Simple Summary: The incidence of brain metastases from breast cancer is increasing and the treatment is still a major challenge. Several scores have been developed in order to estimate the prognosis of patients with brain metastases by objective criteria. Here, we validated all three published graded-prognostic-assessment (GPA)-scores in a subcohort of 882 breast cancer patients with brain metastases in the Brain Metastases in the German Breast Cancer (BMBC) registry. Although all three available GPA-scores were associated with OS, they all show limitations mainly in predicting short-term (below 3 months) survival but also in long-term (above 12 months) survival. We discuss the test performances of all scores in our work and provide evidence how physicians should use them as a tool to select patients for different treatment options.
Abstract: Several scores have been developed in order to estimate the prognosis of patients with brain metastases (BM) by objective criteria. The aim of this analysis was to validate all three published graded-prognostic-assessment (GPA)-scores in a subcohort of 882 breast cancer (BC) patients with BM in the Brain Metastases in the German Breast Cancer (BMBC) registry. The median age at diagnosis of BM was 57 years. All in all, 22.3% of patients (n = 197) had triple-negative, 33.4% (n = 295) luminal A like, 25.1% (n = 221) luminal B/HER2-enriched like and 19.2% (n = 169) HER2 positive like BC. Age ≥60 years, evidence of extracranial metastases (ECM), higher number of BM, triple-negative subtype and low Karnofsky-Performance-Status (KPS) were all associated with worse overall survival (OS) in univariate analysis (p < 0.001 each). All three GPA-scores were associated with OS. The breast-GPA showed the highest probability of classifying patients with survival above 12 months in the best prognostic group (specificity 68.7% compared with 48.1% for the updated breast-GPA and 21.8% for the original GPA). Sensitivities for predicting 3 months survival were very low for all scores. In this analysis, all GPA-scores showed only moderate diagnostic accuracy in predicting the OS of BC patients with BM.
Purpose: While more advanced COVID-19 necessitates medical interventions and hospitalization, patients with mild COVID-19 do not require this. Identifying patients at risk of progressing to advanced COVID-19 might guide treatment decisions, particularly for better prioritizing patients in need for hospitalization.
Methods: We developed a machine learning-based predictor for deriving a clinical score identifying patients with asymptomatic/mild COVID-19 at risk of progressing to advanced COVID-19. Clinical data from SARS-CoV-2 positive patients from the multicenter Lean European Open Survey on SARS-CoV-2 Infected Patients (LEOSS) were used for discovery (2020-03-16 to 2020-07-14) and validation (data from 2020-07-15 to 2021-02-16).
Results: The LEOSS dataset contains 473 baseline patient parameters measured at the first patient contact. After training the predictor model on a training dataset comprising 1233 patients, 20 of the 473 parameters were selected for the predictor model. From the predictor model, we delineated a composite predictive score (SACOV-19, Score for the prediction of an Advanced stage of COVID-19) with eleven variables. In the validation cohort (n = 2264 patients), we observed good prediction performance with an area under the curve (AUC) of 0.73 ± 0.01. Besides temperature, age, body mass index and smoking habit, variables indicating pulmonary involvement (respiration rate, oxygen saturation, dyspnea), inflammation (CRP, LDH, lymphocyte counts), and acute kidney injury at diagnosis were identified. For better interpretability, the predictor was translated into a web interface.
Conclusion: We present a machine learning-based predictor model and a clinical score for identifying patients at risk of developing advanced COVID-19.
Background: The adequate allocation of inpatient care resources requires assumptions about the need for health care and how this need will be met. However, in current practice, these assumptions are often based on outdated methods (e.g. Hill-Burton Formula). This study evaluated floating catchment area (FCA) methods, which have been applied as measures of spatial accessibility, focusing on their ability to predict the need for health care in the inpatient sector in Germany.
Methods: We tested three FCA methods (enhanced (E2SFCA), modified (M2SFCA) and integrated (iFCA)) for their accuracy in predicting hospital visits regarding six medical diagnoses (atrial flutter/fibrillation, heart failure, femoral fracture, gonarthrosis, stroke, and epilepsy) on national level in Germany. We further used the closest provider approach for benchmark purposes. The predicted visits were compared with the actual visits for all six diagnoses using a correlation analysis and a maximum error from the actual visits of ± 5%, ± 10% and ± 15%.
Results: The analysis of 229 million distances between hospitals and population locations revealed a high and significant correlation of predicted with actual visits for all three FCA methods across all six diagnoses up to ρ = 0.79 (p < 0.001). Overall, all FCA methods showed a substantially higher correlation with actual hospital visits compared to the closest provider approach (up to ρ = 0.51; p < 0.001). Allowing a 5% error of the absolute values, the analysis revealed up to 13.4% correctly predicted hospital visits using the FCA methods (15% error: up to 32.5% correctly predicted hospital). Finally, the potential of the FCA methods could be revealed by using the actual hospital visits as the measure of hospital attractiveness, which returned very strong correlations with the actual hospital visits up to ρ = 0.99 (p < 0.001).
Conclusion: We were able to demonstrate the impact of FCA measures regarding the prediction of hospital visits in non-emergency settings, and their superiority over commonly used methods (i.e. closest provider). However, hospital beds were inadequate as the measure of hospital attractiveness resulting in low accuracy of predicted hospital visits. More reliable measures must be integrated within the proposed methods. Still, this study strengthens the possibilities of FCA methods in health care planning beyond their original application in measuring spatial accessibility.
Aging is accompanied by unisensory decline. To compensate for this, two complementary strategies are potentially relied upon increasingly: first, older adults integrate more information from different sensory organs. Second, according to the predictive coding (PC) model, we form “templates” (internal models or “priors”) of the environment through our experiences. It is through increased life experience that older adults may rely more on these templates compared to younger adults. Multisensory integration and predictive coding would be effective strategies for the perception of near-threshold stimuli, which may however come at the cost of integrating irrelevant information. Both strategies can be studied in multisensory illusions because these require the integration of different sensory information, as well as an internal model of the world that can take precedence over sensory input. Here, we elicited a classic multisensory illusion, the sound-induced flash illusion, in younger (mean: 27 years, N = 25) and older (mean: 67 years, N = 28) adult participants while recording the magnetoencephalogram. Older adults perceived more illusions than younger adults. Older adults had increased pre-stimulus beta-band activity compared to younger adults as predicted by microcircuit theories of predictive coding, which suggest priors and predictions are linked to beta-band activity. Transfer entropy analysis and dynamic causal modeling of pre-stimulus magnetoencephalography data revealed a stronger illusion-related modulation of cross-modal connectivity from auditory to visual cortices in older compared to younger adults. We interpret this as the neural correlate of increased reliance on a cross-modal predictive template in older adults leading to the illusory percept.
Introduction and hypothesis: We conducted a systematic review of the effectiveness of local preemptive analgesia for postoperative pain control in women undergoing vaginal hysterectomy.
Methods: MEDLINE, EMBASE, the Cochrane Central Register of Controlled Trials and the Cochrane Database of Systematic Reviews were searched systematically to identify eligible studies published through September 25, 2019. Only randomized controlled trials and systematic reviews addressing local preemptive analgesia compared to placebo at vaginal hysterectomy were considered. Data were extracted by two independent reviewers. Results were compared, and disagreement was resolved by discussion. Forty-seven studies met inclusion criteria for full-text review. Four RCTs, including a total of 197 patients, and two SRs were included in the review.
Results: Preemptive local analgesia reduced postoperative pain scores up to 6 h and postoperative opioid requirements in the first 24 h after surgery.
Conclusion: Preemptive local analgesia at vaginal hysterectomy results in less postoperative pain and less postoperative opioid consumption.
Background: There is evidence of a volume outcome relationship for liver transplantation. In Germany, there is a minimum volume threshold of 20 transplantations per year for each center. Thresholds potentially lead to centralization of the healthcare supply, generating longer travel times. Objective: This study assessed whether patients are willing to travel longer times to transplantation centers for better outcomes (lower hospital mortality and higher 3-year survival) and identified patient characteristics influencing their choices. Methods: Participants were recruited in hospitals and via random samples at registration offices. Discrete choice experiments were used to identify trade-offs in their choices between local and regional centers. Descriptive statistics and logistic regression models were used to measure patients’ preferences and quantify potentially influencing characteristics. Results: Overall, 82.22% (in-hospital mortality) and 84.44% (3-year survival) of the participants opted to accept a longer travel time in order to receive a liver transplantation with better outcomes. Conclusion: Most participants were willing to trade shorter travel times for lower mortality risks and higher 3-year survival in cases of liver transplantation.
Prenatal and postnatal experiences associated with epigenetic changes in the adult mouse brain

To analyze the influences of early-life history on the brain epigenome, the offspring of mouse dams kept in an enriched or standard environment were exposed postnatally to enriched, standard, or adverse conditions. The methylation patterns of 7 candidate genes (9 loci) involved in developmental programming of stress vulnerability/resilience and psychiatric disease were analyzed in 6 brain regions of adult male and female mice. Exposure to an enriched prenatal environment was associated with widespread epigenetic changes (all of small effect size), affecting 29 of 324 (9%) gene/region-specific methylation patterns. The effects of either adverse or enriched postnatal conditions were tested separately in the two prenatal cohorts. Significant changes were observed in 2 of 324 (0.6%) loci in offspring of dams in a standard environment and 6 of 324 (1.9%) loci in animals that were exposed prenatally to an enriched environment. Prenatal life experiences appear to have a bigger effect on the adult brain epigenome than postnatal experiences. Positive prenatal life experiences may increase epigenetic plasticity of the brain later in life. All observed between-group differences were sex-specific, consistent with largely different developmental trajectories of the male and female brain. Multiple changes of small effect size are consistent with a multifactorial model of developmental programming of adult behavior and disease susceptibility.
Background: Duodenal obstruction is a rare cause of congenital bowel obstruction. Prenatal ultrasound could be suggestive of duodenal atresia if polyhydramnios and the double bubble sign are visible. Prenatal diagnosis should prompt respective prenatal care, including surgery. The aim of this study was to investigate the rate and importance of prenatally diagnosed duodenal obstruction, comparing incomplete and complete duodenal obstruction. Methods: A retrospective, single-center study was performed using data from patients operated on for duodenal obstruction between 2004 and 2019. Prenatal ultrasound findings were obtained from maternal logbooks and directly from the investigating obstetricians. Postnatal data were obtained from electronic charts, including imaging, operative notes and follow-up. Results: A total of 33/64 parents of respective patients agreed to provide information on prenatal diagnostics. In total, 11/15 patients with complete duodenal obstruction and 0/18 patients with incomplete duodenal obstruction showed typical prenatal features. Prenatal diagnosis prompted immediate surgical treatment after birth. Conclusion: Prenatal diagnosis of congenital duodenal obstruction is only achievable in cases of complete congenital duodenal obstruction by sonographic detection of the pathognomonic double bubble sign. Patients with incomplete duodenal obstruction showed no sign of duodenal obstruction on prenatal scans and thus were diagnosed and treated later.
Objective: To assess the current medical practice in Europe regarding prenatal dexamethasone (Pdex) treatment of congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency.
Design and methods: A questionnaire was designed and distributed, including 17 questions collecting quantitative and qualitative data. Thirty-six medical centres from 14 European countries responded and 30 out of 36 centres were reference centres of the European Reference Network on Rare Endocrine Conditions, EndoERN.
Results: Pdex treatment is currently provided by 36% of the surveyed centres. The treatment is initiated by different specialties, that is paediatricians, endocrinologists, gynaecologists or geneticists. Regarding the starting point of Pdex, 23% stated to initiate therapy at 4–5 weeks postconception (wpc), 31% at 6 wpc and 46 % as early as pregnancy is confirmed and before 7 wpc at the latest. A dose of 20 µg/kg/day is used. Dose distribution among the centres varies from once to thrice daily. Prenatal diagnostics for treated cases are conducted in 72% of the responding centres. Cases treated per country and year vary between 0.5 and 8.25. Registries for long-term follow-up are only available at 46% of the centres that are using Pdex treatment. National registries are only available in Sweden and France.
Conclusions: This study reveals a high international variability and discrepancy in the use of Pdex treatment across Europe. It highlights the importance of a European cooperation initiative for a joint international prospective trial to establish evidence-based guidelines on prenatal diagnostics, treatment and follow-up of pregnancies at risk for CAH.
Background & Aims: Acute-on-chronic liver failure (ACLF) is a syndrome associated with organ failure and high short-term mortality. Recently, the role of surgery as a precipitating event for ACLF has been characterised. However, the impact of preoperative transjugular intrahepatic portosystemic shunt (TIPS) placement on ACLF development in patients with cirrhosis undergoing surgery has not been investigated yet.
Methods: A total of 926 patients (363 with cirrhosis undergoing surgery and 563 patients with TIPS) were screened. Forty-five patients with preoperative TIPS (TIPS group) were 1:1 propensity matched to patients without preoperative TIPS (no-TIPS group). The primary endpoint was the development of ACLF within 28 and 90 days after surgery. The secondary endpoint was 1-year mortality. Results were confirmed by a differently 1:2 matched cohort (n = 176).
Results: Patients in the no-TIPS group had significantly higher rates of ACLF within 28 days (29 vs. 9%; p = 0.016) and 90 days (33 vs. 13%; p = 0.020) after surgery as well as significantly higher 1-year mortality (38 vs. 18%; p = 0.023) compared with those in the TIPS group. Surgery without preoperative TIPS and Chronic Liver Failure Consortium–Acute Decompensation (CLIF-C AD) score were independent predictors for 28- and 90-day ACLF development and 1-year mortality after surgery, especially in patients undergoing visceral surgery. In the no-TIPS group, a CLIF-C AD score of >45 could be identified as cut-off for patients at risk for postoperative ACLF development benefiting from TIPS.
Conclusions: This study suggests that preoperative TIPS may result in lower rates of postoperative ACLF development especially in patients undergoing visceral surgery and with a CLIF-C AD score above 45.
Lay summary: Acute-on-chronic liver failure (ACLF) is a syndrome that is associated with high short-term mortality. Surgical procedures are a known precipitating event for ACLF. This study investigates the role of preoperative insertion of a transjugular intrahepatic portosystemic shunt (TIPS) on postoperative mortality and ACLF development. Patients with TIPS insertion before a surgical procedure exhibit improved postoperative survival and lower rates of postoperative ACLF, especially in patients undergoing visceral surgery and with a high CLIF-C AD prognostic score. Thus, this study suggests preoperative TIPS insertion in those high-risk patients.
[4-(3-Bromoacetylpyridinio)-butyl]adenosine pyrophosphate as a structural analog of NAD+ reacts covalently with the sulfhydryl groups of thiopropyl agarose. 10-20 μmol can be bound to 1 ml gel. Stabilization of the insoluble coenzym e is attained by treatment with sodium boro hydride (NaBH4). This complex when applied to column chromatography, allow s the separation of various dehydrogenases as a result of their different complex stability coefficients. Alcohol dehydrogenase from liver, lactate dehydrogenase, and adenylate kinase, which all bind to the ADP-analog residues of the gel matrix, can thus be separated by different salt gradients. Alcohol dehydrogenase from yeast, however, does not form a complex and can easily be eluted from the column with phosphate buffer. Glyceraldehyde-3 phosphate and aldehyde dehydrogenases can be eluted by the addition of NAD+ or NADH to the buffer. The uncharged 1,4-dihydropyridin ring of the reduced coenzyme produces a more stable complex with the dehydrogenases than the oxidized form.
Purpose: To evaluate the prevalence and treatment patterns of speech and language disorders in Germany.
Methods: A retrospective analysis of data collected from 32% of the German population, insured by the statutory German health insurance (AOK, Local Health Care Funds). We used The International Statistical Classification of Diseases and Related Health Problems, 10th revision, German Modification (ICD-10 GM) codes for stuttering (F98.5), cluttering (F98.6), and developmental disorders of speech and language (F80) to identify prevalent and newly diagnosed cases each year. Prescription and speech therapy reimbursement data were used to evaluate treatment patterns.
Results: In 2017, 27,977 patients of all ages were diagnosed with stuttering (21,045 males, 75% and 6,932 females, 25%). Stuttering prevalence peaks at age 5 years (boys, 0.89% and girls, 0.40%). Cluttering was diagnosed in 1,800 patients of all ages (1,287 males, 71.5% and 513 females, 28.5%). Developmental disorders of speech and language were identified in 555,774 AOK-insurants (61.2% males and 38.8% females). Treatment data indicate a substantial proportion newly diagnosed stuttering individuals receive treatment (up to 45% of 6-year-old patients), with slightly fewer than 20 sessions per year, on average. We confirmed a previous study showing increased rates of atopic disorders and neurological and psychiatric comorbidities in individuals with stuttering, cluttering, and developmental disorders of speech and language.
Conclusion: This is the first nationwide study using health insurance data to analyze the prevalence and newly diagnosed cases of a speech and language disorder. Prevalence and gender ratio data were consistent with the international literature. The crude prevalence of developmental disorders of speech and language increased from 2015 to 2018, whereas the crude prevalence for stuttering remained stable. For cluttering, the numbers were too low to draw reliable conclusions. Proportional treatment allocation for stuttering peaked at 6 years of age, which is the school entrance year, and is later than the prevalence peak of stuttering.
Sleep disordered breathing (SDB) is a frequent comorbidity in cardiac disease patients. Nevertheless, the prevalence and relationship between SDB and severe primary mitral regurgitation (PMR) has not been well investigated to date. Methods: A cohort of 121 patients with significant PMR undergoing mitral valve surgery were prospectively enrolled and received a cardiorespiratory single night polygraphy screening using ApneaLink before surgery. Eighty-two of them underwent a follow-up examination including a follow-up single-night sleep study 3 months after surgery. Results: The mean age of patients was 65.3 ± 12.0 years. Sixty patients (49.6%) were female. The mean EuroSCORE II was 2.5 ± 2.4%. Initially, 91 (75.2%) patients presented with SDB, among whom 50.4% (46 patients, 38.0% of total cohort) were classified as moderate to severe. These patients tended to require significantly longer postoperative intensive care and mechanical ventilation. Among the 82 patients who completed follow-up exams, mitral valve surgery led to a significant reduction in relevant SDB (20.7%). The apnea-hypopnea index (from 11/h [4;18] to 4/h [3;14] (p = 0.04)), the oxygenation-desaturation index (from 8/h [3;18] to 5/h [3;12] (p = 0.008)) as well as the saturation time below 90% (from 32 min [13;86] to 18 min [5;36] (p = 0.005)), were all shown to be improved significantly. Conclusion: The prevalence of SDB is very high in patients with severe primary mitral regurgitation and may contribute to postoperative complications and prolonged intensive care. A significantly reduced but still high prevalence of SDB was observed 3 months after mitral valve surgery, highlighting the bidirectional relationship between SDB and heart failure.
Background: One of the lesser recognized complications of diabetes mellitus are musculoskeletal (MSK) complications of the upper and lower extremity. No prevalence studies have been conducted in general practice. Thus, the aim of this study was to investigate the prevalence of upper extremity MSK disorders in patients with type 2 diabetes (T2DM) in the Netherlands. Methods: We conducted a cross-sectional study with two different approaches, namely a representative Dutch primary care medical database study and a questionnaire study among patients with T2DM. Results: In the database study, 2669 patients with T2DM and 2669 non-diabetes patients were included. MSK disorders were observed in 16.3% of patients with T2DM compared to 11.2% of non-diabetes patients (p < 0.001, OR 1.53, 95% CI 1.31, 1.80). In the questionnaire study, 200 patients with T2DM were included who reported a lifetime prevalence of painful upper extremity body sites for at least four weeks of 67.3%. Conclusion: We found that upper extremity MSK disorders have a high prevalence in Dutch patients with T2DM presenting in general practice. The prevalence ranges from 16% based on GP registered disorders and complaints to 67% based on self-reported diagnosis and pain. Early detection and treatment of these disorders may play a role in preventing the development of chronic MSK disorders.
Recent studies suggest that synaptic lysophosphatidic acids (LPAs) augment glutamate-dependent cortical excitability and sensory information processing in mice and humans via presynaptic LPAR2 activation. Here, we studied the consequences of LPAR2 deletion or antagonism on various aspects of cognition using a set of behavioral and electrophysiological analyses. Hippocampal neuronal network activity was decreased in middle-aged LPAR2−/− mice, whereas hippocampal long-term potentiation (LTP) was increased suggesting cognitive advantages of LPAR2−/− mice. In line with the lower excitability, RNAseq studies revealed reduced transcription of neuronal activity markers in the dentate gyrus of the hippocampus in naïve LPAR2−/− mice, including ARC, FOS, FOSB, NR4A, NPAS4 and EGR2. LPAR2−/− mice behaved similarly to wild-type controls in maze tests of spatial or social learning and memory but showed faster and accurate responses in a 5-choice serial reaction touchscreen task requiring high attention and fast spatial discrimination. In IntelliCage learning experiments, LPAR2−/− were less active during daytime but normally active at night, and showed higher accuracy and attention to LED cues during active times. Overall, they maintained equal or superior licking success with fewer trials. Pharmacological block of the LPAR2 receptor recapitulated the LPAR2−/− phenotype, which was characterized by economic corner usage, stronger daytime resting behavior and higher proportions of correct trials. We conclude that LPAR2 stabilizes neuronal network excitability upon aging and allows for more efficient use of resting periods, better memory consolidation and better performance in tasks requiring high selective attention. Therapeutic LPAR2 antagonism may alleviate aging-associated cognitive dysfunctions.
General practices are rooted in the local community and considered to be particularly well-positioned for engaging in preventive and health-promoting activities. The overall aim of the scoping review is to identify priorities and gaps in research published in the past 20 years on preventive and health-promoting activities provided by general practitioners or their teams in general practices in Germany. MEDLINE and Embase databases were systematically searched in November 2020. Papers were selected in dual-review mode and extracted in single-review mode. Data analysis was finished by May 2021. In total, 530 papers were included in the synthesis. Little research has been carried out into collaboration opportunities both within the general practice team and in communities as a whole, with specialists (18%), hospitals (9%), and health insurance companies (6%) being the most frequent cooperation partners of GPs. 15%–20% of papers each dealt with ‘early detection’, ‘information provision’ and ‘cardiovascular prevention’. Secondary (53%) and tertiary prevention (43%) was more often the subject of research than primary (39%) and quaternary prevention (15%). Healthy subjects (26%) were less often studied than people with pre-existing conditions (42%) and risk factors (48%). Little information was available on preventive activities in terms of gender, young people, migration background, housing conditions or educational background. Personal counselling (15%) was the most frequently described approach to health promotion in general practices, along with printed information materials (10%). This scoping review provides information on which to base targeted interventions and future research that can contribute towards transforming general practices into promoters of health within the community.
Background: Unruptured intracranial aneurysm (UIA) poses a therapeutic dilemma in which the risk-benefit analysis of invasive intervention has to be balanced against the natural history of the disease. To date, there is no medical treatment to prevent aneurysm development and subsequent progression to rupture. We explored the vitamin D system because of its known anti-inflammatory and anti-tissue-remodeling effect as a potential treatment for UIA.
Methods: 25-vitaminD3 levels tested between 2008-2016 and data of SAH patients admitted during the months with a peak versus nadir of VitD3-values were analyzed, retrospectively. We prospectively correlated VitD3 with size and number of aneurysms at the rupture time in patients admitted between 2017-2019. An experimental mice shear stress model and cell culture model were used to investigate the effect of 1,25-dihydroxy-vitaminD3 (1,25-VitD3) and acting mediators in this mechanism.
Results: Based on the retrospective analysis demonstrating an increased frequency of aneurysm rupture rate in patients during the low vitamin D period in winter, we started the prospective study evaluating plasma vitamin D levels at admission. VitD levels were inversely correlated with aneurysm size as well as number of aneurysms. Low number of aneurysms was significantly associated with sufficient plasma Vitamin D level as an independent factor in a multivariate analysis.
From bedside back to bench, active 1,25-VitD3 hormone attenuated the natural history of remodeling in mice basilar artery. Deletion of the vitamin-D-receptor in myeloid cells decreased the protective 1,25-VitD3 effect. Cell-culture of vascular fibroblasts confirmed the anti-tissue remodeling effect of 1,25-VitD3.
Conclusion: 1,25-VitD3 attenuates aneurysm development and subsequent progression to rupture. However, VitD-administration should be tested as optional treatment in management of patients with UIA.
Background Stigma has been considered a significant barrier both in treatment, rehabilitation and help-seeking behaviours of people diagnosed with depression. This study aimed to assess the influence of the type of previous experience with depression on depression stigma, identify the effects of previous experience with depression on stigma and to analyse the effects of stigma on help-seeking attitudes.
Methods A total of 1693 participants with a mean age of 47.2 (SD=18.17) completed the Depression Stigma Scale (DSS), the Attitude Toward Seeking Professional Psychological Help (ATSPPH), and a sociodemographic questionnaire. We categorised participants into four comparison groups: no previous experience with depression (n=479), indirect experience with depression (n=661), direct experience with depression (n=137), and both direct and indirect experience with depression (n=416). Data were analysed using SPSS 24.0.
Results Levels of personal stigma were lower in people who had family and friends experiencing depression in comparison with individuals with no history of depression experience. Better attitudes towards help-seeking were evident in those with lower personal stigma, and worse help-seeking attitudes were associated with higher perceived stigma in the indirect previous experience group.
Limitations Duration of participant exposure to depression was not collected.
Conclusions The individual’s experience with depression influences the development of personal stigmatisation towards depression and plays a role in help-seeking behaviours. Addressing people’s experience of depression might be a practical way of reducing depression stigma and improve help-seeking behaviours.
Introduction: Mental disorders such as depression are common, and an estimated 264 million people are affected by them throughout the world. In recent years, studies on digital health interventions to treat mental disorders have shown evidence of their efficacy, and interest in using them has increased as a result. In the primary care setting, depression and anxiety are the two most frequently diagnosed and treated mental disorders. When they do not refer them to specialists, primary care professionals such as general practitioners treat patients with mental disorders themselves but have insufficient time to treat them adequately. Furthermore, there is a shortage of psychotherapists and those that exist have long waiting lists for an appointment. The purpose of this mixed methods systematic review is to explore the attitudes of primary care professionals towards the use of digital health interventions in the treatment of patients with mental disorders. Their attitudes will provide an indication whether digital mental health interventions can effectively complement standard care in the primary care setting.
Methods and analysis: We searched for qualitative, quantitative and mixed methods studies published in English, German, Spanish, Russian, French and Dutch after January 2010 for inclusion in the review. The included studies must involve digital mental health interventions conducted via computer and/or mobile devices in the primary care setting. The search was conducted in July 2020 in the following electronic bibliographic databases: MEDLINE, Embase, CINAHL, PsycINFO and Web of Science Core Collection. Two reviewers will independently screen titles, abstracts and full texts and extract data. We will use the ‘Integrated methodology’ framework to combine both quantitative and qualitative data.
Ethics and dissemination: Ethical approval is not required. We will disseminate the results of the mixed methods systematic review in a peer-reviewed journal and scientific conferences.
Acute myeloid leukemia (AML) is a malignant disorder derived from neoplastic myeloid progenitor cells characterized by abnormal proliferation and differentiation. Although novel therapeutics have recently been introduced, AML remains a therapeutic challenge with insufficient cure rates. In the last years, immune-directed therapies such as chimeric antigen receptor (CAR)-T cells were introduced, which showed outstanding clinical activity against B-cell malignancies including acute lymphoblastic leukemia (ALL). However, the application of CAR-T cells appears to be challenging due to the enormous molecular heterogeneity of the disease and potential long-term suppression of hematopoiesis. Here we report on the generation of CD33-targeted CAR-modified natural killer (NK) cells by transduction of blood-derived primary NK cells using baboon envelope pseudotyped lentiviral vectors (BaEV-LVs). Transduced cells displayed stable CAR-expression, unimpeded proliferation, and increased cytotoxic activity against CD33-positive OCI-AML2 and primary AML cells in vitro. Furthermore, CD33-CAR-NK cells strongly reduced leukemic burden and prevented bone marrow engraftment of leukemic cells in OCI-AML2 xenograft mouse models without observable side effects.
Introduction: The aim of this article is to show a new concept of indication and application of the MUTARS® RS Cup System in primary and revision hip arthroplasty. This integrated system is applicable for different acetabular cup replacements in patients with acetabular fractures or instable defects, as well as periprosthetic acetabular fractures. The MUTARS® RS Cup System is a cementless revision cup for insertion into the acetabulum with an integrated polyethylene cup, which fits to a regular or bipolar head. This system replaces the conventional approach for acetabular revision with a Burch-Schneider ring, in which a normal polyethylene cup is cemented. This interface with its complications is avoided with this system of a titanium revision cup with integrated polyethylene cup. Steps of preoperative planning and the intraoperative implementation will be highlighted in this article.
Material and methods: This system was applied in 49 patients with 52 MUTARS® RS Cup Implantations in 30 males, 22 females, with an average age of 76,1 years (36,9–94,4 years).
Results and discussion: The system shows a good operative feasibility, as well as a reliable handling and safe method for stable treatment of non-reconstructable acetabular fractures or acetabular incongruencies and instabilities.
Die Bestimmung von Procalcitonin im Serum stellt einen wesentlichen Bestandteil der Diagnostik, Verlaufskontrolle und Therapieüberwachung septischer Infektionen dar. Das Procalcitonin ist ein Marker, der in der Diagnostik von Infektionen, schweren Entzündungen und Sepsis wertvolle und therapieentscheidende Aussagen ermöglicht. Er sollte allerdings nicht zum Screening asymptomatischer Personen im Rahmen arbeitsmedizinischer Vorsorgen oder sog. Manager-Untersuchungen genutzt werden, sondern lediglich beim klinischen Verdacht einer vorliegenden systemischen Infektion bei entsprechenden Symptomen.
The iron-sulfur proteins of the cytochrome bc1 complexes of Schizosaccharomyces pombe and Saccharomyces cerevisiae contain the three amino acid motif RX( downward arrow)(F/L/I)XX(T/S/G)XXXX (downward arrow) that is typical for proteins that are cleaved sequentially in two steps by matrix processing peptidase (MPP) and mitochondrial intermediate peptidase (MIP). Despite the presence of this recognition sequence the S. pombe iron-sulfur protein is processed only once during import into mitochondria, whereas the S. cerevisiae protein is processed in two steps. Import of S. pombe iron-sulfur protein in which the putative MIP or MPP recognition sites are eliminated by site-directed mutagenesis and import of iron-sulfur protein into mitochondria from yeast mutants that lack MIP activity indicate that one step processing of the S. pombe iron-sulfur protein is independent of those sites and of MIP activity. Sequencing of the mature protein obtained after import in vitro and of the endogenous iron-sulfur protein isolated from mitochondrial membranes by preparative 2D-electrophoresis shows that MPP recognizes a second site in the presequence and processing occurs between residues 43 and 44. If proline-20 of the S. pombe presequence is changed into a serine, a second cleavage step is induced. Conversely, if serine-24 of the S. cerevisiae presequence is changed to a proline, the first cleavage step that is normally catalyzed by MPP is blocked, causing precursor iron-sulfur protein to accumulate. Together these results indicate that a single amino acid change in the presequence is responsible for one-step processing in S. pombe versus two-step processing in S. cerevisiae.
Aims: Parkinson's disease (PD) is frequently associated with a prodromal sensory neuropathy manifesting with sensory loss and chronic pain. We have recently shown that PD-associated sensory neuropathy in patients is associated with high levels of glucosylceramides. Here, we assessed the underlying pathology and mechanisms in Pink1−/−SNCAA53T double mutant mice. Methods: We studied nociceptive and olfactory behaviour and the neuropathology of dorsal root ganglia (DRGs), including ultrastructure, mitochondrial respiration, transcriptomes, outgrowth and calcium currents of primary neurons, and tissue ceramides and sphingolipids before the onset of a PD-like disease that spontaneously develops in Pink1−/−SNCAA53T double mutant mice beyond 15 months of age. Results: Similar to PD patients, Pink1−/−SNCAA53T mice developed a progressive prodromal sensory neuropathy with a loss of thermal sensitivity starting as early as 4 months of age. In analogy to human plasma, lipid analyses revealed an accumulation of glucosylceramides (GlcCer) in the DRGs and sciatic nerves, which was associated with pathological mitochondria, impairment of mitochondrial respiration, and deregulation of transient receptor potential channels (TRPV and TRPA) at mRNA, protein and functional levels in DRGs. Direct exposure of DRG neurons to GlcCer caused transient hyperexcitability, followed by a premature decline of the viability of sensory neurons cultures upon repeated GlcCer application. Conclusions: The results suggest that pathological GlcCer contribute to prodromal sensory disease in PD mice via mitochondrial damage and calcium channel hyperexcitability. GlcCer-associated sensory neuron pathology might be amenable to GlcCer lowering therapeutic strategies.

Das intrahepatische cholangiozelluläre Karzinom ist ein seltener, hoch aggressiver Tumor2, der zu den cholangiozellulären Karzinomen gehört1 und sich mit einem Fünfjahresüberleben von lediglich 18% durch seine ungünstige Prognose auszeichnet. Bei weltweit steigender Inzidenz und Mortalität stellt die radikale Resektion zum jetzigen Zeitpunkt die einzige kurative Behandlungsoption dar. Aufgrund der niedrigen Fallzahlen und uneinheitlichen Klassifikationen, ist der internationale Studienvergleich erschwert und die Frage nach Prognosefaktoren noch nicht abschließend geklärt.
Ziele
Ziel der Studie war es, Prognosefaktoren anhand von patienten-, tumor- und therapiespezifischen Charakteristika zu detektieren, um eine prognoseadjustierte Therapieentscheidung zu erleichtern und Ausblick hinsichtlich des Gesamtüberlebens geben zu können.
Methoden
Es handelt sich um eine retrospektive unizentrische Kohortenstudie aus der Klinik für Allgemein- und Viszeralchirurgie des Universitätsklinikums Frankfurt am Main. Eingeschlossen in die Analyse wurden 69 erwachsene Patienten mit intrahepatischem cholangiozellulärem Karzinom, die im Zeitraum von Juni 2001 und August 2013 in kurativer Absicht eine Operation als Primärtherapie erhielten.
Um den Einfluss auf das Gesamtüberleben festzustellen, wurden 48 Variablen zunächst mittels univariater Cox-Regressionsanalyse auf ihre Signifikanz getestet. Im Anschluss erfolgte eine multivariate Cox-Regressionsanalyse um Zusammenhangs-, bzw. Abhängigkeitsstrukturen zwischen den Variablen zu erkennen.
Ergebnisse
In der univariaten Cox-Regressionsanalyse ergaben sich, was die prätherapeutischen Symptome betrifft, Hinweise darauf, dass das Vorhandensein von Ikterus (p=,047), Nachtschweiß (p=,026) und Schmerzen (p=,023) die Prognose signifikant verschlechtern könnte, ebenso ein erhöhtes CEA (p=,021).
Bezüglich intraoperativer Merkmale, ließ sich außerdem ein signifikanter Zusammenhang zwischen Prognose und intraoperativem EK-Bedarf (p=,006) sowie der Anlage einer biliodigestiven Anastomose (p=,007) vermuten. Davon abgesehen konnten auch postoperative Komplikationen, wie die Galleleckage (p=,015), die schwerste eingetretene Komplikation, klassifiziert nach Dindo-Clavien (p=,001) und ein hoher CCI-Wert (p=,000) mit dem Gesamtüberleben in Verbindung gebracht werden, ebenso das Vorhandensein von Fernmetastasen (p=,015).
In der multivariaten Cox-Regressions-Analyse konnten Schmerzen, Nachtschweiß, die Anlage einer biliodigestiven Anastomose, eine postoperative Galleleckage, die schwerste Komplikation nach Dindo-Clavien und ein hoher CCI-Wert als störungsfreie, signifikante Prognosefaktoren mit unabhängigem Einfluss auf das Gesamtüberleben gewertet werden.
Schlussfolgerung
Limitiert wird die Übertragung unserer Studienergebnisse durch die relativ geringe Fallzahl. Dennoch konnten wir zeigen, dass es bestimmte Risikogruppen gibt, die nach Resektion in kurativer Intention eine schlechtere Prognose hinsichtlich des Gesamtüberlebens haben, sodass auf jene Patienten vermehrt geachtet werden sollte.
Purpose: The role of obesity in glioblastoma remains unclear, as previous analyses have reported contradicting results. Here, we evaluate the prognostic impact of obesity in two trial populations; CeTeG/NOA-09 (n = 129) for MGMT methylated glioblastoma patients comparing temozolomide (TMZ) to lomustine/TMZ, and GLARIUS (n = 170) for MGMT unmethylated glioblastoma patients comparing TMZ to bevacizumab/irinotecan, both in addition to surgery and radiotherapy.
Methods: The impact of obesity (BMI ≥ 30 kg/m2) on overall survival (OS) and progression-free survival (PFS) was investigated with Kaplan–Meier analysis and log-rank tests. A multivariable Cox regression analysis was performed including known prognostic factors as covariables.
Results: Overall, 22.6% of patients (67 of 297) were obese. Obesity was associated with shorter survival in patients with MGMT methylated glioblastoma (median OS 22.9 (95% CI 17.7–30.8) vs. 43.2 (32.5–54.4) months for obese and non-obese patients respectively, p = 0.001), but not in MGMT unmethylated glioblastoma (median OS 17.1 (15.8–18.9) vs 17.6 (14.7–20.8) months, p = 0.26). The prognostic impact of obesity in MGMT methylated glioblastoma was confirmed in a multivariable Cox regression (adjusted odds ratio: 2.57 (95% CI 1.53–4.31), p < 0.001) adjusted for age, sex, extent of resection, baseline steroids, Karnofsky performance score, and treatment arm.
Conclusion: Obesity was associated with shorter survival in MGMT methylated, but not in MGMT unmethylated glioblastoma patients.
In der vorliegenden Dissertation wurde untersucht, ob die endotracheale Intubation (ETI) der alternativen Atemwegssicherung mittels Larynxtubus (LT) bezüglich der Überlebenswahrscheinlichkeit bei außerklinisch reanimierten Patienten überlegen ist.
Das retrospektiv erfasste Kollektiv dieser monozentrischen Studie umfasst 222 Patienten, die in den Jahren 2006 bis 2014 nach nicht-traumatischem Herz-Kreislauf-Stillstand außerhalb der Klinik (engl.: Out-of-hospital cardiac arrest, OHCA) präklinisch primär mit Endotrachealtubus (ET) oder LT versorgt und anschließend auf die internistische Intensivstation des Universitätsklinikums Frankfurt am Main aufgenommen wurden. Endpunkte der Studie waren die innerklinische Gesamtmortalität während des Krankenhausaufenthaltes sowie die Überlebensrate nach 24 Stunden.
In die Analyse wurden 208 Patienten einbezogen, von denen präklinisch 48 Patienten mit LT (23 %) und 160 Patienten mit ET (77 %) intubiert wurden. Die innerklinische Sterblichkeitsrate lag insgesamt bei 85 % (n=176); darunter 23 % LT- und 77 % ET-versorgte Patienten. Zwischen den beiden Methoden zur Atemwegssicherung zeigten sich sowohl in einer univariaten Analyse (Hazard ratio [HR]=0,98; 95 % Konfidenzintervall [K.I.] 0,69-1,39; p=0,92) als auch in einer multivariaten Cox-Regressionsmodell (adjustierte HR=1,01; 95 % K.I. 0,76-1,56; p=0,62) keine Unterschiede. Die ersten 24 Stunden nach OHCA überlebten 38 % aller Patienten; auch hier unterschieden sich die beiden Patientenkollektive nicht signifikant voneinander (univariate HR=1,04; 95 % K.I. 0,71-1,52; p=0,83).
Ferner veranschaulichte eine Propensity-Score-Matching-Analyse (PSM) mit einer Subgruppe von 120 Patienten, zusammengestellt in einem 3:1 Verhältnis (ET:LT), sowohl mit Blick auf die Überlebensrate bis zur Krankenhausentlassung (Propensity-adjustierte HR=0,99; 95 % K.I. 0,65-1,51; p=0,97) als auch auf die Mortalität in den ersten 24 Stunden (Propensity-adjustierte HR=1,04; 95 % K.I. 0,44-2,36; p=0,96) vergleichbare Ergebnisse beim Atemwegsmanagement mit LT bzw. ET.
Die frühe Durchführung einer Herzkatheteruntersuchung (HR=0,47; 95 % K.I. 0,28-0,77; p=0,003) sowie der frühe Beginn einer systemischen empirischen antibiotischen Therapie (HR=0,28; 95% K.I. 0,17-0,45; p<0,001) konnten überdies als signifikant positive Prädiktoren für das Überleben in einer angepassten Cox-Regressionsanalyse herausgearbeitet werden.
Im Gesamten konnte somit demonstriert werden, dass die initiale präklinische Anwendung des LT nach OHCA der ETI hinsichtlich der Überlebenswahrscheinlichkeit nicht untergeordnet ist. Neuester Literatur zufolge ist sie womöglich sogar überlegen. Dies gilt es in weiteren Studien zu bestätigen.
Die Faktoren, welche das Fortschreiten einer H l V-Infektion anzeigen, sind noch ungenügend bekannt. Nebenklinischen und immunologischen Parametern (z.B. CD4-Lymphozytenzahl) sind virusserologische Marker von Interesse, die eine prognostische Aussage über den individuellen Krankheitsverlauf erlauben könnten. In unserem Beitrag werden einige dieser HIV-serologischen Parameter und ihre Nachweismethoden dargestellt. Das p24-Antigen kann schon in der Frühphase der HIV-Infektion nachweisbar sein und verschwindet nach der Antikörperbildung. Sein Wiederauftreten oder seine Persistenz deutet auf eine Progression der HIV-Erkrankung zum AIDS hin. Die Antigenpositivität ist mit einem signifikanten Abfall der p24-Antikörper in fortgeschrittenen Stadien der H l V-Infektion assoziiert. Die Analyse der Immunglobulinklassen zeigt, daß IgM-Antikörper bei der akuten HIV-Infektion nachweisbar sein können, jedoch keinen Reaktivierungsmarker darstellen. HIV-IgG-Antikörper gehören im wesentlichen der Subklasse lgG1 an; bei 50% der LAS/AI DS-Patienten findet man auch HIV-spezifisches IgG3. Bei Patienten mit AIDS-Enzephalopathie kann eine autochthone, intrathekale Antikörperproduktion durch die vergleichende Serum- und Liquoruntersuchung nachgewiesen werden. Als weitere Parameter, die auf eine Progression der HIV-Infektion hinweisen, werden erhöhte Antikörpertiter gegen andere opportunistische Virusinfektionen vorgeschlagen (z. B. Cytomegalie- oder Epstein-Barr-Virus.
Sleep has been shown to subtly disrupt the spatial organization of functional connectivity networks in the brain, but in a way that largely preserves the connectivity within sensory cortices. Here we evaluated the hypothesis that sleep does impact sensory cortices, but through alteration of activity dynamics. We therefore examined the impact of sleep on hemodynamics using a method for quantifying non-random, high frequency signatures of the blood-oxygen-level dependent (BOLD) signal (amplitude variance asymmetry; AVA). We found that sleep was associated with the elimination of these dynamics in a manner that is restricted to auditory, motor and visual cortices. This elimination was concurrent with increased variance of activity in these regions. Functional connectivity between regions showing AVA during wakefulness maintained a relatively consistent hierarchical structure during wakefulness and N1 and N2 sleep, despite a gradual reduction of connectivity strength as sleep progressed. Thus, sleep is related to elimination of high frequency non-random activity signatures in sensory cortices that are robust during wakefulness. The elimination of these AVA signatures conjointly with preservation of the structure of functional connectivity patterns may be linked to the need to suppress sensory inputs during sleep while still maintaining the capacity to react quickly to complex multimodal inputs.
This study investigated the effects of a daily plyometric hopping intervention on running economy (RE) in amateur runners. In a randomized, controlled trial, thirty-four amateur runners (29 ± 7 years, 27 males) were allocated to a control or a hopping exercise group. During the six-week study, the exercise group performed 5 min of double-legged hopping exercise daily. To progressively increase loading, the number of hopping bouts (10 s each) was steadily increased while break duration between sets was decreased. Pre- and post-intervention, RE, peak oxygen uptake (VO2peak), and respiratory exchange ratio (RER) were measured during 4-min stages at three running speeds (10, 12, and 14 km/h). ANCOVAs with baseline values and potential cofounders as cofactors were performed to identify differences between groups. ANCOVA revealed an effect of hopping on RE at 12 km/h (df = 1; F = 4.35; p < 0.05; η2 = 0.072) and 14 km/h (df = 1; F = 6.72; p < 0.05; η2 = 0.098), but not at 10 km/h (p > 0.05). Exercise did not affect VO2peak (p > 0.05), but increased RER at 12 km/h (df = 1; F = 4.26; p < 0.05; η2 = 0.059) and 14 km/h (df = 1; F = 36.73; p < 0.001; η2 = 0.520). No difference in RER was observed at 10 km/h (p > 0.05). Daily hopping exercise is effective in improving RE at high running speeds in amateurs and thus can be considered a feasible complementary training program.

Fat grafting is a well-established method in plastic surgery. Despite many technical advances, standardised recommendations for the use of prophylactic antibiotics in fat grafting are not available. This retrospective multicentre study aims to analyse the use of prophylactic antibiotics in fat grafting and to compare complication rates for different protocols. A retrospective medical chart review of 340 patients treated with fat grafting of the breast from January 2007 to March 2019 was performed in three plastic surgery centres. Complications, outcomes, and antibiotic regimes were analysed. The Clavien-Dindo classification was applied. All patients received perioperative antibiotic prophylaxis: 33.8% (n = 115) were treated with a single shot (group 1), 66.2% (n = 225) received a prolonged antibiotic scheme (group 2). There was no significant difference in the number of sessions (P = .475). The overall complication rate was 21.6% (n = 75), including graft resorption, fat necrosis, infection, and wound healing problems. Complication rates were not significantly different between groups. Risk factors for elevated complication rates in this specific patient group are smoking, chemotherapy, and irradiation therapy. The complication rate for lipografting of the breast is low, and it is not correlated to the antibiotic protocol. The use of prolonged prophylactic antibiotics does not lower the complication rate.
Peptide receptor radionuclide therapy (PRRT) of metastatic neuroendocrine tumors (NET) can be successfully repeated but may eventually be dose-limited. Since 177Lu-DOTATATE dose limitation may come from hematological rather than renal function, hematological peripheral blood stem cell backup might be desirable. Here, we report our initial experience of peripheral blood stem-cell collection (PBSC) in patients with treatment-related cytopenia and therefore high risk of bone-marrow failure. Five patients with diffuse bone-marrow infiltration of NET and relevant myelosuppression (≥grade 2) received PBSC before one PRRT cycle with 177Lu-DOTATATE (7.6 ± 0.8 GBq/cycle). Standard stem-cell mobilization with Granulocyte-colony stimulating factor (G-CSF) was applied, and successful PBSC was defined as a collection of >2 × 106/kg CD34+ cells. In case of initial failure, Plerixafor was administered in addition to G-CSF prior to apheresis. PBSC was successfully performed in all patients with no adverse events. Median cumulative activity was 44.8 GBq (range, 21.3–62.4). Three patients had been previously treated with PRRT, two of which needed the addition of Plerixafor for stem-cell mobilization. Only one of five patients required autologous peripheral blood stem-cell transplantation during the median follow up time of 28 months. PBSC collection seems to be feasible in NET with bone-marrow involvement and might be worth considering as a backup strategy prior to PRRT, in order to overcome dose-limiting bone-marrow toxicity.
Our lives (and deaths) have by now been dominated for two years by COVID-19, a pandemic that has caused hundreds of millions of disease cases, millions of deaths, trillions in economic costs, and major restrictions on our freedom. Here we suggest a novel tool for controlling the COVID-19 pandemic. The key element is a method for a population-scale PCR-based testing, applied on a systematic and repeated basis. For this we have developed a low cost, highly sensitive virus-genome-based test. Using Germany as an example, we demonstrate by using a mathematical model, how useful this strategy could have been in controlling the pandemic. We show using real-world examples how this might be implemented on a mass scale and discuss the feasibility of this approach.
Propranolol as a potentially novel treatment of arteriovenous malformations: from bench to bedside

Background: Propranolol is a non-selective blocker of the β-adrenergic receptor and has been used for treatment of proliferative infantile hemangiomas. The vasoconstrictive and antiangiogenic effects of propranolol led us to explore its potential application for the treatment of AVMs.
Methods: AVM tissue was cultured after surgical resection in the presence of 100μM propranolol or solvent DMSO. After incubation for 72 hours, tissue was harvested for testing. The expression levels of SDF1α, CXCR4, VEGF and HIF-1 was measured by rt-PCR. Furthermore, data of patients in 2 vascular centres harboring AVM was retrospectively interrogated for a time period of 20 years. The database included information about hemorrhage, AVM size and antihypertensive medication. Descriptive analyses were performed, focusing on the risk of hemorrhage, size of the lesion at presentation and clinical follow-up in patients on β-blocker medication versus those who were not.
Results: Among 483 patients, 73 (15%) were under β-blocker-treatment. 48% AVMs presented with hemorrhage at diagnosis. Patients under β-blocker-treatment had a lower risk of hemorrhage at the time of diagnosis in a univariate analysis (p<0,0001;OR13). Patients under β-blocker-treatment showed a significant higher chance for a lower Spetzler-Martin-grade ≤III (p<0,0001;OR6,5) and a lower risk for the presence of an associated aneurysm (p<0,0001;OR3,6).
Multivariate analysis including Spetzler-Martin-Grading, young age ≤50, presence of associated aneurysm and β-blocker-treatment showed reduced risk for hemorrhage under β-blocker-treatment (p<0,01,OR0,2).
The expression of CXCR4 was suppressed by propranolol most likely through the HIF-1-pathways. The gene-expression of vasculogenesis factors was decreased in with propranolol incubated AVMs.
Conclusion: β-Blocker medication seems to be associated with a decreased risk of AVM-related hemorrhage and AVM-size at presentation or during follow-up. Propranolol inhibits SDF1α-induced vasculogenesis by suppressing the expression of CXCR4 most likely through the HIF-1-pathways. Therefore, SDF1α/CXCR4 axis plays an important role in the vasculogenesis and migration of inflammatory cells in AVM lesions.
Die traumatische anteriore Schulterluxation ist bei jungen Männern ein häufiges Trauma, bei einer jährlichen Inzidenz von 1,7% bis 2%. Die häufigste schulterinstabilitätsspezifische Pathologie ist die Bankart Läsion. Diese kann zu einer chronisch rezidivierenden Instabilität, Schmerzen und Bewegungseinschränkungen führen. Operativ kann dieses Verletzungsmuster mit einem arthroskopischen Bankart-Repair versorgt werden. Der Goldstandard zur operativen Versorgung der weichteiligen Bankart Läsion ist die Rekonstruktion des Kapsel-Labrum-Komplexes mittels Fadenankern. Nur wenige Studien haben die propriozeptiven Eigenschaften der Gelenkkapsel und des Kapsel-Labrum-Komplexes untersucht. Eine prospektive propriozeptive Nachuntersuchung mittels Winkelreproduktionstests nach arthroskopischem Bankart Repair ist bisher nicht in der Literatur beschrieben. Die 32 Patienten, hauptsächlich junge (circa 25 Jahre alte) gesunde Männer (94%), wurden propriozeptiv und klinisch zu verschiedenen Zeitpunkten vor und nach dem arthroskopischen Bankart-Repair untersucht. Die Diagnose der anterioren primär traumatischen Schulterluxation mit Bankart-Läsion wurde mit einer MRT-Bildgebung verifiziert. Die Follow-Ups erfolgten präoperativ sowie nach 7, 14 und 31 Monaten postoperativ. Pro Patient wurden 3,7 Untersuchungen durchgeführt. Hierbei wurden klinische Scores erhoben sowie Außenrotationsdefizite mithilfe aktiver Winkelreproduktionstests mit Vicon Kameras und dem Cybex Gerät gemessen. Die Abweichung der Winkel wurde als Maß für die propriozeptiven Fähigkeiten verwendet.
Beim Vergleich der Winkelabweichungen der verletzten Schulter zu verschiedenen Zeitpunkten zeigt sich keine signifikante Differenz (p=0,424).
Auch in der Gegenüberstellung der gesunden und verletzten Schulter mithilfe einer Varianzanalyse besteht kein signifikanter Unterschied (p=0,065).
Als möglicher Einflussfaktor wurden die dominante Schulterseite und die Anzahl der implantierten Fadenanker (2 versus 3) definiert. Aufgrund der sehr geringen Probandenanzahl in den Subgruppen erfolgte eine graphische Tendenzanalyse, bei der sich bei beiden Subgruppenvergleichen ähnliche Entwicklungen darstellen ließen.
Beim Methodenvergleich wurden die Messwerte zum Zeitpunkt FU0 (präoperativ) gegenübergestellt. Dabei kann keine signifikante Differenz nachgewiesen werden (p=0,073).
Für die klinische Beurteilung wurden der Walch-Duplay-, Constant- und Rowe-Score verwendet, diese zeigten alle eine deutliche Verbesserung über die Messzeitpunkte: der Walch-Duplay-Score mit 94,78 Punkten (FU0: 41,20), der Constant-Score mit 95,83 (FU0: 75,10) und der Rowe-Score mit 91,96 (FU0: 42,5). Die erreichten Punktzahlen geben Auskunft über die Stabilität, Schulterfunktion und Beweglichkeit und liegen im ausgezeichneten Bereich.
Die Studie zeigt, dass das untersuchte Patientenkollektiv mit der gewählten Therapiemethode, dem arthroskopisch-durchgeführten Bankart-Repair, geringe propriozeptive Defizite, gute klinische Ergebnisse und eine hohe Patientenzufriedenheit erzielen konnte. Die Winkelabweichungen zeigten sich im Studienvergleich im oberen Mittelfeld. Es scheint weder durch die Bankart-Läsion noch dem arthroskopischen Bankart-Repair zu einer signifikanten Veränderung der propriozeptiven Wahrnehmung der Schulter zu kommen. Dieses Ergebnis spricht für die gute Therapie mittels dem arthroskopischen Bankart-Repair sowie die geringe Relevanz der Propriozeptoren des Kapsel-Labrum-Ligament-Komplexes.
Weiterführende Studien in längsschnittigem Studiendesign sowie die Etablierung weiterer Methoden zur standardmäßigen propriozeptiven Untersuchung wären wünschenswert und könnten zur Vergleichbarkeit und Untersuchung größerer Probandenmengen führen.
Prospective evaluation of interrater agreement between EEG technologists and neurophysiologists

We aim to prospectively investigate, in a large and heterogeneous population, the electroencephalogram (EEG)-reading performances of EEG technologists. A total of 8 EEG technologists and 5 certified neurophysiologists independently analyzed 20-min EEG recordings. Interrater agreement (IRA) for predefined EEG pattern identification between EEG technologists and neurophysiologits was assessed using percentage of agreement (PA) and Gwet-AC1. Among 1528 EEG recordings, the PA [95% confidence interval] and interrater agreement (IRA, AC1) values were as follows: status epilepticus (SE) and seizures, 97% [96–98%], AC1 kappa = 0.97; interictal epileptiform discharges, 78% [76–80%], AC1 = 0.63; and conclusion dichotomized as “normal” versus “pathological”, 83.6% [82–86%], AC1 = 0.71. EEG technologists identified SE and seizures with 99% [98–99%] negative predictive value, whereas the positive predictive values (PPVs) were 48% [34–62%] and 35% [20–53%], respectively. The PPV for normal EEGs was 72% [68–76%]. SE and seizure detection were impaired in poorly cooperating patients (SE and seizures; p < 0.001), intubated and older patients (SE; p < 0.001), and confirmed epilepsy patients (seizures; p = 0.004). EEG technologists identified ictal features with few false negatives but high false positives, and identified normal EEGs with good PPV. The absence of ictal features reported by EEG technologists can be reassuring; however, EEG traces should be reviewed by neurophysiologists before taking action.
Purpose: Monocentric, prospective study to investigate whether concomitant support of cochlear implant (CI) patients by CI-trained otolaryngologists and application of a standardized head bandage can minimize potential complications during magnetic resonance imaging (MRI).
Methods: Thirty-seven patients with 46 CIs underwent MRI with a prophylactic head bandage. All participants and the otolaryngologist at the CI center completed pre- and post-MRI questionnaires documenting body region scanned, duration of MRI and bandage wear, field strength during the scan, and any complications. If pain was experienced, it was assessed using a visual analog scale (1–10).
Results: MRI was performed without adverse events in 37.8% of cases. Magnet dislocation requiring surgical revision occurred in 2% of cases. Pain was reported in 86% of cases, often due to the tightness of the dressing. Patients with rotating, MRI-compatible magnets reported significantly less pain than participants with older-generation implants. In 11% of cases, the MRI was discontinued.
Conclusion: Serious complications during MRI in cochlear implant patients are rare. Pain is the most common adverse event, probably mainly due to the tight bandage required by most implant types. With newer generations of magnets, these patients experience less pain, no dislocation of the magnets, and no need for bandaging. Although magnet dislocation cannot be completely prevented in older generations of implants, it appears to be reduced by good patient management, which recommends examination under the guidance of physicians trained in the use of hearing implants.
Objective: Despite increased awareness of the serious epilepsy complication sudden unexpected death in epilepsy (SUDEP), a substantial population of people with epilepsy (PWE) remain poorly informed. Physicians indicate concern that SUDEP information may adversely affect patients' health and quality of life. We examined SUDEP awareness and the immediate and long-term effects of providing SUDEP information to PWE.
Methods: Baseline knowledge and behaviors among PWE and behavioral adjustments following the provision of SUDEP information were evaluated in a prospective, multicenter survey using the following validated scales: Neurological Disorders Depression Inventory for Epilepsy for depression symptoms, the EuroQoL five-dimension scale for health-related quality of life (HRQoL), a visual analog scale for overall health, the revised Epilepsy Stigma Scale for perceived stigma, and the Seizure Worry Scale for seizure-related worries. The prospective study collected data through semiquantitative interviews before (baseline), immediately after, and 3 months after the provision of SUDEP information.
Results: In total, 236 participants (mean age = 39.3 years, range = 18-77 years, 51.7% women) were enrolled, and 205 (86.9%) completed long-term, 3-month follow-up. One patient died from SUDEP before follow-up. No worsening symptoms from baseline to 3-month follow-up were observed on any scale. At baseline, 27.5% of participants were aware of SUDEP. More than 85% of participants were satisfied with receiving SUDEP information. Three quarters of participants were not concerned by the information, and >80% of participants recommended the provision of SUDEP information to all PWE. Although most patients reported no behavioral adjustments, 24.8% reported strong behavioral adjustments at 3-month follow-up.
Significance: The provision of SUDEP information has no adverse effects on overall health, HRQoL, depressive symptoms, stigma, or seizure worry among PWE, who appreciate receiving information. SUDEP information provision might improve compliance among PWE and reduce but not eliminate the increased mortality risk.
Hintergrund: Seit mehr als 50 Jahren werden in Deutschland Herzschrittmacher-Implantationen durchgeführt, mittlerweile mit mehr als 100.000 Implantationen pro Jahr. Obwohl es sich um einen gängigen Eingriff handelt, existieren wenig prospektiv randomisierte Studien zu technischen Aspekten der Implantation, insbesondere dem Wundverschluss am Ende der Operation. Ziel der vorliegenden Arbeit war es, an einem Kollektiv von Patienten unerwünschte Ereignisse und kosmetische Ergebnisse, in Abhängigkeit des beim Hautverschluss verwendeten Nahtmaterials (resorbierbarer bzw. nicht-resorbierbarer Faden), miteinander zu vergleichen.
Methoden: In einem Zeitraum von Juli 2018 bis April 2019 wurden Patienten mit geplanter de novo Herzschrittmacher-Implantation ohne Defibrillationstherapie prospektiv in die Studie eingeschlossen und anhand einer Randomisierungliste in zwei Probandengruppen eingeteilt: nicht-resorbierbares Nahtmaterial (Gruppe Prolene®) bzw. resorbierbares Nahtmaterial (Gruppe Monocryl®).
Ein Tag (Beobachtungszeitpunkt 1), sechs Wochen (Beobachtungszeitpunkt 2) und ein Jahr post-OP (Beobachtungszeitpunkt 3) erfolgte die Beurteilung der Narbe bezüglich des kosmetischen Ergebnisses und klinisch relevanter, unerwünschter Ereignisse. Zur kosmetischen Beurteilung diente die Wundbreite in mm, eine auftretende Kelloidbildung und die „Patient and Observer Scar Assessment Scale“ (POSA-Score). Dieser wurde zu Beobachtungszeitpunkt 1 seitens des Patienten auf zwei Fragen (Schmerzhaftigkeit, Juckreiz) reduziert. Die erhobenen klinisch relevanten Parameter waren Nachblutungen, Infektionen, Insuffizienz der Naht und Revisions-OP aufgrund eines Lokalbefundes.
Ergebnisse: Es konnten 114 Patienten in die Studie eingeschlossen werden. Zu Beobachtungszeitpunkt 2 und Beobachtungszeitpunkt 3 belief sich die Anzahl auf jeweils 92 Probanden. Zu allen drei Beobachtungszeitpunkten konnte zwischen beiden Gruppen weder ein signifikanter Unterschied im kosmetischen Ergebnis noch im Auftreten klinisch relevanter Ereignisse festgestellt werden.
Schlussfolgerung: Anhand der vorliegenden Studie scheint das verwendete Nahtmaterial keinen großen Einfluss auf das kosmetische Ergebnis der Narbe, sowie auf das Auftreten von unerwünschten Ereignissen zu haben. Eine multizentrische prospektiv randomisierte Studie mit größerer Patientenanzahl ist notwendig, um die hier erhobenen Daten zu verifizieren.
Menschen mit Epilepsie (engl. PWE) haben im Vergleich zur Allgemeinbevölkerung ein erhöhtes Risiko, vorzeitig zu versterben. Der plötzliche, unerwartete Tod bei Epilepsie (engl. Sudden Unexpected Death in Epilepsy, kurz SUDEP) stellt die häufigste epilepsiebedingte Todesursache dar. Obwohl das Thema in Fachkreisen zunehmende Aufmerksamkeit erfährt, die Empfehlung zur SUDEP Aufklärung zunehmend in nationalen Leitlinien aufgenommen wird, und der Patientenwunsch nach einer generellen SUDEP Aufklärung in verschiedenen Studien gezeigt werden konnte, besteht weiterhin ein Informationsdefizit unter PWE. Ursache hierfür scheint insbesondere die Sorge der behandelnden Neurolog*innen zu sein, Menschen mit Epilepsie übermäßig emotional zu belasten und ihre Lebensqualität zu mindern. Diese Studie untersucht sowohl das Vorwissen über SUDEP als auch unmittelbare sowie langfristige Auswirkungen einer SUDEP Aufklärung auf Erwachsene mit Epilepsie. Ziel ist mögliche negative Auswirkungen der Aufklärung sowie Auswirkungen auf das Verhalten aufzudecken. Aus diesem Zweck wählten wir ein prospektives, multizentrisches, longitudinales Studiendesign. Die Daten wurden in halbquantitativen Interviews vor (vor der Aufklärung), unmittelbar nach (nach der Aufklärung) und drei Monate nach (3-Monats Follow-up) der SUDEP Aufklärung erhoben. Um die direkte Vergleichbarkeit zwischen den Zeitpunkten zu ermöglichen wurden folgende validierte Instrumente verwendet: das Neurological Disorders Depression Inventory for Epilepsy (NDDI-E) zur Erfassung depressiver Symptome, der EuroQoL (EQ-5D) zur Erfassung der gesundheitsbezogenen Lebensqualität (HRQoL), eine visuelle Analogskala (VAS) zur Erfassung des allgemeinen Gesundheitszustandes, die revised Epilepsy Stigma Scale (rESS) zur Erfassung der wahrgenommenen Stigmatisierung und die Seizure Worry Scale zur Erfassung anfallsbezogener Sorgen. Insgesamt wurden 236 Teilnehmende (Durchschnittsalter: 39,3 Jahre, Spannweite: 18-77 Jahre, 51,7 % Frauen) in die Studie eingeschlossen. 205 (86,9 %) der Teilnehmenden konnten erneut im Langzeit Follow-up nach drei Monaten befragt werden. Eine der Teilnehmenden verstarb im Zeitraum des Follow-ups an SUDEP. Keines der validierten Instrumente zeigte zwischen den Zeitpunkten vor der Aufklärung und dem 3-Monats Follow-up eine Verschlechterung. Vor der Aufklärung hatten nur 27,5 % der Teilnehmenden von SUDEP gehört, und nur 9,3 % gaben an, diese Informationen von ihrer bzw. ihrem Neurolog*in erhalten zu haben. Nach der Aufklärung gaben mehr als 85 % der Teilnehmenden an, mit der SUDEP Aufklärung zufrieden oder sehr zufrieden zu sein. Drei Viertel der Teilnehmenden gaben an, durch die Aufklärung nicht oder überhaupt nicht belastet zu sein. Mehr als 80 % der Teilnehmenden befürworteten eine generelle SUDEP Aufklärung für alle Menschen mit Epilepsie. Bei dem 3-Monats Follow-up gab die Mehrheit der Teilnehmenden an, keine Verhaltensänderung vorgenommen zu haben, 24,8 % berichteten jedoch von starken Verhaltensänderungen.
Unsere Studie zeigt, dass eine SUDEP Aufklärung keine negativen Auswirkungen auf den allgemeinen Gesundheitszustand, die HRQoL, depressive Symptome, krankheitsbezogene Stigmatisierung oder Sorgen vor Anfällen hat. Insgesamt zeigte sich eine hohe Zustimmung zur SUDEP Aufklärung. Eine generelle SUDEP Aufklärung könnte sich zudem positiv auf die Compliance auswirken und das Mortalitätsrisiko senken. Eine SUDEP Aufklärung bietet allerdings keinen sicheren Schutz vor SUDEP.
Aufgrund wachsender Evidenz zu guten Langzeitergebnissen und geringen Komplikationsraten gewinnt die Prostataarterienembolisation (PAE) in der Therapie des Benignen Prostatasyndroms an Bedeutung. Durch ihren hohen technischen Anspruch bedarf es im Vorfeld einer umfassenden Untersuchung der Beckengefäßanatomie. Das Vorliegen eines zum Teil jungen Patientenkollektivs rückt zudem das Einsparen von Strahlung in den Fokus. In diesem Rahmen gewinnt die Magnet-Resonanz-Angiografie (MRA) an Aufmerksamkeit. Obwohl bereits erste Studien Erfolg versprechen, wird die MRA zur PAE-Planung zum Teil kritisch betrachtet, da sie aufwändiger und in der Auflösung unterlegen sei. In dieser Arbeit wurde untersucht, welche Vorteile die MRA im Zuge der PAE-Planung bietet und ob die klinische Effektivität der PAE unbeeinträchtigt bleibt. Weiterhin wurde untersucht, ob eine erfolgreiche MRA-geführte Planung die benötigte Strahlendosis reduziert.
In diese retrospektive Analyse wurden 56 Patienten, die zwischen Januar 2017 und April 2018 im Frankfurter Institut für Diagnostische und Interventionelle Radiologie eine PAE erhielten und bei denen ein vollständiger, die Interventionszeit und Strahlungsparameter umfassender Datensatz sowie eine MRA vor der PAE vorlagen, eingeschlossen. Zusätzlich wurden mittels International Prostate Symptom Score (IPSS), Quality of Life (QoL) und International Index of Erectile Function (IIEF) klinische Daten vor und nach der PAE erhoben. In der Magnet-Resonanz-Tomografie (MRT) vor der PAE wurden das Prostatavolumen, die Intravesical Prostatic Protrusion (IPP) und der Prostatic Urethral Angle (PUA) untersucht. Zur Analyse der Prostataarterie wurden Maximum Intensity Projection (MIP) und ein dreidimensionales Modell verwendet. Um die Auswirkungen einer erfolgreichen Urspungsanalyse auf Interventionszeit und Strahlungsparameter zu untersuchen, wurden diese Faktoren zwischen zwei Gruppen verglichen. In der ersten Gruppe konnte die Prostataarterie mittels MRA ermittelt werden, in der zweiten Gruppe war dies v.a. aufgrund von technischen Mängeln der Bildakquisition nicht möglich.
Der Nachweis des Ursprungs gelang bei 84,73% (111 von 131) der Prostataarterien, davon entsprangen 52,25% der A. pudenda interna, 18,92% zusammen mit der A. vesicalis superior, 13,51% seltenen Ursprüngen, 10,81% der A. obturatoria und 4,51% der vorderen Division der A. iliaca interna unterhalb der A. vesicalis inferior. Die Gruppe mit erfolgreicher Ursprungsanalyse mittels MRA zeigte signifikant geringere Werte in Fluoroskopiezeit (-26,96%, p = 0,0282), Dosisflächenprodukt (-38,04%, p = 0,0025) und Eingangsdosis (-37,10%, p = 0,0020). Die PAE bedingte eine signifikante Verbesserung in IPSS (p < 0,0001), Lebensqualität (p < 0,0001) und IIEF (p = 0,0016), dabei konnte der von den Patienten angegebene IPSS-Wert um durchschnittlich 9,42 Punkte (-43,37%) und der QoL-Wert um 2 Punkte (-50,00%) reduziert werden. Das Prostatavolumen (p < 0,0001), IPP (p = 0,0004) und PUA (p < 0,0001) zeigten sich ebenfalls signifikant reduziert. Das Volumen der Prostata schrumpfte um 4,92 ml (-8,35%), die IPP um 1,2 mm (-9,2%) und der PUA um 5,5° (-8,10%). Signifikante Zusammenhänge konnten zwischen IPSS- und QoL-Reduktion (p < 0,0001, r = 0,7555), sowie zwischen Höhe des IPSS vor der PAE und der absoluten IPSS-Reduktion (p = 0,0041, r = -0,4434) nachgewiesen werden.
Die MRA ermöglicht eine strahlungsfreie Analyse des Abgangs der Prostataarterie. Durch diese Auswertung konnte die benötigte Strahlendosis signifikant reduziert werden. Die MRA-geplante PAE erzielte eine deutliche Verbesserung der Symptomatik und der Lebensqualität. Die erektile Funktion konnte signifikant verbessert werden. Prostatavolumen, IPP und PUA zeigten zwar signifikante Veränderungen, wiesen jedoch keinen Zusammenhang zu klinischen Entwicklungen auf. Zwischen dem Ausgangsvolumen der Prostata und dem klinischen Ergebnis konnte ebenfalls keine signifikante Korrelation festgestellt werden, jedoch scheint der Ausgangswert des IPSS eine prädiktive Funktion zu haben.
Die MRA-geplante PAE ist klinisch effektiv und ermöglicht durch die Analyse der Prostataarterie eine Reduktion der benötigten Strahlung. Zusammen mit der MRT unterstützt sie die Indikationsstellung und Planung der PAE.
In recent years, the number and type of treatment options in advanced bladder cancer (BC) have been rapidly evolving. To select an effective therapy and spare unnecessary side effects, predictive biomarkers are urgently needed. As the host’s anti-cancer immune response is by far the most effective system to impede malignant tumor growth, immune system-based biomarkers are promising. We have recently described altered proteasomal epitope processing as an effective immune escape mechanism to impair cytotoxic T-cell activity. By altering the neoantigens’ characteristics through different proteasomal peptide cleavage induced by non-synonymous somatic mutations, the ability for T-cell activation was decreased (“processing escapes”). In the present study, we analyzed primary chemo-naïve tissue samples of 26 adjuvant platinum-treated urothelial BC patients using a targeted next-generation sequencing panel followed by the epitope determination of affected genes, a machine-learning based prediction of epitope processing and proteasomal cleavage and of HLA-affinity as well as immune activation. Immune infiltration (immunohistochemistries for CD8, granzyme B, CD45/LCA) was digitally quantified by a pathologist and clinico-pathological and survival data were collected. We detected 145 epitopes with characteristics of a processing escape associated with a higher number of CD8-positive but lower number of granzyme B-positive cells and no association with PD-L1-expression. In addition, a high prevalence of processing escapes was associated with unfavorable overall survival. Our data indicate the presence of processing escapes in advanced BC, potentially creating a tumor-promoting pro-inflammatory environment with lowered anti-cancerous activity and independence from PD-L1-expression. The data also need to be prospectively validated in BC treated with immune therapy.
In this special issue of Matrix Biology centered on proteoglycan biology we have assembled a blend of articles focused on the state-of-the-art of proteoglycanology. The field has greatly expanded in the past three decades and now encompasses all the areas of biology. This special issue is divided into five chapters describing hyaluronan metabolism, biosynthetic and catabolic pathways of proteoglycans and their roles in inflammation, cancer, repair and development. We hope that the new original work and the reviews from recognized leaders will stimulate investigations in this exciting and fertile field of research.
Correct cellular function is ensured by a complex network of proteins and enzymes, regulating protein synthesis and degradation. This protein network, maintaining the so-called protein homeostasis, regulates those processes on multiple levels, producing new or degrading old proteins to cope with changing intra- and extracellular environments. Disturbance of this tightly regulated machinery can have severe effects on the cell and can lead to a variety of pathologies on organism level. Diseases including cancer, neurodegeneration and infections are associated with causative or consequent alterations in protein homeostasis. To understand the pathologies of these diseases, it is therefore critical to examine how perturbations of protein homeostasis affect cellular pathways and physiology. In the recent years, analysis of protein homeostasis networks has resulted in the development of novel therapeutic approaches. However, for many factors it remains unclear how the cell is affected, if they are disturbed. Protein synthesis and degradation represent immediate responses of the cell to changes and need to be studied in the right timeframe, making them difficult to access by common methodology. In this work we developed a new mass spectrometry (MS) based method to study protein synthesis and degradation on a system-wide scale. Multiplexed enhanced protein dynamic (mePROD) MS was developed, overcoming these limitations by special sample mixing and novel data analysis protocols. MePROD thereby enables the measurement of rapid and transient (e.g. minutes) changes in protein synthesis of thousands of proteins. During responses of the cell to stressors (e.g. protein misfolding, oxidation or infection), two major pathways regulate the protein synthesis: the Integrated Stress Response (ISR) and mammalian target of rapamycin (mTOR). Both pathways have been connected with various diseases in the past and are common therapy targets. Although both pathways target protein synthesis in stress responses, the set of targets regulated by these pathways was believed to differ. Through the new mePROD MS method we could measure a comprehensive comparison of both pathways for the first time, revealing comparable system-wide patterns of regulation between the two pathways. This changed the current view on the regulation elicited by these pathways and furthermore represents a useful resource for the whole field of research. We could further develop the mePROD method and decrease MS measurement time needed to obtain an in-depth dataset. Through implementation of logic based instrument methods, it was possible to enhance the number of measured proteins by approximately three-fold within the same measurement time.
The dynamics of protein synthesis and degradation are frequently modulated by pathogens infecting the cell to promote pathogen replication. At the same time, the cell counteracts the infection by modulating protein dynamics as well. To develop useful therapy approaches to fight infections, it therefore is necessary to understand the complex changes within the host cell during infections on a system-wide scale. In 2019, a novel coronavirus spread around the world, causing a world-wide health-crisis. To better understand this novel virus and its infection of the host cell we conducted a study applying the mePROD methodology and classical proteomics to characterize the dynamic changes during the infection course in vitro. We discovered that the infection remodeled a diverse set of host cell pathways (e.g. mRNA splicing, glycolysis, DNA synthesis and protein homeostasis) and thereby showed possible targets for antiviral therapy. By targeted inhibition of these pathways, we could observe that these pathways indeed are necessary for SARS-CoV-2 replication and their inhibition could reduce viral load in the cells. Another experimental approach focused on the dynamic changes of protein modification, namely phosphorylation, after infection with SARS-CoV-2. Here, we could show the very important participation of growth factor signaling pathways in viral proliferation. Both studies together revealed critical pathways that are needed for the viral proliferation and hence are promising candidates for further therapies. Subsequent targeting of these pathways by either already approved drugs (Ribavirin and Sorafenib) or drugs in clinical trials (2-deoxyglucose, Pladienolide-B, NMS-873, Pictilisib, Omipalisib, RO5126766 and Lonafarnib) could block viral replication in vitro and suggests important clinical approaches targeting SARS-COV-2 infection.
Kinase fusions are considered oncogenic drivers in numerous types of cancer. In lung adenocarcinoma 5–10% of patients harbor kinase fusions. The most frequently detected kinase fusion involves the Anaplastic Lymphoma Kinase (ALK) and Echinoderm Microtubule-associated protein-Like 4 (EML4). In addition, oncogenic kinase fusions involving the tyrosine kinases RET and ROS1 are found in smaller subsets of patients. In this study, we employed quantitative mass spectrometry-based phosphoproteomics to define the cellular tyrosine phosphorylation patterns induced by different oncogenic kinase fusions identified in patients with lung adenocarcinoma. We show that exogenous expression of the kinase fusions in HEK 293T cells leads to widespread tyrosine phosphorylation. Direct comparison of different kinase fusions demonstrates that the kinase part and not the fusion partner primarily defines the phosphorylation pattern. The tyrosine phosphorylation patterns differed between ALK, ROS1, and RET fusions, suggesting that oncogenic signaling induced by these kinases involves the modulation of different cellular processes.
Acute kidney injury is associated with mortality in COVID-19 patients. However, host cell changes underlying infection of renal cells with SARS-CoV-2 remain unknown and prevent understanding of the molecular mechanisms that may contribute to renal pathology. Here, we carried out quantitative translatome and whole-cell proteomics analyses of primary renal proximal and distal tubular epithelial cells derived from human donors infected with SARS-CoV-2 or MERS-CoV to disseminate virus and cell type–specific changes over time. Our findings revealed shared pathways modified upon infection with both viruses, as well as SARS-CoV-2-specific host cell modulation driving key changes in innate immune activation and cellular protein quality control. Notably, MERS-CoV infection–induced specific changes in mitochondrial biology that were not observed in response to SARS-CoV-2 infection. Furthermore, we identified extensive modulation in pathways associated with kidney failure that changed in a virus- and cell type–specific manner. In summary, we provide an overview of the effects of SARS-CoV-2 or MERS-CoV infection on primary renal epithelial cells revealing key pathways that may be essential for viral replication.
Objectives: To analyse carbapenemases in Proteus mirabilis and assess the performance of carbapenemase detection assays.
Methods: Eighty-one clinical P. mirabilis isolates with high-level resistance at least to ampicillin (>32 mg/L) or previous detection of carbapenemases were selected and investigated by three susceptibility testing methods (microdilution, automated susceptibility testing, and disk diffusion), six phenotypic carbapenemase assays (CARBA NP, modified carbapenemase inactivation method [CIM], modified zinc-supplemented CIM, simplified CIM, faropenem, and carbapenem-containing agar), two immunochromatographic assays, and whole-genome sequencing.
Results: Carbapenemases were detected in 43 of 81 isolates (OXA-48-like [n = 13]; OXA-23 [n = 12]; OXA-58 [n = 12]; New Delhi metallo-β-lactamase (NDM) [n = 2]; Verona integron–encoded metallo-β-lactamase (VIM) [n = 2]; Imipenemase (IMP) [n = 1]; Klebsiella pneumoniae carbapenemase (KPC) [n = 1]). Carbapenemase-producing Proteus were frequently susceptible to ertapenem (26/43; 60%), meropenem (28/43; 65%), ceftazidime (33/43; 77%), and some even to piperacillin-tazobactam (9/43; 21%). Sensitivity/specificity of phenotypic tests were 30% (CI: 17–46%)/89% (CI: 75–97%) for CARBA NP, 74% (CI: 60–85%)/82% (CI: 67–91%) for faropenem, 91% (CI: 78–97%)/82% (CI: 66–92%) for simplified CIM, and 93% (CI: 81–99%)/100% (CI: 91–100%) for modified zinc-supplemented CIM. An algorithm for improved detection was developed, which demonstrated sensitivity/specificity of 100% (CI: 92–100%)/100% (CI: 91–100%) on the 81 isolates, and 100% (CI: 29–100%)/100% (CI: 96–100%) in a prospective analysis of additional 91 isolates. Interestingly, several OXA-23-producing isolates belonged to the same clonal lineage reported previously from France.
Discussion: Current susceptibility testing methods and phenotypic tests frequently fail to detect carbapenemases in P. mirabilis, which could result in inadequate antibiotic treatment. In addition, the non-inclusion of blaOXA-23/OXA-58 in many molecular carbapenemase assays further impedes their detection. Therefore, the prevalence of carbapenemases in P. mirabilis is likely underestimated. With the herein proposed algorithm, carbapenemase-producing Proteus can be easily identified.
Prothesenallergie : Diagnostik und Risikobewertung bei 172 Patienten mit Gelenk- oder Zahnersatz

Komplikationen nach prothetischer Versorgung wirken sich häufig gravierend auf die Lebensqualität der betroffenen Patienten aus. Welche Rolle eine Prothesenallergie als Ursache der Beschwerden spielt, ist bisher nicht hinreichend geklärt. In dieser Arbeit werden 172 Patienten auf die Häufigkeiten prothesenrelevanter Sensibilisierungen sowie deren Korrelation mit der klinischen Symptomatik und allergischer Vorgeschichte untersucht. Das Kollektiv gliedert sich in orthopädische und zahnärztliche Patienten, die entweder vor einer Prothesenimplantation oder nach erfolgter Behandlung getestet wurden. Alle Patienten erhalten einen Epikutantest, der neben häufigen Kontaktallergenen auch die jeweils prothesenspezifischen Stoffe enthält. Die statistische Auswertung erfolgt zunächst mit der Einteilung der Patienten in übergeordnete Kohorten, innerhalb derer die Korrelation jeweils eines speziellen Merkmals mit der Häufigkeit von Sensibilisierungen untersucht wird. Die untersuchten Merkmale sind: Orthopädische gegenüber zahnärztlichen Patienten, präoperative gegenüber postoperativen Beschwerden, Patienten mit gegenüber Patienten ohne atopische Diathese, das Geschlecht und Patienten mit beziehungsweise ohne Typ IV-Allergie. Eine weitere Aufarbeitung erfolgt nach Einteilung in sechs Patientengruppen. Die untersuchten Patientengruppen umfassen die Eigenschaften Prothesenträger, bekannte Typ IV-Allergie, bekannte atopische Diathese, aktuelle prothesenassoziierte Beschwerden beziehungsweise das Fehlen dieser Eigenschaften.
The group of proton-sensing G-protein coupled receptors (GPCRs) consists of the four receptors GPR4, TDAG8 (GPR65), OGR1 (GPR68), and G2A (GPR132). These receptors are cellular sensors of acidification, a property that has been attributed to the presence of crucial histidine residues. However, the pH detection varies considerably among the group of proton-sensing GPCRs and ranges from pH of 5.5 to 7.8. While the proton-sensing GPCRs were initially considered to detect acidic cellular environments in the context of inflammation, recent observations have expanded our knowledge about their physiological and pathophysiological functions and many additional individual and unique features have been discovered that suggest a more differentiated role of these receptors in health and disease. It is known that all four receptors contribute to different aspects of tumor biology, cardiovascular physiology, and asthma. However, apart from their overlapping functions, they seem to have individual properties, and recent publications identify potential roles of individual GPCRs in mechanosensation, intestinal inflammation, oncoimmunological interactions, hematopoiesis, as well as inflammatory and neuropathic pain. Here, we put together the knowledge about the biological functions and structural features of the four proton-sensing GPCRs and discuss the biological role of each of the four receptors individually. We explore all currently known pharmacological modulators of the four receptors and highlight potential use. Finally, we point out knowledge gaps in the biological and pharmacological context of proton-sensing GPCRs that should be addressed by future studies.
Background: Clinical trial registries increase transparency in medical research by making information and results of planned, ongoing, and completed studies publicly available. However, the registration of clinical trials remains a time-consuming manual task complicated by the fact that the same studies often need to be registered in different registries with different data entry requirements and interfaces.
Objective: This study investigates how Health Level 7 (HL7) Fast Healthcare Interoperability Resources (FHIR) may be used as a standardized format for exchanging and storing clinical trial records.
Methods: We designed and prototypically implemented an open-source central trial registry containing records from university hospitals, which are automatically exported and updated by local study management systems.
Results: We provided an architecture and implementation of a multisite clinical trials registry based on HL7 FHIR as a data storage and exchange format.
Conclusions: The results show that FHIR resources establish a harmonized view of study information from heterogeneous sources by enabling automated data exchange between trial centers and central study registries.
Providing an interactive undergraduate elective on safety culture online – concept and evaluation

Background: The COVID-19 pandemic has made it more difficult to maintain high quality in medical education. As online formats are often considered unsuitable, interactive workshops and seminars have particularly often been postponed or cancelled. To meet the challenge, we converted an existing interactive undergraduate elective on safety culture into an online event. In this article, we describe the conceptualization and evaluation of the elective.
Methods: The learning objectives of the safety culture elective remained unchanged, but the teaching methods were thoroughly revised and adapted to suit an online setting. The online elective was offered as a synchronous two-day course in winter semester 2020/21 during the “second wave” of the COVID-19 pandemic in Germany. At the end of each day, participating students evaluated the elective by completing an online survey. Items were rated on a six-point Likert scale. We used SPSS for data analysis.
Results: Twenty medical undergraduates completed the elective and rated it extremely positively (1.1 ± 0.2). Students regard safety culture as very important and felt the learning objectives had been achieved. Moreover, they were very satisfied with the design and content of the elective, and especially with interactive elements like role-play. Around 55% of participants would recommend continuing to offer the online elective after the pandemic.
Conclusions: It makes sense to offer undergraduate medical students online elective courses on safety culture, especially during a pandemic. The elective described here can serve as a best practice example of how to teach safety culture to undergraduates, especially when physical presence is unfeasible. Electives requiring a high degree of interaction can also function well online.
Since the early 1970s several studies have reported distal splenic artery embolization, better known as partial spleen embolization (PSE), as an efficacious treatment of portal hypertensive variceal bleeding and hypersplenism in cirrhosis.(1, 2) However, the effect of PSE on portal pressure is secondary to the induction of splenic infarction. Depending on both the infarct volume and possible infection, PSE can induce serious complications including death.(2, 3) On the other hand, proximal splenic artery embolization (PSAE), which mimics surgical splenic artery ligation, prevents large infarction of the spleen, favoring collateral perfusion of its intact distal vasculature.(3) For this, PSAE has been extensively preferred over PSE for reducing portal hyperflow and treating refractory ascites (RA) after whole or partial liver transplantation (LT).(3, 4) We report here a case of PSAE used to treat RA in a patient with cirrhosis not eligible for transjugular intrahepatic portosystemic shunt (TIPS) and LT.
Zwangsmaßnahmen stellen in der psychiatrischen Versorgung ein umstrittenes Thema dar. Obwohl soweit wie möglich eine zwangsfreie Psychiatrie angestrebt wird, können psychiatrische Erkrankungen in einzelnen Fällen die Selbstbestimmungsfähigkeit der Patienten einschränken und Zwangsmaßnahmen notwendig machen. Diese können eine erhebliche Belastung für die betroffenen Patienten darstellen und bringen das medizinische Personal unter Umständen in ein ethisches Dilemma zwischen Fürsorgepflicht für erkrankte Patienten und deren Autonomiewahrung. Da sowohl national als auch international noch kein einheitlicher Konsens bezüglich möglicher Prädiktoren einer Zwangseinweisung und deren Auswirkung auf den Therapieverlauf erreicht wurde, besteht dazu noch weiterer Forschungsbedarf.
Um diese Lücke zu verkleinern, wurde eine aktuelle, retrospektive Studie auf der Grundlage der besonderen rechtlichen Situation in Hessen bezüglich des Hessischen Freiheitsentzugsgesetzes (HFEG) durchgeführt. Hierfür wurden die Patientenakten aller volljährigen Patienten, die freiwillig oder unfreiwillig (nach §10 HFEG, §1906 BGB und §1 HFEG) auf eine der drei geschützten Stationen der Psychiatrie des Universitätsklinikums Frankfurt am Main während der Quartale I/2015 und II/2015 aufgenommen wurden, ausgewertet. Dabei wurden die Unterschiede der freiwillig und der unfreiwillig aufgenommenen Patienten in soziodemographischen Prädiktoren und die Auswirkung des Aufnahmemodus auf den Therapieverlauf durch Signifikanzprüfungen und dem Verfahren der logistischen Regression betrachtet. Analysiert wurden 543 Patienten, von denen ca. 70% freiwillig und 30% unfreiwillig aufgenommen wurden.
Es konnten vier mögliche Prädiktoren einer unfreiwilligen Aufnahme identifiziert werden: Initiierung der Aufnahme, Aufnahmezeitpunkt, Hauptdiagnose und somatische Nebendiagnosen. Eine unfreiwillige Aufnahme wurde eher durch Passanten, den Betreuer/die Einrichtung oder Nachbarn/Freunde initiiert, während freiwillige Aufnahmen eher durch die Familie oder Eigeninitiative motiviert waren. Die Betrachtung der Initiierung einer Aufnahme erfolgte in der Literatur bisher selten. Übereinstimmend mit anderen Studien wurden zwangseingewiesene Patienten eher wochentags außerhalb der Kernarbeitszeiten oder am Wochenende/Feiertag aufgenommen, während freiwillige Aufnahmen vermehrt in der Kernarbeitszeit stattfanden. Die Diagnosen F0 und F4 erwiesen sich in der Regressionsanalyse als Risikofaktoren für eine Zwangseinweisung, wodurch der in der Literatur bestehende Kenntnisstand eines Zusammenhangs zwischen der Diagnose und dem Aufnahmemodus bestätigt wurde. Unfreiwillig aufgenommene Patienten wiesen zudem signifikant weniger somatische Nebendiagnosen auf. Der Einfluss der Nebendiagnosen ist in der Literatur noch nicht ausreichend untersucht, auch wenn die Ergebnisse der vorhandenen Studien größtenteils mit der vorliegenden Studie übereinstimmen.
Hinsichtlich des Therapieverlaufs unterschieden sich die beiden Gruppen in den Variablen Aufenthaltsdauer, Unterbringung im Verlauf, Zwangsfixierung, Zwangsmedikation, medikamentöse Einstellung, Antidepressiva und andere Medikamente. Zwangseingewiesene Patienten zeigten eine kürzere Aufenthaltsdauer, was möglicherweise der besonderen rechtlichen Situation in Hessen geschuldet ist. Sie wurden zudem häufiger im Verlauf des Aufenthalts untergebracht, waren öfter von Zwangsmaßnahmen wie Fixierungen oder Zwangsmedikationen betroffen und erhielten generell weniger Medikamente. Möglicherweise wurde dies durch eine fehlende Behandlungsmotivation oder eine kürzere Aufenthaltsdauer bedingt. In zusätzlichen Analysen zeigte sich, dass sich einfach und mehrfach aufgenommene Patienten in Baseline-Parametern nicht signifikant unterschieden.
Auch wenn Limitierungen wie das Vorliegen einer unizentrischen Studie oder einem Auswertungszeitraum von einem halben Jahr vorliegen, geben die in dieser Studie gefundenen Zusammenhänge wichtige Hinweise, um das Zustandekommen und die Auswirkungen von Zwangseinweisungen besser zu verstehen. Mit dieser Studie als Vergleichsgrundlage kann in Zukunft eine erstmalige Evaluierung der PsychKHG-Einführung in Hessen durchgeführt werden.
Das prämenstruelle Syndrom (PMS) ist ein häufiges Krankheitsbild, das bei 20–30 % der gebärfähigen Frauen auftritt. Es wird durch zyklusabhängige psychische und somatische Symptome definiert.
Die prämenstruelle dysphorische Störung (PMDS) ist eine schwere Form des PMS mit vor allem psychischen Auffälligkeiten, die bei 2–8 % der gebärfähigen Frauen vorkommt. Die Ätiologie von PMS und PMDS bleibt unklar. Eine PMDS geht oft mit anderen psychischen Erkrankungen einher.
Die Symptome treten in der zweiten Zyklushälfte auf und lassen mit Beginn der Periode nach. Zur Diagnosestellung hilft die Führung eines Zykluskalenders.
Ein gesunder Lebensstil (Sport, ausgeglichene Ernährung etc.) stellt die Basis für alle Therapieoptionen dar. Zahlreiche Studien konnten die Effektivität der Behandlung mit Antidepressiva (aus der SSRI- oder SNRI-Gruppe) sowie Ovulationshemmern darlegen.
Beide Medikamentengruppen werden in Deutschland im „off label use“ zur Behandlung von PMS/PMDS-Beschwerden angewendet. Für die Therapieempfehlung sind die Grunderkrankungen der Patientin zu berücksichtigen.
In der präoperativen Diagnostik fokaler maligner Leberläsionen nimmt die MRT mit Resovist® einen zunehmend höheren Stellenwert ein.
T2 gewichtete TSE- und FS-Sequenzen der Resovist® unterstützten MRT sind dabei sensitiver als die in der präoperativen Diagnostik etablierte CTAP mit Ultravist® 370, einem nichtionischen jodhaltigen Kontrastmittel.
Die visuelle Differenzierung und Charakterisierung fokaler maligner Leberläsionen und deren Abgrenzung vom Leberparenchym in der Resovist® unterstützten MRT kann durch die Analyse der Signalintensitätskurven optimiert werden. Eine effektivere Charakterisierung der fokalen Leberläsionen wirkt sich damit positiv auf die Sensitivität und Spezifität der SPIO verstärkten MRT aus.
Es bleibt daher offen, die Signalintensitäten der primären und sekundären Leberläsionen der hier nicht untersuchten Gewebearten in weiteren Studien zu analysieren.
Desweiteren könnten nachfolgende Untersuchungen das Ziel haben, die Sensitivität und Spezifität der Resovist® unterstützten MRT durch eine Verminderung der Schichtdicke zu verbessern, womit sich die Detektion von Läsionen < 1 cm effektiver gestalten würde.
Nierensteine sind eine häufige Diagnose, welche Patient und Gesundheitssystem gleichermaßen belasten. In dieser Arbeit sollten deshalb bekannte präoperative und intraoperative Faktoren bestätigt und neue identifiziert werden, welche das Ergebnis bei der endourologischen Steintherapie durch rigide oder flexible Ureterorenoskopie vorhersagen können. Die untersuchten Outcome-Variablen waren die Steinfreiheit, die postoperative Schmerzfreiheit, sowie die ökonomischen Faktoren OP-Zeit und Verweildauer. Ist eine Prädiktion dieser Variablen möglich, so wird der Krankenhausaufenthalt für Patient und Kliniken besser planbar, zudem kann anhand der ökonomischen Faktoren abgeschätzt werden, wie rentabel die Behandlung sein wird. Zu diesem Zweck sollten aus den Prüfvariablen Scores erstellt werden, welche die Steinfreiheit möglichst zuverlässig vorhersagen und bei gleicher Prädiktionskraft einfacher anzuwenden sind als der bekannte S.T.O.N.E. Score zur Abschätzung der Steinfreiheit nach starrer und flexibler URS. Zudem sollten erstmals auch Outcome-Scores für die OP-Zeit, die Verweildauer und die postoperative Schmerzfreiheit erstellt werden.
Hierfür wurden zunächst Patientendaten, sowie radiologische und intraoperative Ergebnisse zusammengetragen und mittels statistischer univariater Analyse auf einen Zusammenhang mit den Outcome-Faktoren überprüft. Hierbei wurden die starre und die flexible URS getrennt analysiert. Im nächsten Schritt wurden in multivariater Analyse die unabhängigen Faktoren identifiziert, welche das Outcome beeinflussen. Aus diesen Variablen wurden schließlich Scores errechnet und deren Prädiktionskraft im Hinblick auf das klinische und ökonomische Outcome nach URS mittels ROC-Analyse untersucht und verglichen. Für die Vorhersage der Steinfreiheit konnte zu jedem Eingriff ein Score erstellt werden, der bei gleicher oder besserer Prädiktionskraft mit weniger Variablen auskommt, als der bisher bekannteste publizierte S.T.O.N.E. Score und somit leichter anzuwenden ist. Der Renewal-Score für die starre URS umfasst die Parameter Steinlänge, Steinlokalisation, Steinanzahl und initiale Notfallvorstellung der Patienten, der Flexfree-Score für die flexible URS beinhaltet hingegen die Steinlänge, eine präinterventionelle DJ-Kathetereinlage und die Erfahrung des Urologen. Auch für die ökonomischen Parameter Operations- und Verweildauer konnten erstmals spezifische Outcome-Scores erstellt werden, lediglich die Schmerzfreiheit ließ sich mit den gesammelten Daten nicht vorhersagen. Bei der flexiblen URS konnte der zur gemeinsamen Prädiktion von OP- und Verweildauer geeignete Fleconomy-Score aus den Variablen Steinbreite und Steinvorgeschichte errechnet werden. Bei der starren URS mussten getrennte Scores erstellt werden. Für die OP-Dauer wurde der Ritime-Score aus den Parametern Steinlänge, Steinbreite, Steinlokalisation und Notfallvorstellung errechnet. Auch der Renewal-Score zur Vorhersage der Steinfreiheit nach rigider URS eignete sich zur Prädiktion der Operationszeit. Der Ristay-Score zur Vorhersage der Verweildauer nach starrer URS umfasst hingegen die Faktoren präoperative DJ-Kathetereinlage, den präinterventionellen Kreatininwert und die OP-Zeit. Auch die ökonomischen Tests sind klinisch einfach zu bestimmen und kommen bei hoher Vorhersagegüte mit wenigen Variablen aus. Alle erstellten Scores sind praxistauglich und stellen eine Weiterentwicklung der bisher zur Verfügung stehenden Tools oder komplette Neuerungen zur Vorhersage des Outcomes nach endourologischer Steintherapie dar. Dies ist nicht nur für den Patienten von Bedeutung, sondern hilft auch den Kliniken OP- und Verweiltage besser zu planen und somit den Behandlungsertrag zu kalkulieren.
Hintergrund: Anämie gehört zu den häufigsten Erkrankungen weltweit und stellt daher ein zentrales globales Gesundheitsproblem dar. Etwa ein Drittel aller chirurgischen Patienten weisen präoperativ eine Anämie auf. Dies wird als unabhängiger Risikofaktor für eine erhöhte Morbidität und Mortalität sowie für das vermehrte Auftreten postoperativer Komplikationen angesehen. Vor diesem Hintergrund wurde von der Weltgesundheitsorganisation (World Health Organization, WHO) ein patientenzentriertes Behandlungskonzept namens Patient Blood Management (PBM) gefordert, um unter anderem Anämien frühzeitig zu diagnostizieren und einen rationalen Einsatz von Fremdblutprodukten zu fördern. Trotz der Tatsache, dass PBM vor etwa 10 Jahren eingeführt wurde, scheint Deutschland im internationalen Vergleich eine überdurchschnittlich hohe Menge an Erythrozytenkonzentrat-(EK)Einheiten pro Kopf zu transfundieren. Die Ursachen dafür sind bislang unklar und könnten möglicherweise durch eine erhöhte Anämie- Prävalenz bei chirurgischen Patienten und dem damit verbundenen erhöhten Bedarf an Transfusionen einhergehen.
Zielsetzung: Ziel dieser Arbeit ist die multizentrische Erfassung der präoperativen Anämie-Prävalenz innerhalb des Zeitraums 2007-2019 in Deutschland.
Methoden: In dieser retrospektiven, multizentrischen Beobachtungsstudie wurden alle Patienten jeden Alters, die im Monat März der Jahre 2007, 2012, 2015, 2017 und 2019 in acht teilnehmenden Krankenhäusern operiert wurden, erfasst. Patientencharakteristika und klinische Daten wurden aus den Krankenhausinformationssystemen der teilnehmenden Krankenhäuser entnommen. Primäres Ziel war die Prävalenz der Anämie bei Krankenhausaufnahme. Sekundäre Endpunkte waren der Zusammenhang zwischen Anämie und Anzahl der transfundierten EKs, Dauer des Krankenhausaufenthalts und Sterblichkeit im Krankenhaus.
Ergebnisse: Von insgesamt 30.763 Patienten konnten 23.836 Patienten aus acht Krankenhäusern in die Analyse eingeschlossen werden. Im untersuchten Zeitraum zeigte sich eine Reduktion der präoperativen Anämie-Prävalenz erwachsener Patienten von 37% auf 32,2%. Die Zahl, der mit Erythrozytenkonzentraten -5- transfundierten Patienten, ging signifikant von 16,5 % im Jahr 2007 auf 7,8 % im Jahr 2019 zurück, wobei Patienten mit präoperativer Anämie im Vergleich zu Patienten ohne Anämie sechsmal mehr EKs erhielten. Insgesamt verkürzte sich die Krankenhausverweildauer seit 2007 deutlich, zeigte jedoch eine signifikante Verlängerung bei Vorliegen einer präoperativen Anämie. Die Sterblichkeitsrate war über die Jahre hinweg konstant, dennoch signifikant höher bei anämischen Patienten. Eine multivariate Regressionsanalyse mit festen Effekten ergab, dass präoperative Anämie und EK-Transfusion Prädiktoren für die Sterblichkeitsrate waren.
Diskussion: Die Prävalenz der präoperativen Anämie lag in unserer Studienpopulation im Jahr 2019 bei 32,2 %, was der weltweiten Prävalenz entspricht. Das Implementieren von PBM-Strategien, um präoperative Anämien frühzeitig zu identifizieren und zu therapieren sowie Blutprodukte rational einzusetzen, nimmt daher weiterhin einen großen Stellenwert ein. Diese perioperativen Maßnahmen sind für alle chirurgischen Patienten von zentraler Bedeutung, da eine präoperative Anämie den Bedarf an EK-Transfusionen erhöhen, die Liegezeit verlängern und mit einer erhöhten Sterblichkeitsrate assoziiert sein kann.
Neben einer guten Tanztechnik spielt die Körperform von professionellen Tänzer*innen eine entscheidende Rolle. Im klassischen Tanz gilt eine leptosom-grazile Form mit langen Gliedmaßen bei Frauen als ideal. Dementsprechend ist die Prävalenz von Untergewicht hoch. Dabei ist es nicht ausgeschlossen, dass dieses physische Erscheinungsbild mittels Diäten erreicht wird. Auffällig ist eine höhere Prävalenz von Störungen des Essverhaltens und Essstörungen vor allem bei Tänzerinnen im Vergleich zu nicht tanzenden Vergleichsgruppen. Im Zusammenhang mit dem Untergewicht können hormonelle Störungen, wie Amenorrhoe, Osteoporose oder Leistungseinschränkungen auftreten. Daher ist es wichtig, Tänzer*innen regelmäßig zu untersuchen und präventive Maßnahmen zu ergreifen. Ziel der vorliegenden Übersicht ist die Darstellung der Literatur zu diesem Thema.
Mit S-303 pathogenreduzierten Erythrozytenkonzentraten können sowohl infektiöse also auch nicht-infektiöse Risiken einer Transfusionen möglicherweise weiter reduziert werden. Frühere Studien mussten abgebrochen werden, da Antikörper gegen S-303-behandelte Erythrozyten festgestellt wurden. S-303 reagiert als unerwünschten Nebeneffekt auch mit Oberflächenmolekülen der roten Blutkörperchen. Dabei wird die Acridin-Komponente von S-303 auf der Oberfläche der roten Blutkörperchen gebunden. Gegen diese Acridin-Komponente können nach Transfusion von pathogenreduzierten EK immuninduzierte AK gebildet werden. Es existieren aber auch natürlich präformierte AK gegen S-303-behandelte Erythrozytenkonzentrate. Das Verfahren wurde modifiziert, indem der Anteil von GSH erhöht wurde. Dadurch wird vermehrt S-303 im Extrazellulärraum abgefangen und weniger Acridin auf der Oberfläche der Erythrozyten gebunden.
In dieser Studie wurden 9671 potentiell transfusionsbedürftigen Patienten auf einen natürlich präformierten Antikörper gegen S-303-behandelte Erythrozytenkonzentrate untersucht. Diese Patienten hatten zuvor noch keine S-303-behandelten Erythrozytenkonzentrate erhalten. Es wurden Testzellen von Blutspendern der Blutgruppe 0 hergestellt. Aus jedem EK wurden drei unterschiedliche Testzellen hergestellt (Erstgenerationszellen, Zweitgenerationszellen, Kontrollzellen). Das Screening erfolgte mittels IAT in Gelkartentechnik mit einer Testzelle der ersten und zweiten Generation. Die Antikörper wurden weiter charakterisiert bezüglich Spezifität, Titer, Antikörperklasse und –subklasse, Affinität und Temperaturreaktivität.
Bei zwölf Patienten konnte ein AK gegen S-303 pathogenreduzierte Erythrozy-tenkonzentrate nachgewiesen werden und somit konnte gezeigt werden, dass die Prävalenz eines solchen Antikörpers gering ist. Bei elf Patienten handelt es sich um einen Anti-Acridin-AK. Alle elf Patienten hatten immer einen positiven AKS mit der Erstgenerationszelle, jedoch nicht mit der Zweitgenerationszelle. Die Sensitivität, einen AK gegen Acridin mit der Erstgenerationszelle zu finden, ist höher als mit der Zweitgenerationszelle. Die Erstgenerationszelle hat auf ihrer Oberfläche mehr Acridin gebunden als die Zweitgenerationszelle. Bei einem Patienten konnte ein AK unbekannter Spezifität nachgewiesen werden, welcher jedoch ausschließlich mit der Zweitgenerationszelle reagierte. Vor einer möglichen Transfusion von S-303 pathogenreduzierten Erythrozytenkonzentraten wird empfohlen immer eine Testung auf Antikörper gegen Testzellen der ersten und der zweiten Generation durchzuführen. Eine Transfusion sollte grundsätzlich nur erfolgen, wenn weder ein AK gegen die Erstgenerationszelle noch gegen die Zweitgenerationszelle nachgewiesen wurde. Insgesamt handelt es sich überwiegend um niedrigtitrige IgG-AK der Subklasse 2 oder 4. Es lässt sich daher vermuten, dass natürlich präformierten AK gegen S-303-behandelte Erythrozytenkonzentrate nur eine geringe klinische Relevanz haben.
Eine klinischen Studie mit 51 kardiochirurgischen Patienten zeigte, dass nach Transfusion von pathogenreduzierten Erythrozytenkonzentraten der zweiten Generation keine immuninduzierten Antikörper gebildet wurden. Somit ist davon auszugehen, dass die Zweitgenerationszelle wegen der geringen Expression von Acridin auf der Zelloberfläche weniger immunogen ist als die Erstgenerationszelle. Klinische Studien mit einer größeren Anzahl an Patienten können nun grundsätzlich geplant werden.
Generell sollten Patienten mit natürlich präformierten Antikörpern gegen S-303-behandelte Erythrozytenkonzentrate keine pathogenreduzierten Erythrozytenkonzentrate erhalten. Patienten, welche nach Transfusion von S-303-behandelten Erythrozytenkonzentraten AK gegen die pathogenreduzierten Erythrozyten gebildet haben, sollten regelmäßig nachuntersucht werden, um eine Hämolyse frühzeitig zu entdecken.
Die spontan bakterielle Peritonitis (SBP) ist eine spontan auftretende, bakterielle Infektion des Peritoneums und gilt als eine der schwerwiegendsten Akutkomplikationen der dekompensierten Leberzirrhose. Die Diagnosestellung der SBP erfolgt aufgrund der okkulten klinischen Symptome häufig verspätet, was mit einer deutlich verkürzten Lebenserwartung einhergeht. Auch im Falle einer Kolonisation oder Infektion durch multiresistente Erreger (MRE) ist die Prognose bei Patienten mit dekompensierter Leberzirrhose deutlich vermindert. Der pathophysiologische Zusammenhang sowie die prognostische Relevanz von MRE als Auslöser einer SBP sind bislang nicht bekannt. Die Prävalenz der MRE bei Patienten mit Leberzirrhose ist jedoch steigend. Die Identifizierung von Patienten mit hohem Risiko für eine Infektion mit MRE ist unerlässlich für eine frühzeitige Anpassung der Antibiotikatherapie bei diesen Patienten. In einem Ansatz, die Wahrscheinlichkeit einer durch MRE ausgelösten SBP (MRE-SBP) zu beurteilen, wurde untersucht, ob in Asziteskulturen nachgewiesene MRE auch durch nicht-invasive Screening-Verfahren ermittelt werden können.
Die vorliegende Arbeit wurde am Leberzentrum des Universitätsklinikums Frankfurt am Main durchgeführt. In einer retrospektiven Studie untersuchten wir Patienten aus den Jahren 2011 bis 2016 mit Diagnose einer SBP, bei denen aerobe und anaerobe Asziteskulturen entnommen worden sind und ein vollständiges MRE-Screening (multiresistente gram-negative Bakterien, Vancomycin-resistente Enterokokken (VRE) oder Methicillin-resistenter Staphylokokkus aureus (MRSA)) durchgeführt worden ist. Ziel der Studie war letztlich die Charakterisierung von Patienten mit SBP und MRE-Nachweis (MRE- SBP), die Untersuchung des Überlebens dieser Patienten und die Definition von Risikofaktoren für eine MRE-SBP.
133 Patienten erfüllten die Einschlusskriterien. Unter diesen hatten 75/133 Patienten (56%) positive Asziteskulturen und 22/133 (17%) eine SBP mit kulturellem MRE-Nachweis im Aszites (MRE-SBP). Von den Patienten mit positiven Asziteskulturen hatten 29/75 (39%) gram-positive, 25/75 (33%) gram- negative und 21/75 (28%) sowohl gram-positive als auch gram-negative Erreger
im Aszites. Multiresistente Erreger (multiresistente gram-negative Erreger, VRE oder MRSA) wurden insgesamt bei 72/133 Patienten (54%) ermittelt. Unter den Patienten mit MRE-SBP dominierten der multiresistente Escherichia coli (E.coli) (10/22; 46%) und VRE (7/22; 31%). Rektalabstriche identifizierten MRE bei 17/22 Patienten (77%), die eine MRE-SBP mit einer zeitabhängigen Sensitivität von 77% und 87% innerhalb von 30 bzw. 90 Tagen nach dem Screening entwickelten; der negativ prädiktive Wert betrug 83% bzw. 76%. Es bestand kein relevanter Unterschied des MELD-, CLIF-AD- und ALBI-Scores bei Patienten mit und ohne MRE-SBP. Eine Sepsis war auch bei Patienten mit MRE-SBP die Haupttodesursache (8/22; 36%; p<0.001) verglichen mit anderen Todesursachen in 7/22 Fällen (32%). Eine Umstellung der initialen antiinfektiven Therapie ging mit einem erhöhten Sterblichkeitsrisiko bei Patienten mit (p=0.002) und ohne MRE-SBP (p=0.014) einher.
Wir konnten zeigen, dass unabhängig von der Leberfunktion das Überleben bei Patienten mit MRE-SBP stärker beeinträchtigt ist als bei anderen Patienten mit SBP. Das Auftreten von multiresistenten Organismen ist eine schwerwiegende Komplikation bei Patienten mit dekompensierter Leberzirrhose und Ansätze zur Verbesserung des Überlebens dieser Patienten sind erforderlich. Unter Berücksichtigung der Hospitalisierungsdauer, MRE-Historie und ggfs. des zu erwartenden Erregerspektrums der hiesigen Stationen muss daher bereits initial eine individuelle, empirische antiinfektive Therapie gewählt werden. Eine entsprechende Anpassung nach Erhalt des Antibiogramms kann erforderlich sein. Weiterhin untersuchten wir den Nutzen eines nicht-invasiven Screenings (rektales, nasopharyngeales und kutanes MRE-Screening), um Patienten mit Risiko für eine MRE-SBP zu identifizieren. Das MRE-Screening kann als nicht- invasives diagnostisches Instrument hierfür dienen. Patienten mit dekompensierter Leberzirrhose sollten vom ersten Tag der stationären Behandlung auf Kolonisierung mit MRE untersucht werden.
Es wurde die Seroprävalenz von Erregern mit und ohne Tropismus zu Gefäßendothelzellen, wie Chlamydia (C.) pneumoniae, C. trachomatis, C. psittaci, verschiedene Herpesviren (Cytomegalie-Virus (CMV), Epstein-Barr-Virus (EBV), Herpes simplex Typ l und 2-Virus, Varizella Zoster-Virus (VZV)) sowie Masern- und Mumpsviren, bei Patienten mit koronarer Herzerkrankung (KHK) (n=167) und zwei Kontrollkollektiven ohne Herzerkrankung (n=400, n=108) ermittelt. Die IgG-Antikörperprävalenzen betrugen im KHK-Kollektiv für C. pneumoniae 79,6% bzw. in den Kontrollkollektiven 72,5% und 66,7%, für C. trachomatis 4,8% bzw. 6,8% und 2,8%, für C. psittaci 3% bzw. 3,5% und 6,5%, für CMV 72,9% bzw. 74,3% und 79,2%, für EBV 95,1% bzw. 93.1% und 94%, für Herpes simplex 1/2 91,8% bzw. 87,4% und 91,3%, für Masern 99,2% bzw. 100% und für Mumps 93,4% bzw. 86,5% und 84,8%. Die Prävalenzen der VZV-IgA waren 60,3% bzw. 57,3% und 54%. Bei dieser Untersuchung zeigten sich somit keine signifikanten Unterschiede in den Antikörperprävalenzen zwischen den einzelnen Kollektiven. Wurden kranke, stationäre Patienten aus dem Kontrollkollektiv ausgeschlossen, so fand sich in diesem zweiten Kontrollkollektiv mittels Chi2-Test eine signifikant niedrigere Prävalenz (66,7%) der C. pneumoniae-IgG-Antikörper im Vergleich zu Patienten mit KHK (79,6%), (p=0,02). Die Untersuchung der geschlechtsspezifischen Prävalenzen zeigte für Männer (82,6% bzw. 78,5% und 73,2%) eine höhere Durchseuchung als für Frauen (55,6% bzw. 63% und 59,6%). Beim Vergleich gleichgeschlechtlicher Gruppen fanden sich keine signifikanten Unterschiede. Eine Assoziation von C. pneumoniae oder CMV mit der atherosklerotischen, koronaren Herzerkrankung konnte somit durch unsere* Untersuchung nicht bestätigt werden. Die Wahl der Kontroll-Gruppen kann möglicherweise die Ergebnisse der Metaanalyse früherer Assoziationsstudien beeinflußt haben.
Die traditionellen Behandlungspositionen der Zahnärzt/innen hinter, neben und vor dem/r Patienten/in führen zur asymmetrischen Neigung und Verdrehung des Kopfes sowie des Rumpfes. Die Folge können Fehlhaltungen sein, die Muskel-Skelett-Erkrankungen verursachen. Das erklärt wahrscheinlich die hohe Prävalenz bei Zahnärzt/innen und zahnmedizinischen Fachangestellten. Daher werden in dieser Übersicht mögliche Ursachen und Konsequenzen der Prävalenz sowie ergonomische Maßnahmen für diese Berufsgruppen aufgeführt. Zudem erläutern wir ergonomische Empfehlungen für die Sitzhaltung von Zahnärzt/innen auf Basis der vorhandenen Literatur.
Trotz der Spenderauswahl, des serologischen Screenings der Blutspenden auf antiHIV-1, anti-HIV-2, anti-HCV und HBsAg, Inaktivierungs- und Eliminierungsverfahren bleibt ein Restrisiko für hämatogen übertragbare Viren bei Plasmapools und den daraus hergestellten Präparaten. Als zusätzliche Screeningmethode zur Erhöhung der Virussicherheit wird inzwischen die Testung der Einzelspende bzw. von Minipools auf HCV-RNA verlangt. In der vorliegenden Arbeit wurden 142 HBsAg, anti-HCV- und anti-HIV-11-2 negative Plasmapools, sowie Plasmapräparate (Immunglobulinpräparat und F IX Präparat), welche zum Teil aus für HGV-RNA PCR-positiven Ausgangspools hergestellt worden waren, mittels PCR auf das Vorhandensein von HGV-Nukleinsäure untersucht. Alle untersuchten Pools bzw. Plasmapräparate stammten von Spenden zwischen 1994 und 1996, also vor der Einführung der genannten Pflichttestung auf HCV-RNA. HGV-RNA wurde in 117 der 142 Plasmapools (82,4%) amplifiziert. Allerdings war HGV-RNA nur in einer (6,3%) von 16 IgG-Chargen aus für HGV-Nukleinsäure positiven Kryoüberständen nachweisbar. In 2 (6,5%) von 31 unselektierten Immunglobulinpräparaten war eine HGV- Kontamination vorhanden. Eine routinemäßige Anwendung der PCR ist zur Zeit aus technischen sowie Kosten-Nutzen-Überlegungen für HGV nicht zu empfehlen, solange die klinische Relevanz nicht gesichert ist. Die Ergebnisse unterstreichen die Wichtigkeit der im Herstellungsprozeß integrierten Viruseliminierungsverfahren, denn bei der vorliegenden Studie konnte nur in einem geringen Anteil der Endproduktchargen HGV-Nukleinsäure nachgewiesen werden.
Cystic fibrosis (CF) lung disease is aggravated by recurrent and ultimately chronic bacterial infections. One of the key pathogens in adult CF lung disease is P. aeruginosa (PA). In addition to bacteria, respiratory viral infections are suggested to trigger pulmonary exacerbations in CF. To date, little is known on how chronic infections with PA influence susceptibility and response to viral infection. We investigated the interactions between PA, human rhinovirus (HRV) and the airway epithelium in a model of chronic PA infection using differentiated primary bronchial epithelial cells (pBECs) and clinical PA isolates obtained from the respiratory sample of a CF patient. Cells were repeatedly infected with either a mucoid or a non-mucoid PA isolate for 16 days to simulate chronic infection, and subsequently co-infected with HRV. Key cytokines and viral RNA were quantified by cytometric bead array, ELISA and qPCR. Proteolytic degradation of IL-6 was analyzed by Western Blots. Barrier function was assessed by permeability tests and transepithelial electric resistance measurements. Virus infection stimulated the production of inflammatory and antiviral mediators, including interleukin (IL)-6, CXCL-8, tumor necrosis factor (TNF)-α, and type I/III interferons. Co-infection with a non-mucoid PA isolate increased IL-1β protein concentrations (28.88 pg/ml vs. 6.10 pg/ml), but in contrast drastically diminished levels of IL-6 protein (53.17 pg/ml vs. 2301.33 pg/ml) compared to virus infection alone. Conditioned medium obtained from co-infections with a non-mucoid PA isolate and HRV was able to rapidly degrade recombinant IL-6 in a serine protease-dependent manner, whereas medium from individual infections or co-infections with a mucoid isolate had no such effect. After co-infection with HRV and the non-mucoid PA isolate, we detected lower mRNA levels of Forkhead box J1 (FOXJ1) and Cilia Apical Structure Protein (SNTN), markers of epithelial cell differentiation to ciliated cells. Moreover, epithelial permeability was increased and barrier function compromised compared to single infections. These data show that PA infection can influence the response of bronchial epithelial cells to viral infection. Altered innate immune responses and compromised epithelial barrier function may contribute to an aggravated course of viral infection in PA-infected airways.
Chronische pulmonale Infektionen mit Pseudomonas aeruginosa (PA) betreffen die überwiegende Mehrheit der erwachsenen Mukoviszidose (Cystische Fibrose, CF) Patienten.
Diese Infektionen führen gesichert zu einer Abnahme der Lungenfunktion und Zunahme der Mortalität der Patienten. Atemwegsviren stehen im Verdacht pulmonale Exazerbationen bei CF-Patienten auszulösen. Unklar ist jedoch, welchen Einfluss eine chronische Infektion mit PA auf die Anfälligkeit und Reaktion des Atemwegsepithels auf virale Infektionen hat.
Das Ziel dieser Arbeit war es daher, die Interaktionen zwischen PA, humanen Rhinoviren (HRV) und primären bronchialen Epithelzellen zu untersuchen. Hierfür wurden Zellen von jeweils drei Patienten mit CF und mit Lungenemphysem aus Lungenexplantaten isoliert und in einem speziellen Air- Liquid-Interface Zellkulturmodell zu einem mukoziliär differenzierten mehrreihigen Flimmerepithel kultiviert. Chronische Infektionen wurden mit klinischen PA Isolaten für einen Gesamtzeitraum von 16 Tagen durchgeführt. Anschließend wurden die Zellen mit HRV infiziert. Schlüsselzytokine, Interferone und virale RNA wurden mittels Cytometric bead array, ELISA und qPCR bestimmt.
Rein virale Infektionen mit HRV führten zu einem Anstieg von IL-1, -6, -8, TNF- α, IP10 und IFN-b, IFN-l1 sowie ISGs und in ähnlichem Ausmaß konnte dies auch bei Coinfektionen mit einem mukoiden PA-Isolat beobachtet werden. Coinfektionen mit einem nicht-mukoiden PA-Isolat führten im Vergleich zu rein viralen Infektionen zu vermehrter Expression von IL-1β und IL-6 mRNA. Während es unter diesen Bedingungen auch auf Proteinebene zu einem Anstieg der IL-1β Konzentration kam, lag die Konzentration von freiem IL-6 Protein in nahezu allen Proben unter der Nachweisgrenze. Zellkulturmedium aus Coinfektionen mit diesem nicht-mukoiden PA-Isolat führten zudem zu einem Abbau oder einer Bindung von extern zugegebenen rekombinantem IL-6.
IL-8, IP-10, TNF-α Protein und mRNA von IFN-β, -λ1 und ISGs, sowie die Viruslast waren vergleichbar zwischen rein viralen Infektionen und bakteriell- viralen Coinfektionen. Ebenfalls keine Unterschiede wurden zwischen Zellen von Emphysem und CF-Spendern gefunden. Insgesamt zeigen diese Daten, dass eine PA-Infektion die Antwort differenzierter bronchialer Epithelzellen auf eine Virusinfektion verändern kann. Die hierdurch veränderte Immunantwort und möglicherweise eingeschränkten epithelialen Reparaturmechanismen könnten eine Ursache aggravierter viraler Infektionen in P. aeruginosa-infizierten Atemwegen darstellen.

The developmental and epileptic encephalopathies encompass a group of rare syndromes characterised by severe drug-resistant epilepsy with onset in childhood and significant neurodevelopmental comorbidities. The latter include intellectual disability, developmental delay, behavioural problems including attention-deficit hyperactivity disorder and autism spectrum disorder, psychiatric problems including anxiety and depression, speech impairment and sleep problems. Classical examples of developmental and epileptic encephalopathies include Dravet syndrome, Lennox–Gastaut syndrome and tuberous sclerosis complex. The mainstay of treatment is with multiple anti-seizure medications (ASMs); however, the ASMs themselves can be associated with psychobehavioural adverse events, and effects (negative or positive) on cognition and sleep. We have performed a targeted literature review of ASMs commonly used in the treatment of developmental and epileptic encephalopathies to discuss the latest evidence on their effects on behaviour, mood, cognition, sedation and sleep. The ASMs include valproate (VPA), clobazam, topiramate (TPM), cannabidiol (CBD), fenfluramine (FFA), levetiracetam (LEV), brivaracetam (BRV), zonisamide (ZNS), perampanel (PER), ethosuximide, stiripentol, lamotrigine (LTG), rufinamide, vigabatrin, lacosamide (LCM) and everolimus. Bromide, felbamate and other sodium channel ASMs are discussed briefly. Overall, the current evidence suggest that LEV, PER and to a lesser extent BRV are associated with psychobehavioural adverse events including aggressiveness and irritability; TPM and to a lesser extent ZNS are associated with language impairment and cognitive dulling/memory problems. Patients with a history of behavioural and psychiatric comorbidities may be more at risk of developing psychobehavioural adverse events. Topiramate and ZNS may be associated with negative effects in some aspects of cognition; CBD, FFA, LEV, BRV and LTG may have some positive effects, while the remaining ASMs do not appear to have a detrimental effect. All the ASMs are associated with sedation to a certain extent, which is pronounced during uptitration. Cannabidiol, PER and pregabalin may be associated with improvements in sleep, LTG is associated with insomnia, while VPA, TPM, LEV, ZNS and LCM do not appear to have detrimental effects. There was variability in the extent of evidence for each ASM: for many first-generation and some second-generation ASMs, there is scant documented evidence; however, their extensive use suggests favourable tolerability and safety (e.g. VPA); second-generation and some third-generation ASMs tend to have the most robust evidence documented over several years of use (TPM, LEV, PER, ZNS, BRV), while evidence is still being generated for newer ASMs such as CBD and FFA. Finally, we discuss how a variety of factors can affect mood, behaviour and cognition, and untangling the associations between the effects of the underlying syndrome and those of the ASMs can be challenging. In particular, there is enormous heterogeneity in cognitive, behavioural and developmental impairments that is complex and can change naturally over time; there is a lack of standardised instruments for evaluating these outcomes in developmental and epileptic encephalopathies, with a reliance on subjective evaluations by proxy (caregivers); and treatment regimes are complex involving multiple ASMs as well as other drugs.
Background Stigma is one of the most significant constraints on people living with depression. There is a lack of validated scales in Portugal to measure depression stigma; therefore, validation of the Depression Stigma Scale (DSS) is an essential step to the depression stigma research in Portugal.
Methods We developed the adaptation process with the ITC Guidelines for Translation and Adapting Tests taken into consideration. We collected the sample as part of the OSPI program – Optimizing suicide prevention programs and their implementation in Europe, specifically within the application in Portugal, and included 1693 participants. Floor-ceiling effects and response ranges were analyzed, and we calculated Cronbach alphas, conducted a Principal Component Analysis and Confirmatory Analysis. Validity evidence was tested with two well-documented hypotheses, using data on gender and depression symptoms.
Results The sample was well comparable with the general Portuguese population, indicating its representativeness. We identified a three-factor structure in each subscale (personal and perceived stigma): weak-not-sick, discrimination, and dangerous/unpredictable. The Cronbach’s alphas were satisfactory, and validity was confirmed.
Conclusions This study established the validity and demonstrated good psychometric properties of the DSS in the Portuguese population. The validation of the DSS can be beneficial in exploring stigma predictors and evaluating the effectiveness of stigma reduction interventions.

Hintergrund: Die Einnahme vieler Antidepressiva und Antipsychotika in der frühen Schwangerschaft kann nach aktuellem Forschungsstand als sicher bezüglich des Risikos für Fehlbildungen erachtet werden. Daten hinsichtlich der Sicherheit einer Psychopharmakotherapie während der Stillperiode sind hingegen rar. Die meisten Betroffenen würden einerseits gerne ihre Kinder stillen, andererseits wollen sie aber auch sicher sein, dass sie ihrem Nachwuchs dadurch nicht schaden.
Ziele: Unser Ziel war es herauszufinden ob eine Korrelation zwischen der Tagesdosis der Psychopharmaka und der Medikamentenkonzentration im Serum sowie in der Muttermilch (MM) besteht. Weiterhin untersuchten wir, ob es eine allgemeingültige Regeln gibt, wie sich Muttermilchmedikamentenkonzentrationen im Tagesverlauf verhalten. Außerdem wollten wir herausfinden ob die Medikamentenexposition in der Schwangerschaft und/oder Stillperiode einen Einfluss auf die Entwicklung des Kindes in den ersten zwölf Lebensmonaten hat.
Methoden: Wir untersuchten therapeutische Medikamentenspiegel parallel in Serum und MM zu verschiedenen Zeitpunkten um herauszufinden wann die höchsten/niedrigsten Konzentrationen ermittelt werden konnten. Neben dem Talspiegel, nach 12 h oder 24 h (T1), sammelten wir Proben 4 h (T5) und 8 h (T6) nach der Medikamenteneinnahme sowie direkt nach der Medikamenteneinnahme (T2), 1 h nach T2 (T3) und direkt beim nächsten Stillen (T4). Die Messungen wurden im TDM Labor im Zentrum für psychsiche Gesundheit, Universitätsklinikum Würzburg mittels isokratischer reversed-phase high performance liquid Chromatographie durchgeführt. Wir rekrutierten 25 Patientinnen, die in der ambulanten Spezialsprechstunde für psychiatrische Erkrankungen in Schwangerschaft und Stillzeit behandelt wurden. 68% der Patientinnen nahmen in der Schwangerschaft schon ein Psychopharmakon ein. Die Patientinnen litten an unipolaren Depressionen (n = 15), bipolaren Störungen (n = 2), schizoaffektiven Störungen (n = 2) sowie Angst- (n = 4) und Zwangsstörungen (n = 2). Das Spektrum von Antidepressiva und Antipsychotika erstreckte sich von Amitriptylin/Nortriptylin, Clomipramin/N-Desmethyl-Clomipramin, Mirtazapin, Escitalopram, Citalopram, Sertralin, Venlafaxin/ O-Desmethyl-Venlafaxin bis hin zu Lamotrigin und Quetiapin. Weiterhin erfassten wir ob es postnatale Auffälligkeiten sowie im Verlauf Entwicklungsauffälligkeiten bei den exponierten Kinder gab (Daten aus den routinemäßigen U-Untersuchungen der ersten 12 Lebensmonate). Um mehr Daten zur Kindesentwicklung zu sammeln, interviewten wir in einem retrospektiven Unterprojekt 14 weitere Patientinnen, die in der Vergangenheit von der Thematik betroffen waren.
Ergebnisse: Die parallel bestimmten Talspiegel in Serum und MM korrelierten nicht signifikant bei der Analyse aller Medikamente zusammen oder bei der Analyse der einzelnen Medikamente (Spearman Rho Korrelation, p > 0,05). Die Talspiegel im Serum korrelierten weder signifikant mit den Muttermilchkonzentration nach 4 h (T5), noch mit denen nach 8 h (T6) (Spearman Rho Korrelation, p = 0,46; p = 0,08). Die Tagesdosis korrelierte signifikant mit dem Talspiegel im Serum (p = 0,001), jedoch nicht mit den gleichzeitig bestimmten Muttermilchkonzentrationen (p = 0,88). Die Konzentrationen von Venlafaxin in der MM fielen relativ hoch aus. Es zeigte sich eine große interindividuelle Variation der Konzentrationen der unterschiedlichen Medikamente zu verschiedenen Zeitpunkten. Clomipramin und Quetiapin konnten gar nicht oder nur in sehr niedrigen Konzentrationen in der MM gemessen werden (n = 2, n = 4) wie auch Sertralin (ohne Ausreißer-Werte) und Escitalopram. Auch konnte Quetiapin nicht im Serum eines gestillten Kindes nachgewiesen werden, bei dem wir Blutproben gewinnen konnten (n = 1). Keines der gestillten Kinder zeigte negative Auswirkungen aufgrund der Medikation. Die höchste „concentration-bydose-ratio“ (C/D) in der MM wurden für Venlafaxin (n = 4, C/D MM = 5,11) und Lamotrigin (n = 1, C/D MM = 10,0) errechnet, während Clomipramin (n = 2, C/D MM = 0,0), Quetiapin (n = 4, C/D MM = 0,02) und Sertralin (n = 3, C/D MM = 0,03) die niedrigsten Quotienten zeigten. Wir kalkulierten die höchste „milk-to-plasma-ratio“ (M/P) für Mirtazapin (n = 3, M/P = 3,92) und Venlafaxin (n = 2, M/P = 2,4) sowie die niedrigsten für Clomipramin (n = 2, M/P = 0), Quetiapin (n = 3, M/P = 0,03) und Sertralin (n = 3, M/P = 0,08), was für ein niedriges Eindringungsvermögen vom Serum in die MM spricht.
The effects of exercise interventions on unspecific chronic low back pain (CLBP) have been investigated in many studies, but the results are inconclusive regarding exercise types, efficiency, and sustainability. This may be because the influence of psychosocial factors on exercise induced adaptation regarding CLBP is neglected. Therefore, this study assessed psychosocial characteristics, which moderate and mediate the effects of sensorimotor exercise on LBP. A single-blind 3-arm multicenter randomized controlled trial was conducted for 12-weeks. Three exercise groups, sensorimotor exercise (SMT), sensorimotor and behavioral training (SMT-BT), and regular routines (CG) were randomly assigned to 662 volunteers. Primary outcomes (pain intensity and disability) and psychosocial characteristics were assessed at baseline (M1) and follow-up (3/6/12/24 weeks, M2-M5). Multiple regression models were used to analyze whether psychosocial characteristics are moderators of the relationship between exercise and pain, meaning that psychosocial factors and exercise interact. Causal mediation analysis were conducted to analyze, whether psychosocial characteristics mediate the exercise effect on pain. A total of 453 participants with intermittent pain (mean age = 39.5 ± 12.2 years, f = 62%) completed the training. It was shown, that depressive symptomatology (at M4, M5), vital exhaustion (at M4), and perceived social support (at M5) are significant moderators of the relationship between exercise and the reduction of pain intensity. Further depressive mood (at M4), social-satisfaction (at M4), and anxiety (at M5 SMT) significantly moderate the exercise effect on pain disability. The amount of moderation was of clinical relevance. In contrast, there were no psychosocial variables which mediated exercise effects on pain. In conclusion it was shown, that psychosocial variables can be moderators in the relationship between sensorimotor exercise induced adaptation on CLBP which may explain conflicting results in the past regarding the merit of exercise interventions in CLBP. Results suggest further an early identification of psychosocial risk factors by diagnostic tools, which may essential support the planning of personalized exercise therapy.
Background: Despite advances in treatment of patients with non-small cell lung cancer, carriers of certain genetic alterations are prone to failure. One such factor frequently mutated, is the tumor suppressor PTEN. These tumors are supposed to be more resistant to radiation, chemo- and immunotherapy.
Results: We demonstrate that loss of PTEN led to altered expression of transcriptional programs which directly regulate therapy resistance, resulting in establishment of radiation resistance. While PTEN-deficient tumor cells were not dependent on DNA-PK for IR resistance nor activated ATR during IR, they showed a significant dependence for the DNA damage kinase ATM. Pharmacologic inhibition of ATM, via KU-60019 and AZD1390 at non-toxic doses, restored and even synergized with IR in PTEN-deficient human and murine NSCLC cells as well in a multicellular organotypic ex vivo tumor model.
Conclusion: PTEN tumors are addicted to ATM to detect and repair radiation induced DNA damage. This creates an exploitable bottleneck. At least in cellulo and ex vivo we show that low concentration of ATM inhibitor is able to synergise with IR to treat PTEN-deficient tumors in genetically well-defined IR resistant lung cancer models.
Background & Aims: In ACLF patients, an adequate risk stratification is essential, especially for liver transplant allocation, since ACLF is associated with high short-term mortality. The CLIF-C ACLF score is the best prognostic model to predict outcome in ACLF patients. While lung failure is generally regarded as signum malum in ICU care, this study aims to evaluate and quantify the role of pulmonary impairment on outcome in ACLF patients.
Methods: In this retrospective study, 498 patients with liver cirrhosis and admission to IMC/ICU were included. ACLF was defined according to EASL-CLIF criteria. Pulmonary impairment was classified into three groups: unimpaired ventilation, need for mechanical ventilation and defined pulmonary failure. These factors were analysed in different cohorts, including a propensity score-matched ACLF cohort.
Results: Mechanical ventilation and pulmonary failure were identified as independent risk factors for increased short-term mortality. In matched ACLF patients, the presence of pulmonary failure showed the highest 28-day mortality (83.7%), whereas mortality rates in ACLF with mechanical ventilation (67.3%) and ACLF without pulmonary impairment (38.8%) were considerably lower (p < .001). Especially in patients with pulmonary impairment, the CLIF-C ACLF score showed poor predictive accuracy. Adjusting the CLIF-C ACLF score for the grade of pulmonary impairment improved the prediction significantly.
Conclusions: This study highlights that not only pulmonary failure but also mechanical ventilation is associated with worse prognosis in ACLF patients. The grade of pulmonary impairment should be considered in the risk assessment in ACLF patients. The new score may be useful in the selection of patients for liver transplantation.
Brain metastases are a common finding upon initial diagnosis of otherwise locally limited non-small cell lung cancer. We present a retrospective case series describing three cases of patients with symptomatic, synchronous brain metastases and resectable lung tumors. The patients received local ablative treatment of the brain metastases followed by neoadjuvant immunochemotherapy with pemetrexed, cisplatin, and pembrolizumab. Afterwards, resection of the pulmonary lesion with curative intent was performed. One patient showed progressive disease 12 months after initial diagnosis, and passed away 31 months after initial diagnosis. Two of the patients are still alive and maintain a good quality of life with a progression-free survival and overall survival of 28 and 35 months, respectively, illustrating the potential of novel combinatorial treatment approaches.
Manufacturing processes of custom implant abutments may contaminate their surfaces with micro wear deposits and generic pollutants. Such particulate debris, if not removed, might be detrimental and provoke inflammatory reactions in peri-implant tissues. Although regulatory guidelines for adequate cleaning, disinfection, or sterilization exist, there does not appear to be a consistent application and data on the amount and extent of such contaminants is lacking. The aim of the present in vitro study was to evaluate the quality and quantity of processing-related surface contamination of computer-aided design/computer-aided manufacturing (CAD/CAM) abutments in the state of delivery and after ultrasonic cleaning. A total of 28 CAD/CAM monotype and hybrid abutments were cleaned and disinfected applying a three-stage ultrasonic protocol (Finevo protocol). Before and after cleaning, the chemical composition and the contamination of the abutments were assessed using scanning electron microscopy (SEM), dispersive X-ray spectroscopy(EDX),andcomputer-aidedplanimetricmeasurement(CAPM).Inthedeliverycondition, monotype abutments showed a significantly higher amount of debris compared to hybrid abutments (4.86±6.10% vs. 0.03 ± 0.03%, p < 0.001). The polishing process applied in the laboratory after bonding the hybrid abutment components reduces the surface roughness and thus contributes substantially to their purity. The extent of contamination caused by computer-aided manufacturing of custom abutments can be substantially minimized using a three-stage ultrasonic protocol.
Purpose: The treatment with a cochlear implant (CI) is the gold standard in therapy of patients with profound hearing loss or deafness. Successful hearing rehabilitation with a CI is a complex, multi-stage process. In medicine, “Clinical Practice Guidelines” (CPG) are widely accepted for the standardization of such processes. These are supplemented by medical registries in which data regarding the treatment can be collected and evaluated. The aim of this paper is to identify currently existing CI-related CPGs and registries in Europe.
Methods: Between 01/2021 and 06/2021, 42 countries on the European continent, including the United Kingdom, Russia and Turkey, were screened using an internet search (search engine: Google) and a key word search in the Pubmed database. Search terms were the respective country name combined with the following terms: “Cochlear Implant”, “CI”, “Cochlear implant clinical practice guideline”, “CI Guideline”, “Cochlear Implant Registry”, “CI Registry”, “Ear nose throat society”. The internet search was conducted in English as well as in the corresponding national language. The objective was to identify a CI-related CPG or registry.
Results: A CPG was found in 16 of 42 (38%) countries. In terms of population, this accounts for 645 million out of 838 million people (77%). A registry existed in 4 of the 42 (10%) countries assessed. This corresponds to 102 million out of 838 million (12%) people. In total, 4 out of 42 countries (10%) had both a CPG and a registry.
Conclusion: Our work shows numerous efforts in Europe to standardize CI care at the national level. While most people in Europe already live in countries with a CPG, this is not the case for CI registries. European-wide consensus on CPGs or registries does not yet exist. The present study thus provides a first assessment of the distribution of CI-related CPGs and registries.
Objective. We investigated the health-related quality of life (HRQoL) of patients with gastrointestinal stromal tumours (GIST). Methods: In the multicentre PROSa study, the HRQoL of adult GIST patients was assessed between 2017 and 2019 using the European Organisation for Research and Treatment of Cancer HRQoL questionnaire (EORTC QLQ-C30). We performed group comparisons and multivariate linear regressions. Results: Among 130 patients from 13 centres, the mean global HRQoL was 63.3 out of 100 points. Higher sores indicate better HRQoL. The highest restrictions were in emotional, social, role functioning, insomnia, fatigue, and pain. In multivariate linear regression, we found no significant differences between patients receiving tyrosine kinase inhibitor (TKI) treatment and those without TKI treatment as well as between patients treated with curative or with palliative intent. Patients who received multiple lines of TKI treatment had the most restrictions, notably in physical (unstandardized regression coefficient [B] = −15.7), role (B = −25.7), social (B = −18.4), and cognitive functioning (B = −19.7); fatigue (B = 15.93); general health (B = −14.23); and EORTC-sum score (B = −13.82) compared to all other patients. Conclusion: The highest HRQoL restrictions were in GIST patients receiving multiple lines of TKI therapy. Underlying causes need further investigation.
Aims: The study aimed to evaluate the prevalence of mental distress in patients with newly diagnosed bladder cancer, the cancer-information search behavior, and the influence of information seeking on distress. Methods: One hundred and one bladder cancer patients answered 2 established questionnaires (“Hospital Anxiety and Depression Scale” [HADS] and the “Fragebogen zur Belastung von Krebskranken” [FBK-R23]) for evaluation of mental distress and a self-developed questionnaire with questions concerning information seeking and socioeconomic facts. Results: Regarding risk group stratification, 57.4% were classified as high-risk and 42.6% as low-risk tumor-bearing patients. Analysis of mental distress showed that 23.2% had a score above the HADS-A cutoff, 25.3% above the HADS-D cutoff, and 21.4% showed a pathologic FBK-R23 score. Overall, 75% felt well informed about their illness. Risk group stratification did not correlate with HADS-A, HADS-D, or FBK-R23 score. Furthermore, active search for information or the use of the Internet did not correlate with the HADS-A, HADS-D, or FBK-R23 score. However, the quality of the urologist’s information and the feeling of being informed correlated with the grade of mental distress. Conclusion: Besides the treatment of bladder cancer, informing the patient about the disease in a psychologically wholesome manner and working together with psycho-oncologically trained psychologists are essential tasks for the treating urologist.
Die Neuregelung der Qualitätssicherung für Hormone und Pharmaka gemäß den Qualitätssicherungsrichtlinien der Bundesärztekammer (Rili-BÄK) unterliegt einer intensiven Diskussion, besonders in Hinsicht auf die Praktikabilität Für die ärztlichen Laboratorien und die Diagnostikahersteller ergeben sich erhebliche Veränderungen, die aufgezeigt werden.
Die HIV-Infizierung von Zellkulturen in vitro ist essentiell für das Verstehen der Kinetik der Virusreplikation, für die Aufdeckung von Resistenzentwicklungen gegenüber antiretroviraler Medikamente und für die Entwicklung neuer antiretroviraler Therapiestrategien. Voraussetzung hierfür ist ein geeignetes Monitoring der HIV-Infektion von in vitro infizierten Zellen. Die vorliegende Arbeit beschäftigt sich mit dem Monitoring der HIV-Replikation von in vitro infizierten Zellen mittels der Real-Time TaqMan™ PCR. Die Ergebnisse der Real-Time TaqMan™ PCR wurden mit denen eines p24 ELISAs verglichen. Der p24 ELISA diente als etablierte Standardmethode zum Monitoring einer in vitro HIV-Infektion. HUT 78-Zellen wurden in vitro mit vier unterschiedlichen HIV-1 IIIb Infektionsdosen (MOI 0,05; MOI 0,01; MOI 0,002; MOI 0,0005) infiziert. Mittels der Real-Time TaqMan™ PCR wurde die HIV-1 gag cDNA quantifiziert. Mittels ELISA erfolgte die Quantifizierung des HIV-p24. Zusätzlich dazu wurde die Anzahl an proviralen HIV-1 Transkripten in den Zellkulturen mittels der TaqMan™ PCR quantifiziert. Die Quantifizierung der HIV-1 gag cDNA und des p24 ergaben nahezu identische Kurvenverläufe der Infektionskinetiken. Beide Nachweismethoden zeigten vergleichbare Daten bezüglich des exponentiellen Ansteigens und der sich daran anschließenden Plateauphase der HIV-Replikation. Die Sensitivität beider Nachweismethoden war ebenfalls vergleichbar. Ein großer Unterschied lag in den Messbereichen beider Methoden. Bei der Real-Time TaqMan™ PCR konnte eine Linearität über 7 log-Stufen
demonstriert werden. Dies hatte den Vorteil, dass die Zellkulturproben vor der Quantifizierung der HIV-1 gag cDNA nicht verdünnt werden mussten. Im Gegensatz dazu war der Messbereich des HIV-p24 ELISAs sehr eng und erforderte in den meisten Fällen eine Verdünnung der Messproben. Bezüglich des Arbeitsaufwandes und der aufkommenden Kosten ergaben sich für die Quantifizierung der HIV-1 gag cDNA mittels der Real-Time TaqMan ™ und für die Quantifizierung des p24 mittels ELISA nahezu identische Werte. Der Verlauf der Werte an proviralen HIV-1 Transkripten ähnelt dem der HIV-1 gag cDNA Kinetik. Mittels der Quantifizierung der proviralen HIV-1 Kopien kann jedoch keine Aussage über die HIV-Replikation getroffen werden. Abschließend ist zu sagen, dass die Real-Time TaqMan™ PCR eine zuverlässige und sensitive Methode ist, eine HIV-1 Replikation von in vitro infizierten Zellen zu quantifizieren und den Replikationsverlauf zu beschreiben. Die Real-Time TaqMan™ PCR stellt eine alternative Methode zum HIV-p24 ELISA dar, um eine in vitro HIV-Replikation zu dokumentieren.
Event-related potentials (ERPs) are widely used in basic neuroscience and in clinical diagnostic procedures. In contrast, neurophysiological insights from ERPs have been limited, as several different mechanisms lead to ERPs. Apart from stereotypically repeated responses (additive evoked responses), these mechanisms are asymmetric amplitude modulations and phase-resetting of ongoing oscillatory activity. Therefore, a method is needed that differentiates between these mechanisms and moreover quantifies the stability of a response. We propose a constrained subspace independent component analysis that exploits the multivariate information present in the all-to-all relationship of recordings over trials. Our method identifies additive evoked activity and quantifies its stability over trials. We evaluate identification performance for biologically plausible simulation data and two neurophysiological test cases: Local field potential (LFP) recordings from a visuo-motor-integration task in the awake behaving macaque and magnetoencephalography (MEG) recordings of steady-state visual evoked fields (SSVEFs). In the LFPs we find additive evoked response contributions in visual areas V2/4 but not in primary motor cortex A4, although visually triggered ERPs were also observed in area A4. MEG-SSVEFs were mainly created by additive evoked response contributions. Our results demonstrate that the identification of additive evoked response contributions is possible both in invasive and in non-invasive electrophysiological recordings.
Purpose: To identify transjugular intrahepatic portosystemic shunt (TIPS) thrombosis in abdominal CT scans applying quantitative image analysis.
Materials and methods: We retrospectively screened 184 patients to include 20 patients (male, 8; female, 12; mean age, 60.7 ± 8.87 years) with (case, n = 10) and without (control, n = 10) in-TIPS thrombosis who underwent clinically indicated contrast-enhanced and unenhanced abdominal CT followed by conventional TIPS-angiography between 08/2014 and 06/2020. First, images were scored visually. Second, region of interest (ROI) based quantitative measurements of CT attenuation were performed in the inferior vena cava (IVC), portal vein and in four TIPS locations. Minimum, maximum and average Hounsfield unit (HU) values were used as absolute and relative quantitative features. We analyzed the features with univariate testing.
Results: Subjective scores identified in-TIPS thrombosis in contrast-enhanced scans with an accuracy of 0.667 – 0.833. Patients with in-TIPS thrombosis had significantly lower average (p < 0.001), minimum (p < 0.001) and maximum HU (p = 0.043) in contrast-enhanced images. The in-TIPS / IVC ratio in contrast-enhanced images was significantly lower in patients with in-TIPS thrombosis (p < 0.001). No significant differences were found for unenhanced images. Analyzing the visually most suspicious ROI with consecutive calculation of its ratio to the IVC, all patients with a ratio < 1 suffered from in-TIPS thrombosis (p < 0.001, sensitivity and specificity = 100%).
Conclusion: Quantitative analysis of abdominal CT scans facilitates the stratification of in-TIPS thrombosis. In contrast-enhanced scans, an in-TIPS / IVC ratio < 1 could non-invasively stratify all patients with in-TIPS thrombosis.
Autism spectrum disorders (ASD) are highly heritable and are characterized by deficits in social communication and restricted and repetitive behaviors. Twin studies on phenotypic subdomains suggest a differing underlying genetic etiology. Studying genetic variation explaining phenotypic variance will help to identify specific underlying pathomechanisms. We investigated the effect of common variation on ASD subdomains in two cohorts including >2500 individuals. Based on the Autism Diagnostic Interview-Revised (ADI-R), we identified and confirmed six subdomains with a SNP-based genetic heritability h2SNP = 0.2–0.4. The subdomains nonverbal communication (NVC), social interaction (SI), and peer interaction (PI) shared genetic risk factors, while the subdomains of repetitive sensory-motor behavior (RB) and restricted interests (RI) were genetically independent of each other. The polygenic risk score (PRS) for ASD as categorical diagnosis explained 2.3–3.3% of the variance of SI, joint attention (JA), and PI, 4.5% for RI, 1.2% of RB, but only 0.7% of NVC. We report eight genome-wide significant hits—partially replicating previous findings—and 292 known and novel candidate genes. The underlying biological mechanisms were related to neuronal transmission and development. At the SNP and gene level, all subdomains showed overlap, with the exception of RB. However, no overlap was observed at the functional level. In summary, the ADI-R algorithm-derived subdomains related to social communication show a shared genetic etiology in contrast to restricted and repetitive behaviors. The ASD-specific PRS overlapped only partially, suggesting an additional role of specific common variation in shaping the phenotypic expression of ASD subdomains.
UV-mikrospektrophotometrische Messungen der Nucleinsäuren- und Eiweißkörper-Konzentration sowie der Kern- und Nukleolengröße nach Virusinfektion und unspezifischer Reizung der Chorion-Allantoismembran zeigen, daß es in beiden Fällen zu einer gleich starken Stimulierung des nucleinsäuren- und eiweißkörperbildenden Systems der Zelle kommt. Bei der Infektion mit Vaccinevirus auf das Ektoderm setzt die Reaktion der Membranzellen in der Eklipse ein, nach Infektion mit Newcastle-Disease-Virus fällt der Titeranstieg mit der Zellreaktion zeitlich zusammen.
Zur quantitativen Plasmaproteinbestimmung sind die Immunnephelometrie und die Immunturbidimetrie häufig eingesetzte Bestimmungstechniken. Aufgrund der kürzeren Analysenzeit bei vergleichbarer oder besserer Präzision und Empfindlichkeit haben diese Techniken die radiale Immundiffusion in vielen Laboratorien ersetzt.
Die mechanisierte Plasmaproteinbestimmung erfolgt als Antigen-Antikörper-Reaktion entweder mit Proteinanalyzern oder mit klinisch-chemischen Analysensystemen, an denen normalerweise Enzyme und Substrate bestimmt werden.
Zwischen den verschiedenen Plasmaproteinen im Serum besteht ein bis zu 10.OOOfacher Konzentrationsunterschied und die biologische Varianz des einzelnen Plasmaproteins ist breit Damit die Plasmaproteine mit hoher analytischer Sensitivität über einen weiten Konzentrationsbereich, mit guter Präzision und der erforderlichen Richtigkeit mechanisiert bestimmt werden können, ist eine gute Adaption des Immunreagenzes auf das mechanisierte Analysensystem erforderlich. Diese Übersicht soll dazu Grundlagen vermitteln.
The aim of this study is to provide a systematic assessment of the influence of the position on the arterial input function (AIF) for perfusion quantification. In 39 patients with a wide range of left ventricular function the AIF was determined using a diluted contrast bolus of a cardiac magnetic resonance imaging in three left ventricular levels (basal, mid, apex) as well as aortic sinus (AoS). Time to peak signal intensities, baseline corrected peak signal intensity and upslopes were determined and compared to those obtained in the AoS. The error induced by sampling the AIF in a position different to the AoS was determined by Fermi deconvolution. The time to peak signal intensity was strongly correlated (r2 > 0.9) for all positions with a systematic earlier arrival in the basal (− 2153 ± 818 ms), the mid (− 1429 ± 928 ms) and the apical slice (− 450 ± 739 ms) relative to the AoS (all p < 0.001). Peak signal intensity as well as upslopes were strongly correlated (r2 > 0.9 for both) for all positions with a systematic overestimation in all positions relative to the AoS (all p < 0.001 and all p < 0.05). Differences between the positions were more pronounced for patients with reduced ejection fraction. The error of averaged MBF quantification was 8%, 13% and 27% for the base, mid and apex. The location of the AIF significantly influences core parameters for perfusion quantification with a systematic and ejection fraction dependent error. Full quantification should be based on obtaining the AIF as close as possible to the myocardium to minimize these errors.
Astrozyten erfüllen verschiedene Funktionen im Zentralnervensystem, welche sich in die Bereiche Entwicklung, Durchblutung, Metabolismus, Strukturerhalt und Gliotransmission unterteilen lassen. Astrozyten sind an der synaptischen Informationsverarbeitung beteiligt und wirken an zahlreichen höheren Hirnfunktionen mit. Durch Regulation der synaptischen Transmission und Plastizität sind Astrozyten am Lernverhalten und Erinnerungsvermögen, sowie an der Verhaltensmodulation und Verarbeitung emotionaler Reize involviert. Im Zuge dieser zahlreichen Funktionen können Astrozyten auf externe Stimuli mit der gezielten Freisetzung von Gliotransmittern reagieren.
In kultivierten Astrozyten konnte Keil143 das TGN, bestehend aus Zisternen und Vesikeln, darstellen und mit anti-Rab6 identifizieren. Rab6 mit seinen Subtypen A und B gehört der Superfamilie der monomeren Ras-GTPasen an, die den intrazellulären Membran- und Vesikelverkehr regulieren. Rab6 spielt in HeLa-Zellen beim Transport vesikulärer Organellen vom TGN zur Zellmembran eine wichtige Rolle. Assoziationsanalysen von Rab6A mit vesikulären Glutamattransportern, Serinracemase und Markern der regulierten Exozytose in kultivierten Astrozyten143 deuten darauf hin, dass dieses Rab6A-Organellsystem die ultrastrukturelle Grundlage für die Freisetzung von Gliotransmittern wie D-Serin und Glutamat bildet.
Zur Untersuchung, ob Rab6A tatsächlich ein System der Glia-Neuron-Kommunikation im Gehirn darstellt, war es zunächst unabdingbar das Vorkommen von Rab6A in situ zu untersuchen. Die durchgeführten immunzytochemischen Färbungen an Hirnschnitten der Maus zeigen das gleichmäßige und ubiquitäre Vorkommen von Rab6A in allen untersuchten Hirnregionen. Durch verblindet durchgeführte Kolokalisationsanalysen von Rab6A mit den etablierten astrozytären Markern Glutaminsynthetase (GS), Glial fibrillary acidic protein (GFAP), Aldh1L1 und Sox9 konnte eine Lokalisation von Rab6A in allen Astrozyten gezeigt werden. Weitere Analysen schließen die Lokalisation von Rab6A in Mikroglia (Iba1), NG2-Zellen (NG2) und Oligodendrozyten (CNPase) aus. Die Astrozyten unterscheiden sich in Größe und subzellulärem Verteilungsmuster der Rab6A+ Strukturen, wonach eine Kategorisierung in vier Typen vorgenommen wurde. Anhand der Einteilung kann vermutet werden, dass größere Rab6A+ TGN-Zisternen bis weit in die Zellperipherie transportiert werden und kleine Rab6A+ Vesikel erst dort ausknospen und der Exozytose zugeführt werden. Zur Frage der möglichen astrozytären Subpopulationen konnte gezeigt werden, dass alle untersuchten Astrozyten GS+, Aldh1L1+, Sox9+ und Rab6A+ sind, jedoch nicht GFAP+.
Um die prinzipielle Übertragbarkeit der gewonnenen Befunde auf den Menschen zu überprüfen, wurde reseziertes Cortex-Gewebe von drei Patienten mit unterschiedlicher pathologischer Genese untersucht. Rab6A ist im massiven Ausmaß in humanen Astrozyten lokalisiert, was nahelegt, dass die zuvor an der Maus gewonnenen Ergebnisse auf den Menschen übertragbar sind.
Die mögliche funktionelle Bedeutung von astrozytärem Rab6A im Gehirn wurde an HFS-Schnitten untersucht. Die Untersuchung zeigt einen signifikanten Anstieg der Rab6A+ Intensität in der gesamten Molekularschicht der Fascia dentata der stimulierten im Vergleich zur unstimulierten Seite. Da die HFS ein etabliertes LTP-Modell darstellt, könnte es infolge dieser zu einer strukturellen, intrazellulären Veränderung der Astrozyten mit erhöhter Freisetzung von D-Serin oder Glutamat aus Rab6A+ Vesikeln kommen, was das Lernverhalten beeinflussen könnte. Die dargestellten Ergebnisse legen eine Auswirkung der HFS auf Rab6A nahe.
Zur Bestätigung der immunzytochemischen Untersuchungen wurde die mRNA-Expression von Rab6A in Astrozyten bereits publizierter Transkriptomanalysen untersucht. Die in den Publikationen verwendeten Genom-Chips treffen allenfalls indirekt eine Aussage zu Rab6A, da Rab6 allgemein und nur Rab6B spezifisch untersucht wurde, jedoch keine spezifische Rab6A Sonde erwähnt wird.
Zusammenfassend kann Rab6A als spezifisches und selektiv in Astrozyten vorkommendes Protein dargestellt und als neuer astrozytärer Marker etabliert werden, der auch Astrozyten des humanen Gewebes markiert. Durch die gewonnenen Befunde kann in nachfolgenden Studien die mögliche Bedeutung von Rab6A in neuropathologischen und neurophysiologischen Prozessen untersucht werden.
Background: Radiofrequency ablation is a minimal invasive therapy in the treatment of bone metastases. In this study we present a new ablation system enabling an ablation in multiple directions and with an adaptable size and shape.
Material and methods: VX-2 tumor was used for the induction of experimental bone metastases in the femur of six New Zealand white rabbits. X-ray imaging as well as CT and MRI scans before and after treatment was carried out. After detecting bone tumor, radiofrequency ablation was performed. The ablation instrument contained a 10 g bipolar, articulated extendable electrode and a proprietary generator with an impedance controlled algorithm. All bones and the soft tissue were examined histologically.
Results: All animals developed local bone tumor. Mean duration until first osteolytic lesions on CT-scans was 48±14 days. The mean lesion area was 26 mm(2). No systemic tumor spread was seen. 6 radiofrequency procedures were carried out with a mean application time of 6 min±2:30 and an average temperature in the region of effect of 55 °C±4. MRI imaging demonstrated an ablation zone of 23±6 mm around the electrode. Histopathology showed an extensive heat necrosis with no remaining tumor cells in the ablation area.
Conclusion: Radiofrequency ablation is a quickly developing treatment option on the field of minimal invasive bone tumor therapy. The electrode enables an ablation adapted to size and shape of the metastases. Further clinical studies are necessary to test and enhance this radiofrequency system.
In context of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), patients with certain comorbidities and high age, as well as male sex are considered to represent the risk group for severe course of disease. Corona-virus disease 2019 (COVID-19) typical CT-patterns include bilateral, peripheral ground glass opacity (GGO), septal thickening, bronchiectasis, consolidation as well as associated pleural effusion. We report a 77-year-old heart transplanted patient with confirmed COVID-19 infection and coronary heart disease, diabetes type II and other risk factors. Notably, only slight clinical symptoms were reported and repeated computed tomography (CT) scans showed an atypical course of CT findings during his hospitalization.
In combination with radiotherapy, immunotherapy is becoming an increasingly used strategy in treating advanced, recurrent, or metastatic cancer. The evident impact of radiotherapy on local and systemic immune response is an indication of the synergistic effect of these two modalities. There is a strong rationale to combine radiotherapy and immunotherapy to enhance response rates and overcome resistances. Therefore, the combination of radio- and immunotherapy holds a variety of opportunities as well as challenges in treating primary cancer and is progressively tested in curative settings. Brachytherapy is also known as internal radiation therapy and only offers a local therapy option at first glance: due to tumor-specific antigens, released by a high local radiation dose, a systemic immune response could be plausible and eminent. Accordingly, brachytherapy could be an underestimated partner with immuno-therapeutic approaches in both curative and palliative settings, to generate local and systemic response. In this review, we summarized the potential benefit of a potential combination of brachytherapy and immuno-therapeutic approaches vs. the background of limited data.
Background: Addition of temozolomide (TMZ) to radiotherapy (RT) improves overall survival (OS) in patients with glioblastoma (GBM), but previous studies suggest that patients with tumors harboring an unmethylated MGMT promoter derive minimal benefit. The aim of this open-label, phase III CheckMate 498 study was to evaluate the efficacy of nivolumab (NIVO) + RT compared with TMZ + RT in newly diagnosed GBM with unmethylated MGMT promoter.
Methods: Patients were randomized 1:1 to standard RT (60 Gy) + NIVO (240 mg every 2 weeks for eight cycles, then 480 mg every 4 weeks) or RT + TMZ (75 mg/m2 daily during RT and 150–200 mg/m2/day 5/28 days during maintenance). The primary endpoint was OS.
Results: A total of 560 patients were randomized, 280 to each arm. Median OS (mOS) was 13.4 months (95% CI, 12.6 to 14.3) with NIVO + RT and 14.9 months (95% CI, 13.3 to 16.1) with TMZ + RT (hazard ratio [HR], 1.31; 95% CI, 1.09 to 1.58; P = .0037). Median progression-free survival was 6.0 months (95% CI, 5.7 to 6.2) with NIVO + RT and 6.2 months (95% CI, 5.9 to 6.7) with TMZ + RT (HR, 1.38; 95% CI, 1.15 to 1.65). Response rates were 7.8% (9/116) with NIVO + RT and 7.2% (8/111) with TMZ + RT; grade 3/4 treatment-related adverse event (TRAE) rates were 21.9% and 25.1%, and any-grade serious TRAE rates were 17.3% and 7.6%, respectively.
Conclusions: The study did not meet the primary endpoint of improved OS; TMZ + RT demonstrated a longer mOS than NIVO + RT. No new safety signals were detected with NIVO in this study. The difference between the study treatment arms is consistent with the use of TMZ + RT as the standard of care for GBM.

Two-person neuroscience (2 PN) is a recently introduced conceptual and methodological framework used to investigate the neural basis of human social interaction from simultaneous neuroimaging of two or more subjects (hyperscanning). In this study, we adopted a 2 PN approach and a multiple-brain connectivity model to investigate the neural basis of a form of cooperation called joint action. We hypothesized different intra-brain and inter-brain connectivity patterns when comparing the interpersonal properties of joint action with non-interpersonal conditions, with a focus on co-representation, a core ability at the basis of cooperation. 32 subjects were enrolled in dual-EEG recordings during a computerized joint action task including three conditions: one in which the dyad jointly acted to pursue a common goal (joint), one in which each subject interacted with the PC (PC), and one in which each subject performed the task individually (Solo).
A combination of multiple-brain connectivity estimation and specific indices derived from graph theory allowed to compare interpersonal with non-interpersonal conditions in four different frequency bands. Our results indicate that all the indices were modulated by the interaction, and returned a significantly stronger integration of multiple-subject networks in the joint vs. PC and Solo conditions. A subsequent classification analysis showed that features based on multiple-brain indices led to a better discrimination between social and non-social conditions with respect to single-subject indices. Taken together, our results suggest that multiple-brain connectivity can provide a deeper insight into the understanding of the neural basis of cooperation in humans.
Background: Glucagon-like peptide-1 receptor agonists may be a treatment option in patients with non-alcoholic fatty liver disease (NAFLD). Aims: To investigate the effects of semaglutide on liver stiffness and liver fat in subjects with NAFLD using non-invasive magnetic resonance imaging (MRI) methods. Methods: This randomised, double-blind, placebo-controlled trial enrolled subjects with liver stiffness 2.50-4.63 kPa by magnetic resonance elastography (MRE) and liver steatosis ≥10% by MRI proton density fat fraction (MRI-PDFF). The primary endpoint was change from baseline to week 48 in liver stiffness assessed by MRE. Results: Sixty-seven subjects were randomised to once-daily subcutaneous semaglutide 0.4 mg (n = 34) or placebo (n = 33). Change from baseline in liver stiffness was not significantly different between semaglutide and placebo at week 48 (estimated treatment ratio 0.96 (95% CI 0.89, 1.03; P = 0.2798); significant differences in liver stiffness were not observed at weeks 24 or 72. Reductions in liver steatosis were significantly greater with semaglutide (estimated treatment ratios: 0.70 [0.59, 0.84], P = 0.0002; 0.47 [0.36, 0.60], P < 0.0001; and 0.50 [0.39, 0.66], P < 0.0001) and more subjects achieved a ≥ 30% reduction in liver fat content with semaglutide at weeks 24, 48 and 72, (all P < 0.001). Decreases in liver enzymes, body weight and HbA1c were also observed with semaglutide. Conclusions: The change in liver stiffness in subjects with NAFLD was not significantly different between semaglutide and placebo. However, semaglutide significantly reduced liver steatosis compared with placebo which, together with improvements in liver enzymes and metabolic parameters, suggests a positive impact on disease activity and metabolic profile. Background: Berotralstat (BCX7353) is a recently approved, oral, once-daily kallikrein inhibitor for hereditary angioedema (HAE) prophylaxis. In the APeX-2 trial, berotralstat reduced HAE attack rates over 24 weeks, with a favorable safety and tolerability profile.
Objective: Evaluate berotralstat safety, tolerability, and effectiveness over 48 weeks.
Methods: APeX-2 is a phase 3, parallel-group, multicenter trial (NCT03485911) in patients with HAE due to C1 esterase inhibitor deficiency. Part 1 was double-blind and placebo-controlled, with patients randomized to 24 weeks of berotralstat 150 mg, 110 mg, or placebo. In part 2, patients continued berotralstat the same dose or, if initially randomized to placebo, were rerandomized to berotralstat 150 mg or 110 mg through weeks 24 to 48. The primary end point was safety and tolerability.
Results: One hundred eight patients received 1 or more doses of berotralstat in part 2. Treatment-emergent adverse events (TEAEs) occurred in 30 of 39 patients (77%) in the placebo group during part 1, and 25 of 34 patients (74%) re-randomized from placebo to berotralstat 110 mg or 150 mg in part 2, with drug-related TEAEs in 13 of 39 (33%), and 11 of 34 (32%) in the same groups. Most TEAEs were mild or moderate, with no serious drug-related TEAEs. The most common TEAEs were upper respiratory tract infections, abdominal pain, diarrhea, and vomiting. Mean (±standard error of the mean) monthly attack rates at baseline and week 48 were 3.06 (±0.25) and 1.06 (±0.25) in the berotralstat 150mg 48-week group and 2.97 (±0.21) and 1.35 (±0.33) in the berotralstat 110mg 48-week group.
Conclusions: The safety, tolerability, and effectiveness of berotralstat were maintained over 48 weeks of treatment.
The rapid detection of pathogens in infected wounds can significantly improve the clinical outcome. Wound exudate, which can be collected in a non-invasive way, offers an attractive sample material for the detection of pathogens at the point-of-care (POC). Here, we report the development of a nucleic acid lateral flow immunoassay for direct detection of isothermally amplified DNA combined with fast sample preparation. The streamlined protocol was evaluated using human wound exudate spiked with the opportunistic pathogen Pseudomonas aeruginosa that cause severe health issues upon wound colonization. A detection limit of 2.1 × 105 CFU per mL of wound fluid was achieved, and no cross-reaction with other pathogens was observed. Furthermore, we integrated an internal amplification control that excludes false negative results and, in combination with the flow control, ensures the validity of the test result. The paper-based approach with only three simple hands-on steps has a turn-around time of less than 30 min and covers the complete analytical process chain from sample to answer. This newly developed workflow for wound fluid diagnostics has tremendous potential for reliable pathogen POC testing and subsequent target-oriented therapy.
Objectives: We explore the importance of SARS-CoV-2 sentinel surveillance testing in primary care during a regional COVID-19 outbreak in Austria.
Design: Prospective cohort study.
Setting: A single sentinel practice serving 22 829 people in the ski-resort of Schladming-Dachstein.
Participants: All 73 patients presenting with mild-to-moderate flu-like symptoms between 24 February and 03 April, 2020.
Intervention: Nasopharyngeal sampling to detect SARS-CoV-2 using real-time reverse transcriptase-quantitative PCR (RT-qPCR).
Outcome measures: We compared RT-qPCR at presentation with confirmed antibody status. We split the outbreak in two parts, by halving the period from the first to the last case, to characterise three cohorts of patients with confirmed infection: early acute (RT-qPCR reactive) in the first half; and late acute (reactive) and late convalescent (non-reactive) in the second half. For each cohort, we report the number of cases detected, the accuracy of RT-qPCR, the duration and variety of symptoms, and the number of viral clades present.
Results: Twenty-two patients were diagnosed with COVID-19 (eight early acute, seven late acute and seven late convalescent), 44 patients tested SARS-CoV-2 negative and 7 were excluded. The sensitivity of RT-qPCR was 100% among all acute cases, dropping to 68.1% when including convalescent. Test specificity was 100%. Mean duration of symptoms for each group were 2 days (range 1–4) among early acute, 4.4 days (1–7) among late acute and 8 days (2–12) among late convalescent. Confirmed infection was associated with loss of taste. Acute infection was associated with loss of taste, nausea/vomiting, breathlessness, sore throat and myalgia; but not anosmia, fever or cough. Transmission clusters of three viral clades (G, GR and L) were identified.
Conclusions: RT-qPCR testing in primary care can rapidly and accurately detect SARS-CoV-2 among people with flu-like illness in a heterogeneous viral outbreak. Targeted testing in primary care can support national sentinel surveillance of COVID-19.
