Alle lebenden Organismen sind in der Lage, sich an den re-gelmäßigen Wechsel von Licht und Dunkelheit und den zeitli-che Veränderungen im Takt der Jahreszeiten anzupassen. Die-se Synchronisierung der Aktivitäts- und Ruhephasen, sowie von physiologischen Stoffwechselprozessen an die vorgegebe-nen tageszeitlichen und saisonalen Zyklen findet beim Säu-getier in der inneren Uhr im Nucleus Suprachiasmaticus (SCN) statt. Das Licht, als wichtigster Zeitgeber für die Synchronisation der inneren Uhr, findet Eingang zum SCN über die Retina und den retinohypothalamischen Trakt (RTH), der Glutamat als Neurotransmitter nutzt. Ist dieses System fehlerhaft, führt dies zu Störung der oben beschriebenen Anpassungsprozesse. Dies hat eine gestörte Homöostase des Organismus zu Folge, aus denen sich wiederum Veränderungen im Tag/Nacht- Rhythmus, Schlafstörungen und depressive Ver-stimmungen ergeben können. Die genannten Symptome decken sich mit den Frühsymptomen den neurodegenerativen Erkran-kung Morbus Parkinson.
Ziel der vorliegenden Arbeit war es, Störungen im photoneu-roendokrinen System, insbesondere Veränderungen in der Re-tina an den photosensitiven Ganglienzellen mit dem Photo-pigment Melanopsin und dem SCN bei transgene Mäuse mit dem humanen alpha-Synuclein zu untersuchen. Hierbei wurden transgene Mäuse mit dem gesunden humanen alpha-Synuclein (Wildtyp) und transgene Mäuse mit der für Parkinson spezi-fischen Mutation im alpha-Synuclein Ala53Thr (A53T) vergli-chen.
Die immunochemischen Untersuchungen an Retina und SCN zei-gen einen signifikanten Anstieg der alpha-Synuclein Immun-reaktion bei der A53T Maus im Vergleich zum Wildtyp.
Parallel dazu wurden Unterschiede in Bezug auf das Photo-pigment Melanopsin zwischen den beiden Gruppen untersucht. Melanopsin ist lichtsensitiv und trägt, durch Übermittlung der aktuellen Lichtverhältnisse über den retinohypothalami-schen Trakt zum SCN, zur Synchronisation der circadianen Rhythmik bei. Durch den in dieser Arbeit nachgewiesene Me-lanopsindefizit und des deutlich reduzierten Vglut2 im hy-pothalamischen Trakt der A53T Maus lässt sich die Hypothese ableiten, dass möglicherweise die Überexpression des mu-tierten alpha-Synuclein in der Retina einen Untergang von melanopsinhaltigen Ganglienzellen herbeiführt und dadurch die Synchronisation der inneren Uhr durch Licht gestört ist. Diese Hypothese wird durch die Aktivitätsprofile ge-stützt, die durch die Aufzeichnung der lokomotorischen Ak-tivität der Tiere erstellt wurden.
Da in beiden Gruppen unter Dauerdunkel (DD) ein endogener zirkadianer Rhythmus beobachtet werden konnte, lässt dies auf die Funktionstüchtigkeit der inneren Uhr im SCN schlie-ßen. Im anschließenden Versuch die endogene Rhythmik an exogenen Reize anzupassen, zeigte sich bei dem A53T Stamm eine fehlende Synchronisierung an vorgegebene Lichtverhält-nisse mit gesteigerter Tagaktivität und reduzierten Schlaf-phasen. Somit trägt der fehlerhaft verarbeitete Lichtreiz bei A53T Mutanten zur Destabilisierung des zirkadianen Rhythmus der Lokomotion bei. Trotz des gestörten glutama-tergen Signalweges im retinohypothalamischen Trakt konnten keine Unterschiede in der Expression der Homerproteine zwi-schen Wildtyp und A53T unter Standard-Photoperiode und nach Schlafdeprivation nachgewiesen werden.
Die vorliegenden Befunde liefern Erkenntnisse zur Entste-hung der Frühsymptome bei Morbus Parkinson. Dies könnte neue Ansatzpunkte für die Therapie und Linderung von Schlafstörungen sowie Veränderungen im Tag/Nachtrhythmus liefern.
Einführung: Die höchste Offenheitsrate in der arteriellen Bypasschirurgie der unteren Extremitäten wird mit einem Segment der Vena saphena magna (VSM) von ausreichender Länge und Durchmesser erreicht. Dabei ist der Venendurchmesser der einzige signifikant korrelierende Faktor für das Auftreten einer Transplantatsstenose und damit der beste Prädiktor für die primäre und die sekundäre 2-Jahre-Durchgängigkeitsrates. In 20-40% der Patienten fehlt ein bzgl. des Lumens geeignetes Segment der Vena saphena magna zur Herstellung eines Bypasses bei der primären Operation. Daher wurde eine Technik entwickelt, kleinlumige Vena saphena magna Segmente mittels einer Valvulotomie zu erweitern.
Ziel: Eine zu kleinlumige Vena saphena magna soll VSM für eine spätere Verwendung als autologes Bypassmaterial der unteren Extremität konditioniert werden. Die häufigsten Indikationen für eine derartige Konditionierung sind die Bypassverwendung bei der operativen Therapie der peripheren arteriellen Verschlusskrankheit (pAVK) und beim Aneurysma der Arteria poplitea (PA).
Methoden: Diese prospektive Pilotstudie würde zwischen Juni 2007 und November 2011 durchgeführt. 25 Patienten (26 Fälle) mit einem VSM Durchmesser zwischen 2 und 3mm erhielten eine in situ VSM-Valvulotomie. Die Zerstörung der suffizienten Venenklappen ermöglicht einen retrograden Blutstrom in die Vena saphena magna. Der hierdurch erzeugte Wachstumsreiz soll zu einer Durchmesserzunahme der Vene führen. Nach Erreichen eines Durchmessers von > 3mm für infragenuale Rekonstruktionen und von > 3,5 mm für supragenuale Rekonstruktion wurden die konditionierten Venensegmente als Bypass implantiert. Bei 23 Patienten war die Bypassindikation eine nicht kritische pAVK. Bei 2 Patienten war ein Aneurysma der Arteria poplitea die Indikation. Einschlusskriterium für Teilnahme an der Studie war eine nicht variköse VSM mit einem Querdurchmesser zwischen 2 und 3 mm. Die Venenevaluation erfolgte duplexsonografisch im Stehen durch eine Beurteilung der epifaszialen Vena saphena magna von der Leiste bis zum Innenknöchel. Messpunkte waren: 10 cm unterhalb des Hiatus saphenus, 10 cm oberhalb des Knies, 10 cm unterhalb des Knies und 10 cm oberhalb des Innenknöchels. Um das Venenwachstum zu ermitteln, diente als Messpunkt der kleinste ermittelte Durchmesser über einem Messbereich von 10 mm.
Drei Patienten, bei denen ein femoropoplitealer Bypass oberhalb des Kniegelenkes implantiert wurde, hatten bei der Implantation einen Durchmesser der konditionierten VSM > 3.5 mm. Bei 13 Patienten mit Bypassimplantation unterhalb des Kniegelenkes war die konditionierte VSM > 3mm. Eine Wiederholung der Valvulotomie war in keinem der Patienten nötwendig. Die valvulotomierte VSM wurde bei allen Bypassoperationen offen chirurgisch entnommen. Zur postoperativen Kontrolle erfolgten eine Duplexuntersuchung und eine ABI-Messung 1,3,6,12,18 und 24 Monate nach der Implantation.
Ergebnisse: Durchschnittlich vergrößerte sich der VSM-Durchmesser von initial 2,5±0,18 mm vor Valvulotomie auf 2,8±0,2 mm, 3,0±0,3mm und 3,2±0,4 mm jeweils 30, 60 und 90 Tage nach Valvolutomie. Bei der Entnahme betrug der durchschnittliche Bypassdurchmesser 3.7±0.6 mm. Die primäre 1-Jahres-Offenheitsrate mit der valvulotomierten VSM war 81±9,8% im Vergleich zu in der Literatur publizierten 58,0%±8,4% für alloplastische Transplantate, 51% für kleinkalibrige VSM und 81,6%±3,6% für Armvenentransplantate. Die sekundäre 1-Jahres-Offenheitsrate mit der Valvulotomietechnik betrug 87%±8,3% verglichen mit 82,6% für in der Literatur angegebene Transplantationen mit kleinkalibriger VSM. Die primäre 2-Jahres-Offenheitsrate mit der Valvulotomietechnik betrug 69%±11,8%, verglichen mit 50% in der Literatur angegebener Transplantationen von kleinkalibrige VSM benutzt wurden und 72%, falls Armvenentransplantate benutzt wurden. Die sekundäre 2-Jahres-Offenheitsrate mit der Valvulotomietechnik lag bei 75%±11%.
Fazit: Die Valvulotomie kann zur Zunahme des Venendurchmessers vor Anlage eines femoro-distalen Bypasses verwendet werden, weil sie regelhaft zum Reflux in der valvulotomierten Vene führt und dadurch ein relevantes Venenwachstum verursacht. Zwischen 60 und 90 Tagen nach Valvulotomie wird ein Venenduchmesser > 3.5 mm erreicht und die valvulotomierte VSM kann als femoropoplitealer Bypass oberhalb des Kniegelenkes implantiert werden. Für eine Anastomose unterhalb des Kniegelenkes genügt ein Wachstum bis > 3 mm. Wenn die valvulotomierte Vene einen entsprechenden Durchmesser erreicht, kann die konditionierte VSM benutzt werden, um einen Bypass mit guter Offenheitsprognose zu konstruieren oder einen nicht funktionierenden Bypass zu ersetzen.
Introduction: DACCORD is an observational, non-interventional study being conducted in German primary and secondary care centres. The study aims to describe the impact of disease (including exacerbations) and treatments over 2 years on ‘real-life’ patients with chronic obstructive pulmonary disease (COPD).
Materials and methods: Patients had a clinical and spirometry diagnosis of COPD, were aged ≥40 years and, on recruitment, were initiating or changing COPD maintenance medication. The only exclusion criteria were asthma and randomised clinical trial participation. Exacerbations data were collected every 3 months. COPD medication, COPD Assessment Test (CAT) and forced expiratory volume in 1 s (FEV1) were recorded at baseline and after 1 and 2 years.
Results: A total of 6122 patients were recruited, 3137 (51.2%) of whom completed the 2-year visit. The mean age of these patients was 65.6 years, 59% were male, 69% had mild or moderate airflow limitation, and their mean COPD Assessment Test (CAT) total score was 20.3. Overall, there was a trend towards decreasing COPD exacerbation rates over the 2-year follow-up period, with rates of 0.390 during Year 1 and 0.347 during Year 2. Rates were lower in patients with no exacerbation during the 6 months prior to entry (0.263 and 0.251 during Years 1 and 2, respectively), with 51.6% of patients having no exacerbation during the 6 months prior to entry and over the 2-year follow-up. Approximately 50% of the overall population experienced a clinically relevant improvement from baseline in CAT total score at Year 1 and 2. When assessed by treatment class (or classes), persistence to medication was high (77.8% in Year 1 and 71.4% in Year 2).
Conclusions: Overall, the 2-year follow-up data from DACCORD suggest that for most patients with COPD exacerbations are a rare event. For the majority of patients, the focus should be on managing symptoms, and the impact that these symptoms have on their daily lives. Even for those patients who do exacerbate, although prevention of exacerbations is an important factor, management of symptoms should be a key consideration. DACCORD also suggests that COPD disease progression is not inevitable – providing patients are receiving pharmacological treatment.
Ligand stimulation of CD95 induces activation of Plk3 followed by phosphorylation of caspase-8

Upon interaction of the CD95 receptor with its ligand, sequential association of the adaptor molecule FADD (MORT1), pro-forms of caspases-8/10, and the caspase-8/10 regulator c-FLIP leads to the formation of a death-inducing signaling complex. Here, we identify polo-like kinase (Plk) 3 as a new interaction partner of the death receptor CD95. The enzymatic activity of Plk3 increases following interaction of the CD95 receptor with its ligand. Knockout (KO) or knockdown of caspase-8, CD95 or FADD prevents activation of Plk3 upon CD95 stimulation, suggesting a requirement of a functional DISC for Plk3 activation. Furthermore, we identify caspase-8 as a new substrate for Plk3. Phosphorylation occurs on T273 and results in stimulation of caspase-8 proapoptotic function. Stimulation of CD95 in cells expressing a non-phosphorylatable caspase-8-T273A mutant in a rescue experiment or in Plk3-KO cells generated by CRISPR/Cas9 reduces the processing of caspase-8 prominently. Low T273 phosphorylation correlates significantly with low Plk3 expression in a cohort of 95 anal tumor patients. Our data suggest a novel mechanism of kinase activation within the Plk family and propose a new model for the stimulation of the extrinsic death pathway in tumors with high Plk3 expression.
Impact of human mesenchymal stromal cells on antifungal host response against Aspergillus fumigatus

Mesenchymal stromal cells (MSCs) are increasingly given as immunotherapy to hematopoietic stem cell transplant (HSCT) recipients with refractory graft-versus-host disease (GvHD). Whereas the immunosuppressive properties of MSCs seem to be beneficial in GvHD, there is, at the same time, major concern that MSCs increase the risk for infection. We therefore investigated the interplay of human MSCs with Aspergillus fumigatus and the impact of MSCs on different arms of the anti-Aspergillus host response in vitro. Although A. fumigatus hyphae increase mRNA levels of IL6 in MSCs, the extracellular availability of IL-6 and other pro-inflammatory cytokines remains unaffected. Human MSCs are able to phagocyte Aspergillus conidia, but phagocytosis of conidia is not associated with an alteration of the cytokine production by MSCs. In addition, human MSCs do not affect activation and function of A. fumigatus specific CD4+ T cells, and MSCs do not negatively impact the oxidative burst activity of phagocytes. Our in vitro data indicate that administration of human MSCs is not associated with a negative impact on the host response against A. fumigatus and that the fungus does not stimulate MSCs to increase the release of those cytokines which play a central role in the pathophysiology of GvHD.
Information theory provides a formal framework within which information processing and its disorders can be described. However, information theory has rarely been applied to modeling aspects of the cognitive neuroscience of schizophrenia. The goal of this article is to highlight the benefits of an approach based on information theory, including its recent extensions, for understanding several disrupted neural goal functions as well as related cognitive and symptomatic phenomena in schizophrenia. We begin by demonstrating that foundational concepts from information theory—such as Shannon information, entropy, data compression, block coding, and strategies to increase the signal-to-noise ratio—can be used to provide novel understandings of cognitive impairments in schizophrenia and metrics to evaluate their integrity. We then describe more recent developments in information theory, including the concepts of infomax, coherent infomax, and coding with synergy, to demonstrate how these can be used to develop computational models of schizophrenia-related failures in the tuning of sensory neurons, gain control, perceptual organization, thought organization, selective attention, context processing, predictive coding, and cognitive control. Throughout, we demonstrate how disordered mechanisms may explain both perceptual/cognitive changes and symptom emergence in schizophrenia. Finally, we demonstrate that there is consistency between some information-theoretic concepts and recent discoveries in neurobiology, especially involving the existence of distinct sites for the accumulation of driving input and contextual information prior to their interaction. This convergence can be used to guide future theory, experiment, and treatment development.
The inner boundary and the cristae membrane are connected by pore-like structures termed crista junctions (CJs). The MICOS complex is required for CJ formation and enriched at CJs. Here, we address the roles of the MICOS subunits Mic27 and Mic10. We observe a positive genetic interaction between Mic27 and Mic60 and deletion of Mic27 results in impaired formation of CJs and altered cristae membrane curvature. Mic27 acts in an antagonistic manner to Mic60 as it promotes oligomerization of the F1FO-ATP synthase and partially restores CJ formation in cells lacking Mic60. Mic10 impairs oligomerization of the F1FO-ATP synthase similar to Mic60. Applying complexome profiling, we observed that deletion of Mic27 destabilizes the MICOS complex but does not impair formation of a high molecular weight Mic10 subcomplex. Moreover, this Mic10 subcomplex comigrates with the dimeric F1FO-ATP synthase in a Mic27-independent manner. Further, we observed a chemical crosslink of Mic10 to Mic27 and of Mic10 to the F1FO-ATP synthase subunit e. We corroborate the physical interaction of the MICOS complex and the F1FO-ATP synthase. We propose a model in which part of the F1FO-ATP synthase is linked to the MICOS complex via Mic10 and Mic27 and by that is regulating CJ formation.
Background: Computed-tomography-guided interventions are attractive for tissue sampling of paediatric tumor lesions; however, it comes with exposure to ionizing radiation. The aim of this study was to analyse the radiation dose, accuracy and speed of CT-guided interventions in paediatric patient cohort.
Methods: We retrospectively reviewed CT-guided interventions over a 10 -year period in 65 children. The intervention site consisted of bones in 38, chest (lung) in 15 and abdomen (liver, lymph nodes) in 12 cases. Radiation dose and duration of the procedures were analysed. The statistical analysis was performed using dedicated statistical software (BiAS 8.3.6 software, Epsilon Verlag, North Hasted).
Results: All interventions were performed successfully. Mean target access path to lesion within the patients was 6.0 cm (min 3.5 cm, max 11.2 cm). Time duration to complete intervention was 25:15 min (min 17:03 min, max 43:00 min). The dose-length product (DLP) of intervention scan was 29.5 mGy · cm (min 6 mGy · cm, max 85 mGy · cm) with the lowest dose for biopsies in the region of the chest (p = 0.04).
Conclusions: With justified indications, CT-guided paediatric interventions are safe, effective and can be performed both, with short intervention times and low radiation exposure.
Objectives: This study aimed to analyse and compare differences in occupational stress, depressive symptoms, work ability and working environment among residents working in various medical specialties.
Methods: 435 German hospital residents in medical training working in 6 different medical specialties participated in a cross-sectional survey study. Physicians were asked about their working conditions and aspects of mental health and work ability. The Copenhagen Psychosocial Questionnaire, the Work Ability Index, the ICD-10 Symptom Rating and the Perceived Stress Questionnaire were used to measure working conditions, mental health and work ability.
Results: Results show that up to 17% of the physicians reported high levels of occupational distress and 9% reported high levels of depressive symptoms. 11% of the hospital physicians scored low in work ability. Significant differences between medical specialties were demonstrated for occupational distress, depressive symptoms, work ability, job demands and job resources. Surgeons showed consistently the highest levels of perceived distress but also the highest levels of work ability and lowest scores for depression. Depressive symptoms were rated with the highest levels by anaesthesiologists. Significant associations between physicians’ working conditions, occupational distress and mental health-related aspects are illustrated.
Conclusions: Study results demonstrated significant differences in specific job stressors, demands and resources. Relevant relations between work factors and physicians' health and work ability are discussed. These findings should be reinvestigated in further studies, especially with a longitudinal study design. This work suggests that to ensure physicians' health, hospital management should plan and implement suitable mental health promotion strategies. In addition, operational efficiency through resource planning optimisation and work process improvements should be focused by hospital management.
Pirinixic acid derivatives, a new class of drug candidates for a range of diseases, interfere with targets including PPARα, PPARγ, 5-lipoxygenase (5-LO), and microsomal prostaglandin and E2 synthase-1 (mPGES1). Since 5-LO, mPGES1, PPARα, and PPARγ represent potential anti-cancer drug targets, we here investigated the effects of 39 pirinixic acid derivatives on prostate cancer (PC-3) and neuroblastoma (UKF-NB-3) cell viability and, subsequently, the effects of selected compounds on drug-resistant neuroblastoma cells. Few compounds affected cancer cell viability in low micromolar concentrations but there was no correlation between the anti-cancer effects and the effects on 5-LO, mPGES1, PPARα, or PPARγ. Most strikingly, pirinixic acid derivatives interfered with drug transport by the ATP-binding cassette (ABC) transporter ABCB1 in a drug-specific fashion. LP117, the compound that exerted the strongest effect on ABCB1, interfered in the investigated concentrations of up to 2μM with the ABCB1-mediated transport of vincristine, vinorelbine, actinomycin D, paclitaxel, and calcein-AM but not of doxorubicin, rhodamine 123, or JC-1. In silico docking studies identified differences in the interaction profiles of the investigated ABCB1 substrates with the known ABCB1 binding sites that may explain the substrate-specific effects of LP117. Thus, pirinixic acid derivatives may offer potential as drug-specific modulators of ABCB1-mediated drug transport.
Purpose: The PELICAN trial evaluates for the first time efficacy and safety of pegylated liposomal doxorubicin (PLD) versus capecitabine as first-line treatment of metastatic breast cancer (MBC).
Methods: This randomized, phase III, open-label, multicenter trial enrolled first-line MBC patients who were ineligible for endocrine or trastuzumab therapy. Cumulative adjuvant anthracyclines of 360 mg/m2 doxorubicin or equivalent were allowed. Left ventricular ejection fraction of >50 % was required. Patients received PLD 50 mg/m2 every 28 days or capecitabine 1250 mg/m2 twice daily for 14 days every 21 days. The primary endpoint was time-to-disease progression (TTP).
Results: 210 patients were randomized (n = 105, PLD and n = 105, capecitabine). Adjuvant anthracyclines were given to 37 % (PLD) and 36 % (capecitabine) of patients. No significant difference was observed in TTP [HR = 1.21 (95 % confidence interval, 0.838–1.750)]. Median TTP was 6.0 months for both PLD and capecitabine. Comparing patients with or without prior anthracyclines, no significant difference in TTP was observed in the PLD arm (log-rank P = 0.64). For PLD versus capecitabine, respectively, overall survival (median, 23.3 months vs. 26.8 months) and time-to-treatment failure (median, 4.6 months vs. 3.7 months) were not statistically significantly different. Compared to PLD, patients on capecitabine experienced more serious adverse events (P = 0.015) and more cardiac events among patients who had prior anthracycline exposure (18 vs. 8 %; P = 0.31).
Conclusion: Both PLD and capecitabine are effective first-line agents for MBC.
Background: To meet the requirements imposed by the time-dependency of acute stroke therapies, it is necessary 1) to initiate structural and cultural changes in the breadth of stroke-ready hospitals and 2) to find new ways to train the personnel treating patients with acute stroke. We aimed to implement and validate a composite intervention of a stroke team algorithm and simulation-based stroke team training as an effective quality initiative in our regional interdisciplinary neurovascular network consisting of 7 stroke units.
Methods: We recorded door-to-needle times of all consecutive stroke patients receiving thrombolysis at seven stroke units for 3 months before and after a 2 month intervention which included setting up a team-based stroke workflow at each stroke unit, a train-the-trainer seminar for stroke team simulation training and a stroke team simulation training session at each hospital as well as a recommendation to take up regular stroke team trainings.
Results: The intervention reduced the network-wide median door-to-needle time by 12 minutes from 43,0 (IQR 29,8–60,0, n = 122) to 31,0 (IQR 24,0–42,0, n = 112) minutes (p < 0.001) and substantially increased the share of patients receiving thrombolysis within 30 minutes of hospital arrival from 41.5% to 59.6% (p < 0.001). Stroke team training participants stated a significant increase in knowledge on the topic of acute stroke care and in the perception of patient safety. The overall course concept was regarded as highly useful by most participants from different professional backgrounds.
Conclusions: The composite intervention of a binding team-based algorithm and stroke team simulation training showed to be well-transferable in our regional stroke network. We provide suggestions and materials for similar campaigns in other stroke networks.
Loss of HIF-1α in macrophages attenuates AhR/ARNT-mediated tumorigenesis in a PAH-driven tumor model

Activation of hypoxia-inducible factor (HIF) and macrophage infiltration of solid tumors independently promote tumor progression. As little is known how myeloid HIF affects tumor development, we injected the polycyclic aromatic hydrocarbon (PAH) and procarcinogen 3-methylcholanthrene (MCA; 100 μg/100 μl) subcutaneously into myeloid-specific Hif-1α and Hif-2α knockout mice (C57BL/6J) to induce fibrosarcomas (n = 16). Deletion of Hif-1α but not Hif-2α in macrophages diminished tumor outgrowth in the MCA-model. While analysis of the tumor initiation phase showed comparable inflammation after MCA-injection, metabolism of MCA was impaired in the absence of Hif-1α. An ex vivo macrophage/fibroblast coculture recapitulated reduced DNA damage after MCA-stimulation in fibroblasts of cocultures with Hif-1α LysM-/- macrophages compared to wild type macrophages. A loss of myeloid Hif-1α decreased RNA levels of arylhydrocarbon receptor (AhR)/arylhydrocarbon receptor nuclear translocator (ARNT) targets such as Cyp1a1 because of reduced Arnt but unchanged Ahr expression. Cocultures using Hif-1α LysM-/- macrophages stimulated with the carcinogen 7,12-dimethylbenz[a]anthracene (DMBA; 2 μg/ml) also attenuated a DNA damage response in fibroblasts, while the DNA damage-inducing metabolite DMBA-trans-3,4-dihydrodiol remained effective in the absence of Hif-1α. In chemical-induced carcinogenesis, HIF-1α in macrophages maintains ARNT expression to facilitate PAH-biotransformation. This implies a metabolic activation of PAHs in stromal cells, i.e. myeloid-derived cells, to be crucial for tumor initiation.

AKTIVA-MCI is a program for patients with mild cognitive impairment (MCI) that aims to enhance participation in cognitively stimulating leisure activities. Participation in cognitively stimulating activities seems to be a potential strategy for people with MCI delaying cognitive decline for a while. In total, 35 MCI patients were enrolled in the pilot study of whom 29 completed the whole program (16 female, 71.1±7.5 years; Mini Mental Status Examination score: 28±2.2). Daily activity protocols were used to measure the frequency of participation in cognitively stimulating activities during the program (12 sessions). Additional standardized psychometric tests and questionnaires were used to assess cognition, mood, and subjective memory decline. Analyses of the daily activity protocols showed that during the intervention participants increased the frequency of several cognitively stimulating leisure activities. Comparison of pre-post data indicates no changes in cognitive status, mood, and subjective memory decline. These findings indicate that the program is suitable for patients with MCI.
Causality assessment of suspected drug induced liver injury (DILI) and herb induced liver injury (HILI) is hampered by the lack of a standardized approach to be used by attending physicians and at various subsequent evaluating levels. The aim of this review was to analyze the suitability of the liver specific Council for International Organizations of Medical Sciences (CIOMS) scale as a standard tool for causality assessment in DILI and HILI cases. PubMed database was searched for the following terms: drug induced liver injury; herb induced liver injury; DILI causality assessment; and HILI causality assessment. The strength of the CIOMS lies in its potential as a standardized scale for DILI and HILI causality assessment. Other advantages include its liver specificity and its validation for hepatotoxicity with excellent sensitivity, specificity and predictive validity, based on cases with a positive reexposure test. This scale allows prospective collection of all relevant data required for a valid causality assessment. It does not require expert knowledge in hepatotoxicity and its results may subsequently be refined. Weaknesses of the CIOMS scale include the limited exclusion of alternative causes and qualitatively graded risk factors. In conclusion, CIOMS appears to be suitable as a standard scale for attending physicians, regulatory agencies, expert panels and other scientists to provide a standardized, reproducible causality assessment in suspected DILI and HILI cases, applicable primarily at all assessing levels involved.
Juvenile neuronal ceroid lipofuscinosis (JNCL or Batten disease) caused by mutations in the CLN3 gene is the most prevalent inherited neurodegenerative disease in childhood resulting in widespread central nervous system dysfunction and premature death. The consequences of CLN3 mutation on the progression of the disease, on neuronal transmission, and on central nervous network dysfunction are poorly understood. We used Cln3 knockout (Cln3Δex1-6) mice and found increased anxiety-related behavior and impaired aversive learning as well as markedly affected motor function including disordered coordination. Patch-clamp and loose-patch recordings revealed severely affected inhibitory and excitatory synaptic transmission in the amygdala, hippocampus, and cerebellar networks. Changes in presynaptic release properties may result from dysfunction of CLN3 protein. Furthermore, loss of calbindin, neuropeptide Y, parvalbumin, and GAD65-positive interneurons in central networks collectively support the hypothesis that degeneration of GABAergic interneurons may be the cause of supraspinal GABAergic disinhibition.
