Hintergrund. Die Achtung der individuellen Autonomie ist eines von vier medizinethischen Prinzipien, das im Kontext von Medizin und Forschung insbesondere in Bezug auf die informierte Einwilligung einer Person thematisiert wird. Menschen mit Demenz können aufgrund innerer oder äußerer Faktoren in ihrer Einwilligungsfähigkeit beeinträchtigt sein, was zu einer Einschränkung ihres Rechts auf Selbstbestimmung führen kann. Im diesbezüglichen Spannungsfeld zwischen Fürsorge und Autonomie soll Entscheidungsassistenz zur Ermöglichung selbstbestimmter Entscheidungen beitragen.
Zielrichtung der Arbeit. Ziel der vorliegenden Dissertation ist die Definition, Implementierung und Evaluation von Entscheidungsassistenzmaßnahmen für Menschen mit Demenz, um deren Autonomie in Entscheidungsprozessen zu unterstützen. Drei Teilprojekte umfassen die Ermittlung des internationalen Forschungsstands zu Entscheidungsassistenz bei Demenz, die Definition und Pilotierung von Unterstützungstools in der Praxis und die Analyse des individuellen Erlebens der vereinfachten Aufklärungsgespräche durch Menschen mit Demenz.
Methode. Im ersten Teilprojekt wurde eine am PRISMA-Standard orientierte systematische Literaturrecherche in Medline und PsycINFO durchgeführt. Die extrahierten relevanten Informationen wurden inhaltlich systematisiert. Aufbauend auf diesen Ergebnissen wurden im zweiten Teilprojekt konkrete Unterstützungstools definiert und in reale Aufklärungsgespräche (Lumbalpunktion) implementiert. Die Tools wurden in der Pilotierung in der Praxis sowie in einem iterativen Diskussionsprozess mit Experten weiterentwickelt. Im dritten Teilprojekt wurde das individuelle Erleben der Teilnehmer der vereinfachten Aufklärungsgespräche mittels problemzentrierter Interviews untersucht und die Daten einer qualitativen Inhaltsanalyse unterzogen.
Ergebnisse. Die Datenbankrecherche ergab initial 2348 Treffer. Nach Screenings der Titel, Abstracts und Volltexte konnten 11 Artikel eingeschlossen werden. Vier der eingeschlossenen Studien sind Interventionsstudien, die übrigen sieben qualitative Interviewstudien. Die identifizierten Unterstützungsmaßnahmen wurden zunächst den beiden Kategorien Interventionen und Strategien und anschließend unter Zuhilfenahme des Konzepts des Contextual Consents fünf komplexitätssteigernden Dimensionen einer Entscheidungssituation zugeordnet (individuelle, soziale, medizinische, informationelle und Folgendimension). Darauf aufbauend wurden im zweiten Teilprojekt acht Entscheidungsassistenzmaßnahmen abgeleitet: (1) Gesprächsstruktur, (2) Elaborierte klare Sprache, (3) Ambiente / Raumgestaltung, (4) Stichwortlisten, (5) Prioritätenkarten, (6) Visualisierung, (7) Vereinfachte schriftliche Einverständniserklärung sowie (8) Personenzentrierte Haltung des Entscheidungsassistenten (1-7: Tools, 8: Grundeinstellung). Die Tools zielen überwiegend auf eine Komplexitätsreduktion in der informationellen Dimension unter Berücksichtigung der fähigkeitsbezogenen und der bedürfnisbezogenen individuellen Dimension ab. Durch Anpassungen der Informationsdarbietung oder der kommunikativen Interaktion im Gespräch dienen sie mehrheitlich der Förderung des (Informations-) Verständnisses. Die Analyse der qualitativen Daten im dritten Teilprojekt zeigt, dass die Erfahrung der vereinfachten Aufklärungsgespräche durch drei übergreifende Themen gekennzeichnet ist. Die Kategorie Formalität versus Informationsgewinn illustriert die individuelle Bedeutung des Aufklärungsgesprächs für die Teilnehmer und deren Bewertung des Prozesses der informierten Einwilligung. Die Kategorie Wahrnehmung der Unterstützung skizziert die Bewertungen der angewandten Unterstützungstools durch die Teilnehmer. Die Kategorie Der Wahrheit ins Auge sehen müssen stellt dar, dass die erlebte Situation des vereinfachten Aufklärungsgesprächs wesentlich durch die Verdachtsdiagnose Demenz bestimmt ist, die im Rahmen aller Aufklärungsgespräche besprochen wurde.
Fazit. Bislang gibt es wenig empirische Forschung zu Entscheidungsassistenz für Menschen mit Demenz und Unterstützungsmaßnahmen werden überwiegend unsystematisch entwickelt und angewendet. Die Wirksamkeit einzelner Unterstützungsmaßnahmen kann aufgrund fehlender Interventionsstudien selten beurteilt werden. Unterstützungsmaßnahmen zielen überwiegend auf eine Komplexitätsreduktion in der Informationsdarbietung und im kommunikativen Interaktionsprozess ab, wobei sie kognitive Beeinträchtigungen und Interaktions-/ Entscheidungsbedürfnisse von Menschen mit Demenz berücksichtigen. Die definierten Tools können als erste konkret handhabbare Werkzeuge verstanden werden, die das strukturierte Leisten von Entscheidungsassistenz für Menschen mit Demenz erleichtern sollen. Sie sind übertragbar auf verschiedene Entscheidungssituationen. Eine Bewertung der Wirksamkeit der definierten Tools sollte in weiteren Entscheidungssituationen und mit größeren Stichproben weiteruntersucht werden. Die Ergebnisse der Evaluation liefern jedoch erste Hinweise darauf, dass einige Teilnehmer sich von einzelnen Tools unterstützt gefühlt haben und die anvisierte Komplexitätsreduktion in der informationellen Dimension in einigen Fällen erfolgreich war. Eine wesentliche Komplexitätssteigerung in der untersuchten Entscheidungssituation entstand durch die negative Emotionen auslösende Vermittlung einer potentiellen Demenzdiagnose (Folgendimension). Dieses Ergebnis impliziert, dass die definierte „verständnisfördernde Toolbox“ um Unterstützungsmaßnahmen zur emotionalen Entlastung von Menschen mit Demenz erweitert werden muss, da davon ausgegangen werden kann, dass vielfältige Entscheidungssituationen für Menschen mit Demenz emotional hoch belastend sind.

This issue of Zeitschrift für Epileptologie is published in his honor with the aim of tracing the work of Dieter Janz over the last five decades and summarizing new findings on the Janz syndrome (Juvenile Myoclonic Epilepsy), which is named after him.
A message from the human placenta: structural and immunomodulatory defense against SARS-CoV-2

The outbreak of the coronavirus disease 2019 (COVID-19) pandemic has caused a global public health crisis. Viral infections may predispose pregnant women to a higher rate of pregnancy complications, including preterm births, miscarriage and stillbirth. Despite reports of neonatal COVID-19, definitive proof of vertical transmission is still lacking. In this review, we summarize studies regarding the potential evidence for transplacental transmission of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), characterize the expression of its receptors and proteases, describe the placental pathology and analyze virus-host interactions at the maternal-fetal interface. We focus on the syncytium, the barrier between mother and fetus, and describe in detail its physical andstructuraldefenseagainstviralinfections. Wefurtherdiscussthepotentialmolecularmechanisms, whereby the placenta serves as a defense front against pathogens by regulating the interferon type III signaling, microRNA-triggered autophagy and the nuclear factor-κB pathway. Based on these data, we conclude that vertical transmission may occur but rare, ascribed to the potent physical barrier, the fine-regulatedplacentalimmunedefenseandmodulationstrategies. Particularly,immunomodulatory mechanismsemployedbytheplacentamaymitigateviolentimmuneresponse,maybesoftencytokine storm tightly associated with severely ill COVID-19 patients, possibly minimizing cell and tissue damages, and potentially reducing SARS-CoV-2 transmission.
In this paper we present a new approach to deterministic modelling of COVID-19 epidemic. Our model dynamics is expressed by a single prognostic variable which satisfies an integro-differential equation. All unknown parameters are described with a single, time-dependent variable R(t). We show that our model has similarities to classic compartmental models, such as SIR, and that the variable R(t) can be interpreted as a generalized effective reproduction number. The advantages of our approach are the simplicity of having only one equation, the numerical stability due to an integral formulation and the reliability since the model is formulated in terms of the most trustable statistical data variable: the number of cumulative diagnosed positive cases of COVID-19. Once this dynamic variable is calculated, other non-dynamic variables, such as the number of heavy cases (hospital beds), the number of intensive-care cases (ICUs) and the fatalities, can be derived from it using a similarly stable, integral approach. The formulation with a single equation allows us to calculate from real data the values of the sample effective reproduction number, which can then be fitted. Extrapolated values of R(t) can be used in the model to make reliable forecasts, though under the assumption that measures for reducing infections are maintained. We have applied our model to more than 15 countries and the ongoing results are available on a web-based platform [1]. In this paper, we focus on the data for two exemplary countries, Italy and Germany, and show that the model is capable of reproducing the course of the epidemic in the past and forecasting its course for a period of four to five weeks with a reasonable numerical stability.
Purpose: Auditory functional MRI (fMRI) often uses silent inter-volume delays for stimulus presentation. However, maintaining the steady-state of the magnetization usually requires constant delays. Here, a novel acquisition scheme dubbed “pre-Saturated EPI using Multiple delays in Steady-state” (SEPIMS) is proposed, using spin saturation at a fixed delay before each volume to maintain steady-state conditions, independent of previous spin history. This concept allows for variable inter-volume delays and thus for flexible stimulus design in auditory fMRI. The purpose was to compare the signal stability of SEPIMS and conventional sparse EPI (CS-EPI). Methods: The saturation module comprises two non-selective adiabatic saturation pulses. The efficiency of the saturation and its effect on the SEPIMS signal stability is tested in vitro and in vivo. Results: Data show that SEPIMS yields the same signal stability as CS-EPI, even for extreme variations between inter-volume delay durations. However, dual saturation pulses are required to achieve sufficiently high saturation efficiency in compartments with long T1 values. Importantly, spoiler gradient pulses after the EPI readout have to be optimized to avoid eddy-current-induced image distortions. Conclusion: The proposed SEPIMS sequence maintains high signal stability in the presence of variable inter-volume durations, thus allowing for flexible stimulus design.
Background: Brodalumab is a fully human monoclonal immunoglobulin IgG2 antibody that binds to the human IL-17 receptor subunit A and by that inhibits the biologic action of IL-17A, IL-17F, IL-17C and IL-17E. Therapy with fumaric acid esters (FAE) is a well established and widely used first-line systemic treatment for subjects with moderate-to-severe plaque psoriasis. Objectives: To compare brodalumab to FAE in terms of clinical efficacy, patient-reported outcomes and safety in subjects with moderate-to-severe plaque psoriasis who were naïve to systemic treatment. Methods: Eligible subjects were randomized 1 : 1 to 210 mg brodalumab injections or oral FAE according to product label in this 24-week, open-label, assessor-blinded, multi-centre, head-to-head phase 4 trial. The primary endpoints were having PASI75 and having sPGA score of 0 or 1 (sPGA 0/1). Subjects with missing values for the primary endpoints were considered non-responders. Results: A total of 210 subjects were randomized. 91/105 subjects completed brodalumab treatment and 58/105 subjects completed FAE treatment. At Week 24, significantly more subjects in the brodalumab group compared to the FAE group had PASI75 (81.0% vs. 38.1%, P < 0.001) and sPGA 0/1 (64.8% vs. 20.0%, P < 0.001). In the brodalumab group, the median time to both PASI75 and to PASI90 was significantly shorter than in the FAE group (4.1 weeks vs. 16.4 weeks, and 7.4 weeks vs. 24.4 weeks, respectively, P < 0.0001 for both). The rate of adverse events was lower in subjects treated with brodalumab compared to subjects treated with FAE (616.4 vs. 1195.8 events per 100 exposure years). No new safety signals were detected for brodalumab. Conclusions: Brodalumab was associated with rapid and significant improvements in signs and symptoms of moderate-to-severe plaque psoriasis, with a superior efficacy profile to what was observed with FAE in systemic-naïve subjects over 24 weeks.
Background: Combined inhibition of phosphatidylinositol 3-kinase (PI3K) and the mammalian target of rapamycin (mTOR) complexes may be an efficient treatment for acute leukemia. The primary objective of this phase I single center open label study was to determine the maximum tolerated dose (MTD) and recommended phase II dose (RP2D) of the dual pan-class I PI3K and mTOR inhibitor BEZ235 in patients with advanced leukemia.
Methods: Herein patients > 18 years of age who had relapsed or showed refractory leukemia were treated with BEZ235 (orally at 300–400 mg BID (cohort − 1/1)) to assess safety, tolerability, preliminary efficacy and pharmacokinetic (PK). Adverse events data and serious adverse events were analyzed and haematological and clinical biochemistry toxicities were assessed from laboratory test parameters. Response was assessed for the first time at the end of cycle 1 (day 29) and after every subsequent cycle. Pharmacokinetic and pharmacodynamic analyses of BEZ235 were also included (BEZ235 plasma levels, phosphorylation of AKT, S6 and 4EBP1). On statistics this trial is a multiple ascending dose study in which a following variant of the 3 + 3 rule (“Rolling Six”), a minimum of 6 and a maximum of 12 patients was recruited for the dose escalation and another 5 were planned for the expansion phase.
Results: Twenty-four patients with ALL (n = 11) or AML (n = 12) or CML-BP (n = 1) were enrolled. All patients had failed one (n = 5) or more lines of therapy (n = 5) and 14 patients were in refractory / refractory relapse. No formal MTD was defined, stomatitis and gastrointestinal toxicity at 400 mg BID dose was considered incompatible with prolonged treatment. The RP2D of BEZ235 was defined as 300 mg BID. Four of 24 patients showed clinical benefit. Twenty-two of 24 patients discontinued because of progression, (median time to progression 27 days (4d-112d). There was no association between PK parameters and efficacy or tolerability.
Conclusions: Combined inhibition of PI3K and mTOR inhibits a clinically meaningful driver pathway in a small subset of patients with ALL, with no benefit in patients with AML.

Emerging evidence suggests a complex relationship between sphingosine 1-phosphate (S1P) signaling and stroke. Here, we show the kinetics of S1P in the acute phase of ischemic stroke and highlight accompanying changes in immune cells and S1P receptors (S1PR). Using a C57BL/6 mouse model of middle cerebral artery occlusion (MCAO), we assessed S1P concentrations in the brain, plasma, and spleen. We found a steep S1P gradient from the spleen towards the brain. Results obtained by qPCR suggested that cells expressing the S1PR type 1 (S1P1+) were the predominant population deserting the spleen. Here, we report the cerebral recruitment of T helper (TH) and regulatory T (TREG) cells to the ipsilateral hemisphere, which was associated with differential regulation of cerebral S1PR expression patterns in the brain after MCAO. This study provides insight that the S1P-S1PR axis facilitates splenic T cell egress and is linked to the cerebral recruitment of S1PR+ TH and TREG cells. Further insights by which means the S1P-S1PR-axis orchestrates neuronal positioning may offer new therapeutic perspectives after ischemic stroke.
Purpose: The diagnosis of abusive head trauma (AHT) is complex and neuroimaging plays a crucial role. Our goal was to determine whether non-neuroradiologists with standard neuroradiology knowledge perform as well as neuroradiologists with experience in pediatric neuroimaging in interpreting MRI in cases of presumptive AHT (pAHT).
Methods: Twenty children were retrospectively evaluated. Patients had been diagnosed with pAHT (6 patients), non-abusive head trauma-NAHT (5 patients), metabolic diseases (3 patients), and benign enlargement of the subarachnoid spaces (BESS) (6 patients). The MRI was assessed blindly, i.e., no clinical history was given to the 3 non-neuroradiologists and 3 neuroradiologists from 2 different institutions.
Results: Blindly, neuroradiologists demonstrated higher levels of sensitivity and positive predictive value in the diagnosis of pAHT (89%) than non-neuroradiologists (50%). Neuroradiologists chose correctly pAHT as the most probable diagnosis 16 out of 18 times; in contrast, non-neuroradiologists only chose 9 out of 18 times. In our series, the foremost important misdiagnosis for pAHT was NAHT (neuroradiologists twice and non-neuroradiologists 5 times). Only victims of motor vehicle accidents were blindly misdiagnosed as pAHT. No usual household NAHT was not misdiagnosed as pAHT. Neuroradiologists correctly ruled out pAHT in all cases of metabolic diseases and BESS.
Conclusion: MRI in cases of suspected AHT should be evaluated by neuroradiologists with experience in pediatric neuroimaging. Neuroradiologists looked beyond the subdural hemorrhage (SDH) and were more precise in the assessment of pAHT and its differential diagnosis than non-neuroradiologists were. It seems that non-neuroradiologists mainly assess whether or not a pAHT is present depending on the presence or absence of SDH.
Achieving functional neuronal dendrite structure through sequential stochastic growth and retraction

Class I ventral posterior dendritic arborisation (c1vpda) proprioceptive sensory neurons respond to contractions in the Drosophila larval body wall during crawling. Their dendritic branches run along the direction of contraction, possibly a functional requirement to maximise membrane curvature during crawling contractions. Although the molecular machinery of dendritic patterning in c1vpda has been extensively studied, the process leading to the precise elaboration of their comb-like shapes remains elusive. Here, to link dendrite shape with its proprioceptive role, we performed long-term, non-invasive, in vivo time-lapse imaging of c1vpda embryonic and larval morphogenesis to reveal a sequence of differentiation stages. We combined computer models and dendritic branch dynamics tracking to propose that distinct sequential phases of stochastic growth and retraction achieve efficient dendritic trees both in terms of wire and function. Our study shows how dendrite growth balances structure–function requirements, shedding new light on general principles of self-organisation in functionally specialised dendrites.

Background: The epidermal growth factor receptor (EGFR) signaling pathway is genetically activated in approximately 50% of glioblastomas (GBs). Its inhibition has been explored clinically but produced disappointing results, potentially due to metabolic effects that protect GB cells against nutrient deprivation and hypoxia. Here, we hypothesized that EGFR activation could disable metabolic adaptation and define a GB cell population sensitive to starvation.
Methods: Using genetically engineered GB cells to model different types of EGFR activation, we analyzed changes in metabolism and cell survival under conditions of the tumor microenvironment.
Results: We found that expression of mutant EGFRvIII as well as EGF stimulation of EGFR-overexpressing cells impaired physiological adaptation to starvation and rendered cells sensitive to hypoxia-induced cell death. This was preceded by adenosine triphosphate (ATP) depletion and an increase in glycolysis. Furthermore, EGFRvIII mutant cells had higher levels of mitochondrial superoxides potentially due to decreased metabolic flux into the serine synthesis pathway which was associated with a decrease in the NADPH/NADP+ ratio.
Conclusions: The finding that EGFR activation renders GB cells susceptible to starvation could help to identify a subgroup of patients more likely to benefit from starvation-inducing therapies.
Purpose: Acute-on-chronic subdural hematoma (acSDH) describes acute bleeding into a chronic subdural hematoma (SDH), after surgery or second trauma. Because seizures are a well-known complication of SDH, associated with substantial morbidity and mortality, we aimed to analyze the incidence of acute symptomatic seizures (ASz), including status epilepticus, and determine the functional outcomes in this specific cohort of patients.
Methods: A retrospective analysis was performed, including patients with acSDH who were admitted to our department between 2010 and 2019. The incidence and timely onset of ASz and status epilepticus were evaluated. Functional outcomes at discharge and at 3–6 month follow-up were analyzed based on the modified Rankin scale.
Results: Of 506 patients with chronic SDH, 29 patients (5.7%) were diagnosed with acSDH. The overall incidence of ASz and status epilepticus were 72.4% and 10.3%, respectively. Favorable outcomes were identified in 11 patients (52.4%) in the ASz group compared with 6 patients (75%) in the non-ASz group. The mortality rate was higher in the ASz group compared with that in the control group (29% vs 0%). At follow-up, favorable outcomes were similar to those observed at discharge (52.4% in the ASz group and 71.4% in the control group). The mortality rate was still higher in the ASz group, at 32% compared with 14% for the control group.
Conclusion: AcSDH has a high risk for ASz, including status epilepticus, and is associated with unfavorable outcomes and high mortality. Thus, prophylactic treatment with antiepileptic drugs should be considered among this specific cohort of patients.
Background: The purpose of this pilot study was to create a valid and reliable set of assessment questions for examining Evidence-based Dentistry (EbD) knowledge. For this reason, we adapted and validated for dental students the Berlin Questionnaire (BQ), which assesses Evidence-based Medicine (EbM) abilities.
Methods: The Berlin Questionnaire was validated with medical residents. We adapted it for use in a dentistry setting. An expert panel reviewed the adapted BQ for content validity. A cross-sectional cohort representing four training levels (EbD-novice dental students, EbD-trained dental students, dentists, and EbM−/EbD-expert faculty) completed the questionnaire. A total of 140 participants comprised the validation set. Internal reliability, item difficulty and item discrimination were assessed. Construct validity was assessed by comparing the mean total scores of students to faculty and comparing proportions of students and faculty who passed each item.
Results: Among the 133 participants (52 EbD-novice dental students, 53 EbD-trained dental students, 12 dentists, and 16 EbM-/ EbD-expert faculty), a statistically significant (p < 0.001) difference was evident in the total score corresponding to the training level. The total score reliability and psychometric properties of items modified for discipline-specific content were acceptable. Cronbach’s alpha was 0.648.
Conclusion: The adapted Berlin Questionnaire is a reliable and valid instrument to assess competence in Evidence-based Dentistry in dental students. Future research will focus on refining the instrument further.
Background: Unwanted anticholinergic effects are both underestimated and frequently overlooked. Failure to identify adverse drug reactions (ADRs) can lead to prescribing cascades and the unnecessary use of over-thecounter products. The objective of this systematic review and meta-analysis is to explore and quantify the frequency and severity of ADRs associated with amitriptyline vs. placebo in randomized controlled trials (RCTs) involving adults with any indication, as well as healthy individuals. Methods: A systematic search in six electronic databases, forward/backward searches, manual searches, and searches for Food and Drug Administration (FDA) and European Medicines Agency (EMA) approval studies, will be performed. Placebo-controlled RCTs evaluating amitriptyline in any dosage, regardless of indication and without restrictions on the time and language of publication, will be included, as will healthy individuals. Studies of topical amitriptyline, combination therapies, or including <100 participants, will be excluded. Two investigators will screen the studies independently, assess methodological quality, and extract data on design, population, intervention, and outcomes ((non-)anticholinergic ADRs, e.g., symptoms, test results, and adverse drug events (ADEs) such as falls). The primary outcome will be the frequency of anticholinergic ADRs as a binary outcome (absolute number of patients with/without anticholinergic ADRs) in amitriptyline vs. placebo groups. Anticholinergic ADRs will be defined by an experienced clinical pharmacologist, based on literature and data from Martindale: The Complete Drug Reference. Secondary outcomes will be frequency and severity of (non-)anticholinergic ADRs and ADEs. The information will be synthesized in meta-analyses and narratives. We intend to assess heterogeneity using metaregression (for indication, outcome, and time points) and I2 statistics. Binary outcomes will be expressed as odds ratios, and continuous outcomes as standardized mean differences. Effect measures will be provided using 95% confidence intervals. We plan sensitivity analyses to assess methodological quality, outcome reporting etc., and subgroup analyses on age, dosage, and duration of treatment. Discussion: We will quantify the frequency of anticholinergic and other ADRs/ADEs in adults taking amitriptyline for any indication by comparing rates for amitriptyline vs. placebo, hence, preventing bias from disease symptoms and nocebo effects. As no standardized instrument exists to measure it, our overall estimate of anticholinergic ADRs may have limitations.
Determination of a minimal postmortem interval via age estimation of necrophagous diptera has been restricted to the juvenile stages and the time until emergence of the adult fly, i.e. up until 2–6 weeks depending on species and temperature. Age estimation of adult flies could extend this period by adding the age of the fly to the time needed for complete development. In this context pteridines are promising metabolites, as they accumulate in the eyes of flies with increasing age. We studied adults of the blow fly Lucilia sericata at constant temperatures of 16 °C and 25 °C up to an age of 25 days and estimated their pteridine levels by fluorescence spectroscopy. Age was given in accumulated degree days (ADD) across temperatures. Additionally, a mock case was set up to test the applicability of the method. Pteridine increases logarithmically with increasing ADD, but after 70–80 ADD the increase slows down and the curve approaches a maximum. Sex had a significant impact (p < 4.09 × 10−6) on pteridine fluorescence level, while body-size and head-width did not. The mock case demonstrated that a slight overestimation of the real age (in ADD) only occurred in two out of 30 samples. Age determination of L. sericata on the basis of pteridine levels seems to be limited to an age of about 70 ADD, but depending on the ambient temperature this could cover an extra amount of time of about 5–7 days after completion of the metamorphosis.
Einleitung: Die Obduktion nimmt einen wichtigen Stellenwert in der Medizin ein, da sie nicht nur der Klärung der Todesart und -ursache eines Verstorbenen dient, sondern auch zum Verständnis der Pathophysiologie von Erkrankungen beiträgt. In diesem zweiten Teil der Studie wurden aktuelle Normwerte für das Gewicht für die folgenden adulten Organe entwickelt: Leber, Lunge, Milz, Nieren. Zudem wurden Zusammenhänge zwischen Organgewichten und der Todesart untersucht. Material und Methoden: Die im Dreijahreszeitraum von 2011 bis 2013 im Institut für Rechtsmedizin in Frankfurt am Main durchgeführten Obduktionen wurden retrospektiv ausgewertet. Die statistischen Berechnungen erfolgten mithilfe des Programmes „BiAS. für Windows“ (epsilon-Verlag GbR, Hochheim-Darmstadt, Deutschland). Ergebnisse: Folgende Normwerte bzw. -bereiche wurden an der Studienpopulation erhoben: Leber 1047,0–2740,0 g (♂, n = 191) bzw. 749,0–2182,0 g (♀, n = 115), linke Lunge 230,0–840,0 g (♂, n = 119) bzw. 186,8–891,3 g (♀, n = 97), rechte Lunge 249,3–1005,8 g (♂, n = 116) bzw. 215,3–907,5 g (♀, n = 100), Milz 55,0–373,2 g (♂, n = 306) bzw. 50,0–355,0 g (♀, n = 204), linke Niere 110,0–255,0 g (♂, n = 258) bzw. 71,8–215,0 g (♀, n = 137), rechte Niere 100,0–270,0 g (♂, n = 266) bzw. 75,0–212,1 g (♀, n = 140). Für die am stärksten mit Organgewichten korrelierenden Körpermaße, nämlich Body-Mass-Index (BMI), Körperoberfläche („body surface area“, BSA) und Körpergewicht, wurden nach Subgruppen getrennte Normwerte ermittelt. Ein signifikanter Unterschied des Organgewichtes je nach Todesart lag bei Männern bei der Milz und bei den Nieren vor. Bei Frauen war bei keinem der Organe ein von der Todesart abhängiger signifikanter Gewichtsunterschied feststellbar. Außerdem wurden Organindizes entwickelt, mittels derer der Anwender berechnen kann, ob ein Organgewicht, Körpermaßen bzw. Alter entsprechend, im Normbereich liegt. Diskussion: Organgewichte unterliegen wie Körpermaße einem säkularen Trend, welcher jedoch nicht linear und für jedes Organ individuell verläuft. Für die Auswertung von Organgewichten im Rahmen der Obduktion werden deshalb aktuelle, an einer vergleichbaren Population erhobene Normtabellen benötigt. Bei deren Erstellung können sowohl Fälle mit natürlichem als auch mit nichtnatürlichem Tod unter weitestgehendem Ausschluss pathologisch veränderter Organe herangezogen werden.
In Deutschland erkranken pro Jahr bis zu 12.000 Menschen neu an Leukämie. Leukämie ist eine schwere onkologische Erkrankung, bei der reifes Knochenmarkgewebe in Folge von Mutationen unreifer und defekter Vorläuferzellen (leukämischen Blasten) verdrängt wird. Dies führt zu einer zunehmend eingeschränkten Blutbildung. Akute Leukämieformen können unbehandelt innerhalb von wenigen Wochen zum Tode führen und erfordern deshalb eine umgehende Diagnostik sowie einen raschen Therapiebeginn. Heilungschancen bestehen dann, wenn durch die Transplantation von gesunden hämatopoetischen Stammzellen (HSZT) das erkrankte Knochenmark ausreichend ersetzt wird. Leider sind Abstoßungsreaktionen des Spendermaterials (engl.: Graft-versus-Host-Disease, GvHD) keine Seltenheit.
Natürliche Killerzellen (NK-Zellen) stellen die kleinste Lymphozytenpopulation im menschlichen Blut dar und werden dem angeborenen Immunsystem zugerechnet. Sie wurden erstmals 1975 durch die Forscher Kiessling, Klein et al. entdeckt.17 Aufgrund ihrer Fähigkeit bestimmte Tumorzellen in vitro zu töten, wächst das Interesse an der Erforschung ihrer aktivierenden und inhibierenden Oberflächenrezeptoren. Die Killer-Zell-Immunoglobulin-ähnlichen Rezeptoren (KIRs) bilden dabei eine besonders diverse NKZell-Rezeptorfamilie. Lokalisiert auf Chromosom 19 liegen bis zu 17 hochpolymorphe KIRGene. Die genetische Ausstattung und Oberflächenexpression variiert von Individuum zu Individuum und bildet die Voraussetzung für die vorhandene Diversität KIR-exprimierender NK-Zellen. NK-Zellen besitzen die Fähigkeit, Gewebezellen in „körpereigen“ oder „fremd“ zu kategorisieren. Inhibitorische Killer-Immunoglobulin-ähnliche Rezeptoren (iKIR) nutzen dazu HLA-Klasse-I-Proteine (MHC-I) auf der Oberfläche gesunder Zellen. Diese schützen sie vor einem zytotoxischen NK-Zell-Angriff. NK-Zellen durchlaufen im Vorfeld einen komplexen Ausbildungssprozess48, an dessen Ende lizensierte Effektorzellen stehen. Diese können mittels gezielter Zytolyse krebstransformierte, zellulär-gestresste, sowie viralinfizierte Zellen im intakten Organismus erkennen und abtöten.
Die Spenderauswahl ist ein wichtiger Faktor für den Erfolg einer Stammzelltransplantation. Infundierte Spender NK-Zellen schützen das Transplantat, indem sie als wirksame Effektorzellen verbleibende Leukämiezellen aktiv eliminieren. Diese wünschenswerte Nebenwirkung wird als Graft-versus-Leukämie (GvL)-Effekt bezeichnet. Ruggeri et al. konnte zeigen, dass insbesondere Transplantationsstrategien, die auf KIR-Ligand-Fehlpaarungen (engl.: KIR-HLA-mismatch) basieren, zu weniger Rückfällen, weniger GvHD und einem besseren Gesamtüberleben bei Patienten mit akuter myeloischer Leukämie (AML) nach HSZT führt. Der KIR-HLA-mismatch wird mittlerweile aufgrund ausreichender Datenlage bei der Auswahl passender NK-Zell-Spender berücksichtigt und die Untersuchung auf die An- bzw. Abwesenheit bestimmter KIR-Gene (Haplotypisierung)
mittlerweile neben der HLA-Typisierung standardisiert in vielen Instituten durchgeführt. Daneben finden sich immer mehr Hinweise dafür, dass bereits einzelne allelische Polymorphismen innerhalb der KIR-Gene einzelner Spender großen Einfluss auf die Funktionalität ihrer NK-Zellen nehmen. Die allelische Subtypisierung von KIRs stellt aufgrund stetig steigender Zahlen neu entdeckter Allele eine Herausforderung dar. Im Januar 2019 sind für KIR2DL1 bereits 66 Allele beschrieben und für KIR3DL1 sogar 150 Allele in der Immuno Polymorphism Database (IPD) hinterlegt.
Die vorliegende Arbeit präsentiert ein praktikables Subtypisierungsverfahren, um allelische Unterschiede innerhalb der Genloci der NK-Zell-Rezeptoren KIR2DL1 und KIR3DL1 zu untersuchen. Für die Experimente wurden NK-Zellen von 20 gesunden Spendern funktionell untersucht und KIR-genetisch analysiert. Ziel war es innerhalb dieser Individuen besonders potente NK-Zellspender zu identifizieren und diese anhand bestimmter Polymorphismen und/ oder der Expression von KIR-Rezeptoren zu charakterisieren. Bei der Subtypisierung der KIR2DL1-Gene konnten 12 verschiedene, bereits bekannte KIR2DL1-Allele bestimmt werden. Die häufigsten Allele waren dabei 2DL1*001, *00201, *00302, *00401 und *00403. 5 der 20 Spender konnten der funktionell hochpotenten R245–Allelgruppe (AS Arginin an Pos. 245) zugeordnet werden. Spender 13 zeigte bei negativer KIR2DL1-SSP eine vermeintlich neue Nullallelvariante, Spender 20 eine neue heterozygote Variante, resultierend in der Kombination eines Arginin mit Alanin (R/A). Bei der allelischer Subtypisierung von KIR3DL1 wurden 25 verschiedene, bereits bekannte KIR3DL1-Allele bei den 20 Spendern bestimmt. Spender 2 zeigt zahlreiche, vorwiegend homozygote Abweichungen von der Referenzfrequenz, insbesondere im Exon 5, und wurde als neue Allelvariante gewertet. Die häufigsten KIR3DL1-Allele waren 3DL1*00101, *002 und *087. Mittels durchflusszytometrischer Messung konnte gezeigt werden, dass das bekannte Nullallel 3DL1*0040101 bei Spender 14 zu keiner Oberflächenexpression des Rezeptors führt215, während Spendern 11 und 16 als Träger des 3DL1*00402 Allels eine Oberflächenexpression von rund 10% präsentierten. Um die Spender NK-Zellen der gebildeten Gruppen funktionell zu testen, wurden die NK-Zellen experimentell mit vier unterschiedlichen transgenen L721.221-Zelllinien stimuliert. Die funktionelle Potenz der
gespendeten NK-Zellen wurde mittels eines CD107-Degranulationsassays gemessen. Nach aktuellem Stand sind nur für 53 der 150 KIR3DL1-Allele die allelischen Expressionsmuster untersucht worden. Dies bedeutet im Umkehrschluss, dass für rund 65% der bekannten KIR3DL1-Allele Daten zur Funktionalität fehlen, und damit der größte Anteil der ermittelten Allele von 6 Spendern der unknown-Expression (KIR3DL1u/u) Gruppe zugeordnet wurde. Die Spender 4 und 19 der high-Expressiongruppe (KIR3DL1h/h), sowie Spender 5 und 10 der KIR3DL1u/u-Gruppe mit den Allelen 3DL1*053, *087, *109 und Spender 2 als Träger zweier neuer KIR3DL1-Allele, zeigten in toto die besten funktionellen Ergebnisse in den Experimenten gegen die verwendete B-lymphoblastoide L721.221-Zellinien. Die Ergebnisse der vorliegenden Arbeit zeigen, dass bei der Spenderauswahl für NK-Zellbasierten Immuntherapie neben der Genotypisierung die allelische KIR-Subtypisierung als wertvolles Werkzeug entschiedener berücksichtigt werden sollte. Dafür ist es jedoch notwendig weiter an KIR-Subtypisierung und -Gruppierung Strategien zu arbeiten, um Natürlichen Killerzellen Wege in die klinische Standardpraxis zu bahnen.
Alcoholism is one of the leading and increasingly prevalent reasons of liver associated morbidity and mortality worldwide. Alcoholic hepatitis (AH) constitutes a severe disease with currently no satisfying treatment options. Lipoxin A4 (LXA4), a 15-lipoxygenase (ALOX15)-dependent lipid mediator involved in resolution of inflammation, showed promising pre-clinical results in the therapy of several inflammatory diseases. Since inflammation is a main driver of disease progression in alcoholic hepatitis, we investigated the impact of endogenous ALOX15-dependent lipid mediators and exogenously applied LXA4 on AH development. A mouse model for alcoholic steatohepatitis (NIAAA model) was tested in Alox12/15+/+ and Alox12/15−/− mice, with or without supplementation of LXA4. Absence of Alox12/15 aggravated parameters of liver disease, increased hepatic immune cell infiltration in AH, and elevated systemic neutrophils as a marker for systemic inflammation. Interestingly, i.p. injections of LXA4 significantly lowered transaminase levels only in Alox12/15−/− mice and reduced hepatic immune cell infiltration as well as systemic inflammatory cytokine expression in both genotypes, even though steatosis progressed. Thus, while LXA4 injection attenuated selected parameters of disease progression in Alox12/15−/− mice, its beneficial impact on immunity was also apparent in Alox12/15+/+ mice. In conclusion, pro-resolving lipid mediators may be beneficial to reduce inflammation in alcoholic hepatitis.
Ataxia telangiectasia (A-T) is a devastating multi-system disorder characterized by progressive cerebellar ataxia and immunodeficiency. The neurological decline may be caused by multiple factors of which ongoing inflammation and oxidative stress may play a dominant role. The objective of the present investigation was to determine cerebrospinal fluid (CSF) proteins and possible low-grade inflammation and its relation to age and neurological deterioration. In the present study, we investigated 15 patients with A-T from 2 to 16 years. Our investigation included blood and CSF tests, clinical neurological examination, A-T score, and MRI findings. The albumin ratio (AR) was analyzed to determine the blood–brain-barrier function. In addition, inflammatory cytokines (IL-1α, IL-6, IL-8, IL-12 p40, IL-17A, IFN-γ, TNF-α) were measured by the multiplex cytometric bead array. We compared the results with those from an age-matched control group. Three of the A-T patients were analyzed separately (one after resection of a cerebral meningioma, one after radiation and chemotherapy due to leukemia, one after stem cell transplantation). Patient had significantly more moderate and severe side effects due to CSF puncture (vomiting, headache, need for anti-emetic drugs) compared with healthy controls. Total protein, albumin, and the AR increased with age indicating a disturbed blood barrier function in older children. There were no differences for cytokines in serum and CSF with the exception of IL-2, which was significantly higher in controls in serum. The AR is significantly altered in A-T patients, but low-grade inflammation is not detectable in serum and CSF.
Three AKT serine/threonine kinase isoforms (AKT1/AKT2/AKT3) mediate proliferation, metabolism, differentiation and anti-apoptotic signals. AKT isoforms are activated down- stream of PI3-kinase and also by PI3-kinase independent mechanisms. Mutations in the lipid phosphatase PTEN and PI3-kinase that increase PIP3 levels increase AKT signaling in a large proportion of human cancers. AKT and other AGC kinases possess a regulatory mechanism that relies on a conserved hydrophobic motif (HM) C-terminal to the catalytic core. In AKT, the HM is contiguous to the serine 473 and two other newly discovered (serine 477 and tyrosine 479) regulatory phosphorylation sites. In AKT genes, this regulatory HM region is encoded in the final exon. We identified a splice variant of AKT2 (AKT2-13a), which contains an alternative final exon and lacks the HM regulatory site. We validated the presence of mRNA for this AKT2-13a splice variant in different tissues, and the presence of AKT2-13a protein in extracts from HEK293 cells. When overexpressed in HEK293 cells, AKT2-13a is phosphorylated at the activation loop and at the zipper/turn motif phosphoryla- tion sites but has reduced specific activity. Analysis of the human transcriptome correspond- ing to other AGC kinases revealed that all three AKT isoforms express alternative transcripts lacking the HM regulatory motif, which was not the case for SGK1-3, S6K1-2, and classical, novel and atypical PKC isoforms. The transcripts of splice variants of Akt1-3 excluding the HM regulatory region could lead to expression of deregulated forms of AKT.
Onkologische Erkrankungen im Kindesalter und jungen Erwachsenenalter haben nicht selten eine gute Prognose. Entsprechend wird für Betroffene früher oder später die Frage relevant, inwieweit nach einer onkologischen Behandlung die Fertilität beeinträchtigt ist. Nicht nur der Zeitraum der Fertilität, sondern auch die Wahrscheinlichkeit eines vorzeitigen Ovarialversagens mit allen Risiken eines längerfristigen Östrogenmangels ist für die Lebensplanung der Frauen wichtig. Mittlerweile können vor Behandlung fertilitätserhaltende Maßnahmen angeboten werden. Sie bieten manchmal die einzige Chance, auf ovarielle Reserven nach Behandlung zurückgreifen zu können, sind aber nicht immer nötig und von späterem Nutzen. Das Anti-Müller-Hormon (AMH) hat sich als validester Marker für die Beurteilung der ovariellen Reserve herausgestellt. Mithilfe dessen sind Prognosen über die Ovarreserve vor und nach der onkologischen Therapie möglich. Dies erleichtert die Entscheidung für die Indikation für fertilitätserhaltende Maßnahmen und kann wegweisend in der Lebensplanung der Frauen und Familien sein.
Analyse der Genauigkeit des neurochirurgischen Operationsroboters Robotic Surgery Assistant (ROSA)

In der vorliegenden Arbeit sollte untersucht werden, ob der Roboter ROSA bei der Durchführung von intrakraniellen Biopsien oder Elektrodenimplantationen eine Alternative zur klassischen, rahmenbasierten Stereotaxie darstellt. Dazu sollte die mechanische und die Anwendungsgenauigkeit des Systems ermittelt werden. Zur Bestimmung der mechanischen Genauigkeit wurde eine experimentelle Phantomstudie durchgeführt. Hier wurden durch den Roboter wiederholt zehn Trajektorien an einem Stereotaxiephantom angefahren. Der Abstand der robotischen Nadel zum Zielpunkt im Phantom wurde anhand von Röntgenbildern bestimmt. Die Wiederholung des Versuchsaufbaus unter Variation der Planungsbildgebung erlaubte den Vergleich verschiedener Schichtdicken sowie zwischen low-dose und normal-dose Verfahren. Die Anwendungsgenauigkeit sollte durch die Analyse operativer Ergebnisse der ROSA erfasst werden. Dazu wurde anhand von postoperativen Bildern die Genauigkeit anhand des Abstands zwischen geplanter und tatsächlicher Lage von Stereoelektroenzephalographie-Elektroden ermittelt. Es wurden verschiedene Referenzierungstechniken, die der Orientierung des Roboters dienen und bei denen eine präoperative Planungsbildgebung (CT oder MRT) mit einem Abbild des OP-Gebietes (durch Oberflächenerkennung oder durch einen Stereotaxierahmen) referenziert wird, verglichen, nämlich CT-Laser; CT-Leksell-Rahmen und MRT-Laser. Die Ergebnisse wurden einer statistischen Analyse unterzogen. Dabei zeigte sich, dass der ROSA-Roboter eine sehr hohe mechanische Genauigkeit im Submillimeterbereich erreicht. Genauigkeitseinbußen bei einer größeren Schichtdicke der zur Planung verwendeten Computertomographie sind messbar, aber gering. Ein signifikanter Einfluss bei der Verwendung eines low-dose-Protokolls konnte nicht festgestellt werden. Dennoch zeigte sich, dass der entscheidende Teil der Ungenauigkeiten in der klinischen Anwendung entsteht und dabei insbesondere durch die Referenzierungstechnik bestimmt wird. Referenzierungen, die auf einer Computertomographie basierten, erwiesen sich als zufriedenstellend genau und als konkurrenzfähig zur konventionellen Methode. Der Unterschied zwischen dem rahmenbasierten und dem auf Oberflächenerkennung basierenden Verfahren war dabei so gering, dass letzteres sich angesichts seiner Vorteile in der Anwendung als besonders günstiges Verfahren hervortut. Im Gegensatz dazu stand das MRT-Laser-Verfahren, welches bei relativ hohen Abweichungen nur eingeschränkt anwendbar scheint und sich damit eher für Anwendungsbereiche mit geringeren Genauigkeitsanforderungen eignet, wie bspw. Biopsien. Weiterhin kann der Verlauf der Trajektorie an den höheren Sicherheitsabstand angepassten werden. Bei der Einordnung der ermittelten Genauigkeiten ist zu beachten, dass es viele weitere, von der Referenzierungs- und Bildgebungsmethode unabhängige Einflussfaktoren gibt. In dieser Arbeit war der Einfluss der erfassten externen Paramter zwar limitiert, bei anderen Autoren zeigte sich jedoch ein signifikanter Effekt. Dennoch deckt sich die Gesamtgenauigkeit mit den Ergebnissen anderer Arbeiten.
In Zusammenschau der Ergebnisse weist die vom ROSA-Assistenzsystem assistierte Stereotaxie eine verbesserte Prozessqualität auf, unter anderem durch die erhebliche Zeitersparnis, ggf. der Wegfall des Transports des narkotisierten Patienten, die Adaptionsmöglichkeiten der Prozessteilschritte an den Patienten, sowie eine hohe Nutzerfreundlichkeit. Entscheidend ist jedoch, dass es sich um ein sehr sicheres Verfahren handelt: Durch die hohe Genauigkeit wird das Operationsrisiko minimiert, gleichzeitig erlauben Laser-gestützte Registrierungsverfahren eine Reduktion der Strahlenexposition. Zur Konsolidierung der in dieser Arbeit gewonnenen Erkenntnisse sind weitere klinische Daten notwendig.
Ziel dieser klinischen Studie war es, die Ergebnisse der operativen Behandlung eines idiopathischen Makulaforamens durch Vitrektomie, ggf. in Kombination mit einer Phakoemulsifikation, ILM-Peeling und einer Tamponade entweder mit Luft oder mit 20%-igem SF6-Gas zu vergleichen. Primärer Endpunkt war die Verschlussrate nach einer Vitrektomie und die Reoperationsrate bei primär nicht geschlossenen Makulaforamina. Sekundärer Endpunkt war die Visusentwicklung in der ersten postoperativen Woche und nach 3 Monaten.
In der vorliegenden Arbeit wurden hierzu retrospektiv 117 Augen von 117 konsekutiven Patienten analysiert. Es wurden die Ergebnisse von 2 Patientengruppen verglichen. In der ersten Gruppe wurden 66 Augen (m=27, w=39, Altersmedian 70 Jahre), bei denen am Ende der Vitrektomie eine Tamponade mit Luft erfolgte, untersucht. In der zweiten Gruppe wurden die Ergebnisse von 51 Augen (m=20, w=31, Altersmedian 71 Jahre), bei denen 20%-SF6-Gas als Tamponade verwendet wurde, ausgewertet. Bei etwa 70 % der Augen beider Gruppen erfolgte eine simultane Phakoemulsifikation. Der Verschluss des Makulaforamens wurde bereits in den ersten postoperativen Tagen mit einem modifizierten Fourier Domain-OCT untersucht. Sobald der Verschluss des Makulaforamens im OCT gesichert werden konnte, wurde die postoperative „Gesicht nach unten“-Lagerung („Bauchlage“) beendet. Die mediane Lagerungszeit betrug 1 Tag (Spanne 1-6 Tage). Der Zeitraum der Nachbeobachtung betrug 3 Monate (Median).
Die Verschlussrate nach einer Vitrektomie aller Makulaforamina betrug 87,2% (102/117 Augen). Die primäre Verschlussrate der Luft-Gruppe und der SF6-Gas-Gruppe betrug 83,3% (55/66 Augen) bzw. 92,2% (47/51 Augen). Bei Augen mit einem persistierenden Foramen erfolgte eine frühe Reoperation. Die Verschlussrate nach 3 Monaten lag insgesamt bei 99,1% (116/117 Augen), in der Luft-Gruppe bei 98,5% (65/66 Augen) und in der SF6-Gas-Gruppe bei 100% (51/51 Augen). Es zeigte sich kein statistisch signifikanter Unterschied der Verschlussrate bzw. der anatomischen Ergebnisse zwischen den beiden Endotamponade-Gruppen.
Es konnte jedoch ein statistisch signifikanter Unterschied des Fernvisus bei der Entlassung zwischen beiden Endotamponadegruppen nachgewiesen werden. Der logMAR-Visus bei Entlassung lag in der Luft-Gruppe mit 1,3 (Median) deutlich unter dem Visus der SF6-Gas-Gruppe mit 1,9 (Median). Die Anwendung von Luft führte somit im Vergleich zu SF6 zu einem schnelleren postoperativen Visusanstieg. In beiden Gruppen zeigte sich nach 3 Monaten eine signifikante Visusbesserung. Es ließ sich kein signifikanter Unterschied des Fernvisus zwischen den beiden Gruppen mehr nachweisen.
Die Makulaforamenchirurgie mit pars plana Vitrektomie und Peeling der ILM erzielte sowohl mit einer Luft- als auch mit einer 20%-SF6-Gas-Tamponade guten anatomischen und funktionellen Ergebnissen. Mit einer kurz wirksamen Endotamponade erscheint eine postoperative Lagerung essenziell, dürfte aber in den meisten Fällen nur wenige Tage erforderlich sein, um den Verschluss eines durchgreifenden Makulaforamens zu erreichen. Die Reoperations-Rate nach einer Lufttamponade war bei sehr großen Foramina (> 600 µm) signifikant höher als bei Foramina mit einem Durchmesser kleiner 600 µm. Daher scheint die Anwendung von Gasen, die länger als Luft wirken, nur bei sehr großen Foramina erforderlich zu sein. Vorteil einer kurz wirkenden Lufttamponade ist die postoperativ um einige Tage schneller einsetzende visuelle Rehabilitation, so dass die Patienten früher in ihren Alltag zurückkehren können. Eine frühe postoperative Untersuchung der Makula unter Einsatz der optischen Kohärenztomographie ist Voraussetzung für eine individualisierte Steuerung der Lagerung und für eine möglichst frühe Erkennung und Reoperation von nicht geschlossenen Makulalöchern.
Cerumen was found to be a promising alternative specimen for the detection of drugs. In a pilot study, drugs of abuse were identified at a higher detection rate and a longer detection window in cerumen than in urine. In this study, cerumen from subjects was analyzed after they ingested the designer stimulant 4-fluoroamphetamine (4-FA) in a controlled manner. Methods: Twelve subjects ingested placebo and 100 mg of 4-FA. Five of them were also given 150 mg of 4-FA in 150 mL Royal Club bitter lemon drink at least after 7 days. Cerumen was sampled using cotton swabs at baseline, 1 h after the ingestion of the drug and at the end of the study day (12 h). After extraction with ethyl acetate followed by solid-phase extraction, the extracts were analyzed using liquid chromatography coupled with tandem mass spectrometry (LC–MS/MS). Results and discussion: In the cerumen of all 12 subjects, 4-FA was detected 12 h after its ingestion; in most subjects, cerumen was detected after 1 h of ingestion, ranging from 0.06 to 13.90 (median 1.52) ng per swab. The detection of 4-FA in cerumen sampled 7 days or more after the first dose suggested a long detection window of cerumen. Conclusions: Cerumen can be successfully used to detect a single drug ingestion even immediately after the ingestion when a sufficient amount of cerumen is used.
Aims: Acetylsalicylic acid (ASA) is widely used for the prevention of atherothrombotic events in patients with chronic coronary artery disease (CAD) and peripheral artery disease (PAD), but the risk of vascular events remains high. We aimed at identifying randomised controlled trials (RCTs) on antithrombotic treatments in patients with chronic CAD or PAD.
Methods: Searches were conducted on MEDLINE, EMBASE, and CENTRAL on March 1st, 2018. This systematic review (SR) uses a narrative synthesis to summarize the evidence for the efficacy and safety of antiplatelet and anticoagulant therapies in the population of both chronic CAD or PAD patients.
Results: Four RCTs from 27 publications were included. Study groups included 15,603 to 27,395 patients. ASA alone was the most extensively studied (n = 3); other studies included rivaroxaban with or without ASA (n = 1), vorapaxar alone (n = 1), and clopidogrel with (n = 1) or without ASA (n = 1). Clopidogrel alone and clopidogrel plus ASA compared to ASA presented similar efficacy with comparable safety profile. Rivaroxaban plus ASA significantly reduced the risk of the composite of cardiovascular death, myocardial infarction, and stroke compared to ASA alone, although major bleeding with rivaroxaban plus ASA increased.
Conclusion: There is limited and heterogeneous evidence on the prevention of atherothrombotic events in patients with chronic CAD or PAD. Clopidogrel alone and clopidogrel plus ASA did not demonstrate superiority over ASA alone. A combination of rivaroxaban plus ASA may offer significant additional benefit in reducing cardiovascular outcomes, yet it may increase the risk of bleeding, compared to ASA alone.
The aim of the study was to obtain volumetric data of the components of the inner ear using three-dimensional reconstruction of high-resolution cone-beam computed tomography (CBCT) images. Two hundred three CBCT image series of the temporal bone from 118 anatomically normal patients (55 women and 63 men; mean age: 49.4 ± 20.4 years) with different suspected disorders were included in this study. Normative volumetric measurements of the inner ear, the cochlea, the semicircular canals (SSC), and the vestibule were determined using a semi-automated reconstruction method of the Workstation. Volumetric measurements were successfully completed in all 118 patients. Mean inner ear, cochlear, and vestibule volumes were statistically significantly larger in males than in females on both sides (p < 0.001). Regarding the semicircular canals, no statistically significant (p = 0.053) volume difference was found. The difference between the volumes on both sides was not significant. No correlation between the patient’s age and the volume of the compartments was seen (p > 0.05). There was no significant difference between mean bony inner ear volumes when the clinical diagnoses were compared (p > 0.05 for all clinical diagnoses and volumes). Our study concluded that three-dimensional reconstruction and assessment of the volumetric measurements of the inner ear can be obtained using high-resolution CBCT imaging.
This case series assessed a commercial airline flight from Tel Aviv, Israel, to Frankfurt, Germany, that occurred on March 9th, 2020. Among 102 passengers on a Boeing 737-900 aircraft were 24 members of a tourist group. Starting 7 days earlier, the group had contact with a hotel manager who later received a diagnosis of coronavirus disease 2019 (COVID-19). No member of the group had received a diagnosis of COVID-19 before the flight, and no measures to prevent transmission (eg, wearing of masks) had been applied. The flight duration was 4 hours 40 minutes.
Objectives: The aim of this study was to investigate the relationship between anamnestic, axiographic and occlusal parameters and postural control in healthy women aged between 41 and 50 years. Materials and methods: A total of 100 female participants aged between 41 and 50 (45.12 ± 2.96) years participated in the study. In addition to completing a general anamnesis questionnaire, lower jaw movements were measured axiographically, dental occlusion parameters were determined using a model analysis and postural parameters were recorded using a pressure measurement platform. The significance level was 5%. Results: An increasing weight and a rising BMI lead to a weight shifted from the rearfoot (p ≤ 0.01/0.04) to the forefoot (p ≤ 0.01/0.02). A limited laterotrusion on the right resulted in a lower forefoot load and an increased rearfoot load (p ≤ 0.01). Laterotrusion to the left (extended above the standard) showed a lower frontal sway (p ≤ 0.02) and a reduced elliptical area, height and width (p ≤ 0.01, 0.02, 0.03). Thus, the extent of deviation correlated with reduced right forefoot loading (p ≤ 0.03) and the extent of deflection correlated with increased left foot loading (p ≤ 0.01). The higher the extent of angle class II malocclusion, the larger the ellipse area (p ≤ 0.04) and the ellipse height (p ≤ 0.02) resulted. Conclusions: There is a connection between weight, BMI and laterotrusion, as well as between angle class II malocclusion and postural control in women aged between 41 and 50 years. Interdisciplinary functional examinations of mandibular movements treating possible limitations can be conducive for an improvement of postural control. Clinical relevance: Angle class II malocclusion has a negative influence on postural control.
Background Reward processing has been proposed to underpin atypical social behavior, a core feature of autism spectrum disorder (ASD). However, previous neuroimaging studies have yielded inconsistent results regarding the specificity of atypicalities for social rewards in ASD. Utilizing a large sample, we aimed to assess altered reward processing in response to reward type (social, monetary) and reward phase (anticipation, delivery) in ASD.
Methods Functional magnetic resonance imaging during social and monetary reward anticipation and delivery was performed in 212 individuals with ASD (7.6-30.5 years) and 181 typically developing (TD) participants (7.6-30.8 years).
Results Across social and monetary reward anticipation, whole-brain analyses (p<0.05, family-wise error-corrected) showed hypoactivation of the right ventral striatum (VS) in ASD. Further, region of interest (ROI) analysis across both reward types yielded hypoactivation in ASD in both the left and right VS. Across delivery of social and monetary reward, hyperactivation of the VS in individuals with ASD did not survive correction for multiple comparisons. Reward type by diagnostic group interactions, and a dimensional analysis of autism trait scores were not significant during anticipation or delivery. Levels of attention-deficit/hyperactivity disorder (ADHD) symptoms did not affect reward processing in ASD.
Conclusions Our results do not support current theories linking atypical social interaction in ASD to specific alterations in processing of social rewards. Instead, they point towards a generalized hypoactivity of VS in ASD during anticipation of both social and monetary rewards. We suggest that this indicates attenuated subjective reward value in ASD independent of social content and ADHD symptoms.
Background: Autism spectrum disorder (“autism”) is a highly heterogeneous neurodevelopmental condition with few effective treatments for core and associated features. To make progress we need to both identify and validate neural markers that help to parse heterogeneity to tailor therapies to specific neurobiological profiles. Atypical hemispheric lateralization is a stable feature across studies in autism, but its potential as a neural stratification marker has not been widely examined. Methods: In order to dissect heterogeneity in lateralization in autism, we used the large EU-AIMS (European Autism Interventions—A Multicentre Study for Developing New Medications) Longitudinal European Autism Project dataset comprising 352 individuals with autism and 233 neurotypical control subjects as well as a replication dataset from ABIDE (Autism Brain Imaging Data Exchange) (513 individuals with autism, 691 neurotypical subjects) using a promising approach that moves beyond mean group comparisons. We derived gray matter voxelwise laterality values for each subject and modeled individual deviations from the normative pattern of brain laterality across age using normative modeling. Results: Individuals with autism had highly individualized patterns of both extreme right- and leftward deviations, particularly in language, motor, and visuospatial regions, associated with symptom severity. Language delay explained most variance in extreme rightward patterns, whereas core autism symptom severity explained most variance in extreme leftward patterns. Follow-up analyses showed that a stepwise pattern emerged, with individuals with autism with language delay showing more pronounced rightward deviations than individuals with autism without language delay. Conclusions: Our analyses corroborate the need for novel (dimensional) approaches to delineate the heterogeneous neuroanatomy in autism and indicate that atypical lateralization may constitute a neurophenotype for clinically meaningful stratification in autism.

Hintergrund: Die stationäre Aufnahme von Patienten mit Prellungen wird in Kliniken der Akutversorgung regelhaft praktiziert. Dabei stehen die pathophysiologischen Unfallfolgen oft im Hintergrund. Ziel dieser retrospektiven monozentrischen Untersuchung war die Untersuchung der Ätiologie sowie der kostenverursachenden Faktoren und Refinanzierung bei Aufnahmen durch Prellungen.
Methodik: Es erfolgte die Abfrage der Patienten entsprechend den Entlassdiagnosen aus dem krankenhausinternen Informationssystem (KIS). Eingeschlossen wurden 117 Patienten in einem Zeitraum von 2 Jahren. Es erfolgten hier die Klassifizierung nach Unfallmechanismus sowie die Einteilung in Altersgruppen. Des Weiteren erfolgte die Kostenkalkulation anhand von abteilungs- und klinikspezifischen Tagessätzen.
Ergebnisse: Bezüglich der Ätiologie war der häusliche Sturz die häufigste Ursache (48,7 %), gefolgt von dem Hochrasanztrauma (22,8 %). Innerhalb der Gruppe des häuslichen Sturzes lag das Durchschnittsalter im Mittel bei 77,8 Jahre. Diese Gruppe zeigte die längste Verweildauer (VWD) mit 5,2 Tagen. Im Rahmen der kalkulierten Kosten zeigte die Gruppe nach häuslichem Sturz die höchsten Kosten mit 2596,24 € bei einem mittleren DRG-Erlös von 1464,51 €.
Diskussion: Die Auswertung der klinikinternen Daten bestätigte die subjektive Wahrnehmung, dass ein Großteil der nach Prellung aufgenommenen Patienten aus der Altersgruppe >65 Jahre stammt. Die Aufnahme erfolgt hier vor dem Hintergrund der in dieser Altersgruppe zunehmenden Komorbiditäten sowie zur Abwendung von Folgeerkrankungen und Folgen der Immobilisierung. Ebenfalls konnte gezeigt werden, dass die Versorgungskosten gesundheitsökomisch relevant sind und die Behandlung in diesen Fällen nicht kostendeckend ist.
Hintergrund: Den derzeitigen Therapiestandard in der Behandlung des lokal begrenzten Prostatakarzinoms stellen die radikale Prostatektomie und perkutane Radiotherapie dar. Aufgrund der besonderen Strahlenbiologie des PCa (α-/β-Wert 1,5 Gy) ist von einer höheren Sensitivität gegenüber einer Hypofraktionierung auszugehen. Die aktuellen Literaturdaten zeigen eine Nichtunterlegenheit der moderaten Hypofraktionierung gegenüber dem konventionellen Regime, jedoch sind Langzeitergebnisse hinsichtlich der Spättoxizität noch spärlich. In dieser Arbeit werden onkologische Ergebnisse sowie GI-/ und GU-Toxizität nach moderat hypofraktionierter, intensitätsmodulierter (IMRT/VMAT) und bildgeführter (IGRT) Radiotherapie mit simultan integriertem Boost (SIB) in einem universitären Tumorzentrum untersucht.
Patienten und Methoden: In den Jahren 2010 bis 2016 wurden an der Uniklinik Frankfurt 170 Patienten mit histologisch gesichertem lokal begrenzten bis fortgeschrittenen PCa (T1-4, N0-1, M0) mittels moderat hypofraktionierter EBRT in IG-IMRT- bzw. IG-VMAT- und SIB-Technik behandelt. Alle Patienten wurden nach D’Amico-Klassifikation stratifiziert und risikoadaptiert therapiert. Die GI- und GU-Toxizität wurde gemäß CTC v4.0 wöchentlich unter der Therapie und während der Nachbeobachtungszeit erfasst. Ein biochemisches Rezidiv wurde definiert als PSA-Nadir + 2 ng/ml. Die Nachbeobachtung erfolgte 6-8 Wochen, 2-3 Monate und 6 Monate nach Radiotherapie sowie anschließend mindestens einmal jährlich. Die Datenauswertung erfolgte retrospektiv. Für die statistischen Analysen wurden die Software-Programme Excel und SPSS verwendet. Die Kaplan-Meier-Überlebensanalysen wurden mittels Log-Rank-Test verglichen und bei signifikanten Unterschieden in eine Cox-Regression-Analyse aufgenommen. Ein möglicher Zusammenhang zwischen dosimetrischen/klinischen Parametern und GI-/GU-Toxizität wurde mittels Chi-Quadrat-Test nach Pearson geprüft. Für alle durchgeführten Analysen galt ein p-Wert von <0,05 als statistisch signifikant. Ergebnisse: Patienten der low-risk- (n = 15) sowie intermediate- und high-risk-Gruppe mit Z.n. TURP (n= 14) erhielten eine GRD von 73,6 Gy für das PTV1 bzw. 58,24 Gy (PTV2) mit einer Einzeldosis von 2,3 Gy (SIB) bzw. 1,82 Gy in 32 Fraktionen. Patienten der intermediate- (n = 70) und high-risk- (n = 71) Gruppe erhielten eine GRD von 75,9 Gy (PTV1) bzw. 60,06 Gy (PTV2) mit einer Einzeldosis von 2,3 Gy (SIB) bzw. 1,82 Gy in 33 Fraktionen. Eine zusätzliche Bestrahlung des LAG (PTV3) mit einer GRD von 46,0 Gy und 1,84 Gy Einzeldosis in 25 Fraktionen erhielten alle Patienten mit radiologischer cN1-Situation bzw. im Falle eines LK-Befallsrisikos von >20% nach Roach-Formel (n = 86). Eine (neo-) adjuvante AHT erhielten insg. 98 Patienten. Die mediane Nachbeobachtungszeit betrug 36 Monate (Range: 5-84 Monate). Die 24-, 36-, und 60-Monate-ÜLR lagen in der IR-Gruppe bei 98,4%, 96,1% bzw. 89,7% (OS), 98,6% bzw. 94,9% (BC), 97% bzw. 84,5% (DFS) und 98,6% (FFM), in der HR-Gruppe bei 93,1% bzw. 88,8% (OS), 89%, 84,7% bzw. 75,3% (BC), 82,5%, 74,2% bzw. 65,9% (DFS) und 98%, 95,7% bzw. 84,8% (FFM). Weder für die AHT noch die RT des LAG konnte eine Verbesserung des OS, DFS, FFM sowie der BC gezeigt werden. Eine Akuttoxizität Grad 2 trat bei 6,5% (GI) bzw. 16,5% (GU) und Grad 3 bei 0,6% (GI) bzw. 0% (GU) auf. Eine Spättoxizität Grad 2 trat bei 2,9% (GI) bzw. 12,4% (GU) und Grad 3 bei 0,6% (GI) bzw. 1,2% (GU) auf.
Schlussfolgerung: Mit Implementierung der moderat hypofraktionierten EBRT nach o.g. Schema (73,6 Gy (79,9 Gy nach EQD2 (αβ 1,5)) bzw. 75,9 Gy (82,4 Gy nach EQD2 (αβ 1,5)) konnten im Rahmen dieser retrospektiven Kohortenstudie sehr gute Ergebnisse für intermediate- und high risk-Patienten des lokal begrenzten bis fortgeschrittenen PCa hinsichtlich der onkologischen Endpunkte sowie der GI-/GU-Toxizität, vergleichbar mit den besten Ergebnissen aktueller Publikationen, erzielt werden. Diese Ergebnisse bekräftigen die zunehmende Anwendung dieser Bestrahlungstechnik. Die Kohorte sollte zur Validierung der Ergebnisse weiter expandiert und mit einer längeren Nachbeobachtungszeit erneut ausgewertet werden.
Hintergrund: Mit dem Masterplan 2020 und den an mehreren Universitäten eingeführten Modellstudiengängen befindet sich das Medizinstudium aktuell im Umbruch. Sowohl im Regel- als auch im Modellstudiengang werden medizinrechtliche Aspekte überwiegend im Rahmen rechtsmedizinischer Ausbildungsabschnitte unterrichtet. Allerdings werden Studierende bereits während Famulaturen oder im praktischen Jahr mit juristischen Fragen konfrontiert.
Ziel der Studie war es herauszufinden, ob und in welchem Umfang Studierende der Humanmedizin insbesondere zur ärztlichen Aufklärung und zu den ärztlichen Informationspflichten bis zum Beginn des 4. bzw. 5. klinischen Semesters auf medizinrechtliche Aspekte vorbereitet wurden, und ob Verbesserungen bei der medizinrechtlichen Lehre gewünscht werden.
Material und Methoden: Zwischen den Sommersemestern 2017 und 2019 wurde zu Beginn des Kurses für Rechtsmedizin eine quantitative, standardisierte Umfrage mit insgesamt 373 Studierenden durchgeführt.
Ergebnisse: Wenngleich 98,8 % der Studierenden angaben, Aufklärungsgespräche bereits (mehrfach) praktisch ausgeübt zu haben, bestanden deutliche Defizite in Bezug auf die juristischen Anforderungen an das ärztliche Aufklärungsgespräch und dessen Delegation. So gaben lediglich 5,1 % der Studierenden an, die rechtlichen Grundlagen der ärztlichen Aufklärung sowie die entsprechende Norm aus dem Zivilrecht zu kennen. Über 80 % der Befragten fühlten sich unzureichend auf die rechtlichen Aspekte des praktischen Jahres vorbereitet. Über 90 % der Studierenden wünschten sich eine bessere medizinrechtliche Ausbildung.
Diskussion: Eine fächerübergreifende Etablierung sowie eine über das gesamte Studium verteilte Lehre von Medizinrecht könnte die Vorbereitung auf das praktische Jahr verbessern und das Verständnis für die rechtlichen Anforderungen an die ärztliche Berufstätigkeit fördern. Nach dem derzeitigen Stand der Umsetzung des Masterplans 2020 soll das Medizinrecht in der Learning Opportunities, Objectives and Outcomes Platform (LOOOP) als verbindlicher Ausbildungs- und Lehrinhalt etabliert werden.
With obesity having doubled in the last decade, hypertension is on the rise. In one-third of hypertensive patients the metabolic syndrome is present. This might be one factor for the increasing number of prescriptions for angiotensin receptor blockers and calcium-channel blockers besides a more favorable risk-to-benefit ratio. The aim of the present study was to evaluate a therapeutic drug monitoring (TDM) method for assessment of adherence based on cut-offs in inpatients and to compare it to an established urine drug screening in outpatients. A method for quantification of calcium-channel blockers and angiotensin receptor blockers using high-performance liquid chromatography-tandem mass spectrometric analysis (LC-MS/MS) was developed and validated. The method was applied to serum samples of 32 patients under supervised medication to establish cut-off values for adherence assessment based on dose-related concentrations (DRC, calculated from pharmacokinetic data). Furthermore, corresponding urine and blood samples of 42 outpatients without supervised medication were analysed and the results compared with regard to adherence assessment. All serum concentrations measured for amlodipine (n = 40), lercanidipine (n = 14), candesartan (n = 10), telmisartan (n = 4) and valsartan (n = 10) in inpatients were above the patient specific lower DRC confirming adherence. Of 42 outpatients the identification of analytes in urine as well as the quantification in serum exhibited differing results. According to urinalysis, adherence was demonstrated in only 87.0% of prescriptions, compared to 91.3% for serum analyses. Differences were observed for amlodipine, lercanidipine and candesartan which can be explained by a higher specificity of the serum analysis approach due to pharmacokinetics. The present study confirms that assessing adherence based on serum drug concentrations with individually calculated lower DRCs is more accurate than using qualitative urine analysis. In particular, drugs with low bioavailability, low renal excretion or high metabolism rate such as lercanidipine and candesartan may lead to underestimation of adherence via urine analysis.
Due to anticipated postoperative neuropsychological sequelae, patients with gliomas infiltrating the corpus callosum rarely undergo tumor resection and mostly present in a poor neurological state. We aimed at investigating the benefit of glioma resection in the corpus callosum, hypothesizing neuropsychological deficits were mainly caused by tumor presence. Between 01/2017 and 1/2020, 21 patients who underwent glioma resection in the corpus callosum were prospectively enrolled into this study. Neuropsychological function was assessed preoperatively, before discharge and after 6 months. Gross total tumor resection was possible in 15 patients, and in 6 patients subtotal tumor resection with a tumor reduction of 97.7% could be achieved. During a median observation time of 12.6 months 9 patients died from glioblastoma after a median of 17 months. Preoperatively, all cognitive domains were affected in up to two thirds of patients, who presented a median KPS of 100% (range 60–100%). After surgery, the proportion of impaired patients increased in all neurocognitive domains. Most interestingly, after 6 months, significantly fewer patients showed impairments in attention, executive functioning, memory and depression, which are domains considered crucial for everyday functionality. Thus, the results of our study strongly support our hypothesis that in patients with gliomas infiltrating the corpus callosum the benefit of tumor resection might outweigh morbidity.
Chronic treatment with the mTOR inhibitor, everolimus, fails long-term in preventing tumor growth and dissemination in cancer patients. Thus, patients experiencing treatment resistance seek complementary measures, hoping to improve therapeutic efficacy. This study investigated metastatic characteristics of bladder carcinoma cells exposed to everolimus combined with the isothiocyanate sulforaphane (SFN), which has been shown to exert cancer inhibiting properties. RT112, UMUC3, or TCCSUP bladder carcinoma cells were exposed short- (24 h) or long-term (8 weeks) to everolimus (0.5 nM) or SFN (2.5 µM), alone or in combination. Adhesion and chemotaxis along with profiling details of CD44 receptor variants (v) and integrin α and β subtypes were evaluated. The functional impact of CD44 and integrins was explored by blocking studies and siRNA knock-down. Long-term exposure to everolimus enhanced chemotactic activity, whereas long-term exposure to SFN or the SFN-everolimus combination diminished chemotaxis. CD44v4 and v7 increased on RT112 cells following exposure to SFN or SFN-everolimus. Up-regulation of the integrins α6, αV, and β1 and down-regulation of β4 that was present with everolimus alone could be prevented by combining SFN and everolimus. Down-regulation of αV, β1, and β4 reduced chemotactic activity, whereas knock-down of CD44 correlated with enhanced chemotaxis. SFN could, therefore, inhibit resistance-related tumor dissemination during everolimus-based bladder cancer treatment.
Objective: Relative to urban populations, rural patients may have more limited access to care, which may undermine timely bladder cancer (BCa) diagnosis and even survival.
Methods: We tested the effect of residency status (rural areas [RA < 2500 inhabitants] vs. urban clusters [UC ≥ 2500 inhabitants] vs. urbanized areas [UA, ≥50,000 inhabitants]) on BCa stage at presentation, as well as on cancer-specific mortality (CSM) and other cause mortality (OCM), according to the US Census Bureau definition. Multivariate competing risks regression (CRR) models were fitted after matching of RA or UC with UA in stage-stratified analyses.
Results: Of 222,330 patients, 3496 (1.6%) resided in RA, 25,462 (11.5%) in UC and 193,372 (87%) in UA. Age, tumor stage, radical cystectomy rates or chemotherapy use were comparable between RA, UC and UA (all p > 0.05). At 10 years, RA was associated with highest OCM followed by UC and UA (30.9% vs. 27.7% vs. 25.6%, p < 0.01). Similarly, CSM was also marginally higher in RA or UC vs. UA (20.0% vs. 20.1% vs. 18.8%, p = 0.01). In stage-stratified, fully matched CRR analyses, increased OCM and CSM only applied to stage T1 BCa patients.
Conclusion: We did not observe meaningful differences in access to treatment or stage distribution, according to residency status. However, RA and to a lesser extent UC residency status, were associated with higher OCM and marginally higher CSM in T1N0M0 patients. This observation should be further validated or refuted in additional epidemiological investigations.
22q11.2 Deletion Syndrome (22q11.2DS) is the most common microdeletion in humans, with a heterogenous clinical presentation including medical, behavioural and psychiatric conditions. Previous neuroimaging studies examining the neuroanatomical underpinnings of 22q11.2DS show alterations in cortical volume (CV), cortical thickness (CT) and surface area (SA). The aim of this study was to identify (1) the spatially distributed networks of differences in CT and SA in 22q11.2DS compared to controls, (2) their unique and spatial overlap, as well as (3) their relative contribution to observed differences in CV. Structural MRI scans were obtained from 62 individuals with 22q11.2DS and 57 age-and-gender-matched controls (aged 6–31). Using FreeSurfer, we examined differences in vertex-wise estimates of CV, CT and SA at each vertex, and compared the frequencies of vertices with a unique or overlapping difference for each morphometric feature. Our findings indicate that CT and SA make both common and unique contributions to volumetric differences in 22q11.2DS, and in some areas, their strong opposite effects mask differences in CV. By identifying the neuroanatomic variability in 22q11.2DS, and the separate contributions of CT and SA, we can start exploring the shared and distinct mechanisms that mediate neuropsychiatric symptoms across disorders, e.g. 22q11.2DS-related ASD and/or psychosis/schizophrenia.
Cabozantinib (Cabometyx®) is a potent multikinase inhibitor targeting the vascular endothelial growth factor (VEGF) receptor 2, the mesenchymal-epithelial transition factor (MET) receptor, and the “anexelekto” (AXL) receptor tyrosine kinase. It is approved for the treatment of advanced hepatocellular carcinoma (HCC) after failure of sorafenib in Europe (since November 2018) and in the USA (since January 2019). The approval of cabozantinib was based on results of the randomized, placebo-controlled, phase 3 CELESTIAL trial in patients with unresectable HCC, who received one or two prior lines of treatment including sorafenib. At the second planned interim analysis, the trial was stopped, because the primary end point overall survival was clearly in favor for cabozantinib. Additionally, median progression-free survival was superior to placebo. The most common ≥ grade 3 relevant adverse events in patients with HCC treated with cabozantinib were palmar–plantar erythrodysesthesia, hypertension, fatigue, and diarrhea. In this review, current data on cabozantinib for the treatment of patients with advanced HCC, with a focus on the management of common adverse events and ongoing clinical trials, are discussed.
Cancer‐associated venous thromboembolism (VTE) is a frequent, potentially life‐threatening event that complicates cancer management. Anticoagulants are the cornerstone of therapy for the treatment and prevention of cancer‐associated thrombosis (CAT); factor Xa–inhibiting direct oral anticoagulants (DOACs; apixaban, edoxaban, and rivaroxaban), which have long been recommended for the treatment of VTE in patients without cancer, have been investigated in this setting. The first randomized comparisons of DOACs against low‐molecular‐weight heparin for the treatment of CAT indicated that DOACs are efficacious in this setting, with findings reflected in recent updates to published guidance on CAT treatment. However, the higher risk of bleeding events (particularly in the gastrointestinal tract) with DOACs highlights the need for appropriate patient selection. Further insights will be gained from additional studies that are ongoing or awaiting publication. The efficacy and safety of DOAC thromboprophylaxis in ambulatory patients with cancer at a high risk of VTE have also been assessed in placebo‐controlled randomized controlled trials of apixaban and rivaroxaban. Both studies showed efficacy benefits with DOACs, but both studies also showed a nonsignificant increase in major bleeding events while on treatment. This review summarizes the evidence base for rivaroxaban use in CAT, the patient profile potentially most suited to DOAC use, and ongoing controversies under investigation. We also describe ongoing studies from the CALLISTO (Cancer Associated thrombosis—expLoring soLutions for patients through Treatment and Prevention with RivarOxaban) program, which comprises several randomized clinical trials and real‐world evidence studies, including investigator‐initiated research.
CD4+ T cell lymphopenia predicts mortality from Pneumocystis pneumonia in kidney transplant patients

Background: Pneumocystis jirovecii pneumonia (PcP) remains a life-threatening opportunistic infection after solid organ transplantation, even in the era of Pneumocystis prophylaxis. The association between risk of developing PcP and low CD4+ T cell counts has been well established. However, it is unknown whether lymphopenia in the context of post-renal transplant PcP increases the risk of mortality. Methods: We carried out a retrospective analysis of a cohort of kidney transplant patients with PcP (n = 49) to determine the risk factors for mortality associated with PcP. We correlated clinical and demographic data with the outcome of the disease. For CD4+ T cell counts, we used the Wilcoxon rank sum test for in-hospital mortality and a Cox proportional-hazards regression model for 60-day mortality. Results: In univariate analyses, high CRP, high neutrophils, CD4+ T cell lymphopenia, mechanical ventilation, and high acute kidney injury network stage were associated with in-hospital mortality following presentation with PcP. In a receiver-operator characteristic (ROC) analysis, an optimum cutoff of ≤200 CD4+ T cells/µL predicted in-hospital mortality, CD4+ T cell lymphopenia remained a risk factor in a Cox regression model. Conclusions: Low CD4+ T cell count in kidney transplant recipients is a biomarker for disease severity and a risk factor for in-hospital mortality following presentation with PcP.
Complexome profiling is an emerging ‘omics approach that systematically interrogates the composition of protein complexes (the complexome) of a sample, by combining biochemical separation of native protein complexes with mass-spectrometry based quantitation proteomics. The resulting fractionation profiles hold comprehensive information on the abundance and composition of the complexome, and have a high potential for reuse by experimental and computational researchers. However, the lack of a central resource that provides access to these data, reported with adequate descriptions and an analysis tool, has limited their reuse. Therefore, we established the ComplexomE profiling DAta Resource (CEDAR, www3.cmbi.umcn.nl/cedar/), an openly accessible database for depositing and exploring mass spectrometry data from complexome profiling studies. Compatibility and reusability of the data is ensured by a standardized data and reporting format containing the “minimum information required for a complexome profiling experiment” (MIACE). The data can be accessed through a user-friendly web interface, as well as programmatically using the REST API portal. Additionally, all complexome profiles available on CEDAR can be inspected directly on the website with the profile viewer tool that allows the detection of correlated profiles and inference of potential complexes. In conclusion, CEDAR is a unique, growing and invaluable resource for the study of protein complex composition and dynamics across biological systems.
In murine models, the expression of inducible nitric oxide synthase (iNOS) in myocardial infarction (MI) has been reported to be the result of tissue injury and inflammation. In the present study, mRNA expression of iNOS, hypoxia-inducible factor-1α (HIF-1α), and vascular endothelial growth factor (VEGF) was investigated in postmortem human infarction hearts. Since HIF-1α is the inducible subunit of the transcription factor HIF-1, which regulates transcription of iNOS and VEGF, the interrelation between the three genes was observed, to examine the molecular processes during the emergence of MI. iNOS and VEGF mRNAs were found to be significantly upregulated in the affected regions of MI hearts in comparison to healthy controls. Upregulation of HIF-1α was also present but not significant. Correlation analysis of the three genes indicated a stronger and significant correlation between HIF-1α and iNOS mRNAs than between HIF-1α and VEGF. The results of the study revealed differences in the expression patterns of HIF-1 downstream targets. The stronger transcription of iNOS by HIF-1 in the affected regions of MI hearts may represent a pathological process, since no correlation of iNOS and HIF-1α mRNA was found in non-affected areas of MI hearts. Oxidative stress is considered to cause molecular changes in MI, leading to increased iNOS expression. Therefore, it may also represent a forensic marker for detection of early changes in heart tissue.
Purpose: Trauma is the leading cause of death in children. In adults, blood transfusion and fluid resuscitation protocols changed resulting in a decrease of morbidity and mortality over the past 2 decades. Here, transfusion and fluid resuscitation practices were analysed in severe injured children in Germany.
Methods: Severely injured children (maximum Abbreviated Injury Scale (AIS) ≥ 3) admitted to a certified trauma-centre (TraumaZentrum DGU®) between 2002 and 2017 and registered at the TraumaRegister DGU® were included and assessed regarding blood transfusion rates and fluid therapy.
Results: 5,118 children (aged 1–15 years) with a mean ISS 22 were analysed. Blood transfusion rates administered until ICU admission decreased from 18% (2002–2005) to 7% (2014–2017). Children who are transfused are increasingly seriously injured. ISS has increased for transfused children aged 1–15 years (2002–2005: mean 27.7–34.4 in 2014–2017). ISS in non-transfused children has decreased in children aged 1–15 years (2002–2005: mean 19.6 to mean 17.6 in 2014–2017). Mean prehospital fluid administration decreased from 980 to 549 ml without affecting hemodynamic instability.
Conclusion: Blood transfusion rates and amount of fluid resuscitation decreased in severe injured children over a 16-year period in Germany. Restrictive blood transfusion and fluid management has become common practice in severe injured children. A prehospital restrictive fluid management strategy in severely injured children is not associated with a worsened hemodynamic state, abnormal coagulation or base excess but leads to higher hemoglobin levels.
Background: Alterations in the SCN5A gene encoding the cardiac sodium channel Nav1.5 have been linked to a number of arrhythmia syndromes and diseases including long-QT syndrome (LQTS), Brugada syndrome (BrS) and dilative cardiomyopathy (DCM), which may predispose to fatal arrhythmias and sudden death. We identified the heterozygous variant c.316A > G, p.(Ser106Gly) in a 35-year-old patient with survived cardiac arrest. In the present study, we aimed to investigate the functional impact of the variant to clarify the medical relevance.
Methods: Mutant as well as wild type GFP tagged Nav1.5 channels were expressed in HEK293 cells. We performed functional characterization experiments using patch-clamp technique.
Results: Electrophysiological measurements indicated, that the detected missense variant alters Nav1.5 channel functionality leading to a gain-of-function effect. Cells expressing S106G channels show an increase in Nav1.5 current over the entire voltage window.
Conclusion: The results support the assumption that the detected sequence aberration alters Nav1.5 channel function and may predispose to cardiac arrhythmias and sudden cardiac death.
