Objective. We investigated the health-related quality of life (HRQoL) of patients with gastrointestinal stromal tumours (GIST). Methods: In the multicentre PROSa study, the HRQoL of adult GIST patients was assessed between 2017 and 2019 using the European Organisation for Research and Treatment of Cancer HRQoL questionnaire (EORTC QLQ-C30). We performed group comparisons and multivariate linear regressions. Results: Among 130 patients from 13 centres, the mean global HRQoL was 63.3 out of 100 points. Higher sores indicate better HRQoL. The highest restrictions were in emotional, social, role functioning, insomnia, fatigue, and pain. In multivariate linear regression, we found no significant differences between patients receiving tyrosine kinase inhibitor (TKI) treatment and those without TKI treatment as well as between patients treated with curative or with palliative intent. Patients who received multiple lines of TKI treatment had the most restrictions, notably in physical (unstandardized regression coefficient [B] = −15.7), role (B = −25.7), social (B = −18.4), and cognitive functioning (B = −19.7); fatigue (B = 15.93); general health (B = −14.23); and EORTC-sum score (B = −13.82) compared to all other patients. Conclusion: The highest HRQoL restrictions were in GIST patients receiving multiple lines of TKI therapy. Underlying causes need further investigation.
Aims: The study aimed to evaluate the prevalence of mental distress in patients with newly diagnosed bladder cancer, the cancer-information search behavior, and the influence of information seeking on distress. Methods: One hundred and one bladder cancer patients answered 2 established questionnaires (“Hospital Anxiety and Depression Scale” [HADS] and the “Fragebogen zur Belastung von Krebskranken” [FBK-R23]) for evaluation of mental distress and a self-developed questionnaire with questions concerning information seeking and socioeconomic facts. Results: Regarding risk group stratification, 57.4% were classified as high-risk and 42.6% as low-risk tumor-bearing patients. Analysis of mental distress showed that 23.2% had a score above the HADS-A cutoff, 25.3% above the HADS-D cutoff, and 21.4% showed a pathologic FBK-R23 score. Overall, 75% felt well informed about their illness. Risk group stratification did not correlate with HADS-A, HADS-D, or FBK-R23 score. Furthermore, active search for information or the use of the Internet did not correlate with the HADS-A, HADS-D, or FBK-R23 score. However, the quality of the urologist’s information and the feeling of being informed correlated with the grade of mental distress. Conclusion: Besides the treatment of bladder cancer, informing the patient about the disease in a psychologically wholesome manner and working together with psycho-oncologically trained psychologists are essential tasks for the treating urologist.
Die Neuregelung der Qualitätssicherung für Hormone und Pharmaka gemäß den Qualitätssicherungsrichtlinien der Bundesärztekammer (Rili-BÄK) unterliegt einer intensiven Diskussion, besonders in Hinsicht auf die Praktikabilität Für die ärztlichen Laboratorien und die Diagnostikahersteller ergeben sich erhebliche Veränderungen, die aufgezeigt werden.
Die HIV-Infizierung von Zellkulturen in vitro ist essentiell für das Verstehen der Kinetik der Virusreplikation, für die Aufdeckung von Resistenzentwicklungen gegenüber antiretroviraler Medikamente und für die Entwicklung neuer antiretroviraler Therapiestrategien. Voraussetzung hierfür ist ein geeignetes Monitoring der HIV-Infektion von in vitro infizierten Zellen. Die vorliegende Arbeit beschäftigt sich mit dem Monitoring der HIV-Replikation von in vitro infizierten Zellen mittels der Real-Time TaqMan™ PCR. Die Ergebnisse der Real-Time TaqMan™ PCR wurden mit denen eines p24 ELISAs verglichen. Der p24 ELISA diente als etablierte Standardmethode zum Monitoring einer in vitro HIV-Infektion. HUT 78-Zellen wurden in vitro mit vier unterschiedlichen HIV-1 IIIb Infektionsdosen (MOI 0,05; MOI 0,01; MOI 0,002; MOI 0,0005) infiziert. Mittels der Real-Time TaqMan™ PCR wurde die HIV-1 gag cDNA quantifiziert. Mittels ELISA erfolgte die Quantifizierung des HIV-p24. Zusätzlich dazu wurde die Anzahl an proviralen HIV-1 Transkripten in den Zellkulturen mittels der TaqMan™ PCR quantifiziert. Die Quantifizierung der HIV-1 gag cDNA und des p24 ergaben nahezu identische Kurvenverläufe der Infektionskinetiken. Beide Nachweismethoden zeigten vergleichbare Daten bezüglich des exponentiellen Ansteigens und der sich daran anschließenden Plateauphase der HIV-Replikation. Die Sensitivität beider Nachweismethoden war ebenfalls vergleichbar. Ein großer Unterschied lag in den Messbereichen beider Methoden. Bei der Real-Time TaqMan™ PCR konnte eine Linearität über 7 log-Stufen
demonstriert werden. Dies hatte den Vorteil, dass die Zellkulturproben vor der Quantifizierung der HIV-1 gag cDNA nicht verdünnt werden mussten. Im Gegensatz dazu war der Messbereich des HIV-p24 ELISAs sehr eng und erforderte in den meisten Fällen eine Verdünnung der Messproben. Bezüglich des Arbeitsaufwandes und der aufkommenden Kosten ergaben sich für die Quantifizierung der HIV-1 gag cDNA mittels der Real-Time TaqMan ™ und für die Quantifizierung des p24 mittels ELISA nahezu identische Werte. Der Verlauf der Werte an proviralen HIV-1 Transkripten ähnelt dem der HIV-1 gag cDNA Kinetik. Mittels der Quantifizierung der proviralen HIV-1 Kopien kann jedoch keine Aussage über die HIV-Replikation getroffen werden. Abschließend ist zu sagen, dass die Real-Time TaqMan™ PCR eine zuverlässige und sensitive Methode ist, eine HIV-1 Replikation von in vitro infizierten Zellen zu quantifizieren und den Replikationsverlauf zu beschreiben. Die Real-Time TaqMan™ PCR stellt eine alternative Methode zum HIV-p24 ELISA dar, um eine in vitro HIV-Replikation zu dokumentieren.
Event-related potentials (ERPs) are widely used in basic neuroscience and in clinical diagnostic procedures. In contrast, neurophysiological insights from ERPs have been limited, as several different mechanisms lead to ERPs. Apart from stereotypically repeated responses (additive evoked responses), these mechanisms are asymmetric amplitude modulations and phase-resetting of ongoing oscillatory activity. Therefore, a method is needed that differentiates between these mechanisms and moreover quantifies the stability of a response. We propose a constrained subspace independent component analysis that exploits the multivariate information present in the all-to-all relationship of recordings over trials. Our method identifies additive evoked activity and quantifies its stability over trials. We evaluate identification performance for biologically plausible simulation data and two neurophysiological test cases: Local field potential (LFP) recordings from a visuo-motor-integration task in the awake behaving macaque and magnetoencephalography (MEG) recordings of steady-state visual evoked fields (SSVEFs). In the LFPs we find additive evoked response contributions in visual areas V2/4 but not in primary motor cortex A4, although visually triggered ERPs were also observed in area A4. MEG-SSVEFs were mainly created by additive evoked response contributions. Our results demonstrate that the identification of additive evoked response contributions is possible both in invasive and in non-invasive electrophysiological recordings.
Purpose: To identify transjugular intrahepatic portosystemic shunt (TIPS) thrombosis in abdominal CT scans applying quantitative image analysis.
Materials and methods: We retrospectively screened 184 patients to include 20 patients (male, 8; female, 12; mean age, 60.7 ± 8.87 years) with (case, n = 10) and without (control, n = 10) in-TIPS thrombosis who underwent clinically indicated contrast-enhanced and unenhanced abdominal CT followed by conventional TIPS-angiography between 08/2014 and 06/2020. First, images were scored visually. Second, region of interest (ROI) based quantitative measurements of CT attenuation were performed in the inferior vena cava (IVC), portal vein and in four TIPS locations. Minimum, maximum and average Hounsfield unit (HU) values were used as absolute and relative quantitative features. We analyzed the features with univariate testing.
Results: Subjective scores identified in-TIPS thrombosis in contrast-enhanced scans with an accuracy of 0.667 – 0.833. Patients with in-TIPS thrombosis had significantly lower average (p < 0.001), minimum (p < 0.001) and maximum HU (p = 0.043) in contrast-enhanced images. The in-TIPS / IVC ratio in contrast-enhanced images was significantly lower in patients with in-TIPS thrombosis (p < 0.001). No significant differences were found for unenhanced images. Analyzing the visually most suspicious ROI with consecutive calculation of its ratio to the IVC, all patients with a ratio < 1 suffered from in-TIPS thrombosis (p < 0.001, sensitivity and specificity = 100%).
Conclusion: Quantitative analysis of abdominal CT scans facilitates the stratification of in-TIPS thrombosis. In contrast-enhanced scans, an in-TIPS / IVC ratio < 1 could non-invasively stratify all patients with in-TIPS thrombosis.
Autism spectrum disorders (ASD) are highly heritable and are characterized by deficits in social communication and restricted and repetitive behaviors. Twin studies on phenotypic subdomains suggest a differing underlying genetic etiology. Studying genetic variation explaining phenotypic variance will help to identify specific underlying pathomechanisms. We investigated the effect of common variation on ASD subdomains in two cohorts including >2500 individuals. Based on the Autism Diagnostic Interview-Revised (ADI-R), we identified and confirmed six subdomains with a SNP-based genetic heritability h2SNP = 0.2–0.4. The subdomains nonverbal communication (NVC), social interaction (SI), and peer interaction (PI) shared genetic risk factors, while the subdomains of repetitive sensory-motor behavior (RB) and restricted interests (RI) were genetically independent of each other. The polygenic risk score (PRS) for ASD as categorical diagnosis explained 2.3–3.3% of the variance of SI, joint attention (JA), and PI, 4.5% for RI, 1.2% of RB, but only 0.7% of NVC. We report eight genome-wide significant hits—partially replicating previous findings—and 292 known and novel candidate genes. The underlying biological mechanisms were related to neuronal transmission and development. At the SNP and gene level, all subdomains showed overlap, with the exception of RB. However, no overlap was observed at the functional level. In summary, the ADI-R algorithm-derived subdomains related to social communication show a shared genetic etiology in contrast to restricted and repetitive behaviors. The ASD-specific PRS overlapped only partially, suggesting an additional role of specific common variation in shaping the phenotypic expression of ASD subdomains.
UV-mikrospektrophotometrische Messungen der Nucleinsäuren- und Eiweißkörper-Konzentration sowie der Kern- und Nukleolengröße nach Virusinfektion und unspezifischer Reizung der Chorion-Allantoismembran zeigen, daß es in beiden Fällen zu einer gleich starken Stimulierung des nucleinsäuren- und eiweißkörperbildenden Systems der Zelle kommt. Bei der Infektion mit Vaccinevirus auf das Ektoderm setzt die Reaktion der Membranzellen in der Eklipse ein, nach Infektion mit Newcastle-Disease-Virus fällt der Titeranstieg mit der Zellreaktion zeitlich zusammen.
Zur quantitativen Plasmaproteinbestimmung sind die Immunnephelometrie und die Immunturbidimetrie häufig eingesetzte Bestimmungstechniken. Aufgrund der kürzeren Analysenzeit bei vergleichbarer oder besserer Präzision und Empfindlichkeit haben diese Techniken die radiale Immundiffusion in vielen Laboratorien ersetzt.
Die mechanisierte Plasmaproteinbestimmung erfolgt als Antigen-Antikörper-Reaktion entweder mit Proteinanalyzern oder mit klinisch-chemischen Analysensystemen, an denen normalerweise Enzyme und Substrate bestimmt werden.
Zwischen den verschiedenen Plasmaproteinen im Serum besteht ein bis zu 10.OOOfacher Konzentrationsunterschied und die biologische Varianz des einzelnen Plasmaproteins ist breit Damit die Plasmaproteine mit hoher analytischer Sensitivität über einen weiten Konzentrationsbereich, mit guter Präzision und der erforderlichen Richtigkeit mechanisiert bestimmt werden können, ist eine gute Adaption des Immunreagenzes auf das mechanisierte Analysensystem erforderlich. Diese Übersicht soll dazu Grundlagen vermitteln.
The aim of this study is to provide a systematic assessment of the influence of the position on the arterial input function (AIF) for perfusion quantification. In 39 patients with a wide range of left ventricular function the AIF was determined using a diluted contrast bolus of a cardiac magnetic resonance imaging in three left ventricular levels (basal, mid, apex) as well as aortic sinus (AoS). Time to peak signal intensities, baseline corrected peak signal intensity and upslopes were determined and compared to those obtained in the AoS. The error induced by sampling the AIF in a position different to the AoS was determined by Fermi deconvolution. The time to peak signal intensity was strongly correlated (r2 > 0.9) for all positions with a systematic earlier arrival in the basal (− 2153 ± 818 ms), the mid (− 1429 ± 928 ms) and the apical slice (− 450 ± 739 ms) relative to the AoS (all p < 0.001). Peak signal intensity as well as upslopes were strongly correlated (r2 > 0.9 for both) for all positions with a systematic overestimation in all positions relative to the AoS (all p < 0.001 and all p < 0.05). Differences between the positions were more pronounced for patients with reduced ejection fraction. The error of averaged MBF quantification was 8%, 13% and 27% for the base, mid and apex. The location of the AIF significantly influences core parameters for perfusion quantification with a systematic and ejection fraction dependent error. Full quantification should be based on obtaining the AIF as close as possible to the myocardium to minimize these errors.
Astrozyten erfüllen verschiedene Funktionen im Zentralnervensystem, welche sich in die Bereiche Entwicklung, Durchblutung, Metabolismus, Strukturerhalt und Gliotransmission unterteilen lassen. Astrozyten sind an der synaptischen Informationsverarbeitung beteiligt und wirken an zahlreichen höheren Hirnfunktionen mit. Durch Regulation der synaptischen Transmission und Plastizität sind Astrozyten am Lernverhalten und Erinnerungsvermögen, sowie an der Verhaltensmodulation und Verarbeitung emotionaler Reize involviert. Im Zuge dieser zahlreichen Funktionen können Astrozyten auf externe Stimuli mit der gezielten Freisetzung von Gliotransmittern reagieren.
In kultivierten Astrozyten konnte Keil143 das TGN, bestehend aus Zisternen und Vesikeln, darstellen und mit anti-Rab6 identifizieren. Rab6 mit seinen Subtypen A und B gehört der Superfamilie der monomeren Ras-GTPasen an, die den intrazellulären Membran- und Vesikelverkehr regulieren. Rab6 spielt in HeLa-Zellen beim Transport vesikulärer Organellen vom TGN zur Zellmembran eine wichtige Rolle. Assoziationsanalysen von Rab6A mit vesikulären Glutamattransportern, Serinracemase und Markern der regulierten Exozytose in kultivierten Astrozyten143 deuten darauf hin, dass dieses Rab6A-Organellsystem die ultrastrukturelle Grundlage für die Freisetzung von Gliotransmittern wie D-Serin und Glutamat bildet.
Zur Untersuchung, ob Rab6A tatsächlich ein System der Glia-Neuron-Kommunikation im Gehirn darstellt, war es zunächst unabdingbar das Vorkommen von Rab6A in situ zu untersuchen. Die durchgeführten immunzytochemischen Färbungen an Hirnschnitten der Maus zeigen das gleichmäßige und ubiquitäre Vorkommen von Rab6A in allen untersuchten Hirnregionen. Durch verblindet durchgeführte Kolokalisationsanalysen von Rab6A mit den etablierten astrozytären Markern Glutaminsynthetase (GS), Glial fibrillary acidic protein (GFAP), Aldh1L1 und Sox9 konnte eine Lokalisation von Rab6A in allen Astrozyten gezeigt werden. Weitere Analysen schließen die Lokalisation von Rab6A in Mikroglia (Iba1), NG2-Zellen (NG2) und Oligodendrozyten (CNPase) aus. Die Astrozyten unterscheiden sich in Größe und subzellulärem Verteilungsmuster der Rab6A+ Strukturen, wonach eine Kategorisierung in vier Typen vorgenommen wurde. Anhand der Einteilung kann vermutet werden, dass größere Rab6A+ TGN-Zisternen bis weit in die Zellperipherie transportiert werden und kleine Rab6A+ Vesikel erst dort ausknospen und der Exozytose zugeführt werden. Zur Frage der möglichen astrozytären Subpopulationen konnte gezeigt werden, dass alle untersuchten Astrozyten GS+, Aldh1L1+, Sox9+ und Rab6A+ sind, jedoch nicht GFAP+.
Um die prinzipielle Übertragbarkeit der gewonnenen Befunde auf den Menschen zu überprüfen, wurde reseziertes Cortex-Gewebe von drei Patienten mit unterschiedlicher pathologischer Genese untersucht. Rab6A ist im massiven Ausmaß in humanen Astrozyten lokalisiert, was nahelegt, dass die zuvor an der Maus gewonnenen Ergebnisse auf den Menschen übertragbar sind.
Die mögliche funktionelle Bedeutung von astrozytärem Rab6A im Gehirn wurde an HFS-Schnitten untersucht. Die Untersuchung zeigt einen signifikanten Anstieg der Rab6A+ Intensität in der gesamten Molekularschicht der Fascia dentata der stimulierten im Vergleich zur unstimulierten Seite. Da die HFS ein etabliertes LTP-Modell darstellt, könnte es infolge dieser zu einer strukturellen, intrazellulären Veränderung der Astrozyten mit erhöhter Freisetzung von D-Serin oder Glutamat aus Rab6A+ Vesikeln kommen, was das Lernverhalten beeinflussen könnte. Die dargestellten Ergebnisse legen eine Auswirkung der HFS auf Rab6A nahe.
Zur Bestätigung der immunzytochemischen Untersuchungen wurde die mRNA-Expression von Rab6A in Astrozyten bereits publizierter Transkriptomanalysen untersucht. Die in den Publikationen verwendeten Genom-Chips treffen allenfalls indirekt eine Aussage zu Rab6A, da Rab6 allgemein und nur Rab6B spezifisch untersucht wurde, jedoch keine spezifische Rab6A Sonde erwähnt wird.
Zusammenfassend kann Rab6A als spezifisches und selektiv in Astrozyten vorkommendes Protein dargestellt und als neuer astrozytärer Marker etabliert werden, der auch Astrozyten des humanen Gewebes markiert. Durch die gewonnenen Befunde kann in nachfolgenden Studien die mögliche Bedeutung von Rab6A in neuropathologischen und neurophysiologischen Prozessen untersucht werden.
Background: Radiofrequency ablation is a minimal invasive therapy in the treatment of bone metastases. In this study we present a new ablation system enabling an ablation in multiple directions and with an adaptable size and shape.
Material and methods: VX-2 tumor was used for the induction of experimental bone metastases in the femur of six New Zealand white rabbits. X-ray imaging as well as CT and MRI scans before and after treatment was carried out. After detecting bone tumor, radiofrequency ablation was performed. The ablation instrument contained a 10 g bipolar, articulated extendable electrode and a proprietary generator with an impedance controlled algorithm. All bones and the soft tissue were examined histologically.
Results: All animals developed local bone tumor. Mean duration until first osteolytic lesions on CT-scans was 48±14 days. The mean lesion area was 26 mm(2). No systemic tumor spread was seen. 6 radiofrequency procedures were carried out with a mean application time of 6 min±2:30 and an average temperature in the region of effect of 55 °C±4. MRI imaging demonstrated an ablation zone of 23±6 mm around the electrode. Histopathology showed an extensive heat necrosis with no remaining tumor cells in the ablation area.
Conclusion: Radiofrequency ablation is a quickly developing treatment option on the field of minimal invasive bone tumor therapy. The electrode enables an ablation adapted to size and shape of the metastases. Further clinical studies are necessary to test and enhance this radiofrequency system.
In context of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), patients with certain comorbidities and high age, as well as male sex are considered to represent the risk group for severe course of disease. Corona-virus disease 2019 (COVID-19) typical CT-patterns include bilateral, peripheral ground glass opacity (GGO), septal thickening, bronchiectasis, consolidation as well as associated pleural effusion. We report a 77-year-old heart transplanted patient with confirmed COVID-19 infection and coronary heart disease, diabetes type II and other risk factors. Notably, only slight clinical symptoms were reported and repeated computed tomography (CT) scans showed an atypical course of CT findings during his hospitalization.
In combination with radiotherapy, immunotherapy is becoming an increasingly used strategy in treating advanced, recurrent, or metastatic cancer. The evident impact of radiotherapy on local and systemic immune response is an indication of the synergistic effect of these two modalities. There is a strong rationale to combine radiotherapy and immunotherapy to enhance response rates and overcome resistances. Therefore, the combination of radio- and immunotherapy holds a variety of opportunities as well as challenges in treating primary cancer and is progressively tested in curative settings. Brachytherapy is also known as internal radiation therapy and only offers a local therapy option at first glance: due to tumor-specific antigens, released by a high local radiation dose, a systemic immune response could be plausible and eminent. Accordingly, brachytherapy could be an underestimated partner with immuno-therapeutic approaches in both curative and palliative settings, to generate local and systemic response. In this review, we summarized the potential benefit of a potential combination of brachytherapy and immuno-therapeutic approaches vs. the background of limited data.
Background: Addition of temozolomide (TMZ) to radiotherapy (RT) improves overall survival (OS) in patients with glioblastoma (GBM), but previous studies suggest that patients with tumors harboring an unmethylated MGMT promoter derive minimal benefit. The aim of this open-label, phase III CheckMate 498 study was to evaluate the efficacy of nivolumab (NIVO) + RT compared with TMZ + RT in newly diagnosed GBM with unmethylated MGMT promoter.
Methods: Patients were randomized 1:1 to standard RT (60 Gy) + NIVO (240 mg every 2 weeks for eight cycles, then 480 mg every 4 weeks) or RT + TMZ (75 mg/m2 daily during RT and 150–200 mg/m2/day 5/28 days during maintenance). The primary endpoint was OS.
Results: A total of 560 patients were randomized, 280 to each arm. Median OS (mOS) was 13.4 months (95% CI, 12.6 to 14.3) with NIVO + RT and 14.9 months (95% CI, 13.3 to 16.1) with TMZ + RT (hazard ratio [HR], 1.31; 95% CI, 1.09 to 1.58; P = .0037). Median progression-free survival was 6.0 months (95% CI, 5.7 to 6.2) with NIVO + RT and 6.2 months (95% CI, 5.9 to 6.7) with TMZ + RT (HR, 1.38; 95% CI, 1.15 to 1.65). Response rates were 7.8% (9/116) with NIVO + RT and 7.2% (8/111) with TMZ + RT; grade 3/4 treatment-related adverse event (TRAE) rates were 21.9% and 25.1%, and any-grade serious TRAE rates were 17.3% and 7.6%, respectively.
Conclusions: The study did not meet the primary endpoint of improved OS; TMZ + RT demonstrated a longer mOS than NIVO + RT. No new safety signals were detected with NIVO in this study. The difference between the study treatment arms is consistent with the use of TMZ + RT as the standard of care for GBM.

Two-person neuroscience (2 PN) is a recently introduced conceptual and methodological framework used to investigate the neural basis of human social interaction from simultaneous neuroimaging of two or more subjects (hyperscanning). In this study, we adopted a 2 PN approach and a multiple-brain connectivity model to investigate the neural basis of a form of cooperation called joint action. We hypothesized different intra-brain and inter-brain connectivity patterns when comparing the interpersonal properties of joint action with non-interpersonal conditions, with a focus on co-representation, a core ability at the basis of cooperation. 32 subjects were enrolled in dual-EEG recordings during a computerized joint action task including three conditions: one in which the dyad jointly acted to pursue a common goal (joint), one in which each subject interacted with the PC (PC), and one in which each subject performed the task individually (Solo).
A combination of multiple-brain connectivity estimation and specific indices derived from graph theory allowed to compare interpersonal with non-interpersonal conditions in four different frequency bands. Our results indicate that all the indices were modulated by the interaction, and returned a significantly stronger integration of multiple-subject networks in the joint vs. PC and Solo conditions. A subsequent classification analysis showed that features based on multiple-brain indices led to a better discrimination between social and non-social conditions with respect to single-subject indices. Taken together, our results suggest that multiple-brain connectivity can provide a deeper insight into the understanding of the neural basis of cooperation in humans.
Background: Glucagon-like peptide-1 receptor agonists may be a treatment option in patients with non-alcoholic fatty liver disease (NAFLD). Aims: To investigate the effects of semaglutide on liver stiffness and liver fat in subjects with NAFLD using non-invasive magnetic resonance imaging (MRI) methods. Methods: This randomised, double-blind, placebo-controlled trial enrolled subjects with liver stiffness 2.50-4.63 kPa by magnetic resonance elastography (MRE) and liver steatosis ≥10% by MRI proton density fat fraction (MRI-PDFF). The primary endpoint was change from baseline to week 48 in liver stiffness assessed by MRE. Results: Sixty-seven subjects were randomised to once-daily subcutaneous semaglutide 0.4 mg (n = 34) or placebo (n = 33). Change from baseline in liver stiffness was not significantly different between semaglutide and placebo at week 48 (estimated treatment ratio 0.96 (95% CI 0.89, 1.03; P = 0.2798); significant differences in liver stiffness were not observed at weeks 24 or 72. Reductions in liver steatosis were significantly greater with semaglutide (estimated treatment ratios: 0.70 [0.59, 0.84], P = 0.0002; 0.47 [0.36, 0.60], P < 0.0001; and 0.50 [0.39, 0.66], P < 0.0001) and more subjects achieved a ≥ 30% reduction in liver fat content with semaglutide at weeks 24, 48 and 72, (all P < 0.001). Decreases in liver enzymes, body weight and HbA1c were also observed with semaglutide. Conclusions: The change in liver stiffness in subjects with NAFLD was not significantly different between semaglutide and placebo. However, semaglutide significantly reduced liver steatosis compared with placebo which, together with improvements in liver enzymes and metabolic parameters, suggests a positive impact on disease activity and metabolic profile. Background: Berotralstat (BCX7353) is a recently approved, oral, once-daily kallikrein inhibitor for hereditary angioedema (HAE) prophylaxis. In the APeX-2 trial, berotralstat reduced HAE attack rates over 24 weeks, with a favorable safety and tolerability profile.
Objective: Evaluate berotralstat safety, tolerability, and effectiveness over 48 weeks.
Methods: APeX-2 is a phase 3, parallel-group, multicenter trial (NCT03485911) in patients with HAE due to C1 esterase inhibitor deficiency. Part 1 was double-blind and placebo-controlled, with patients randomized to 24 weeks of berotralstat 150 mg, 110 mg, or placebo. In part 2, patients continued berotralstat the same dose or, if initially randomized to placebo, were rerandomized to berotralstat 150 mg or 110 mg through weeks 24 to 48. The primary end point was safety and tolerability.
Results: One hundred eight patients received 1 or more doses of berotralstat in part 2. Treatment-emergent adverse events (TEAEs) occurred in 30 of 39 patients (77%) in the placebo group during part 1, and 25 of 34 patients (74%) re-randomized from placebo to berotralstat 110 mg or 150 mg in part 2, with drug-related TEAEs in 13 of 39 (33%), and 11 of 34 (32%) in the same groups. Most TEAEs were mild or moderate, with no serious drug-related TEAEs. The most common TEAEs were upper respiratory tract infections, abdominal pain, diarrhea, and vomiting. Mean (±standard error of the mean) monthly attack rates at baseline and week 48 were 3.06 (±0.25) and 1.06 (±0.25) in the berotralstat 150mg 48-week group and 2.97 (±0.21) and 1.35 (±0.33) in the berotralstat 110mg 48-week group.
Conclusions: The safety, tolerability, and effectiveness of berotralstat were maintained over 48 weeks of treatment.
The rapid detection of pathogens in infected wounds can significantly improve the clinical outcome. Wound exudate, which can be collected in a non-invasive way, offers an attractive sample material for the detection of pathogens at the point-of-care (POC). Here, we report the development of a nucleic acid lateral flow immunoassay for direct detection of isothermally amplified DNA combined with fast sample preparation. The streamlined protocol was evaluated using human wound exudate spiked with the opportunistic pathogen Pseudomonas aeruginosa that cause severe health issues upon wound colonization. A detection limit of 2.1 × 105 CFU per mL of wound fluid was achieved, and no cross-reaction with other pathogens was observed. Furthermore, we integrated an internal amplification control that excludes false negative results and, in combination with the flow control, ensures the validity of the test result. The paper-based approach with only three simple hands-on steps has a turn-around time of less than 30 min and covers the complete analytical process chain from sample to answer. This newly developed workflow for wound fluid diagnostics has tremendous potential for reliable pathogen POC testing and subsequent target-oriented therapy.
Objectives: We explore the importance of SARS-CoV-2 sentinel surveillance testing in primary care during a regional COVID-19 outbreak in Austria.
Design: Prospective cohort study.
Setting: A single sentinel practice serving 22 829 people in the ski-resort of Schladming-Dachstein.
Participants: All 73 patients presenting with mild-to-moderate flu-like symptoms between 24 February and 03 April, 2020.
Intervention: Nasopharyngeal sampling to detect SARS-CoV-2 using real-time reverse transcriptase-quantitative PCR (RT-qPCR).
Outcome measures: We compared RT-qPCR at presentation with confirmed antibody status. We split the outbreak in two parts, by halving the period from the first to the last case, to characterise three cohorts of patients with confirmed infection: early acute (RT-qPCR reactive) in the first half; and late acute (reactive) and late convalescent (non-reactive) in the second half. For each cohort, we report the number of cases detected, the accuracy of RT-qPCR, the duration and variety of symptoms, and the number of viral clades present.
Results: Twenty-two patients were diagnosed with COVID-19 (eight early acute, seven late acute and seven late convalescent), 44 patients tested SARS-CoV-2 negative and 7 were excluded. The sensitivity of RT-qPCR was 100% among all acute cases, dropping to 68.1% when including convalescent. Test specificity was 100%. Mean duration of symptoms for each group were 2 days (range 1–4) among early acute, 4.4 days (1–7) among late acute and 8 days (2–12) among late convalescent. Confirmed infection was associated with loss of taste. Acute infection was associated with loss of taste, nausea/vomiting, breathlessness, sore throat and myalgia; but not anosmia, fever or cough. Transmission clusters of three viral clades (G, GR and L) were identified.
Conclusions: RT-qPCR testing in primary care can rapidly and accurately detect SARS-CoV-2 among people with flu-like illness in a heterogeneous viral outbreak. Targeted testing in primary care can support national sentinel surveillance of COVID-19.
Sulfhydryl Groups, Methylmercury Containing Inactivator, Coenzyme Analogue Nicotinamide-(S-methylmercury-thioinosine) dinucleotide was formed by reaction of nicotin amide-(6-thiopurine) dinucleotide with methylmercury chloride. The compound exhibits coenzyme properties in the test with LDH (Km=1.5 × 10-4 м , Vmax=12500) and LADH (Km=1.7 × 10-4 м, Vmax=27) and inactivates YADH and GAPDH. From incubations with LDH and LADH the mercury containing coenzyme could be regained by column chromatography. The compound seems to be qualified for the X-ray structure analysis of the coenzyme-enzyme complex for some dehyrogenases based on the proportion of the heavy metal.
Reactive oxygen species (ROS) are important mediators of both physiological and pathophysiological signal transduction in the cardiovascular system. The effects of ROS on cellular processes depend on the concentration, localization, and duration of exposure. Cellular stress response mechanisms have evolved to mitigate the negative effects of acute oxidative stress. In this study, we investigate the short-term and long-term metabolic and transcriptomic response of human umbilical vein endothelial cells (HUVEC) to different types and concentrations of ROS. To generate intracellular H2O2, we utilized a lentiviral chemogenetic approach for overexpression of human D-amino acid oxidase (DAO). DAO converts D-amino acids into their corresponding imino acids and H2O2. HUVEC stably overexpressing DAO (DAO-HUVEC) were exposed to D-alanine (3 mM), exogenous H2O2 (10 µM or 300 µM), or menadione (5 µM) for various timepoints and subjected to global untargeted metabolomics (LC-MS/MS) and RNAseq by MACE (Massive analysis of cDNA ends). A total of 300 µM H2O2 led to pronounced changes on both the metabolic and transcriptomic level. In particular, metabolites linked to redox homeostasis, energy-generating pathways, and nucleotide metabolism were significantly altered. Furthermore, 300 µM H2O2 affected genes related to the p53 pathway and cell cycle. In comparison, the effects of menadione and DAO-derived H2O2 mainly occurred at gene expression level. Collectively, all types of ROS led to subtle changes in the expression of ribosomal genes. Our results show that different types and concentration of ROS lead to a different metabolic and transcriptomic response in endothelial cells.
Soluble guanylyl cyclase (sGC) is the major cytosolic receptor for nitric oxide (NO) that converts GTP into the second messenger cGMP in a NO-dependent manner. Other factors controlling this key enzyme are intracellular proteins such as Hsp90 and PSD95, which bind to sGC and modulate its activity, stability, and localization. To date little is known about the effects of posttranslational modifications of sGC, although circumstantial evidence suggests that reversible phosphorylation may contribute to sGC regulation. Here we demonstrate that inhibitors of protein-tyrosine phosphatases such as pervanadate and bisperoxo(1,10-phenanthroline)oxovanadate(V) as well as reactive oxygen species such as H2O2 induce specific tyrosine phosphorylation of the β1 but not of the α1 subunit of sGC. Tyrosine phosphorylation of sGCβ1 is also inducible by pervanadate and H2O2 in intact PC12 cells, rat aortic smooth muscle cells, and in rat aortic tissues, indicating that tyrosine phosphorylation of sGC may also occur in vivo. We have mapped the major tyrosine phosphorylation site to position 192 of β1, where it forms part of a highly acidic phospho-acceptor site for Src-like kinases. In the phosphorylated state Tyr(P)-192 exposes a docking site for SH2 domains and efficiently recruits Src and Fyn to sGCβ1, thereby promoting multiple phosphorylation of the enzyme. Our results demonstrate that sGC is subject to tyrosine phosphorylation and interaction with Src-like kinases, revealing an unexpected cross-talk between the NO/cGMP and tyrosine kinase signaling pathways at the level of sGC.
Das Peptidhormon Orexin (Hypokretin), das insbesondere in Neuronen des lateralen Hypothalamus synthetisiert wird, hat nach neueren Untersuchungen neben dem Einfluss auf das Essverhalten eine entscheidende Funktion im Schlaf-Wach Verhalten. In optogenetischen Untersuchungen, in denen modifizierte Zellen durch Licht aktiviert werden, konnte durch die Hochregulation von Orexin eine deutliche Wachheits- und Aktivitätszunahme der Tiere verzeichnet werden. Bei erhöhter MCH Freisetzung war eine vermehrte Schlafneigung beobachtet worden. Orexin und MCH scheinen demnach gegensinnige Funktionen in der Schlaf-Wach Regulation einzunehmen.
Mit der vorliegenden Arbeit wurden genauere Einblicke in das orexinerge System im Gehirn von zwei unterschiedlichen Mäusestämmen gewonnen. Es ist nach unserem Wissensstand die erste Arbeit, die das Reaktivitätsmuster von Orexin und MCH bei C3H- und C57BL-Mäusen im Hinblick auf Schlaf und Schlafentzug beleuchtet. Der Vergleich zwischen den Mäusestämmen ist im Besonderen interessant, weil die C57BL-Mäuse das pineale Schlafhormon Melatonin nicht bilden.
Beide Mäusestämme wurden nach Adaptation während drei unterschiedlicher Funktionszustände semiquantitativ immunhistochemisch untersucht: im Schlaf, im aktiven Zustand sowie nach 6-stündigem Schlafentzug. Nach Fixierung der Gehirne wurden die angefertigten Hirnschnitte immunhistochemisch gefärbt und mikroskopiert. Die Semi-Quantifizierung der Immunreaktivität erfolgte durch eine etablierte Bildbearbeitungsmethodik.
Das Verteilungsmuster Orexin- und MCH-ir Neurone ist zwischen den jeweiligen Mäusestämmen gleich und zeigt eine gegenseitige Innervation. Dies spricht für eine geregelte Interaktion beider Botenstoffsysteme.
Weiterhin zeigte sich eine deutliche Schlaf-physiologische Korrelation orexinerger Neurone mit der höchsten Immunreaktivität während der Wachheit. Es konnte jedoch kein Unterschied der Immunreaktivität in Bezug auf Lokalisation und Stadien zwischen C3H- und C57BL-Mäusen nachgewiesen werden, sodass davon auszugehen ist, dass die Melatonindefizienz der C57BL keine bedeutende Rolle in der zirkadianen Regulation von Orexin spielt.
Im Gegensatz zu Orexin konnte kein signifikanter Unterschied in der Immunreaktivität MCH-ir Neurone zu den unterschiedlichen Vigilanzstadien festgestellt werden.
Vermutlich spielt die relative Inaktivität von Orexin in Kombination mit aktiver Sekretion von MCH eine wichtige Rolle in der Induktion und Kontrolle von Schlaf.
Es sind noch viele Fragen offen; insbesondere die Interaktion zwischen Wachheit- und Schlaf-induzierenden Neuronen deren Regulation. Auch der Einfluss vom Nucleus suprachiasmaticus auf Oreginerge/MCHerge Neurone, sowie das Verhältnis von Melatonin zu Orexin und MCH bedarf weiterer Forschungen.
Real time 3D - Ultraschallsimulation in der Akutmedizin : Entwicklung einer neuen Technologie

In der vorliegenden Arbeit wurde die real time 3D - Ultraschallsimulation für den Einsatz in der Akutmedizin untersucht und weiterentwickelt.
Hintergrund: Die Ultraschallsimulation stellt eine neue Lernmethode für die Ultraschalluntersuchung dar. Es kann die korrekte Anlotung, die Interpretation und die Durchführung der Untersuchung patientenunabhängig trainiert werden. Damit eignet sich die Ultraschallsimulation besonders für den Bereich der Notfallsonographie, da hier aufgrund der akuten Erkrankungen nur eingeschränkte Lehrmöglichkeiten zur Verfügung stehen. Dieses Projekt setzt sich aus vier Teilen zusammen. Es sollten Module zum Training des fokussierten Assessments mit Sonographie bei Traumapatienten (FAST) und der Sonographie peripherer Nerven und Gefäße entwickelt werden. Der dritte Bereich bestand aus der technischen Integration des Ultraschallsimulators in eine herkömmliche ALS-Trainingspuppe, um dann im vierten Abschnitt die Auswirkungen der fokussierten echokardiographischen Evaluation während der Reanimation auf deren Qualität mit dem kombinierten Ultraschall-ALS-Simulator zu überprüfen.
Methodik: Zur Entwicklung der Module für die fokussierte abdominelle Sonographie bei Traumapatienten wurde das Freihandaufnahmesystem des Ultraschallsimulators mit einem high-end Ultraschallgerät gekoppelt. Die 3D-Ultraschallvolumen wurden durch Transversalanlotungen an gesunden Probanden und Patienten mit kontinuierlicher Peritonealdialyse aufgenommen und Multivolumina in einen Schaumstofftorso mithilfe eines elektromagnetischen Trackingsystems positioniert. Die Anwendbarkeit des Ultraschallsimulators wurde jeweils während eines studentischen Ultraschallseminares und eines Ultraschallkurses für Postgraduierte überprüft. Für die Module zum Training der Sonographie peripherer Nerven und Gefäße wurde mit dem oben genannten System Volumen von verschiedenen Körperregionen gesunder Patienten aufgenommen, innerhalb des Torsos positioniert und durch einen erfahrenen Ultraschallanwender mit Originalultraschallaufnahmen verglichen. Zur Integration des Ultraschallsimulators in einen ALS-Simulator musste die elektromagnetische Lokalisationseinheit des Ultraschallsimulators innerhalb des ALS Simulators so positioniert werden, dass Thoraxkompressionen weiterhin problemlos möglich sind und die Funktion des Ultraschallsimulators nicht gestört wird. Für die Überprüfung der Auswirkungen einer fokussierten Echokardiographie auf die Qualität der Reanimation führten in der Studie Rettungsassistenten, Rettungssanitäter und Studenten in 2er Teams jeweils zwei Reanimationsabläufe durch. Der FEEL Algorithmus wurde von einem in FEEL eingewiesenen Notarzt unangekündigt während der CPR angewendet. Die Variablen Eindringtiefe der Thoraxkompressionen, Frequenz der Thoraxkompressionen und das Volumen pro Maskenbeatmung wurden als Güte der CPR angesehen und ausgewertet.
Ergebnisse: Für das Training der FAST Untersuchung konnten mehrere Module mit pathologischen und normalen Sonographiebefunden des Abdomens erstellt und von Studenten sowie Ärzten am Simulator richtig interpretiert werden. Die Entwicklung von nahezu originalgetreuen Aufnahmen peripherer Nerven und Gefäße stellt eine vollkommen neue Methode zum Training der ultraschallgestützten Regionalanästhesie dar. Erstmals wurde ein Ultraschallsimulator in einen ALS-Simulator integriert und evaluiert. Beide Geräte können als Einheit komplikationslos genutzt werden. Bei der Untersuchung der Auswirkungen des FEEL Algorithmus auf die Qualität der Reanimation zeigten sich keine Unterschiede bei den analysierten Variablen.
Schlussfolgerung: Durch das Projekt konnte der Ultraschallsimulator als neue Lernmethode im Bereich der Notfallsonographie weiterentwickelt und etabliert werden. Der Simulator wird nun regelmäßig in Ultraschallkursen für Studenten und Ärzte eingesetzt, ohne das Training am realen Patienten zu ersetzen. Neben der Ausbildung kann der Ultraschallsimulator auch zur Überprüfung der Kenntnisse von Notfallsonographieanwendern im Sinne einer Qualitätssicherung und Re-Zertifizierung genutzt werden. Die Kombination aus ALS- und Ultraschallsimulator eignet sich, um eine ALS-konforme Echokardiographie zu trainieren.
Objective: The term ‘precision medicine’ describes a rational treatment strategy tailored to one person that reverses or modifies the disease pathophysiology. In epilepsy, single case and small cohort reports document nascent precision medicine strategies in specific genetic epilepsies. The aim of this multicentre observational study was to investigate the deeper complexity of precision medicine in epilepsy. Methods: A systematic survey of patients with epilepsy with a molecular genetic diagnosis was conducted in six tertiary epilepsy centres including children and adults. A standardised questionnaire was used for data collection, including genetic findings and impact on clinical and therapeutic management. Results: We included 293 patients with genetic epilepsies, 137 children and 156 adults, 162 females and 131 males. Treatment changes were undertaken because of the genetic findings in 94 patients (32%), including rational precision medicine treatment and/or a treatment change prompted by the genetic diagnosis, but not directly related to known pathophysiological mechanisms. There was a rational precision medicine treatment for 56 patients (19%), and this was tried in 33/56 (59%) and was successful (ie, >50% seizure reduction) in 10/33 (30%) patients. In 73/293 (25%) patients there was a treatment change prompted by the genetic diagnosis, but not directly related to known pathophysiological mechanisms, and this was successful in 24/73 (33%). Significance: Our survey of clinical practice in specialised epilepsy centres shows high variability of clinical outcomes following the identification of a genetic cause for an epilepsy. Meaningful change in the treatment paradigm after genetic testing is not yet possible for many people with epilepsy. This systematic survey provides an overview of the current application of precision medicine in the epilepsies, and suggests the adoption of a more considered approach.
Background: Clinical practice guidelines for patients with primary biliary cholangitis (PBC) have been recently revised and implemented for well-established response criteria to standard first-line ursodeoxycholic acid (UDCA) therapy at 12 months after treatment initiation for the early identification of high-risk patients with inadequate treatment responses who may require treatment modification. However, there are only very limited data concerning the real-world clinical management of patients with PBC in Germany. Objective: The aim of this retrospective multicenter study was to evaluate response rates to standard first-line UDCA therapy and subsequent Second-line treatment regimens in a large cohort of well-characterized patients with PBC from 10 independent hepatological referral centers in Germany prior to the introduction of obeticholic acid as a licensed second-line treatment option. Methods: Diagnostic confirmation of PBC, standard first-line UDCA treatment regimens and response rates at 12 months according to Paris-I, Paris-II, and Barcelona criteria, the follow-up cut-off alkaline phosphatase (ALP) ≤ 1.67 × upper limit of normal (ULN) and the normalization of bilirubin (bilirubin ≤ 1 × ULN) were retrospectively examined between June 1986 and March 2017. The management and hitherto applied second-line treatment regimens in patients with an inadequate response to UDCA and subsequent response rates at 12 months were also evaluated. Results: Overall, 480 PBC patients were included in this study. The median UDCA dosage was 13.2 mg UDCA/kg bodyweight (BW)/d. Adequate UDCA treatment response rates according to Paris-I, Paris-II, and Barcelona criteria were observed in 91, 71.3, and 61.3% of patients, respectively. In 83.8% of patients, ALP ≤ 1.67 × ULN were achieved. A total of 116 patients (24.2%) showed an inadequate response to UDCA according to at least one criterion. The diverse second-line treatment regimens applied led to significantly higher response rates according to Paris-II (35 vs. 60%, p = 0.005), Barcelona (13 vs. 34%, p = 0.0005), ALP ≤ 1.67 × ULN and bilirubin ≤ 1 × ULN (52.1 vs. 75%, p = 0.002). The addition of bezafibrates appeared to induce the strongest beneficial effect in this cohort (Paris II: 24 vs. 74%, p = 0.004; Barcelona: 50 vs. 84%, p = 0.046; ALP < 1.67 × ULN and bilirubin ≤ 1 × ULN: 33 vs. 86%, p = 0.001). Conclusion: Our large retrospective multicenter study confirms high response rates following UDCA first-line standard treatment in patients with PBC and highlights the need for close monitoring and early treatment modification in high-risk patients with an insufficient response to UDCA since early treatment modification significantly increases subsequent response rates of these patients.
Molecular and cellular research modalities for the study of liver pathologies have been tremendously improved over the recent decades. Advanced technologies offer novel opportunities to establish cell isolation techniques with excellent purity, paving the path for 2D and 3D microscopy and high-throughput assays (e.g., bulk or single-cell RNA sequencing). The use of stem cell and organoid research will help to decipher the pathophysiology of liver diseases and the interaction between various parenchymal and non-parenchymal liver cells. Furthermore, sophisticated animal models of liver disease allow for the in vivo assessment of fibrogenesis, portal hypertension and hepatocellular carcinoma (HCC) and for the preclinical testing of therapeutic strategies. The purpose of this review is to portray in detail novel in vitro and in vivo methods for the study of liver cell biology that had been presented at the workshop of the 8th meeting of the European Club for Liver Cell Biology (ECLCB-8) in October of 2018 in Bonn, Germany.
Internalin B–mediated activation of the membrane-bound receptor tyrosine kinase MET is accompanied by a change in receptor mobility. Conversely, it should be possible to infer from receptor mobility whether a cell has been treated with internalin B. Here, we propose a method based on hidden Markov modeling and explainable artificial intelligence that machine-learns the key differences in MET mobility between internalin B–treated and –untreated cells from single-particle tracking data. Our method assigns receptor mobility to three diffusion modes (immobile, slow, and fast). It discriminates between internalin B–treated and –untreated cells with a balanced accuracy of >99% and identifies three parameters that are most affected by internalin B treatment: a decrease in the mobility of slow molecules (1) and a depopulation of the fast mode (2) caused by an increased transition of fast molecules to the slow mode (3). Our approach is based entirely on free software and is readily applicable to the analysis of other membrane receptors.
Ependymomas encompass a heterogeneous group of central nervous system (CNS) neoplasms that occur along the entire neuroaxis. In recent years, extensive (epi-)genomic profiling efforts have identified several molecular groups of ependymoma that are characterized by distinct molecular alterations and/or patterns. Based on unsupervised visualization of a large cohort of genome-wide DNA methylation data, we identified a highly distinct group of pediatric-type tumors (n = 40) forming a cluster separate from all established CNS tumor types, of which a high proportion were histopathologically diagnosed as ependymoma. RNA sequencing revealed recurrent fusions involving the pleomorphic adenoma gene-like 1 (PLAGL1) gene in 19 of 20 of the samples analyzed, with the most common fusion being EWSR1:PLAGL1 (n = 13). Five tumors showed a PLAGL1:FOXO1 fusion and one a PLAGL1:EP300 fusion. High transcript levels of PLAGL1 were noted in these tumors, with concurrent overexpression of the imprinted genes H19 and IGF2, which are regulated by PLAGL1. Histopathological review of cases with sufficient material (n = 16) demonstrated a broad morphological spectrum of tumors with predominant ependymoma-like features. Immunohistochemically, tumors were GFAP positive and OLIG2- and SOX10 negative. In 3/16 of the cases, a dot-like positivity for EMA was detected. All tumors in our series were located in the supratentorial compartment. Median age of the patients at the time of diagnosis was 6.2 years. Median progression-free survival was 35 months (for 11 patients with data available). In summary, our findings suggest the existence of a novel group of supratentorial neuroepithelial tumors that are characterized by recurrent PLAGL1 fusions and enriched for pediatric patients.
Selecting the k best features is a common task in machine learning. Typically, a few features have high importance, but many have low importance (right-skewed distribution). This report proposes a numerically precise method to address this skewed feature importance distribution in order to reduce a feature set to the informative minimum of items. Computed ABC analysis (cABC) is an item categorization method that aims to identify the most important items by partitioning a set of non-negative numerical items into subsets "A", "B", and "C" such that subset "A" contains the "few important" items based on specific properties of ABC curves defined by their relationship to Lorenz curves. In its recursive form, the cABC analysis can be applied again to subset "A". A generic image dataset and three biomedical datasets (lipidomics and two genomics datasets) with a large number of variables were used to perform the experiments. The experimental results show that the recursive cABC analysis limits the dimensions of the data projection to a minimum where the relevant information is still preserved and directs the feature selection in machine learning to the most important class-relevant information, including filtering feature sets for nonsense variables. Feature sets were reduced to 10% or less of the original variables and still provided accurate classification in data not used for feature selection. cABC analysis, in its recursive variant, provides a computationally precise means of reducing information to a minimum. The minimum is the result of a computation of the number of k most relevant items, rather than a decision to select the k best items from a list. In addition, there are precise criteria for stopping the reduction process. The reduction to the most important features can improve the human understanding of the properties of the data set. Cancer is the leading cause of death worldwide after cardiovascular diseases. This has been the case for the last few decades despite there being an increase in the number of cancer treatments. One reason for the apparent lack of drug effectiveness might be, at least in part, due to unspecificity for tumors; which often leads to substantial side effects. One way to improve the treatment of cancer is to increase the specificity of the treatment in accordance with the concept of individualized medicine. This will help to prevent further progression of an existing cancer or even to reduce the tumor burden. Alternatively it would be much more attractive and efficient to prevent the development of cancer in the first place. Therefore, it is important to understand the risk factors and the mechanisms of carcinogenesis in detail. One such risk factor, often associated with tumorigenesis and tumor progression, is an increased abundance of reactive oxygen species (ROS) arising from an imbalance of ROS-producing and -eliminating components. A surplus of ROS can induce oxidative damage of macromolecules including proteins, lipids and DNA. In contrast, ROS are essential for an adequate signal transduction and are known to regulate crucial cellular processes like cellular quiescence, differentiation and even apoptosis. Therefore, regulated ROS-formation at physiological levels can inhibit tumor formation and progression. With this review we provide an overview on the current knowledge of redox control in cancer development and progression.
We have investigated the role of reactive oxygen species and thiol-oxidizing agents in the induction of cell death and have shown that adenocarcinoma gastric (AGS) cells respond differently to the oxidative challenge according to the signaling pathways activated. In particular, apoptosis in AGS cells is induced via the mitochondrial pathway upon treatment with thiol-oxidizing agents, such as diamide. Apoptosis is associated with persistent oxidative damage, as evidenced by the increase in carbonylated proteins and the expression/activation of DNA damage-sensitive proteins histone H2A.X and DNA-dependent protein kinase. Resistance to hydrogen peroxide is instead associated with Keap1 oxidation and rapid translocation of Nrf2 into the nucleus. Sensitivity to diamide and resistance to hydrogen peroxide are correlated with GSH redox changes, with diamide severely increasing GSSG, and hydrogen peroxide transiently inducing protein-GSH mixed disulfides. We show that p53 is activated in response to diamide treatment by the oxidative induction of the Trx1/p38(MAPK) signaling pathway. Similar results were obtained with another carcinoma cell line, CaCo2, indicating that these findings are not limited to AGS cells. Our data suggest that thiol-oxidizing agents could be exploited as inducers of apoptosis in tumor histotypes resistant to ROS-producing chemotherapeutics.
Pathologies associated with tissue ischemia/reperfusion (I/R) in highly metabolizing organs such as the brain and heart are leading causes of death and disability in humans. Molecular mechanisms underlying mitochondrial dysfunction during acute injury in I/R are tissue-specific, but their details are not completely understood. A metabolic shift and accumulation of substrates of reverse electron transfer (RET) such as succinate are observed in tissue ischemia, making mitochondrial complex I of the respiratory chain (NADH:ubiquinone oxidoreductase) the most vulnerable enzyme to the following reperfusion. It has been shown that brain complex I is predisposed to losing its flavin mononucleotide (FMN) cofactor when maintained in the reduced state in conditions of RET both in vitro and in vivo. Here we investigated the process of redox-dependent dissociation of FMN from mitochondrial complex I in brain and heart mitochondria. In contrast to the brain enzyme, cardiac complex I does not lose FMN when reduced in RET conditions. We proposed that the different kinetics of FMN loss during RET is due to the presence of brain-specific long 50 kDa isoform of the NDUFV3 subunit of complex I, which is absent in the heart where only the canonical 10 kDa short isoform is found. Our simulation studies suggest that the long NDUFV3 isoform can reach toward the FMN binding pocket and affect the nucleotide affinity to the apoenzyme. For the first time, we demonstrated a potential functional role of tissue-specific isoforms of complex I, providing the distinct molecular mechanism of I/R-induced mitochondrial impairment in cardiac and cerebral tissues. By combining functional studies of intact complex I and molecular structure simulations, we defined the critical difference between the brain and heart enzyme and suggested insights into the redox-dependent inactivation mechanisms of complex I during I/R injury in both tissues.
Für die Funktion und das Überleben von Zellen ist die Aufrechterhaltung und Regulation der Redoxhomöostase durch Produktion und Elimination von radikalen Sauerstoffspezies (ROS) von entscheidender Bedeutung. In Tumorzellen finden sich höhere basale ROS-Level als in gesunden Zellen, was jedoch trotz des vermehrten oxidativen Stresses nicht zur Zelltodinduktion führt, da kompensatorisch antioxidative Mechanismen ebenfalls gesteigert sind. Vor allem zwei antioxidative Systeme sind hauptsächlich bei der Elimination von ROS involviert: das Glutathion (GSH)-System und das Thioredoxin (TRX)-System. Hierbei spielt eine beständige Regeneration von GSH, als auch von TRX, eine wichtige Rolle, da diese bei der Reduktion von Sauerstoffradikalen verbraucht werden. Im hepatozellulären Karzinom (HCC) ist die gestörte Redoxhomöostase mit gesteigerten ROS Leveln ein wichtiger Faktor in der Karzinogenese. Das HCC wird oft erst im fortgeschrittenen, nicht mehr kurativen Stadium diagnostiziert und ist resistent gegenüber nahezu allen Formen von Chemotherapien. Dies verdeutlich die immense Bedeutung der Erforschung der teilweise unverstandenen Tumorgenese, aber auch die Notwendigkeit für die Entwicklung von neuen Therapien.
In der hier dargestellten experimentellen Arbeit gingen wir deshalb der Frage nach, ob die alleinige Inhibierung der antioxidativen Schutzmechanismen durch sog. ROS Modulatoren ausreicht, um eine Zelltodinduktion in humanen HCC-Zelllinien herbeizuführen und damit eine potenzielle neue Therapiestrategie aufzuzeigen.
Wir konnten zeigen, dass die simultane Inhibierung dieser zwei antioxidativen Hauptmechanismen im HCC durch die Kombination von Auranofin (TXR-Inhibitor) mit Buthionine-Sulfoximin (BSO, Glutathion-Inhibitor) und Erastin (indirekter Glutathion-Inhibitor) mit BSO zur ROS-abhängigen Zelltodinduktion im HCC in vitro führt. Interessant ist, dass die gesteigerten ROS-Level jedoch nicht den Zelltod im HCC induzierten, wenn nur eines der beiden antioxidativen Systeme inhibiert wurde. Offenbar ist die Tumorzelle in der Lage durch Hochregulierung anderer antioxidativer Systeme das induzierte ROS zu neutralisieren. Unsere Untersuchungen zum Wirkmechanismus der Zelltodinduktion durch die Kombinationsbehandlungen Auranofin + BSO bzw. Erastin + BSO identifizierten unerwarteterweise eine Caspasen-unabhängige, nicht-apoptotische Zelltodform, die sog Ferroptose, welche durch ROS-Produktion und Lipidperoxidierung charakterisiert ist.
Weitere Experimente konnten untermauern, dass mit der Induktion der Ferroptose durch die selektive ROS-Modulation die Apoptoseresistenz der HCC Zellen umgangen werden kann.
Mechanistisch kann diese erstmals 2012 beschriebene eisenabhängige Zelltodform, Ferroptose, durch zwei verschiedene Signalwege induziert werden: Erstens durch den kanonischen Pfad, bei welchem die Inhibierung der Glutathionperoxidase 4 (GPX4), einem Protein, welches die Zellmembran vor Lipidperoxidation schützt, eine zentrale Rolle spielt, und zweitens durch den nicht-kanonischen Pfad, welcher durch eine Anhäufung von zweiwertigem Eisen u. a. durch Aktivierung von Hämoxygenasen (HO), vermittelt durch den Transkriptionsfaktor Nuclear factor erythroid 2-related factor 2 (Nrf2), gekennzeichnet ist. Wir konnten feststellen, dass in unseren Kombinationsbehandlungen beide Pfade involviert sind und eine Herunterregulierung von GPX4 als auch eine Akkumulation von Nrf2 und Hämoxygenase-1 (HO-1) besteht.
Zusammenfassend konnte unsere Arbeit zeigen, dass mittels pharmakologischer Adressierung zweier antioxidativer Systeme die Therapieresistenz der HCC-Zellen umgangen werden kann, und dass die Induktion der Ferroptose zukünftig eine vielversprechende Therapieoption im HCC darstellen könnte.
Nucleoredoxin is a thioredoxin-like redoxin that has been recognized as redox modulator of WNT signaling. Using a Yeast-2-Hybrid screen, we identified calcium calmodulin kinase 2a, Camk2a, as a prominent prey in a brain library. Camk2a is crucial for nitric oxide dependent processes of neuronal plasticity of learning and memory. Therefore, the present study assessed functions of NXN in neuronal Nestin-NXN-/- deficient mice. The NXN-Camk2a interaction was confirmed by coimmunoprecipitation, and by colocalization in neuropil and dendritic spines. Functionally, Camk2a activity was reduced in NXN deficient neurons and restored with recombinant NXN. Proteomics revealed reduced oxidation in the hippocampus of Nestin-NXN-/- deficient mice, including Camk2a, further synaptic and mitochondrial proteins, and was associated with a reduction of mitochondrial respiration. Nestin-NXN-/- mice were healthy and behaved normally in behavioral tests of anxiety, activity and sociability. They had no cognitive deficits in touchscreen based learning & memory tasks, but omitted more trials showing a lower interest in the reward. They also engaged less in rewarding voluntary wheel running, and in exploratory behavior in IntelliCages. Accuracy was enhanced owing to the loss of exploration. The data suggested that NXN maintained the oxidative state of Camk2a and thereby its activity. In addition, it supported oxidation of other synaptic and mitochondrial proteins, and mitochondrial respiration. The loss of NXN-dependent pro-oxidative functions manifested in a loss of exploratory drive and reduced interest in reward in behaving mice.

Due to numerous mutations in the spike protein, the SARS-CoV-2 variant of concern Omicron (B.1.1.529) raises serious concerns since it may significantly limit the antibody-mediated neutralization and increase the risk of reinfections. While a rapid increase in the number of cases is being reported worldwide, until now there has been uncertainty about the efficacy of vaccinations and monoclonal antibodies. Our in vitro findings using authentic SARS-CoV-2 variants indicate that in contrast to the currently circulating Delta variant, the neutralization efficacy of vaccine-elicited sera against Omicron was severely reduced highlighting T-cell mediated immunity as essential barrier to prevent severe COVID-19. Since SARS-CoV-2 Omicron was resistant to casirivimab and imdevimab, genotyping of SARS-CoV-2 may be needed before initiating mAb treatment. Variant-specific vaccines and mAb agents may be required to treat COVID-19 due to Omicron and other emerging variants of concern.

Background: Alcohol drinking is associated with a serious risk of developing health problems as well as with a large number of traumatic injuries. Although chronic alcohol misuse is known to contribute to severe inflammatory complications, the effects of an acute alcohol misuse are still unclear. Here, the impact of acute alcohol drinking on leukocyte counts and their cellular functions were studied.
Methods: Twenty-two healthy volunteers (12 female, 10 male) received a predefined amount of a whiskey-cola mixed drink (40% v/v), at intervals of 20 min, over 4 h to achieve a blood alcohol concentration of 1‰. Blood samples were taken before drinking T0, 2 h (T2), 4 h (T4), 6 h (T6), 24 h (T24) and 48 h (T48) after starting drinking alcohol. Leukocytes, monocytes and granulocyte counts and their functions regarding the production of reactive oxidative species (ROS), phagocytosis and apoptosis were analyzed by flow cytometry.
Results: Total leukocyte counts significantly increased at T2 and T4, while granulocyte and monocyte counts decreased at T4 and T6 vs. T0. Monocytes increased significantly at T24 and T48 vs. T0. While the total number of ROS-producing leukocytes and notably granulocytes significantly increased, in parallel, the intracellular ROS intensity decreased at T2 and T6. The numbers of ROS-positive monocytes have shown a delayed modulation of ROS, with a significant reduction in the total number of ROS-producing cells at T48 and a significantly reduced intracellular ROS-intensity at T24. Phagocyting capacity of leukocytes significantly decreased at T4 and T6. In general leukocytes, and notably granulocytes demonstrated significantly increased early (T2), while monocyte exerted significantly increased late apoptosis (T24 and T48).
Conclusions: Alcohol drinking immediately impacts leukocyte functions, while the impact on monocytes occurs at even later time points. Thus, even in young healthy subjects, alcohol drinking induces immunological changes that are associated with diminished functions of innate immune cells that persist for days.

However, because of a changing legal landscape and rising interest in therapeutic utility, there is an increasing trend in (long-term) use and possibly cannabis impairment. Importantly, a growing body of evidence suggests that regular cannabis users develop tolerance to the impairing, as well as the rewarding, effects of the drug. However, the neuroadaptations that may underlie cannabis tolerance remain unclear. Therefore, this double-blind, randomized, placebo-controlled, cross-over study assessed the acute influence of cannabis on the brain and behavioral outcomes in two distinct cannabis user groups. Twelve occasional and 12 chronic cannabis users received acute doses of cannabis (300-μg/kg delta-9-tetrahydrocannabinol) and placebo and underwent ultrahigh field functional magnetic resonance imaging and magnetic resonance spectroscopy. In occasional users, cannabis induced significant neurometabolic alterations in reward circuitry, namely, decrements in functional connectivity and increments in striatal glutamate concentrations, which were associated with increases in subjective high and decreases in performance on a sustained attention task. Such changes were absent in chronic users. The finding that cannabis altered circuitry and distorted behavior in occasional, but not chronic users, suggests reduced responsiveness of the reward circuitry to cannabis intoxication in chronic users. Taken together, the results suggest a pharmacodynamic mechanism for the development of tolerance to cannabis impairment, of which is important to understand in the context of the long-term therapeutic use of cannabis-based medications, as well as in the context of public health and safety of cannabis use when performing day-to-day operations.

Ziel: Obwohl es eine Fülle an Literatur zur Messung der Jodaufnahme mittels Dual-Energy-CT bei unterschiedlichen Pathologien gibt, wurden bisher noch keine zuverlässigen physiologischen Referenzwerte für diese Messtechnik bestimmt. Aus diesem Grund wurde die Jodaufnahme von morphologisch gesunden abdominellen Organen sowie Organen des Beckens in einer großen Studienkohorte untersucht.
Material und Methoden: Aufeinanderfolgende portalvenöse abdominelle DECT-Aufnahmen wurden überprüft und morphologisch gesunde Untersuchungsunterlagen wurden eingeschlossen (n=599; weiß/asiatisch=574, durchschnittliches Alter=58 ± 16,6Jahre; 263w/ 336m). ROI-Messungen wurden in den folgenden anatomischen Regionen durchgeführt (Anzahl der ROIs): Leber (9), Pankreas (3), Milz (3), Nebenniere (2), Nieren (6), Prostata (4), Uterus (2), Blasenwand (1) und Lymphknoten (3). Die Jodaufnahme wurde zwischen unterschiedlichen Organen verglichen und eine Subgruppenanalyse fand statt (jung vs alt/ männlich vs weiblich/niedriger BMI vs. hoher BMI).
Ergebnisse: Die gesamte Jodaufnahme war wie folgt (mg/ml): Leber=1,96 ± 0,59, Pankreas=2,07 ± 0,58, Milz=2,45 ± 0,66, Nebenniere=1,61 ± 0,43, Nieren=6,1 ± 1,32, Prostata=1,1 ± 0,53, Uterus=1,01 ± 0,68, Blasenwand=0,69 ± 0,26 und Lymphknoten=0,71 ± 0,28.
Bei Frauen kommt es zu einer höheren Jodaufnahme in der Leber (2,12 ± 0,54 vs 1,84 ± 0,43 mg/ml), Pankreas (2,27 ± 0,54 vs 1,91±0,46 mg/ml), Milz (2,7 ± 0,64 vs 2,3 ± 0,51 mg/ml), Nebenniere (1,72 ± 0,47 vs 1,52 ± 0,39 mg/ml) und Niere (6,6 ± 1,31 vs 5,65 ± 1,1 mg/ml) als bei Männern (P < 0,001). In älteren Patienten nimmt die Jodaufnahme in der Leber (1,80 ± 0,45 vs 2,04 ± 0,52 mg/ml), im Pankreas (1,87 ± 0,46 vs. 2,21 ± 0,52 mg/ml) und der Milz (2,21 ± 0,46 vs 2,64 ± 0,65 mg/ml) verglichen mit jüngeren Studienteilnehmern zu (P ≤ 0.040). Einzig im Uterus präsentierten sich niedrigere Werte in älteren Frauen (01,31 ± 0,72 vs 0,75 ± 0,46 mg/ml, P < 0,001). Bei Patienten mit einem hohen BMI kommt es in der Leber (2,32 ± 0,54 vs. 1,84 ± 0,45 mg/ml), im Pankreas (2,54 ± 0,67 vs. 1,95 ± 0,48 mg/ml), in der Milz (2,85 ± 0,66 vs. 2,32 ± 0,55 mg/ml) und in den Nebennieren (1,79 ± 0,38 vs. 1,56 ±0,44 mg/ml) zu einer signifikanten Abnahme der Jodaufnahme (P < 0,001).
Fazit: Die physiologische Jodaufnahme zeigt alters-, geschlechts- und BMI-abhängige Unterschiede für Leber, Pankreas und Milz. Pankreas und Nebennieren zeigen einen höheren Jod Durchfluss in Frauen. Während die Perfusion des Prostataparenchyms sich unverändert über die gesamte Altersspanne zeigt, nimmt die Jodaufnahme des Uterus bei Frauen im Laufe des Lebens ab. Lymphknoten als auch die Blase zeigen sich unbeeinflusst von demographischen Faktoren sowie von der Veränderung des BMI.
Klinische Relevanz: Wir definierten physiologische Referenzwerte für den statischen Durchfluss der abdominellen Organe wie angegeben mittels DECT-Jodaufnahme-Messungen in einer großen Studienkohorte und beschrieben die zusammenhängenden Unterschiede in Bezug auf Alter und Geschlecht. Dies soll die zuverlässige klinische Anwendung erleichtern und zu einer potentiellen Implementierung in zukünftige Richtlinien führen.
Purpose: To analyze refractive and topographic changes secondary to Descemet membrane endothelial keratoplasty (DMEK) in pseudophakic eyes with Fuchs’ endothelial dystrophy (FED). Methods: Eighty-seven pseudophakic eyes of 74 patients who underwent subsequent DMEK surgery for corneal endothelial decompensation and associated visual impairment were included. Median post-operative follow-up time was 12 months (range: 3–26 months). Main outcome measures were pre- and post-operative manifest refraction, anterior and posterior corneal astigmatism, simulated keratometry (CASimK) and Q value obtained by Scheimpflug imaging. Secondary outcome measures included corrected distance visual acuity (CDVA), central corneal densitometry, central corneal thickness, corneal volume (CV), anterior chamber volume (ACV) and anterior chamber depth (ACD). Results: After DMEK surgery, mean pre-operative spherical equivalent (± SD) changed from + 0.04 ± 1.73 D to + 0.37 ± 1.30 D post-operatively (p = 0.06). CDVA, proportion of emmetropic eyes, ACV and ACD increased significantly during follow-up. There was also a significant decrease in posterior corneal astigmatism, central corneal densitometry, central corneal thickness and corneal volume over time (p = 0.001). Only anterior corneal astigmatism and simulated keratometry (CASimK) remained fairly stable after DMEK. Conclusion: Despite tendencies toward a hyperopic shift, changes in SE were not significant and refraction remained overall stable in pseudophakic patients undergoing DMEK for FED. Analysis of corneal parameters by Scheimpflug imaging mainly revealed changes in posterior corneal astigmatism pointing out the relevance of posterior corneal profile changes during edema resolution after DMEK.
Ziel unserer retrospektiven Studie war es, die refraktiven und kornealen Veränderungen nach DMEK bei pseudophaken Patienten, die sich auf Grund einer endothelialen Hornhauterkrankung behandeln ließen, zu untersuchen. Durch unsere einheitlich pseudophake Patientenkohorte wollten wir untersuchen, ob sich die refraktiven Veränderungen nach DMEK von den bereits bekannten Änderungen bei einer simultan durchgeführten Katarakt- und DMEK-Operation sogenannte „Triple“-DMEK unterscheiden. Primärer Endpunkt der Studie war die Veränderung der Refraktion unter besonderer Berücksichtigung des sphärischen Äquivalents (SEQ) des jeweiligen pseudophaken Auges nach DMEK. Sekundäre Endpunkte umfassten die Entwicklung des Visus, der CCT, der ECD und verschiedener kornealer Parameter, die mittels Scheimpflug- Tomographie ermittelt wurden.
In der vorliegenden Arbeit erfolgte hierzu die retrospektive Analyse von Daten, die in den Patientenakten dokumentiert und digital gespeichert waren (Pentacam® HR). Es wurden 109 Augen von 95 Patienten, die sich im Zeitraum von Februar 2015 bis Dezember 2018 mittels DMEK in unserem Zentrum behandeln ließen, in die Studie eingeschlossen. Davon stammten 66 Augen (61%) von weiblichen und 43 Augen (39%) von männlichen Patienten. Es handelte sich bei 61 Augen (56%) um ein linkes und bei 48 Augen (44 %) um ein rechtes Auge. Die Patienten waren 20 bis 91 Jahre alt. Das mittlere Alter zum Zeitpunkt der DMEK-Operation betrug 71,9 Jahre (SD ±10,23). Der Altersmittelwert der Männer lag bei 70,4 Jahren (SD 11,23, Spannweite: 20-84) und der der Frauen bei 72,9 Jahren (SD 9,49, Spannweite: 47-91). Der mittlere Nachbeobachtungszeitraum betrug 10,47 Monate (SD 6,78, Spannweite: 1-28 Monate).
Für das SEQ konnte bei Betrachtung aller ausgewerteten Daten eine leichte Tendenz in Richtung eines „hyperopen shifts“ mit einer mittleren Veränderung des SEQ von + 0,1 D gezeigt werden, die jedoch nicht statistisch signifikant war. Zwischenzeitlich kam es im Nachbeobachtungszeitraum zu einer klaren Tendenz hinsichtlich einer Myopisierung bei Betrachtung aller ausgewerteten Daten. Der „hyperope shift“ konnte erst am Ende, möglicherweise infolge einer deutlich reduzierten Patientenzahl, beobachtet werden. In der Subgruppe „vollständige Kontrollen“ für das SEQ zeigte sich eine Tendenz hinsichtlich einer leichten Abnahme des SEQ, die jedoch ebenfalls nicht statistisch signifikant war und aufgrund der geringen Gruppengröße (n=32) kritisch betrachtet werden sollte. In unserer Kohorte konnte somit keine eindeutige Aussage über eine postoperative Änderung des SEQ in Richtung eines „hyperopen shifts“ oder „myopen shifts“ gemacht werden. Die Refraktion verhielt sich in unserem Patientenkollektiv nach DMEK insgesamt weitestgehend stabil. Insofern sind refraktive "Überraschungen", wie sie weiterhin häufig nach "Triple"-DMEK zu beobachten sind, bei zuvor pseudophakisierten Patienten in einem weitaus geringeren Maße zu erwarten. Unter den mittels Scheimpflug-Technologie untersuchten kornealen Parametern wies lediglich der posteriore Astigmatismus signifikante Veränderungen im Sinne einer Reduktion der kornealen Krümmung auf. Indirekt heben unsere Ergebnisse damit die Bedeutung des posterioren Hornhautprofils auf die postoperative Refraktionsentwicklung und somit auch auf die IOL-Kalkulation bei "Triple"-DMEK-Prozeduren hervor. Außerdem scheint der postoperative Anstieg der Sehschärfe mit den gleichfalls signifikanten Änderungen der kornealen Densitometrie in der 2-6 mm Zone, des Hornhautvolumens und der zentralen Hornhautdicke umgekehrt korreliert zu sein.
Darüber hinaus lässt sich festhalten, dass auch unsere Untersuchung in einem gewissen Maße die Überlegenheit der DMEK gegenüber etablierten Techniken, wie beispielsweise der PK verdeutlicht. Sowohl anhand der refraktiven Stabilität als auch der Visusergebnisse konnten wir belegen, dass das DMEK-Verfahren nach der anfänglichen Lernkurve die wohl besten funktionellen Ergebnisse in der Behandlung von Patienten mit endothelialen Hornhauterkrankungen liefert. Besonders bei bereits pseudophaken Patienten weist die DMEK durch die zu erwartende hohe postoperative refraktive Stabilität viele Vorteile auf
und erscheint insbesondere hinsichtlich der Vorhersagbarkeit des postoperativen refraktiven Ergebnisses der „Triple“- DMEK überlegen.
Refraktives und visuelles Ergebnis nach Neuausrichtung von rotierten torischen Intraokularlinsen

Hintergrund und Ziel der Studie
Das Ziel der Studie ist die Beurteilung des refraktiven und visuellen Ergebnisses von Patienten mit fehlausgerichteten torischen IOLs nach operativer Neuausrichtung, mit und ohne Rückberechnung der torischen Achse nach Implantation der IOL.
Methoden
Dies ist eine retrospektive Analyse von 39 Patienten, die sich von August 2013 bis Dezember 2019 an der Klinik für Augenheilkunde der Goethe-Universität Frankfurt am Main einer zweiten Operation zur Neuausrichtung einer fehlausgerichteten torischen IOL unterzogen haben. Die zweite Operaton zur Korrektur der Achsenlage wurde auf zwei Weisen durchgeführt: In der ersten Gruppe wurde auf die primär berechnete Achse rotiert und in der zweiten Gruppe wurde die Rotation auf eine neu berechnete Achse mit dem Rückrechner von astigmatisfix.com vorgenommen.
Ergebnisse
In dieser retrospektiven Studie wurden 39 Augen von 39 Patienten eingeschlossen. Die Ergebnisse nach der ersten Operation (die Linsenoperation) zeigte eine Besserung der (BCVA) von 0,28 ± 0,22 logMAR (20/40) auf 0,15 ± 0,14 logMAR (20/32). Nach der Linsenoperation zeigten die torischen IOLs durchschnittlich eine postoperative Fehlstellung von 25,69° ± 26,06°. Verglichen vor und nach der Rotation der torischen IOL blieb der BCVA unverändert bei 0,14 ± 0,14 logMAR (20/32), während sich der UDVA signifikant von 0,39 ± 0,29 logMAR (20/50) auf 0,27 ± 0,18 logMAR (20/40) verbesserte. Das refraktive Ergebnis zeigte eine Reduktion sowohl in der Restsphäre sowie in dem Zylinder. Die Ergebnisse der Gruppenanalyse demonstrierte für die erste Gruppe (Neuausrichtung auf die präoperativ berechnete Achse) eine postoperative UDVA 0,24 ± 0,16 logMAR mit einem Zylinder von 0,90 ± 0,90 D. In der zweiten Gruppe (Ausrichtung auf eine zurückberechnete Achse) betrug die UDVA 0,32 ± 0,20 logMAR mit einem Zylinder von 0,76 ± 0,72 D. Die IOLs mit hoher Zylinderstärke (≥ 2 D) zeigten bei der Rückberechnung eine stärkere Abnahme des Restzylinders als IOL mit niedriger Zylinderstärke (< 2 D) (27 % vs. 9 %). Der mittlere sphärische Äquivalent-Vorhersagefehler der Rückberechnung betrug 0,54 ± 0,55 D.
Schlussfolgerung
Die Neuausrichtung von fehlausgerichteten torischen IOLs verbessert die Sehschärfe und reduziert verbleibende Brechungsfehler. Insbesondere bei IOL mit hoher Zylinderstärke kann ein besseres refraktives Ergebnis erzielt werden, wenn vor der Neuausrichtung eine Rückberechnung durchgeführt wird.
Background: The treatment of severely injured patients, especially in older age, is complex, and based on strict guidelines. Methods: We conducted a retrospective study by analyzing our internal registry for mortality risk factors in deceased trauma patients. All patients that were admitted to the trauma bay of our level-1-trauma center from 2014 to 2018, and that died during the in-hospital treatment, were included. The aim of this study was to carry out a quality assurance concerning the initial care of severely injured patients. Results: In the 5-year period, 135 trauma patients died. The median (IQR) age was 69 (38–83) years, 71% were male, and the median (IQR) Injury Severity Score (ISS) was 25 (17–34) points. Overall, 41% of the patients suffered from severe traumatic brain injuries (TBI) (AIShead ≥ 4 points). For 12.7%, therapy was finally limited owing to an existing patient’s decree; in 64.9% with an uncertain prognosis, a ‘therapia minima’ was established in consensus with the relatives. Conclusion: Although the mortality rate was primarily related to the severity of the injury, a significant number of deaths were not exclusively due to medical reasons, but also to a self-determined limitation of therapy for severely injured geriatric patients. The conscientious documentation concerning the will of the patient is increasingly important in supporting medical decisions.
Background: Experienced and anticipated regret influence physicians’ decision-making. In medicine, diagnostic decisions and diagnostic errors can have a severe impact on both patients and physicians. Little empirical research exists on regret experienced by physicians when they make diagnostic decisions in primary care that later prove inappropriate or incorrect. The aim of this study was to explore the experience of regret following diagnostic decisions in primary care.
Methods: In this qualitative study, we used an online questionnaire on a sample of German primary care physicians. We asked participants to report on cases in which the final diagnosis differed from their original opinion, and in which treatment was at the very least delayed, possibly resulting in harm to the patient. We asked about original and final diagnoses, illness trajectories, and the reactions of other physicians, patients and relatives. We used thematic analysis to assess the data, supported by MAXQDA 11 and Microsoft Excel 2016.
Results: 29 GPs described one case each (14 female/15 male patients, aged 1.5–80 years, response rate < 1%). In 26 of 29 cases, the final diagnosis was more serious than the original diagnosis. In two cases, the diagnoses were equally serious, and in one case less serious. Clinical trajectories and the reactions of patients and relatives differed widely. Although only one third of cases involved preventable harm to patients, the vast majority (27 of 29) of physicians expressed deep feelings of regret.
Conclusion: Even if harm to patients is unavoidable, regret following diagnostic decisions can be devastating for clinicians, making them ‘second victims’. Procedures and tools are needed to analyse cases involving undesirable diagnostic events, so that ‘true’ diagnostic errors, in which harm could have been prevented, can be distinguished from others. Further studies should also explore how physicians can be supported in dealing with such events in order to prevent them from practicing defensive medicine.
Die Endothelzellmigration ist ein wesentlicher Prozess für die Angiogenese, Neovaskularisierung und Reendothelialisierung. Im ersten Teil der Arbeit wurde der Effekt von Schubspannung auf die Endothelzellmigration, die Beteiligung der Integrine und der Integrin-abhängigen Signaltransduktionswege mittels "scratched wound assay" untersucht. Die Schubspannungs-induzierte Endothelzellmigration war signifikant durch Integrin-blockierende RGD-Peptide oder neutralisierende Antikörper gegen die Integrin-Untereinheiten α5β1 reduziert, wohingegen Antikörper gegen αvβ3 oder α2β1 keinen Effekt hatten. Die Integrin-Expression von α5 und β1 war besonders in der migrierenden Zellfront der Wunde erhöht. Passend zu der wichtigen Rolle der Integrine in der Schubspannungs-induzierten Endothelzellmigration hemmte eine Blockade des Integrin-assoziierten Adapterproteins Shc durch eine dominant negative Mutante die Schubspannungs-induzierte Zellmigration. Zusätzlich konnte gezeigt werden, dass die pharmakologische Hemmung der MAP Kinase ERK1/2 oder der PI3K die Schubspannungs-induzierte Endothelzellmigration verhinderte. Im Gegensatz dazu hatte die Hemmung der NO-Synthase keinen Effekt.
Im zweiten Teil der Arbeit wurde die VEGF-vermittelte Endothelzellmigration untersucht. Im Gegensatz zu den Befunden, dass laminare Schubspannung NO-unabhängig die Endothelzellmigration stimuliert, konnte die VEGF-vermittelte Endothelzellmigration durch NOS-Inhibitoren blockiert werden. Des weiteren wurde die Beteiligung der Akt-mediierten eNOS Phosphorylierung in der VEGF- induzierten Endothelzellmigration ebenfalls mittels "scratched wound assay" untersucht, da bekannt ist, dass Akt die eNOS über eine Phosphorylierung an Serin 1177 aktivieren kann. Die Überexpression einer dominant-negativen Akt-Mutante verhindert die VEGF-induzierte Zellmigration. Im Gegensatz dazu stimulierte die Überexpression einer konstitutiv-aktiven Akt-Mutante die Endothelzellmigration, auch in Abwesenheit von VEGF. Die Überexpression eines phosphomimetischen eNOS-Konstruktes (S1177D) führte ebenfalls zu einer verstärkten Zellmigration, wohingegen die nicht mehr phosphorylierbare und somit nicht mehr aktivierbare eNOS-Mutante (S1177A) die VEGF-induzierte Endothelzellmigration komplett hemmte.
Zusammengefasst zeigen diese Daten, dass die VEGF- und Schubspannungsinduzierte Endothelzellmigration wesentlich zu der beschleunigten Reendothelialisierung von verletztem Endothel beiträgt, wie es beispielsweise nach einer Ballondilatation der Fall ist. Es konnte gezeigt werden, dass laminarer Blutfluss über die Integrine α5β1 NO-unabhängig die Migration mediiert und dass der Wachstumsfaktor VEGF über die Protein Kinase Akt NO-abhängig die Endothelzellmigration stimuliert.
Despite increased public health awareness, atherosclerosis remains a leading cause of mortality worldwide. Significant variations in response to statin treatment have been noted among different populations suggesting that the efficacy of statins may be altered by both genetic and environmental factors. The existing literature suggests that certain long noncoding RNAs (lncRNAs) might be up- or downregulated among patients with atherosclerosis. LncRNA may act on multiple levels (cholesterol homeostasis, vascular inflammation, and plaque destabilization) and exert atheroprotective or atherogenic effects. To date, only a few studies have investigated the interplay between statins and lncRNAs known to be implicated in atherosclerosis. The current review characterizes the role of lncRNAs in atherosclerosis and summarizes the available evidence related to the effect of statins in regulating lncRNAs.
NAD(P)H oxidase, the main source of reactive oxygen species in vascular cells, is known to be regulated by redox processes and thiols. However, the nature of thiol-dependent regulation has not been established. Protein disulfide isomerase (PDI) is a dithiol/disulfide oxidoreductase chaperone of the thioredoxin superfamily involved in protein processing and translocation. We postulated that PDI regulates NAD(P)H oxidase activity of rabbit aortic smooth muscle cells (VSMCs). Western blotting confirmed robust PDI expression and shift to membrane fraction after incubation with angiotensin II (AII, 100 nm, 6 h). In VSMC membrane fraction, PDI antagonism with bacitracin, scrambled RNase, or neutralizing antibody led to 26-83% inhibition (p < 0.05) of oxidase activity. AII incubation led to significant increase in oxidase activity, accompanied by a 6-fold increase in PDI refolding isomerase activity. AII-induced NAD(P)H oxidase activation was inhibited by 57-71% with antisense oligonucleotide against PDI (PDIasODN). Dihydroethidium fluorescence showed decreased superoxide generation due to PDIasODN. Confocal microscopy showed co-localization between PDI and the oxidase subunits p22(phox), Nox1, and Nox4. Co-immunoprecipitation assays supported spatial association between PDI and oxidase subunits p22(phox), Nox1, and Nox4 in VSMCs. Moreover, in HEK293 cells transfected with green fluorescent protein constructs for Nox1, Nox2, and Nox4, each of these subunits co-immunoprecipitated with PDI. Akt phosphorylation, a known downstream pathway of AII-driven oxidase activation, was significantly reduced by PDIasODN. These results suggest that PDI closely associates with NAD(P)H oxidase and acts as a novel redox-sensitive regulatory protein of such enzyme complex, potentially affecting subunit traffic/assembling.
Die Erythropoese wird durch das Hormon Erythropoetin (EPO), welches nach Geburt größtenteils in der Niere gebildet wird, aktiviert. Die Bildung von EPO erfolgt sauerstoffabhängig über eine vermehrte Aktivität des hypoxieinduzierbaren Faktors (HIF-1), welcher die Transkription der EPO Gene reguliert. HIF-1 besteht aus zwei Untereinheiten, der HIF-1α- und der HIF-1β-Untereinheit. Das Vorhandensein von HIF-1 wird über die veränderte Stabilität der HIF-1α-Untereinheit reguliert.
Mycophenolate mofetil (MMF) ist ein Immunsuppressivum, welches eine wichtige Rolle in der Transplantationsmedizin einnimmt. Es ist ein nicht kompetitiver, reversibler Hemmer der Inosinmonophosphatdehydrogenase (IMPDH), welche für die de novo Purinsynthese notwendig ist. Lymphozyten, welche im Gegensatz zu anderen Zellen keine Möglichkeit haben Purine über einen Wiederverwertungsmechanismus bereitzustellen, sind von der IMPDH abhängig. Dieser Mechanismus ermöglicht MMF eine selektive Wirkung auf Lymphozyten ohne andere Zellen zu schädigen. Trotzdem haben einige Multicenter Studien gezeigt, dass unter MMF Therapie nach Nierentransplantation in bis zu 15% der Fälle Anämien auftreten.
Wir haben in früheren Versuchen gezeigt, dass es nach einer Behandlung der HepG2 Zellen mit 5 μM MMF über 24 Stunden zu einer spezifische Minderung der EPO Sekretion kam. Es konnte so ein Zusammenhang zwischen der vermindert EPO Sekretion unter MMF und Anämien unter immunsuppressiver Therapie mit MMF angenommen werden.
Ziel dieser Arbeit war es, mögliche Ansätze für einen Regulationsmechanismus zu untersuchen, der für die reduzierte EPO Sekretion unter MMF verantwortlich ist. Es zeigte sich, mittels PCR und Western Blot Versuchen, eine zeitabhängige Verminderung der EPO mRNA parallel zu einer reduzierten EPO Sekretion, was auf eine Regulation auf RNA Ebene hindeutet. MMF depletiert den intrazellulären Guanosinpool durch Hemmung der IMPDH. Nach Zugabe von exogenem Guanosin zu den HepG2 Zellen war die MMF induzierte Reduktion der EPO mRNA vollständig reversibel. Dies zeigt, dass der beobachtete Effekt IMPDH abhängig und somit spezifisch für die MMF Wirkung ist. Des weiteren konnten wir zeigen, dass die Stabilität des EPOs und der EPO mRNA durch MMF nicht verändert war. Allerdings zeigte sich eine Verminderung der HIF-1α-Untereinheit unter MMF nach 14 und 16 Stunden. Da keine eindeutige Abnahme der HIF-1α mRNA beobachtet werden konnte, gehen wir hier von einer Regulation auf Proteinebene aus.
Im Hinblick auf diese Ergebnisse scheint eine Substitution von EPO eine gute Möglichkeit zur Behandlung von MMF induzierten Anämien. Die Ergebnisse weisen außerdem auf eine Beteiligung des HIF-1 in der Entstehung von MMF induzierten Anämien hin und sollten Grundlage für weitere Forschungsansätze zur Klärung der Mechanismen sein. Hier ist es von Interesse, ob andere Zielgene von HIF-1 (z.B. Transferrin und dessen Rezeptor, VEGF, induzierbare NO-Synthetase und Glukose Transporter 1) ebenfalls eine verminderte Expression unter MMF Stimulation erfahren. Dies würde unsere These unterstützen, dass MMF über HIF-1 die EPO Transkription beeinflusst. Außerdem könnten genauere Untersuchungen bezüglich der Stabilität der HIF-1-alpha Untereinheit unter MMF Einfluss und Forschungen bezüglich der möglichen Beteiligung von anderen Transkriptionsfaktoren (z.B. GATA-2 und NFкB) in Zukunft wegweisend sein, um den Regulationsmechanismus von MMF induzierten Anämien weiter zu erforschen. Hier könnten weitere Erkenntnisse in Zukunft neue Möglichkeiten für medikamentöse Angriffspunkte der MMF induzierten Anämien eröffnen.
Positive-strand RNA viruses such as hepatitis C virus (HCV) hijack key factors of lipid metabolism of infected cells and extensively modify intracellular membranes to support the viral lifecycle. While lipid metabolism plays key roles in viral particle assembly and maturation, viral RNA synthesis is closely linked to the remodeling of intracellular membranes. The formation of viral replication factories requires a number of interactions between virus proteins and host factors including lipids. The structure–function relationship of those proteins is influenced by their lipid environments and lipids that selectively modulate protein function. Here, we review our current understanding on the roles of phospholipids in HCV replication and of lipid–protein interactions in the structure–function relationship of the NS5A protein. NS5A is a key factor in membrane remodeling in HCV-infected cells and is known to recruit phosphatidylinositol 4-kinase III alpha to generate phosphatidylinositol 4-phosphate at the sites of replication. The dynamic interplay between lipids and viral proteins within intracellular membranes is likely key towards understanding basic mechanisms in the pathobiology of virus diseases, the mode of action of specific antiviral agents and related drug resistance mechanisms.
The optimal follow-up care for relapse detection in acute myeloid leukemia (AML) patients in first remission after consolidation therapy with intensive chemotherapy is not established. In this retrospective study, we evaluate the diagnostic value of an intensive relapse surveillance strategy by regular bone marrow aspirations (BMA) in these patients. We identified 86 patients with newly diagnosed non-promyelocytic AML who had reached complete remission (CR) after intensive induction and consolidation chemotherapy between 2007 and 2019. Annual relapse rates were 40%, 17%, and 2% in years 1–3, respectively. Patients in CR were surveilled by BMA scheduled every 3 months for 2 years, followed by BMA every 6 months. This surveillance regimen detected 29 of 55 relapses (53%), 11 of which were molecular relapses (20%). The remaining 26 of 55 relapses (47%) were diagnosed by non-surveillance BMA prompted by specific suspicion of relapse. Most patients showed concurrent morphological abnormalities in peripheral blood (PB) at time of relapse. Seven percent of all morphological relapses occurred without simultaneous PB abnormalities and would have been delayed without surveillance BMA. Intensified monthly PB assessment paired with BMA every 3 months during the first 2 years may be a highly sensitive relapse surveillance strategy.

Aging is associated with increased white matter hyperintensities (WMHs) and with alterations of alpha oscillations (7–13 Hz). However, a crucial question remains, whether changes in alpha oscillations relate to aging per se or whether this relationship is mediated by age-related neuropathology like WMHs. Using a large cohort of cognitively healthy older adults (N = 907, 60–80 years), we assessed relative alpha power, alpha peak frequency, and long-range temporal correlations from resting-state EEG. We further associated these parameters with voxel-wise WMHs from 3T MRI. We found that a higher prevalence of WMHs in the superior and posterior corona radiata as well as in the thalamic radiation was related to elevated alpha power, with the strongest association in the bilateral occipital cortex. In contrast, we observed no significant relation of the WMHs probability with alpha peak frequency and long-range temporal correlations. Finally, higher age was associated with elevated alpha power via total WMH volume. We suggest that an elevated alpha power is a consequence of WMHs affecting a spatial organization of alpha sources.

In this study, using a large cohort (N=907) of elderly participants (60-80 years), we assessed relative alpha power (AP), individual alpha peak frequency (IAPF) and long-range temporal correlations (LRTC) from resting-state EEG. We further associated these parameters with voxel-wise WMHs from 3T MRI. We found that higher prevalence of WMHs in the superior and posterior corona radiata was related to elevated relative AP, with strongest correlations in the bilateral occipital cortex, even after controlling for potential confounding factors. In contrast, we observed no significant relation of probability of WMH occurrence with IAPF and LRTC. We argue that the WMH-associated increase of AP reflects generalized and likely compensatory changes of AO leading to a larger number of synchronously recruited neurons.
Relationship between regional white matter hyperintensities and alpha oscillations in older adults

Objective: To investigate whether regional white matter hyperintensities (WMHs) relate to alpha oscillations (AO) in a large population-based sample of elderly individuals.
Methods: We associated voxel-wise WMHs from high-resolution 3-Tesla MRI with neuronal alpha oscillations (AO) from resting-state multichannel EEG at sensor (N=907) and source space (N=855) in older participants of the LIFE-Adult study (60–80 years). In EEG, we computed relative alpha power (AP), individual alpha peak frequency (IAPF), as well as long-range temporal correlations (LRTC) that represent dynamic properties of the signal. We implemented whole-brain voxel-wise regression models to identify regions where parameters of AO were linked to probability of WMH occurrence. We further used mediation analyses to examine whether WMH volume mediated the relationship between age and AO.
Results: Higher prevalence of WMHs in the superior and posterior corona radiata was related to elevated relative AP, with strongest correlations in the bilateral occipital cortex, even after controlling for potential confounding factors. The age-related increase of relative AP in the right temporal brain region was shown to be mediated by total WMH volume.
Conclusion: A high relative AP corresponding to increased regional WMHs was not associated with age per se, in fact, this relationship was mediated by WMHs. We argue that the WMH-associated increase of AP reflects a generalized and likely compensatory spread of AO leading to a larger number of synchronously recruited neurons. Our findings thus suggest that longitudinal EEG recordings might be sensitive to detect functional changes due to WMHs.
Objective: This study was undertaken to elicit patients' preferences for attributes characterizing antiseizure medication (ASM) monotherapy options before treatment consultation, and to explore the trade-offs patients consider between treatment efficacy and risks of side effects. Further objectives were to explore how treatment consultation may affect patient preferences, to elicit physicians' preferences in selecting treatment, and to compare patient and physician preferences for treatment.
Methods: This prospective, observational study (EP0076; VOTE) included adults with focal seizures requiring a change in their ASM monotherapy. Patients completed a discrete choice experiment (DCE) survey before and after treatment consultation. Physicians completed a similar survey after the consultation. The DCE comprised 12 choices between two hypothetical treatments defined by seven attributes. The conditional relative importance of each attribute was calculated.
Results: Three hundred ten patients (mean [SD] age = 46.8 [18.3] years, 52.3% female) were enrolled from eight European countries, of whom 305 completed the survey before consultation and 273 completed the survey before and after consultation. Overall, this preference study in patients who intended to receive a new ASM monotherapy suggests that patient preferences were ordered as expected, with better outcomes being preferred to worse outcomes; patients preferred a higher chance of seizure freedom, lower risk of developing clinical depression, and fewer severe adverse events; avoiding moderate-to-severe “trouble thinking clearly” was more important than avoiding any other side effect. There were qualitative differences in what patients and physicians considered to be the most important aspects of treatment for patients; compared with patients, physicians had a qualitatively stronger preference for greater chance of seizure freedom and avoiding personality changes. Patients' preference weights were qualitatively similar before and after treatment consultation.
Significance: For patients, seizure freedom and avoiding trouble thinking clearly were the most important treatment attributes. Physicians and patients may differ in the emphasis they place on specific attributes.
Background: Biotin, a water-soluble B vitamin, has demonstrable anti-inflammatory properties. A biotin-deficient diet induced a colitis-like phenotype in mice, alleviable by biotin substitution. Mice with dextran sulfate sodium (DSS)-induced colitis showed biotin deficiency and diminished levels of sodium-dependent multivitamin transporter, a protein involved in biotin absorption. Biotin substitution induced remission by reducing activation of NF-κB, a transcription factor involved in intestinal permeability and inflammatory bowel disease (IBD). We investigated for the first time a possible clinical role of biotin status in IBD. Methods: In a comparative, retrospective, cross-sectional study, serum samples of 138 patients with IBD (67 female; 72 Crohn’s disease (CD), 66 ulcerative colitis (UC)) aged 18–65 years and with a mean age (±SD) of 42.5 ± 14.3 years as well as 80 healthy blood donors (40 female; 40.0 ± 10.0 years; range 20–60 years) were analyzed. Inflammation was defined as hsCRP ≥5 mg/L, and to determine biotin status, serum 3-hydroxyisovaleryl carnitine (3HIVc) levels were measured by LC-MS/MS. Results: A total of 138 patients with IBD (67f; 72CD/66 UC; 42.5 ± 14.3 years) were enrolled: 83/138 had inflammation. Mean serum 3HIVc levels were significantly higher in IBD patients but unaffected by inflammation. Biotin deficiency (95th percentile of controls: >30 nmol/L 3HIVc) was significantly more common in IBD patients versus controls. Conclusion: High serum 3HIVc levels and biotin deficiency were associated with IBD but not inflammatory activity or disease type. Our findings suggest biotin may play a role as cause or effect in IBD pathogenesis. Routine assessment and supplementation of biotin may ameliorate IBD and support intestinal integrity.
Relevance of the natural HDAC inhibitor sulforaphane as a chemopreventive agent in urologic tumors

Due to an increased understanding of molecular biology and the genomics of cancer, new and potent agents have been approved by the Food and Drug Administration (FDA) to fight this disease. However, all of these drugs cause severe side effects and resistance inevitably develops, re-activating tumor growth and dissemination. For this reason, patients turn to natural compounds as alternative or complementary treatment options, since it has been found that natural plant products may block, inhibit, or reverse cancer development. The present review focusses on the role of the natural compound sulforaphane (SFN) as an anti-tumor agent in urologic cancer. SFN is a natural compound found in cruciferous vegetables from the Brassicaceae family such as broccoli, cauliflower and cabbage. Several epidemiologic and clinical studies have documented chemopreventive properties of SFN, making it an interesting candidate for additive cancer treatment. SFN shows remarkable anti-tumor effects in vitro and in vivo without exerting toxicity. The review summarizes the current understanding of SFN and provides insights into its molecular mode of action with particular emphasis on epigenetic tumor control.
Einleitung: Die Intention dieser Arbeit war es zu evaluieren, ob das Prüfungsformat eines OSPE (Objective Structured Practical Examination), durchgeführt im Fach Zahnerhaltungskunde (6. Fachsemester), Aussagen über den Studienerfolg im praktischen Teil des Staatsexamens (11. Fachsemester) im selben Fach zu treffen erlaubt. Ferner sollte unter Berücksichtigung allgemeiner Angaben der StudienteilnehmerInnen (Abitursnote, Physikumsnote, Studiendauer, Kohorte und Geschlecht) analysiert werden, ob bezüglich der Gesamt- sowie Teilnoten des OSPE und der Staatsexamensprüfung im Fach Zahnerhaltungskunde Zusammenhänge oder Unterschiede bestehen.
Methoden: Im Rahmen dieser longitudinalen, retrospektiven Studie wurden über einen Zeitraum von 11 Semestern prüfungsbezogene Daten von Studierenden (N = 223) des Fachbereichs Zahnmedizin in Frankfurt am Main erhoben und untersucht. Ferner wurden persönliche Angaben der Studienteilnehmer zu Alter, Geschlecht, Abitursnote, Physikumsnote und Studiendauer evaluiert sowie eine Zuordnung zu Kohorten vorgenommen. Für die statistische Auswertung der Daten wurden Spearman Rangkorrelationen, Partialkorrelationen, Korrelationskoeffizienten nach Pearson, und Multiple Regressionen (SPSS Statistics 21, IBM Corporation, New York) berechnet.
Ergebnisse: Die Ergebnisse zeigen, dass die Durchführung einer OSPE (Cronbachs α = .87) mit dem Erfolg im praktischen Teil des Staatsexamens im Fach Zahnerhaltungskunde korreliert (p = .01, r = .17). Als eine weitere signifikante Korrelation mit der Examensleistung erwies sich die Dauer des Studiums (p = .001, r = .23). Gemeinsam vermögen diese beiden Variablen einen signifikanten Beitrag zur Vorhersage der Examensnote (p = .001, R2 = .076) zu leisten. Das zeigte sich im größeren Umfang bei weiblichen Studierenden. Zudem wurde festgestellt, dass diese bessere Abitursnoten als männliche Studierende aufweisen (F = 6.09, p = .01, η² = .027) und dass es nur bei männlichen Studierenden eine signifikante Korrelation zwischen der Physikumsnote (Zahnärztliche Vorprüfung) und der OSPE-Benotung gab (r = .17, p = .01).
Schlussfolgerung: In der vorliegenden Untersuchung konnte der prädiktive Effekt eines durchgeführten klinischen OSPE auf die Prüfungsleistung im Staatsexamen nachgewiesen werden. Unter Berücksichtigung der Limitation der Studie empfiehlt sich aus unserer Sicht die Durchführung eines solchen Prüfungsformats im Rahmen des klinischen Studienabschnitts im 6. Semester im Fach Zahnmedizin.
Reliable and efficient recording of the error-related negativity with a speeded Eriksen Flanker task

There is accumulating evidence that the error-related negativity (ERN), an event-related potential elicited after erroneous actions, is altered in different psychiatric disorders and may help to guide treatment options. Thus, the ERN is a promising candidate as a psychiatric biomarker. Basic methodological requirements for a biomarker are standardized and reliable measurements. Additional psychiatry specific requirements are time efficiency and patient-friendliness.
The aim of the present study is to establish ERN acquisition in a reliable, time-efficient and patient-friendly way for use in clinical practice.
Healthy subjects (N=27) performed a modified Eriksen Flanker Task with adaptive reaction time window and only incongruent stimuli that maximizes the number of errors. All participants were tested for mental health by the Mini International Neuropsychiatric Interview (M.I.N.I.). The first N=12 subjects were part of a pilot study and further N=14 subjects were included for analysis (one subject was excluded due to technical problems). In a test-retest design with two sessions separated by 28 days the reliability of the ERN has been assessed. To ensure external validity, we aimed to replicate previously reported correlation patterns of ERN amplitude with (1) number of errors and (2) negative affect. State affect of each subject was measured by the Positive and Negative Affect Schedule. In order to optimize the clinical use of the task, we determined to which extent the task can be shortened while keeping reliability >0.80.
We found excellent reliability of the ERN (intraclass correlation coefficient =0.806-0.947) and replicated specific correlation patterns (ERN amplitude with relative number of errors: r=0.394; p=0.082; ERN amplitude with negative affect: r=-0.583, p=0.014). The task can be shortened to a patient-friendly and clinically feasible length of only 8 minutes keeping reliability >0.80.
To conclude, the present modified task provides reliable and efficient recording of the ERN, facilitating its use as a psychiatric biomarker.
Background: Intraoperative blood salvage (IBS) is regarded as an alternative to allogeneic blood transfusion excluding the risks associated with allogeneic blood. Currently, IBS is generally avoided in tumor surgeries due to concern for potential metastasis caused by residual tumor cells in the erythrocyte concentrate.
Methods: The feasibility, efficacy and safety aspects of the new developed Catuvab procedure using the bispecific trifunctional antibody Catumaxomab was investigated in an ex-vivo pilot study in order to remove residual EpCAM positive tumor cells from the autologous erythrocyte concentrates (EC) from various cancer patients, generated by a IBS device.
Results: Tumor cells in intraoperative blood were detected in 10 of 16 patient samples in the range of 69–2.6 × 105 but no residual malignant cells in the final erythrocyte concentrates after Catuvab procedure. IL-6 and IL-8 as pro-inflammatory cytokines released during surgery, were lowered in mean 28-fold and 52-fold during the Catuvab procedure, respectively, whereas Catumaxomab antibody was detected in 8 of 16 of the final EC products at a considerable decreased and uncritical residual amount (37 ng in mean).
Conclusion: The preliminary study results indicate efficacy and feasibility of the new medical device Catuvab allowing potentially the reinfusion of autologous erythrocyte concentrates (EC) produced by IBS device during oncological high blood loss surgery. An open-label, multicenter clinical study on the removal of EpCAM-positive tumor cells from blood collected during tumor surgery using the Catuvab device is initiated to validate these encouraging results.
Zielsetzung der vorliegenden Dissertation war die Validierung einer deutschsprachigen Version, der bereits für andere Sprachen validierten Repetitiven Verhaltensskala – Revidiert (RBS-R) für Kinder- und Jugendliche im Alter zwischen 4-17 Jahren. Die RBS-R ist ein aus 43 Items bestehendes, gut untersuchtes Screeninginstrument, welches restriktives, repetitives Verhalten (RRV) erfasst. RRV stellen einen heterogenen Symptomkomplex dar, welcher durch stereotype motorische, sprachliche und kognitive Verhaltensmuster gekennzeichnet ist.
RRV sind neben Abweichungen sozialer Interaktion und Kommunikation eines der Hauptkriterien für eine Autismus-Spektrum-Störung (ASS). RRV sind jedoch nicht nur Teil des Symptomspektrums einer ASS, sondern treten häufig auch bei anderen psychiatrischen Erkrankungen (PE), einer Intelligenzminderung (IM) oder bei altersentsprechend entwickelten Kindern (AE), beispielsweise in bestimmten Phasen des Heranwachsens auf. Bisher wurde die RBS-R fast ausschließlich an ASS untersucht. Es besteht die Notwendigkeit für ein valides und zuverlässiges Messinstrument für den deutschen Sprachraum, welches RRV hinsichtlich ihrer Heterogenität transdiagnostisch erfassen und den Schweregrad der Ausprägung ermitteln kann. Das Ziel dieser Arbeit war, dies durch eine möglichst große und heterogene Stichprobe (n= 948) mit Inklusion von Probanden mit einer ASS (n= 218), IM (n= 120), PE (n= 166) und AE (n = 444) zu gewährleisten. Zur Überprüfung der psychometrischen Eignung erfolgte nach der Adaptation der RBS-R für den deutschen Sprachraum eine Evaluation gemäß den Prinzipien der klassischen Testtheorie, insbesondere hinsichtlich Validität und Reliabilität. Darüber hinaus erfolgte unter Einbeziehung der vier Stichproben eine explorative Faktoranalyse (EFA) um eine geeignete Faktorlösung zu finden. Diese sollte sowohl den Anspruch Klinisch-Tätiger als auch Forschender erfüllen und eine Anwendung der RBS-R auch außerhalb von ASS ermöglichen. Zudem sollten RRV in den unterschiedlichen Kohorten, sowie in Bezug auf Alters- und Geschlechtseffekte bei ASS untersucht werden.
Die Bedeutung der Ergebnisse dieser Arbeit ist, ob der schon breit erforschten Eigenschaften der RBS-R, vor allem im Kontext mit den vorliegenden Studien zu betrachten. Einzigartig für diese deutschsprachige Validierung der RBS-R ist die Diversität und große Anzahl der teilnehmenden Probanden.
Die vorliegende Validierung der RBS-R erbrachte gute Ergebnisse für die Reliabilität und konvergente Validität der Skalen, vergleichbar mit den Ergebnissen vorheriger Studien. Die untersuchte Itemschwierigkeit war relativ gering, was durch die geringe Antwortvarianz in der großen Kohorte der AE erklärbar ist. Die Itemgesamtkorrelation zeigte gute Werte. Auch Items mit niedrigen Ergebnissen für die Itemzustimmung wurden nicht exkludiert. Dies geschah konkordant zum Vorgehen vorheriger Studien und hatte den Hintergrund, eine internationale Vergleichbarkeit des RBS-R beizubehalten. Die EFA bestätigte die gute Anwendbarkeit einer Vielzahl von Faktorlösungen. Wir legten eine 4-Faktorlösung für die deutschsprachige Version der RBS-R fest. Grundlage hierfür war das Bestreben die ursprünglichen Subskalen für eine internationale Vergleichbarkeit so getreu wie möglich beizubehalten. Die 4 Faktoren bildeten treffend RRV niedriger und hoher Ordnung ab. Gegen eine 5- oder 6-Faktorlösung sprach hierbei, dass dies zu Subskalen mit wenig Items und schwachen psychometrischen Eigenschaften führte (Lam et al, kritisiert durch Georgiades et al). Gegen eine 2- oder 3-Faktorlösung sprach die eingeschränkte klinische Interpretierbarkeit durch Subskalen mit vielen kumulierten Items. Hinsichtlich der Kohortenzugehörigkeit kann der RBS-R übereinstimmend mit Fulceri et al valide zwischen RRV in ASS und AE unterscheiden. Eine valide Unterscheidung zwischen ASS, PE und IM konnten wir nicht verifizieren. Betreffend der untersuchten Alterseffekts bestätigte sich, dass RRV bei ASS mit dem Alter abnehmen. Im Gegensatz zu einigen vorherigen Studien, welche keinen Geschlechtsunterschied bezüglich RRV bei ASS fanden, zeigten die männlichen Probanden der ASS Kohorte bei uns häufiger und ausgeprägter RRV.
Borders and edges are salient and behaviourally relevant features for navigating the environment. The brain forms dedicated neural representations of environmental boundaries, which are assumed to serve as a reference for spatial coding. Here we expand this border coding network to include the retrosplenial cortex (RSC) in which we identified neurons that increase their firing near all boundaries of an arena. RSC border cells specifically encode walls, but not objects, and maintain their tuning in the absence of direct sensory detection. Unlike border cells in the medial entorhinal cortex (MEC), RSC border cells are sensitive to the animal’s direction to nearby walls located contralateral to the recorded hemisphere. Pharmacogenetic inactivation of MEC led to a disruption of RSC border coding, but not vice versa, indicating network directionality. Together these data shed light on how information about distance and direction of boundaries is generated in the brain for guiding navigation behaviour.
Tensiomyography measures the radial displacement of a muscle during an electrically evoked twitch contraction. The rate of muscle displacement is increasingly reported to assess contractile properties. Several formulas currently exist to calculate the rate of displacement during the contraction phase of the maximal twitch response. However, information on the reproducibility of these formulas is scarce. Further, different rest intervals ranging from 10 s to 30 s are applied between consecutive stimuli during progressive electrical stimulation until the maximum twitch response. The effect of different rest intervals on the rate of displacement has not been investigated so far. The first aim of this study is to investigate the within and between-day reliability of the most frequently used formulas to calculate the rate of displacement. The second aim is to investigate the effect of changing the inter-stimulus interval on the rate of displacement. We will determine the rectus femoris and biceps femoris rate of displacement of twenty-four healthy subjects’ dominant leg on two consecutive days. The maximum displacement curve will be determined two times within three minutes on the first day and a third time 24 h later. On day two, we will also apply three blocks of ten consecutive stimuli at a constant intensity of 50 mA. Inter-stimuli intervals will be 10 s, 20 s or 30 s in each block, respectively, and three minutes between blocks. The order of inter-stimulus intervals will be randomized. This study will allow a direct comparison between the five most frequently used formulas to calculate the rate of displacement in terms of their reproducibility. Our data will also inform on the effect of different inter-stimulus intervals on the rate of displacement. These results will provide helpful information on methodical considerations to determine the rate of displacement and may thus contribute to a standardized approach.
The antiviral drugs tecovirimat, brincidofovir, and cidofovir are considered for mpox (monkeypox) treatment despite a lack of clinical evidence. Moreover, their use is affected by toxic side-effects (brincidofovir, cidofovir), limited availability (tecovirimat), and potentially by resistance formation. Hence, additional, readily available drugs are needed. Here, therapeutic concentrations of nitroxoline, a hydroxyquinoline antibiotic with a favourable safety profile in humans, inhibited the replication of 12 mpox virus isolates from the current outbreak in primary cultures of human keratinocytes and fibroblasts and a skin explant model by interference with host cell signalling. Tecovirimat, but not nitroxoline, treatment resulted in rapid resistance development. Nitroxoline remained effective against the tecovirimat-resistant strain and increased the anti-mpox virus activity of tecovirimat and brincidofovir. Moreover, nitroxoline inhibited bacterial and viral pathogens that are often co-transmitted with mpox. In conclusion, nitroxoline is a repurposing candidate for the treatment of mpox due to both antiviral and antimicrobial activity.

Requirements for the interaction of mouse Polkappa with ubiquitin and its biological significance

Polkappa protein is a eukaryotic member of the DinB/Polkappa branch of the Y-family DNA polymerases, which are involved in the tolerance of DNA damage by replicative bypass. Despite universal conservation through evolution, the precise role(s) of Polkappa in this process has remained unknown. Here we report that mouse Polkappa can physically interact with ubiquitin by yeast two-hybrid screening, glutathione S-transferase pulldown, and immunoprecipitation methods. The association of Polkappa with ubiquitin requires the ubiquitin-binding motifs located at the C terminus of Polkappa. In addition, Polkappa binds with monoubiquitinated proliferating cell nuclear antigen (PCNA) more robustly than with non-ubiquitinated PCNA. The ubiquitin-binding motifs mediate the enhanced association between monoubiquitinated PCNA and Polkappa. The ubiquitin-binding motifs are also required for Polkappa to form nuclear foci after UV radiation. However, the ubiquitin-binding motifs do not affect Polkappa half-life. Finally, we have examined levels of Polkappa expression following the exposure of mouse cells to benzo[a]pyrene-dihydrodiol epoxide or UVB radiation.
Prolonged treatment of leukemic cells with chemotherapeutic agents frequently results in development of drug resistance. Moreover, selection of drug-resistant cell populations may be associated with changes in malignant properties such as proliferation rate, invasiveness, and immunogenicity. In the present study, the sensitivity of cytarabine (1-β-d-arabinofuranosylcytosine, araC)-resistant and parental human leukemic cell lines (T-lymphoid H9 and acute T-lymphoblastic leukemia Molt-4) to natural killer (NK) cell-mediated killing was investigated. The results obtained demonstrate that araC-resistant H9 and Molt-4 (H9rARAC100 and Molt-4rARAC100) cell lines are more sensitive to NK cell-mediated lysis than their respective parental cell lines. This increased sensitivity was associated with a higher surface expression of ligands for the NK cell-activating receptor NKG2D, notably UL16 binding protein-2 (ULBP-2) and ULBP-3 in H9rARAC100 and Molt-4rARAC100 cell lines. Blocking ULBP-2 and ULBP-3 or NKG2D with monoclonal antibody completely abrogated NK cell lysis. Constitutive phosphorylated extracellular signal-regulated kinase (ERK) but not pAKT was higher in araC-resistant cells than in parental cell lines. Inhibition of ERK using ERK inhibitor PD98059 decreased both ULBP-2/ULBP-3 expression and NK cell cytotoxicity. Furthermore, overexpression of constitutively active ERK in H9 parental cells resulted in increased ULBP-2/ULBP-3 expression and enhanced NK cell lysis. These results demonstrate that increased sensitivity of araC-resistant leukemic cells to NK cell lysis is caused by higher NKG2D ligand expression, resulting from more active ERK signaling pathway.
Inhibitors of the mammalian target of rapamycin (mTOR) have improved the treatment of renal cell carcinoma (RCC). However, chronic drug exposure may trigger resistance, limiting the utility of these agents. The metastatic behavior of RCC cells, susceptible (RCC(par)) or resistant (RCC(res)) to the mTOR inhibitor temsirolimus, was investigated. Adhesion to vascular endothelium or immobilized collagen and fibronectin was quantified. Chemotactic motility was evaluated with a modified Boyden chamber assay. Integrin α and β subtype receptors were analyzed by flow cytometry and Western blot analysis. The physiological relevance of the integrins was then determined by blocking studies and small interfering RNA knockdown. Adhesion to endothelial cells and to fibronectin (not to collagen) and chemotaxis were enhanced in RCC(res) compared to RCC(par). RCC(res) detached from fibronectin and motile activity further increased under retreatment with low-dosed temsirolimus. α5 integrin was diminished inside the cell and at the cell surface, whereas the β3 subtype was reduced intracellularly but elevated at the plasma membrane. In RCC(par), blocking α5 surface receptors enhanced RCC-collagen but reduced RCC-fibronectin interaction, whereas the opposite was true for RCC(res). Chemotaxis of RCC(par) but not of RCC(res) was strongly diminished by the α5 antibody. Blocking β3 significantly lowered chemotaxis with stronger effects on RCC(res), compared to RCC(par). Importantly, β3 knockdown reduced chemotaxis of RCC(par) but upregulated the motile behavior of RCC(res). Temsirolimus resistance is characterized by quantitative alterations of integrin α5 and β3 expression, coupled to functional changes of the integrin molecules, and forces a switch from RCC adhesion to RCC migration.
Infektionen durch multiresistente Erreger führen jährlich zum Tod von ca. 33.000 Menschen in Europa.192 Insbesondere ist eine weltweite Zunahme von multiresistenten Gram-negativen Bakterien zu verzeichnen.193 Im Rahmen dieser Arbeit wurden zwei Projekte bezüglich der Resistenzmechanismen gegenüber Beta-Laktam-Antibiotika bei Gram-negativen Bakterien bearbeitet.
Das Gammaproteobakterium Psychrobacter sanguinis PS2578 wurde im März 2015 von einem Neonaten isoliert und verursachte eine early onset Neugeboreneninfektion. Aufgrund der insuffizienten Datenlage bezüglich Diagnostik, Antibiotikaresistenz und Pathogenität von Psychrobacter spp. wurden diese Aspekte weiter evaluiert. P. sanguinis zeigte geringes Wachstum auf Blutagar und keinerlei Wachstum in Standardnährmedien. Als optimales Nährmedium erwies sich das Spezialmedium BHI mit 10% Fetalem Kälberserum, wobei eine Abhängigkeit des Wachstums von FCS beobachtet wurde. Die Virulenz des klinischen Isolats sowie des Referenzstamms P. sanguinis DSM 23635 war in einem in vivo Infektionsmodell vergleichbar mit klinischen Escherichia coli und Klebsiella pneumoniae Isolaten sowie dem phylogenetisch nahe verwandten Acinetobacter calcoaceticus DSM 30006. Demnach ist die Spezies P. sanguinis moderat virulent und als humanpathogen anzusehen. Als molekulares Äquivalent der phänotypischen Penicillinresistenz wurde die Carbenicillinase CARB-8 (RTG-3) identifiziert, wodurch die These gestützt wird, dass der Genus Psychrobacter spp. ein mögliches Genreservoir von Carbenicillinasen darstellt.3 Im Rahmen dieser Arbeit konnte das Genom der Spezies erstmals komplett sequenziert werden. Es beinhaltete ein Chromosom von 2.946.289 bp, ein größeres Plasmid von 49.981 bp und ein kleineres Plasmid von 11.576 bp, welches blaCARB-8 kodierte.
Der Nachweis von Carbapenem-resistenten Gammaproteobacteria hat sich von 2010 zu 2017 in Deutschland mehr als verzehnfacht. Dabei ist OXA-48 die häufigste Carbapenemase in Europa und tritt vor allem bei den Spezies K. pneumoniae und E. coli auf.75 In dieser Arbeit wurden 62 klinische Stämme hinsichtlich ihres Plasmidtyps und ihres Verwandtschaftsgrads untersucht.
In der Klonalitätsanalyse gehörten 25 (n=44) K. pneumoniae derselben klonalen Linie an. Hiervon wurden 22 (n=25) Isolate in den Jahren 2010 und 2011 isoliert, was einen klonalen Ausbruch vermuten lässt.158 Bei der Spezies E. coli (n=8) waren lediglich zwei Stämme klonal verwandt. Insgesamt indizierten diese Ergebnisse eine hohe Diversität der klinischen Isolate. Die Plasmidtypisierung hingegen zeigte, dass 95% der Stämme (n=62) ein IncL Plasmid aufwiesen. Basierend auf den Selektionskriterien Klonalität und Plasmidtyp wurden 21 Stämme für weitere Analysen mittels Multilocus-Sequenz-Typisierung und Transkonjugation ausgewählt. Es konnten sehr hohe Konjugationsfrequenzen von 7,51 x 10-1 im Intraspezies- und von 8,15 x 10-1 im Intergenus- HGT für das blaOXA-48 IncL Plasmid in vitro ermittelt werden. Unter Verwendung von Galleria mellonella Larven als in vivo Transkonjugationsmodell wurde eine Transkonjugationsfrequenz von nahezu 100% detektiert. Daher lässt sich vermuten, dass der HGT von Antibiotikaresistenzgenen im Darm eines Patienten eine sehr viel höhere Effizienz aufweisen könnte, als bisher basierend auf in vitro generierten Daten angenommen. Dies impliziert, dass die globale Verbreitung von OXA-48 auf dem effizienten horizontalen Gentransfers eines einzigen IncL Plasmids beruht und nicht auf der Expansion einer bestimmten klonalen Linie.
The aim of this study was to detect a response difference in primary (PLC) and secondary liver tumors (SLC) with magnetic resonance elastography (MRE) after TACE therapy. Thirty-one patients (25/31 male; mean age 69.6 years [range: 39–85 years]) with repeated TACE therapy of HCC were compared with twenty-seven patients (27/27 female; mean age 61.2 years [range 39–81 years]) with repeated TACE therapy of metastatic liver disease due to breast cancer. Both groups underwent either one (n = 31) or two (n = 27) repetitive magnetic resonance imaging (MRI) and MRE exams in 4- to 6-week intervals using a 1.5-T-scanner. MRE-based liver stiffness and size measurements were evaluated in tumorous lesions and in healthy liver lobe controls. PLC showed a significantly larger tumor size compared to SLC (26.4 cm2 vs. 11 cm2, p = 0.007) and a higher degree of stiffness (5.8 kPa vs. 5.1 kPa, p = 0.04). Both tumors decreased in size during the cycles (PLC: p = 0.8 and SLC: p < 0.0001) and lesions showed an increase in stiffness (PLC: p = 0.002 and SLC: p = 0.006). MRE demonstrates that PLC and SLC have similar responses to TACE therapy. PLC had a greater increase in stiffness and SLC got smaller. An increasing stiffness and decrease in size could show a good response.
Response to upfront azacitidine in juvenile myelomonocytic leukemia in the AZA-JMML-001 trial

Allogeneic hematopoietic stem cell transplantation (HSCT) is the only curative therapy for most children with juvenile myelomonocytic leukemia (JMML). Novel therapies controlling the disorder prior to HSCT are needed. We conducted a phase 2, multicenter, open-label study to evaluate the safety and antileukemic activity of azacitidine monotherapy prior to HSCT in newly diagnosed JMML patients. Eighteen patients enrolled from September 2015 to November 2017 were treated with azacitidine (75 mg/m2) administered IV once daily on days 1 to 7 of a 28-day cycle. The primary end point was the number of patients with clinical complete remission (cCR) or clinical partial remission (cPR) after 3 cycles of therapy. Pharmacokinetics, genome-wide DNA-methylation levels, and variant allele frequencies of leukemia-specific index mutations were also analyzed. Sixteen patients completed 3 cycles and 5 patients completed 6 cycles. After 3 cycles, 11 patients (61%) were in cPR and 7 (39%) had progressive disease. Six of 16 patients (38%) who needed platelet transfusions were transfusion-free after 3 cycles. All 7 patients with intermediate- or low-methylation signatures in genome-wide DNA-methylation studies achieved cPR. Seventeen patients received HSCT; 14 (82%) were leukemia-free at a median follow-up of 23.8 months (range, 7.0-39.3 months) after HSCT. Azacitidine was well tolerated and plasma concentration-–time profiles were similar to observed profiles in adults. In conclusion, azacitidine monotherapy is a suitable option for children with newly diagnosed JMML. Although long-term safety and efficacy remain to be fully elucidated in this population, these data demonstrate that azacitidine provides valuable clinical benefit to JMML patients prior to HSCT. Results of a prospective non-interventional post-authorization safety study of idelalisib in Germany

Background: In pivotal studies, idelalisib demonstrated remarkable efficacy and manageable tolerability in patients with chronic lymphocytic leukemia (CLL) and follicular lymphoma (FL). This prospective, multicenter, non-interventional post-authorization study assessed the characteristics, clinical management, and outcome of CLL and FL patients receiving idelalisib in routine clinical practice in Germany.
Patients: Observational study in CLL and FL patients treated with idelalisib between September 2015 and December 2020.
Results: A total of 147 patients with CLL and FL were included with a median age of 75 and 71 years, respectively. More than 80% of patients presented with comorbidity and many CLL patients with documented high-risk genetic features, including del(17p)/TP53 mutation or unmutated IGHV. The median progression-free survival (PFS) and overall survival (OS) were not reached in the CLL cohort irrespective of del(17p)/TP53 or unmutated IGHV. The estimated 6-month PFS and OS rates in CLL were 82% and 92%. The estimated 6-month PFS and OS rates for FL were 32.2% and 77.2%. Overall response rates in the CLL and FL cohorts were 70.4% and 36.4%, with the presence of high-risk genetics having no negative impact. No unexpected adverse events were observed. Most frequently reported adverse drug reactions (ADRs) were diarrhea, nausea, pneumonia, rash, and fatigue.
Conclusion: This real-world study shows that idelalisib is an effective therapy for CLL and FL, regardless of age and high-risk genetic features, consistent with results from previous clinical trials. Collected safety data and the pattern of ADRs reflect those from previous studies.
Calcium (Ca2+) elevation is an essential secondary messenger in many cellular processes, including disease progression and adaptation to external stimuli, e.g., gravitational load. Therefore, mapping and quantifying Ca2+ signaling with a high spatiotemporal resolution is a key challenge. However, particularly on microgravity platforms, experiment time is limited, allowing only a small number of replicates. Furthermore, experiment hardware is exposed to changes in gravity levels, causing experimental artifacts unless appropriately controlled. We introduce a new experimental setup based on the fluorescent Ca2+ reporter CaMPARI2, onboard LED arrays, and subsequent microscopic analysis on the ground. This setup allows for higher throughput and accuracy due to its retrograde nature. The excellent performance of CaMPARI2 was demonstrated with human chondrocytes during the 75th ESA parabolic flight campaign. CaMPARI2 revealed a strong Ca2+ response triggered by histamine but was not affected by the alternating gravitational load of a parabolic flight.
Hintergrund: Die Endometriose und die Infertilität sind beide Erkrankungsbilder, die häufig miteinander assoziiert sind. So werden Literaturen zur Folge bei mindestens 30% der Endometriose Patientinnen eine Sterilität diagnostiziert, während wiederum bei etwa 25 - 50% der Patientinnen eine Endometriose als Hauptursache des unerfüllten Kinderwunsches erkannt wird. Die Laparoskopie dient bei Patientinnen mit Endometriose als wichtiger invasiver Eingriff, der entweder zu diagnostischen Zwecken im Sinne einer Ursachenabklärung dient und / oder zu therapeutischen Zwecken bei bereits bestätigter Endometriose assoziierter Infertilität durchgeführt wird.
Problemstellung und Zielsetzung: Anhand des laparoskopischen Befundes, z.B. durch das Erfassen des rASRM Scores bei Endometriose, kann Patientinnen im Voraus keine zuverlässige Aussage über die Chance auf eine postoperative Schwangerschaft geäußert werden. Das Ziel der hier vorliegenden Arbeit ist es, den Erfolg der laparoskopischen Behandlung bei Endometriose assoziierter Infertilität unter Einbezug von Patientencharakteristika, des intraoperativen Befundes, operativer Verfahren sowie der postoperativen Fertilitätsbehandlung zu beurteilen. Auf diese Weise soll den Frauen geholfen werden, im Voraus bzw. nach laparoskopischer Behandlung der Endometriose die Chance auf eine postoperative Schwangerschaft besser abzuschätzen. Es sollen somit Faktoren ermittelt werden, die die postoperative Fertilität protektiv oder ungünstig beeinflussen.
Material und Methoden: In die Studie aufgenommen wurden alle Patientinnen (n=102), die im Zeitraum von 2007 bis 2017 an der Universitätsklinik Frankfurt am Main einen laparoskopischen Eingriff mit der Diagnose einer Endometriose erhalten haben und einen unerfüllten Kinderwunsch von mehr gleich einem Jahr äußerten. Die Datenerhebung erfolgte über die elektronische Patientenakte sowie die telefonische Befragung der Studienteilnehmerinnen zur Patientencharakteristika und der postoperativen Fertilitätsbehandlung. Die statistische Auswertung wurde durch die Nutzung des Log Rank Tests / der Kaplan Meier Methode sowie uni- und multivariater Analysen mittels
Cox Regression durchgeführt.
Ergebnisse: Von den 102 Patientinnen konnten nach einer mittleren postoperativen Nachbeobachtungszeit von 70 Monaten 71 Patientinnen (69,6%) eine Schwangerschaft verwirklichen. Dabei wurden 21,6% der Frauen durch die alleinige Laparoskopie schwanger, während bei 48% der Frauen die Schwangerschaft durch die zusätzliche Nutzung reproduktionsmedizinischer Verfahren erreicht wurde. Die meisten Schwangerschaften ereigneten sich in den ersten 6 bis 12 Monaten nach der Operation. Die mittlere Dauer bis zur Schwangerschaft betrug 10 Monate. Der Log Rank Test zeigte keine statistisch signifikanten Unterschiede in den verschiedenen rASRM Stadien bei der Betrachtung der kumulativen Schwangerschaftsrate (rASRM 1: 83,3%; rASRM 2: 66,7%; rASRM 3: 93,8%; rASRM 4: 37,9%). Die uni- und multivariaten Analysen zeigten, dass die Dauer der Infertilität (Hazard Ratio: 0,85), das Vorkommen von intestinaler Endometriose (Hazard Ratio: 0,26) und ausgeprägten Adhäsionen (Hazard Ratio: 0,45), die Anzahl der Endometriose Lokalisationen (Hazard Ratio:0,82), die Anzahl der laparoskopischen Eingriffe für Endometriose (Hazard Ratio: 0,66) sowie der Voroperationen (Hazard Ratio: 0,75) und das Vorkommen von Endometriose Rezidiven (Hazard Ratio: 0,4) die Chance auf eine Konzeption signifikant ungünstig beeinträchtigen (p< 0,05). Ein protektiver Einfluss auf die Schwangerschaft zeigte sich nur für die Koagulation am Ovar (p= 0,02; Hazard Ratio: 7,74).
Schlussfolgerung: Die laparoskopische Behandlung scheint die Erfüllung des Kinderwunsches positiv zu beeinflussen, zumal fast ¾ der Patientinnen postoperativ schwanger wurden. Höhere Schwangerschaftsraten wurden durch die zusätzliche Nutzung der assistierten Reproduktionstherapie (ART) erreicht. Aus diesem Grund sollten Frauen für eine zusätzliche postoperative ART motiviert werden. Die Patientinnen sollten darauf aufmerksam gemacht werden, dass sie sich bei einem unerfüllten Kinderwunsch früher bei ihrem Arzt vorstellen sollten. Denn je länger die Zeit bis zur Operation einer Endometriose assoziierten Infertilität beträgt, desto schlechter ist die Chance auf eine postoperative Schwangerschaft (p= 0,02). Endometriosezysten am Ovar sowie deren Entfernung schränken die klinische Fertilität nicht signifikant ein. Die Anzahl der laparoskopischen Eingriffe sowie der abdominellen Voroperationen scheinen ein wichtigerer, ungünstiger Faktor in Hinblick auf die Schwangerschaft zu sein (p = 0,04). Außerdem könnte die Koagulation am Ovar eine wichtige Behandlungsmethode bei Endometriose assoziierter Infertilität sein. Diese Operationsmethode sollte durch weitere Studien und eine größere Stichprobe untersucht werden...
Objective: To assess tooth loss (TL) in initially periodontally healthy/gingivitis (PHG) and periodontally compromised (PC) individuals during a 15- to 25-year follow-up in a specialist practice and to identify the factors influencing TL. Materials and methods: Patients were re-examined 240 ± 60 months after active periodontal therapy (PC) or initial examination (PHG). PHG patients were periodontally healthy or had gingivitis, and PC patients exhibited at least stage II periodontitis. TL, patient-related outcomes, and risk factors for TL were assessed at the patient level (group-relation, gender, age, smoking, bleeding on probing, educational status, mean number of visits/year). Results: Fifty-six PC patients receiving regular supportive periodontal care (12 female, mean age 49.1 ± 10.9 years, stage II: 10, stage III/IV: 46) lost 38 teeth (0.03 ± 0.05 teeth/year). Fifty-one PHG patients (23 female, mean age 34.5 ± 12.4 years) following regular oral prevention lost 39 teeth (0.04 ± 0.05 teeth/year) (p = .631). Both PC and PHG groups did not show any significant differences regarding visual analogue scale measurements [aesthetics (p = .309), chewing function (p = .362), hygiene (p = .989)] and overall Oral Health Impact Profile (p = .484). Age at the start of follow-up was identified as a risk factor for TL (p < .0001). Conclusion: PC and PHG patients exhibited similarly small TL rates over 240 ± 60 months, which should, however, be interpreted with caution in view of the group heterogeneity. Background and Objective: Long-term tooth retention is the ultimate goal of periodontal therapy. Aim of this study was to evaluate tooth loss (TL) during 10 years of supportive periodontal therapy (SPT) in periodontal compromised patients and to identify factors influencing TL on patient level. Material and Methods: Patients were re-examined 120 ± 12 months after active periodontal therapy. TL and risk factors [smoking, initial diagnosis, SPT adherence, interleukin-1 polymorphism, cardiovascular diseases, age at baseline, bleeding on probing (BOP), change of practitioner, insurance status, number of SPT, marital and educational status] influencing TL on patient level were assessed. Results: One-hundred patients (52 female, mean age 65.6 ± 11 years) lost 121 of 2428 teeth (1.21 teeth/patient; 0.12 teeth/patient/y) during 10 years of SPT. Forty-two of these were lost for periodontal reasons (0.42 teeth/patient; 0.04 teeth/patient/y). Significantly more teeth were lost due to other reasons (P < .001). Smoking, baseline severity of periodontitis, non-adherent SPT, positive interleukin-1 polymorphism, marital and educational status, private insurance, older age at baseline and BOP, small number of SPT were identified as patient-related risk factors for TL (P < .05). Conclusion: During 120 ± 12 months of SPT, only a small number of teeth was lost in periodontally compromised patients showing the positive effect of a well-established periodontal treatment concept. The remaining risk for TL should be considered using risk-adopted SPT allocation.
Hintergrund und Ziel der Studie: Der größte Nachteil von Plastikstents ist ihre im Vergleich mit Metallstents hohe Verschlussrate. Das übliche Wechselintervall für biliäre Plastikstents beträgt drei Monate. Im Universitätsklinikum Frankfurt wurde das Intervall für den elektiven Austausch von Plastikstents auf sechs bis acht Wochen festgelegt.
In dieser Studie sollte untersucht werden, ob sich ein kürzeres Intervall von sechs bis acht Wochen auf die mediane vorzeitige Wechselrate (mPER) auswirkt. Hierfür wurden sowohl benigne als auch maligne Gallengangsstenosen untersucht.
Methoden: An die Anwendung verschiedener Ein- und Ausschlusskriterien auf alle seit 2007 im Universitätsklinikum Frankfurt durchgeführten ERCPs inklusive der Implantation mindestens eines Plastikstents, schloss sich die retrospektive Analyse der geeigneten Fälle und die Berechnung der jeweiligen mPER an. Hierfür wurden sowohl verschiedene Stentparameter wie die verwendete Stentgröße, -länge und -anzahl als auch patientenbezogene Parameter wie die indikationsgebende Diagnose erfasst.
Ergebnisse: Insgesamt wurden 3979 ERCPs analysiert, welche sich auf 1199 Patienten verteilen. Eine gutartige Stenose lag in 2717 (68,3 %) Fällen vor. In 1262 (31,7 %) Fällen wurde die Indikation einer Stentimplantation aufgrund einer bösartigen Stenose gestellt.
Die mediane Stentliegedauer (mSP) betrug 41 Tage (Spanne: 14–120) bei planmäßigen Stentwechseln, während sie bei vorzeitig gewechselten Stents bei 17 Tagen (Spanne: 1–75) lag.
Die mPER war bei malignen (28,1 %, Spanne: 35–50 %) im Vergleich zu benignen Strikturen (15,2 %, Spanne: 10–28 %) signifikant höher (p < 0,0001).
Das mSP war in Fällen von einem singulären Stent (34 Tage, Spanne: 1–87) im Vergleich mit Fällen multipler Stentimplantation (41 Tage, Spanne: 1–120) signifikant kürzer (p < 0,0001). Eine signifikant kürzere mSP wurde außerdem in Fällen von nur einem 7 Fr Stent (28 Tage, Spanne: 2–79) im Vergleich zu größeren Stents (34 Tage, Spanne: 1–87, p = 0,001) festgestellt.
Entsprechend war die mPER in Fällen mit einem singulären Stent (23 %) verglichen mit Fällen multiplen Stentings (16,2 %) signifikant höher (p < 0,0001). Beim Vergleich der Implantationen eines einzelnen 7-Fr-Stents (31,3 %) mit jenen größerer Stents (22,4 %) wurde ebenfalls eine signifikant erhöhte mPER der 7-Fr-Stents festgestellt (p = 0,007).
Konklusion: Diese Studie liefert erstmals umfassende Daten von Patienten, welche mit Plastikstents und kürzeren Wechselintervallen behandelt wurden.
Im direkten Vergleich der Stentparameter war bei der Implantation größerer oder multipler Stents die Einliegedauer verlängert. Dies lässt auf ein geringeres Komplikationsrisiko bei der Verwendung multipler oder größerer Stents schließen, welches am ehesten auf die dadurch optimierte Überbrückung der Stenosen und daraus resultierende Drainage zurückzuführen ist.
Weiter lassen die Ergebnisse vermuten, dass ein verkürztes Wechselintervall unter drei Monaten bei benignen biliären Stenosen nicht zu einer klinisch relevanten Reduktion der Komplikationsrate führt.
Zudem war die bei malignen Stenosen beobachtete Rate vorzeitiger Stentwechsel deutlich größer als die der benignen Stenosen. Sollte in diesem Setting ein Wechsel von einem Plastikstent auf einen SEMS geplant sein, ist ein verkürztes Wechselintervall von drei bis vier Wochen in Erwägung zu ziehen. Bei fehlender Möglichkeit der Implantation eines SEMS, ist die Verwendung multipler und größerer Stents das Mittel der Wahl.
Abschließend lässt sich Folgendes festhalten. Ein kürzeres Austauschintervall scheint bei benignen und malignen Stenosen nicht zu einer klinisch bedeutsamen Verringerung der mPER zu führen. Sofern es die Untersuchungsbedingungen zulassen, sollten große und multiple Stents für die Behandlung von Gallengangsstenosen bevorzugt werden.
Retrospektive Analyse der RSV Bronchiolitis bei hospitalisierten Kindern im ersten Lebensjahr

Einleitung: RSV ist der häufigste Erreger einer Bronchiolitis bei Säuglingen und Kleinkindern. Besonders oft betroffen sind Säuglinge in den ersten vier Lebensmonaten. RSV-Infektionen führen saisonal gehäuft weltweit zu zahlreichen Hospitalisierungen. Dennoch gibt es in Deutschland keine aktuellen Daten über die Krankheitslast.
Methoden: Es erfolgte eine retrospektive Analyse der elektronischen Krankenakten an der Klinik für Kinder- und Jugendmedizin des Universitätsklinikums Frankfurt am Main. Erfasst wurden alle von 2007 bis 2017 stationär aufgenommenen Patienten mit positivem RSV-Nachweis. Die Daten wurden aus dem elektronischen Patientenverwaltungsprogramm ORBIS in Exceltabellen übertragen.
Ergebnisse: Insgesamt wurden in den letzten zehn Jahren 800 Kinder mit RSV-Infektion stationär aufgenommen. Die Mehrzahl der Kinder befand sich in den ersten 12 Lebensmonaten, n = 564 (70,5 %) und der Hauptteil bestand aus Patienten in den ersten vier Lebensmonaten, n = 341 (60,4 %). In diesem Patientenkollektiv stand bei 289 Kindern eine elektronische Krankenakte zur Datenauswertung zur Verfügung. Von den 289 Patienten handelte es sich bei 35 (12,1 %) um FG und 25 Patienten mussten intensivmedizinisch betreut werden. Von den intensivmedizinisch betreuten Kindern waren 15 Patienten (60 %) FG. Klassischerweise unterschied sich das Gewicht der FG von dem der RG (Frühgeborene: Median 3,43 kg vs Reifgeborene: 5 kg). Unter den FG zeigte sich folgende Verteilung nach Schwangerschaftswochen bei insgesamt 289 analysierten Patienten: < 28. SSW (extremely preterm): n = 4 (1,38 %) der Patienten; < 32. SSW (very preterm): n = 8 (2,76 %) der Patienten und 32 + 1 bis 36 + 6 SSW (late preterms): n = 23 (7,95 %) der Patienten. Die geringe Anzahl an RSV-Infektionen bei FG < 28. SSW hängt vermutlich mit einer erfolgreichen Palivizumab Prophylaxe zusammen. Dafür spricht auch, dass sich der Anteil der späten FG (late 43 preterms), die keine Prophylaxe erhielten, in unserer Untersuchung ähnlich wie in frühere Studien darstellte.
Von den 254 RG mussten 10 (3,9 %) Kinder auf Intensivstation (Schwergrad 3 der WHO-Klassifikation), während 102 (40,2 %) Patienten eine Bronchiolitis (Grad 2 der WHO-Klassifikation) aufwiesen und 142 (55,9 %) Patienten hatten eine RSV-Infektion mit Sättigungswerten > 95 %.
Schlussfolgerung: Die Krankheitslast der RSV-Bronchiolitis in Deutschland ist sehr hoch, vor allem in den ersten vier Lebensmonaten. Typischerweise ging die RSV-Saison von Dezember bis März, sodass eine Immunprophylaxe bei Risikokindern in dieser Zeit verabreicht werden sollte.
Die CMV-Infektion ist die häufigste opportunistische Infektion nach Nierentransplantation und wird mit einem erhöhten Risiko für Mortalität und Transplantatverlust assoziiert. Das Risiko an einer CMV-Infektion zu erkranken, wird vom Serostatus des Spenders und Empfängers beeinflusst. Dabei sind besonders Patienten mit Hochrisikokonstellation (D+/R–) betroffen. Bei seropositiven Transplantatempfängern (D+/R+, D–/R+) wird ebenfalls ein erhöhtes Risiko durch Reaktivierung des latent vorkommenden Virus oder Sekundärinfektion mit einem exogenen Virusstamm vermutet. Mithilfe von präventiven Maßnahmen wie der präemptiven und prophylaktischen Therapie kann die Rate an Infektionen gesenkt werden.
In dieser Studie wurden 441 Patienten, die in dem Zeitraum von 2006-2013 am Universitätsklinikum Frankfurt eine Nierentransplantation erhielten, retrospektiv untersucht. Ziel der Studie war es, den Einfluss des CMV-Serostatus, der Infektionsprophylaxe sowie der CMV-Infektion auf die Mortalität, das Transplantatüberleben und die Rejektion zu untersuchen. Da CMV-Infektionen meist in den ersten drei Monaten nach Transplantation oder nach Absetzen einer antiviralen Medikation auftreten, wählten wir eine zweijährige Verlaufsbeobachtung. Unsere Hypothese war, dass der Serostatus trotz regelhafter präventiver Maßnahmen mit einer Verschlechterung der Nierenfunktion assoziiert ist. Aufgrund neuer Empfehlungen wurde ab 2010 bei der Mehrheit der Transplantatempfänger eine Prophylaxe mit (Val)Ganciclovir durchgeführt. Folglich wurden die Zeiträume von 2006-2009 sowie von 2010-2013 miteinander verglichen.
In den Jahren von 2010-2013 konnten wir die höchste Infektionsinzidenz für die Hochrisikogruppe, gefolgt von der intermediären Risikogruppe (D+/R+, D–/R+), nachweisen. Beide Gruppen dokumentierten vermehrt CMV-Erkrankungen und Viruslasten im oberen Bereich (> 10.000 Kopien/ml). Diese zeigten jedoch keinen statistisch signifikanten Unterschied zu den anderen Serostatus-Konstellationen auf. Die Infektionsinzidenz in diesem Zeitraum war wiederum mit einem erhöhten Transplantatverlust assoziiert. Durch den prophylaktischen Einsatz von (Val)Ganciclovir konnte das Transplantatüberleben signifikant verlängert werden.
Im Vergleich der Zeiträume vor und nach 2010 stellte sich die D+/R+ Gruppe mit einem verbesserten Transplantatüberleben, die D–/R+ Gruppe mit verbessertem Patientenüberleben dar. Diese Gruppen konnten vermutlich von einer Ausweitung der Prophylaxe profitieren. Bezüglich der Rejektionen empfehlen wir für eine aussagekräftige Auswertung die Beachtung potenzieller Einflussgrößen wie die Immunsuppression und HLA-Merkmale. Rein deskriptiv lag das Auftreten von Rejektionen zeitlich vor den Infektionsereignissen. Die Niedrigrisikogruppe zeigte das geringste Abstoßungsrisiko.
Der Serostatus alleinig erscheint in unserer Studie keinen ausreichenden Hinweis auf das Outcome nach Transplantation zu geben. Allerdings waren CMV-Infektionen in den verschiedenen Zeiträumen maßgeblich mit einem verringerten Transplantat- oder Patientenüberleben assoziiert und sollten deshalb vermieden werden. Die Prophylaxe mit (Val)Ganciclovir stellte sich im gesamten untersuchten Zeitraum als effektive Therapie für ein verbessertes Transplantatüberleben dar.
Ein limitierender Faktor war die Vergleichbarkeit der Gruppen durch die unterschiedliche Patientenanzahl. Daten zur Infektion lagen uns erst ab dem Jahr 2009 vor. Bezüglich der Mortalität schätzen wir die Nachbeobachtungszeit von zwei Jahren zu gering ein, um signifikante Ergebnisse zu erzielen. Hervorzuheben ist, dass durch die Umstellung der Therapiemaßnahmen zwei interessante Patientenkollektive geschaffen wurden, an welchen die Auswirkungen der Prophylaxe dargestellt werden konnten. Mithilfe unserer Daten aus insgesamt acht Jahren mit zweijähriger Verlaufsbeobachtung, schätzen wir das am Universitätsklinikum Frankfurt durchgeführte Therapieregime nach 2010 als wirksame CMV-Prophylaxe ein. Um die Infektionsrate künftig weiterhin zu reduzieren, sind weitere Studien nötig, mit deren Hilfe Risikopatienten identifiziert, die Diagnostik erweitert und Therapien verbessert werden. Ansätze hierfür gibt es beispielsweise beim Einsatz des mTOR-Inhibitors Everolimus, der mit einer niedrigen Rate an CMV-Infektionen assoziiert wird. Auch die Anwendung neuerer Testmethoden, wie z. B. die Messung CMV-spezifischer T-Zellen, sollen dazu beitragen, Patienten mit erhöhtem Infektionsrisiko frühzeitig zu erkennen.
Ziel: Ziel dieser Arbeit war es, das Vorhandensein von Normvarianten der arteriellen Leberversorgung in der Lebertransplantationsevaluation zu beurteilen. Anschließend wurde untersucht, ob das Vorliegen einer Normvariante bei Durchführung einer Lebertransplantation mit einer verlängerten Operations- oder Implantationszeit und einem erhöhten Risiko für postoperative Komplikationen und Versterben korreliert.
Material und Methoden: In dieser retrospektiven Studie wurden die Daten von 210 Patienten ausgewertet, bei denen eine Evaluation zur Lebertransplantation im Zeitraum Januar 2011 bis September 2016 durchgeführt wurde. Zunächst wurden die MR-Angiographien der Patienten auf das Vorhandensein von Normvarianten der arteriellen Leberversorgung untersucht. Anschließend wurde bei durchgeführter Transplantation der operative und postoperative Verlauf in einem Follow-Up von 6 Monaten anhand von Dokumentationen aus dem Krankenhaus-Management-System ORBIS ausgewertet. Bei dieser Evaluation wurde das transplantierte Patientenkollektiv (54 Patienten) in eine Gruppe mit Normtypanatomie (41 Patienten) und eine Gruppe mit Normvarianten (13 Patienten) geteilt und miteinander verglichen.
Ergebnisse: Im Gesamtkollektiv wurde bei 20,73% der Patienten eine Normvariante festgestellt und bei 79,86% eine Normtypanatomie. Im Kollektiv der transplantierten Patienten hatten 24,07% der Patienten eine Normvariante und 75,93% keine arterielle Gefäßanomalie.
Die häufigsten Normvarianten im Evaluations- und Transplantationskollektiv waren eine ersetzende oder akzessorische A. hepatica dextra von der A. mesenterica superior (Hiatt Typ III/Abdullah G2II), eine A. hepatica communis aus der A. mesenterica superior (Hiatt Typ V/Abdullah G1II), eine ersetzende oder akzessorische A. hepatica sinistra von der A. gastrica sinistra (Hiatt Typ II/Abdullah G2I) und eine akzessorische oder ersetzende A. hepatica sinistra vom Truncus coeliacus und/oder eine akzessorische oder ersetzende A. hepatica dextra vom Truncus coeliacus (Abdullah G2V).
Bei Betrachtung des transplantierten Kollektivs ergaben sich keine signifikanten Unterschiede zwischen der Gruppe mit Normtypanatomie und mit Normvarianten in der OP-Dauer (Normtypanatomie: 259,34 ± 57,96 min vs. Normvarianten: 285,15 ± 69,19 min; P=0,172).), der Warmen Ischämie Zeit (Normtypanatomie: 48,31 ± 9,91 min vs. Normvarianten: 51,17 ± 13,58 min; P=0,586), dem Auftreten von primären Transplantatversagen (Normtypanatomie: 7,32% vs. Normvarianten: 0,0%; P=0,316) und Perfusionsstörungen (Normtypanatomie 24,39% vs. Normvarianten: 23,07%; P=0,923), den Retransplantationszahlen (Normtypanatomie: 17,07% vs. Normvarianten 15,38%; P=0,887), sowie der 3-Monats-Mortalität (Normtypanatomie: 24,39% vs. Normvarianten 7,69%; P=0,193). und der 6-Monats-Mortalität (Normtypanatomie: 26,82% vs. Normvarianten 15,38%; P=0,40).
Fazit: Ein ubiquitäres Vorhandensein von Normvarianten konnte in den Versuchsgruppen bestätigt werden. Es gibt keinen Hinweis darauf, dass Patienten mit Anomalien der arteriellen Leberversorgung bei Lebertransplantationen ein höheres Risiko für eine verlängerte Operations- oder Implantationszeit und für postoperative Komplikationen und Versterben haben.
Die Durchführungen einer präoperativen Gefäßdarstellung arterieller Lebergefäße bleibt weiterhin empfohlen.
Einleitung: Im Rahmen dieser retrospektiven Studie wurde die Auswirkung der frühen reduzierten Belastung auf Festigkeit der Implantatverankerung untersucht. Zusätzlich wurde geprüft, welchen Einfluss die Faktoren Implantatlänge, Knochenqualität, Augmentationsart und Implantatlokalisation auf die Festigkeit der Implantatverankerung nach der frühen reduzierten Belastung haben.
Material und Methoden: In die Studie wurden Patienten einbezogen, die sich einer Implantationstherapie in Poliklinik für Zahnärztliche Chirurgie und Implantologie in ZZMK (Carolinum) der J. W. Goethe-Universität in dem Zeitraum von Januar 2001 bis Februar 2010 unterzogen haben. Alle teilnehmenden Patienten wurden von einem Behandler betreut. Es wurden ausschließlich Ankylos Implantate (Dentsply, Mannheim, Deutschland) verwendet. Die Festigkeit der Implantatverankerung wurde mit Periotets gemessen. Die Implantationstherapie lief grundsätzlich nach den Rahmenbedingungen der frühen reduzierten Implantatbelastung ab. Nach einer mindestens sechswöchigen geschlossen Einheilung (statische Phase) wurden die Implantate freigelegt und mit Standardabutment versorgt, anschließend wurde der erste Periotestwert erhoben. Die statische Phase nach externem Sinuslift betrug im Schnitt 5 Monate. Für weitere vier bis acht Wochen (dynamische Phase) wurden die Implantate mit Provisorien in Infraokklusion versorgt, bei zahnlosen Patienten wurden die Implantate durch die provisorische Versorgung grundsätzlich verblockt. Patienten wurden unterwiesen, nur weiche Kost zu sich zu nehmen. Nach vier bis acht Wochen im Anschluss an die dynamische Phase wurde der zweite Periotestwert erhoben. Definitiver Zahnersatz mit korrekt eingestellter Okklusion wurde eingegliedert. Die Periotestwerte vor und nach der frühen reduzierten Implantatbelastung wurden zusammengetragen und statistisch ausgewertet.
Ergebnisse: Bei 247 Patienten wurden im Zeitraum von 01.01.2001 bis 01.03.2010 634 Ankylos Implantate inseriert. In der statischen Phase gingen sieben Implantate verloren, restliche 627 Implantate wurden früh reduziert belastet. Kein Implantat ging in der dynamischen Phase verloren. Innerhalb des ersten Jahres unter voller funktioneller Belastung ging kein Implantat aufgrund von knöcherner Überbelastung verloren. Lediglich ein Implantat musste aufgrund von Abutmentfraktur ein Jahr nach der Eingliederung des definitiven Zahnersatzes entfernt werden. Die Periotestwerte nahmen bei 556 Implantaten um mindestens eine Einheit der Periotestwertskala ab. Diese Veränderung der Periotestwerte war statistisch signifikant (p = 0,0001). Der Einfluss der Faktoren Implantatlänge, Knochenqualität, Implantatlokalisation, Augmentationsverfahren auf die Reduktion der Periotestwerte um eine Einheit war statistisch nicht signifikant. Die Reduktion der Periotestwerte um mindestens zwei Einheiten trat bei 409 von insgesamt 627 Implantaten auf und war statistisch signifikant in den Gruppen „weiche Knochenqualität“, „Implantatlänge 8mm, 9,5mm“, „externer Sinuslift“, „Oberkiefer“ (p = 0,001). In zwei Gruppen, nämlich „11mm und 14mm“ sowie „Standardverfahren, laterale Augmentation, interner Sinuslift“ trat eine Abnahme der Periotestwerte statistisch signifikant seltener auf (p = 0,045 bei „11 und 14 mm“ und p = 0,033 bei „Standardverfahren, laterale Augmentation und interner Sinuslift“).
Schlussfolgerung:Die frühe reduzierte Implantatbelastung hat keinen negativen Einfluss auf den Implantaterfolg. Beim Vorliegen von ungünstigen Voraussetzungen, wie unzureichendem Knochenangebot und Knochenqualität sowie Implantation im Oberkiefer, verbessert sich die Festigkeit der Implantatverankerung besonders deutlich. Das Konzept zeichnet sich durch das breite Indikationsspektrum und die hohe Überlebensrate trotz der verkürzten Therapiezeit aus.
Die Vasektomie stellt eine der einfachsten und effektivsten Methoden der permanenten Empfängnisverhütung dar. Trotzdem bevorzugen mehr Paare die Tubenligatur der Frau, die verglichen mit der Vasektomie sowohl größere Kosten als auch größere Risiken mit sich bringt. Durch mangelnde Informationen haben immer noch viele Männer Angst, dass sich eine Vasektomie negativ auf ihr Sexualleben auswirken könnte. Mit dieser Studie sollten diese Auswirkungen näher untersucht werden, um Männern und auch ihren Partnerinnen die Angst vor möglichen negativen Konsequenzen zu nehmen.
Hierfür wurden 294 vasektomierten Männern und ihren Partnerinnen drei Fragebögen zugeschickt. 95 Männer beantwortete den allgemeinen Fragebogen, 90 den IIEF und 74 Frauen den FSFI Fragebogen. Die Ergebnisse wurden mit Kontrollgruppen aus der Literatur verglichen.
Im IIEF-Fragebogen erreichten die vasektomierten Männer in allen fünf Domänen signifikant höhere Scores als das Vergleichskollektiv mit einer erektilen Dysfunktion aus der Literatur. In vier der fünf Domänen erreichten sie signifikant höhere Scores als die gesunde Kontrollgruppe aus der Literatur. Nur in der Domäne „generelle Zufriedenheit“ gab es keinen signifikanten Unterschied.
Die Partnerinnen der vasektomierten Männer erreichten in allen 19 Fragen des FSFI-Fragebogens sowie in allen sechs Domänen signifikant höhere Scores als Patientinnen mit einer sexuellen Dysfunktion aus der Literatur. In allen 19 Fragen und fünf der sechs Domänen gab es keinen signifikanten Unterschied zu der gesunden Kontrollgruppe aus der Literatur. Lediglich in der Domäne „Erregung“ hatten die Partnerinnen vasektomierter Männer signifikant bessere Ergebnisse.
Die Vasektomie hat keine negativen Auswirkungen auf das Sexualleben des Paares. Die Zufriedenheit der Partnerinnen wird nicht negativ beeinflusst. Die vasektomierten Männer sind mit ihrem Sexualleben sogar zufriedener und erzielen signifikant bessere Werte im IIEF-Fragebogen als eine nicht-vasektomierte Kontrollgruppe.
Der Morbus Basedow zählt zu den häufigsten Ursachen einer Hyperthyreose. Zur Behandlung stehen neben der medikamentösen thyreostatischen Therapie auch ein operatives sowie ein nuklearmedizinisches Verfahren zur Verfügung.
Die beiden zuletzt genannten Behandlungsmöglichkeiten stellen aufgrund ihrer Wirkungsweise ein definitives Verfahren dar, bei dem Schilddrüsengewebe entfernt bzw. zerstört wird. Dadurch ist in der Regel eine dauerhafte Substitution der lebensnotwendigen Schilddrüsenhormone erforderlich. Im Gegensatz dazu bleibt bei der medikamentösen Therapie mit Thyreostatika die gesamte Schilddrüse erhalten und funktionsfähig. Der Nachteil besteht in der hohen Rezidivrate von über 50 % im Vergleich zur definitiven Therapie. Damit mehr Patienten von den Vorteilen der thyreostatischen Therapie profitieren, ist eine Optimierung dieser zur Reduktion der Rezidivrate notwendig.
Ziel dieser Arbeit war es, mittels einer retrospektiven Analyse zu ermitteln, welche anamnestischen, klinischen, sonographischen und laborchemischen Parameter mit einem Rezidiv des Morbus Basedow bei Patienten mit thyreostatischer Therapie in Zusammenhang stehen. Weiterhin erfolgte eine Analyse von sonographischen und laborchemischen Werten im Krankheitsverlauf, um daraus Indikatoren für eine optimale Dauer der thyreostatischen Therapie abzuleiten. Hierzu wurden die Daten von 260 Patienten bezüglich der folgenden Faktoren zwischen Remissions- und Rezidivgruppe verglichen: Erkrankungsalter, Geschlecht, Dauer der Thyreostatikagabe, Vitamin D-Spiegel, Nikotinkonsum, endokrine Orbitopathie, fam. Autoimmunerkrankung, fam. Schilddrüsenerkrankung und Veränderungen im Hormonhaushalt anderer Hormonachsen. Zudem erfolgte eine Zeitreihenanalyse Schilddrüsen-spezifischer Laborwerte (fT3, fT4, TSH, TRAK, anti-TPO-Ak, TgAk) und des sonographisch bestimmten Schilddrüsenvolumens jeweils zu den Zeitpunkten Diagnosestellung sowie sechs und zwölf Monate darauf. Die Rezidivrate im untersuchten Patientenkollektiv betrug 68,8 %.
Für das Erkrankungsalter, die Therapiedauer, das Schilddrüsenvolumen, die Schilddrüsenfunktionsparameter und die TSH-Rezeptor-Antikörper ließen sich signifikante Unterschiede zwischen Remissions- und Rezidivkohorte nachweisen. Patienten, die bei Diagnose das 35. Lebensjahr noch nicht vollendet hatten, erlitten signifikant häufiger ein Rezidiv als ältere Patienten. In der Remissionsgruppe war die Therapiedauer mit zwölf Monaten zudem signifikant länger als in der Rezidivgruppe. Patienten, deren Schilddrüse zum Zeitpunkt der Diagnose oder zwölf Monate darauf in der sonographischen Messung über die Norm vergrößert war, erlitten signifikant häufiger ein Rezidiv des Morbus Basedow, ebenso wie Patienten mit anhaltend pathologischen Schilddrüsenfunktionsparametern sechs und zwölf Monate nach Diagnose. Die Werte der TSH-Rezeptor-Antikörper fielen in der Rezidivgruppe zu allen Erhebungszeitpunkten signifikant höher aus als in der Remissionsgruppe. Diese Ergebnisse lassen für die medikamentöse Behandlung des Morbus Basedow den Schluss zu, dass die Dauer der thyreostatischen Therapie dem Krankheitsverlauf, der sich in den Schilddrüsenfunktionswerten und den Leveln der TSH-Rezeptor-Antikörper widerspiegelt, angepasst werden sollte, um deren Erfolgsrate zu steigern. Weiterhin lässt sich folgern, dass bei jüngeren Patienten und Patienten mit vergrößerter Schilddrüse ein erhöhtes Rezidivrisiko besteht und diese Patienten möglicherweise von einem verlängerten Therapieintervall profitieren.
Während in der aktuellen europäischen Leitlinie zur Behandlung der Immunhyperthyreose eine feste Spanne von zwölf bis achtzehn Monaten für die Gabe der Thyreostatika empfohlen wird, lautet die Empfehlung der amerikanischen Hyperthyreose-Leitlinie die thyreostatische Therapie bis zur Normalisierung der TSH-Rezeptor-Antikörper fortzuführen. Die Ergebnisse der vorliegenden Arbeit sprechen dafür, die europäische Leitlinie dahingehend der amerikanischen Leitlinie anzupassen.
Die vorliegende Promotionsarbeit befasst sich mit der retrospektiven Untersuchung von dentalen Implantaten, die nach Augmentation mit einem synthetischen Knochenersatzmaterial Nanobone® (Artoss, Rostock, Germany) inseriert wurden.
Ziel der vorangegangenen Studie war die Untersuchung, ob durch die Verkürzung der Einheilzeit von 6 auf 3 Monaten ein vergleichbares Ergebnis an Knochenneubildung erreicht werden kann. Es konnte gezeigt werden, dass eine Sinusbodenaugmentation mit dem synthetischen Knochenersatzmaterial Nanobone® schon nach einer Einheilzeit von 3 Monaten ein suffizientes Implantatbett vorweist und somit eine langzeitstabile Implantatrestauration (3-Jahres Follow up) ermöglicht.
Im Rahmen des 10-Jahres Follow-up konnten 12 der insgesamt 14 Patienten klinisch nachuntersucht werden.
Somit standen für die durchgeführte Nachuntersuchung noch 24 der insgesamt 27 Implantate zur Verfügung.
Hiervon wurden 17 Implantate nach 3 Monaten und 7 Implantate nach 6 Monaten inseriert.
Zehn Jahre nach der primären Augmentation und zweizeitigen Implantation zeigten sich bei der Nachuntersuchung 22 der 24 Implantate stabil und reizfrei in situ.
Die Parameter, die in der klinischen Untersuchung erhoben wurden waren, neben der festen und stabilen in situ Insertion der Implantate, eine Periotest-Messung (Medizintechnik Gulden, Modautal, Deutschland), das Bluten auf Sondieren (Bleeding on probing, BOP), die Bestimmung des Plaqueindex sowie die Untersuchung des Weichgewebes auf Rezessionen und Weichgewebsdehiszenzen.
Zwei der 24 Implantate mussten explantiert werden, eines davon bereits ein Jahr nach Implantation aufgrund von progredienten Entzündungszeichen und Lockerung.
Dies entspricht einer mittleren Implantatüberlebensrate von 91,66% oder einer Verlustrate von 8,34%.
Anders als im 3 Jahres Follow-up zeigte sich im Gruppenvergleich 3- zu 6 Monaten ein deutlicher Unterschied mit einer Implantatüberlebensrate in der 6- Monatsgruppe von 100% (7 von 7 Implantaten in situ) zu einer Implantatüberlebensrate der 3-Monatsgruppe von 88,24% (15 von 17 Implantaten in situ).
Somit ergibt sich ein deutlich besseres Outcome 10 Jahre post implantationem im Sinne einer erhöhten Implantatüberlebensrate und einer niedrigeren Komplikationsrate für die 6-Monatskontrollgruppe im Vergleich zu den Patienten, bei denen bereits 3 Monate post augmentationem implantiert wurde.
Die vorliegenden Untersuchungen zeigen, dass die Langzeitergebnisse nach Verwendung von alloplastischen KEM NanoBone®, auch nach 10 Jahren, mit vergleichbaren Langzeitergebnissen zu autologen Knochenersatz zur Knochenaugmentation im Oberkieferbereich mit gegebenenfalls kombinierter Sinusbodenaugmentation und zweizeitiger Implantatversorgung einhergehen.
Hintergrund und Ziel der Arbeit: Aufgrund des demografischen Wandels ist mit einer Änderung des Altersspektrums bei den Obduktionen zu rechnen. Ziel der Arbeit war die Untersuchung der nichtnatürlichen Todesfälle, bei denen die Verstorbenen ein Mindestalter von 65 Jahren erreicht hatten, da dieser Populationsgruppe zukünftig eine wachsende Bedeutung zukommen wird. Material und Methoden: In dieser retrospektiven Mortalitätsstudie wurden alle nichtnatürlichen Todesfälle mit einem Sterbealter ≥ 65 Jahren analysiert, die in den Jahren 2000–2002 (Zeitraum I) und 2013–2015 (Zeitraum II) im Institut für Rechtsmedizin des Universitätsklinikums der Goethe-Universität in Frankfurt am Main obduziert wurden. Für die Analyse der suizidal Verstorbenen wurden zudem Daten nichtobduzierter Selbsttötungen (n = 100) aus Besichtigungen aufgenommen. Ergebnisse: Aus den 1206 Obduktionen resultierten 669 natürliche (55,5 %) und 404 nichtnatürliche (33,5 %) Todesfälle. Darunter ergaben sich 221 Unfälle (Zeitraum I n = 105; Zeitraum II n = 116), 82 Suizide (Zeitraum I n = 55; Zeitraum II n = 27), 41 Todesfälle im Zusammenhang mit medizinischen Interventionen (Zeitraum I n = 7; Zeitraum II n = 34) und 40 Tötungsdelikte (Zeitraum I n = 23; Zeitraum II n = 17). Verkehrsunfälle und Stürze bilden die größten Subgruppen bei den Unfällen. Erhängen und Erschießen sind die am meisten angewandten Suizidarten. Vergleicht man Zeitraum I mit II, so fällt die signifikante Zunahme von Todesfällen im Zusammenhang mit ärztlichen Maßnahmen auf. Eine signifikante Abnahme von Suizidenten ist durch die abnehmende Obduktionsrate in dieser Gruppe zu begründen. Die relative und absolute Fallzahl an Tötungsdelikten im Obduktionsgut weisen keine wesentliche Veränderung auf. Diskussion/Schlussfolgerung: Die Ergebnisse dieser Studie stimmen großteils mit der Literatur überein. Im Zeitvergleich zeigt sich eine relative Zunahme nichtnatürlicher Todesfälle im gerontologischen Obduktionsgut. Dies wird durch den Anstieg von Obduktionen nach iatrogenen Komplikationen wesentlich mitgeprägt.
Ein persistierender Ductus arteriosus (PDA) stellt bei Frühgeborenen mit <1500g Geburtsgewicht einen relevanten Faktor für eine Erhöhung der Morbidität und Mortalität dar. Aufgrund fehlender Evidenz konnte bisher keine allgemeine Therapieempfehlung nach frustranem medikamentösem Verschlussversuch erarbeitet werden, sowohl eine Ligatur im Rahmen eines operativen Eingriffs, als auch eine Katheterintervention sind möglich. In der vorliegenden Arbeit wurde retrospektiv die Auswirkung dieser verschiedenen Therapiemethoden auf die Morbidität und Mortalität bei Frühgeborenen <1500g Geburtsgewicht mit hämodynamisch signifikantem persistierendem Ductus arteriosus untersucht. Insbesondere wurde geprüft, ob eine Katheterintervention das Risiko einer Bronchopulmonalen Dysplasie (BPD) oder des Versterbens der Kinder, sowie andere häufige Folgeerkrankungen des Frühgeborenen erhöht. Analysiert wurden dafür alle 469 stationär behandelten Kinder des Schwerpunktes Neonatologie, die zwischen dem 01.01.2013 und dem 31.12.2019 in der Uniklinik Frankfurt am Main mit einem Geburtsgewicht von <1500g geboren oder innerhalb der ersten 13 Lebenstage aus einem anderen Krankenhaus übernommen wurden. Diese wurden in 4 Gruppen eingeteilt: Kinder ohne hämodynamisch signifikanten PDA (n=341), Kinder die erfolgreich medikamentös therapiert wurden (n=92), solche, die eine Ligatur erhielten (n=16) und zuletzt solche, die interventionell behandelt wurden (n=12). Die Daten der medikamentös behandelten Kinder wurden in dieser Untersuchung nicht berücksichtigt. Es wurden sowohl verschiedene Geburtsparameter um eine Vergleichbarkeit der Gruppen zu untersuchen, als auch relevante Parameter des Outcomes ausgewertet. Es zeigte sich, dass Kinder ohne PDA zum Zeitpunkt der Geburt ein höheres Gestationsalter hatten als solche mit PDA und daraus resultierend eine geringere Intensivmedizinische Betreuung und geringere Morbidität aufwiesen. Im Vergleich der Interventionskinder mit einer Matched-pair Gruppe aus 24 Kindern der Kontrollgruppe ohne PDA (± 8 Schwangerschaftstage bei Geburt und ± 410 g Geburtsgewicht) konnten keine Unterschiede in Mortalität oder Morbidität festgestellt werden. Kinder nach Intervention zeigten kein signifikant schlechteres Outcome (Bronchopulmonale Dysplasie, Intraventrikuläre Hämorrhagie, operativ versorgte nekrotisierende Enterokolitis, therapiebedürftige Retinopathia praematurorum, Periventrikuläre Leukomalazie) im Vergleich zu Kindern nach Ligatur. Eine schwere Bronchopulmonale Dysplasie trat mit einem Relativen Risiko von 2,25 seltener auf. Diese Ergebnisse lassen den Schluss zu, dass die Interventionelle Therapie des hsPDA ein ähnlich gutes Outcome im Vergleich zur Ligatur aufweist und sogar einen Vorteil für die Kinder bringt. Aufgrund der geringen Fallzahlen muss die Relevanz diese Ergebnisse kritisch hinterfragt werden. Eine mögliche randomisierte kontrollierte Studie könnte diese bestätigen.
Therapierefraktärer Schmerz ist ein weit verbreitetes, äußerst belastendes Leitsymptom rheumatischer Erkrankungen. Viele Betroffene weichen daher bei Versagen der Standardmedikation selbstständig auf Cannabis oder die strukturell verwandte Substanz Palmitoylethanolamid (PEA) als Add-On- oder Alternativtherapie aus, obwohl dies in Deutschland bisher nur eingeschränkt zulässig ist. Die deutsche Gesetzgebung ist diesbezüglich nicht eindeutig, weshalb Ärzt:innen in ihrer Entscheidung, Cannabis zu verschreiben, auf Leitlinien, Fallberichte und Expert:innenmeinungen zurückgreifen müssen. Dies führt zu schwierigen Einzelfallentscheidungen, da sich die derzeitige Datenlage zu Cannabis-based Medicine (CBM) bzw. PEA und Rheuma als mangelhaft darstellt und die Leitlinien dementsprechend keine klaren Empfehlungen enthalten. Ziel der vorliegenden Arbeit ist es, die vorhandene Evidenz zusammenzufassen, zu ordnen und anhand der Hill-Kriterien den möglichen kausalen Zusammenhang zwischen der Einnahme von CBM bzw. PEA und der analgetischen Wirkung bei Rheumaschmerzen zu prüfen.
