
After the mixture of liquid PRF with both biomaterials for 15 minutes, the ex vivo results showed that E-CHS was penetrated by cells, whereas P-E-CHS was cell-occlusive. Additionally, P-E-CHS induced a higher release of pro-inflammatory cytokines compared to liquid PRF alone (CG) and E-CHS after 3 days (P< 0.05). Although the biomaterial was pressed, the difference of the iPAC value did not show statistical differences. In vivo, the CG induced at day 3 a higher inflammatory response compared to the experimental groups (EG) (P< 0.05). The intergroup comparison showed that P-E-CHS induced a higher presence of macrophages (CD68+/CC7+) compared to E-CHS at day 3 (P< 0.05). Only CD68+/CCR7+ mononuclear cells (MNCs) were observed without multinucleated giant cells (MNGCs). After 15 days, the presence of macrophages (CD68+ P<0.01 /CCR7+ P<0.001 /CD206+ P<0.05) reduced considerably in the CG. On the contrary, the inflammatory response increased in the EGs (CD68+/CCR7+). The intergroup comparison showed that this increment was statistically significant when comparing E-CHS and P-E-CHS to the CG at day 15 (P<0.01 and P< 0.05 respectively). At this time point, a reduced number of MNGCs were observed in the EGs. In the CG no MNGCs were observed. Furthermore, E-CHS showed a faster degradation rate and was fully invaded by cells and vessels formed in its interior region. On the other hand, P-E-CHS remained occlusive to cell penetration and vessels were formed only in the periphery. After 30 days, the cellular reaction shifted to a higher number of M2 macrophages (CD260+) in all groups and a reduced presence of CD68+ and CCR7+ MNCs. Both biomaterials degraded and only small fragments were found in the implantation bed surrounded by MNGCs (CCR7+).
These results are of high clinical relevance and show that changes in biomaterial properties have a significant impact on their interaction with the body. They also serve as insight into the possibility to develop versatile biomaterials with different applications. For example, E-CHs can be applied to support hemostasis in a bleeding alveolar socket and P-E-CHs by being cell occlusive and having a delayed degradation rate can be applied for guided bone and tissue regeneration.
